Background: Only 55% of anterior cruciate ligament-reconstructed (ACLR) athletes return to competitive sports. This brings into question the usefulness of current return to sport (RTS) criteria. High cognitive demand of sport environment clarifies the value of incorporating neurocognitive tests when making decisions regarding the time of RTS. This preliminary study aimed to compare the neurocognitive functions between healthy controls and ACLR male athletes who passed or failed RTS criteria. Methods: A total of 45 male football players, including 15 ACLR who passed RTS criteria, 15 ACLR who did not pass, and 15 healthy controls participated in this cross-sectional study. The Cambridge Neuropsychological Test Automated Battery was used to measure a battery of neurocognitive tasks, including speed of response, sustained attention, working memory, cognitive flexibility, and response inhibition. Results: The results revealed that compared with both the ACLR-passed and healthy groups, the ACLR-failed group showed greater values of 5-choice movement time (P =.02, P =.01, respectively) but lower values of stop signal reaction time (P =.03, P =.001, respectively) and proportion of successful stops variables (P =.02). In addition, compared with the healthy group, both the ACLR-failed and ACLR-passed groups indicated greater values in between errors (P <.001, P =.008, respectively) and reaction latency variables (P =.002, P =.01, respectively) but lower values of A′ (P <.001, P =.007, respectively), probability of hit (P <.001, P =.03, respectively), and percent correct trials variables (P =.006, P =.02, respectively). Conclusions: Our findings indicated deficits in neurocognitive functions in ACLR male athletes. In addition, poor performance in sustained attention, working memory, and cognitive flexibility measures observed in the ACLR-passed group highlighted the necessity for using a multimodal approach via implementation of neurocognitive measures in conjunction with the functional and muscular assessments when making RTS decisions. Rupture of the anterior cruciate ligament (ACL) disturbs the stability of the knee, and increases the risk of subsequent meniscal damage and the risk of developing degenerative joint disease. Surgical ACL reconstruction (ACLR) is one of the most common orthopedic surgeries. This study aimed to investigate the effective factors on the rehabilitation rate in ACLR with hamstring autograft based on the Tegner activity scale (TAS) and Lysholm knee scoring scale (LKS) one year after the operation. In this descriptive-analytical study, 140 patients who underwent ACLR surgery with hamstring autograft were evaluated. Patients were evaluated based on TAS and LKS scores before and after the surgery and their responses were recorded. Then, the rehabilitation rate was calculated based on these two scales, and the association between the rehabilitation rate and pre-operative and post-operative parameters was investigated. Based on the findings, LKS scores after surgery were significantly associated with age, gender, education, injury mechanism, loss of full flexion ROM, analgesic consumption after surgery, surgical site infection, and BMI. Also, the LKS score was higher in people whose Tegner activity scale score was 8 before the injury. There was no significant association with smoking before surgery, hematoma at the graft site after surgery, knee meniscal lesions, and hemarthrosis formation during the first six hours of ACL injury. Determining the effective factors before and after surgery on the rehabilitation rate of patients undergoing ACLR who were treated with hamstring autograft may be useful in evaluating the rehabilitation rate of patients. Context: Core stability training has been recommended as a vital element in improving movement's pattern and athletic performance. The main objective of this study was to investigate the effect of 12-week McGill core stability training on movement patterns, shooting accuracy, and throwing performance. Design: Randomized controlled clinical trial. Setting: University research laboratory. Methods: Forty male basketball players were randomly assigned to experimental and control groups. The experimental group completed 12-week McGill core stability training, while the control group completed routine exercise training. Patterns of functional movements was measured through functional movement screen (FMS), shooting accuracy measured by static 3-point shooting (S3P) and dynamic 60-second 3-point shooting test, and throwing performance measured by Functional Throwing Performance Index. Results: Comparison revealed that regardless of