Science - Abstract
Abstract Intrinsically disordered regions (IDRs) are highly enriched in the nucleolar proteome but their physiological role in ribosome assembly remains poorly understood. Our study reveals the functional plasticity of the extremely abundant lysine-rich IDRs of small nucleolar ribonucleoprotein particles (snoRNPs) from protists to mammalian cells. We show in Saccharomyces cerevisiae that the electrostatic properties of this lysine-rich IDR, the KKE/D domain, promote snoRNP accumulation in the vicinity of nascent rRNAs, facilitating their modification. Under stress conditions reducing the rate of ribosome assembly, they are essential for nucleolar compaction and sequestration of key early-acting ribosome biogenesis factors, including RNA polymerase I, owing to their self-interaction capacity in a latent, non-rRNA-associated state. We propose that such functional plasticity of these lysine-rich IDRs may represent an ancestral eukaryotic regulatory mechanism, explaining how nucleolar morphology is continuously adapted to rRNA production levels.

Abstract The authors propose a system of closely coupled‐complementary frequency‐selective surfaces (FSSs) based on the four‐arms star geometry with dual‐ and triple‐passbands responses for 5G applications. Four complementary structure configurations are presented, and depending on the chosen configuration, the structure can create two or three transmission bands in one or both polarisations of the electromagnetic waves. The designed FSSs are compact and have stable behaviour at different incident angles. The complementary FSS passes signals around 2.6 and 6.2 GHz, and a structural offset allows the transmission at a third frequency (4.2 GHz). To validate the proposed designs, four prototypes are fabricated and measured. Numerical and experimental characterisations are in good agreement.

Abstract This work proposes the integration of a monopole antenna with a frequency selective surface (FSS) with double square loop geometry, which operates in the ISM (2.4 - 2.4835 GHz) and IEEE 802.11 WLAN (5.15-5.725 GHz) bands. The FSS was developed to be integrated into a microstrip monopole antenna, aiming to improve some of its radiative features, such as gain, directivity, and front-to-back ratio (FBR). We show that the double square loop FSS operates in bands from 1.91 to 3 GHz and from 4.64 to 6.56 GHz, fully covering the bands of interest. A prototype of the proposed structure was assembled and both the numerical simulation and the experimental results show that the antenna integrated with FSS presents significant improvements in all parameters, compared to the antenna without FSS. Because of the interest frequency ranges the proposed structure can be applied in non-invasive biomedical applications.

ABSTRACT Objectives: To evaluate the early and late results of surgical treatment of patients with bronchiectasis, comparing the Video-Assisted Thoracic Surgery (VATS) vs. the open thoracotomy (OT). Methods: Observational retrospective study of patients who underwent surgery for bronchiectasis. Patients were divided into two groups according to surgical access OT/VATS. Variables collected included gender, age, preoperative symptoms, etiology, segments involved, FVC and FEV1, type of surgical resection, complications, mortality, and length of hospital stay. Late surgical results were classified as excellent, complete remission of symptoms; good, significative improvement; and poor, little/no improvement. Results: 108 surgical resections (103 patients). OT group 54 patients (52.4%) vs. VATS 49 (47.6%). A high percentage of complications was observed, but no difference between the OT (29.6%) and VATS (24.5%) groups was found. Post-operative hospital stay was shorter in the VATS group (5.4 days) vs. the OT group (8.7 days (p=0.029). 75% of the patients had a late follow-up; the results were considered excellent in 71.4%, good in 26%, and poor in 2.6%. Regarding bronchiectasis distribution, an excellent percentage was obtained at 82.1% in patients with localized bronchiectasis and 47.5% with non-localized bronchiectasis, p=0.003. Conclusions: VATS leads to similar results regarding morbidity, compared to OT. However, VATS was related to shorter hospital stays, reflecting the early recovery. Late results were excellent in most patients, being better in patients with localized bronchiectasis. VATS should be considered a preferable approach for bronchiectasis lung resection whenever possible.

