2. Nonlinear nonlocal reaction-diffusion problem with local reaction

In this paper we analyse the asymptotic behaviour of some nonlocal diffusion problems with local reaction term in general metric measure spaces. We find certain classes of nonlinear terms, including logistic type terms, for which solutions are globally defined with initial data in Lebesgue spaces. We prove solutions satisfy maximum and comparison principles and give sign conditions to ensure global asymptotic bounds for large times. We also prove that these problems possess extremal ordered equilibria and solutions, asymptotically, enter in between these equilibria. Finally we give conditions for a unique positive stationary solution that is globally asymptotically stable for nonnegative initial data. A detailed analysis is performed for logistic type nonlinearities. As the model we consider here lack of smoothing effect, important focus is payed along the whole paper on differences in the results with respect to problems with local diffusion, like the Laplacian operator.

In this paper we consider the following nonlocal autonomous evolution equation in a bounded domain $\Omega$ in $\mathbb{R}^N$ \[ \partial_t u(x,t) =- h(x)u(x,t) + g \Big(\int_{\Omega} J(x,y)u(y,t)dy \Big) +f(x,u(x,t)) \] where $h\in W^{1,\infty}(\Omega)$, $g: \mathbb{R} \to \mathbb{R}$ and $f:\mathbb{R}^N\times\mathbb{R} \to \mathbb{R}$ are continuously differentiable function, and $J$ is a symmetric kernel; that is, $J(x,y)=J(y,x)$ for any $x,y\in\mathbb{R}^N$. Under additional suitable assumptions on $f$ and $g$, we study the asymptotic dynamics of the initial value problem associated to this equation in a suitable phase spaces. More precisely, we prove the existence, and upper semicontinuity of compact global attractors with respect to kernel $J$.

5. Equivalent formulations for steady periodic water waves of fixed mean-depth with discontinuous vorticity

In this work we prove the equivalence between three different weak formulations of the steady periodic water wave problem where the vorticity is discontinuous. In particular, we prove that generalised versions of the standard Euler and stream function formulation of the governing equations are equivalent to a weak version of the recently introduced modified-height formulation. The weak solutions of these formulations are considered in H\"older spaces.

In particular, we examine a recently-derived exact and explicit solution to the geophysical governing equations in the $\beta-$plane approximation at the Equator which incorporates a constant underlying current.

The aim of this work is to deal with a discontinuous Hamilton-Jacobi equation in the whole euclidian N-dimensional space, associated to a possibly unbounded optimal control problem. Here, the discontinuities are located on a hyperplane and the typical questions we address concern the existence and uniqueness of solutions, and of course the definition itself of solution. We consider viscosity solutions in the sense of Ishii. The convex Hamiltonians are associated to a control problem with specific cost and dynamics given on each side of the hyperplane. We assume that those are Lipschitz continuous but the main difficulty we deal with is that they are potentially unbounded, as well as the control spaces. Using Bellman's approach we construct two value functions which turn out to be the minimal and maximal solutions in the sense of Ishii. Moreover, we also build a whole family of value functions, which are still solutions in the sense of Ishii and connect continuously the minimal solution to the maximal one.

Triple negative breast cancer (TNBC) is highly immunogenic and high levels of tumor infiltrating lymphocytes (TILs) have been associated with a better prognosis and higher probability to achieve pathological complete response. Here, we explore the potential role of stromal TILs level and composition as a prognostic and predictive biomarker in TNBC. 195 Tumor biospecimens from patients diagnosed with TNBC were included. Stromal TILs (sTILs), positive CD4/CD8 cells were evaluated. Differences in clinic-pathological characteristics according to immune infiltration were assessed. The predictive and prognostic value of immune infiltration was analyzed by multivariate models. Higher immune infiltration was observed in patients with favorable clinical-pathological features. Survival analysis showed that longer overall survival times were observed in patients with a higher infiltration of sTILs (p = 0.00043), CD4 + (p = 0.0074) and CD8 + (p = 0.008). In the multivariate analysis, low levels of sTILs were found to be associated with a higher mortality hazard (HR: 1.59, 95% CI 1.01-2.48). CD4 and CD8 immune infiltration were associated with higher odds for pathological complete response (OR: 1.20, 95% CI 1.00-1.46, OR: 1.28, 1.02-1.65, respectively). Our results suggest that immune infiltration could be used as a prognostic marker for overall survival in TNBC patients.

