In the paper, some considerations are made on how the inflection system of the German adjective has been developed in the history of this language. A syntagmatic viewpoint, in which the totality of the phrase structure is taken into account, results as decisive, whereas the traditional paradigmatic perspective is of no use for the acquisition of the rule.

- Abstract
This paper aims to describe Luigi Heilmann’s structural point of view. A philologist and linguist, a specialist in Indology, Semitistics, and Romance dialectology, he subscribed to the Prague School functional perspective and put his structural method in continuity with the comparative-historical method. Heilmann was open to scientific dialogue, also from an interdisciplinary perspective which he based on two principles: a generative humanitas and an interpretative structure.

5. On Eugenio Coseriu‟s Theory of Word Formation

- Abstract
Between 1965 and 1982 Eugenio Coseriu published a series of investigations on word formation processes. In a functional perspective, he developed an innovative proposal, which moved away from traditional models and was not adequately taken into account in lexicological studies, neither in those of his time nor in those of later years. In this contribution, I intend to resume the fundamental aspects of this model, which maintains its validity and originality: in Coseriu's theory of word formation the three levels of the system, the norm and the usage interact strictly, so that both the reasons of the system and the creativity of the speakers are adequately taken into account.

- Abstract
C’è una vulgata sulla “latitanza” della sintassi nello strutturalismo, in particolare europeo, e più ampiamente nei modelli europei di teoria linguistica che merita di essere vagliata nella sua attendibilità. Tra la fine dell’Ottocento e i primi del Novecento, soprattutto in ambiente germanofono, la sintassi venne infatti lungamente scrutata o avvistata come cruciale,a partire da più ambiti disciplinari, spesso fra loro irrelati e ben al di là dell’incipiente strutturalismo: dalla psicologia, dalla logica, dalla filosofia, dalla grammatica, tramille distinguo e un forte impegno di categorizzazione. Se l’obiettivo in Bühler è il monitoraggio del pensiero nel suo nascere, tra i praghesi l’intento è quello di osservare da vicino (modello del codice) le diverse risorse offerte da lingue diverse per l’organizzazione inequivoca e quanto più possibile libera dell’espressione, così come di monitorare (modello della comunicazione) l’esito compiuto dell’avvicendarsi dei pensieri nello scambio dialogico. Si lascia dunque narrare una progressione di indagini coltivate in più sedi, ma certamente spesso ambientate tra Vienna (Miklosich, Brentano, Bühler) e Praga (Marty, Zubaty, Mathesius, Funke, Carnap). Quasi si potrebbe illustrare, anche a proposito di sintassi, la sua semiosi ora propriamente simbolica di relazioni in re, ora più unilateralmente sintomatica, espressiva dell’Erlebnis dei parlanti – l’erlebte Rededi letteraria memoria, ad esempio – ora più intenzionalmente appellativa, quasi indice puntato alla volta dell’interlocutore. Quindi, sì, una semiotica della sintassi, inclusiva – ovviamente – di una semantica della stessa sintassi, che non ne teorizzi né l'estraneità alla lingua né l'autonomia dalle lingue

- Abstract
This contribution aims to describe some semantic components that characterise words used to denote law. For the most part, these characteristics are language-specific and do not appear to be relevant to the understanding of a text. The comparison of structures that are organised in different ways according to the language considered can, however, serve to reconstruct a web of relationships between words that proves crucial for placing law within the constituent practices of a cultural tradition. Words help to organise intellectual experience on the basis of perceived physical reality, so that a language can be considered a 'folk model' of the world.

The paper aims to describe the inflection of German adjectives. A two-stage procedure is pro-posed. In the first, expressions from concrete texts are observed. For this purpose, a corpus-based search is proposed using WebCorp as a software tool that treats Google as a corpus. Then, a description of inflection from a syntagmatic perspective is introduced, describing the principle of monoinflection as an elegant theoretical tool to explain the ability of native speakers to deal with an aspect of grammar that is very difficult for learners to understand., Der Beitrag zielt darauf ab, die Flexion von deutschen Adjektiven zu beschreiben. Es wird ein zweistufiges Verfahren vorgeschlagen. Zuerst werden Ausdrücke aus konkreten Texten beobachtet. Zu diesem Zweck wird eine korpusbasierte Suche vorgeschlagen, bei der WebCorp als Software-Tool verwendet wird, das Google als Korpus behandelt. Anschließend wird eine Be-schreibung der Flexion aus einer syntagmatischen Perspektive eingeführt, wobei das Prinzip der Monoflexion als elegantes theoretisches Instrument beschrieben wird, um die Fähigkeit von Muttersprachlern zu erklären, mit einem Aspekt der Grammatik umzugehen, der für Lernende sehr schwer zu verstehen ist.

