Introduction: In the implementation of public policy, there are many factors and obstacles, the identification of which will lead to the efficient way of implementing the policies. Method: The present research was conducted with the aim of identifying and prioritizing the obstacles to the implementation of Iran's social welfare policies. The research is of a meta-composite type, in the qualitative part, semi-structured interviews were used so as to collect the data with open coding, and finally the obstacles to the implementation of social welfare policies were identified. In the quantitative part, using the review of related literature and research studies, six fundamental research studies in the field of policymaking were selected according to the citations and the items affecting the implementation of policies having been extracted from other studies in the form of commonalities. Then, the 10 experts cooperated, and the three-step Delphi method was used. Findings: Based on Friedman's test, ten priorities were identified, which are as follows: existence of opposing and supporting parties, technical difficulties, lack of use of new technologies, legitimacy of the country's political and governance system, lack of attention to the intellectual and political maturity of the target group, political factors, political conflicts, the welfare state of the target group, the lack of appropriate executive instructions, the lack of alignment of programs, and the lack of optimal allocation of the country's budget. Discussion: It was found that the influential factors are in the hands of the government and the role of the executives in the implementation of social welfare policies is weaker. Therefore, it is suggested that the government should pay more attention to the use of pro and opposition parties, the legitimacy of the ruling political system, and executive guidelines.

Background: Increased NK cells or their cytokine activity in endometrium and peripheral blood of mothers with pre-eclampsia have been reported. NKG2D is a cell surface receptor that can be used to detect NK cell and lymphocytic activity. Plant flavonoids such as silymarin has immunomodulatory effects and potentially inhibit the disease progression. Therefore, an in vitro experiment was designed to investigate the immunomodulatory effects of silymarin in pre-eclampsia.Methods: Blood samples from preeclamptic and pregnant and non-pregnant women were analyzed by Isolation of peripheral blood mononuclear cells (PBMCs) using Ficoll-Paque. Flow cytometry was used for marker assay including NKG2D expression on CD56+CD3- and CD3+ cells. Also, different concentrations of silymarin were applied, andNKG2D expression on the cells were considered, and IFNγ production in the cell culture medium were evaluated using ELISA.Findings: CD3-CD56+ cells and NKG2D receptor expression were augmented in PBMCs in the pre-eclampsia group (P = 0.03). However, treatment of PBMCs with silymarin solutions decrease NKG2D markers on the lymphocytes. PBMCs treated cells by silymarin in culture medium indicates reduce IFNɣ production even in the presence of IL-2 stimulation by 2-3 folds (P = 0.0001).Conclusion: Increase in NK cell population, NKG2D receptor expression and IFNɣ production play a crucial role in the samples of preeclamptic patients, and well express involvement of immunological factors of the disease. The effectiveness of silymarin in cell culture medium and modulation of inflammatory factors necessitate the design of clinical model for using silymarin in patients by risk of increased NK activity in pre-eclampsia patients.

This paper reports on the status of a comprehensive ten-year research and development effort toward hybrid system-in-foil (HySiF). In HySiF, the merits of high-performance integrated circuits on ultra-thin chips and of large-area and discrete electronic component implementation are combined in a complementary fashion in and on a flexible carrier substrate. HySiF paves the way to entirely new applications of electronic products where form factor, form adaptivity and form flexibility are key enablers. In this review paper the various aspects of thin-chip fabrication and embedding, device and circuit design under impact of unknown or variable mechanical stress, and the on- and off-chip implementation of sensor, actuator, microwave, and energy supply components are addressed.

Abstract Teaching language has challenges for many reasons, including learners’ backgrounds, learning styles, motivation, and socio-economic status. However, EFL students can benefit from differentiation across the learning continuum to a large extent. This study investigated Afghan EFL learners’ perceptions of differentiated instruction and the factors that contributed to catering to their needs. This study revolved around five main elements of differentiated instruction: learning environment, content, process, product, and assessment. In this study, a questionnaire was developed and distributed to 102 EFL students at Herat University, Afghanistan, to record their perceptions of differentiated instruction in their classes. Three teachers of the English department at the same university were also interviewed to understand their experiences implementing differentiated instruction and identify their tensions. Data from the survey and interviews highlight that while DI requires significant teacher commitment and time, properly designed classes catering to learner specifics can enhance efficiency and be reused in future heterogeneous settings. The study’s results suggested that it is practical to implement differentiated instruction in mixed-level, large, heterogeneous classrooms.