A 78-year-old woman presented to hospital with altered mental status. Hyponatremia was diagnosed and treated. She maintained mental confusion despite normal sodium. No headache, fever or focal signs were present. CSF analysis showed chronic meningitis; MRI demonstrated basilar enhancement. She was empirically treated for tuberculosis, with no improvement. PCR for Sporothrix in the CSF was positive. After treatment with Amphotericin followed by oral itraconazole, she completely recovered her cognitive abilities. Follow-up CSF was normal. This report illustrates the need to consider sporotrichosis in the differential diagnosis of chronic meningitis in immunocompetent, and the importance of PCR as a diagnostic tool.

Abstract This paper introduces a novel reconfigurable superstrate using a band-stop frequency selective surface (FSS) based on four arms star geometry. The proposed reconfigurable superstrate is applied to a dipole antenna for use in 5G NR technology with a frequency of 3.545 GHz. The study includes both simulated and experimental results which confirm the redirection of the antenna radiation pattern. The developed structure exhibits radiation in only one half-space, high polarization isolation, and a redirection of the radiation pattern up to ±55°. The ideal cases were considered, where the ON and OFF states of the diodes were replaced by short circuits and gaps, respectively. Overall, the results demonstrate the effectiveness of the proposed FSS as a promising solution for 5G NR technology applications.

ABSTRACT Purpose: This study evaluated the prevalence of complications in the postoperative period of esophagogastric oncological surgeries. Methods: We conducted a retrospective cross-sectional study, adhering to the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) guidelines. The study size implied 163 patients who underwent surgical treatment for esophageal and gastric cancer and experienced postoperative complications between January 2018 and December 2022. These patients were treated at the Liga Norte Riograndense Contra o Câncer, a high-complexity oncology center and a reference for cancer treatment in Northeast Brazil. Results: The prevalence found was 88.3%. The most prevalent complications were Clavien-Dindo I and II, and infection was the most common. According to our statistics analysis, hypoalbuminemia showed a positive correspondence with the occurrence of postoperative complications (odds ratio = 8.60; 95% confidence interval 1.35–54.64, p = 0.0358). Conclusions: Postoperative complications of gastroesophageal surgeries increase patient morbidity and mortality.

Abstract The equivalent wave impedance circuit model (ECM-WI) is presented for the analysis of frequency selective surfaces (FSS). The development of the analytical model is based on homogenization theory, as well as analogies between wave theory and transmission line theory. The coplanar strip model and the concept of wave impedance are used to scale the Marcuvitz model for periodic strip gratings of conductive strips, as well as to consider the influence of the presence of a lossy dielectric substrate on the frequency response of the FSS. In particular, the model is applied to the analysis of square loop FSSs and considering the normal incidence. The results obtained are presented by comparing full-wave simulations for the two proposed models, obtaining average deviations in absolute values (%) of 2.11 and 0.38 for resonance frequency and 3.45 and 5.49 for bandwidth. The prototype manufactured in FR-4 obtained a deviation of 2.56% for resonance frequency and 2.79% for bandwidth.

Abstract This paper presents a novel nature-inspired tri-band frequency selective surface (FSS) characterized by closely spaced bands, angular stability, and polarization independence. The FSS element adopts a segmented Fibonacci spiral geometry. While a single-segmented Fibonacci spiral does not inherently possess polarization independence, the proposed proof of concept achieves this by cascading the same design geometry rotated by 90° between them. Resonant frequencies of 1.63 GHz, 2.46 GHz, and 3.43 GHz are achieved, with minimal separation ratios of 1.5 from the second to the first frequency and 1.4 from the third to the second. Additionally, angular stability is confirmed up to 45°. Simulation of the FSS is conducted using HFSS software, and the simulated results are rigorously validated against experimental data, demonstrating excellent agreement.

The Last Photo” (A Última Fotografia).

