2. Non-diabetes status after diagnosis of impaired glucose tolerance and risk of long-term death and vascular complications: A post hoc analysis of the Da Qing Diabetes Prevention Outcome Study

Background The association between years of non-diabetes status after diagnosis of impaired glucose tolerance (IGT) and the risk of long-term death and cardiovascular outcomes needed to be clarified. Methods and findings In this post hoc analysis, we included 540 individuals with IGT who participated in the original Da Qing Diabetes Prevention Study (DQDPS). In the DQDPS, all participants were diagnosed with IGT by a 75 g oral glucose tolerance test and randomized to intervention or control groups with a 6-year lifestyle intervention trial. After the completion of the trial, death, cardiovascular events, and microvascular complications were monitored over a 30-year follow-up. In this post hoc analysis, the Cox analysis assessed the extended risk of these outcomes in individuals who either remained non-diabetes status or progressed to diabetes at the end of 2, 4, and 6 years after diagnosis of IGT. In all participants, the difference in the cumulative incidence rate of the outcomes between the diabetes and non-diabetes group gradually increased over 30 years. Compared with the diabetes group, a significantly lower risk of all-cause death (hazard ratio [HR]: 0.74; 95% confidence interval [CI]: 0.57 to 0.97, p = 0.026), cardiovascular events (HR: 0.63; 95% CI: 0.49 to 0.82, p < 0.001), and microvascular complications (HR: 0.62; 95% CI: 0.45 to 0.86, p = 0.004) first emerged in individuals who remained non-diabetes at the 4 years visit, whereas the significant risk reduction in cardiovascular death was first observed at the end of 6 years (HR: 0.56; 95% CI: 0.39 to 0.81, p = 0.002) after adjustment for age, sex, smoking status, BMI, systolic blood pressure, blood glucose, total cholesterol, intervention, and medications (including insulin plus oral hypoglycaemics, antihypertensives, and lipid-lowering agents). The results in the original intervention group alone were similar to the whole group. The main limitations of our study are the limited number of participants and the sole ethnicity of the Chinese population. Aims: We intended to characterize the superiority of triglyceride glucose‐body mass index (TyG‐BMI) in predicting type 2 diabetes mellitus (T2DM) compared with triglyceride glucose (TyG) and homeostatic model assessment for insulin resistance (HOMA‐IR). Methods: A total of 699 nondiabetic participants in the Da Qing IGT and Diabetes Study were involved in the present analysis and classified according to the median of baseline TyG‐BMI, namely the G1 (low TyG‐BMI) and G2 (high TyG‐BMI) groups. Information on developing diabetes was assessed from 1986 to 2020. Results: During the 34‐year follow‐up, after adjustment for confounders, the G2 group had a higher risk of developing type 2 diabetes than the G1 group (hazard ratio [HR]: 1.92, 95% confidence interval [CI]: 1.51–2.45, p < 0.0001). Restricted cubic spline analyses showed that increased TyG‐BMI was linearly related to higher risks of type 2 diabetes (p for non‐linearity>0.05). Time‐dependent receiver operator characteristics curves suggested that TyG‐BMI exhibited higher predictive ability than TyG (6‐year: area under the curve [AUC]TyG‐BMI vs. AUCTyG, 0.78 vs. 0.70, p = 0.03; 34‐year: AUCTyG‐BMI vs. AUCTyG, 0.79 vs. 0.73, p = 0.04) and HOMA‐IR (6‐year: AUCTyG‐BMI vs. AUCHOMA‐IR, 0.78 vs. 0.70, p = 0.07; 34‐year: AUCTyG‐BMI vs. AUCHOMA‐IR, 0.79 vs. 0.71, p = 0.04) in both short and long terms, and the thresholds of TyG‐BMI to predict type 2 diabetes were relatively stable (195.24–208.41) over the 34‐year follow‐up. Conclusions: In this post hoc study, higher TyG‐BMI was associated with an increased risk of type 2 diabetes and demonstrated better predictability than TyG and HOMA‐IR, favoring the application of TyG‐BMI as a potential tool for evaluating the risk of type 2 diabetes in clinical practice. Background: This study aimed to investigate the potential differences in the influence of impaired glucose tolerance (IGT) with and without metabolic syndrome (MetS) on cardiovascular (CV) events and mortality. Methods: Participants having IGT with MetS (IGT_MetS), those having IGT