Objective To compare clinical data from the physical examination with arthroscopic findings in rotator cuff rupture. Methods A total of 177 patients submitted to arthroscopic treatment of rotator cuff rupture were selected, and arthroscopic findings were compared with physical examination. Results The impingement tests showed high sensitivity for rotator cuff rupture. Among the strength tests, the most sensitive was the Patte test (85.7%), and the one with the highest positive predictive value (PPV) was the Jobe test (95%). The Drop Sign test showed higher specificity and negative predictive value (NPV) (98.7 and 95.9%, respectively). Patients with a history of trauma were 3.5 times more likely to have a positive Lift Off test. Conclusion The impingement tests had high sensitivity for rotator cuff ruptures. The Jobe test showed similar sensitivity among patients who had partial or total supraspinal injury. For complete lesions of the subscapularis, The Lift Off and Belly Press tests showed high sensitivity and specificity. Resumo Objetivo Comparar dados clínicos do exame físico com os achados artroscópicos na ruptura do manguito rotador. Métodos Foram selecionados 177 pacientes submetidos ao tratamento artroscópico de ruptura do manguito rotador e foram comparados os achados artroscópicos com o exame físico. Resultados Os testes irritativos de impacto apresentaram alta sensibilidade para ruptura do manguito rotador. Dentre os testes de força, o de maior sensibilidade foi o teste de Patte (85,7%) e o de maior valor preditivo positivo (VPP) foi o teste de Jobe (95%). O teste Drop Sign apresentou maior especificidade e valor preditivo negativo (VPN) (98,7 e 95,9%, respectivamente). Pacientes com história de trauma possuíam 3,5 vezes mais chances de apresentarem o teste Lift Off positivo. Conclusão Os testes irritativos tiveram alta sensibilidade para rupturas do manguito rotador. O teste de Jobe apresentou sensibilidade semelhante entre os pacientes que tinham lesão parcial ou total do supraespinhal. Para as lesões completas do subescapular, as testes Lift Off e Belly Press apresentaram alta sensibilidade e especificidade.

15. Arthroscopic Bankart Procedure: Clinical Outcomes with a Minimum Follow-Up of 10 Years

Objective To evaluate the functional outcome of patients submitted to arthroscopic Bankart repair in the long-term. Methods Retrospective evaluation of 41 patients (45 shoulders) operated between 1996 and 2009 followed-up for a mean period of 14.89 years. Functional scores were analyzed by the University of California, Los Angeles (UCLA) and Carter-Rowe scores, physical examination, and analysis of medical records. Results The Carter-Rowe score showed an average improvement of 46.11 points, with a final average of 85.89 points, and the UCLA score showed an average improvement of 31.33 points. Ten patients (22.22%) relapsed, with the number of preoperative dislocations being the most correlated factor. Conclusion It was demonstrated that the number of preoperative dislocations negatively influenced the failure rate. Resumo Objetivo Avaliar o desfecho funcional dos pacientes submetidos ao reparo de Bankart artroscópico no longo prazo. Métodos Avaliação retrospectiva de 41 pacientes (45 ombros) operados entre 1996 e 2009 acompanhados por um período médio de 14,89 anos. Foram feitas análises das pontuações funcionais de University of California, Los Angeles (UCLA) e Carter-Rowe, exame físico e análises de prontuários. Resultados O escore Carter-Rowe apresentou melhora média de 46,11 pontos, com média final de 85,89 pontos, e o UCLA apresentou melhora de 31,33 pontos. Um total de 10 pacientes (22,22%) apresentou recidiva, sendo o número de luxações pré- operatórias o fator mais correlacionado. Conclusão Foi demonstrado que o número de luxações pré-operatórias influenciou negativamente na taxa de falha.

