Abstract

To make robust projectios of the impacts of climate change, it is critical to understand how abiotic factors may interact to constrain the distribution and productivity of marine flora and fauna. We evaluated the effects of projected end of the century ocean acidification (OA) and warming (OW) on the thermal tolerance of an important living marine resource, the sea urchin Loxechinus albus, a benthic shallow water coastal herbivore inhabiting part of the Pacific coast of South America. After exposing young juveniles for a 1-month period to contrasting pCO2 (~500 and 1400 μatm) and temperature (~15 °C and 20 °C) levels, critical thermal maximum (CTmax) and minimum (CTmin) as well as thermal tolerance polygons were assessed based on self-righting success as an end point. Transcription of heat shock protein 70 (HSP70), a chaperone protecting cellular proteins from environmental stress, was also measured. Exposure to elevated pCO2 significantly reduced thermal tolerance by increasing CTmin at both experimental temperatures and decreasing CTmax at 20 °C. There was also a strong synergistic effect of OA × OW on HSP70 transcription levels which were 75 times higher than in control conditions. If this species is unable to adapt to elevated pCO2 in the future, the reduction in thermal tolerance and HSP response suggests that near-future warming and OA will disrupt their performance and reduce their distribution with ecological and economic consequences. Given the wider latitudinal range (6 to 56°S) and environmental tolerance of L. albus compared to other members of this region's benthic invertebrate community, OW and OA may cause substantial changes to the coastal fauna along this geographical range. Abstract

Abstract Background Mounting evidence shows that multi-intervention programmes for hypertension treatment are more effective than an isolated pharmacological strategy. Full economic evaluations of hypertension management programmes are scarce and contain methodological limitations. The aim of the study was to evaluate if a hypertension management programme for elderly patients is cost-effective compared to usual care from the perspective of a third-party payer. Methods We built a cost-effectiveness model using published evidence of effectiveness of a comprehensive hypertension programme vs. usual care for patients 65 years or older at a community hospital in Buenos Aires, Argentina. We explored incremental cost-effectiveness between groups. The model used a life-time framework adopting a third-party payer's perspective. Incremental cost-effectiveness ratio (ICER) was calculated in International Dollars per life-year gained. We performed a probabilistic sensitivity analysis (PSA) to explore variable uncertainty. Results The ICER for the base-case of the "Hypertension Programme" versus the "Usual care" approach was 1,124 International Dollars per life-year gained. PSA did not significantly influence results. The programme had a probability of 43% of being dominant (more effective and less costly) and, overall, 95% chance of being cost-effective. Discussion Results showed that "Hypertension Programme" had high probabilities of being cost-effective under a wide range of scenarios. This is the first sound cost-effectiveness study to assess a comprehensive hypertension programme versus usual care. This study measures hard outcomes and explores robustness through a probabilistic sensitivity analysis. Conclusions The comprehensive hypertension programme had high probabilities of being cost-effective versus usual care. This study supports the idea that similar programmes could be the preferred strategy in countries and within health care systems where hypertension treatment for elderly patients is a standard practice.

Odanacatib, a cathepsin K inhibitor, reduces bone resorption while maintaining bone formation. Previous work has shown that odanacatib increases bone mineral density in postmenopausal women with low bone mass. We aimed to investigate the efficacy and safety of odanacatib to reduce fracture risk in postmenopausal women with osteoporosis.

32. Does Selective Secondary Schools Make a Difference? A Quasi-Experimental Study in Chilean Public Schools

Abstract

This study utilizes longitudinal data from two 10th-grade student cohorts in Chile to investigate the effects of selective secondary schools. Employing propensity scores and difference-in-difference methods, we find significant and positive short-term effects, ranging from 0.2 to 0.3 SD, with minor variations across subjects and cohorts studied. Likewise, these schools are not a viable alternative for vulnerable students, even though the greatest measured effect is on students with lower socioeconomic status. Our findings suggest that the additional value of selective schools may not be directly linked to standardized test performance but rather related to the interactions among equally talented students.