received training, after 12 weeks both groups showed significant improvement in all outcome measures. However, experimental group had significantly higher post test scores in FMS (P = .02), S3P (P = .007), and dynamic 60-second 3-point shooting test (P = .01). For Functional Throwing Performance Index, there was no group differences (P = .96). The results of follow-up assessments showed for all measurements including FMS (P = .03), S3P (P = .004), dynamic 60-second 3-point shooting test (P < .001), and Functional Throwing Performance Index (P = .005); experimental group had higher scores than the control group. Conclusions: According to the results, implementing McGill core stability training in basketball routine training would be advisable since significant improvement can be obtained in the measured parameters. Introduction: There is little evidence regarding the effect of trunk‐stabilizing muscle training on the improvement of stress urinary incontinence (SUI) symptoms. Objective: To investigate the effect of trunk‐stabilizing muscle training on transabdominal ultrasonography (TAUS) and clinical urological indices, and on the quality of life (QoL) in women with SUI. Design: Randomized controlled trial. Setting: A university hospital. Participants: Forty‐six women with SUI, ages 20–55 years, were randomly assigned to an experimental (n = 23) and control group (n = 23). Interventions: The experimental group performed trunk‐stabilization exercises according to the Sapsford protocol, whereas the control group performed pelvic floor muscle (PFM) exercises for 8 weeks. Main Outcome Measures: The primary outcome measure was bladder base displacement (BBD), assessed by TAUS during PFM contraction (PFMC), Valsalva, and abdominal curl. The secondary outcome measures were PFM strength, the severity of urinary incontinence (UI), voiding diary, and QoL, assessed by the Modified Oxford Grading System, the severity index, frequency chart, and lower urinary tract symptoms‐QoL questionnaire, respectively. All variables were assessed at baseline and after 8 weeks of intervention. Results: The interaction of group and time was not significant for BBD during PFMC (p =.98), Valsalva (p =.28), abdominal curl (p =.34), and secondary variables (p >.05). The main effect of time was significant in both groups for BBD during PFMC, PFM strength, the severity of UI, voiding diary, and QoL (p <.001), with effect size (d) of 0.30, 0.80, 2.05, 1.07, and 1.03 in the control; and 0.49, 0.52, 1.75, 0.66, and 0.88 in the experimental group, respectively. The main effect of the group was not significant for BBD during PFMC (p =.68), Valsalva (p =.22), abdominal curl (p =.53), and secondary variables (p >.05). Conclusions: Trunk‐stabilizing muscle training and PFM exercise are equally effective in the improvement of PFM function, UI symptoms, and QoL in women with SUI. The goal of this study was to see whether the Western Ontario and McMaster Universities Osteoarthritis index (WOMAC), Lequesne Algofunctional index and the Arthritis Impact Measurement Scale-short form (AIMS2-SF) could changes after physiotherapy interventions (i.e., responsiveness) and to determine Minimal Clinically Important Difference (MCID) in the performance of the people with knee osteoarthritis. A convenient sample of 116 people with knee osteoarthritis completed the tools at baseline and then again after 10 sessions physiotherapy intervention. Two techniques were used to determine responsiveness: The receiver operating characteristics (ROC) method and the correlation analysis. All of the tools showed the AUC of greater than 0.70 (AUC ranges = 0.72 − 0.83). For the WOMAC, Lequesne Algofunctional index, AIMS2-SF and VAS-pain, optimal cutoff points were 12.5, 2.75, 4.5 and 2.5 points, respectively. The gamma correlation between WOMAC, Lequesne Algofunctional index, AIMS2-SF, VAS-pain, and Global Rating Change (GRC) scores was 0.55, 0.52, 0.40, and 0.46, respectively. In people with knee osteoarthritis, the WOMAC has the maximum responsiveness to clinical changes. The MCID values identify in this study will aid in determining whether or not an individual with knee osteoarthritis has undergone a true improvement since receiving physiotherapy. The results of this study provide valuable information regarding to the ability of outcome measures to detect treatment effects in patients with knee osteoarthritis. The WOMAC questionnaire is a responsive tool to measure the changes in functional activity due to physiotherapy intervention in patients with knee osteoarthritis. Background: The Knee Injury Osteoarthritis Outcome Score (KOOS) is a widely used