Abstract

The problem of providing surgical navigation using image overlays on the operative scene can be split into four main tasks--calibration of the optical system; registration of preoperative images to the patient; system and patient tracking, and display using a suitable visualization scheme. To achieve a convincing result in the magnified microscope view a very high alignment accuracy is required. We have simulated an entire image overlay system to establish the most significant sources of error and improved each of the stages involved. The microscope calibration process has been automated. We have introduced bone-implanted markers for registration and incorporated a locking acrylic dental stent (LADS) for patient tracking. The LADS can also provide a less-invasive registration device with mean target error of 0.7 mm in volunteer experiments. These improvements have significantly increased the alignment accuracy of our overlays. Phantom accuracy is 0.3-0.5 mm and clinical overlay errors were 0.5-1.0 mm on the bone fiducials and 0.5-4 mm on target structures. We have improved the graphical representation of the stereo overlays. The resulting system provides three-dimensional surgical navigation for microscope-assisted guided interventions (MAGI). Abstract

Optical coherence tomography is a diagnostic imaging technique allowing two dimensional tomographic imaging of tissue architecture. This is a review article on the use of optical coherence tomography in Otolaryngology including original images from human laryngeal tissue and temporal bones (cochlea) in our laboratory. Tissue specimens from normal larynges were imaged with an 850 nm OCT system. Our results showed good correlation between OCT image s and the corresponding haematoxylin-eosin stained histology sections in the normal larynx. Human temporal bones were also imaged using an 1300 nm OCT system. Limited morphological details were obtained due to the high scattering properties of the bony labyrinth.

30. AcouStick: A Tracked A-Mode Ultrasonography System for Registration in Image-Guided Surgery.

Abstract

In this paper, we describe a system for noninvasively determining bone surface points using an optically tracked A-mode ultrasound transducer. We develop and validate a calibration method; acquire cranial surface points for a skull phantom, three volunteers, and one patient; and register these points to surfaces extracted from CT images of the phantom and patient. Our results suggest that the bone surface point localization error of this system is less than 0.5 mm. The target registration error (TRE) of the cranial surface-based registration for the skull phantom was computed by using as a reference gold standard the point-based registration obtained with eight bone-implanted markers. The mean TRE for a 150-surface-point registration is 1.0 mm, and ranges between 1.0 and 1.7 mm for six 25-surface-point registrations. Our preliminary results suggest that accurate, noninvasive, image-to-physical registration of head images may be possible using an A-mode ultrasound-based system. [ABSTRACT FROM AUTHOR]

Abstract

• The Cavitron ultrasonic surgical aspirator system was used to clear superficial tissue from the greater auricular nerve in four human subjects. Following stimulation, the nerves were removed together with an untreated section and processed for examination by light microscopy. Significant damage had been sustained by these nerves, which was not apparent in the control sections. Disruption of the perineurium, intraneural hemorrhage, endoneural edema, and degenerative changes of the myelinated nerve fibers were seen. Damage to neural tissue was evident 0.5 to 1.5 mm in advance of the point of contact with the cutting tip.(Arch Otolaryngol Head Neck Surg 1987; 113:530-532)

40. Bilateral squamous cell carcinoma of the temporal bones.

Abstract

We report a case of bilateral squamous cell carcinoma of the temporal bones in a 66-year-old woman who underwent a left subtotal petrosectomy and a right lateral temporal bone resection. Hearing rehabilitation was successfully achieved by fitting a bone-anchored hearing aid (BAHA) on the right side. We feel that in cases where the extent of the tumor permits a lateral temporal bone resection in at least one side, fitting of a BAHA can achieve a good outcome without resorting to tympanoplasty techniques that may compromise the medial end of the surgical resection.

41. Hydroxyurea treatment of meningiomas: a pilot study.

Abstract

Objectives: To present a pilot study of hydroxyurea chemotherapy in the management of surgically difficult meningiomas., Design: Prospective case series., Setting: Tertiary referral center., Participants: Six patients were enrolled: five had symptomatic recurrent or residual WHO Grade I meningiomas and one had an unoperated anterior clinoidal meningioma., Main Outcome Measures: MRI volumetry before and after treatment, hematological state, progression to surgery., Results: Hydroxyurea was administered for 1 year, starting at a dose of 15 mg/kg/day. No tumors reduced in size on treatment and one continued to grow rapidly. Three remained stable. Full volumetry was unavailable on two patients. One patient was withdrawn from the study because of myelosuppression and underwent further surgery. Three experienced significant drug-related side effects. Two patients underwent further surgery., Conclusions: From this small pilot study we conclude that although hydroxyurea may be associated with disease stabilization in some patients, it did not reduce tumor mass and it caused significant side effects in some of our subjects.

42. An Osteoma of the Middle Ear Presenting with the Tullio Phenomenon.

Abstract

The Tullio phenomenon is an extremely rare condition in which loud noise induces a brisk vestibular response. Osteomas of the middle ear cleft are also uncommon. We report a patient with an osteoma of the middle ear cleft that became symptomatic with progressive facial palsy and mixed hearing loss. The onset was heralded by the Tullio phenomenon, which she had experienced for 10 years. The differential diagnosis of the Tullio phenomenon and the management of middle ear osteomas are discussed.