Abstract

Abstract The effects of ocean warming and ocean acidification on developmental and reproductive traits of Lessonia trabeculata were evaluated. Meiospores were cultured for 35 days in an experimental mesocosm where temperature (~15 and 19 °C) and partial CO 2 pressure (p CO 2 , ~400 and 1300 μatm) were controlled. The results indicate that germination was reduced at 19 °C, whereas the increase of p CO 2 only had effects at 15 °C. Likewise, the increase in temperature significantly affected the vegetative growth of female gametophytes. Sex ratio was not affected significantly by any of the variables studied. Fertility and reproductive success decreased by about 50% at 19 °C. The p CO 2 levels had no significant effects on most early developmental traits. The results suggest that ocean warming or periodic warming events (e.g. an El Niño event) might affect the recruiting capacity of this or other similar species by affecting their early developmental stages. Highlights • Climate predictions suggest changes in temperature and p CO 2 levels of the ocean. • The effects of those changes on the kelp Lessonia trabeculata were studied. • The p CO 2 levels had no effects on most early developmental traits. • Temperature increments affected female gametophyte growth and the kelp fertility. • We suggest that mainly temperature increments affect the kelp recruitment. ABSTRACTIntroduction: The obesity epidemic continues to grow. Bariatric surgery is part of the arsenal to treat the disease. Surgery results in an effective option for patients with severe obesity but also when obesity is associated with significant comorbidities. Weight regain is frequent after bariatric surgery. Consequently, the addition of anti-obesity drugs to prevent and manage weight regain are commonly recommended even when the quality of the evidence supporting this recommendation is relatively weak.Areas covered: The aim of this review is to summarize the available evidence concerning long-term pharmacotherapy of obesity in patients that have undergone bariatric surgery with a focus on pharmacological prevention and management of weight regain. The etiology and epidemiology of weight regain are summarized, as well as the available information about the benefits and risks of long-term pharmacotherapy in the prevention and management of recidivism.Expert opinion: The available information, mainly obtained from observational studies and small trials, is encouraging but calls for a prudent approach in the selection of appropriate agents for each individual patient and a careful follow-up to detect adverse reactions or drug interactions. Results from well-designed trials are upcoming. In the meantime, a cautious, individualized approach is advisable.

• Burden of stroke is high in low- and middle-income countries. • EstEPA is a population-based project assessing prevalence, incidence, mortality and burden of stroke in Argentina. • The overall incidence rate of first-ever strokes was 124.2 per 100,000 population and 86.9 per 100,000 when standardized by WHO World population. • Case fatality rate at 30 days of first-ever strokes was 27%. • Incidence was lower than other countries in the region and similar to a recent incidence study in Argentina. Stroke epidemiology varies among different populations. The burden of stroke is high in low- and middle-income countries. Reliable population data is needed to assess the impact of stroke and to develop policies aimed to improve stroke care in our region. EstEPA is a population-based project assessing prevalence, incidence, mortality and burden of stroke in General Villegas Department, Buenos Aires, Argentina (pop=30,864 inhabitants). We determined incidence of stroke (first-ever and recurrent stroke) and stroke case-fatality rate from 2017 to 2020. First-ever strokes, recurrent strokes and transient ischemic attacks were ascertained and case-fatality rate was obtained. Diagnoses were based on standard AHA/WHO definitions. Study population included all persons residing in General Villegas during the three-year period. Hospitals, households, nursing homes, death certificates and several overlapping sources were surveyed. We assessed 92,592 person-years. There were 155 cerebrovascular events aged 70 years (SD ± 13 years), of which 115 were first-ever strokes (74%), 21 recurrent strokes (13.5%) and 19 transient ischemic attacks (12.5%). The crude overall incidence rate of first-ever strokes was 124.2 per 100,000 population (86.9 per 100,000 [95% CI 58.5-115.2] when standardized by WHO World population and 109.7 per 100,000 [95% CI 89.7-129.8] when standardized by Argentine population) and 317.0 per 100,000 population in subjects older than 40 years. Case fatality rate at 30 days of first-ever strokes was 27%. In this population-based comprehensive stroke epidemiological study in Argentina, first-ever stroke incidence in an urban population was 124.2 per 100,000 population (86.9 per 100,000 adjusted by the WHO World population). This is lower than the incidence in other countries in the region and similar to a recent incidence study in Argentina. It is also comparable to reported incidence in most middle- and high-income countries. Stroke case-fatality rate was comparable to other population-based Latin-American studies. Abstract

