Abstract

The Integrated Services Project for Clients with Challenging Behaviour (ISP) aims to decrease the adverse impact of challenging behaviour on clients, the community, and the service system. The Project does this by developing intervention and support plans that better respond to the individual needs of clients with challenging behaviour, as well as improving their service access, housing, health and social conditions. The ISP is delivered through the NSW Department of Ageing, Disability and Home Care in cooperation with the NSW Department of Health and Housing NSW. The present document provides a preliminary cost effectiveness analysis based on initial client outcomes. The report begins with a brief overview of the ISP before describing the approach taken to the cost effectiveness analysis and the broad characteristics of the 38 ISP clients who were in ISP 2-32 months, from October 2005 to March 2008. The report analyses client outcomes in the following key areas: changes in challenging behaviour; client perceptions of personal wellbeing and health; living skills; social and economic participation; and changes in the use of community health, hospital, and criminal justice services., SPRC Report Series 14/09

Abstract

This report, by the Social Policy Research Centre (SPRC), examined the effectiveness of individual funding of disability support and aimed to inform policy to improve the provision of disability support. The study was commissioned by the Australian Government Department of Families, Housing, Community Services and Indigenous Affairs (FaHCSIA).The research methodology included a literature review; secondary data analysis of the Commonwealth State Territory Disability Agreement (CSTDA) National Minimum Data Set (NMDS); interviews with policy officials from all states and territories; case studies with 10 disability service providers, involving interviews with people with disabilities and staff; and questionnaires for people with disabilities and service providers.

Abstract

Forty-nine obstetricians from three European countries were asked when they would advise delivery for a preterm fetus failing to thrive in utero, given various gestational ages, and a range of either umbilical artery Doppler flow velocity waveforms or CTG variability measures. Their responses indicated a wide area of disagreement about the correct timing of delivery, and a willingness to randomize patients to clinical trials of management. The area of uncertainty corresponded to the gestational age and Doppler bands at which participants have been entered to the pilot phase of a randomised trial of timed delivery, the Growth Restriction Intervention Trial (GRIT).

46. A comparison of the long gamma nail with the sliding hip screw for the treatment of AO/OTA 31-A2 fractures of the proximal part of the femur: a prospective randomized trial.

Abstract

Background: Controversy exists with regard to whether to treat AO/OTA 31-A2 fractures of the proximal part of the femur with an intramedullary device or an extramedullary device. A prospective, randomized, controlled trial was performed to compare the outcome of treatment of these unstable fractures of the proximal part of the femur with either a sliding hip screw or a long gamma nail.Methods: Two hundred and ten patients presenting with an AO/OTA 31-A2 fracture of the proximal part of the femur were randomized, at the time of admission, to fixation with use of either a long gamma nail or a sliding hip screw. The primary outcome measure was reoperation within the first postoperative year. Secondary measures included mortality, length of hospital stay, transfusion rate, change in mobility and residence, and quality of life as measured with the EuroQol 5D outcome score.Results: There was no significant difference between the reoperation rates for the two groups. In total, five patients (three from the long-gamma-nail group and two from the sliding-hip-screw group) underwent revision surgery because of cut-out. Tip-apex distance was found to correlate with the implant cut-out rate. There was no significant difference between the two groups in terms of the EuroQol 5D outcome scores, the mortality rates after correction for the mini-mental score, or any of the secondary outcome measures.Conclusions: When compared with the long gamma nail, the sliding hip screw should remain the gold standard for the treatment of AO/OTA 31-A2 fractures of the proximal part of the femur because it is associated with similar outcomes with less expense.
