Abstract

International quality concerns regarding long-term residential care, home to many of the most vulnerable among us, prompted our examination of the audit and inspection processes in six different countries. Drawing on Donabedian’s (Evaluation & Health Professions, 6(3), 363–375, 1983) categorization of quality criteria into structural, process and outcome indicators, this paper compares how quality is understood and regulated in six countries occupying different categories according to Esping Andersen’s (1990) typology: Canada, England, and the United States (liberal welfare regimes); Germany (conservative welfare regime); Norway, and Sweden (social democratic welfare regimes). In general, our review finds that countries with higher rates of privatization (mostly the liberal welfare regimes) have more standardized, complex and deterrence-based regulatory approaches. We identify that even countries with the lowest rates of for profit ownership and more compliance-based regulatory approaches (Norway and Sweden) are witnessing an increased involvement of for-profit agencies in managing care in this sector. Our analysis suggests there is widespread concern about the incursion of market forces and logic into this sector, and about the persistent failure to regulate structural quality indicators, which in turn have important implications for process and outcome quality indicators.

Abstract

"Im Auftrag des Bundesministeriums für Bildung und Forschung (BMBF) führte die Sozialforschungsstelle Dortmund von November 2000 bis Mai 2001 das Projekt 'Gender Mainstreaming und Demographischer Wandel' durch. Ziel des Projektes war die Erhebung von Forschungslücken und Forschungsbedarf zum Zusammenhang von Gender Mainstreaming und demographischem Wandel sowie die Entwicklung von Empfehlungen zur Aufnahme der Geschlechter- Perspektive in die Forschung zum demographischen Wandel. Die Ergebnisse des Projektes sind Inhalt der vorliegenden Dokumentation. Basis für die Bearbeitung des Projektes war eine intensive Auseinandersetzung mit dem Konzept des Gender Mainstreamings. Was Gender Mainstreaming meint, welche Bedeutung es für Wissenschaft und Forschung hat, wie es die Politik der europäischen Union bestimmt und wie es in den Bundesministerien - insbesondere im bmb+f - aufgegriffen wird, ist Inhalt des zweiten Kapitels. Die Ergebnisse der Analyse der Projekte aus dem Förderschwerpunkt 'Demographischer Wandel' werden im dritten Kapitel im Rahmen einer Bestandsaufnahme wiedergegeben. Hier sind die Gender-relevanten Aspekte der Forschungsverbünde sowie der Umsetzungs- und Transferprojekte (Stand März 2001) vermerkt und zum Teil tabellarisch aufbereitet. Auf Basis der Recherchen und einer Blindstellenanalyse wurden in rascher Abfolge fünf Expert/innen-Foren durchgeführt. Mit einer ausgewählten Gruppe von Sachverständigen wurde auf der Grundlage vorbereiteter Thesen eine dialogische Forschungsbedarfsabschätzung zu den Gender-Dimensionen des demographischen Wandels vorgenommen. Dabei standen folgende Themen im Mittelpunkt: Die Foren basierten auf der Idee des kumulativen Erkenntnisprozesses, das heißt, sie standen trotz eigener Themenschwerpunkte in einem thematischen Zusammenhang, so dass in allen Foren ein breites Spektrum von Fragen aufgegriffen und die Ergebnisse aufeinander bezogen werden konnten. Mit dieser Methode sollten Lücken bisheriger Forschung sichtbar gemacht

Abstract

The article discusses the study conducted on the quality of long-term residential care in six different countries including U.S., England and Canada. Reporting that the countries with highest rates of privatization were having complex, standardized and regulatory approach effects whereas, the lowest rates have for profit organizations with regulated approach in managing care. Thus, the report states that market forces, logic used and structural quality holds the implications in the sector.