During the last two decades Latin American countries liberalised and, most of them, privatised their telecommunications industry. The reform aimed at increasing and maintaining public telephone networks while introducing a real and sustainable competition. All in all, the analysis reveals that, although teledensity rates have increased, the results are worse than expected. At the same time, and since the start of this decade, governments are more aware of the fact that the access of the whole population to telecommunication services is essential for the economic development of their countries and for the reduction of poverty. Thus, twenty years after the beginning of this process, an analysis of representative Latin American countries policies and their performance on the fixed telephone network evolution allows to provide some guidelines for the design of policies oriented towards the achievement of not just the traditional approach of universal service, but a new service and access universalisation. Cassava (Manihot esculenta) is a root crop of tropical American origin and is the fourth most important staple crop in the tropics. In the developing world, it is surpassed only by maize, rice, and sugarcane as a source of calories; cassava's starchy roots produce more food energy per unit of land than any other staple crop. Cassava is grown almost exclusively in the arid and semiarid tropics, where it accounts for approximately 10% of the total caloric value of staple crops. 46. Nonresponders to clopidogrel: pharmacokinetics and interactions involved

Abstract

Importance of the field:The use of clopidogrel and aspirin has become standard therapy in patients with acute coronary syndromes and stent implantation. However, concern arises because about 25% of subjects are nonresponders to clopidogrel. This nonresponsiveness is associated with a threefold increase in adverse outcomes. Clopidogrel resistance is multifactorial, but genetic polymorphisms in clopidogrel's metabolic activation (e.g., cytochrome P450 2C19) and drug–drug interactions at this level (e.g., between proton pump inhibitors (PPIs) and clopidogrel) are both associated with decreased clopidogrel efficacy. Despite all PPIs being potent inhibitors of CYP2C19, evidence about their clinical impact is controversial.Areas covered in this review:Pharmacogenomic and pharmacokinetic aspects of clopidogrel nonresponsiveness were considered in detail.What the reader will gain:The reader will gain an exhaustive review of the current state of the controversial issues regarding genetic polymorphisms and drug–drug interactions affecting clopidogrel efficacy.Take home message:It is important to consider clopidogrel resistance in some patients and establish strategies to handle this problem (e.g., genotyping, platelet aggregability tests, new antiplatelet drugs). The combined use of PPIs and clopidogrel is at present regulated by the FDA and EMEA; however, the risk/benefit balance should be made for each patient individually.

Abstract

Diabetes mellitus is a chronic disease that usually requires multiple insulin injections to achieve adequate glycaemic control. This represents a major cause of reduced compliance to treatment. Consequently, other routes for insulin administration have been explored. During recent years, much progress in the development of inhaled insulin has been made. Inhaled insulin has shown favourable properties, such as a rapid onset of action, improved bioavailability and good tolerability; thereby providing satisfaction and ease of administration. However, long-term safety of inhaled insulin needs to be assessed, and the cost would be higher than injectable insulin. Nasal, oral and transdermal insulins are undergoing early phases of pharmacological development. The purpose of this review is to describe the latest developments in the area of non-invasive routes for insulin delivery.

50. Investigational treatments for Type 2 diabetes mellitus: exenatide and liraglutide

Abstract

Although a number of compounds are currently used to treat Type 2 diabetes mellitus, achieving a sustained glycaemic control over time is often not possible using oral antidiabetics. Endogenous incretins exhibit beneficial effects that could be useful for Type 2 diabetes mellitus treatment, such as stimulating insulin secretion during hyperglycaemia, improving -cell mass and function, reducing glucagon secretion, delaying gastric emptying, reducing postprandial hyperglycaemia and diminishing body weight; however, their short half-life makes them unsuitable for treatment. Incretin mimetics such as liraglutide and exenatide were developed to overcome this limitation. This review discusses the effects of these compounds and their potential as a new class of antidiabetic agents.
