1. Role of Rucaparib in the Treatment of Prostate Cancer: Clinical Perspectives and Considerations

This is important for the development of therapeutics for the condition, as metastatic castrate-resistant prostate cancer (mCRPC) is resistant to standard androgen deprivation therapy (ADT) and has a relatively poor prognosis. We conducted a literature review on rucaparib, a poly(adenosine diphosphate-ribose) polymerase (PARP) inhibitor that is currently indicated for the treatment of patients with mCRPC who harbor mutations in BRCA1/2 (homologous recombination repair [HRR] genes) and who have already tried androgen receptor-axis-targeted therapies (ARAT) and a taxane chemotherapy. We describe rucaparib’s FDA approval, which was based on the results of the single-arm, open-label, Phase II TRITON2 clinical trial, which found an objective response rate (ORR) of 43.5%, a duration of response (DOR) of over six months in length and an acceptable safety profile. Rucaparib’s dosage and clinical considerations for use were also discussed. We also compared rucaparib’s use and safety profile with Olaparib, niraparib and talazoparib, three other PARP inhibitors tested for the treatment of mCRPC. Overall, initial results show that the safety profile of all four drugs in mCRPC was relatively similar, and further testing is currently indicated for all four. Differences in their metabolism, however, also warrant further research. The clinical validity of rucaparib will be tested by the follow-up TRITON3 clinical trial, which is comparing the effect of rucaparib compared to standard therapies for mCRPC harboring BRCA1/2 or ATM mutations. Other than TRITON3, other clinical trials are testing rucaparib’s ability against other cancers (prostate or otherwise) with HRR mutations, and also the efficacy of combination therapies involving rucaparib. Finally, more research is needed to elucidate rucaparib’s effect on HRR mutations other than BRCA1/2. Emerging techniques warrant an investigation into the efficacy of TRUS, how it compares to new techniques, and options to increase the accuracy of prostate cancer diagnosis. Currently, TRUS is used to guide both transrectal and transperineal biopsy approaches with similar cancer detection rates, but lower rates of infection have been reported with the transperineal approach, while lower rates of urinary retention are often reported with the transrectal approach. Multiparametric MRI has substantial benefits for prostate cancer diagnosis and triage such as lesion location, grading, and can be combined with TRUS to perform fusion biopsies targeting specific lesions. Micro-ultrasound generates higher resolution images that traditional ultrasound and has been shown effective at diagnosing PCa, giving it the potential to become a future standard of care. Finally, high-intensity focused ultrasound focal therapy administered via TRUS has been shown to offer safe and effective short-term oncological control for localized disease with low morbidity, and the precise nature makes it a viable option for salvage and repeat therapy.Keywords: transrectal ultrasound, prostate cancer, transperineal biopsy

The primary endpoint was the percentage of subjects in each group presenting adverse events (AEs). Secondary endpoints were visual analog scale (VAS) pain scores at Visit 2 (day 30) and Visit 3 (day 60) in relation to pretreatment values, Roland–Morris Questionnaire (RMQ) scores and finger-to-floor distance (FFD) (percentage of subjects per group presenting improvement ≥ 5 points and ≥ 3cm, respectively).Results: Seventy-five (24%) and 105 (33%) subjects (P=0.21) presented 133 and 241 AEs, with 3159% of subjects presenting ≥ 2 AEs (P=0.0019) in Group A and Group B, respectively. Twenty-four subjects in Group B were discontinued due to AEs, while no AE-related discontinuations occurred in Group A (P< 0.0001). VAS score reduction after 30 and 60 days of treatment was statistically significant (P< 0.0001) in both groups, with Group A showing greater reduction at Visit 2 (P< 0.0001). RMQ score improvement ≥ 5 points occurred in 99% of subjects from each group, and FFD improvement ≥ 3 cm occurred in all subjects.Conclusion: Treatment with nucleotides+B12 was associated with a lower number of total AEs, fewer AEs per subject, and no AE-related treatment discontinuation. Pain intensity (VAS) reduction was superior at 30 days of treatment in the nucleotides+B12 group and equivalent between groups at 60 days of treatment. Methods: A randomized, double-blind, controlled trial, consisting of a 30-day oral treatment period: Group A (n=200) receiving nucleotides + vitamin B12, and Group B (n=200) receiving vitamin B12 alone. The primary study endpoint was the percentage of subjects presenting pain visual analog scale (VAS) scores ≤20 at end of study treatment period. Secondary study endpoints included the percentage of subjects presenting improvement ≥5 points on the patient functionality questionnaire (PFQ); percentage of subjects presenting pain reduction (reduction in VAS scores at study end in relation to pretreatment); and number of subjects presenting adverse events. Results: The results of this study showed a more expressive improvement in efficacy evaluations among subjects treated with the combination of nucleotides + vitamin B12, with a statistically significant superiority of the combination in pain reduction (evidenced by VAS scores). There were adverse events in both treatment groups, but these were transitory and no severe adverse event was recorded during the study period. Safety parameters were maintained throughout the study in both treatment groups.Conclusion: The combination of uridine, cytidine, and vitamin B12 was safe and effective in the treatment of neuralgias arising from neural compression associated with degenerative orthopedic alterations and trauma. Abstract

