NEC is associated with prematurity, a hyperinflammatory response, and dysregulation of intestinal barrier function. We hypothesize that patients with NEC will have, and continue to have after recovery, an increased hyperinflammatory intestinal response compared to those patients without NEC.Methods: Neonates with NEC, those that have recovered from NEC, and those without NEC undergoing intestinal resections had specimens collected and snap frozen or generated into enteroids. The enteroids were treated with 100ug/mL lipopolysaccharide (LPS) and subjected to 24 hr of hypoxia together, then compared with untreated controls. Expression of Tumor Necrosis Factor (TNF-α) and interleukin 8 (IL-8) were evaluated via RT-qPCR and ELISA to measure inflammatory response. ANOVA determined statistical significance (p< 0.05).Results: There was no difference in inflammatory markers in recovered NEC tissue compared to non-NEC tissue on RTqPCR (p=0.701 TNF-α and 0.861 IL-8). However, recovered NEC enteroids demonstrate elevated levels of inflammatory markers after treatment compared to non-NEC enteroids after treatment on RTqPCR (p=0.0485 TNF-α, p=0.0057 IL-8) and ELISA (p=0.0354 TNF-α, p=0.0011 IL-8). Recovered NEC enteroids that underwent treatment demonstrated increased inflammatory markers compared to recovered NEC enteroids without treatment on RTqPCR (p=0.0045 TNF-α, p=0.0002 IL-8) and ELISA (p=0.034 TNF-α, p=0.0002 IL-8) suggesting a heightened inflammatory response to a second hit.Conclusion: Intestinal tissue resected from neonates with NEC has an elevated hyperinflammatory response compared to neonates recovered from NEC and neonates without NEC. Enteroids generated from patients that have recovered from NEC have a heightened inflammatory response in response to NEC inducing stimuli compared to controls. This tendency towards an increased hyperinflammatory state may be correlated with an infant’s proclivity to develop NEC and demonstrates the significance of a second hit on this tissue creating a heightened inflammatory response. This could be correlated with the impact and trajectory of an illness post recovery from NEC.Keywords: necrotizing enterocolitis, recovery, hyperinflammatory, enteroids

Objective. To explore potential of anti-Pseudomonas vaccine for the prophylaxis and treatment of infections caused by Pseudomonas aeruginosa in patients with severe burn trauma. Materials and Methods. This was a pilot, singlecenter randomized, parallel group study which enrolled patients with burns involving 10 to 60% of the body surface area. A total of 24 patients have received a course of «Pseudovac» vaccine (vaccination group), and 24 patients were included in the control arm (non-vaccination group). Treatment arms were compared by mortality rate and incidence of nosocomial infections caused by P. aeruginosa and other microorganisms. Assessment of antimicrobial treatment duration and antibiotic consumption using ATC/DDD methodology was performed. Results. Incidence rate of nosocomial infections was 70.8% and 83.3% in vaccinated and non-vaccinated patients, respectively (p=0.4936). A total of 9 (37.5%) vaccinated patients and 15 (62.5%) non-vaccinated patients were found to be infected and colonized by P. aeruginosa strains (p=0.1489). The average day of Pseudomonas infection onset was 17 (14.3–28.3) and 12 (6.9–21.1) in vaccinated and non-vaccinated patients, respectively (p=0.0726). The difference in mortality rate was not significant between the groups (12.5% and 25% of patients, respectively; p=0.4613). There was 9.3% decrease in antibiotic consumption (from 667 to 610 NDDD/1000 bed-days) in vaccinated patients. There were no significant changes in a total number of patients who received anti-pseudomonas antibiotics. The overall consumption of imipenem, meropenem, amikacin, cefepime, and piperacillin/tazobactam was reduced by 84.5% (from 190 to 103 NDDD/1000 bed-days). Conclusions. The «Pseudovac» vaccine used for the prophylaxis and treatment of infections caused by P. aeruginosa in patients with burns was shown to be safe. The use of anti-pseudomonas vaccine may contribute to reducing consumption of anti-pseudomonas agents.

