Introduction: The temporomandibular joint (TMJ) is the most active joint of the body. Limitations of the mouth opening can be caused by bony or fibrous ankylosis of the TMJ as a sequela to trauma, infection, autoimmune disease or failed surgery. A significant effect of TMJ ankylosis is obstructive sleep apnea (OSA) which can progressively worsen and the symptoms associated with OSA can dominate sidelining the primary disorder itself. Aim/Objectives: The purpose of this study was to investigate and compare the use of conventional rotary bur technique and piezosurgery unit for gap arthroplasty in TMJ ankylosis. Method Used: Twenty patients were examined clinically and radiologically for the treatment. Out of the twenty patients, ten patients were operated using conventional rotary bur technique (Group A) and the remaining ten patients were operated using piezosurgery unit (Group B). Two major parameters that is blood loss and duration of surgery were evaluated intraoperatively and other parameters like pain, mouth opening, swelling and paraesthesia were evaluated on Post Operative Day (POD)-1, 5, 10, 15. Result: We observed that there was a significant reduction in blood loss intraoperatively with the piezosurgery unit though the operating time was prolonged. Post-operative physiotherapy had increased the inter-incisal distance in both groups. Pain levels and swelling were significantly higher in Group A. Other complications like paresthesia were not observed in either of the groups. Conclusion: This comparative study suggests piezosurgery can be a valuable alternative method to the bur technique due to its efficacy in the evaluated parameters.

PURPOSE: The aim of this study is to comprehensively review and analyse pure odontogenic maxillofacial space infections in a tertiary care hospital in Navi Mumbai over a period of 5 years. METHODS: A retrospective analysis of 315 patients treated at Dr. D. Y. Patil Dental College and Hospital at Nerul, Navi Mumbai, from January 2007 to December 2011 was done. Multiple variables were analysed. Localised infections like dentoalveolar infections without space involvement and infection of non-odontogenic cause were excluded from the study. RESULTS: Analysis of the records was done. Majority of patients were from lower socioeconomic background and were daily wage workers who had either consulted a general physician or a general dental practitioner or had self-medicated themselves before presenting to us with acute symptoms. Early recognition and prompt treatment involving intravenous antibiotics with extraction of involved tooth/teeth and incision and drainage helped in resolution of infections in a span of 72 h. Medically compromised patients had longer duration of hospital stay as compared to the patients who had no underlying medical condition. Majority of space infections involved multiple spaces and local anaesthesia with sedation was found to be the satisfactory mode of anaesthesia. Complications were very few. CONCLUSION: We concurred that any form of odontogenic maxillofacial space infection should be rendered prompt and aggressive treatment and hospitalisation should be recommended wherever required.

Glandular odontogenic cyst is an extremely rare lesion of the jaw. Its aggressive behavior and high recurrence rate makes it important while excising. Both clinical and radiographic features were nonspecific due to the similarities with other lesions therefore it can be misdiagnosed very easily. This article presents another case of Glandular odontogenic cyst in 17 year of male patient who was unaware of the lesion which was detected on routine examination.

The radicular cyst is the most common inflammatory odontogenic cystic lesion of the jaws. It usually originates as a sequel to a periapical inflammatory process, following chemical, physical or bacterial injury. Due to its chronic etiology, the cyst usually appears towards the later stage of life. It has a male sex predilection, with the maxillary anterior region as the most common site of involvement. This article reports a rare case of a large radicular cyst in the mandible, its management and follow up along one year.

Lichen Planus is a muco-cutaneous disorder affecting females. The oral lesions precede the skin lesions, with malignant potential rate, ranging from 0.3 to 3%. Malignant changes are usually associated with stress and cocarcinogens. Usually, malignant transformation is presented as solitary lesion in the oral cavity, Oral Squamous Cell Carcinoma (OSCC), developing at multiple sites, as a very rare finding in patients of Oral Lichen Planus (OLP). A case of OLP without risk factors and strong history of stress, the Grinspan syndrome with field cancerization is presented in this article

Purpose: The purpose of this study was to analyze the incidence of various complications following routine exodontia performed using fixed protocols. Materials and Methods: A total of 22,330 extractions carried out in 14,975 patients, aged between 14 and 82 years, who reported to the Department of Oral and Maxillofacial Surgery at Padmashree Dr. D. Y. Patil Dental College and Hospital, Nerul, Navi Mumbai, were evaluated for various complications. Results: The most common complications encountered were tooth fracture, trismus, fracture of cortical plates and dry socket. Wound dehiscence, postoperative pain and hemorrhage were encountered less frequently. Luxation of adjacent teeth, fracture of maxillary tuberosity, and displacement of tooth into adjacent tissue spaces were rare complications. Conclusion: The practice of exodontia inevitably results in complications from time to time. It is imperative for the clinician to recognize impending complications and manage them accordingly.

Existence of unicystic ameloblastoma with desmoplasia is a seldom occurrence. It was first described by Waldron and El-Mofty in the year 1987 as the term hybrid ameloblastoma. Here, we account a case of hybrid lesion depicting outstanding unicystic, follicular ameloblastoma with desmoplasia and mucus cell differentiation showing inductive changes. The patient is a 38-year-old male with a swelling in the lower right posterior region of the jaw. Radiologically, a mixed radiopaque-radiolucent lesion was sighted in this region. Taking into account of patient's complaint, clinical, radiographic, and histologic features; diagnosis of ameloblastic fibrodentinoma was made. It was followed by complete excision of the tumor. The challenge in the management of this tumor was to render complete excision as recurrence may occur in incomplete removal and also to reconstruct the bony defect to give a moderate cosmetic and functional result to the patient. The excisional biopsy report was established to be of hybrid ameloblastoma. Since there are few cases reported in the past of this lesion, the present case is an addition for better perception of this odontogenic tumor. This article identifies some profound concepts and new developments in the histopathologic design of ameloblastoma and its hybrid variant. It also briefly reviews the literature referring to this rare tumor entity. Considering the characteristics of ameloblastoma, priority of surgical management for hybrid lesion is also described.