In this paper, a compact matryoshka DGS using dielectric resonator (DR) is proposed. Despite the use of dielectric resonators to obtain band-stop frequency response not being a new idea, the proposed compact matryoshka DGS is an original contribution, since only recently the matryoshka geometry was used in DGS applications. Furthermore, a numerical analysis of the positioning of the DR is presented, which makes it possible to determine the region of the DGS for which the minimum resonant frequency is reached, and the resonant frequency range that can be obtained. To the best of the authors’ knowledge, this numerical analysis is an unpublished result, even more so, for the matryoshka DGS. The matryoshka geometry was described, including initial design equations. To verify the expected characteristics (miniaturization, selectivity, and resonant frequency tunability), two compact matryoshka DGSs (DGS1 and DGS2) were designed using a high permittivity ceramic (MCT-115) as DR. The obtained numerical and experimental results showed good agreement, and the initial design equations proved to be applicable, which allows new dimensions to be determined for other resonant frequencies, according to the application requirements. Resonant frequencies ranging from the maximum value to 33% (DGS1) and 27% (DGS2) were achieved, depending on the DR position. Considering $\lambda _{0}$ the wavelength in free space, corresponding to the resonant frequency, DGS1 and DGS2 achieved an occupied area of $0.04\lambda _{0}\times 0.04\lambda _{0}$ , and $0.05\lambda _{0}\times 0.05\lambda _{0}$ , respectively, a good miniaturization. The proposed compact matryoshka DGS may be especially attractive for applications that require a very selective band-stop frequency response. If a wider band-stop is required, the proposed DGS can be cascaded.

Abstract This paper proposes a methodology for developing antipodal Vivaldi antennas using RSIW (Ridge Substrate Integrated Waveguide) technology for a specific cut-off frequency. The technique employs metallic-vias and metalized cylindrical posts of reduced sizes inserted in the dielectric substrate. The simulations were performed using the ANSYS® Electronics Desktop software. The antenna RSIW is designed for a resonant frequency of 4 GHz and was fabricated on FR4 (ɛr=4.4 and tanδ=0.02). The structure is compared to an antipodal Vivaldi antenna (AVA) fed by a SIW. The structure proposed had an increase in bandwidth of 4.09 GHz and an increase in gain of 1.41 dBi.

ABSTRACT Although lung cancer (LC) is one of the most common and lethal tumors, only 15% of patients are diagnosed at an early stage. Smoking is still responsible for more than 85% of cases. Lung cancer screening (LCS) with low-dose CT (LDCT) reduces LC-related mortality by 20%, and that reduction reaches 38% when LCS by LDCT is combined with smoking cessation. In the last decade, a number of countries have adopted population-based LCS as a public health recommendation. Albeit still incipient, discussion on this topic in Brazil is becoming increasingly broad and necessary. With the aim of increasing knowledge and stimulating debate on LCS, the Brazilian Society of Thoracic Surgery, the Brazilian Thoracic Association, and the Brazilian College of Radiology and Diagnostic Imaging convened a panel of experts to prepare recommendations for LCS in Brazil. The recommendations presented here were based on a narrative review of the literature, with an emphasis on large population-based studies, systematic reviews, and the recommendations of international guidelines, and were developed after extensive discussion by the panel of experts. The following topics were reviewed: reasons for screening; general considerations about smoking; epidemiology of LC; eligibility criteria; incidental findings; granulomatous lesions; probabilistic models; minimum requirements for LDCT; volumetric acquisition; risks of screening; minimum structure and role of the multidisciplinary team; practice according to the Lung CT Screening Reporting and Data System; costs versus benefits of screening; and future perspectives for LCS.