Relevance. The new coronavirus infection (COVID-19), which appeared in late 2019 in China, has spread to almost all countries of the world in just a few months. The explosive nature of its spread was accompanied by the formation of large epidemic foci in organizations of various profiles, including leisure and sports. Aims. To establish the conditions and causes of the spread of SARS-CoV-2 among the members of one of the sports clubs based on an in-depth epidemiological analysis. Materials and methods. To study the features of the spread of the SARS-CoV-2 virus in a sports organization, the following documents were used previously developed by the authors and successfully tested in practice: «Act of epidemiological investigation of group and outbreak morbidity of new coronavirus infection (COVID-19) at an enterprise/organization/institution» and «Individual card of a patient with a new coronavirus infection (COVID-19) at the enterprise / organization/institution». In the process of epidemiological investigation, in order to detect SARS-CoV-2 RNA in PCR, a laboratory examination of sports club participants (sick and contact) was conducted, followed by genome-wide sequencing of isolated SARS-CoV-2 viruses on the basis of the Laboratory of Molecular Virology of the A. A. Smorodintsev Influenza Research Institute, which performs these types of studies. Results. Within 17 days, 26 cases of COVID- 19 were registered among the sports team members and staff from the support group (coaching staff, medical staff, administrators), which was 74.3% of their actual number. The majority of patients (76.9%) had mild acute respiratory infection, two (7.7%) had no symptoms, and four (15.4%) had interstitial pneumonia. Of the clinical manifestations of the disease, the most frequent were weakness, fever, headache, muscle and joint pain, difficulty in nasal breathing and serous-mucous discharge from the nose, sore throat, cough, shortness of breath, anosmia and dyspeptic manifestations in the form of diarrhea, nausea or vomiting. The occurrence of the outbreak was the result of the introduction of infection from the opposing team at the tournament. The leading factors that contributed to the spread of COVID-19 among sports club members were the admission to games and training of athletes with acute respiratory infections, prolonged close contact between players during training and competitions, violations in the use of personal protective equipment, compliance with hygiene and hand antiseptics, disinfection measures in the premises of sports institutions and defects in the implementation of the regulations for the examination of teams for SARS-CoV-2 during tournaments. Conclusion. Based on the results of the study, data were obtained on the features of the spread of SARS-CoV-2 in sports organizations, which can be used in conducting preventive and anti-epidemic measures in sports and leisure institutions.

Introduction. In the context of a pandemic of a new coronavirus infection (COVID-19), research on the peculiarities of the formation of an immune response to SARS-CoV-2 in patients who have been ill and vaccinated is of particular relevance. However, most studies are currently devoted to evaluating only the humoral link of immunity, and its cellular component remains insufficiently studied.The aim of the study was to evaluate the features of the formation and changes of the T-cell link of immunity in patients with a new coronavirus infection and vaccinated against this disease.Materials and methods. The study was performed on the basis of the clinical and diagnostic laboratory of the European Medical Center “UMMC-Health “LLC. Specific T-cell immunity was evaluated using ELISPOT technology. In the course of the study, 72 blood samples of employees of medical organizations were analyzed, including 26 from those who had a new coronavirus infection, 23 from persons who were intact according to COVID-19 before vaccination and 23 from the same employees after vaccination («Gam-CovidVac»).In addition, each of the study participants was examined to determine specific class G antibodies (IgG) by solid-phase enzyme immunoassay using SARS-CoV-2-IgG-ELISA-BEST test systems (manufactured by VECTOR-BEST JSC).Results and discussion. In the group of patients (26 people), T-lymphocytes capable of specifically reacting to SARSCoV-2 antigens were detected in 100% of cases, even in individuals with IgG elimination. It should be noted that the response was more pronounced when meeting with M-and N-pepdids, compared with S-protein.22 out of 23 COVID-19 intact individuals had no T-cell immunity to coronavirus infection before vaccination, but one employee had a response to 3 proteins-M, N, S, which indicates that he had previously encountered the SARS-CoV-2 virus. After vaccination with the drug “Gam-Covid-Vac”, 22 (95.6%) employees revealed a T-cell response, while 21-only to S-protein, and an employee with a previously detected immune response-after vaccination, the response to M -, Nproteins remained almost at the same level, and the cellular response to S-peptide doubled.Conclusion. Thus, based on the results of the study, important materials were obtained on the peculiarities of the formation of a specific T-cell immune response to a new coronavirus infection. The obtained data provide a broader understanding of the immune response in new coronavirus infection in patients who have been ill and vaccinated and can be used in the future when planning preventive and antiepidemic measures.

