Εισαγωγή/Σκοπός: Η πολλαπλή σκλήρυνση είναι η συχνότερη απομυελινωτική νόσος του κεντρικού νευρικού συστήματος. Ασθενείς που πάσχουν από πολλαπλή σκλήρυνση συνήθως εκδηλώνουν συμπτώματα δυσλειτουργίας του κατώτερου ουροποιητικού. Σκοπός της μελέτης είναι να μελετηθεί και να συγκριθεί η αποτελεσματικότητα και η ασφάλεια της θεραπείας των ασθενών με πολλαπλή σκλήρυνση και συμπτώματα δυσλειτουργίας του κατώτερου ουροποιητικού με την χρήση είτε β3 αγωνιστή (mirabegron) είτε αντιχολινεργικών. Το πρωταρχικό καταληκτικό σημείο της μελέτης είναι η βελτίωση της συμπτωματολογίας από το κατώτερο ουροποιητικό σύστημα. Υλικό και μέθοδος: Πρόκειται για μία τυχαιοποιημένη, πολυκεντρική, μονά τυφλή μελέτη σύγκρισης στην οποία περιλαμβάνονται 91 ασθενείς με πολλαπλή σκλήρυνση και συμπτώματα από το κατώτερο ουροποιητικό. Στην 1η επίσκεψη οι ασθενείς εξετάστηκαν λεπτομερώς κλινικά, υποβλήθηκαν σε γενική/καλλιέργεια ούρων και υπερηχογράφημα ΝΟΚ και συμπλήρωσαν ημερολόγια ούρησης και συγκεκριμένα έγκυρα και αξιόπιστα ως προς την ελληνική τους μετάφραση ερωτηματολόγια (το MusiQoL και το NBSS). Κατά την 2η επίσκεψη στους ασθενείς χορηγήθηκε είτε β3 αγωνιστής (mirabegron) είτε αντιχολινεργικό. Η θεραπεία των ασθενών γινόταν πάντα παράλληλα με την θεραπεία για την πολλαπλή σκλήρυνση. Μετά την πάροδο τριών μηνών από την 1η επίσκεψη έγινε επαναξιολόγηση των ασθενών με τον ίδιο κλινικό και απεικονιστικό έλεγχο που είχε διενεργηθεί κατά την 1η επίσκεψη. Αποτελέσματα: Έγινε σύγκριση διαφόρων κλινικών και απεικονιστικών παραμέτρων, ανάμεσα στις 2 ομάδες ασθενών στην 1η επίσκεψη και μετά τον 3ο μήνα από την έναρξη της θεραπείας. Δεν ανεδείχθη καμία στατιστικά σημαντική διαφορά ανάμεσα στην ομάδα του mirabegron και την ομάδα των αντιχολινεργικών ως προς τη βελτίωση των συμπτωμάτων από το κατώτερο ουροποιητικό σύστημα. Και στις 2 ομάδες ασθενών καταγράφηκε ωστόσο σημαντική βελτίωση μετά την θεραπεία εν συγκρίσει με την 1η επίσκεψη στις περισσότερες παραμέτρους που μελετήθηκαν. Πραγματοποιήθηκε στατιστική ανάλυση με την μέθοδο του t-test για εξαρτημένα δείγματα για τη σύγκριση των μεταβλητών κάθε ομάδας ασθενών στην αρχή και τρεις μήνες μετά τη θεραπεία. Στη συνέχεια εφαρμόστηκε η μέθοδος του t-test για ανεξάρτητα δείγματα για τη σύγκριση των στατιστικά σημαντικών μεταβλητών μεταξύ των δύο ομάδων ασθενών. Οι ανεπιθύμητες ενέργειες της θεραπείας καταγράφηκαν και στις 2 ομάδες ασθενών. Κανένας ασθενής δεν σταμάτησε την θεραπεία λόγω ανεπιθύμητων ενεργειών. Συμπεράσματα: Οι ασθενείς με πολλαπλή σκλήρυνση που έλαβαν είτε mirabegron είτε αντιχολινεργικό παράγοντα παρουσίασαν βελτίωση στην συμπτωματολογία από το κατώτερο ουροποιητικό στις περισσότερες από τις παραμέτρους που μελετήθηκαν. Παρόλα αυτά δεν ανεδείχθη στατιστικά σημαντική διαφορά ανάμεσα στις 2 ομάδες ασθενών στους 3 μήνες από την έναρξη της θεραπείας ως προς την αποτελεσματικότητα των διαφορετικών φαρμάκων που χρησιμοποιήθηκαν. Introduction and objective: Multiple Sclerosis is the most frequent autoimmune demyelinating disease of the Central Nervous System. Patients usually present with lower urinary tract dysfunction symptoms. Objective of this study is to evaluate and compare the efficacy and safety of treating multiple sclerosis patients with lower urinary tract dysfunction symptoms with either a b3 agonist (mirabegron) or anticholinergics. The study’s primary outcome is the lower urinary tract symptom improvement. Material and Methods: This is a randomized, multi-center, single-blinded, comparative study including 91 multiple sclerosis patients with lower urinary tract dysfunction. At baseline patients underwent thorough clinical examination, urine cultivation and abdominal ultrasound and completed urination diaries and specific, validated questionnaires (NBSS, MusiQoL). At second visit patients were administered either mirabegron or anticholinergics. Treatment was always carried out alongside with multiple sclerosis treatment. Reevaluation was performed 3 months after first visit. Patients underwent the same clinical and imaging tests that were carried out at first visit. Results: We compared several clinical and imaging parameters between the two groups at first visit and month 3 after treatment. Νo statistical difference was noted between the mirabegron group and the anticholinergic group in terms of lower urinary tract symptom improvement. On the other hand, in both groups, improvement from baseline regarding lower urinary tract symptoms was recorded. Statistical analysis was performed using the paired and unpaired t-test method. Side effects were recorded in both groups. No patient discontinued either medication due to side-effects. Conclusions: Multiple sclerosis patients receiving either mirabegron or anticholinergic therapy for lower urinary tract symptoms showed improvement in most of the tested parameters. Nevertheless, no statistical difference was noted between the two cohorts at 3 months in terms of drug efficacy.

