2. Transitioning to Higher Education: Students' Expectations and Realities

- Abstract
Background: The transition from school to higher education (HE) involves multiple challenges including academic issues, motivation and identity adaptation. In this scenario, the role of family, peers and institutions can be fundamental in the success of the transition process. Purpose: We sought to deepen our understanding of the transition phenomenon by studying students' perceptions on HE expectations versus the reality encountered, identifying the main challenges they faced and analysing the strategies used to cope with the difficulties encountered in adjusting to HE. Method: This is a follow-up interview study that examined the perceptions of 14 first- and second-year HE students in Chile, who had participated in a previous research study two years earlier whilst in secondary school. Interview data were analysed using a qualitative approach. Findings: The analysis identified contrasts between students' expectations and what occurred during the first two years of HE. Associated with academic difficulties, the students felt that the main challenges included increased complexity of content, time management and study strategies. To cope with these challenges, the evidence suggested that the students adapted their academic strategies and relied strongly on their families and peers. Conclusions: Our findings draw attention to a mismatch that can occur between the expectations and the reality that students face once they have transitioned to HE. Implications for institutions include building a transition bridge between secondary and postsecondary education, which goes beyond providing information about funding or the labour market. In terms of supporting successful transition, issues such as academic demand, studying time, study strategies and prior familiarity with HE staff and peers are all relevant aspects for consideration.

- Abstract
Background: The transition of high school students to higher education can be an overwhelming experience which may impact on academic outcomes. Despite increases in access, course completion rates remain problematic in Chile. Students' expectations of higher education can play an important role in their decision-making, especially in terms of choices made about programme and institution. Purpose: The purpose of this study was to better understand Chilean high school students' expectations of the benefits and costs involved in the transition process from secondary to postsecondary education. Also, it aimed to examine the factors that influence those expectations. Method: We used a qualitative design to collect information from 76 university-tracked high-school seniors through nine focus groups. All focus group data were double coded using a directed content analysis approach. Findings: The analysis indicated that students mentioned expectations of non-monetary benefits frequently, and in all focus groups. Though monetary benefits were not referred to directly, students identified them implicitly through non-monetary benefits. Expectations about costs were mentioned less often than were benefits. Conclusions: Our study highlights that the construction of expectations appeared chiefly influenced by the information obtained through family and peers. The findings from this study should help focus institutional and educational policymakers' efforts in supporting the decision-making process of students transitioning to higher education.

Dravet Syndrome (DS) is a rare autosomic encephalopathy with epilepsy linked to Nav1.1 channel mutations and defective GABAergic signaling. Effective therapies for this syndrome are lacking, urging a better comprehension of the mechanisms involved. In a recognized mouse model of DS, we studied GABA tonic current, a form of inhibition largely neglected in DS, in brain slices from developing mice before spontaneous seizures are reported. In neurons from the temporal cortex (TeCx) and CA1 region, GABA tonic current was reduced in DS mice compared to controls, while in the entorhinal cortex (ECx) it was not affected. In this region however allopregnanonole potentiation of GABA tonic current was reduced in DS mice, suggesting altered extrasynaptic GABAA subunits. Using THIP as a selective agonist, we found reduced δ subunit mediated tonic currents in ECx of DS mice. Unexpectedly in the dentate gyrus (DG), a region with high δ subunit expression, THIP-evoked currents in DS mice were larger than in controls. An immunofluorescence study confirmed that δ subunit expression was reduced in ECx and increased in DG of DS mice. Finally, considering the importance of neuroinflammation in epilepsy and neurodevelopmental disorders, we evaluated classical markers of glia activation. Our results show that DS mice have increased Iba1 reactivity and GFAP expression in both ECx and DG, compared to controls. Altogether we report that before spontaneous seizures, DS mice develop significant alterations of GABA tonic currents and glial cell activation. Understanding all the mechanisms involved in these alterations during disease maturation and progression may unveil new therapeutic targets.

- Abstract
Multicenter, prospective, observational study to compare the relative bioavailability of once-daily tacrolimus formulations in de novo kidney transplant recipients. De novo kidney transplant recipients who started a tacrolimus-based regimen were included 14 days post-transplant and followed up for 6 months. Data from 218 participants were evaluated: 129 in the LCPT group (Envarsus) and 89 in the PR-Tac (Advagraf) group. Patients in the LCPT group exhibited higher relative bioavailability (Cmin /total daily dose [TDD]) vs. PR-Tac (61% increase; P < .001) with similar Cmin and 30% lower TDD levels (P < .0001). The incidence of treatment failure was 3.9% in the LCPT group and 9.0% in the PR-Tac group (P = .117). Study discontinuation rates were 6.2% in the LCPT group and 12.4% in the PR-Tac group (P = .113). Adverse events, renal function and other complications were comparable between groups. The median accumulated dose of tacrolimus in the LCPT group from day 14 to month 6 was 889 mg. Compared to PR-Tac, LCPT showed higher relative bioavailability, similar effectiveness at preventing allograft rejection, comparable effect on renal function, safety, adherence, treatment failure and premature discontinuation rates.