without MetS (IGT_non_MetS), and those having normal glucose tolerance (NGT) without MetS (NGT_non_MetS) (N = 246, N = 294, and N = 471, respectively) were included in this study. Cox proportional hazards regression was used to examine the relationship among these three groups and CV events and mortality. Results: Over the 30‐year follow‐up period, 57 (12.1%) participants having NGT_non_MetS, 55 (18.71%) with IGT_non_MetS, and 74 (30.08%) with IGT_MetS experienced CV mortality. After adjusting for risk factors, the hazard ratios for CV mortality were 2 (95% confidence interval [CI], 1.38–2.91) for the IGT_non_MetS group and 2.96 (95% CI, 2.09–4.19) for the IGT_MetS group, compared with the NGT_non_MetS group. Similar patterns were observed for CV events, with hazard ratios of 1.49 (95% CI, 1.19–1.88) for the IGT_non_MetS group and 1.97 (95% CI, 1.58–2.47) for the IGT_MetS group. Sensitivity analysis revealed that the hazard ratios of the IGT_non_MetS and IGT_MetS groups indicated a higher risk of all‐cause mortality, myocardial infarction events or myocardial infarction mortality, and stroke events or stroke mortality compared with that of the NGT_non_MetS group. Conclusion: IGT_non_MetS increased the risk of CV mortality and events. Furthermore, when it occurred in conjunction with MetS, it further increased the risk of CV mortality and events. Aim: To evaluate whether 1‐hour plasma glucose (1hPG) can be a comparable measurement to 2‐hour plasma glucose (2hPG) in identifying individuals at high risk of developing diabetes. Methods: A total of 1026 non‐diabetic subjects in the Da Qing IGT and Diabetes Study were included and classified according to baseline postload 1hPG. The participants were followed up and assessed at 6‐, 20‐ and 30year follow‐up for outcomes including diabetes, all‐cause and cardiovascular mortality, cardiovascular disease (CVD) events, and microvascular disease. We then conducted a proportional hazards analysis in this post hoc study to determine the risks of developing type 2 diabetes and its complications in a '1hPG‐normal' group (1hPG <8.6 mmol/L) and a '1hPG‐high' group (≥8.6 mmol/L). The predictive values of 1hPG and 2hPG were evaluated using a time‐dependent receiver‐operating characteristic (ROC) curve. Results: Compared with the 1hPG‐normal group, the 1hPG‐high group had increased risk of diabetes (hazard ratio [HR] 4.45, 95% CI 3.43–5.79), all‐cause mortality (HR 1.46, 95% CI 1.07–2.01), CVD mortality (HR 1.84, 95% CI 1.16–2.95), CVD events (HR 1.39, 95% CI 1.03–1.86) and microvascular disease (HR 1.70, 95% CI: 1.03–2.79) after adjusting for confounders. 1hPG exhibited a higher area under the ROC curve (AUC) for predicting diabetes than 2hPG during the long‐term follow‐up (AUC [1hPG vs. 2hPG]: 10 years: 0.86 vs. 0.84, p = 0.08; 20 years: 0.88 vs. 0.87, p = 0.04; 30 years: 0.85 vs. 0.82, p = 0.009). Conclusions: Elevated 1hPG level (≥8.6 mmol/L) was associated with increased risk of developing type 2 diabetes and its long‐term complications, and could be considered as a suitable measurement for identifying individuals at high risk of type 2 diabetes. Aim: We aimed to investigate the long‐term influence of a diet and/or exercise intervention on long‐term mortality and cardiovascular disease (CVD) events. Methods: The Da Qing Diabetes Prevention Study had 576 participants with impaired glucose tolerance (IGT) randomized to diet‐only, exercise‐only and diet‐plus‐exercise intervention group and control group. The participants underwent lifestyle interventions for 6 years. The subsequent Da Qing Diabetes Prevention Outcome Study was a prospective cohort study to follow‐up the participants for up to 24 years after the end of 6‐year intervention. In total, 540 participants completed the follow‐up, while 36 subjects lost in follow‐up. Cox proportional hazards analysis was applied to assess the influence of lifestyle interventions on targeted outcomes. Results: Compared with controls, the diet‐only intervention in people with IGT was significantly associated with a reduced risk of all‐cause death [hazard ratio (HR) 0.77, 95% confidence interval (CI) (0.61‐0.97)], CVD death [HR 0.67, 95% CI (0.46‐0.97)] and CVD events [HR 0.72, 95% CI (0.54‐0.96)]. The diet‐plus‐exercise intervention was significantly associated