Abstract Necrotizing enterocolitis (NEC) is a multifactorial and complex disease. Our knowledge of the cellular and genetic basis of NEC have expanded considerably as new molecular mechanisms have been identified. This article will focus on the current understanding of the molecular pathogenesis of NEC with a focus on the inflammatory, immune, infectious, and genetic mechanisms that drive disease development.

Relevance. Active mass vaccination of the population against a new COVID-19 is being carried out on the territory of the Russian Federation, which is recognized as a priority strategy for the country's healthcare for the near and long-term periods. One of the main risk groups that are subject to priority vaccination is medical workers. Aims. To evaluate the effectiveness of vaccination of medical organizations ' employees against COVID-19 based on the results of a 6-month prospective follow-up. Materials and methods. The observation group consisted of 356 employees of a medical organization who were vaccinated against COVID-19 with the drug «Gam-Covid-Vac» from December 2020 to April 2021. The effectiveness of vaccination of employees was evaluated by the coefficient of IgG positivity to SARS-CoV-2 by solid-phase enzyme immunoassay 3 weeks after the first administration and 3–4 weeks after the second administration of the vaccine and then 1 time per month. Employees who were revaccinated after 5–6 months. After the initial vaccination, they were examined 10–14 days after the introduction of the first component «Gam-Covid-Vac». A total of 1921 serum samples were studied. A specific T-cell immune response was determined in two study participants without seroprotection after administration of two components of the «Gam-Covid-Vac» vaccine and eight employees with the elimination of IgG antibodies 4–5 months after vaccination using ELISPOT technology. In addition, 92 blood serum samples of 32 employees from the observation group were examined for specific antibodies to adenovirus by indirect enzyme immunoassay. From December 2020 to June 2021, the study participants were subjected to dynamic clinical observation, once a week they were examined by PCR to detect SARS-CoV-2 RNA in smears from the pharynx and nose (a total of 5696 samples). Results. After the completed course of immunization, the formation of both a humoral (in 99.4% of cases) and cellular immune response (100% among the studied samples) was confirmed. In the next 6 months after vaccination, cases of coronavirus infection were registered in 4.8% of those vaccinated, including 1 person – in the first month after vaccination and 16 – 3–5 months after vaccination. In all cases, the disease occurred in the form of an acute respiratory infection of mild or moderate severity and was characterized by a shorter period of virus isolation compared to similar data on the persistence of the virus in unvaccinated patients (15 days in vaccinated compared to 22 days in unvaccinated). It was found that the presence of immunity to adenovirus infection during vaccination with the drug «Gam-Covid-Vac» did not affect the possibility of forming an immune response to COVID-19. In the group of persons re-vaccinated with the first component of «Gam-Covid-Vac» after 5–6 months. after the initial vaccination, an immune response was received during the follow-up period. Conclusion. Thus, according to the results of the study, a high immunological and epidemiological effectiveness of vaccination against COVID-19 with the drug «Gam-Covid-Vac» was established in a group of medical workers, and the effectiveness and safety of the administration of a booster dose of the vaccine after primary vaccination was also shown.