Objective To describe a new presentation of tears and retears of the rotator cuff, which we denominate captured rotator cuff (CRC). We also aim to evaluate it clinically and through images. Methods We assessed retrospectively 16 patients with intraoperative diagnosis of CRC between March 2005 and September 2017; by means of imaging (radiography and magnetic resonance imaging [MRI]) and functional scores (UCLA and Constant & Murley). In images we analyzed the evolution for rotator cuff arthropathy and presence of retears. Functionally, we compared the affected side with the contralateral side and extensive lesions with nonextensive. Results Five (31.25%) patients presented with rotator cuff arthropathy, and 10 (62.5%) with retears. Three (75%) patients with nonextensive lesions had good/excellent UCLA and Constant & Murley scores. In patients with extensive lesions, when the Constant & Murley score was evaluated, 6 (50%) presented good/excellent results, and in the UCLA score, 7 (58.3%). Comparing the affected side (Constant 74.72 points; UCLA 20 points) with the contralateral side (Constant 96.96 points; UCLA 25.63 points), there were worse functional results with statistical significance. Conclusion The diagnosis of CRC is suspected by characteristic findings on MRI and confirmed in arthroscopy. The affected shoulders present worse functional postoperative scores. Resumo Objetivo Descrever uma nova apresentação de ruptura e rerruptura do manguito rotador (MR), a qual denominamos manguito capturado (MC). Objetivamos também avaliá-la clinicamente e por meio de imagens. Métodos Foram avaliados retrospectivamente 16 pacientes com diagnóstico intraoperatório de MC no período de março de 2005 a setembro de 2017; por meio de exames de imagem (radiografia e ressonância magnética [RM]) e escores funcionais (UCLA e Constant & Murley). Nas imagens, analisamos a evolução para artropatia do manguito rotador e presença de rerrupturas. Funcionalmente, comparamos o lado afetado com o contralateral e as lesões extensas com nãoextensas. Resultados Cinco (31,25%) pacientes evoluíram com artropatia do manguito rotador e 10 (62,5%) tiveram rerrupturas. Três (75%) pacientes com lesões não extensas tiveram UCLA e Constant & Murley bons/excelentes. Nos pacientes com lesões extensas, quando avaliado Constant & Murley, 6 (50%) apresentaram resultados bons/excelentes, e no escore UCLA, 7 (58,3%). Comparando o lado acometido (Constant 74,72 pontos; UCLA 20 pontos) com o contralateral (Constant 96,96 pontos; UCLA 25,63 pontos), houve pior resultado funcional com significância estatística. Conclusão O diagnóstico de MC é suspeitado por achados característicos na RM e confirmado na artroscopia. Os ombros acometidos apresentam piores escores funcionais pós-operatórios.