with a decreased risk of all‐cause death [HR 0.64, 95% CI (0.48‐0.84)], CVD death [HR 0.54, 95% CI (0.30‐0.97)] and CVD events [HR 0.68, 95% CI (0.52‐0.90)]. Unexpectedly, the exercise‐only intervention was not significantly associated with the reduction of any of these outcomes, although there was a consistent trend towards reduction. Conclusions: A diet‐only intervention and a diet‐plus‐exercise intervention in people with IGT were significantly associated with a reduced risk of all‐cause death, CVD death and CVD events, while an exercise‐only intervention was not. Aim: Whether systolic/diastolic blood pressure (SBP/DBP) values of 130‐139/80‐89 mmHg should be defined as hypertension has been debated for decades. We aimed to characterize the effect of high‐normal BP on cardiovascular disease (CVD) events and deaths. Methods: In total, 1726 individuals from the original Da Qing IGT and Diabetes Study were enrolled, and divided into the normal BP group (SBP <130 mmHg and DBP <80 mmHg), high‐normal BP group (SBP 130‐139 mmHg and/or DBP 80‐89 mmHg) and hypertension group (SBP ≥140 mmHg and/or DBP ≥90 mmHg). CVD events and their components were assessed from 1986 to 2016. Results: During the 30‐year follow‐up, the high‐normal BP group was not at higher risk for CVD events [hazard ratio (HR) 1.05, 95% confidence interval (CI) 0.84‐1.30, p =.68], coronary heart disease (HR 1.12, 95% CI 0.77‐1.63, p =.57), stroke (HR 1.05, 95% CI 0.82‐1.34, p =.71), or CVD deaths (HR 1.15, 95% CI 0.82‐1.60, p =.41) compared with the normal BP group, after adjusting for covariates. However, the hypertension group exhibited significantly increased cardiovascular risk (CVD events, HR 1.91, 95% CI 1.48‐2.46, p <.0001; coronary heart disease, HR 1.73, 95% CI 1.12‐2.67, p =.01; stroke, HR 1.90, 95% CI 1.43‐2.52, p <.0001; CVD deaths, HR 2.07, 95% CI 1.43‐3.01, p =.0001) than the normal BP group. Subgroup analyses showed that, regardless of the presence of diabetes, high‐normal BP did not increase CVD events compared with normal BP. Conclusions: This post‐hoc study provided no evidence that the high‐normal BP increased cardiovascular risk in the Da Qing study population, suggesting that it was reasonable to continue to define hypertension at 140/90 mmHg in China. Aims: The extent that pre‐diabetic fasting plasma glucose (FPG) levels influence the effectiveness of lifestyle interventions in preventing type 2 diabetes (T2DM) is uncertain. We aimed to determine if the outcome of lifestyle intervention in people with impaired glucose tolerance (IGT) differs in those with normal or impaired FPG levels. Materials and Methods: Data were used from the Da Qing Diabetes Prevention Outcome Study, which was a 30‐year follow‐up of a 6‐year randomized trial of lifestyle intervention in 576 people with IGT. We then conducted a post‐hoc analysis to compare the efficacy of intervention to reduce the incidence of T2DM and its complications in those with baseline FPG <100 mg/dL and FPG ≥100 mg/dL. Results: Lifestyle intervention reduced the cumulative incidence of T2DM by 37%‐46% in those with baseline FPG <100 mg/dL and by 47%‐51% in those with FPG ≥100 mg/dL. The FPG <100 mg/dL group had a lower cumulative incidence of diabetes and 6.41 years median delay in its onset compared with 2.21 years delay in the FPG ≥100 mg/dL group. In those with FPG <100 mg/dL intervention was associated with at least as great a reduction in cardiovascular disease and all‐cause mortality as in the FPG ≥100 mg/dL group. Conclusions: Lifestyle intervention reduced the incidence of T2DM in people with IGT regardless of baseline FPG levels, and in those with FPG <100 mg/dL led to a substantial delay in its onset. All persons with IGT, with normal or impaired FPG levels, may benefit from lifestyle intervention to delay its onset and mitigate the incidence of T2DM. [ABSTRACT FROM AUTHOR]

- Abstract