A previous Delphi survey from the Rational Use of Analgesics (RUA) project involving Italian palliative care specialists revealed some discrepancies between current guidelines and clinical practice with a lack of consensus on items regarding the use of strong opioids in treating cancer pain. Those results represented the basis for a new Delphi study addressing a better approach to pain treatment in patients with cancer. The study consisted of a two-round multidisciplinary Delphi study. Specialists rated their agreement with a set of 17 statements using a 5-point Likert scale (0 = totally disagree and 4 = totally agree). Consensus on a statement was achieved if the median consensus score (MCS) (expressed as value at which at least 50% of participants agreed) was at least 4 and the interquartile range (IQR) was 3–4. This survey included input from 186 palliative care specialists representing all Italian territory. Consensus was reached on seven statements. More than 70% of participants agreed with the use of low dose of strong opioids in moderate pain treatment and valued transdermal route as an effective option when the oral route is not available. There was strong consensus on the importance of knowing opioid pharmacokinetics for therapy personalization and on identifying immediate-release opioids as key for tailoring therapy to patients’ needs. Limited agreement was reached on items regarding breakthrough pain and the management of opioid-induced bowel dysfunction. These findings may assist clinicians in applying clinical evidence to routine care settings and call for a reappraisal of current pain treatment recommendations with the final aim of optimizing the clinical use of strong opioids in patients with cancer.

The paper aims at describing forms and functions of Polish Yes-No interrogative sentences. two types of structures are given: the former is introduced by the interrogative marker czy which is not related to a specific pragmatic function, although it does not occur with certain kinds of request for a confirmation. the latter type of Yes-No interrogative is structurally very similar to a declarative sentence – only intonation can distinguish them. Particles can be used to express an expectation, but they can also occur in a blending of a question with other pragmatic functions such as requests or offers.

- Abstract
This paper focuses on the history of a language as a field of research which considers concrete linguistic phenomena in a linguistic area characterised by variation. A linguistic phenomenon can be related to the territory and the social environment in which it is used; it can also provide information about the social relationships between individuals, and its structure can vary according to the phonic or graphic substance of which it is made. Language observed on the diachronic axis is related to discursive habits that spread through society and across geographical space. Thus, the history of a language is related to the history of society and culture.

- Abstract
Two fundamental types of interrogative sentences are taken into account here: yes-no interrogatives and wh-interrogatives. A bundle of formal features can be considered to distinguish them from other sentences like indicative sentences and imperative sentences. Interrogatives are used to express questions, i.e. speech acts which are made to receive an answer. But interrogatives can be used also for other purposes, e.g. for polite requests or for rhetorical questions - the latter represent a strategy for stating something. Moreover, a series of interrogative structures can be found that reveal differences in form and function from the fundamental patterns for interrogatives but are frequently used in interactions.

- Abstract
The paper presents some observations concerning the treatment of sentence types (i.e. declarative, interrogative, imperative sentences) in a series of grammars of the Italian language up to the second half of the 19th century. Sentence type is used here as equivalent to sentence mood and does not involve other dimensions of grammatical structure. The works considered are characterized both by the doctrine of the moods as it had been outlined in the Grammaire de Port Royal and by the Classical conception, which over the centuries had been assimilated largely thanks to Boethius. Some grammars offer a series of considerations that show attempts to develop the available models in new directions.

The implicit dimension is a crucial component of communication; it operates alongside explicit components, both verbal and non-verbal in nature. Often the decisive factor in a communication event lies precisely in what is not said, and it is up to the interlocutor to find access to it by means of inferential processes. Here, the focus is on how the implicit dimension is decisive for interpreting questions that arise in everyday conversation.

