Monitoring chemical substances is of paramount importance in various industries and environmental contexts. In industrial settings, the presence of certain chemical substances may indicate leaks, spills, or malfunctioning equipment, posing immediate threats to both human health and the environment. Moreover, continuous monitoring of chemical compounds is crucial for ensuring compliance with safety regulations and environmental standards. In the context of air quality management, monitoring chemical compounds helps identify sources of pollution, assess the impact on public health, and implement effective pollution control measures. Timely detection and response to chemical compounds also play a vital role in preventing long-term environmental degradation. Overall, monitoring chemical substances is an indispensable component of proactive risk management, environmental stewardship, and the safeguarding of human well-being. The development of automatic and portable devices for online monitoring is an important need in the chemical industry. The smartwatch designed and presented is a home-developed prototype and built from commercially available components. All components of the smartwatch are protected with a plastic casing capable of allowing air to pass to the sensors, the device is also capable of measuring temperature and relative humidity, magnitudes that influence the detection of different odors or volatile compounds. This device is based on a microcontroller that offers low-power performance, integrated Bluetooth low energy at an affordable price, and the measurements of four digital MOX gas sensors, models BME680 SGP40, ENS160 and STC31 through I2C interface. Data are shown on a LCD display and also transmitted via Bluetooth to a smartphone at a sampling period time of 2 s. It is powered using a 3.7 V lithium polymer battery. The smartwatch has a graphical interface to show the user the data provided by the sensors. The designed smartwatch has been validated by measuring different industrial gases like toluene, xylene, and ethylbenzene at low concentrations. Toluene was measured at 6 ppm, xylene at 8 ppm, and ethylbenzene at 10 ppm. Good discrimination between the three different gases was achieved using Principal Component Analysis as multivariable analysis. [ABSTRACT FROM AUTHOR]

In recent years, there has been a growing interest in developing portable and personal devices for measuring air quality and surrounding pollutants, partly due to the need for ventilation in the aftermath of COVID-19 situation. Moreover, the monitoring of hazardous chemical agents is a focus for ensuring compliance with safety standards and is an indispensable component in safeguarding human welfare. Air quality measurement is conducted by public institutions with high precision but costly equipment, which requires constant calibration and maintenance by highly qualified personnel for its proper operation. Such devices, used as reference stations, have a low spatial resolution since, due to their high cost, they are usually located in a few fixed places in the city or region to be studied. However, they also have a low temporal resolution, providing few samples per hour. To overcome these drawbacks and to provide people with personalized and up-to-date air quality information, a personal device (smartwatch) based on MEMS gas sensors has been developed. The methodology followed to validate the performance of the prototype was as follows: firstly, the detection capability was tested by measuring carbon dioxide and methane at different concentrations, resulting in low detection limits; secondly, several experiments were performed to test the discrimination capability against gases such as toluene, xylene, and ethylbenzene. principal component analysis of the data showed good separation and discrimination between the gases measured. [Display omitted] Epigallocatechin-3-gallate (EGCG) exhibits several pharmacological activities with potential benefits for human health, however, it has low oral bioavailability. A promising approach is to transport EGCG in a nanostructured system to protect it until it reaches the site of action and also allow combining chemotherapy with phototherapy to improve its therapeutic efficiency. The aim of this work was to synthesize GNR@mSiO 2 -NH 2 /EGCG and characterize the adsorption process, its antioxidant activity, properties and photothermal stability, for its potential use in chemo-photothermal therapy. The nanosystem presented good encapsulation efficiency (19.2 %) and EGCG loading capacity (6.0 %). The DPPH• free radical scavenging capacity (RSA) and chelating activity of the nanosystem was 60.7 ± 6.9 % and 71.0 ± 6.4 % at an EGCG equivalent concentration of 1 µg/mL and 30 µg/mL, respectively. The core–shell NPs presented a good photothermal transduction efficiency of 17 %. EGCG free, as well as its RSA and chelating activity, remained stable after NIR irradiation (808 nm, 7 W/cm2). The morphology of GNR@mSiO 2 remained intact after being irradiated with NIR, however, ultrasmall gold NPs could be observed, probably a product of photocracking of GNR. In summary, the nanosystem has good antioxidant activity, photothermal stability, and photothermal transduction ability making it potentially useful for chemo-photothermal therapy. Abstract Introduction: Sepsis is one of the leading causes of death in children, and for patients with cancer the risk is even greater. A recent study by our group shows that the main cause of admission to the Paediatric Intensive Care Unit (PICU) of the National Cancer Institute (INC) is septic shock. It is important to better describe this population to look for potential early and effective interventions. Objective: To describe the clinical course of paediatric patients with cancer and with a diagnosis of septic shock who were admitted to the PICU of the INC between October 2011 and December 2013. Materials and methods: Descriptive study of all cancer patients under 18 years of age who were admitted to the PICU with a diagnosis of septic shock. A record was made of demographic data, admission diagnoses, any microorganisms isolated, as well as clinical and treatment outcomes. The observed mortality was compared to the expected mortality using the PIM3 score (Paediatric Index of Mortality 3) Results: The study included a total of 109 patients, with a mean age of 7 years old. The median length of stay in the unit was 7 days (IRQ 10.1). Febrile neutropenia with shock (27%) was the most frequent infectious diagnosis. The large majority (79%) of the patients had haematological malignancies. Gram-negative bacteria were the most common microorganism isolated. The observed mortality was 24.7%, while the expected mortality was 14.3%. Conclusion: Although the patients in this group have similar characteristics to those reported in other series, the observed mortality was greater than the expected mortality. This finding should be explored by additional studies to establish if it is a question of low calibration of the PIM3 score or of particular differences in the diagnosis and management of these patients.