Objective To explore the effect of glucagon-like peptide-1 receptor agonist (GLP-1RA) on cardiovascular risk in overweight/obese type 2 diabetic patients complicated with coronary heart disease. Methods A retrospective study was proceeded in 200 overweight/obese diabetic patients with coronary heart disease at Fuwai Hospital from October 2010 to October 2016. Patients were divided into GLP-1RA treatment group (n=105) and the conventional treatment group (n=95). Data of body weight, body mass index (BMI), glycated hemoglobin A1c (HbA1c), fasting plasma glucose (FPG) and blood lipids before and after treatment for 6 months or drug withdrawal after 30 months were analyzed. The incidence rates of recurrent cardiovascular events / the cardiovascular risks were also evaluated between these 2 groups. Kruskal-Wallis test was used to compare the continuous variables in non-normal distribution, Student-t test was used to compare the continuous variables in normal distribution, and paired t-test was used to analyze the follow-up data and baseline data. Chi-Square test was used to compare the classified variable between groups. Multivariate Logistic regression analysis was used to analyze the related risk factors of cardiovascular events. Results Weight [(-5.7±4.0) vs (-1.3±2.4)kg], BMI [(-1.9±1.3) vs (-0.4±0.7)kg/m²], HbA1c [(-1.5±1.3)% vs (-0.8±0.6)%] and low density lipoprotein cholesterol after 6-month treatment were significantly decreased in GLP-1RA treatment group than those in conventional treatment group (t=2.42 -4.19, all P<0.05). After 30-month following-up, the incidence rates of cardiovascular events in GLP-1RA group were significantly lower than those in conventional treatment group [2.86%(3/105) vs 11.58%(11/95), χ²=4.57, P<0.05]. Compared with conventional treatment group after adjustment of multiple risk factors, the risk of recurrent cardiovascular events in GLP-1RA treatment group was significantly decreased (OR:0.23, 95%CI: 0.06-0.89, P=0.04). Conclusion Short-term application of GLP-1RA could alleviate body weight, improve glycolipid metabolic disorder, and reduce the risk of recurrent cardiovascular events in overweight/obese type 2 diabetic patients with coronary heart disease. Objective: People with impaired glucose tolerance (IGT) have increased risk of mortality and a high risk of progression to diabetes, but the extent that the excess mortality is associated with IGT per se or is the result of subsequent diabetes is unclear.Research Design and Methods: We compared mortality before and after the development of diabetes among 542 persons with IGT initially who participated in a 6-year lifestyle diabetes prevention trial and were followed-up from 1986 to 2009.Results: During the 23-year follow-up, 174 (32.1%) died, with an overall death rate of 15.9/1,000 person-years. The majority of deaths (74.7%; 130 of 174) occurred after progression to type 2 diabetes, with age-adjusted death rates of 11.1/1,000 person-years (95% CI 8.2-12.0) before and 19.4/1,000 person-years (95% CI 11.9-23.3) after the development of type 2 diabetes. The cumulative mortality was 37.8% (95% CI 33.1-42.2%) in participants who developed type 2 diabetes during first 10 years of follow-up, 28.6% (95% CI 21.6-35.0%) in those who progressed to type 2 diabetes in 10-20 years, and 13.9% (95% CI 7.0-20.3%) in those who did not develop to type 2 diabetes within 20 years. Time-dependent multivariate Cox proportional hazards analyses, with adjustment for baseline age, sex, intervention, and other potential confounding risk factors, showed that the development of type 2 diabetes was associated with a 73% higher risk of death (hazard ratio 1.73 [95% CI 1.18-2.52]).Conclusions: As elsewhere, IGT is associated with increased risk of mortality in China, but much of this excess risk is attributable to the development of type 2 diabetes. Despite its growing prevalence in China, the extent to which diabetes leads to excess mortality and its underlying causes are ill-defined. Six-hundred and six persons with newly diagnosed diabetes (NDD) and a comparison group of 494 with normal glucose tolerance (NGT) were identified in 1986 by screening 110,660 adults aged 25-74 years who agreed to participate in The Da Qing IGT and Diabetes study. A follow-up study of the original study participants was conducted in 2016. We compared death rates and causes of death between the two groups during 30 years of follow-up. Four-hundred and forty (71.7%) participants with NDD and 153 (31.0%) with NGT died. Overall mortality (rates/1,000 person-years + 95% CI) were much higher in those with NDD than with NGT, 40.8 (35.6-46.5) vs.15.8 (13.0-19.1) in men (P< 0.0001), and 30.4 (26.4-34.9) vs. 6.9 (5.0-9.3) in women (P<0.0001). Earlier age at diagnosis of diabetes was significantly related to the relative risk of death (RR (+95% CI)), 5.0 (2.9-8.6) in those diagnosed at age 25∼39y, 3.2 (2.3-4.4)] at 40∼49y, and 2.8 (2.1-3.6) at 50∼74y. Cardiovascular disease (CVD) was the predominant cause of death in those with diabetes (66.5% in men and 62.8% in women), almost half of which was due to stroke (52.1% in men and 40.6% in women). After age-sex adjustment, the NDDs had significant increased mortality from CVD (RR 3.9 [95% CI, 2.9-5.2]), renal failure (RR 15.6 [95% CI, 6.3-38.9]), infection (RR 4.2 [95% CI, 2.0-8.5]), and cancer (RR 1.5 [95% CI, 1.0-2.3]). In conclusion, diabetes is associated with substantially increased risks of death in Chinese adults, especially from CVD, renal failure, infection and cancer. Background The effects of empagliflozin in patients with chronic kidney disease who are at risk for disease progression are not well understood. The EMPA-KIDNEY trial was designed to assess the effects of treatment with empagliflozin in a broad range of such patients. Methods We enrolled patients with chronic kidney disease who had an estimated glomerular filtration rate (eGFR) of at least 20 but less than 45 ml per minute per 1.73 m2 of body-surface area, or who had an eGFR of at least 45 but less than 90 ml per minute per 1.73 m2 with a urinary albumin-to-creatinine ratio (with albumin measured in milligrams and creatinine measured in grams) of at least 200. Patients were randomly assigned to receive empagliflozin (10 mg once daily) or matching placebo. The primary outcome was a composite of progression of kidney disease (defined as end-stage kidney disease, a sustained decrease in eGFR to Results A total of 6609 patients underwent randomization. During a median of 2.0 years of follow-up, progression of kidney disease or death from cardiovascular causes occurred in 432 of 3304 patients (13.1%) in the empagliflozin group and in 558 of 3305 patients (16.9%) in the placebo group (hazard ratio, 0.72; 95% confidence interval [CI], 0.64 to 0.82; PConclusions Among a wide range of patients with chronic kidney disease who were at risk for disease progression, empagliflozin therapy led to a lower risk of progression of kidney disease or death from cardiovascular causes than placebo. - Abstract
Objective: To investigate if a six-year intensive lifestyle intervention in people with pre-diabetes lead to reduction of cardiovascular events and cardiovascular disease (CVD) mortality in subsequent 23 years., Methods: Five hundreds and nineteen subjects with normal glucose tolerance (NGT) and 577 subjects with impaired glucose tolerance (IGT) in Da Qing city were recruited in the study in 1986. The IGT subjects randomly assigned to either the no-intervention group or one of three lifestyle intervention groups (diet, exercise, or diet plus exercise) to receive a 6-year lifestyle intervention. In 2009, the participants were followed up to assess the primary outcomes of cardiovascular events and CVD mortality by a questionnaire and medical records., Results: Subjects in IGT no-intervention group had the highest incidences of cardiovascular events (44.44%) and CVD mortality (20.00%), while those in NGT group had the lowest incidences of cardiovascular events (29.59%) and CVD mortality (7.52%) after 23-year follow-up. The incidences of cardiovascular events and CVD mortality in IGT intervention subjects were 37.84% and 12.53%, respectively. The multivariable analyses showed that, after controlling of age, gender, BMI smoking, blood pressure and cardiovascular event at baseline, the CVD mortality and incidence of cardiovascular events in IGT no-intervention group was 1.89 (HR = 1.89, 95%CI 1.11-3.22, P = 0.02) and 1.38 (HR = 1.38, 95%CI 1.01-1.90, P = 0.04) times of those in NGT group. However, the CVD mortality and incidence of cardiovascular events were not different in the IGT intervention group compared with those in the NGT group (HR = 1.39, 95%CI 0.89-2.18, P = 0.15 and HR = 1.25, 95%CI 0.98-1.59, P = 0.07, respectively)., Conclusions: Subjects with IGT were at high risk for cardiovascular events and mortality. A six-year lifestyle intervention in this population can reduce both the incidence of cardiovascular event and CVD mortality.