Questo volume nasce come un omaggio al lavoro di Aldo Grasso che, dagli anni Settanta, attraverso l’attività di ricerca e di didattica presso l’Università Cattolica del Sacro Cuore di Milano, ma anche – dagli anni Novanta – con il quotidiano ‘corpo a corpo’ coi programmi – «i testi» – della televisione italiana, e i suoi protagonisti, ha insegnato ai suoi allievi, e al più vasto pubblico raggiunto dalle pagine del «Corriere della Sera», un approccio critico e contemporaneamente appassionato al medium saldamente centrale nella cultura italiana della seconda metà del Novecento e, ancora, del nuovo secolo. Il volume si focalizza su alcune questioni rilevanti: in che modo è possibile riflettere criticamente e fare ricerca sul ruolo di un mezzo tanto immediato e popolare quanto complesso nelle sue articolazioni e rilevante dal punto di vista culturale e sociale? - Abstract
Languages historically define physical, historical and relational spaces. In the modern age they have been an instrument for the affirmation of states to the point of providing the basis on which to build national identities. In many cases, thanks to the imperial projection, European languages have "colonized" extra-European spaces, leaving important - and controversial - legacies. Political-cultural hegemonies have often corresponded to linguistic hegemonies, such as French in the nineteenth century and English in the twentieth, in connection with the affirmation of an American way of life that still makes its effects felt despite the presence - at the beginning of the 21st century - of a stronger anti-Americanism in many non-Western contexts. In the world scenario increasingly marked by globalization, relations between states are also very much at the level of languages, an important instrument of communication and penetration of soft power for old and new international players. Global multipolarity is a Babel in which it is important to spread or defend one's own language. The geopolitics of languages is thus measured in terms of the impact of a language outside its own areas of rooting, constituting in many cases an important access key available to states to foster new relationships, economic and cultural exchanges, political influence. At the same time, however, languages also impose themselves as strong factors of resistance to globalization, vectors for identity and localist statements that are strengthened in reaction to the homogenized tendencies of many global dynamics. The geopolitics of languages, in short, represents an original point of view to reflect on the contemporary world and on its rapid and contradictory transformations.

- Abstract
The contribution of Kazimierz Ajdukiewicz to categorial grammar is well known to the specialists in theoretical syntax. Nevertheless, the background of his contribution has been little investigated. In this paper, a first attempt is made to reconsider Husserl's treatment of meaning in syntax and its influence on Audikiewicz's conception of "semantic categories". In line with the ancient and medieval logico-linguistic tradition, grammar is taken to represent a highly abstract level of meaning that gives syntax a dynamic character.

- Abstract
Der Beitrag behandelt die Geschichte des Deutschunterrichts in den Gymnasien und Lyzeen der Lombardei zwischen 1818 und 1859, wobei den Mailänder Schulen besondere Aufmerksamkeit gewidmet wird. Während der napoleonischen Herrschaft war in den Gymnasien der Lombardei das Französische als Pflichtfach unterrichtet worden. Beim Wiener Kongress nahm Österreich die Lombardei wieder in Besitz, und Französisch wurde durch Deutsch ersetzt, das aber nur als Wahlfach in Zusatzstunden angeboten wurde. Die Einführung des Schulfachs Deutsch war mit Schwierigkeiten verbunden: In der Region gab es nur wenige gut ausgebildete Deutschlehrer, und die Lehrwerke entsprachen nicht immer den damaligen Standards. Darüber hinaus war man in den kleineren Städten wenig bereit, Deutsch zu lernen. In den folgenden Jahrzehnten verändert sich die Situation kaum, zumal keine Deutschkenntnisse verlangt wurden, um eine Stelle im öffentlichen Dienst zu bekommen. In diesem Rahmen konnte der Deutschunterricht in Gesellschaft und Kultur nur wenig Anklang finden. Erst im Schuljahr 1858/59 wurde Deutsch zum Pflicht-und Prüfungsfach. Es war allerdings auch das letzte Jahr der habsburgischen Ära in der Lombardei.

Il lavoro intende offire un quadro generale dello stato dell’italiano nel mondo e dei fattori legati alla sua diffusione e attrattività a livello internazionale. Dopo aver illustrato la posizione dell’italiano tra le lingue del mondo, vengono presentati i dati relativi alla diffusione dello studio dell’italiano nel mondo nei diversi contesti di insegnamento e le attività di promozione della lingua e della cultura italiana sostenute da istituzioni politiche e culturali nazionali. La considerazione degli elementi che costituiscono la forza di attrazione e di espansione dell’italiano consente poi di avviare alcune riflessioni di carattere linguistico e glottodidattico.