OBJETIVO: Avaliar os resultados funcionais e anatômicos do tratamento cirúrgico via artroscópica nas roturas completas do manguito rotador usando imagens de ultrassom e o índice funcional de Constant e Murley, investigando a correlação entre eles. MÉTODOS: Avaliados 100 pacientes (110 ombros). Seguimento médio de 48,8 ± 33,28 (12 a 141) meses. Média de idade de 60,25 ± 10,09 (36 a 81) anos. Rotura isolada do tendão supra-espinal esteve presente em 85 casos (77%); em associação com o infraespinal, em 20 (18%), e associado ao subescapular, em quatro ombros (4%). A associação de lesões supra-espinal, infraespinal e subescapular foi encontrada em um ombro (1%). De acordo com DeOrio e Cofield, as lesões foram classificadas em pequenas/médias em 85 ombros (77%) e, grandes/extensas em 25 (23%). Avaliação clínica de resultados realizada de acordo com critérios de Constant e Murley. Resultados do ultrassom (US) se referem aos laudos emitidos por diferentes radiologistas. Análise estatística de acordo com os métodos Qui-quadrado, teste exato de Fisher, teste t de Student, correlação de Pearson, Kruscall-Wallis e regressão logística (significância p < 0,05). RESULTADOS: Média da avaliação de Constant de 85,3 ± 10,06 nos ombros normais e 83,96 ± 8,67 nos operados (p = 0,224). Excelentes e bons resultados observados em 74 ombros (67%), satisfatórios e regulares em 32 (29%) e maus em quatro (4%). Avaliação por ultrassonografia evidenciou 38 ombros com re-rotura (35%) e integridade em 71 (65%). Nos 74 (67%) ombros com excelentes/bons resultados, 22 (30%) apresentaram laudo ultrassonográfico de re-rotura (p = 0,294). Nos quatro (4%) ombros com maus resultados, dois (50%) apresentaram laudos de tendões íntegros (p = 0,294). CONCLUSÃO: Não há correlação estatisticamente válida entre o diagnóstico ultrassonográfico e o método clínico de avaliação de resultados de pacientes submetidos ao reparo artroscópico de roturas completas do manguito rotador. Os resultados clínicos nos reparos das roturas completas do manguito rotador por via artroscópica apresentam alto nível de recuperação funcional (Constant 83,96) quando comparado com o ombro contralateral. Os laudos de ultrassom pós-operatórios apresentam alta porcentagem de re-rotura (35%). A força pós-operatória é maior nos pacientes com menos de 60 anos de idade (p = 0,002) e em casos de lesões menores ou iguais a 3cm (p = 0,003). OBJECTIVE: To evaluate the functional and anatomic results of surgical treatment under arthroscopy in cases of rupture of the rotator cuff using ultrasound images and the Constant and Murley functional score to investigate the correlation between them. METHODS: 110 patients (110 shoulders) were evaluated. The medium follow up was 48.8 ± 33.28 months (12 to 141 months). The average age was 60.25 ± 10.09 (36 to 81 years). Isolated rupture of the supraspinal tendon occurred in 85 cases (77%), associated with infraspinatus in 20 cases (18%) and subscapularis in four shoulders (4%). Associated tear lesions of the supraspinatus, infraspinatus and subscapularis was found in one shoulder (1%). The lesions were classified according to the DeOrio and Cofield score as small/medium in 85 shoulders (77%) and large/massive in 25 (23%). The clinical results were assessed according to the Constant and Murley criteria. The results of the ultrasound (US) refer to the reports of several radiologists. Statistical analysis was carried out according to the chi-square methods, Fisher's exact test, Student T test, Pearson, Kruscall-Wallis correlation and logistic regression (significance value p < 0.05). RESULTS: the Constant evaluation average was 85.3 ± 10.06 (46 to 99) in the normal shoulders and 83.96 ± 8.67 (59 to 99) in the operated shoulders (p = 0.224). Excellent and good results were found in 74 shoulders (67%), satisfactory and adequate results in 32 (29%) and poor results in 4 (4%). The ultrasound evaluation showed 38 shoulders with re-rupture (35%), absence of rupture in 71 (65%). Of the 74 (67%) shoulders with excellent/good results, 22 (30%) presented re-rupture in the ultrasound report (p = 0.294). Of the four shoulders (4%) with poor results, two (50%) presented intact tendons (p = 0.294). CONCLUSION: There is no statistically valid correlation between the ultrasound diagnosis and the clinical evaluation of results in patients who underwent arthroscopic repair to treat full tear lesions of the rotator cuff. The clinical results in complete rotator cuff repairs under arthroscopy present a high level of functional recovery (Constant 83.96) when compared with the contralateral shoulder. The postoperative ultrasound image reports present a high percentage of re-rupture (35%); The postoperative strength is higher in patients aged under 60 years (p = 0.002) and in cases of lesions less than or equal to 3 cm. (p = 0.003).