Abstract Trained immunity is a phenomenon in which brief exposure to an infectious agent or a vaccine can induce long-lasting changes in the host's immune system, enhancing protection against subsequent infections. The concept of trained immunity has a significant impact on the field of immunology and has the potential to revolutionize how we approach vaccination and infectious disease control. Investigations into trained immunity are rapidly advancing and have led to the development of new vaccines and immunotherapeutic strategies that harness the power of this phenomenon. While more investigations are needed to fully understand the mechanisms of trained immunity and its potential limitations, the prospects for its future application in clinical practice are promising. Here, we describe trained immunity as a biological process and explore the innate cues, epigenetic changes, and metabolic reprogramming activities that affect how trained immunity is induced.

Despite a burgeoning body of research on teacher identity development, little is known about how incongruities between the ideal and real-life teaching practices create identity tensions. The present study adopted an arts-based approach and examined teachers' ideal and actual teaching practices, and the associated identity tensions and related coping strategies in the context of Afghanistan. Data were collected from six English teachers via drawings and semi-structured interviews. Data analysis indicated that the teachers experienced unattainable ideal praxis in terms of 'time, space and facility', 'ideal teaching methods', 'ideal instructional materials', and 'technology-enhanced classroom', which resulted in various identity tensions. However, thinking reflectively about their imagined community and ideal practices offered affordances for the teachers to be creative to manage the tensions, and build resilience and reflexivity. The study concludes with implications for teachers, teacher educators, and policymakers in developing awareness of ideal-real mismatches to support teacher identity development.

Antimicrobial resistance is a global threat. On the other hand, Bangladesh produces high-quality mangoes, yet the mango coat and seed kernel, which contain medicinal components, remain unused. Therefore, this study investigated the antibacterial potential and toxicity of ethanol extracts from mango (Mangifera indica) seed kernels, which specifically target the bacterial strains Staphylococcus aureus, Bacillus cereus, Escherichia coli, and Klebsiella sp. Amrapali variant mango seeds were collected, dried, ground into a fine powder, and extracted with ethanol at various ratios. The efficacy of the crude extract was tested via the disc diffusion method. The results demonstrated significant antibacterial activity against gram-positive bacteria (S. aureus and B. cereus), with clear zones of inhibition observed, especially in a dose-dependent manner. The pure crude extract inhibited the growth of S. aureus with a zone of 23 mm, identical to that produced by doxycycline. However, the extract exhibited limited activity against gram-negative bacteria (E. coli and Klebsiella sp.). Additionally, the extract was effective against multidrug-resistant S. aureus. The pure crude extract produced a 22.5 mm zone of inhibition against multidrug-resistant S. aureus, which was slightly smaller than that of gentamicin (23 mm) but larger than those of chloramphenicol (21 mm), vancomycin (20 mm), and tetracycline (16 mm). In vivo toxicity was assessed in mice, revealing no significant adverse effects on the hepatic structure or renal cortex at lower doses (100 μl of pure crude extract). However, higher doses caused mild histopathological changes in the liver and kidneys. These findings suggest that mango seed kernel extract holds promise as an alternative antibacterial agent, particularly against gram-positive and antibiotic-resistant bacteria, while being relatively safe at lower doses. Further research is needed to elucidate the active compounds, mechanisms of action, and broader applications of this extract in combating antibiotic-resistant bacterial infections.