patient-reported questionnaire to evaluate individuals with knee osteoarthritis or a history of knee injuries. Objectives: The aim of this study was to evaluate the responsiveness and establish the minimal important change (MIC) for the Arabic version of KOOS in Iraqi individuals with knee osteoarthritis following physiotherapy intervention. Methods: Fifty volunteers (13 male and 37 female, meanage: 59.3 ± 9.6 years old) with an orthopedic diagnosis of knee osteoarthritis participated in this study. The Arabic KOOS was completed by the participants once before the intervention and once after completing a 4-week physiotherapy treatment program. A seven-item global rating of change scale was used as an external anchor to assess the participants' opinion about the treatment and its possible changes. Responsiveness was evaluated by calculating the receiver operating characteristic (ROC) curve, and the MIC was established by identifying an optimal cut-off on the ROC curve. Results: All subscales of the Arabic KOOS showed an area under the curve > 0.70. The MICs reflecting the values required by the participants to feel the improvement were 15.5, 14.5, 11, 11.5, and 12.5 for the pain, symptoms, activities of daily living, sports and recreation, and quality of life subscales, respectively. Conclusions: All subscales of the Arabic KOOS are responsive to physiotherapy treatment. Patients with nonspecific chronic low back pain (NSCLBP) complain of pain and fatigue during functional activities. Poor back muscle endurance has been associated with recurrent pain and long-term disability. However, the relationship between proximal muscle fatigue and postural stability is not well understood. To investigate the effects of lumbar extensor and hip abductor fatigue on postural stability in NSCLBPs. To induce isolated fatigue, participants (24 LBPs, 24 controls) were asked to perform multiple back extension and hip abduction in the separate sessions until the muscle force reached 60% of pre-fatigued force. The overall (OSI), anteroposterior (APSI), and mediolateral (MLSI) stability indices were measured during single-leg stance using the Biodex dynamic platform. The results revealed higher postural instability (APSI and OSI) following fatiguing lumbar extensor and hip abductor muscles (greater effect size for APSI than OSI) in LBPs than controls. Furthermore, significant and inverse relationships were found between the level of physical activity, pain, and post-fatigue postural instability (OSI) in NSCLBPs. Fatigue of proximal muscles could differentiate postural stability primarily in the sagittal plane between NSCLBPs and controls. Objective The primary purpose of physiotherapy in patients with Anterior Cruciate Ligament (ACL) reconstruction is to reduce pain, improve Range of Motion (ROM), muscle strength, and balance after surgery. Thus, therapists need outcome measurement tools with acceptable validity, reproducibility, and responsiveness to assess these changes after the intervention. This study investigates the responsiveness of the tools used to measure pain, ROM, knee muscle strength, and dynamic balance. We also want to determine minimal clinically significant change for these outcomes in patients with ACL reconstruction after physiotherapy. Materials & Methods The study participants were 54 young patients with ACL reconstruction evaluated using impairment-based outcome measures of visual analog scale, goniometer, hand-held dynamometer, and Star Excursion Balance Test (SEBT) at baseline (2 weeks after ACL reconstruction) and 4 weeks after physiotherapy. At the second phase of evaluation, the participants also completed the 7-point global change rating scale. For assessing the responsiveness of the tools, we used the Receiver Operating Characteristics (ROC) curve and correlation analysis. Results Analyzing the ROC curve showed that the knee extension ROM, quadriceps and hamstring strength, and medial and posteromedial directions under SEBT had acceptable responsiveness (>0.70). The Spearman correlation coefficient between the scores obtained for these outcomes and the score of 7-point global change rating scale were significant in the range of 0.36-0.51. For all the study outcome measurement tools, minimal significant clinical changes were reported. Conclusion The results of this study support the responsiveness of goniometer for knee extension ROM, hand-held dynamometer for quadriceps and hamstring strength, and SEBT for dynamic balance at medial and posteromedial directions in the assessment of clinical changes in patients with ACL reconstruction. The reported minimal significant clinical changes for