OBJETIVO: Avaliar os resultados de longo prazo e fatores de risco no tratamento cirúrgico por via artroscópica da instabilidade anterior traumática do ombro. MÉTODOS: Estudo retrospectivo de 302 pacientes (314 ombros). Seguimento de 24 a 140 meses, média de 76,5 meses. Avaliação através de exame físico e radiográfico. Classificação de resultados segundo os critérios da UCLA e Carter-Rowe. Análise estatística através de programa Epi Info e testes t de Student e exato de Fisher. RESULTADOS: Observado índice de recidivas em 8,9% dos ombros. Segundo a UCLA, excelentes/bons resultados observados em 97,6% e, regulares/ruins em 2,4% dos ombros. Segundo Rowe, excelentes/bons resultados em 89,2% e regulares/ruins em 10,8% dos ombros. Observada significância (p = 0,013) entre uso de materiais adequados, curva de aprendizado e índice de recidivas. História de convulsões (p = 0,0039) e prática de esportes de contato (p = 0,004) tiveram forte correlação com recidivas. Não se evidenciou correlação entre lesão de Bankart (p = 0,546), lesão de Hill-Sachs (p = 0,62) e recidivas, considerando-se lesões ósseas menores que 25% da glenóide e cabeça umeral, respectivamente. CONCLUSÕES: 1) Há correlação estatisticamente identificada entre o índice de recidiva da instabilidade anterior traumática do ombro e: a) convulsões no pós-operatório; b) prática de esportes de contato; c) presença das lesões de Bankart com Hill-Sachs caracterizadas por 25% ou mais de lesão óssea da glenóide ou da cabeça umeral, respectivamente. 2) Parece haver correlação entre o não reparo da lesão SLAP e o índice de recidiva da instabilidade anterior traumática do ombro. OBJECTIVE: To evaluate the long term results and risk factors in the surgical arthroscopic treatment of the traumatic anterior instability of the shoulder. METHODS: Retrospective study of 302 patients (314 shoulders) 24 to 140 month follow-up, with a mean of 76.5 months. Evaluation performed in clinical and radiological exams. Results classified according to the UCLA and Carter-Rowe. Statistical analysis with the Epi Info program, T-student calculations, and Fisher Exact Test. RESULTS: A rate of 8.9% of recurrence was seen in the shoulders. According to UCLA classification, excellent/good results were seen in 97.6%, and regular/poor results, in 2.4% of the shoulders. There was significance (p = 0.013) among the use of adequate materials, learning curve, and recurrence rate. A history of seizures (p = 0.0039) and the practice of contact sports (p = 0.004) had a marked correlation with recurrence. No correlation was seen between the Bankart lesion (p = 0.546), the Hill-Sachs lesion (p = 0,62) and recurrences, considering bone lesions smaller than 25% of the glenoid and humeral head, respectively. CONCLUSIONS: 1. There is a statistically identified correlation between the rate of recurrence of traumatic anterior stability of the shoulder and: a) seizures in the post-operative period; b) practice of contact sports; c) presence of Bankart lesions with Hill-Sachs characterized by 25% or more of bone lesion of the glenoid or of the humeral head, respectively. 2. There seems to be a correlation between the non-repair of the SLAP lesion and the recurrence rate of traumatic anterior stability of the shoulder.

Objective To evaluate and compare subscapular muscle function among patients undergoing subscapular tenotomy (Group A) and lesser tuberosity osteotomy (Group B), in patients treated with total and partial anatomic shoulder arthroplasty for primary osteoarthrosis. Methods Retrospective study of patients with primary glenohumeral osteoarthrosis surgically undergoing total or partial anatomic shoulder prosthesis implant, evaluated by clinical examination, imaging exams, analogue pain scale and Constant and Murley functional score. A total of 28 patients were evaluated, totalizing a sample of 32 operated shoulders. The minimum follow-up was of 12 months (mean 47.45 months). Results Among patients submitted to subscapularis tendon tenotomy, 10 had an ultrasound with total rupture of its thickness (56%). All of the patients of the group B showed lesser tuberosity healing. There was no difference between groups comparing strength evaluated by Belly press and Bear hug tests as well as clinical outcome, through the Constant and Murley score. Conclusions We did not find differences between Groups A and B evidenciated by comparing strength in the Lift-off test, in the Belly press and Bear hug tests and through he Constant and Murley score. Resumo Objetivo Avaliar e comparar a função do músculo subescapular entre os grupos submetidos a tenotomia do subescapular (Grupo A) e osteotomia do tubérculo menor (Grupo B), em pacientes tratados por artroplastia anatômica total e parcial de ombros apresentando osteoartrose primária. Métodos Estudo retrospectivo de pacientes portadores de osteoartrose primária glenoumeral tratados cirurgicamente com prótese anatômica total ou parcial de ombro, avaliados por exame clínico, exames de imagens, escala visual analógica de dor e escore funcional de Constant e Murley. Foram avaliados 28 pacientes, totalizando uma amostra de 32 ombros operados. O seguimento mínimo foi de 12 meses (média 47,45 meses). Resultados Nos pacientes submetidos à tenotomia do tendão subescapular, dez obtiveram resultado ultrassonográfico com ruptura de sua espessura total (56%). A consolidação ocorreu em todos pacientes submetidos a osteotomia do tubérculo menor. Não houve diferença entre os grupos na comparação do teste Lift Off, da força nos testes Belly Press e Bear hug, e no escore de Constant e Murley. Conclusões Não encontramos diferença entre os grupos A e B na comparação do teste Lift Off, da força nos testes Belly Press e Bear hug, e no escore de Constant e Murley.