There may be a high-energy cutoff of neutrino events in IceCube data. In particular, IceCube does not observe either continuum events above 2 PeV, or the Standard Model Glashow-resonance events expected at 6.3 PeV. There are also no higher energy neutrino signatures in the ANITA and Auger experiments. This absence of high-energy neutrino events motivates a fundamental restriction on neutrino energies above a few PeV. We postulate a simple scenario to terminate the neutrino spectrum that is Lorentz-invariance violating, but with a limiting neutrino velocity that is always smaller than the speed of light. If the limiting velocity of the neutrino applies also to its associated charged lepton, then a significant consequence is that the two-body decay modes of the charged pion are forbidden above two times the maximum neutrino energy, while the radiative decay modes are suppressed at higher energies. Abstract

The Large Hadron Collider presents an unprecedented opportunity to probe the realm of new physics in the TeV region and shed light on some of the core unresolved issues of particle physics. These include the nature of electroweak symmetry breaking, the origin of mass, the possible constituent of cold dark matter, new sources of CP violation needed to explain the baryon excess in the universe, the possible existence of extra gauge groups and extra matter, and importantly the path Nature chooses to resolve the hierarchy problem - is it supersymmetry or extra dimensions. Many models of new physics beyond the standard model contain a hidden sector which can be probed at the LHC. Additionally, the LHC will be a top factory and accurate measurements of the properties of the top and its rare decays will provide a window to new physics. Further, the LHC could shed light on the origin of neutralino masses if the new physics associated with their generation lies in the TeV region. Finally, the LHC is also a laboratory to test the hypothesis of TeV scale strings and D-brane models. An overview of these possibilities is presented in the spirit that it will serve as a companion to the Technical Design Reports (TDRs) by the particle detector groups ATLAS and CMS to facilitate the test of the new theoretical ideas at the LHC. Abstract

Within the context of mass-varying neutrinos, we construct a cosmological model that has a phase transition in the electromagnetic fine structure constant \alpha at a redshift of 0.5. The model accommodates hints of a time variable \alpha in quasar spectra and the nonobservance of such an effect at very low redshifts. It is consistent with limits from the recombination and primordial nucleosynthesis eras and is free of instabilities., Comment: 6 pages, 1 figure. Version to appear in PLB

Abstract

Ultra-high energy cosmic rays create black holes in scenarios with extra dimensions and TeV-scale gravity. In particular, cosmic neutrinos will produce black holes deep in the atmosphere, initiating quasi-horizontal showers far above the standard model rate. At the Auger Observatory, hundreds of black hole events may be observed, providing evidence for extra dimensions and the first opportunity for experimental study of microscopic black holes. If no black holes are found, the fundamental Planck scale must be above 2 TeV for any number of extra dimensions.

12. Exact Nonperturbative Unitary Amplitudes for 1->8 Transitions in a Field Theoretic Model

Abstract

We present a quantum mechanical model with an infinite number of (discrete) degrees of freedom, which can serve as a laboratory for multiparticle production in a collision. There is a cubic coupling between modes without, however, any problems associated with unstable ground states. The model is amenable to precise numerical calculations of nonperturbative 1->N transition amplitudes. On an ordinary workstation, time and memory limitations effectively restrict N to be $\le\ 8,$ and we present results for this case. We find (1) that there is reasonable period of time for which there is a constant rate for the 1->8 transition; (2) at the end of the linear period, the eight particle amplitude attains a maximum value $\aemax$ which is about $3-4$ orders of magnitude larger than the comparable amplitude for excitation of the $N=8$ state in the anharmonic oscillator; (3) for values of the coupling in the region where the Born approximation fails, the amplitude is much larger than the naive estimates $A_8\simeq \exp{(-1/\g2)}\ $ or $\ \exp{(-8)};$ it is more like $A_8\sim\exp{(-0.20/\g2)}.$, Comment: 9 pages, phyzzx format (4 figures available on request) NUB-3043/92-TH