each tool can help the clinicians and researchers to decide on determining the actual significant change in the ACL patient's clinical conditions after physiotherapy. IntroductionPurposeMethodResultsConclusionThe International Knee Documentation Committee (IKDC) and Anterior Cruciate Ligament – Quality of Life Questionnaire (ACL-QOL) are frequently used patient-reported instruments designed for individuals with anterior cruciate ligament reconstruction (ACL-R).To evaluate responsiveness and determine minimal important change (MIC) for the Persian-version of IKDC and ACL-QOL in athletes with ACL-R following physiotherapy intervention.One hundred athletes with ACL-R, undergoing 16-week physiotherapy completed IKDC and ACL-QOL at weeks 4 and 20 post ACL-R. Participants also rated their overall changes on a 7-point global rating of change at follow-up. Responsiveness was evaluated using the receiver operating characteristics (ROC) curve and correlation analysis. In addition, the minimal important change (MIC) was determined on the ROC curve.Acceptable responsiveness was reached by the IKDC and most of the subscales of ACL-QOL (area under the ROC curve (AUC) of 0.72–0.79). Also, the subscale of Lifestyle issues of ACL-QOL (AUC of 0.81 (95% CI = 0.72–0.89) and the overall of ACL-QOL (AUC of 0.87 (95% CI = 0.80–0.93) showed the good level of responsiveness. The MIC scores of IKDC and the Overall ACL-QOL were determined 25 and 26 points, respectively.The IKDC and ACL-QOL questionnaires have adequate responsiveness and are able to measure the change in athletes with ACL-R following a 16-week physiotherapy intervention. To evaluate the responsiveness and determine the minimal clinically important changes (MCICs), anchored by the patient response to a 7-point global rating scale, for Knee Injury and Osteoarthritis Outcome Score (KOOS), and Tegner activity scale in athletes undergoing physiotherapy treatment after anterior cruciate ligament reconstruction (ACL-R). Methods: Fifty-four patients undergoing physiotherapy completed the Persian versions of KOOS and Tegner scales at weeks 6 and 10 post ACL-R. The 7-point global rating of change was also completed at week 10. Responsiveness was calculated via receiver operating characteristic curve and correlation analysis. Results: Acceptable responsiveness was reached by the KOOS sports and recreation subscale (Sport/Rec) (area under the curve (AUC) = 0.72; Gamma = 0.37) and Tegner scale (AUC = 0.75; Gamma = 0.59). The MCIC scores of KOOS subscales and Tegner scale were reported. Conclusion: Our findings demonstrated that the KOOS Sport/Rec subscale and Tegner scale have adequate responsiveness between weeks 6 and 10 of physiotherapy. Therefore, these scales should be used to evaluate the effects of physiotherapy treatment and the changes in activity levels in this population. BACKGROUND: Decision making and the quality of care provided for chronic diseases have been shown to improve through patient participation. The HeartQoL questionnaire is a core healthrelated quality of life (HRQOL) tool specifically designed for individuals with ischemic heart disease (IHD) who have undergone interventions such as cardiac rehabilitation (CR). METHODS: In this cross-sectional and multicenter study, 150 patients were recruited. The participants completed the HeartQoL, MacNew Heart Disease Questionnaire, and Short Form Health Survey (SF-36) on entering CR for validity assessment. The HeartQoL along with a Global Rating of Change (GRoC) scale (for responsiveness measurement) were completed by 100 participants 3 months later. RESULTS: The mean age of all participants in validity assessment was 61.87 ± 8.13 years. Cronbach's alphas of the total scales ranged from 0.70 to 0.81 and of the subscales from 0.70 to 0.82. The Pearson correlation coefficient was used to determine construct validity; similar constructs were confirmed with correlation coefficients ranging from 0.50 to 0.69 and dissimilar constructs with correlation coefficients ranging from 0.28 to 0.29 (P < 0.010). The assessment of the responsiveness of the questionnaire indicated that the area under curve (AUC) was greater than 0.70 (range: 0.74 to 0.91) and the optimal cut-off point was 0.65. CONCLUSION: The Persian version of the HeartQoL questionnaire demonstrated satisfactory psychometric properties in the sample of participants admitted to CR after coronary artery bypass grafting (CABG). The present study results showed that the HRQOL can be used by clinicians and researchers in conjunction with other outcome measures to gain additional information about symptoms relevant to HRQOL in patients referred