In March 2020, the World Health Organization declared a pandemic situation. Exposure to other risk factors for mental disorders caused an increase in their prevalence. Orthopedic surgeons had to deal with an unusual professional and financial situation. The excellent training of new professionals in orthopedics-traumatology also had their activities affected by a change in the teaching pattern. Objectives: To describe the economic and professional impact during the covid-19 pandemic and to assess the prevalence of major depressive episode and generalized anxiety disorder in shoulder and elbow surgeons in Brazil. Methods: This is a cross-sectional study, in which specialist shoulder and elbow orthopedic surgeons, full members of the Brazilian Society of Shoulder and Elbow Surgery (SBCOC) were evaluated. The sampling process adopted was for non-probabilistic convenience. The information was obtained through a self-administered online questionnaire about demographic characteristics, professional and income changes during the pandemic and mental health. Generalized anxiety disorder and major depressive episode were assessed using the Generalized Anxiety Disorder-7 (GAD-7) and Patient Health Questionnaire-9 (PHQ-9) scales, respectively. The survey was applied over 30 days through disclosure by the registered e-mail of SBCOC members. Initially, a univariate analysis was performed describing the occurrence of depression and anxiety, the variables referring to the economic and professional impact of the covid-19 pandemic and the demographic characteristics. Then, the chi-square test of heterogeneity and linear trend was used for bivariate analysis of the prevalence of mental disorders according to economic, professional and demographic characteristics. For all tests, a significance level of 5% was adopted. The analysis will be performed using the STATA v.17 program (StataCorp, College Station, TX, USA). Results: 119 participants were evaluated. The commitment to teaching was stated by 87.7% of the surgeons, there was a reduction of more than 50% in the surgical volume to about 4 in 10 surgeons and a reduction in income above 50% to about 17.3%, with 55 .5% of professionals did not have professional stability. Major depressive episode had a prevalence of 26.7% and generalized anxiety disorder, 20%. The bivariate analysis of the prevalence of mental disorders 11 according to economic, professional and demographic characteristics using the chi-square test showed no association between the variables due to the insufficient sample size. Conclusion: The combination of professional and financial factors may have affected shoulder and elbow surgeons in Brazil, generating high levels of anxiety and depression. Em março de 2020, a Organização Mundial da Saúde declarou situação de pandemia. A exposição a outros fatores de risco para transtornos mentais causaram um aumento em sua prevalência. Os cirurgiões ortopédicos tiveram que lidar com uma situação profissional e financeira inusitada. As formações com excelência dos novos profissionais em ortopedia-traumatologia também tiveram suas atividades afetadas por uma mudança no padrão de ensino. Objetivos: Descrever o impacto econômico e profissional durante a pandemia da covid-19 e avaliar a prevalência de episódio depressivo maior e de transtorno de ansiedade generalizada nos cirurgiões de ombro e cotovelo do Brasil. Métodos: Trata-se de um estudo transversal, no qual foram avaliados os cirurgiões ortopedistas especialistas de ombro e cotovelo, membros titulares da Sociedade Brasileira de Cirurgia de Ombro e Cotovelo (SBCOC). O processo de amostragem adotado foi por conveniência não probabilística. As informações foram obtidas por meio de questionário online autoaplicado sobre características demográficas, alterações profissionais e de rendimentos durante a pandemia e saúde mental. Transtorno de ansiedade generalizada e episódio depressivo maior foram avaliados pelas escalas Generalized Anxiety Disorder-7 (GAD-7) e Patient Health Questionnaire-9 (PHQ-9), respectivamente. A pesquisa foi aplicada no decorrer de 30 dias através de divulgação pelo e-mail cadastrado dos membros da SBCOC. Inicialmente, foi realizada uma análise univariada descrevendo a ocorrência de depressão e de ansiedade, as variáveis referentes ao impacto econômico e profissional da pandemia de covid-19 e as características demográficas. A seguir, foi utilizado teste de qui-quadrado de heterogeneidade e de tendência linear para análise bivariada da prevalência dos transtornos mentais de acordo com as características econômicas, profissionais e demográficas. Para todos os testes, foi adotado um nível de significância de 5%. A análise será realizada no programa STATA v.17 (StataCorp, College Station, TX, EUA). Resultados: Foram avaliados 119 participantes. O comprometimento no ensino foi afirmado por 87,7% dos cirurgiões, houve redução acima de 50% do volume cirúrgico para cerca de 4 em cada 10 cirurgiões e redução da renda acima de 50% para cerca de 17,3%, sendo que 55,5% dos profissionais não apresentavam estabilidade profissional. O episódio depressivo maior apresentou uma prevalência de 26,7% e o transtorno de ansiedade generalizada de 20%. A 9 análise bivariada da prevalência dos transtornos mentais de acordo com as características econômicas, profissionais e demográficas através do teste qui-quadrado não demonstrou associação entre as variáveis decorrente ao número amostral insuficiente. Conclusão: A combinação dos fatores profissionais e financeiros pode ter afetado os cirurgiões de ombro e cotovelo do Brasil, gerando elevados índices de ansiedade e depressão.