Abstract: This study investigates the presence of bacterial dominance in one of the most studied sewage-driven eutrophic coastal lagoons, the Imboassica Lagoon in Macaé (RJ), Brazil, utilizing high-throughput sequencing of 16S rDNA. Water samples were collected from three sites within the lagoon. Total microbial DNA was extracted, and the V3-V4 region of the 16S rRNA gene was amplified and sequenced on the Illumina MiSeq platform. A total of 744,879 partial 16S rRNA sequences were clustered, revealing the absence of a single bacterial dominance in the sewage-driven eutrophic coastal lagoon. The prominent phyla detected in the lagoon were Cyanobacteria (27.8%), Proteobacteria (23.7%), and Actinobacteria (14.6%). Proteobacteria emerged as the most abundant phylum in the sewage-impacted lagoon site, whereas Cyanobacteria dominated the other two sampling sites. Among families, Synechococcaceae predominated with genus Synechococcus exhibited the highest prevalence. Families of potentially toxic Cyanobacteria represented less than 1% of the total families. The sewage-impacted lagoon section displayed greater bacterial diversity and richness. The dominance of bacterial communities associated with raw sewage, such as members of the Enterobacteriaceae family, was not confirmed, constituting only 0.75% of the families in the most affected site. This study presents the initial analysis of the bacterial community in the Imboassica Lagoon and suggests that dominance in the lagoon responds to the eutrophication and sewage discharge.

To review the evidence about inspiratory muscle training (IMT) in patients in postoperative of cardiac surgery. We conducted this systematic review used the databases Ovid, LILACS, CINAHL, PubMed, PEDro, and CENTRAL. Randomized clinical trials that addressed IMT after cardiac surgery were selected. The outcomes assessed were maximum inspiratory pressure (MIP), maximum expiratory pressure (MEP), tidal volume (TV), peak expiratory flow (PEF), functional capacity (6-minute walk test) and length of hospital stay. The mean difference between groups and the respective 95% confidence interval (CI) were calculated and used to quantify the effect of continuous outcomes. Seven studies were selected. The IMT was superior to the control over MIP 15.77 cmH2O (95% CI, 5.95–25.49), MEP 15.87 cmH2O (95% CI, 1.16–30.58), PEF 40.98 L/min (95% CI, 4.64–77.32), TV 184.75 mL (95% CI, 19.72–349.77), hospital stay -1.25 days (95% CI, -1.77 to -0.72), but without impact on functional capacity 29.93 m (95% CI, -27.59 to 87.45). Based on the results presented, IMT was beneficial as a form of treatment for patients after cardiac surgery.

Abstract This work presents the development of a reconfigurable frequency selective surface (RFSS) for application in the 2400-2483.5 MHz band, standard IEEE 802.11b/g/n (Wi-Fi). The proposed RFSS is based on the four-arms star geometry and PIN diodes were used as switching elements. In the initial design, numerical and measured results, especially for y polarization, PIN diode on-state, shown a difference of 15% for the resonant frequency. To overcome this drawback, a scaling factor was adopted and RFSS was redesigned and characterized, achieving the desired frequency response. The RFSS reconfigurability is confirmed, with a variation of at least 15 dB, when switching the states of the PIN diode. Moreover, the signal strength was measured directly on a notebook, confirming the reconfiguration of the RFSS. Finally, for the y polarization, it was found that for this polarization the frequency response remains virtually unchanged, even for angles of incidence up to 45°. In addition to the Wi-Fi signal control, the observed features make the developed RFSS especially attractive for reconfigurable antennas applications.

Abstract Frequency Selective Surfaces (FSS) are increasingly being used in telecommunications systems due to the numerous advantages presented by this sort of structure, among them low cost, ease of fabrication, and low profile stand out. This work reports the design, fabrication, and characterization of a multilayer frequency-selective absorber (FSA) for broadband operation. The proposed structure shows an absorption performance (operating in the frequency range between 2 GHz and 6 GHz) within the transmission band. The absorber consists of cascaded frequency selective surfaces, which are composed of both conductive square loops, which reflect incident signals, and resistive ones, which act as an absorbing layer. To verify the absorbing structure performance, full-wave numerical simulations and measurements are presented. The measured results are in good agreement with the numerical ones and show that the design performs absorption above 80% within the range of 2.48 GHz to 6.13 GHz, which verifies the project properties. Besides that, numerical results show that the proposed absorber has a proper response under oblique incidence up to 30°.