Relevance. Despite significant progress in the prevention of many infectious diseases, a number of organizational issues remain in the area of special attention. One of them is to ensure the timeliness and completeness of vaccination coverage in the decreed age groups, which is possible only with a high level of public commitment to vaccination. During the pandemic of the new coronavirus infection (COVID-19), achieving a high level of vaccination coverage of the National Calendar of Preventive vaccinations is of particular relevance. Aim. Using state-of-the-art online resources, assess population vaccination adherence to develop effective communication risk management technologies for immunoprophylaxis in the COVID-19 pandemic. Materials and methods. To assess the commitment of various population groups and professional contingents of a large industrial city to vaccine prevention, a survey of 965 parents was conducted at the initial stage, and subsequently a survey of 2,632 employees of medical organizations, including 1,422 people in the period before the COVID - 19 pandemic, 572 people in the «first wave» of the pandemic and 638 people in the «second wave». Instagram Facebook, Instagram, and VKontakte social networks were used to quickly obtain the necessary information (Google forms, corporate e-mails, WhatsApp, Telegram messengers, etc.). The work uses epidemiological, sociological and statistical methods of research. Results and discussion. When assessing the attitude of parents to vaccination, it was found that 78.8% of respondents were positive about vaccinations, 14.6% doubted their effectiveness and safety, and 6.6% denied their need. The main source of information that forms the attitude to vaccinations of parents was the opinion of medical professionals. In a survey of 1,422 employees, 190 people (13.3%) were identified, mainly from among the trained specialists, who themselves had a negative attitude to vaccination. When assessing the commitment of employees of medical organizations to the vaccination of a new coronavirus infection, the attitude to vaccination was ambiguous, especially in the group of secondary medical personnel and female employees of non-medical specialties, who are a potential risk group for forming a negative attitude to vaccinations and can act as «opinion leaders» on this issue for other categories of the population. Insufficient adherence to COVID-19 vaccine prevention has also been observed in other countries among various population groups, which is a serious problem on a global scale, hindering the fight against the pandemic of a new disease, which requires corrective measures. Conclusion. In this study, using modern online resources, a rapid assessment of the vaccination adherence of various population groups (parents and medical professionals) was carried out. It is shown that the use of modern technologies and software tools makes it possible to analyze a large array of data to solve problems of overcoming communication risks. The results obtained point to the need for prompt corrective measures in terms of the introduction of educational programs to increase adherence to vaccination, especially in groups at potential risk of the disease, including medical professionals, which is a priority for health care in all countries.

Relevance. Prevention of the outbreak of a new coronavirus infection in medical organizations (MO) is one of the tasks facing the Rospotrebnadzor service and practical health care. Analysis of the causes and conditions of the spread of SARS-CoV-2 in the MO allows us to determine the most effective measures to contain the infection and prevent the occurrence of subsequent diseases in the focus.The aim is to characterize the epidemiological features of a new coronavirus infection during its nosocomial spread in medical organizations based on the analysis of outbreak incidence.Materials and methods. The data of the reporting forms «General information on patients with positive COVID-19», «General information on patients with community-acquired pneumonia with positive COVID-19» (138 pieces of information), copies of «Acts of epidemiological investigation of foci of infectious (parasitic) diseases with the establishment of a causal relationship» (57 pieces of information), copies of extraordinary reports on an emergency of a sanitary and epidemiological nature (57 pieces of information) were analyzed. The work uses epidemiological and statistical methods of research.Results. When analyzing 57 COVID-19 outbreaks in 30 medical organizations, it was found that the outbreak incidence was more often registered in multidisciplinary hospitals – 70.2% compared to 12.3% in specialized organizations of noninfectious profile, 10.5% of outbreaks were in polyclinics, 1.8% at emergency medical stations. The total number of people affected by the spread of the infection was 961, with a small proportion of children (15 people or 1.7%). Among those who fell ill with the spread of infection in the foci, the share of employees of medical organizations was 62.6%, including doctors – 16.6%, middle and junior medical workers-50.6% and 11.3%, respectively, and other categories – 21.5%. In nosocomial outbreaks of COVID-19, the focality index corresponded to 16.9, and the average duration of the focus was 32.4 days. The construction of the so-called «combined» outbreak diagram in the course of the epidanalysis allowed us to establish its total duration, which was 71 days, with the peaks of morbidity within the outbreak at intervals of 3–7 days, the greatest number of cases in the 1st and 8th days, and the gradual «extinction» of its activity in dynamics.Conclusions. The outbreak of COVID-19 in medical organizations usually occurred when there was an epidemic problem with COVID-19 in the territory. The peculiarity of nosocomial outbreaks was a significant proportion of adults (98.3%), mainly employees of medical organizations, who were often active sources of infection, as well as the involvement of structural units and hospitals for adults in the epidemic process, a high frequency of etiological interpretation of outbreaks (95.9%), a significant proportion of asymptomatic forms of infection and carrier, and the complexity of differential diagnosis of cases of out – of-hospital and nosocomial infection. The patterns of occurrence and spread of morbidity in medical organizations can be used in practical anti-epidemic work.