The value of the word nadežda in the Russian cultural horizon is reconstructed through an in-depth analysis of its lexical definition in the dictionary entries of some of the most important monolingual Russian dictionaries. A contrastive analysis of the word here at issue in Italian, French, English and German brings into light a shared lexical definition in Romance, Germanic and Slavic languages, characterised by a shared cultural stance towards the concept of hope.

Si valuta la produzione lessicografica russa degli ultimi venti anni per quanto riguarda i vocabolari russi dell'uso. Le linee fondamentali del suo sviluppo sono: lo sforzo di aggiornamento e digitalizzazione del matrimonio lessicografico esistente, la creazione di nuovi vocabolari che registrino e normalizzino la rapida evoluzione del lessico russo post sovietico, la nascita di vocabolari sperimentali e innovativi come risultato della ricerca linguistica e dell'evoluzione dei corporea digitali. I principali vocabolari russi dell'uso vengono illustrati, analizzati e valutati.

Il volume si pone il duplice obiettivo di illuminare la relazione fra lingua, cultura e ideologia nella lessicografia italiana e russa, e di offrire uno strumento per la consultazione dei vocabolari russi e bilingui, mostrando quanto essa richieda sia competenza tecnica (familiarità con i tecnicismi, con numerose categorie semantiche e grammaticali, sigle ecc.) sia acutezza ermeneutica, basata sulla conoscenza del contesto culturale in cui sono stati prodotti i testi a cui il vocabolario fa riferimento, così da ricostruire in modo inferenziale gli impliciti culturali del lessicografo, presenti nel suo processo interpretativo. Scopo del libro è mostrare che le parole, per il fatto stesso di circoscrivere concetti, interpretano la realtà, veicolano un certo modo di vederla e, dunque, una certa cultura; inoltre, quando le componenti implicite vengono imposte in modo occulto dal lessicografo mediante la compilazione delle voci dizionariali, senza essere condivise dai parlanti, nei vocabolari si attua una manipolazione ideologica del lessico. Ciò viene mostrato dagli autori analizzando le voci lessicografiche di lessemi ideologicamente sensibili, di cui è possibile esporre la semantica in modo strumentale e parziale.

Nell’Unione Europea si contano ca. 90 milioni persone di madrelingua tedesca che costituiscono il gruppo linguistico più forte. Il tedesco è lingua nazionale in Germania, Svizzera e Austria ed esistono minoranze di lingua tedesca in Francia, Lussemburgo, Belgio e in Italia. La Germania si colloca come una delle più importanti economie non solo all’interno dell’Unione Europea ma anche a livello mondiale, e tra l’Italia e la Germania esistono stretti legami culturali e rapporti commerciali consolidati nel corso della storia di entrambi i paesi. Tuttavia in molte facoltà non umanistiche degli atenei italiani si registra ormai da anni una flessione del numero degli studenti che decidono di includere nel loro piano di studi la lingua tedesca. La stessa tendenza si osserva anche nella facoltà di economia dell’Università degli Studi di Milano-Bicocca (di seguito Università di Milano-Bicocca). Questa tendenza dovrebbe però far riflettere, alla luce del fatto che la Lombardia ha particolari legami commerciali ed industriali con la Germania e che anche geograficamente si trova vicino all’aerea di lingua tedesca. Dopo una breve rassegna delle più importanti modifiche ai piani di studi dei differenti corsi di laurea di economia presso l’Università di Milano-Bicocca e dopo l’analisi dei dati relativi agli studenti che decidono di apprendere il tedesco oppure di approfondire le loro conoscenze della stessa lingua precedentemente acquisite, si individuano i fattori che condizionano la scelta degli studenti a favore di una lingua diversa dal tedesco. Infine si formulano delle proposte relative alle misure adottabili per incoraggiare gli studenti ad inserire nel loro piano di studi anche il tedesco.

