Abstract

Background As a strong opioid, methadone has unique advantages in relieving refractory carcinous pain, but also has some limitations, such as higher risk of accumulation due to long elimination half-life, difficulties in dosage calculation and dose conversion due to significant differences between individuals, and limited clinical evidence on the use of this medicine in large doses. Objective To investigate the efficacy and safety of methadone conversion in patients with refractory carcinous pain. Methods A total of 100 inpatients with refractory carcinous pain were recruited from Cancer Hospital of the University of Chinese Academy of Sciences/Zhejiang Cancer Hospital from July 2017 to December 2021. The conversion of morphine to methadone was carried out by rapid conversion or 3-day conversion method. The doses of methadone used at one week and two weeks after the conversion, the daily mean pain intensity assessed by the Numeric Rating Scale (NRS), number of breakthrough pain and adverse reactions before and after methadone conversion were recorded. The efficacy and safety of methadone conversion were analyzed. Results There was a dynamic conversion ratio between morphine and methadone. The conversion ratio between morphine and methadone increased with the growth of the daily dose of morphine. Ninety-six cases finally completed methadone conversion. Abstract

Introduction Small-cell lung cancer (SCLC) is the most aggressive and highly metastatic subtype of lung cancer and accounts for approximately 15% of all lung cancer cases (1). The 5-year survival [...], Metastasis is the dominant cause of patient death in small-cell lung cancer (SCLC), and a better understanding of the molecular mechanisms underlying SCLC metastasis may potentially improve clinical treatment. Through genome-scale screening for key regulators of mouse [Rb1.sup.-/-] [Trp53.sup.-/-] SCLC metastasis using the pooled CRISPR/Cas9 library, we identified Cullin5 (CUL5) and suppressor of cytokine signaling 3 (SOCS3), two components of the Cullin-RING E3 ubiquitin ligase complex, as top candidates. Mechanistically, the deficiency of CUL5 or SOCS3 disrupted the functional formation of the E3 ligase complex and prevented the degradation of integrin [beta]1, which stabilized integrin [beta]1 and activated downstream focal adhesion kinase/SRC (FAK/SRC) signaling and eventually drove SCLC metastasis. Low expression levels of CUL5 and SOCS3 were significantly associated with high integrin [beta]1 levels and poor prognosis in a large cohort of 128 clinical patients with SCLC. Moreover, the CUL5-deficient SCLCs were vulnerable to the treatment of the FDA-approved SRC inhibitor dasatinib. Collectively, this work identifies the essential role of CUL5- and SOCS3- mediated integrin [beta]1 turnover in controlling SCLC metastasis, which might have therapeutic implications.

Abstract

Objective This study aims to investigate the impact of intervention of multidisciplinary team incorporating pharmacists for management of opioid-naive patients with moderate to severe cancer pain. Methods A retrospective cohort study was conducted to compare pre- and post-multidisciplinary intervention groups in opioid-naive patients with moderate to severe cancer pain. Primary outcome was the proportions of appropriate pain assessment and opioid titration. Secondary outcomes were pain intensity (PI), length of hospital stay, opioid escalation index percentage (OEI%) and incidences of opioid-related adverse events. Results A total of 400 patients were included in the study (pre-intervention, n = 200; post-intervention, n = 200). Continuous improvement in pain assessment and titration was recorded after intervention. Though no substantial differences existed between groups in PI on the day of discharge, post-intervention group was associated with reduced length of hospital stay as well as decreased proportion of subjects with OEI% >5%. As for safety, significant decreases in constipation and vomiting were seen. Conclusion Findings suggest that interventions of multidisciplinary team incorporating pharmacists could improve cancer pain management for opioid-naive patients. Pharmacists should be considered as an important member of a multidisciplinary team in good pain management.

Abstract

Abstract: A preliminary conceptual framework is proposed for the estimation of the costs of desertification. This framework consists of assessment of the extent and severity of desertification, identification of various impacts and valuation of the direct and indirect costs of desertification. Based on this framework, three well‐cited studies of the costs of desertification in China were critically reviewed. Limitations in these studies include double counting, confusion of short‐term and long‐term costs and the lack of necessary adjustment of price parameters. Refinement of the estimates indicates that these limitations have caused serious overestimation or underestimation of the costs of desertification. To achieve more reliable and useful estimates, future studies need to overcome the limitations mentioned in the preceding texts and use the latest available data on desertification and its various impacts. A more comprehensive conceptual framework that includes the valuation of dryland ecosystem services could also be developed to guide future studies. Copyright © 2016 John Wiley & Sons, Ltd. 50. Diosgenin inhibits tumor angiogenesis through regulating GRP78-mediated HIF-1α and VEGF/VEGFR signaling pathways

Abstract

The present paper describes the molecular mechanism of diosgenin on tumor microenvironment angiogenesis and the regulation of GRP78 in angiogenesis signaling pathway. CCK8 method was used to evaluate the effect of different concentrations of diosgenin on HUVEC activity in hypoxic microenvironment. Apoptosis was detected by Annexin V/PI staining. Tube Formation experiment was conducted to evaluate angiogenesis. Western Blot assay was applied to detect the expressions and phosphorylation levels of the angiogenesis-related pathways HIF-1α, GRP78, VEGF/VEGFR, PI3K/AKT, ERK, and FAK in rheumatoid HUVEC. Silencing GRP78 by siRNA interference technology was employed to investigate the mechanism of GRP78 involved in the regulation of angiogenesis. The results indicated that diosgenin can significantly inhibit the cell viability of hypoxic HUVEC and the significance is dependent on drug concentration. As the concentration increases, HUVEC activity decreases. Cell apoptosis is induced in a dose-dependent manner and angiogenesis can be significantly inhibited. The hypoxic microenvironment can significantly increase the expressions of HIF-lα, GRP78, VEGF/VEGFR, PI3K/AKT, ERK, FAK proteins in angiogenesis-related pathways, and can also enhance the phosphorylation of AKT, ERK1/2 and FAK proteins, which can be decreased by drug intervention. After silencing GRP78, the angiogenesis-related pathway proteins HIF-lα and VEGF/VEGFR are significantly reduced, thus inhibiting the activation of AKT, ERK1/2, and FAK. The anti-tumor angiogenesis mechanism of diosgenin inhibiting the expression of HIF-lα, GRP78, VEGF/VEGFR, PI3K/AKT, ERK1/2 and FAK signaling pathways may be through multiple pathways and targets, and GRP78 is involved in the regulation of angiogenesis signaling pathway.
