Abstract

Objective We evaluated the association between inflammation and oxidative stress with carotid intima media thickness (cIMT) and elasticity increment module ([E.sub.inc]) in pediatric patients with chronic kidney disease (CKD). Methods This analytical, cross-sectional study assessed 134 children aged 6-17 years with CKD. Anthropometric measurements and biochemistry of intact parathyroid hormone (iPTH), high-sensitivity C-reactive protein (CRP), interleukin (IL)-6, IL-1[beta], reduced glutathione (GSH), malondialdehyde, nitric oxide, and homocysteine were recorded. Bilateral carotid ultrasound (US) was taken. Patients were compared with controls for cIMT and [E.sub.inc] using [greater than or equal to] 75 percentile (PC). Results Mean cIMT was 0.528 [+ or -] 0.089 mm; [E.sub.inc] was 0.174 [+ or -] 0.121 kPa x [10sup.3]; cIMT negatively correlated with phosphorus (r -0.19, p = 0.028) and the calcium x phosphorus (Ca x P) product (r -0.26, p = 0.002), and positively with iPTH (r 0.19, p = 0.024). After adjusting for potential confounders, hemodialysis (HD) ([beta] = 0.111, p = < 0.001), automated peritoneal dialysis (APD) ([beta] = 0.064, p = 0.026), and Ca x P product ([beta] = -0.002, p = 0.015) predicted cIMT ([R.sup.2] = 0.296). In patients on dialysis, HD ([beta] = 0.068, p = 0.010), low-density lipoprotein cholesterol (LDL-C) ([beta] = 0.001, p = 0.048), and GSH ([beta] = -0.0001, p = 0.041) independently predicted cIMT ([R.sup.2] = 0.204); HD, hypoalbuminemia, and highi PTH-increased the risk of increased cIMT. In dialysis, [E.sub.inc] was inversely associated with GSH, and in predialysis, Ca x P correlated with/ predicted [E.sub.inc] ([beta] = 0.001, p = 0.009). Conclusions cIMT and [E.sub.inc] strongly associate with several biochemical parameters and GSH but not with other oxidative stress or inflammation markers. 40. Epidemiology of Type 1 Diabetes in Latin America

Abstract

Latin America is among the regions with the highest diabetes-related burden. Research and treatment programs have increased in number and complexity in recent years, but they are focused in type 2 diabetes, because this condition explains a large proportion of the cases. In contrast, the information regarding the epidemiology of type 1 diabetes is scant in this area. Here, we analyze the available information on this topic and identify potential areas of opportunity to generate new knowledge through the study of type 1 diabetes in Latin Americans. Both, the prevalence and the incidence of type 1 diabetes, are lower in Latin American countries compared to that reported in Europe, North America, southern Asia and northern Africa. Biologic and methodological factors may explain the smaller contribution of type 1. The presence of some putative protective environmental exposures or the absence of those prevalent in a region may explain the lower type 1 diabetes prevalence observed in most Latin American countries. However, the number and quality of the diabetes registries are not enough in this region. During the past decade, the incidence of type 1 diabetes has grown worldwide. The same trend has been reported in Latin America. This epidemiologic transition is a unique opportunity to identify interactions between rapidly changing environmental factors in subjects with different genetic backgrounds (such as the admixed Latin American populations). Finally, on-going therapeutic initiatives in this region are highlighted.