Abstract

BACKGROUND: Retinoblastoma is the most common intraocular cancer worldwide. There is some evidence to suggest that major differences exist in treatment outcomes for children with retinoblastoma from different regions, but these differences have not been assessed on a global scale. We aimed to report 3-year outcomes for children with retinoblastoma globally and to investigate factors associated with survival. METHODS: We did a prospective cluster-based analysis of treatment-naive patients with retinoblastoma who were diagnosed between Jan 1, 2017, and Dec 31, 2017, then treated and followed up for 3 years. Patients were recruited from 260 specialised treatment centres worldwide. Data were obtained from participating centres on primary and additional treatments, duration of follow-up, metastasis, eye globe salvage, and survival outcome. We analysed time to death and time to enucleation with Cox regression models. FINDINGS: The cohort included 4064 children from 149 countries. The median age at diagnosis was 23·2 months (IQR 11·0-36·5). Extraocular tumour spread (cT4 of the cTNMH classification) at diagnosis was reported in five (0·8%) of 636 children from high-income countries, 55 (5·4%) of 1027 children from upper-middle-income countries, 342 (19·7%) of 1738 children from lower-middle-income countries, and 196 (42·9%) of 457 children from low-income countries. Enucleation surgery was available for all children and intravenous chemotherapy was available for 4014 (98·8%) of 4064 children. The 3-year survival rate was 99·5% (95% CI 98·8-100·0) for children from high-income countries, 91·2% (89·5-93·0) for children from upper-middle-income countries, 80·3% (78·3-82·3) for children from lower-middle-income countries, and 57·3% (52·1-63·0) for children from low-income countries. Abstract

High dielectric-constant materials are increasingly needed in microwave circuit miniaturization for outdoor and indoor wireless applications. In this paper, we introduce temperature-stable high dielectric-constant thermoplastic polymer compositions and copper-laminated sheets made out of them for microwave circuits. The compositions are made by mixing suitable ceramics and polymers. The design, fabrication, and properties of these compositions are discussed. The effect of temperature on the resonance frequency of a patch antenna made out of the copper-laminated high dielectric-constant composite sheets is also discussed. Abstract

OBJECTIVES: To present long-term oncological outcomes of patients with paratesticular sarcoma treated by a multidisciplinary team. PATIENTS AND METHODS: Patients managed at the Princess Margaret Cancer Centre, between 1990 and 2012, were analysed. A sarcoma expert performed central pathology review. Kaplan-Meier graphs compared local recurrence (LR), metastasis, and overall survival (OS) of patients treated with hemiscrotectomy vs those who did not. Univariable Cox proportional hazards analysis was performed to delineate predictors of LR, metastasis, and OS. RESULTS: Overall, 51 patients with a median (interquartile range) follow-up of 132 (51.6-226.8) months were analysed. At presentation, 92.2% (47 patients) had localised disease. Only five patients (9.8%) had undergone initially planned hemiscrotectomy. Completion and salvage hemiscrotectomy was performed in 25 (54.3%) and seven (15.2%) patients, respectively. Recurrence and metastasis occurred in 12 (25.5%) and 10 patients (19.6%), respectively. At the last follow-up, 21.6% (11 patients) had died, with eight dying from their disease. Kaplan-Meyer graphs demonstrated that hemiscrotectomy improved LR (median not reached vs 62.4 months, log-rank P = 0.008) and OS (median not reached vs 168 months, log-rank P = 0.081). Univariable analysis found hemiscrotectomy to be associated with a lower LR rate (hazard ratio [HR] 0.21, P = 0.02), whilst positive margins at initial surgery were associated with increased LR (HR 4.81, P = 0.047). No metastasis predictors were found, but age (HR 1.04, 95% confidence interval [CI] 1.0-1.08; P = 0.02) and non-localised disease at presentation (HR5.17, 95% CI 1.33-20.06; P = 0.017) were associated with worse OS. CONCLUSION: Paratesticular sarcoma is a rare tumour, predominantly manifesting as localised disease. Most patients receive an initial suboptimal oncological surgery. Improved long-term outcomes are demonstrated following early hemiscrotectomy.