14. The antioxidant and anti-inflammatory activities of caffeine effectively attenuate nonalcoholic steatohepatitis and thioacetamide-induced hepatic injury in male rats

The antioxidant effect of caffeine, associated with its ability to upregulate the nuclear factor-E2-related factor-2 (Nrf2)-signaling pathway, was explored as a possible mechanism for the attenuation of liver damage. Nonalcoholic steatohepatitis (NASH) was induced in rats by the administration of a high-fat, high-sucrose, high-cholesterol diet (HFSCD) for 15 weeks. Liver damage was induced in rats by intraperitoneal administration of thioacetamide (TAA) for six weeks. Caffeine was administered orally at a daily dose of 50mg/kg body weight during the period of NASH induction to evaluate its ability to prevent disease development. Meanwhile, rats received TAA for three weeks, after which 50 mg/kg caffeine was administered daily for three weeks with TAA to evaluate its capacity to interfere with the progression of hepatic injury. HFSCD administration induced hepatic steatosis, decreased Nrf2 levels, increased oxidative stress, induced the activation of nuclear factor-[kappa]B (NF-[kappa]B), and elevated proinflammatory cytokine levels, leading to hepatic damage. TAA administration produced similar effects, excluding steatosis. Caffeine increased Nrf2 levels; attenuated oxidative stress markers, including malondialdehyde and 4-hydroxynonenal; restored normal, reduced glutathione levels; and reduced NF-[kappa]B activation, inflammatory cytokine levels, and damage. In this work we analyze the behavior of the solutions to nonlocal evolution equations of the form $u_t(x,t) = \int J(x-y) u(y,t) \, dy - h_\epsilon(x) u(x,t) + f(x,u(x,t))$ with $x$ in a perturbed domain $\Omega^\epsilon \subset \Omega$ which is thought as a fixed set $\Omega$ from where we remove a subset $A^\epsilon$ called the holes. We choose an appropriated families of functions $h_\epsilon \in L^\infty$ in order to deal with both Neumann and Dirichlet conditions in the holes setting a Dirichlet condition outside $\Omega$. Moreover, we take $J$ as a non-singular kernel and $f$ as a nonlocal nonlinearity. % Under the assumption that the characteristic functions of $\Omega^\epsilon$ have a weak limit, we study the limit of the solutions providing a nonlocal homogenized equation.

There are well-established disparities in cancer incidence and outcomes by race/ethnicity that result from the interplay between structural, socioeconomic, socio-environmental, behavioural and biological factors. However, large research studies designed to investigate factors contributing to cancer aetiology and progression have mainly focused on populations of European origin. The limitations in clinicopathological and genetic data, as well as the reduced availability of biospecimens from diverse populations, contribute to the knowledge gap and have the potential to widen cancer health disparities. In this review, we summarise reported disparities and associated factors in the United States of America (USA) for the most common cancers (breast, prostate, lung and colon), and for a subset of other cancers that highlight the complexity of disparities (gastric, liver, pancreas and leukaemia). We focus on populations commonly identified and referred to as racial/ethnic minorities in the USA-African Americans/Blacks, American Indians and Alaska Natives, Asians, Native Hawaiians/other Pacific Islanders and Hispanics/Latinos. We conclude that even though substantial progress has been made in understanding the factors underlying cancer health disparities, marked inequities persist. Additional efforts are needed to include participants from diverse populations in the research of cancer aetiology, biology and treatment. Furthermore, to eliminate cancer health disparities, it will be necessary to facilitate access to, and utilisation of, health services to all individuals, and to address structural inequities, including racism, that disproportionally affect racial/ethnic minorities in the USA.

Purpose of the reviewBreast cancer incidence and mortality rates are lower in some Hispanic/Latino subpopulations compared to Non-Hispanic White women. However, studies suggest that the risk of breast cancer-specific mortality is higher in US Hispanics/Latinas. In this review we summarized current knowledge on factors associated with breast cancer incidence and risk of mortality in women of Hispanic/Latino origin.Recent findingsAssociative studies have proposed a multiplicity of factors likely contributing to differences in breast cancer incidence and survival between population groups, including socioeconomic/sociodemographic factors, lifestyle choices as well as access to and quality of care. Reports of association between global genetic ancestry overall as well as subtype-specific breast cancer risk among Hispanic/Latinas suggest that incidence and subtype distribution could result from differential exposure to environmental and lifestyle related factors correlated with genetic ancestry as well as germline genetic variation.SummaryHispanic/Latino in the United States have been largely underrepresented in cancer research. It is important to implement inclusive programs that facilitate the access of this population to health services and that also include education programs for the community on the importance of screening. In addition, it is important to continue promoting the inclusion of Hispanics/Latinos in genomic studies that allow understanding the biological behavior of this disease in the context of all human genetic diversity.

The last 10 years witnessed an acceleration of our understanding of what genetic factors underpin the risk of breast cancer. Rare high- and moderate-penetrance variants such as those in the BRCA genes account for a small proportion of the familial risk of breast cancer. Low-penetrance alleles are expected to underlie the remaining heritability. By now, there are about 180 genetic polymorphisms that are associated with risk, most of them of modest effect. In combination, they can be used to identify women at the lowest or highest ends of the risk spectrum, which might lead to more efficient cancer prevention strategies. Most of these variants were discovered in populations of European descent. As a result, we might be failing to discover additional polymorphisms that could explain risk in other groups. This review highlights breast cancer genetic epidemiology studies conducted in Latin America, and summarizes the information that they provide, with special attention to similarities and differences with studies in other populations. It includes studies of common variants, as well as moderate- and high-penetrance variants. In addition, it addresses the gaps that need to be bridged in order to better understand breast cancer genetic risk in Latin America.

