2. Acessibilidade arquitetônica e percepção de quedas de idosos no peridomicílio: método misto

Abstract

Objetivo: Descrever conteúdos, estrutura e origem das representações sociais sobre queda apresentadas por pessoas idosas e condições estruturais peridomiciliares que predispõem à queda e relacionar as implicações dessas evidências empíricas na rotina de idosos no contexto arquitetônico.Método: Misto convergente por triangulação. Utilizaram-se as abordagens qualitativa (estrutural, n=195 e processual, n=40 da Teoria das Representações Sociais) e quantitativa (seccional descritiva, n=183). Foram entrevistados idosos no domicílio adscritos à atenção primária, em 2018. Técnicas de análise: categorial-temática, prototípica, estatística e dedutiva segundo Leininger.Resultados: Categorias de análise: 1) Peridomicílio: cenário de queda e 2) Envelhecimento e vulnerabilidade: risco de queda no peridomicílio. Apresentaram-se como características ambientais precursoras de quedas: pisos irregulares, buracos, desníveis e objetos no percurso.. Sentimento e comportamentos alocados no possível núcleo central se associam, justificando as quedas e objetivando suas causas.Conclusão: Evidenciou-se a associação entre o ambiente arquitetônico peridomiciliar e as características preditoras do risco de quedas.Palavras-chave: Idoso. 3. Imagem corporal da mulher durante amamentação: análise suportada em teoria de enfermagem

Abstract

Objetivo: Descrever e interpretar a percepção da mulher acerca da sua imagem corporal durante o processo de amamentar.Método: Estudo descritivo qualitativo realizado em hospital universitário da região sudeste, Brasil. Foram entrevistadas 43 puérperas que estavam amamentando.As entrevistas foram submetidas à análise lexical com o software IRAMUTEQ e interpretadas baseadas na Teoria Interativa de Amamentação.Resultados: As mulheres referem insatisfação com as mudanças na imagem corporal durante a amamentação. Mas também, valorizam e desejam manter a amamentação devido as vantagens para o filho. E várias expressam o desejo de realizar cirurgia plástica futuramente frente a essas mudanças corporais.Conclusão: A percepção da mulher sobre suaimagem corporal como satisfatória/insatisfatória aponta que as mudanças corporais revestem o processo de amamentação de sentimentos de ambiguidade.As mudanças corporais são percebidas pelas mulheres de forma pessoal, subjetiva e complexa.Palavras-chave: Imagem corporal. Abstract

To identify a clinical judgment of susceptibility referring to the development of glycemic fluctuations in adults with Diabetes Mellitus undergoing treatment. Theoretical study with construct analysis. The exploration of the phenomena of glycemic variations provided clues for the description of the blood glucose pattern construct and the proposal of a new risk Nursing diagnosis as a judgment of susceptibility to the balance of this pattern. The risk factors for "Risk for Imbalanced Blood Glucose Pattern" are cognitive dysfunction; excessive alcohol consumption; excessive daily exercise; inadequate follow through with treatment regimen; increased frequency of self-monitoring of blood glucose; inadequate knowledge of disease process; inadequate management of amount of food; inadequate regularity of meal consumption; obesity; overweight; smoking; underweight. The elaboration of "Risk for Imbalanced Blood Glucose Pattern" Nursing diagnosis contributes to the advancement in the Nursing classifications and to the elaboration of planning actions and specific interventions. Abstract

11 páginas Objetivos: identificar en la literatura científica los principales factores asociados al destete temprano. Materiales y método: se trata de una revisión sistemática realizada en las bases de datos LILACS y MEDLINE con artículos completos del periodo de 2004 a 2013, en inglés, español y portugués, desde la pregunta orientadora: “¿Cuáles son los factores que influyen el destete temprano?”. Resultados: se identificaron 1.481 artículos y 39 atendieron a los criterios de inclusión. Entre los principales factores que influyen el destete temprano, se encontraron trabajo materno (33,3 %); uso de chupo de entretención (30,8 %); leche materna débil (17,9 %); trauma y dolor mamilar (17,9 %); introducción de otros tipos de leche (15,4 %) y nivel educacional de la madre o del padre (15,4 %). Conclusiones: diversos factores están relacionados al destete temprano, lo que apunta fuerte determinación sociocultural e histórica que se puede evidenciar por la comparación de los estándares de lactancia entre diferentes poblaciones y a través de los tiempos Objective: Identify, in scientific literature, the main factors associated with early weaning. Method: This study is a systematic review of the Lilacs and Medline databases. The focus is on complete articles in English, Spanish and Portuguese published between 2004 and 2013. The guiding question is: "What factors influence early weaning?" Results: In all, 1,481 articles were identified and 39 met the inclusion criteria. The main factors that influence early weaning included, among others, maternal labor (33.3%), use of pacifiers (30.8%), weak milk (17.9%), trauma and nipple pain (17.9%), Introduction of other types of milk (15.4%), and the mother’s/father’s education (15.4%). Conclusion: Several factors are related to early weaning, which exhibits a strong sociocultural and historical determination that can be evidenced by a comparison of breastfeeding patterns among different populations and in the course of time.