to CR and to evaluate change over time. Objective People with Anterior Cruciate Ligament (ACL) reconstruction need to have almost automatic postural control to focus on their athletic skills. Continuous cognitive task is a method for removing attention from postural control. The effects of continuous cognitive task on postural control of people with ACL reconstruction has been assessed by laboratory tools; however, its effect using balance error scoring system is still unknown. Therefore, the present study aimed to investigate the effects of continuous cognitive task on the postural control of individuals with ACL reconstruction using the balance error scoring system. Materials & Methods The present study is a case-control study using simple non-probability sampling. Participants included 20 male football players with ACL reconstruction (13.90±4.15 months after surgery) and 20 healthy men. They were matched for age, sex, height, weight, body mass index, dominant leg, education and activity levels. Tegner activity level scale was used to determine activity level of participants. Postural control was evaluated under single-leg stance test (with treated leg) on firm and foam surfaces with and without continuous cognitive task. Balance error scoring system was used to assess the balance control. In the condition without cognitive task, no feedback was provided. Continuous cognitive task consisted of 10 three-digit numbers each presented with a 2-second interval by audio tape and headphone. In the cognitive condition, individuals were asked to focus on the preselected number and provide the sum of its repetitions in the end of the test. The dependent variable was the number of balance error in each condition. Balance errors included lifting hands from hip, opening eyes, stepping or swinging, lifting forefoot or heal, moving hip into more than 30 degrees of hip abduction or flexion, and remaining out of position for more than 5 seconds. We used SPSS V. 20 software for statistical analysis of data. Kolmogorov-Simonov test was performed to test the normality of data distribution. Intraclass Correlation Coefficient (ICC) was calculated to assess the relative reliability of the rater for assessing the balance error. The absolute reliability of the rater was measured using Standard Error of Measurement (SEM), Minimal Metrically Detectable Change (MMDC) and Coefficient of Variation (CV). Mixed-model of ANOVA (2 groups ×4 stance conditions) was used to investigate the main effect and interaction effect of these factors. We used repeated measure ANOVA to determine the main effect of test condition in every each, and independent-sample t-test and effect size was used to determine the difference between groups. Results Distribution of data in all four test conditions was normal (P>0.05). The rater reliability was excellent for calculating the balance error in all test conditions (ICC>0.90, MMDC<%10 and CV<%5). Interaction between group and test condition was significant (F=11.59, P<0.01). Result of independent-sample t-test showed that subjects had a higher balance error than healthy subjects under single-leg stance test on foam without cognitive task (F=2.01, P<0.01). In this condition, the effect size also showed that difference between groups was large (>0.8). The main effect of test condition in subjects showed that the continuous cognitive task reduced balance error while standing on both firm and foam surfaces (P<0.01), while in healthy subjects, the cognitive task had no effect on the balance (P>0.05). Conclusion It can be concluded that male football players with ACL reconstruction had poorer stability compared to healthy subjects in performing single-leg stance test on foam surface without continuous cognitive task. Introduction: Type 2 diabetes is one of the major causes of progressive microvascular and macro vascular complications leading to retinopathy, nephropathy, and neuropathy as well pulmonary complications due to changes in collagen, elastin, and micro angiopathy. The current study aimed at assessing some respiratory indices in patients with type 2 diabetes and the possible relationships between these parameters and the level of blood glucose control and duration of diabetes. Materials and Methods: In the current descriptive-analytical study, 20 patients with type 2 diabetes were compared with 20 matched healthy subjects. The information of the respiratory system was collected using a chronograph. Parameters for assessment were respiration rate, heartbeats, and oxygen saturation percentage. Results: The results of the study showed a significant reduction in end-expiratory carbon