AKI is also associated with an increased length of stay in intensive care units (ICU) and worse kidney function recovery at hospital discharge. The management of AKI is one of the major challenges for nephrologists and intensivists overall for its early diagnosis. The current KDIGO criteria used to define AKI include the serum creatinine and urinary output that are neither sensitive nor specific markers of kidney function, since they can be altered only after hours from the kidney injury. In order to allow an early AKI detection, in the last years, several studies focused on the identification of new biomarkers. Among all these markers, urinary insulin-like growth factor-binding protein (IGFBP-7) and tissue inhibitor of metalloproteinase (TIMP-2) have been proven as the best-performing and have been proposed as a predictive tool for the AKI detection in the critical settings in order to perform an early diagnosis. Patients undergoing major surgery, cardiac surgery, those with hemodynamic instability or those with sepsis are believed to be the top priority patient populations for the biomarker test. In this view, the urinary [TIMP-2] x [IGFBP-7] becomes an important tool for the early detection of patients at high risk for AKI and its integration with the local ICU experience has to provide a multidisciplinary management of AKI with the institution of a rapid response team in order to assess patients and customize AKI management.Keywords: TIMP-2, IGFBP-7, NGAL, KIM-1, interleukin-18, L-FABP

Abstract

Importance: An intraoperative higher level of positive end-expiratory positive pressure (PEEP) with alveolar recruitment maneuvers improves respiratory function in obese patients undergoing surgery, but the effect on clinical outcomes is uncertain. Objective: To determine whether a higher level of PEEP with alveolar recruitment maneuvers decreases postoperative pulmonary complications in obese patients undergoing surgery compared with a lower level of PEEP. Design, Setting, and Participants: Randomized clinical trial of 2013 adults with body mass indices of 35 or greater and substantial risk for postoperative pulmonary complications who were undergoing noncardiac, nonneurological surgery under general anesthesia. The trial was conducted at 77 sites in 23 countries from July 2014-February 2018; final follow-up: May 2018. Interventions: Patients were randomized to the high level of PEEP group (n = 989), consisting of a PEEP level of 12 cm H2O with alveolar recruitment maneuvers (a stepwise increase of tidal volume and eventually PEEP) or to the low level of PEEP group (n = 987), consisting of a PEEP level of 4 cm H2O. All patients received volume-controlled ventilation with a tidal volume of 7 mL/kg of predicted body weight. Main Outcomes and Measures: The primary outcome was a composite of pulmonary complications within the first 5 postoperative days, including respiratory failure, acute respiratory distress syndrome, bronchospasm, new pulmonary infiltrates, pulmonary infection, aspiration pneumonitis, pleural effusion, atelectasis, cardiopulmonary edema, and pneumothorax. Among the 9 prespecified secondary outcomes, 3 were intraoperative complications, including hypoxemia (oxygen desaturation with Spo2 ≤92% for >1 minute). Results: Among 2013 adults who were randomized, 1976 (98.2%) completed the trial (mean age, 48.8 years; 1381 [69.9%] women; 1778 [90.1%] underwent abdominal operations). In the intention-to-treat analysis, the primary outcome occurred in 211 of 989 patients (21.3%) in the high level of PEEP group compared with 233 of 987 patients (23.6%) in the low level of PEEP group (difference, -2.3% [95% CI, -5.9% to 1.4%]; risk ratio, 0.93 [95% CI, 0.83 to 1.04]; P =.23). Among the 9 prespecified secondary outcomes, 6 were not significantly different between the high and low level of PEEP groups, and 3 were significantly different, including fewer patients with hypoxemia (5.0% in the high level of PEEP group vs 13.6% in the low level of PEEP group; difference, -8.6% [95% CI, -11.1% to 6.1%]; P

Abstract

Introduction: Urinary tract infections (UTI) are the most frequent early and late infectious complications in renal transplant patients. Vesicoureteral reflux (VUR) occurs in more than 86% of renal transplants. It is considered as a possible cause of the onset of complicated urinary tract infections although its actual impact remains uncertain. A therapeutic option in the treatment of VUR, successfully used in particular in pediatric patients, is represented by the injection in the ureter of hyaluronic acid/dextranomer polymers (Dx/HA, Deflux®) performed through the endoscopy procedure. In this study the experience of the Nephrology Unit at the University of Padova is reported. Methods: Six renal transplanted female patients (mean age 45.6 years) who presented a history of recurrent complicated UTIs (more than 3/year), were considered. In these patients a retrograde and voiding cystography documented the presence of VUR at the transplanted kidney. Between February 2016 and July 2018 all these patients underwent endoscopic treatment with Dx/HA. Results: Five patients, after endoscopic treatment with Dx/HA, did not show at the follow up until present any episode of complicated UTIs, confirming the successful of this therapeutic option for the UTI due to VUR in renal transplanted patients. Conclusions: Although based on a limited number of patients, the endoscopic treatment of VUR with hyaluronic acid copolymer Deflux® in kidney transplanted patients is safe, non-invasive, repeatable if necessary, and the results in our patients are favorable. Although considering a period of observation of two years, it might be too short to demonstrate the effectiveness of the treatment in the long-term. However, being able to successfully correct the VUR in kidney transplanted patients represents a very important chance to reduce complicated UTIs in kidney transplanted patients thus increasing the chance for the transplanted organ’s survival.