42. Translation and Cultural Adaptation to Portuguese of the Long Head of Biceps Tendon Score.

- Abstract
Objective To translate and culturally adapt the Long Head of Biceps Tendon (LHB) score into Brazilian Portuguese. Methods The process involved translations by professionals fluent in the target language, followed by independent back translations. Next, a committee compared the original and translated versions, pretested the final version, and concluded it. Results We translated and adapted the questionnaire according to the proposed methodology. In the first version in Portuguese (VP1) there was divergence regarding the translation of twelve terms. Compared to the original version, the back translation of VP1 presented eight diverging terms. A committee prepared a second version in Portuguese (VP2) and applied it to a pretest group consisting of 30 participants. Finally, we conceived the third version in Portuguese, called LHB-pt. 43. Correlation of Physical Examination with Arthroscopic Findings in the Treatment of Rotator Cuff Tear.

Objective To compare clinical data from the physical examination with arthroscopic findings in rotator cuff rupture. Methods A total of 177 patients submitted to arthroscopic treatment of rotator cuff rupture were selected, and arthroscopic findings were compared with physical examination. Results The impingement tests showed high sensitivity for rotator cuff rupture. Among the strength tests, the most sensitive was the Patte test (85.7%), and the one with the highest positive predictive value (PPV) was the Jobe test (95%). The Drop Sign test showed higher specificity and negative predictive value (NPV) (98.7 and 95.9%, respectively). Patients with a history of trauma were 3.5 times more likely to have a positive Lift Off test. Conclusion The impingement tests had high sensitivity for rotator cuff ruptures. The Jobe test showed similar sensitivity among patients who had partial or total supraspinal injury. 44. Arthroscopic Bankart Procedure: Clinical Outcomes with a Minimum Follow-Up of 10 Years.

Objective To evaluate the functional outcome of patients submitted to arthroscopic Bankart repair in the long-term. Methods Retrospective evaluation of 41 patients (45 shoulders) operated between 1996 and 2009 followed-up for a mean period of 14.89 years. Functional scores were analyzed by the University of California, Los Angeles (UCLA) and Carter-Rowe scores, physical examination, and analysis of medical records. Results The Carter-Rowe score showed an average improvement of 46.11 points, with a final average of 85.89 points, and the UCLA score showed an average improvement of 31.33 points. Ten patients (22.22%) relapsed, with the number of preoperative dislocations being the most correlated factor. 45. Captured Rotator Cuff: A Poor Prognostic Factor in Rotator Cuff Repair.

- Abstract
Objective To describe a new presentation of tears and retears of the rotator cuff, which we denominate captured rotator cuff (CRC). We also aim to evaluate it clinically and through images. Methods We assessed retrospectively 16 patients with intraoperative diagnosis of CRC between March 2005 and September 2017; by means of imaging (radiography and magnetic resonance imaging [MRI]) and functional scores (UCLA and Constant & Murley). In images we analyzed the evolution for rotator cuff arthropathy and presence of retears. Functionally, we compared the affected side with the contralateral side and extensive lesions with nonextensive. Results Five (31.25%) patients presented with rotator cuff arthropathy, and 10 (62.5%) with retears. Three (75%) patients with nonextensive lesions had good/excellent UCLA and Constant & Murley scores. In patients with extensive lesions, when the Constant & Murley score was evaluated, 6 (50%) presented good/excellent results, and in the UCLA score, 7 (58.3%). Comparing the affected side (Constant 74.72 points; UCLA 20 points) with the contralateral side (Constant 96.96 points; UCLA 25.63 points), there were worse functional results with statistical significance. Conclusion The diagnosis of CRC is suspected by characteristic findings on MRI and confirmed in arthroscopy. 46. Shoulder Arthroplasty: Tenotomy of the Subscapularis Tendon versus the Lesser Tuberosity Osteotomy.

