Abstract

This methods article concerns analysing data generated from running experiments on agent based models to study industries and organisations. It demonstrates that when researchers study virtual ecologies they can and should discard statistical controls in favour of experiment controls. In the first of two illustrations we show that we can detect an effect with a fraction of the data needed for a traditional analysis, which is valuable given the computational complexity of many models. In the second we show that agent based models can provide control without introducing the biases associated with certain causal structures.

Abstract

Major Depressive Disorder (MDD) is one of the most common causes of disability worldwide. Unfortunately, about one-third of patients do not benefit sufficiently from available treatments and not many new drugs have been developed in this area in recent years. We thus need better and faster ways to evaluate many different treatment options quickly. Platform trials are a possible remedy - they facilitate the evaluation of more investigational treatments in a shorter period of time by sharing controls, as well as reducing clinical trial activation and recruitment times. We discuss design considerations for a platform trial in MDD, taking into account the unique disease characteristics, and present the results of extensive simulations to investigate the operating characteristics under various realistic scenarios. To allow the testing of more treatments, interim futility analyses should be performed to eliminate treatments that have either no or negligible treatment effect. Furthermore, we investigate different randomisation and allocation strategies as well as the impact of the per-treatment arm sample size. We compare the operating characteristics of such platform trials to those of traditional randomised controlled trials and highlight the potential advantages of platform trials.

9. Social Representation of Non-Academic Work in Mexico in the Light of Cultural Artefacts

Abstract

Context: Technical and Vocational Education and Training (TVET) has attracted increasing interest in recent years due to its potential to address productivity and equity challenges, such as better employment prospects, as outlined in the United Nations Sustainable Development Goals (SDGs). Despite the potential of such programmes, the enrolment rate in vocational training at upper secondary level in Mexico is 38.2%, i.e. below the Organisation for Economic Cooperation and Development (OECD) average of 45.7%. This raises the question of possible reasons for the low enrolment rate. Approach: Based on the assumption that attitudes towards non-academic work are culturally anchored in Mexican society, which also shapes the educational and career aspirations of younger generations, the project named Cultural Practice of Non-academic Work in Mexico (KuPraMex), funded by the German Federal Ministry of Education and Research (BMBF), investigates social representations of non-academic work in Mexico. This is done through the analysis of artefacts such as films, murals, etc., as these are part of the tangible culture of a society. As materialised products of human activities or cultural practices, artefacts can be understood as objectifications of social relations and conditions. Therefore, in this context, it is assumed that through the analysis of cultural artefacts, a deeper understanding of how nonacademic work is thought, felt, and valued in Mexican society will emerge. Findings: It has been found that the topic on non-academic work is often associated with informality. Moreover, the representations and narratives in cultural artefacts often show that non-academic work, apart from office work, is physically challenging but cognitively undemanding. In terms of access to a company, social networks seem to have enormous relevance. Hierarchies seem rigid and opportunities for promotion limited. However, narratives with more positive attributions regarding non-academic work can also be identified, which state that young people experience a habitus transformation through work. Conclusion: Nevertheless, non-academic work in Mexico seems to lack prestige, which may affect young people's educational and career aspirations and choices. This could mean that those who can afford it prefer to pursue a career in tertiary education rather than opt for a TVET programme. At the macro level, the mentioned lack of prestige could hamper attempts to offer such programmes.

Abstract

Platform trials can evaluate the efficacy of several treatments compared to a control. The number of treatments is not fixed, as arms may be added or removed as the trial progresses. Platform trials are more efficient than independent parallel-group trials because of using shared control groups. For arms entering the trial later, not all patients in the control group are randomised concurrently. The control group is then divided into concurrent and non-concurrent controls. Using non-concurrent controls (NCC) can improve the trial's efficiency, but can introduce bias due to time trends. We focus on a platform trial with two treatment arms and a common control arm. Assuming that the second treatment arm is added later, we assess the robustness of model-based approaches to adjust for time trends when using NCC. We consider approaches where time trends are modeled as linear or as a step function, with steps at times where arms enter or leave the trial. For trials with continuous or binary outcomes, we investigate the type 1 error (t1e) rate and power of testing the efficacy of the newly added arm under a range of scenarios. In addition to scenarios where time trends are equal across arms, we investigate settings with trends that are different or not additive in the model scale. A step function model fitted on data from all arms gives increased power while controlling the t1e, as long as the time trends are equal for the different arms and additive on the model scale. This holds even if the trend's shape deviates from a step function if block randomisation is used. But if trends differ between arms or are not additive on the model scale, t1e control may be lost. The efficiency gained by using step function models to incorporate NCC can outweigh potential biases. However, the specifics of the trial, plausibility of different time trends, and robustness of results should be considered

Abstract

Criteria for treatment-resistant depression (TRD) and partially responsive depression (PRD) as subtypes of major depressive disorder (MDD) are not unequivocally defined. In the present document we used a Delphi-method-based consensus approach to define TRD and PRD and to serve as operational criteria for future clinical studies, especially if conducted for regulatory purposes. We reviewed the literature and brought together a group of international experts (including clinicians, academics, researchers, employees of pharmaceutical companies, regulatory bodies representatives, and one person with lived experience) to evaluate the state-of-the-art and main controversies regarding the current classification. We then provided recommendations on how to design clinical trials, and on how to guide research in unmet needs and knowledge gaps. This report will feed into one of the main objectives of the EUropean Patient-cEntric clinicAl tRial pLatforms, Innovative Medicines Initiative (EU-PEARL, IMI) MDD project, to design a protocol for platform trials of new medications for TRD/PRD.

Abstract

In the agri‐food chain, while the impact of producers and consumers on sustainability has been well studied, food processing has been less explored. This position paper aims to discuss the potential of food processing to address all food and nutrition security (FNS) outcomes in order to achieve improved food system sustainability. First, FNS dimensions and the four pillars of agro‐food industry sustainability are defined, with a focus on ultra‐processed foods. Second, the food matrix concept is developed as a new paradigm to holistically address all FNS. It is concluded that food processing should become more involved in circular food systems and bioeconomy, and that we need to relocate food production, processing, and consumption to be more aligned with regional food production specificities. For this, minimal processing to preserve food matrices should be preferred. Therefore, the strong current tendency to develop reductionist and siloed innovative solutions to improve the sustainability of food systems should be questioned. [ABSTRACT FROM AUTHOR]