- Abstract
This paper focuses on the function of conflict prevention performed by keywords and key-expressions within medical consultations. Doctor-patient interaction within medical consultations can be interpreted as a problem-solving discussion, as it is oriented to the solution of a certain health problem. As such, medical consultations are based on an argumentative discussion, in which, roughly speaking, both parties try to reason together in order to find a solution for the patient s health problem. In this sense, a non-mixed dispute (van Eemeren and Grootendorst 2004) often arises, in which doctor and patient discuss a standpoint, normally advanced by the doctor, about the patient s diagnosis or therapy. Problem-solving, however, is not the only possible interaction scheme (Rigotti and Rocci 2006) that can be involved in doctor-patient consultations. Indeed, other more confrontational interaction schemes are possible, which may also tend to real controversies; these cases are often triggered by the asymmetry of competences and knowledge which is typical of the relationship between doctors and patients. Such situations, of course, affect the quality of the consultation, because the issue at stake ceases to be the patient s health, and becomes the relationship between the patient and the doctor (in terms of trust, reliability, etc.). Ultimately, this may have negative effects on the patient s health and, thus, more in general, may contradict the raison d être of the health system. Thus during the medical consultation conflicts need to be avoided both by doctor and patient. The choice of keywords and key-expressions appears to be a crucial step within a strategy of conflict prevention . In fact, thanks to their close link to the relevant endoxa, their ability to trigger inferences, and their centrality within the argumentative structure of persuasive texts, keywords can be very important tools in the creation of emotive arguments and the prevention of conflict. In questo lavoro, partendo dal presupposto che il viaggio è sempre stato considerato un paradigma dell'esperienza umana, autentica e diretta, si elabora un'analisi del viaggio così come è affrontato nella poesia anglosassone "pagana" per far emergere come questa esperienza si collochi nella cultura del tempo. L'analisi è rivolta al poema epico-eroico Beowulf, a due elegie Wanderer, Seafarer, allo Scop poem, Widsið, e ad un incantesimo, il Journey Charm. I testi considerati, pur appartenendo a generi letterari diversi, sono componimenti poetici che attestano la ricchezza e la complessa varietà della poesia anglosassone.L'analisi conferma la stretta connessione esistente tra i concetti di viaggio, rischio, pericolo ed esperienza; in questa dimensione, il viaggiatore si misura con una realtà che lo mette alla prova e nello stesso tempo lo trasforma.

L’analisi delle prime grammatiche delle lingue native, scritte dai missionari allo scopo di standardizzare le innumerevoli varietà linguistiche con cui gli europei vennero a contatto durante il periodo della colonizzazione spagnola e portoghese in sud America, consente di gettare una nuova luce sull’evoluzione del pensiero linguistico all’inizio dell’era moderna. Questo lavoro si concentra in particolare sulla prima grammatica dell’aymara, l’Arte y Grammatica muy copiosa de la lengua aymara, scritta in lingua castigliana alla fine del Cinquecento (1596) dal missionario gesuita italiano Ludovico Bertonio e pubblicata a Roma nel 1603.

Despite existing international guidelines, OIC is largely underdiagnosed and undertreated. ECHO OIC is a European project designed to improve the diagnosis and management of OIC at the primary care level. The next phase of the ECHO OIC project is to review and adapt the proposed European pathway at national level, considering the local patient journey and clinical practice. A multidisciplinary group of 12 Italian experts reviewed and discussed the European path and formulated a seven-step guide for the practical management of OIC that is also easily applicable in primary care: 1. When prescribing long-term opioids, the physician should inform the patient of the possibility of the onset of OIC; 2. At opioid prescription, doctors should also prescribe a treatment for constipation, preferably macrogol or stimulant laxatives; 3. The patient should be evaluated for OIC within the second week of initiating opioid treatment, by clinical history and Rome IV criteria; 4. In the presence of constipation despite laxatives, prescription of a PAMORA (Peripherally Acting Mu Opioid Receptor Antagonist) should be considered; 5. When prescribing a PAMORA, prescribing information should be carefully reviewed, and patients should be accurately instructed for appropriate use; 6. Efficacy and tolerability of the PAMORA should be monitored regularly by Bowel Function Index, considering a cut-off of 30 for the possible step-up of OIC treatment; 7. After 4 weeks of treatment, if the efficacy of PAMORA is deemed inadequate, discontinuation of the PAMORA, addition of an anti-constipation drugs, change of opioid type, or referral to a specialist should be considered. Spreading knowledge about the OIC problem as much as possible to the health community is crucial to obtain not only an early treatment of the condition but also to promote its prevention.