7 páginas Neutropenic Enterocolitis (NE) is a heterogeneous disease with wide range of clinical presentation that can affect any segment of the gastrointestinal tract, with varying severity. It is a complication in patients with high-dose chemotherapy for hematologic and solid tumors. NE typically presents with fever and abdominal pain, but diagnosing is still challenging because symptoms can be varied and elusive. A high index of suspicion must be maintained. The early study with computed tomography directs diagnosis. Most patients benefit from conservative treatment consisting on bowel rest, nasogastric tube placement, parenteral nutritional support, analgesic and broad-spectrum antibiotic coverage. However, the surgical intervention is uncommon and is limited to perforation, peritonitis or gastrointestinal bleeding despite correction of coagulopathy.

Bromeliads are a numerous family of vascular epiphytes, though only one epiphytic species inhabits South-American temperate rainforests: the endemic Fascicularia bicolor. This bromeliad is an important driver of canopy biodiversity, but attributes of its hosts are mostly unknown. Here we report (i) the tree species colonized by F. bicolor, (ii) the relationship between tree size and presence of F. bicolor and (iii) the relation between tree size and the number of mats of F. bicolor inhabiting each colonized tree. We sampled 231 trees in seven forest plots recording their species, diameter, heights, and the number of F. bicolor mats growing on them. The dataset was analyzed with a zero-inflated model to relate host tree attributes with F. bicolor occurrence and abundance in a single statistical approach. The occurrence and abundance of F. bicolor depend on host-species identity and diameter. F. bicolor colonization in slow-growing trees started at smaller DBH than that required for other tree species. Nonetheless, the overall occurrence of F. bicolor relies on large trees above 50 cm DBH for most host species. The number of mats occurring on each colonized tree depends on the interaction between tree height and species suggesting the importance of space available for colonization along the tree-trunk, and differential effects due to species' traits. Currently, large trees and old-growth forests are scarce within the distribution range of F. bicolor, which could seriously affect the long-term conservation of this endemic epiphyte, along with the canopy properties and species associated with it. Conservaron asociaron con mayor riesgo de morir: La acidosis persistente OR:10.14(P:0.00001), Injuria renal aguda OR:7.86(P:0.003) y antibioticoterapia tardía OR10.1(P:0.005). Introduction: Sepsis is an increasingly common worldwide health problem, in which the first few hours of treatment have a dramatic effect on the chances of survival. Results from studies on the early use of antibiotic therapy in adults were extrapolated to determine the effects of such use on children. Objectives: To determine if there is a connection between the early use of antibiotic therapy and the mortality rate amongst patients with severe sepsis and/or septic shock, who attended at UCIP of FCI. Methods: A retrospective analytical study was undertaken, of cases and controls of patients aged between 1 month and 18 years old who presented with severe sepsis and/or septic shock, where the time of initial diagnosis was known, as well as the time the first dose of antibiotic was administered, and the final outcome. Patients who had received previous doses of antibiotic therapy were excluded. The mortality rate was compared with the time the course of antibiotic therapy was first started, calculating the OR and controlling other variables associated with the mortality rate. Results: Of 108 cases evaluated the mortality rate was found to be 38.9%. In fatal cases the time between initial diagnostic and the first antibiotic being administered was 9,06 hours while in those who survived it was 6,1 hours. Empirical studies showed that those who received antibiotic therapy late (>6 hours) had a higher fatality rate by a factor of 3.8 (P:0,002). In addition to the late commencement of antibiotic therapy OR10.1(P:0.005) other factors which increase the risk of death include, persistent acidosis OR:10.14(P:0.00001); or acute renal injury OR:7.86(P:0.003). Conclusion: An important connection exists between the early commencement of antibiotic therapy and the mortality rate, in patients who present with severe sepsis or septic shock. It is recommended that a course of antibiotic therapy be started as soon as possible with these patients.