Objectives: The aim of this study is to analyse the efficacy of using a combined infiltration between a popliteal artery and knee cap (IPACK) anaesthetic block and a selective saphenous nerve block compared to local infiltration with anaesthetic in knee replacement surgery. Methods: A retrospective observational study was conducted. A total of 312 patients who underwent primary total knee arthroplasty in our hospital between January 2019 and December 2022 were reviewed. Local intra-articular anaesthesia was used in 207 patients and combined nerve block in 105 patients (IPACK group). The mean age in the LIA group was 72.9 years and 70.4 years in the IPACK group. There were 44% men in the LIA group and 53.3% in the IPACK group. The primary outcome was the presence of poorly controlled pain requiring rescue opioid analgesia in the postoperative period. Secondary outcomes included pain scores, range of motion and length of hospital stay. Results: There were no significant differences in the age or gender distribution of patients between the two groups. One patient treated with anaesthetic blocks required rescue analgesia with opioids, while in the LIA group this occurred in 28.5% of cases. There were statistically significant higher VAS scores in the LIA group (p < 0.001). Range of motion was slightly greater in the block group (4.6°, p < 0.05). There were significant differences in hospital stay (2.4 days in the blocks group and 2.8 days in the LIA group (p < 0.05). Conclusions: In our series, patients treated with anaesthetic blocks showed better results with similar postoperative pain control. However, further studies are needed. [ABSTRACT FROM AUTHOR]

Context: Propofol is a general anesthetic used in multiple clinical scenarios. Despite growing evidence supporting its use in palliative care, propofol is rarely used in palliative sedation. Reluctance toward the adoption of propofol as a sedative agent is often associated with fear of adverse events such as respiratory arrest. Objectives: We aimed to describe efficacy and safety of palliative sedation in refractory sedation with propofol using a protocol based on low, incremental dosing. Methods: A retrospective observational study featuring inpatients receiving sedative treatment with propofol in our palliative care unit in Madrid (Spain) between March 1, 2018 and February 28, 2023, following a newly developed protocol. Results: During the study period, 22 patients underwent sedation with propofol. Propofol was used successfully to control different refractory symptoms, mainly psychoexistential suffering and delirium. All patients had undergone previous failed attempts at sedation with other medications (midazolam or lemovepromazine) and presented risk factors for complicated sedation. All patients achieved satisfactory (profound) levels of sedation measured with the Ramsay Sedation Scale, but total doses varied greatly between patients. Most patients (17, 77%) received combined therapy with propofol and other sedative medications to harness synergies. The median time between start of sedation with propofol and death was 26.0 hours. No cases of apnea or death during induction were recorded. Conclusion: A protocol for palliative sedation with propofol based on low, incremental dosing, with the option of administering an initial induction bolus, shows excellent results regarding adequate levels of sedation, without observing apnea or respiratory depression. Our results promote the use of propofol to achieve palliative sedation in patients with refractory symptoms and risk factors for complicated sedation at the end of life. [ABSTRACT FROM AUTHOR]

The web is changing the way in which data warehouses are designed, used, and queried. With the advent of initiatives such as Open Data and Open Government, organizations want to share their multidimensional data cubes and make them available to be queried online. The RDF data cube vocabulary (QB), the W3C standard to publish statistical data in RDF, presents several limitations to fully support the multidimensional model. The QB4OLAP vocabulary extends QB to overcome these limitations, allowing to im- plement the typical OLAP operations, such as rollup, slice, dice, and drill-across using standard SPARQL queries. In this paper we introduce a formal data model where the main object is the data cube, and define OLAP operations using this model, independent of the underlying representation of the cube. We show then that a cube expressed using our model can be represented using the QB4OLAP vocabulary, and finally we provide a SPARQL implementation of OLAP operations over data cubes in QB4OLAP.

The aim of this paper is to prove that a three dimensional Lagrangian flow which defines equatorially trapped water waves is dynamically possible. This is achieved by applying a mixture of analytical and topological methods to prove that the nonlinear exact solution to the geophysical governing equations, derived by Constantin in J. Geophys. Res., 117 (2012), is a global diffeomorphism from the Lagrangian labelling variables to the fluid domain beneath the free surface.

The aim of this paper is to provide a comprehensive study of some linear nonlocal diffusion problems in metric measure spaces. These include, for example, open subsets in $\mathbb{R}^N$, graphs, manifolds, multi-structures or some fractal sets. For this, we study regularity, compactness, positiveness and the spectrum of the stationary nonlocal operator. Then we study the solutions of linear evolution nonlocal diffusion problems, with emphasis in similarities and differences with the standard heat equation in smooth domains. In particular prove weak and strong maximum principles and describe the asymptotic behaviour using spectral methods.