Multicenter, prospective, observational study to compare the relative bioavailability of once-daily tacrolimus formulations in de novo kidney transplant recipients. OBJECTIVE To investigate the clinical outcomes of patients who underwent cardioversion compared with those who did not have cardioverson in a large dataset of patients with recent onset non-valvular atrial fibrillation. DESIGN Observational study using prospectively collected registry data (Global Anticoagulant Registry in the FIELD-AF-GARFIELD-AF). SETTING 1317 participating sites in 35 countries. PARTICIPANTS 52 057 patients aged 18 years and older with newly diagnosed atrial fibrillation (up to six weeks' duration) and at least one investigator determined stroke risk factor. MAIN OUTCOME MEASURES Comparisons were made between patients who received cardioversion and those who had no cardioversion at baseline, and between patients who received direct current cardioversion and those who had pharmacological cardioversion. Overlap propensity weighting with Cox proportional hazards models was used to evaluate the effect of cardioversion on clinical endpoints (all cause mortality, non-haemorrhagic stroke or systemic embolism, and major bleeding), adjusting for baseline risk and patient selection. RESULTS 44 201 patients were included in the analysis comparing cardioversion and no cardioversion, and of these, 6595 (14.9%) underwent cardioversion at baseline. The propensity score weighted hazard ratio for all cause mortality in the cardioversion group was 0.74 (95% confidence interval 0.63 to 0.86) from baseline to one year follow-up and 0.77 (0.64 to 0.93) from one year to two year follow-up. Of the 6595 patients who had cardioversion at baseline, 299 had a follow-up cardioversion more than 48 days after enrolment. 7175 patients were assessed in the analysis comparing type of cardioversion: 2427 (33.8%) received pharmacological cardioversion and 4748 (66.2%) had direct current cardioversion. During one year follow-up, event rates (per 100 patient years) for all cause mortality in patients who received direct current and pharmacological cardioversion were 1.36 (1.13 to 1.64) and 1.70 (1.35 to 2.14), respectively. CONCLUSION In this large dataset of patients with recent onset non-valvular atrial fibrillation, a small proportion were treated with cardioversion. Direct current cardioversion was performed twice as often as pharmacological cardioversion, and there appeared to be no major difference in outcome events for these two cardioversion modalities. For the overall cardioversion group, after adjustments for confounders, a significantly lower risk of mortality was found in patients who received early cardioversion compared with those who did not receive early cardioversion. - Abstract
El objetivo de la presente investigación es: Determinar cómo se relacionan las estrategias de marketing digital en la interactividad de la fanpage de la Municipalidad de Yanahuara en el año 2021. La muestra fue de 161 personas (88 mujeres y 73 varones) Metodología: La investigación sigue un diseño no experimental, relacional y transversal, se aplicaron dos cuestionarios diseñados por Panta J (2019 y Sotelo E (2017), los cuáles son válidos y confiables. Resultados: Se identifica el uso de marketing de contenido y red social, dónde 41% señala que no se mantiene un diálogo virtual, 50.3% nunca recibe un mensaje por medio de sus redes sociales, 82.6% señala no haber recibido notificaciones personalizadas en su emails o Messenger. El 44.7% consideran como útil la información de la fanpage , 49.1% considera útil el contenido gráfico, 33.5% indica regular satisfacción en respuesta a comentarios dados en la fanpage; 39.1% indica una regular presentación atractiva en las publicaciones y 30.4% señala regular rapidez ante las respuestas a dudas o comentarios realizados en la fanpage. Se observa frecuencia de 1 a 10 reacciones en likes (41.6%), me importa (42.2%), me encanta (46.6%), me divierte (37.9%), me entristece (28.6%), me asombra (23%) y me enoja (24.2%); 75.2% ha realizado menos de 5 comentarios positivos y 91.3% menos de 5 comentarios negativos; 88.2% comparte menos de 5 contenidos de la fanpage. Conclusión: Al análisis de Xi2 de Pearson, con un nivel de significancia menor a 0.05; se comprueba que las estrategias de marketing digital presentan relación directa y significativa con la interactividad de la fan page de la Municipalidad de Yanahuara.

La presente investigación tuvo como objetivo conocer las acciones de endomarketing en la Municipalidad Distrital de Alto Selva Alegre, dentro de los objetivos específicos se encuentran desarrolladas las cuatro “C” del endomarketing. La muestra fue de 212 trabajadores municipales (106 varones y 106 mujeres), se utilizó un muestreo de tipo probabilístico aleatorio. Para la recolección de dato, la técnica utilizada fue la encuesta y el instrumento fue el cuestionario dividido de la siguiente forma: una primera parte sobre datos generales de la persona como sexo, rango de edad, condición laboral y años de servicio, la segunda parte corresponde a preguntas enfocadas a las acciones de endomarketing de acuerdo al esquema de las cuatro “C” En cuanto a los resultados obtenidos en esta investigación que se explican en un total de 22 cuadros y 22 gráficos estadísticos, se encontró que las acciones de endomarketing en la institución estudiada tiene puntos fuertes y débiles, ya que solo algunos conceptos del endomarketing como son Compañía y Coordinación son aplicados correctamente y otros como Costos y Comunicación no son tan eficientes como se requiere para tener a un trabajador satisfecho.

New onset epilepsy and seizures are common neurological disorders in aged people, second only to stroke and dementia. They are frequently related to other pathological conditions including stroke, trauma, tumors and neurological diseases whereas in about one-third of cases the origin is unknown. Besides the origin, the cellular and molecular events that suddenly trigger seizures are poorly defined. Using an acute model of seizure generation that better resembles new onset seizures, we studied GABAergic interneurons and astrocytes during seizure generation. We found that seizures are preceded by a GABAergic rhythmic hyperactivity that synchronizes pyramidal neurons by inducing a rebound spiking that favors seizures' onset.