The article discusses the basic principles of optimizing the status of subsystems of the marine transport industry. Since the positioning of stability management in any segment of transport activities reflects a system of principles, a system of the interdependence of structural elements of merchant shipping was identified. Taking into account the most important provisions for managing the economic stability of shipping and the principles of repayable financing, urgent problems of improving the assessment of business decisions by criteria and indicators reflecting the functioning of shipping companies and trading ports are considered in the article. The structure of the problems and advantages of the formation of an active marine transport strategy is presented. Some factors of the development of specialized segments of the global maritime trade market are disclosed. The regularity and conditions for the formation of the parameters of the marine transport cluster are formulated. The article discusses measures to balance the economic interests of maritime transport and the consumers of its products. On this basis, the economic stimulation of optimal economic results is intensified. The necessity of using a wider range of economic regulation in comparison with the concentration of efforts on one model in optimizing the basic state of the regional complex of maritime transport is considered. The article also takes into account the analysis of factors that contribute to the optimization of the final indicators with the parameters of the maritime trade market. Features of innovative approaches that are of fundamental importance in the transport business and reflect the intellectual property of the company's management or its consultants are highlighted. It is on the life cycle ideas of the fleet that functional and financial stability of positioning can be achieved, despite the fact that new competitors appear at regular intervals. It is recommended to systematize the conditions for achieving the parameters of transport independence in the development of international transport integration, special attention is paid to the choice of mission based on the formation of a merchant shipping system that provides standardized efficiency. This, in turn, should be achieved through the benefits of innovative freight technologies.

Relevance. During the pandemic of the new coronavirus infection (COVID-19), one of the most vulnerable and actively involved in the epidemic process categories of the population were medical workers. The study of the incidence of COVID-19 among medical workers, risk factors for infection and safety measures is an important area of research in modern conditions. The aim of the study is to assess the prevalence of COVID-19 among employees of medical organizations and occupational risk factors for infection in order to develop proposals for corrective measures. Materials and methods. To study the incidence of COVID-19 among employees of medical organizations, an anonymous online questionnaire developed by the authors was used, which was posted on the Google platform and distributed during the first wave of the pandemic among employees of medical organizations in several regions of the Russian Federation through corporate e-mail or specialized online resources adapted for health workers. The survey involved 1,872 medical workers of different genders, ages, positions and work experience. Results and discussion. Among the survey participants, 161 employees have already had a new coronavirus infection. In the majority (64.0%) of them, the disease occurred in the form of acute respiratory infection, in 28.6% - interstitial pneumonia, in 7.4% only the fact of isolation of the SARSCoV-2 antigen without clinical manifestations of infection was documented. The most affected contingents were secondary and junior medical personnel, mainly hospital staff. The likelihood of infection in the workplace was increased by factors such as providing care to patients with confirmed COVID-19, participating in procedures related to aerosol generation, working with biomaterials of patients with COVID-19, contact with surfaces in the environment of a COVID-19 patient, ignorance of infectious safety issues due to lack of instruction before admission to work. When assessing the efficiency of personal protective equipment (PPE) when they are used in the process, it was found that the highest value for the prevention of infection COVID-19 had a PPE for face protection, respiratory and eye, and hand hygiene. At the same time, many employees are faced with the problem of an insufficient number of protective equipment in the workplace. When assessing the availability of SARS-CoV-2 testing to employees, it was found that only 77.4% of survey participants had the opportunity to be tested for coronavirus infection in a timely manner. Conclusion. Thus, for more effective protection of employees of medical organizations, it is necessary to have a sufficient number of personal protective equipment, especially for the face, respiratory organs and vision, the availability of hygienic hand treatment during work, regular instruction on infectious safety, using interactive forms of training, and the organization of screening examinations of previously unvaccinated and unvaccinated employees.