Background: Delays and limitations of palliative care in patients with liver transplantation- ineligible end-stage hepatocellular carcinoma according to Barcelona Clinic Liver Cancer staging system may be explained by different perceptions between hepatologists and palliative care physicians in the absence of shared guidelines., Aim: To assess physicians' attitudes toward palliative care in end-stage hepatocellular carcinoma and to understand what the obstacles are to more effective management and co-shared between palliative care physicians and hepatologists., Design: Members of the Italian Association for the Study of Liver Disease and the Italian Society of Palliative Care were invited to a web-based survey to investigate practical management attitude for patients with liver transplant- ineligible end-stage hepatocellular carcinoma., Participants: Physician members of the of the two associations, representing several hospitals and services in the country., Results: Ninety-seven hepatologists and 70 palliative care physicians completed the survey: >80% regularly follow 1-19 patients; 58% of hepatologists collaborate with palliative care physicians in the management of patients, 55% of palliative care physicians take care of patients without the aid of hepatologists. Management of cirrhosis differed significantly between the two groups in terms of prescription of albumin, esophagogastroduodenoscopy, anti-viral treatment, anticoagulation, indication to paracentesis and management of encephalopathy. Background: In western countries, the increasing life expectancy and the growing number of individuals with advanced chronic conditions have resulted in a greater demand for palliative care. Specifically, Italy has witnessed substantial growth in the palliative care field, marked by the establishment of Palliative Care Networks and an academic fellowship program in 2022. To further enhance this field, it is crucial to conduct high-quality scientific research that produces results applicable in clinical practice., Aim: This article explores challenges and potential solutions in conducting effective palliative care research, considering sample definition, research settings, outcomes, and ethical concerns. While focusing on the Italian context, the presented research framework can be applied to other contexts and regions., Results: Palliative care research is complex and challenging due to its holistic approach, which encompasses various vital dimensions of patients and their families, including physical, emotional, and social needs. The Italian and worldwide experience provides insights into managing these challenges and enhancing the methodological rigor of studies and the practical application of research findings., Conclusions: This article emphasizes the importance of developing protocols tailored to palliative care's unique characteristics, and the necessity of dedicated funding for palliative care research, calling for increased support and recognition. The article advocates for improvement of the quality and relevance of palliative care studies, promoting better patient outcomes and enhanced caregiving.

- Abstract
Cancer and chronic kidney disease prevalence both increase with age. As a consequence, physicians are more frequently encountering older people with cancer who need dialysis, or patients on dialysis diagnosed with cancer. Decisions in this context are particularly complex and multifaceted. Informed decisions about dialysis require a personalised care plan that considers the prognosis and treatment options for each condition while also respecting patient preferences. The concept of prognosis should include quality-of-life considerations, functional status, and burden of care. Close collaboration between oncologists, nephrologists, geriatricians and palliativists is crucial to making optimal treatment decisions, and several tools are available for estimating cancer prognosis, prognosis of renal disease, and general age-related prognosis. Decision regarding the initiation or the termination of dialysis in patients with advanced cancer have also ethical implications. 46. [Dialysis withdrawal and shared end-of-life management between nephrology and palliative care: our three-year experience].

- Abstract
Most dialysis patients with end-stage kidney disease (ESKD) lack access to palliative care services. According to the data of the Dialysis Outcomes and Practice Study (DOPPS), Italy when compared to other countries included in the study, had the lowest rates of dialysis discontinuation. Indeed, there is a growing interest in the implementation of international and national programs for the co-management between nephrology and palliative care in end-of-life decision-making for patients with ESKD. On behalf of this, since 2017, we started in the nephrology outpatient clinic and hemodialysis facilities of the Provincia Autonoma of Trento a shared program between Nephrology and Palliative Care Units to improve the end-of-life quality of care in ESKD patients in conservative and dialytic therapy. Methods: A retrospective analysis, from the 1st of January 2019 to 31st December 2021, of dialysis withdrawal in a cohort of patients undergoing hemodialysis and peritoneal dialysis. Results: Dialysis withdrawal and subsequent death, according to the integrated protocol with the Palliative Care resources, were 24 in 2019, 20 in 2020, and 28 in 2021. The mean age was 75 years in 2019, 78 years in 2020, and in 2021. Most of the patients were male. Dialysis discontinuation was higher in chronic dialysis patients (80% in 2019 and 2020, and 79% in 2021), and considering the annual rates of death of all the dialysis patients, those who died because of dialysis withdrawal were 38% in 2019, 31% in 2020 and 40% in 2021. Survival after dialysis withdrawal was in most of the cases less than 7 days; only few patients lived more than 30 days. Furthermore, the data, in the 3 years considered, showed a reduction of hospitalization and an increase of the rate of death at home. Conclusions: As described in the present study, strategies to expand palliative care by a shared protocol among nephrology and palliative care staff improved the quality of care in the end of life and reduced the hospitalization rates of admission of patients after dialysis withdrawal., (Copyright by Società Italiana di Nefrologia SIN, Rome, Italy.)