dioxide pressure and increased respiratory rate in the patients compared to the control group. The results also showed a strong and inverse relationship between hemoglobin A1c (HbA1c) and respiratory indices. The duration of having diabetes had a negative significant relationship with respiratory indices. Conclusion: In general, the results of the study indicated that lung function is impaired in people with type 2 diabetes. Purpose: The aim of this methodological study was to cross-culturally translate the Shoulder Activity Scale (SAS) into the Persian and determine its clinimetric properties including reliability, validity, and responsiveness in patients with shoulder disorders. Method: Persian version of the SAS was obtained after standard forward-backward translation. Three questionnaires were completed by the respondents: SAS, shoulder pain and disability index (SPADI), and Short-Form 36 Health Survey (SF-36). The patients completed the SAS, 1 week after the first visit to evaluate the test-retest reliability. Construct validity was evaluated by examining the associations between the scores on the SAS and the scores obtained from the SPADI, SF-36, and age of the patients. To assess responsiveness, data were collected in the first visit and then again after 4 weeks physiotherapy intervention. Test-retest reliability and internal consistency were assessed using Intra-class Correlation Coefficient (ICC) and Cronbach's alpha, respectively. To evaluate construct validity, Spearman's rank correlation was used. The ability of the SAS to detect changes was evaluated by the receiver-operating characteristics method. Results: No problem or language difficulties were reported during translation process. Test-retest reliability of the SAS was excellent with an ICC of 0.98. Also, the marginal Cronbach's alpha level of 0.64 was obtained. The correlation between the SAS and the SPADI was low, proving divergent validity, whereas the correlations between the SAS and the SF-36/age were moderate proving convergent validity. A marginally acceptable responsiveness was achieved for the Persian SAS. Conclusions: The study provides some evidences to support the test-retest reliability, internal consistency, construct validity, and responsiveness of the Persian version of the SAS in patients with shoulder disorders. Background and Objective: Postural control in people with nonspecific low back pain (NSLBP) is disturbed. To investigate this disturbance, an external mechanical perturbation can be applied. The effect of local mechanical vibration on postural control through external perturbation in people with NSLBP has been less studied, so the purpose of this study was to investigate the ability to maintain postural stability in this patients following local mechanical vibration under external perturbation. Subjects and Methods: Thirty participants (15 with NSLBP and 15 healthy controls) were recruited. First while standing on a force plate with sampling frequency of 500 HZ, the load was released unexpectedly on a tray which was on the participant's hands. Then, following experiencing a local vibration on lumbar area for 5 minutes, the similar perturbation, was applied. To investigate the effects of vibration on postural control parameters including mean displacement and mean velocity of center of pressure (COP), in Medio-Lateral and Antro-Posterior directions, paired T-test was used. Results: No difference was found in postural control parameters in sagital and frontal planes before and after vibration between two groups and within each group. Conclusion: Local, medium frequency mechanical vibrations for 5 minutes on low back area had no significant effects on postural control in people with low back pain and did not decrease their postural robustness. All variables were assessed at baseline and after 8 weeks of intervention., Results: The interaction of group and time was not significant for BBD during PFMC (p = .98), Valsalva (p = .28), abdominal curl (p = .34), and secondary variables (p > .05). The main effect of time was significant in both groups for BBD during PFMC, PFM strength, the severity of UI, voiding diary, and QoL (p < .001), with effect size (d) of 0.30, 0.80, 2.05, 1.07, and 1.03 in the control; and 0.49, 0.52, 1.75, 0.66, and 0.88 in the experimental group, respectively. The main effect of the group was not significant for BBD during PFMC (p = .68), Valsalva (p = .22), abdominal curl (p = .53), and secondary variables (p > .05)., Conclusions: Trunk-stabilizing muscle training and PFM exercise are equally effective in the improvement of PFM function, UI symptoms, and QoL in women with SUI.