Objective: The experiment aimed to determine the effects of lipopolysaccharide (LPS), polymyxin B, and honey on survival rates, hematological parameters, liver and kidney biomarkers, blood glucose levels, serum insulin levels, and histopathology of the liver, kidney, lungs, brain, and pancreas in LPS-challenged mice. Materials and Methods: 50 male Swiss Albino mice (Mus musculus), aged 3 weeks, were randomly assigned into 5 groups (10 mice per group): Control group (A), LPS (2 mg/kg bwt/day IP in NS) treated group (B), polymyxin B (1.2 mg/kg bwt/day IM) pre-treated plus LPS (2 mg/kg bwt/day IP in NS) treated group (C), honey (10 gm/kg bwt/day PO) pre-treated plus LPS (2 mg/kg bwt/day IP in NS) treated group (D), both polymyxin B (1.2 mg/kg bwt/day IM) and honey (10 gm/kg bwt/day PO) pre-treated plus LPS (2 mg/kg bwt/day IP in NS) treated group (E). The LPS was administered intraperitoneally (IP) at 80 μg/mice/day, diluting in normal saline. After 16 weeks, the mice were sacrificed, and blood samples and organs (liver, kidney, lung, brain, and pancreas) were collected for laboratory tests. Results: The results revealed that in LPS-treated mice, the mortality rate was the highest, and hemato-biochemical parameters were altered. Histopathological examination in the group treated with LPS showed disarrangement of hepatocytes, cellular infiltrations in the glomerulus, alveolar congestion in the lungs, several nerve fiber degenerations in the brain, and degenerative changes in pancreatic islets. The mortality rate and hemato-biochemical and histopathological changes were restored by the combined treatment of polymyxin B and honey. Conclusion: LPS has detrimental effects on survival rate and hemato-biochemistry, which are lessened by taking honey and polymyxin B supplements. [J Adv Vet Anim Res 2024; 11(2.000): 503-515]

Cattle diarrhea poses a significant risk to the global livestock sector, leading to considerable economic setbacks and impacting the well-being of animals. This research aimed to assess the clinical and hematological changes associated with idiopathic diarrhea in cattle in the Baghabari area of the Sirajganj District of Bangladesh. The study involved surveying dairy cattle to record their case history, followed by the collection and measurement of hematological indices in those suffering from idiopathic diarrhea. The survey identified 39 diarrheic crossbred cattle (2 males and 37 females), ranging in age from 1 month to 11 years. The findings revealed a higher prevalence of diarrheal cases in cows (94.87%) compared to oxen (5.13%), particularly among pregnant cows (64.86%). The most common historical observations included repeat breeding (81.08%), abortion (78.38%), decreased milk production (43.24%), fever (15.64%), and congenital defects (17.95%) in diarrheic cattle. Hematological analyses indicated a significant increase in lymphocyte and immunoglobulin levels, while eosinophil, basophil, and red blood cell distribution width levels were notably lower in diarrheic cattle compared to healthy ones. However, other hematological parameters remained unaffected. In summary, the study suggests that bovine diarrhea may influence hematological profiles and body physiology, with the extent depending on the disease's etiology and complexity. Background and Aim: The prevalence of depressive symptoms and major depressive disorder is high among adults living with HIV. Depressive symptoms are associated with increased cardiovascular disease risk. This study examined the association between depressive symptoms and echocardiographic indices of left ventricular diastolic dysfunction (LVDD) among men and women living with and without HIV. Methods: Cross-sectional analysis included individuals in the Multicenter AIDS Cohort Study (MACS) and Women’s Interagency HIV Study (WIHS) who participated in transthoracic echocardiogram substudies and completed measures of depressive symptoms at the same visit as, or up to 6 months prior to, the transthoracic echocardiogram visit. Participants had helper T cells (CD4) >350 cells/mm3 and HIV RNA viral load less than 499 copies/mL. The presence of LVDD was defined according to the Characterizing Heart Function on Antiretroviral Therapy (CHART) criteria. Secondary outcomes were continuous values of each component of the CHART criteria: left ventricular ejection fraction >50%, septal e’ velocity, lateral e’ velocity, left atrial volume index, left ventricular mass index, and relative wall thickness. Logistic and linear regression were used to adjust for sociodemographic, behavioural, cardiometabolic, and HIV-related factors. Results: Among 874 men (51% with HIV) and 1,191 women (76% with HIV), in whom the overall prevalence of LVDD was 22.5% and depressive symptoms 30.8%, depressive symptoms were not significantly associated with LVDD. The associations between individual LVDD components and depression were in the small to medium range, though generally not significant. Conclusion: Findings warrant further research regarding the association between LVDD and depressive symptoms in the era of combination antiretroviral therapy.