Relevance In the context of the new coronavirus infection (COVID-19) pandemic, research on the formation of an immune response to SARS-CoV-2 in patients with various clinical forms of this infection is of particular relevance, which is important for understanding the degree of their participation in the formation of population immunity and assessing individual immunity to SARS-CoV-2 in the future. Aim of the study was to analyze the timing of the formation of specific antibodies to SARS-CoV-2 and the duration of their preservation in patients with a new coronavirus infection. Materials and methods. The timing of the formation of specific antibodies of three classes (IgA, IgM and IgG) to SARS-CoV-2 in 218 patients was evaluated in the first days of the disease. Subsequently, they were re-examined at various times from the onset of the disease, from one to four times (a total of 321 control points). To assess the duration of preservation of antibodies to COVID-19 in patients who were ill, a prospective study was organized, in which 368 people participated. Screening for specific class G antibodies was performed every 2–4 weeks, within one to eight months of the onset of clinical manifestations of COVID-19. In total 919 control points. The antibodies were examined by solid-phase enzyme immunoassay using the SARS-CoV-2IgM-ELISA-BEST and SARS-CoV-2-IgG-ELISA-BEST test systems (manufactured by VECTOR-BEST JSC) and Antigma A (manufactured by Generium). Results. In most of the examined individuals, class M and G antibodies began to form from day 10 of the disease, while class A antibodies were detected from day 5. In mild and asymptomatic forms of coronavirus infection, IgG antibodies to SARS-CoV-2 were not formed in most cases, and seroprotection levels correlated with the severity of the disease. The duration of preservation of IgG antibodies could be at least 8 months, but there were isolated cases of their elimination both after COVID-19 in the form of respiratory infection and after interstitial pneumonia. With continuing problems with COVID-19, there were no cases of recurrent disease among individuals who formed G-antibodies (including those who lost them 4-5 months after the disease) during the follow-up period. Conclusion. Thus, according to the results of the study, important materials were obtained on the peculiarities of the formation of a humoral immune response to a new coronavirus infection. However, to fully understand the immune response to SARS-CoV-2, it is necessary to assess the avidity of IgG antibodies or their ability to neutralizing the virus, as well as to study cellular immunity in patients who have had COVID-19 but have not formed antibodies.

Relevance. In perinatal centres problems of timely vaccination of newborns arise more often than in maternity hospitals, as vaccinations against viral hepatitis B and tuberculosis in accordance with the National Calendar are carried out in an early neonatal period and in time coincide with the stage of treatment and rehabilitation of the newborn, which implies medical withdrawal from vaccination. Vaccination of newborns from high perinatal risk groups is one of the problems of modern vaccinology and immunology.Aims. On the basis of clinical and immunological characteristics of postnatal babies from mothers with gestational diabetes and fetoplacental insufficiency justify the necessity and safety of their vaccination in the maternity hospital of the perinatal center.Materials & Methods. A cohort retrospective case-control study was conducted, and 135 birth and developmental history and newborn disease were analyzed. Statistical processing was carried out package «Statistica».Results. The main nosological forms of pathology in maternity mothers and their newborn children, the volume of primary vaccination (against viral hepatitis B, tuberculosis) in maternity hospital in children with perinatal pathology and healthy newborns have been determined, and the reasons for medical contraindications to vaccinations against viral hepatitis B and tuberculosis have been established.Conclusions. Newborns from high perinatal risk groups can be vaccinated against viral hepatitis B and tuberculosis in the maternity hospital within the time frame specified by the National Calendar of Preventive Vaccinations. In addressing this issue, the fundamental criteria are to assess the clinical condition of newborns from women with gestational diabetes and fetoplacental insufficiency. Changes in immunological values of umbilical cord blood and in children of 1 month of life are not a criterion for medical withdrawal from vaccinations against viral hepatitis B and tuberculosis.

Complex macroscopic organisms are first found in the Ediacaran period, but their ecology during this time is not well understood. Here, Bobrovskiy et al. analyse biomarkers from Ediacaran sediments hosting macrofossils and find evidence for abundant algal food sources available for these organisms.

Relevance. According to official statistics prenatal infections outweigh neonatal purulent-septic infections. The clinical picture of prenatal infections is not the specifics. Doctors put a newborn diagnosed based on the mother's medical history, additional studies are not carried out. This leads to incorrect treatment of newborns and incorrect assessment of the epidemiological situation in the matenityhospital. We need to study prenatal infections as a marker epidemic in the matenity hospital. Materials and methods. The study used data from official statistics and special form developed by researchers. Follow-up was 26 years (1991 to 2016). Total 26601 analyzed the case of newborn infections, including 6330 – prenatal infections. Results. Increasing the number of diagnoses prennatalnyh infections coincides with increasing neonatal sepsis diseases, respiratory tract infections, skin infections and intestinal infections. Growth for the prenatal infections correlates with lack midwife and their high production load, the increasing number of complications during childbirth. Conclusion. Prenatal neonatal infection is a mask of infections associated with medical care. The medical organization requires constant monitoring of the number of prenatal infections. Growth prenatal infections should be regarded as a marker to the complications of the epidemiological situation in the hospital.