Abstract

Background: The diagnosis of a chronic and disabling disease implies profound changes in family dynamics, with increased demands for the provision of nursing care to the family. Objectives: To validate the content of the scale to assess the impact of neurocognitive disorder on the family through the Delphi panel. Methodology: The methodology used was the Delphi method, using a panel of 42 experts in the area to gather consensus about the pertinence and content of a set of items. Data collection and analysis in the first round took place at the beginning of April and the second round at the end of June 2018. Results: Of the 86 items presented to experts, adaptations in 10 items were suggested to make them clearer and easier to understand. The final version consists of 68 items. Conclusion: The results of the study made it possible to find, among experts, the consensus that allows validating the content of the proposal for an instrument to assess the impact of neurocognitive disorders in the family. Abstract

Objective.Identify the factors that influence women in the decision to breastfeed. Methods. Integrative review. Information was gathered from original articles, case studies, theoretical studies, consensus and systematic reviews published between 2007-2013 in Spanish, Portuguese and English and recovered in the databases MEDLINE and LILACS. The descriptors used in this study were: breastfeeding, maternal behavior, risk factors, lactation and newborn. Results. Were included 30 articles, grouped into five categories. Factors influencing the decision of the breastfeeding woman are a convergence of breastfeeding's advantages, benefits and justifications, family, social and professional support, sociodemographic and clinical characteristics of women, personal experience and family tradition and personal choice. Conclusion. The decision to breastfeed by women is influenced by a convergence of factors. It is essential the role of nursing to encourage women in the decision to initiate and maintain breastfeeding her child. Objetivo.Identificar los factores que influencian en las mujeres la decisión de amamantar. Métodos. Revisión integrativa. La información se recolectó a partir de artículos originales, estudios de casos, estudios teóricos, consensos y revisiones sistemáticas e integrativas, publicados entre 2007-2013, disponibles en los idiomas español, portugués e inglés y recuperados en las bases de datos MEDLINE y LILACS. Los descriptores utilizados en la búsqueda fueron: lactancia materna, comportamiento materno, factores de riesgo, lactancia y recién nacido. Resultados. Se incluyeron 30 artículos agrupados en cinco categorías. Los factores que influencian en las mujeres la decisión de amamantar son una convergencia de ventajas, beneficios y justificaciones de la lactancia materna, entre ellos: apoyo familiar, social y profesional; características sociodemográficas y clínicas de las madres; experiencia personal y tradición familiar; y, finalmente, decisión personal. Conclusión. La decisión de amamantar en las mujeres es una convergencia de factores. Por esto, es fundamental el papel de enfermería en fomentar la decisión de la mujer de iniciar y mantener la lactancia materna. Os descritores usados na busca foram: amamentação, comportamento materno, fatores de risco, lactação e recém-nascido. Resultados. Foram incluídos 30 artigos agrupados em cinco categorias. Os fatores que influenciam a decisão da mulher de amamentar são uma convergência de vantagens, benefícios e justificativas da amamentação, suporte familiar, social e profissional, características sócio-demográficas e clínicas das mulheres, experiência pessoal e tradição familiar e escolha pessoal. Conclusão. A decisão das mulheres de amamentar é influenciada por uma convergência de fatores. é fundamental o papel da enfermagem em incentivar a decisão da mulher de iniciar e manter a amamentação de seu filho.

45. Diagnostic evaluation of risk for bleeding in cardiac surgery with extracorporeal circulation.

Abstract

Objective: to identify the risk factors associated with cases of excessive bleeding in patients submitted to cardiac surgery with extracorporeal circulation. Method: case-control study on the factors of risk for bleeding based on the analysis of data from the medical charts of 216 patients submitted to cardiac surgery with elective extracorporeal circulation during a three-year period. Results: variables that are commonly associated with excessive bleeding in studies in the field were analyzed, and the following were considered as risk factors for the nursing diagnosis "risk for bleeding" (00206) in cardiac surgery with extracorporeal circulation: Body mass index lower than 26.35kg/m² (Odds ratio = 3.64); Extracorporeal circulation longer than 90 minutes (Odds ratio = 3.57); Hypothermia lower than 32°C (Odds ratio = 2.86); Metabolic acidosis (Odds ratio = 3.50) and Activated partial thromboplastin time longer than 40 seconds (Odds ratio = 2.55). Conclusion: such variables may be clinical indicators of an operational nature for a better characterization of the risk factor "treatment regimen" and a refinement of knowledge related to coagulopathy induced by extracorporeal circulation, which is currently presumably incorporated into the "treatment regimen" category of the nursing diagnostic classification by NANDA International, Inc.