In particular, AI algorithms have demonstrated remarkable potential in analyzing patient radiology images and histopathological samples, offering insights that can enhance clinical decision-making [1]. This letter explores the emerging role of AI in the diagnosis and treatment of odontogenic tumors (OTs), a group of benign, malignant, and tumor-like malformations arising from the remnants of the tooth-forming apparatus.

Abstract Introduction Lumpy skin disease, an economically significant bovine illness, is now found in previously unheard‐of geographic regions. Vaccination is one of the most important ways to stop its further spread. Aim Therefore, in this study, we applied advanced immunoinformatics approaches to design and develop an effective lumpy skin disease virus (LSDV) vaccine. Methods The membrane glycoprotein was selected for prediction of the different B‐ and T‐cell epitopes by using the immune epitope database. The selected B‐ and T‐cell epitopes were combined with the appropriate linkers and adjuvant resulted in a vaccine chimera construct. Bioinformatics tools were used to predict, refine and validate the 2D, 3D structures and for molecular docking with toll‐like receptor 4 using different servers. The constructed vaccine candidate was further processed on the basis of antigenicity, allergenicity, solubility, different physiochemical properties and molecular docking scores. Results The in silico immune simulation induced significant response for immune cells. In silico cloning and codon optimization were performed to express the vaccine candidate in Escherichia coli. This study highlights a good signal for the design of a peptide‐based LSDV vaccine. Conclusion Thus, the present findings may indicate that the engineered multi‐epitope vaccine is structurally stable and can induce a strong immune response, which should help in developing an effective vaccine towards controlling LSDV infection.

Few patients with triple negative breast cancer (TNBC) benefit from immune checkpoint inhibitors with complete and durable remissions being quite rare. Oncogenes can regulate tumor immune infiltration, however whether oncogenes dictate diminished response to immunotherapy and whether these effects are reversible remains poorly understood. Here, we report that TNBCs with elevated MYC expression are resistant to immune checkpoint inhibitor therapy. Using mouse models and patient data, we show that MYC signaling is associated with low tumor cell PD-L1, low overall immune cell infiltration, and low tumor cell MHC-I expression. Restoring interferon signaling in the tumor increases MHC-I expression. By combining a TLR9 agonist and an agonistic antibody against OX40 with anti-PD-L1, mice experience tumor regression and are protected from new TNBC tumor outgrowth. Our findings demonstrate that MYC-dependent immune evasion is reversible and druggable, and when strategically targeted, may improve outcomes for patients treated with immune checkpoint inhibitors.

Adenoviruses are non-enveloped DNA viruses that cause a wide range of symptoms, from mild infections to life-threatening diseases in a broad range of hosts. Due to the unique characteristics of these viruses, they have also become a vehicle for gene-transfer and cancer therapeutic instruments. Adenovirus vectors can be used in gene therapy by modifying wild-type viruses to render them replication-defective. This makes it possible to swap out particular viral genes for segments that carry therapeutic genes and to employ the resultant vector as a means of delivering genes to specified tissues. In this review, we outline the progressive development of adenovirus vectors, exploring their characteristics, genetic modifications, and range of uses in clinical and preclinical settings. A significant emphasis is placed on their crucial role in advancing gene therapy, cancer therapy, immunotherapy, and the latest breakthroughs in vaccine development for various diseases.