47. Development of a Perinatal Palliative Care Model at a Level II Perinatal Center Supported by a Pediatric Palliative Care Network.

Objective: To describe the model build up to take care of fetuses and newborns eligible to perinatal palliative care (PnPC) followed in an Italian II level perinatal center. Methods: Retrospective chart review of all fetuses and newborn infants eligible to PnPC admitted to level II perinatal center within a 4 years period. Results: Forty-five of 848 infants (0.5%) referred to II level NICU were eligible to PnPC. Twenty-seven percentage had fetal diagnosis. Twenty percentage were preterm infants at the limit of viability, 35% were newborns with life limiting or life threatening disease diagnosed in utero or at the postnatal ward, 45% were newborns not responding to intensive care intervention with high health care needs or medical complexity. Fifty-seven percentage of neonates admitted to NICU died before discharge, while 16 (35% of population considered) were discharged home. Median age at death was 4 days after birth, and delivery room death immediately after birth occurred in six patients (13%). - Abstract
The so-called artificial intelligence tools applied to palliative care (machine learning, natural language processing) have great potential to support clinicians in improving decision-making processes and in identifying those who are at high risk of mortality or at greater risk of inappropriate treatment and/or non-positive outcomes. The improvement of prognostic abilities may help to avoid that some choices of patients with serious diseases are taken only in the last days of life, in the face of treatment options not previously discussed in an adequate and shared way. These tools can facilitate some essential aspects in the practice of palliative care, for example the activation of interviews that have as their objective the advance care planning and the definition of treatments consistent with the needs and desires of patients, especially in final stages of life. The development, also in our country, of projects for the application of artificial intelligence in palliative care requires particular attention to the possible organizational repercussions and to some ethical and relational aspects. It will be necessary to reflect on the most appropriate organizational models and on the specialized resources necessary in relation to the foreseeable increase in the number and variability of patients with early identified palliative care needs. These tools must not interfere in fundamental elements of the relationship between patient and doctor, that is the ability to communicate a poor prognosis in an individualized and ethically appropriate way.

49. Quality of post-anesthetic care in a hospital without a Post-Anesthetic Care Unit. A clinical audit.

- Abstract
Aim: The majority of Italian hospitals are not equipped with a Post- Anesthetic Care Unit. The aim of this study is to evaluate whether it is possible to guarantee post-anesthetic care according to current international quality and safety standards in the absence of such a structure., Methods: Our hospital is not equipped with a Post-Anesthetic Care Unit and post-anesthetic assistance is assured by the anesthetist and anesthetic nurse themselves. In order to evaluate the quality of the post-anesthetic care a Recovery Chart was devised and strict discharge criteria defined: Recovery Score (modified Aldrete's score) = or >7; systolic blood pressure within 20% of the preoperative values; nausea/vomiting and shivering absent; pain absent/mild. A retrospective audit was conducted in 2 orthopedic operating rooms from January 10, 2000 to January 31, 2001 in order to evaluate major complications, observance of discharge criteria, postanesthetic care time., Results: Incidence of complications was 2.6%. Observance of discharge criteria was 74%. In 26% of cases (69/261 cases) discharge criteria were not completely respected: 14 cases with unstable vital parameters; 46 cases with pain not under control; 6 cases with nausea/vomiting; 3 cases with shivering. In these cases monitoring and treatment was continued on the ward according to the anesthetist's prescriptions. None of these patients died or suffered major complications because of a quick discharge to the ward. Mean post-anesthetic care time was 40+/-18 minutes (median 35 minutes)., Conclusion: Where the Post- Anaesthetic Care Unit is not available it is virtually impossible to guarantee post-anesthetic care according to current international quality and safety standards, because production pressure can lead the anesthetist to discharge the patient to the ward before he/she is completely stabilized. In these cases the anesthetist must accurately prescribe the necessary postoperative monitoring and treatment (analgesics, antiemetics, fluids, etc.) that must be continued in the surgical ward to guarantee the patient's safety, but it must be underlined that the surgical ward is not the appropriate place to carry on immediate post-anesthetic care.
