Abstract

Point-of-Care Ultrasound (POCUS) is the practice of clinicians conducting and interpreting ultrasound scans right at the patient's bedside. However, the expertise needed to interpret these images is considerable and may not always be present in emergency situations. This reality makes algorithms such as machine learning classifiers extremely valuable to augment human decisions. POCUS devices are becoming available at a reasonable cost in the size of a mobile phone. The challenge of turning POCUS devices into life-saving tools is that interpretation of ultrasound images requires specialist training and experience. Unfortunately, the difficulty to obtain positive training images represents an important obstacle to building efficient and accurate classifiers. Hence, the problem we try to investigate is how to explore strategies to increase accuracy of classifiers trained with scarce data. We hypothesize that training with a few data instances may not suffice for classifiers to generalize causing them to overfit. Abstract

Detecting elevated intracranial pressure (ICP) is crucial in diagnosing and managing various neurological conditions. These fluctuations in pressure are transmitted to the optic nerve sheath (ONS), resulting in changes to its diameter, which can then be detected using ultrasound imaging devices. However, interpreting sonographic images of the ONS can be challenging. In this work, we propose two systems that actively monitor the ONS diameter throughout an ultrasound video and make a final prediction as to whether ICP is elevated. To construct our systems, we leverage subject matter expert (SME) guidance, structuring our processing pipeline according to their collection procedure, while also prioritizing interpretability and computational efficiency. We conduct a number of experiments, demonstrating that our proposed systems are able to outperform various baselines. Abstract

Point-of-Care Ultrasound (POCUS) refers to clinician-performed and interpreted ultrasonography at the patient's bedside. Interpreting these images requires a high level of expertise, which may not be available during emergencies. In this paper, we support POCUS by developing classifiers that can aid medical professionals by diagnosing whether or not a patient has pneumothorax. We decomposed the task into multiple steps, using YOLOv4 to extract relevant regions of the video and a 3D sparse coding model to represent video features. Given the difficulty in acquiring positive training videos, we trained a small-data classifier with a maximum of 15 positive and 32 negative examples. To counteract this limitation, we leveraged subject matter expert (SME) knowledge to limit the hypothesis space, thus reducing the cost of data collection. We present results using two lung ultrasound datasets and demonstrate that our model is capable of achieving performance on par with SMEs in pneumothorax identification. We then developed an iOS application that runs our full system in less than 4 seconds on an iPad Pro, and less than 8 seconds on an iPhone 13 Pro, labeling key regions in the lung sonogram to provide interpretable diagnoses., Comment: IAAI 2023 (7 pages)

Abstract

BackgroundThe Emergency Neurological Life Support (ENLS) is an educational initiative designed to improve the acute management of neurological injuries. However, the applicability of the course in low-income countries in unknown. We evaluated the impact of the course on knowledge, decision-making skills and preparedness to manage neurological emergencies in a resource-limited country.MethodsA prospective cohort study design was implemented for the first ENLS course held in Asia. Knowledge and decision-making skills for neurological emergencies were assessed at baseline, post-course and at 6 months following course completion. To determine perceived knowledge and preparedness, data were collected using surveys administered immediately post-course and 6 months later.ResultsA total of 34 acute care physicians from across Nepal attended the course. Knowledge and decision-making skills significantly improved following the course (p=0.0008). Knowledge and decision-making skills remained significantly improved after 6 months, compared with before the course (p=0.02), with no significant loss of skills immediately following the course to the 6-month follow-up (p=0.16). At 6 months, the willingness to participate in continuing medical education activities remained evident, with 77% (10/13) of participants reporting a change in their clinical practice and decision-making, with the repeated use of ENLS protocols as the main driver of change.ConclusionsUsing the ENLS framework, neurocritical care education can be delivered in low-income countries to improve knowledge uptake, with evidence of knowledge retention up to 6 months.