Hepatitis B virus (HBV), resistant to several antiviral drugs due to viral genomic mutations, has been reported, which aggravates chronic infection and leads to hepatocellular carcinoma. Therefore, host cellular factors/signaling modulation might be an alternative way of treatment for drug-resistant HBV. Here, we investigated the viral protein expression, replication, and virion production using endoplasmic reticulum (ER) stress-modulating chemicals, tunicamycin (an ER-stress inducer), and salubrinal (an ER-stress inhibitor). We found that ER-stress could be induced by HBV replication in transfected HepG2 cells as well as by tunicamycin as demonstrated by dual luciferase assay. HBV intracellular core-associated DNA quantified by qPCR has been significantly increased by tunicamycin in transfected HepG2 cells. Inversely, intracellular core associated and extracellular particle DNA has been significantly decreased in a dose-dependent manner in salubrinal-treated HepG2 cells transfected with HBV-replicating plasmid pHBI. Similar results were found in stably HBV-expressing hepatoblastoma (HB611) cells treated with salubrinal. However, increased or decreased ER-stress by tunicamycin or salubrinal treatment, respectively, has been confirmed by expression analysis of grp78 using Western blot. In addition, Western blot results demonstrated that the expression of HBV core protein and large HBsAg is increased and decreased by tunicamycin and salubrinal, respectively. In conclusion, the sal-mediated inhibition of the HBV replication and virion production might be due to the simultaneous reduction of core and large HBsAg expression and maintaining the ER homeostasis. These results of HBV replication regulation by modulation of ER-stress dynamics would be useful for designing/identifying anti-HBV drugs targeting cellular signaling pathways.

This review delves into the historical context, current epidemiological landscape, genomics, and pathobiology of monkeypox virus (MPXV). Furthermore, it elucidates the present vaccination status and strategies to curb the spread of monkeypox. Monkeypox, caused by the Orthopoxvirus known as MPXV, is a zoonotic ailment. MPXV can be transmitted from person to person through respiratory droplets during prolonged face-to-face interactions. While many cases of monkeypox are self-limiting, vulnerable groups such as young children, pregnant women, and immunocompromised individuals may experience severe manifestations. Diagnosis predominantly relies on clinical presentations, complemented by laboratory techniques like RT-PCR. Although treatment is often not required, severe cases necessitate antiviral medications like tecovirimat, cidofovir, and brincidofovir. Vaccination, particularly using the smallpox vaccine, has proven instrumental in outbreak control, exhibiting an efficacy of at least 85% against mpox as evidenced by data from Africa. Mitigating transmission requires measures like wearing surgical masks, adequately covering skin lesions, and avoiding handling wild animals.

This study examined students' attitudes toward distance learning, and its relationship with the duration of using Telegram and schooling. It specifically explored students' experiences of the challenges and opportunities that distance learning created during the COVID-19 pandemic. Accordingly, two null hypotheses were formulated: (1) there is no significant relationship between students' attitudes toward distance learning and the duration of using Telegram; and (2) there is no significant relationship between students' attitudes towards distance learning and the duration of schooling. Data were collected from a survey questionnaire and in-depth semi-structured interviews with students from the English Department of Herat University, Afghanistan. The quantitative data were analyzed using SPSS, an independent samples t-test, and ANOVA. The results of the t-test showed that the first hypothesis should be accepted, meaning there is no relationship between students' attitudes toward e-learning and years of Telegram use. Further, the one-way ANOVA test showed that the second null hypothesis was affirmed. Moreover, the qualitative findings indicated that distance learning via Telegram is associated with context-specific challenges and several opportunities.

Emerging data sources are gaining popularity because of their accessibility, pervasiveness, and enormous potential. Blogs, images, Twitter, Foursquare (location sharing), and Flickr (photography) are significant sources of information regarding human activities. There are several elements that influence decision making behavior at the individual, group, and organizational levels, including information technology and decision support systems. Recent advancements in user interfaces for social tools, in conjunction with an increase in the usage of mobile wireless computers, have resulted in the creation of social networks that are instantaneous, widely distributed, mostly unmanaged, and pervasive. The proliferation of information and communication technologies continues to open new doors. In the age of big data and new data sources, the challenge for modern businesses is to align their decision making and organizational processes with data that could help them make more informed decisions. The study also proposed a culturally responsive framework that entails emotioncy and cultuling analysis to support institutions in the process of decision making when using emerging data sources.

This study investigates the perceptions of teachers and students toward formative assessment (FA) in higher education settings. The researchers developed a four-construct perception scale, namely self-assessment, interactive formal assessment, in-class diagnostic assessment, and subjective assessment. Data were collected from 216 participants--91 teachers and 125 students. The findings demonstrated that both teachers and students have identically perceived interactive and in-class diagnostic assessments. Nonetheless, they distinctively perceived self-assessment and subject performance assessment demonstrating a significant difference. The students reported self-assessment greater than the teachers, whereas they perceived the subject-performance assessment lower compared to the teachers. The findings suggest that English as a foreign language (EFL) or English as a second language (ESL) learners benefit from formative assessment if teachers evaluate students' progress based on their own development rather than being evaluated in comparison to other students' development.