Germ Cell Tumours are malignancies derived from primordial germ cells. They are classified as seminomatous and non seminomatous, the latter divided based on the cellular differentiation as Embrionary germ cell, Choriocarcinomas, Yolk salk tumors and mature and inmature teratoma. They are the most frequent tumors in men between the ages of 20 and 35 and correspond to 90% of testicular tumors found as non-symptomatic hard painless testicular masses. Their embryologic origin as well as their migration from the primordial gut toward the seminiferous tubules explains the emergence of GCT in the mediastinum and retroperitoneum. In this review there is a description about embryologic, clinical and pathologic characteristics of the disease, as well as staging and treatment.

To evaluate if the Heliox reduces the resistance in the airway in children and adolescents with obstructive bronchial pathology and mechanical ventilation. Materials and Methods: An observational prospective descriptive study was made in children and adolescents with obstructive bronchial pathology and mechanical ventilation with Fi02 ≤ 0,5. Different variables were measured: resistance, pressure peak, mean airway pressure, pressure plateau, tidal volume, AutoPEEP, compliance, PetCO2, dead space ventilation, before heliox and after 30 minutes, 2, 4, 6, 12, 18 and 24 hours, and daily until heliox suspension by extubation or FiO2 > 0,5. Results: This study with partial results included 9 patients finding significant reduction of expiratory resistance to 30 minutes (51.2 versus 32,3; p=0,0008), 2 hours (51.2 versus 33,4; p=0,0019) and 4 hours (51.2 versus 30,7; p=0,0012) as well as of the inspiratory resistance to hour 2 (48.6 versus 36,2; p = 0.013) and hour 4 (48.6 versus 30; p=0,004). PetCO2 no significant decreasing tendency was observed (52.3 versus 34,3: p=0,06). No changes in variables; autoPEEP, pressure peak, mean airway pressure, compliance, dead space, pressure plateau and tidal volume, before or after the Heliox were observed. Conclusion: Mechanical ventilation with Heliox in Children with bronchial obstructive pathology appears to be that it reduce in a significant way the resistance of the airway, with tendency to the reduction of the PetC02. Prospective studies are needed at least observational analytic that corroborate these findings.

- Abstract
In the pediatric intensive care unit (PICU) is essential for the patients with mechanical ventilation to guarantee optima state of sedation and pain control, in order to improve adaptation, decrease stress and anxiety of the critical situation. A common strategy is to combine midazolam and fentanyl, but there is no evidence about the clinical sedative effects with different preparations of these medications, which was the goal of this study. Design: Retrospective analytical study. The objective was to evaluate the differences in the Comfort sedation scale, with 2 medications: midazolam y fentanyl diluted in normal saline and no diluted. Review of 1321 clinical chards from January 2005 to June 2010. 70 patients met inclusion criteria, they were assigned to 2 cohorts, one with de medications no diluted and another one with the medications diluted. Then an analytical study was done through chi square and logistic regressions. Results: Differential statistics were obtain in favor to use midazolam y fentanyl no diluted in normal saline, to obtain optima sedative states evaluated through the Comfort sedation scale, with a p<0,01. No differences were noted in terms of time in the PICU (p=0.67), duration with mechanical ventilation (p=0.15), need to add other sedative drugs (p=0.1) or diagnosis of withdrawal syndrome (p=0.77). Conclusion: A difference was obtain using midazolam y fentanyl no diluted in normal saline in terms of optima sedation in the Comfort scale