A sanitary challenge was carried out to induce suboptimal herd health while investigating the effect of amino acids supplementation on piglet responses. Weaned piglets of high sanitary status (6.33 ± 0.91 kg of BW) were distributed in a 2 × 2 factorial arrangement into two similar facilities with contrasting sanitary conditions and two different diets. Our results suggest that increased Trp, Thr, and Met dietary supplementation could support the immune systems of piglets under a sanitary challenge. In this manner, AA+ supplementation improved the performance and metabolism of piglets under mixed management and poor sanitary conditions. No major temporal microbiome changes were associated with differences in performance regardless of sanitary conditions or diets. Since piglets often become mixed in multiple-site production systems and facility hygiene is also often neglected, this study suggests that increased Trp, Thr, and Met (AA+) dietary supplementation could contribute to mitigating the side effects of these harmful risk factors in modern pig farms.

A review on planar printed structures that are based on Matryoshka-like geometries is presented. These structures use the well-known principle of Matryoshka dolls that are successively nested inside each other. The well-known advantages of the planar printed technology and of the meandered nested Matryoshka geometries are combined to generate miniaturized, multi-resonance, and/or wideband configurations. Both metal and complementary slot structures are considered. Closed and open configurations were analyzed. The working principles were explored in order to obtain physical insight into their behavior. Low-cost and single-layer applications as frequency-selective surfaces, filters, antennas, and sensors, in the microwave frequency region, were reviewed. Potential future research perspectives and new applications are then discussed.

Quality of life -- Health aspects - Abstract
Objective. The purpose of this study was to compare high-intensity interval training (HIIT) with moderate-intensity continuous training (MICT) for feasibility, safety, adherence, and short- and long-term efficacy in improving functioning and health-related quality of life in survivors of coronavirus disease 2019 (COVID-19). Methods. COVIDEX is a two-pronged, parallel-group, randomized controlled trial with an 8-week training intervention. The study participants will be 94 patients aged >18 years, admitted to a specialized post-COVID center. Participants will be randomized to HIIT (4 * 4 minutes of high-intensity work periods at 85% to 90% of peak heart rate) and MICT (47 minutes at 70% to 75% peak heart rate) groups for biweekly sessions for 8 weeks. The participants will undergo 2 phases of supervised training (phases 1 and 2) of 4 weeks each, in a public, specialized, post-COVID center. In phase 1, we will assess and compare the feasibility, acceptability, and short-term efficacy of HIIT and MICT intervention. In phase 2, the long-term efficacy of HIIT and MICT will be assessed and compared regarding function and health-related quality of life. To prevent any expectation bias, all study participants and assessors will be blinded to the study hypotheses. Group allocation will be masked during the analysis. All statistical analyses will be conducted following intention-to-treat principles. Impact. This study is the first randomized controlled trial that will compare the feasibility, safety, adherence, and efficacy of the HIIT and MICT intervention programs in this population. The findings will potentially provide important information and assist in clinical decision making on exercise to optimize the benefits of clinical health care in survivors of COVID-19. Keywords: Randomized, Feasibility, Participants, Efficacy, Moderate-Intensity Continuous Training, High-Intensity Interval Training, Background and Rationale A significant number of survivors of COVID-19 may experience persistent dyspnea, fatigue, physical impairment, and disabilities. Swine are a major reservoir of an array of zoonotic Salmonella enterica subsp. enterica lineage I serovars including Derby, Typhimurium, and 4,[5],12:i:- (a.k.a. Monophasic Typhimurium). In this study, we assessed the gastrointestinal (GI) microbiome composition of pigs in different intestinal compartments and the feces following infection with specific zoonotic serovars of S. enterica (S. Derby, S. Monophasic, and S. Typhimurium). 16S rRNA based microbiome analysis was performed to assess for GI microbiome changes in terms of diversity (alpha and beta), community structure and volatility, and specific taxa alterations across GI biogeography (small and large intestine, feces) and days post-infection (DPI) 2, 4, and 28; these results were compared to disease phenotypes measured as histopathological changes. As previously reported, only S. Monophasic and S. Typhimurium induced morphological alterations that marked an inflammatory milieu restricted to the large intestine in this experimental model. S. Typhimurium alone induced significant changes at the alpha- (Simpson’s and Shannon’s indexes) and beta-diversity levels, specifically at the peak of inflammation in the large intestine and feces. Increased community dispersion and volatility in colonic apex and fecal microbiomes were also noted for S. Typhimurium. All three Salmonella serovars altered community structure as measured by co-occurrence networks; this was most prominent at DPI 2 and 4 in colonic apex samples. At the genus taxonomic level, a diverse array of putative short-chain fatty acid (SCFA) producing bacteria were altered and often decreased during the peak of inflammation at DPI 2 and 4 within colonic apex and fecal samples. Among all putative SCFA producing bacteria, Prevotella showed a broad pattern of negative correlation with disease scores at the peak of inflammation. In addition, Prevotella 9 was found to be significantly reduced in all Salmonella infected groups compared to the control at DPI 4 in the colonic apex. In conclusion, this work further elucidates that distinct swine-related zoonotic serovars of S. enterica can induce both shared (high resilience) and unique (altered resistance) alterations in gut microbiome biogeography, which helps inform future investigations of dietary modifications aimed at increasing colonization resistance against Salmonella through GI microbiome alterations.