- Abstract
Objective To evaluate and compare subscapular muscle function among patients undergoing subscapular tenotomy (Group A) and lesser tuberosity osteotomy (Group B), in patients treated with total and partial anatomic shoulder arthroplasty for primary osteoarthrosis. Methods Retrospective study of patients with primary glenohumeral osteoarthrosis surgically undergoing total or partial anatomic shoulder prosthesis implant, evaluated by clinical examination, imaging exams, analogue pain scale and Constant and Murley functional score. A total of 28 patients were evaluated, totalizing a sample of 32 operated shoulders. The minimum follow-up was of 12 months (mean 47.45 months). Results Among patients submitted to subscapularis tendon tenotomy, 10 had an ultrasound with total rupture of its thickness (56%). All of the patients of the group B showed lesser tuberosity healing. There was no difference between groups comparing strength evaluated by Belly press and Bear hug tests as well as clinical outcome, through the Constant and Murley score. Conclusions We did not find differences between Groups A and B evidenciated by comparing strength in the Lift-off test, in the Belly press and Bear hug tests and through he Constant and Murley score., Competing Interests: Conflito de Interesses Os autores declaram não haver conflito de interesses., (The Author(s). 47. RESULTS FROM ARTHROSCOPIC REPAIR OF ISOLATED TEARS OF THE SUBSCAPULARIS TENDON.

- Abstract
Objective: To evaluate the functional and clinical outcomes and identify prognostic factors in patients undergoing arthroscopic repair of isolated tears of the subscapularis tendon., Methods: Between January 2003 and May 2009, we identified 18 shoulders with isolated tears or deinsertions that were complete or affected at least one third of the subscapularis tendon and underwent arthroscopic repair., Results: Three shoulders (17%) showed lesions in the upper third of the subscapularis; nine shoulders (50%) showed lesions in the upper two thirds; and six shoulders (33%) presented complete tears. In comparing the range of lateral rotation of the injured shoulder between before surgery and the time of the reevaluation, there was no statistical difference (p = 0.091). The LHBT was damaged in 11 shoulders (61%). According to the Constant score validation, we had excellent and good results in 83% of the cases and 17% were reasonable. The reevaluations on three patients showed re-tearing on MRI. Acromioplasty was performed on ten patients and this procedure did not represent statistical differences in the final results (p = 0.57)., Conclusions: There was no statistically significant difference in relation to preoperative lateral rotation between the injured shoulder and the contralateral side. There was no significant loss of lateral rotation after surgery. The LHBT may be normal in deinsertions of the subscapularis tendon. Acromioplasty did not influence the results. The re-tearing rate for arthroscopic repair of the subscapularis tendon was 16.6%.

48. "ROCAMBOLE-LIKE" BICEPS TENODESIS: TECHNIQUE AND RESULTS.

- Abstract
Objective: To present a new technique for bicipital tenodesis and its results: accomplished partially via arthroscopy and grounded in concepts of the normal and pathological anatomy of the tendon of the biceps long head. It is based on the predisposition of this tendon towards becoming attached to the intertubercular sulcus after rupture or tenotomy (auto-tenodesis)., Methods: Evaluations were conducted on 63 patients (63 shoulders), aged from 32 to 77 years (average 55), consisting of 32 females (51%) and 31 males (49%). Thirty-five of the patients (55.6%) were over 60 years of age and 28 patients (44.4%) were under 60 years of age. Eighteen were sports participants (28.6%). Fourteen had injuries associated with the subscapularis (22.2%). The average follow up was 43 months (ranging from 12 to 74 months). The right shoulder accounted for 48 cases (76.2%), of which one was a left-handed individual and 47 were right-handed. The left shoulder accounted for 15 (23%) of the patients, of whom two were left-handed and 13 were right-handed. There were no bilateral occurrences. The statistical analysis were done using SPSS version 18. Pearson's chi-square test and continuity corrections were used to investigate the statistical significance of associations between variables. Associations were taken to be statistically significant when p was less than 0.05., Results: Residual Popeye deformity was perceived by seven patients (11.1%); it was only observed by the examiner in 15 cases (23.8%); and neither the patient nor the examiner observed it in 41 cases (65%). There were no statistically valid influences from age, participation in contact or throwing sports, subscapularis tendon-associated injury or Popeye deformity. Fifty-eight patients (92.06%) were satisfied, two patients were dissatisfied (3.17%) and three patients were indifferent (4.76%)., Conclusion: The technique presented high patient satisfaction rates (92.06%) and residual deformity was perceived by 11.1% of the patients. The appearance did not have any statistically valid correlation with ages over or under 60 years (p = 0.883), sports practice (p = 0.195) or subscapularis-associated injury (p = 0.958).