Introduction and objective: Even though the only established risk factors for prostate cancer (PCa) are age, ethnic origin and family history, there are data suggesting that environmental factors, such as the presence of metabolic syndrome (MetS), may also play a role in the etiology of the disease. The aim of this study is to correlate MetS with PCa diagnosis and Gleason score (GS) in patients undergoing transrectal ultrasound guided prostate biopsy. Materials and methods: This is a prospective, single-center study including 378 patients who underwent transrectal ultrasound guided prostate biopsy in our department during the years from 2018 to 2019. Patients were divided into two groups according to the presence of PCa. Group A included 197 patients diagnosed with PCa while Group B consisted of 181 patients without PCa in their biopsy result. Multiple variables such as the presence of MetS and its components were evaluated in correlation to the presence of PCa and PCa characteristics. Statistical analysis was performed using the IBM SPSS Statistics v.23 program. Results: Mean PSA value was 8.7 ng/dl in the PCa group and 7.1 ng/dl in the non PCa group, respectively. MetS was diagnosed in 108 patients (54.8%) with PCa and 80 patients (44.2%) without PCa and the difference was statistically significant. Hypertriglyceridemia was the MetS component with statistically higher frequency in PCa patients. Furthermore, the prevalence of MetS was higher in higher Gleason score PCa (GS >= 4+3) patients vs lower Gleason score PCa (GS = 4+3). Conclusions: Patients suffering from MetS who undergo prostate biopsy present with higher rates of PCa diagnosis and higher GS in comparison with patients with a normal metabolic profile.

Bladder Cancer (BC) is11th most common malignancy. In terms of pathology, the vast majority of patients suffer from transitional cell carcinoma. Apart from this common type of BC, there are many morphological subtypes with different oncological characteristics. Plasmacytoid BC is a well-recognized subtype of BC with great diagnostic importance as it usually presents in locally advanced or even metastatic stage.The objective of this study was to evaluate our experience in diagnosing and treating patients with this rare BC subtype.A retrospective analysis of 5 patients diagnosed with plasmacytoid BC in our department during they ears 2014-2016 was performed. Transurethral resection of the tumors was performed in all patients and pathology diagnosis of plasmacytoid variant was based on several morphologic and immunohistochemical parameters. Staging included abdominal and thoracic CT.3 of 5 patients were diagnosed with metastatic disease. These patients were referred to the oncology department. 2 patients presented with non-metastatic BC after initial staging and thus a radical cystectomy was performed. Follow up of all patients was carried out and their survival was recorded.We concluded that despite the fact that the plasmacytoid variant of BC is rare, it is important to take into account the pathologic and clinical features of this tumor in order to manage the optimal treatment of this poor prognosis cancer.INTRODUCCIÓN: El cáncer de vejiga es el 11o cáncer mas común. Del punto de vista de la patología, la mayoría de pacientes tienen carcinoma urotelial de vejiga.A parte de la forma mas común, el cáncer de vejiga plasmocitoide es un subtipo ampliamente reconocido de cáncer de vejiga con una gran importancia diagnóstica ya que habitualmente se presenta como localmente avanzadoo en estadio metastático.OBJETIVO: El objetivo de este estudio fue evaluar nuestra experiencia en el diagnóstico y tratamiento en pacientes con esta entidad.MÉTODOS: Análisis retrospectivo de 5 pacientes diagnosticados con cáncer de vejiga plasmocitoide en nuestro departamento entre 2014 y 2016. Se realizó una resección transuretral de vejiga en todos los pacientes y un análisis histopatológico que demostró la variante plasmocitoide del punto de vista morfológico e immunohistoquímico. El estadiaje incluyó TAC toracoabdominopélvico.RESULTADOS: 3 de los 5 pacientes fueron diagnosticado sen estadio metastático. Estos pacientes pasaron a cargo del departamento de oncología medica. 2 pacientes se diagnosticaron en estadio localizado por lo que se realizó una cistectomía radical. Se realizó el seguimiento y se determinó la supervivencia de estos pacientes. CONCLUSIONES: Se concluye que todo y que el diagnóstico de cáncer de vejiga plasmocitoide es raro; es importante reconocer esta variedad histológica para poderdar un manejo optimo a esta variedad con muy mal pronostico.