21. Adaptive locomotion for crossing a moving obstacle.

Abstract

Crossing moving obstacles requires different space-time adjustments compared with stationary obstacles. Our aim was to investigate gait spatial and temporal parameters in the approach and crossing phases of a moving obstacle. We hypothesized that obstacle speed affects gait parameters, which allow us to distinguish locomotor strategies. Ten young adults walked and stepped over an obstacle that crossed their way perpendicularly, under three obstacle conditions: control-stationary obstacle, slow (1.07m/s) and fast speed (1.71m/s) moving obstacles. Gait parameters were different between obstacle conditions, especially on the slow speed. In the fast condition, the participants adopted predictive strategies during the approach and crossing phases. In the slow condition, they used an anticipatory strategy in both phases. We conclude that obstacle speed affects the locomotor behavior and strategies were distinct in the obstacle avoidance phases. [ABSTRACT FROM AUTHOR]

Também em seguimos as premissas da aprendizagem significativa de Ausubel visto que a proposta de ensino deste tem um carater investigativo e uma demonstração investigativa e uma atividade. A que necessita de onhecimentos previos. Os dados coletados para a pesquisa tiveram origem nas atividades realizadas pelos estudantes e do acompanhamento sistematico da aplicação da sequência didatica. Assim sendo, aplicamos uma serie de atividades, tais omo: diarios de bordo, questionarios os-interveção modelo Likert, gravacões das aulas em vídeos, roteiro semi-estruturado (com o uso do simulador) e entrevistas,configurando um estudo quali-quantitativo Os processos de analise dos dados coletados foram: analise das habilidades, via MODELO DE RASH, em questionarios objetivos no formato Likert; analise dos argumentos produzidos pelos estudantes, segundo a tecnica da ANALISE DE CONTEUDO, sugerida pela Bardin e as analises das entrevistas grupo focal, dentro do referencial da TEORIA FUNDAMENTADA, sugerida pelo Gaskell. Os resultados indicaram ganho no parametro das habilidades dos alunos no teste objetivo, com significancia estatistica, e construcão de conceitos, padrões e conexões com o auxílio do simulador, mediante a analise dos materiais produzidos elos alunos. Este trabalho não tem a pretensão de revolucionar o ensino da Física Moderna, mas sim apenas sugerir uma forma alternativa de abordagem desta.

Para avaliar a sua viabilidade, foram determinados os percentuais de remoção de demanda química de oxigênio (DQO) e óleos e graxas totais (TOG), a concentração de sólidos totais, voláteis e fixos, a condutividade elétrica, o pH, a acidez e a alcalinidade total das amostras do efluente oleoso ao longo do tratamento. Estimou-se, também, o custo energético e operacional para o desenvolvimento da técnica. Neste trabalho construiu-se um reator eletrolítico, de bancada, com capacidade de 4 litros. Em seu interior, eletrodos de alumínio em arranjo paralelo foram conectados a uma placa alternadora de polaridade e a uma fonte de corrente contínua. Testes preliminares foram realizados para definir a melhor relação área do eletrodo por volume de efluente quanto à remoção de DQO. A partir destes testes, fixou-se o maior eletrodo (13,0 X 6,5 X 0,2 cm) por ter apresentado a melhor condição (cerca de 90%). Nos resultados foram verificados: aumento do pH; a condutividade elétrica e a concentração de sólidos apresentaram valores próximos durante o tratamento; a média percentual de remoção de TOG foi de 96%, enquanto que a de DQO foi de 81%. No fim da eletrólise, o efluente resultante ficou qualitativamente transparente. A análise estatística para condutividade elétrica e concentração de sólidos totais mostrou que estas dependem significativamente da concentração de eletrólito no meio (NaCl). Para a DQO e o TOG, somente a distância entre os eletrodos foi o fator significativo. Já a variação de pH inicial não apresentou significância em relação as variáveis analisadas. Quanto ao custo energético, a técnica de eletrofloculação apresentou, em média R$ 0,32/m3, um consumo do eletrodo de 0,60 g.e um custo operacional R$ 3,41/m3. Frente aos resultados, constatou-se que é possível utilizar a técnica de eletrofloculação no tratamento de água oleosa devido aos valores técnico-econômico satisfatórios, comparados com a literatura.

Abstract

In this paper we consider rings R with a partial action α of Z on R. We give necessary and sufficient conditions for partial skew polynomial rings and partial skew Laurent polynomial rings to be quasi-duo rings and in this case we describe the Jacobson radical. Moreover, we give some examples to show that our results are not an easy generalization of the global case.