49. EVALUATION OF ANATOMICAL INTEGRITY USING ULTRASOUND EXAMINATION, AND FUNCTIONAL INTEGRITY USING THE CONSTANT & MURLEY SCORE, OF THE ROTATOR CUFF FOLLOWING ARTHROSCOPIC REPAIR.

- Abstract
Objective: To evaluate the functional and anatomical results from surgical treatment via arthroscopy in cases of complete rupture of the rotator cuff, using ultrasound images and the Constant and Murley functional index to investigate the correlation between them., Methods: 100 patients (110 shoulders) were evaluated. The mean follow-up was 48.8 ± 33.28 months (12 to 141 months). The mean age was 60.25 ± 10.09 (36 to 81 years). Rupture of the supraspinal tendon alone occurred in 85 cases (77%), and in association with the infraspinatus in 20 cases (18%) and subscapularis in four shoulders (4%). An association of supraspinatus, infraspinatus and subscapularis lesions was found in one shoulder (1%). The lesions were classified according to DeOrio and Cofield scores as small/medium in 85 shoulders (77%) and large/extensive in 25 (23%). The clinical results were assessed in accordance with the Constant and Murley criteria. The ultrasound results relate to reports issued by different radiologists. Statistical analysis was carried out using the chi-square test, Fisher's exact test, Student's t test, Pearson's correlation, Kruskal-Wallis correlation and logistic regression (significance: p < 0.05)., Results: The mean Constant evaluation was 85.3 ± 10.06 in the normal shoulders and 83.96 ± 8.67 in the operated shoulders (p = 0.224). Excellent and good results were found in 74 shoulders (67%), satisfactory and moderate results in 32 (29%) and poor results in four (4%). The ultrasound evaluation showed 38 shoulders with re-rupture (35%) and absence of rupture in 71 (65%). Among the 74 shoulders (67%) with excellent/good results, 22 (30%) presented re-rupture in the ultrasound report (p = 0.294). Among the four shoulders (4%) with poor results, two (50%) presented reports of intact tendons (p = 0.294)., Conclusion: There was no statistically valid correlation between the ultrasound diagnosis and the clinical evaluation of results among the patients who underwent arthroscopic repair to treat full tear lesions of the rotator cuff. The clinical results from the complete rotator cuff repairs via arthroscopy presented a high level of functional recovery (Constant 83.96), compared with the contralateral shoulder. The postoperative ultrasound reports presented a high percentage of re-rupture (35%). Postoperative strength was greater among the patients aged under 60 years (p = 0.002) and in cases of lesions less than or equal to 3 cm (p = 0.003).

50. COMPLICATIONS RESULTING FROM THE USE OF METAL ANCHORS IN SHOULDER ARTHROSCOPY.

Unlabelled: To identify the complications concerning the use of metal anchors in shoulder arthroscopic procedures., Methods: 28 shoulders of 28 patients (23 male and 5 female) have been re-operated in the period between December 1997 and August 2007, at Hospital Ortopédico, Belo Horizonte Hospital and Military Police Hospital in Belo Horizonte, MG, as a result of complications such as loose anchors and prominent anchors. The primary surgeries intended to treat 20 anterior traumatic instabilities (71.5%), one posterior instability (3.5%), one slap injury (3.5%), six procedures for treating injuries on the rotator cuff (21.5%). We used the X-ray classification suggested by Samilson and Prieto and Outerbridge arthroscopic classification for assessing patients' degree of arthrosis. All patients were evaluated by the UCLA (University of California at Los Angeles) index criteria., Results: In all patients, arthroscopic reviews were made. In two cases, after anchors removal, clinical signs of instability were seen, leading to the decision of providing open stabilization by Latarjet-Patte technique., Conclusion: the complications with metallic-suture anchors result from inappropriate surgical techniques applied in arthroscopy.