Universal Design for Learning (UDL) is considered to provide equitable opportunities for all learners with or without disabilities. This study aimed to examine the extent to which UDL was implemented in ESL classrooms at the university level. A mixed-method approach was conducted to explore the effective ways to maximise learning outcomes, inclusion, and equity regarding UDL principles; both students' and teachers' voices were addressed. Online questionnaires for students and semi-structured interviews for teachers were used to probe the UDL application in the classrooms. The findings revealed that teachers employed UDL principles in their classrooms to varying degrees. They also explored various strategies to enhance levels of engagement, representation, action, and expression. However, they experienced different tensions during the process. The study concluded with implications for researchers, teachers, and policymakers in developing a more inclusive and equitable learning environment.

Myelination plays an important role in cognitive development and in demyelinating diseases like multiple sclerosis (MS), where failure of remyelination promotes permanent neuro-axonal damage. Modification of cell surface receptors with branched N-glycans coordinates cell growth and differentiation by controlling glycoprotein clustering, signaling, and endocytosis. GlcNAc is a rate-limiting metabolite for N-glycan branching. Here we report that GlcNAc and N-glycan branching trigger oligodendrogenesis from precursor cells by inhibiting platelet-derived growth factor receptor-α cell endocytosis. Supplying oral GlcNAc to lactating mice drives primary myelination in newborn pups via secretion in breast milk, whereas genetically blocking N-glycan branching markedly inhibits primary myelination. In adult mice with toxin (cuprizone)-induced demyelination, oral GlcNAc prevents neuro-axonal damage by driving myelin repair. In MS patients, endogenous serum GlcNAc levels inversely correlated with imaging measures of demyelination and microstructural damage. Our data identify N-glycan branching and GlcNAc as critical regulators of primary myelination and myelin repair and suggest that oral GlcNAc may be neuroprotective in demyelinating diseases like MS.

ObjectivesElite controllers (ECs), viraemic controllers (VCs), and long-term nonprogressors (LTNPs) control HIV viral replication or maintain CD4 T-cell counts without antiretroviral therapy, but may have increased cardiovascular disease (CVD) risk compared to HIV-uninfected persons. We evaluated subclinical carotid and coronary atherosclerosis and inflammatory biomarker levels among HIV controllers, LTNPs and noncontrollers and HIV-uninfected individuals in the Multicenter AIDS Cohort Study (MACS) and the Women's Interagency HIV Study (WIHS).MethodsWe measured carotid plaque presence and common carotid artery intima-media thickness (IMT) in 1729 women and 1308 men, and the presence of coronary artery calcium and plaque in a subgroup of men. Associations between HIV control category and carotid and coronary plaque prevalences were assessed by multivariable regression analyses adjusting for demographics and CVD risk factors. Serum inflammatory biomarker concentrations [soluble CD163 (sCD163), soluble CD14 (sCD14), galectin-3 (Gal-3), galectin-3 binding protein (Gal-3BP) and interleukin (IL)-6] were measured and associations with HIV control category assessed.ResultsWe included 135 HIV controllers (30 ECs) and 135 LTNPs in the study. Carotid plaque prevalence and carotid IMT were similar in HIV controllers, LTNPs and HIV-uninfected individuals. HIV controllers and LTNPs had lower prevalences of carotid plaque compared to viraemic HIV-infected individuals. The prevalence of coronary atherosclerosis was similar in HIV controllers/LTNPs compared to HIV-uninfected and viraemic HIV-infected men. Controllers and LTNPs had higher concentrations of sCD163 and sCD14 compared to HIV-uninfected persons.ConclusionsSubclinical CVD was similar in HIV controllers, LTNPs and HIV-uninfected individuals despite elevated levels of some inflammatory biomarkers. Future studies of HIV controllers and LTNPs are needed to characterize the risk of CVD among HIV-infected persons.