Abstract

Respiratory muscle ultrasound is used to evaluate the anatomy and function of the respiratory muscle pump. It is a safe, repeatable, accurate, and non-invasive bedside technique that can be successfully applied in different settings, including general intensive care and the emergency department. Mastery of this technique allows the intensivist to rapidly diagnose and assess respiratory muscle dysfunction in critically ill patients and in patients with unexplained dyspnea. Furthermore, it can be used to assess patient-ventilator interaction and weaning failure in critically ill patients. This paper provides an overview of the basic and advanced principles underlying respiratory muscle ultrasound with an emphasis on the diaphragm. We review different ultrasound techniques useful for monitoring of the respiratory muscle pump and possible therapeutic consequences. Ideally, respiratory muscle ultrasound is used in conjunction with other components of critical care ultrasound to obtain a comprehensive evaluation of the critically ill patient. Abstract

Brain ultrasonography can be used to evaluate cerebral anatomy and pathology, as well as cerebral circulation through analysis of blood flow velocities. Transcranial colour-coded duplex sonography is a generally safe, repeatable, non-invasive, bedside technique that has a strong potential in neurocritical care patients in many clinical scenarios, including traumatic brain injury, aneurysmal subarachnoid haemorrhage, hydrocephalus, and the diagnosis of cerebral circulatory arrest. Furthermore, the clinical applications of this technique may extend to different settings, including the general intensive care unit and the emergency department. Its increasing use reflects a growing interest in non-invasive cerebral and systemic assessment. Abstract

Purpose: This work aimed to compare physical impairment in survivors of classic ARDS compared with COVID-19-associated ARDS (CARDS) survivors.Material and methods: This is a prospective observational cohort study on 248 patients with CARDS and compared them with a historical cohort of 48 patients with classic ARDS. Physical performance was evaluated at 6 and 12 months after ICU discharge, using the Medical Research Council Scale (MRCss), 6-min walk test (6MWT), handgrip dynamometry (HGD), and fatigue severity score (FSS). We also assessed activities of daily living (ADLs) using the Barthel index.Results: At 6 months, patients with classic ARDS had lower HGD (estimated difference [ED]: 11.71 kg, p < 0.001; ED 31.9% of predicted value, p < 0.001), 6MWT distance (ED: 89.11 m, p < 0.001; ED 12.96% of predicted value, p = 0.032), and more frequent significant fatigue (OR 0.35, p = 0.046). At 12 months, patients with classic ARDS had lower HGD (ED: 9.08 kg, p = 0.0014; ED 25.9% of predicted value, p < 0.001) and no difference in terms of 6MWT and fatigue. At 12 months, patients with classic ARDS improved their MRCss (ED 2.50, p = 0.006) and HGD (ED: 4.13 kg, p = 0.002; ED 9.45% of predicted value, p = 0.005), while those with CARDS did not. Most patients in both groups regained independence in ADLs at 6 months. COVID-19 diagnosis was a significant independent predictor of better HGD (p < 0.0001) and 6MWT performance (p = 0.001), and lower prevalence of fatigue (p = 0.018).Conclusions: Both classic ARDS and CARDS survivors experienced long-term impairments in physical functioning, confirming that post-intensive care syndrome remains a major legacy of critical illness. Surprisingly, however, persisting disability was more common in survivors of classic ARDS than in CARDS survivors. Abstract

Purpose Sleep is an essential physiologic process that helps to restore normal body homeostasis. Sleep disturbances have been shown to be associated with poor clinical outcomes, such as a greater risk of cardiovascular disease and increasing mortality. Critically ill patients, particularly those receiving mechanical ventilation, may be more susceptible to sleep disruption. Methods and Results Mechanical ventilation is an important factor influencing sleep in critically ill patients as it may have positive or negative effects, depending on patient population, mode, and specific settings. Other causes of sleep disruption include the acute illness itself, the daily routine care, and the effects of medications. Improving sleep in patients admitted to an intensive care unit has the potential to improve both short- and long-term clinical outcomes. In this article we review the specific aspects of sleep in critically ill mechanically ventilated patients, including abnormal sleep patterns and loss of circadian rhythm, as well as the effects of mechanical ventilation and intravenous sedatives on sleep quality and quantity.