- Abstract
Background: Disorders affecting the nervous system are diverse and include neurodevelopmental disorders, late-life neurodegeneration, and newly emergent conditions, such as cognitive impairment following COVID-19. Previous publications from the Global Burden of Disease, Injuries, and Risk Factor Study estimated the burden of 15 neurological conditions in 2015 and 2016, but these analyses did not include neurodevelopmental disorders, as defined by the International Classification of Diseases (ICD)-11, or a subset of cases of congenital, neonatal, and infectious conditions that cause neurological damage. Here, we estimate nervous system health loss caused by 37 unique conditions and their associated risk factors globally, regionally, and nationally from 1990 to 2021. Methods: We estimated mortality, prevalence, years lived with disability (YLDs), years of life lost (YLLs), and disability-adjusted life-years (DALYs), with corresponding 95% uncertainty intervals (UIs), by age and sex in 204 countries and territories, from 1990 to 2021. We included morbidity and deaths due to neurological conditions, for which health loss is directly due to damage to the CNS or peripheral nervous system. We also isolated neurological health loss from conditions for which nervous system morbidity is a consequence, but not the primary feature, including a subset of congenital conditions (ie, chromosomal anomalies and congenital birth defects), neonatal conditions (ie, jaundice, preterm birth, and sepsis), infectious diseases (ie, COVID-19, cystic echinococcosis, malaria, syphilis, and Zika virus disease), and diabetic neuropathy. By conducting a sequela-level analysis of the health outcomes for these conditions, only cases where nervous system damage occurred were included, and YLDs were recalculated to isolate the non-fatal burden directly attributable to nervous system health loss. A comorbidity correction was used to calculate total prevalence of all conditions that affect the nervous s

Surgical wounds, when not amenable to primary healing, can be treated using surgical flaps or grafts, increasing surgery complexity and costs. Seeking to facilitate and optimize access to surgical treatment of malignant neoplasms of the skin, the dressing using the Figueiredo technique is a good therapeutic option due to its easy execution and reproducibility, low cost, and effectiveness. We report a series of cases using this technique after the excision of non-melanoma skin tumors in the extremities.
