Introduction: Migraine is a leading cause of disability and suffering worldwide. However, conventional pharmacological migraine preventive therapies are often challenging and accompanied by adverse effects. Recently, structured odour exposure has shown to successfully increase pain thresholds in patients with chronic back pain. Despite the importance of the olfactory system in migraine, there are no studies investigating the impact of structured odour exposure in patients with migraine. Methods and analysis: This double-blind randomised placebo-controlled trial will be conducted at the Headache Clinic of the University Pain Center at TU Dresden, Germany and aims at investigating the impact of a 12-week structured exposure to odours in women with migraine. Fifty-four women between 18 and 55 years with migraine with aura will be recruited and randomised to training with odours and odourless training. The primary outcomes are mechanical and electrical pain thresholds. Secondary outcomes comprise olfactory threshold and the number of headache days. Other exploratory measurements are headache associated pain intensity, acute analgesic intake, symptoms of anxiety and depression, and quality of life. Additionally, this protocol assesses neuroanatomical and neurofunctional changes associated with the 12-week olfactory training. Data analysis will be executed on the basis of the general linear model considering repeated measurements. Ethics and dissemination: Ethical approvals were obtained from the Ethics Board of the TU Dresden (Protocol No. BO-EK-353082020). Participation will only be possible after written informed consent is provided. Findings will be disseminated through peer-reviewed journals and scientific conferences.

Migräne ist die häufigste neurologische Erkrankung und kann mit einem hohen Grad der Beeinträchtigung einhergehen. Neben nichtmedikamentösen Ansätzen zur Reduktion der Migränefrequenz stehen medikamentöse Migräneprophylaxen zur Verfügung. Zur Indikation und Anwendung liegen evidenzbasierte Leitlinien der Deutschen Migräne- und Kopfschmerzgesellschaft (DMKG) und Deutschen Gesellschaft für Neurologie (DGN), Österreichischen Kopfschmerzgesellschaft (ÖKSG) sowie Schweizer Kopfwehgesellschaft (SKG) vor. Für therapierelevante Fragen wie z. B. der notwendigen und sinnvollen Dauer einer medikamentösen Migräneprophylaxe können aus bisherigen Studiendaten keine Rückschlüsse gezogen werden. Ziel dieser Übersicht ist die Formulierung von Empfehlungen zur Therapiedauer bei wirksamer medikamentöser Migräneprophylaxe, die die aktuelle Datenlage und – bei fehlenden Daten – die Expertenmeinungen berücksichtigen., Migraine is the most common neurological disorder and can be associated with a high degree of disability. In addition to non-pharmacological approaches to reduce migraine frequency, pharmacological migraine preventatives are available. Evidencebased guidelines from the German Migraine and Headache Society (DMKG), and German Society for Neurology (DGN), Austrian Headache Society (ÖKSG), and Swiss Headache Society (SKG) are available for indication and application. For therapyrelevant questions such as the duration of a pharmacological migraine prevention, no conclusions can be drawn from currently available study data. The aim of this review is to present a therapy consensus statement that integrates the current data situation and, where data are lacking, expert opinions. The resulting current recommendations on the duration of therapy for pharmacological migraine prophylaxis are shown here.

Zusammenfassung: Die vorliegende Studie untersucht prospektiv den Zusammenhang von Schlaganfällen zum Auftreten von Kopfschmerzen im Zeitraum von 12 Monaten nach dem Schlaganfall, sowie Charakteristika der Kopfschmerzen und Risikofaktoren für deren Auftreten. In den bisherigen Studien wird die Häufigkeit von Kopfschmerzen nach Schlaganfall zwischen 9,5 % (Pollak et al., 2016) und 54 % (Jose M Ferro et al., 1998) angegeben. Diese Studie soll vor allem den Verlauf der Kopfschmerzprävalenz innerhalb eines Jahres darstellen. Es erfolgte eine persönliche Erstbefragung innerhalb der ersten 72 Stunden nach Symptombeginn im Krankenhaus. Anschließend wurden nach drei, sechs und zwölf Monaten postalische Befragungen durchgeführt. 219 Patienten wurden in die Auswertung eingeschlossen. Davon waren 129 männlich und 90 weiblich. Das Alter lag zwischen 25 und 96 Jahren mit einem Mittelwert von 70,5 Jahren. Bei der initialen Befragung berichteten 35,6 % der befragten Schlaganfallpatienten über Kopfschmerzen. In der Mehrzahl der Fälle war der Schmerz drückend (61,6 %). Am häufigsten war der Schmerz in der frontalen Region lokalisiert (50,6 %). 61,0 % der Kopfschmerzpatienten klagten über einen beidseitigen Kopfschmerz. Ebenso wurde der Kopfschmerz eher als dauerhaft (73,2 %) und weniger als attackenförmig (26,8 %) beschrieben. Die durchschnittliche Intensität wurde mit 5 Punkten auf der numerischen Rangskala (NRS) angegeben, bei Werten zwischen 2 und 10 Punkten. In den postalischen Nachbefragungen sank der Anteil der Kopfschmerzpatienten auf 30,9 % nach drei Monaten, auf 29,2 % nach sechs Monaten und schließlich auf 25,3 % nach einem Jahr. Auch die durchschnittliche Kopfschmerzintensität reduzierte sich auf 3,8 Punkte nach einem Jahr. Nach drei Monaten wurden im Durchschnitt 7,7 Kopfschmerztage pro Monat angegeben. Nach sechs Monaten war diese Anzahl nahezu gleichbleibend bei 7,6 Tagen pro Monat, sank jedoch in der abschließenden Befragung nach einem Jahr auf einen Durchschnittswert, Summary: This study investigates the prevalence of headache within 12 months after stroke prospectively. Additionally, it observes the characteristics of the headache as well as probable risk factors of their occurrence. Recent studies describe the frequency of headache after stroke between 9.5 % (Pollak et al., 2016) and 54% (Ferro et al., 1998). The aim of this study is to investigate the headache prevalence within one year after cerebral infarction. The study started with a personal interview of patients within the first 72 hours after symptoms startet in the hospital. Following this, there were three postal surveys: three months, six months and one year after acute event. 219 patients were included in the analysis. Of whom 129 were male and 90 female. The age ranged from 25 to 96 years, with an average of 70.5 years. In the first interview 35.6 % of stroke patients suffered from headaches. In most cases it was a pressing pain (61.6 %). Most common in the frontal cranial region (50.6 %). 61.0 % of participants complained about a both-hemispheric headache. The headache was more reported as permanent (73.2 %) than in form of attacks (26.8 %). The middle intensity was calculated as 5 according to numeric rating scale (NRS), with a range from 2 to 10 points. In the postal surveys the occurrence of headaches decreased from 30.9 % after three months to 29.2 % after six months to finally 25.3 % after one year. Also the average of headache intensity fell to 3.8 points after one year. After three months, patients who suffered from headaches complained of an average of 7.7 days of headaches per month. Three months later, this number was nearly the same and one year after the stroke the number decreased to an averaging 4.7 days per month. Within the observation over the months more people described their headaches as attacks as compared to the first interview in which more permanent headaches were reported.
