Abstract

Objective: \ud To investigate quality of life (QoL) and association with surgical complexity and disease burden after surgical resection for advanced ovarian cancer in centres with variation in surgical approach\ud \ud Design: \ud Prospective multicentre observational study\ud \ud Setting: \ud United Kingdom, Kolkata, India, and Melbourne, Australia gynaecological cancer surgery centres.\ud \ud Participants: \ud Patients undergoing surgical resection for late stage ovarian cancer.\ud \ud Exposure Low, intermediate or high Surgical Complexity Score (SCS) surgery\ud \ud Main outcomes and measures: \ud Primary: EORTC-QLQ-C30 Global score change. Secondary: EORTC OV28, progression free survival.\ud \ud Results: \ud Patients’ pre-operative disease burden and SCS varied between centres, confirming differences in surgical ethos. QoL response rates were 90% up to 18 months. Mean change from the pre-surgical baseline in the EORTC QLQ-C30 was 3.4 (SD 1.8, n=88) in the low, 4.0 (SD 2.1, n=55) in the intermediate and 4.3 (SD 2.1, n=52) in the high SCS group after 6 weeks (p=0.048) and 4.3 (SD 2.1, n=51), 5.1 (SD 2.2, n=41) and 5.1 (SD 2.2, n=35) respectively after 12 months (p=0.133). In a repeated measures model, there were no clinically or statistically meaningful differences in EORTC QLQ-C30 global scores between the three SCS groups, p= 0.840 but there was a small statistically significant improvement in all groups over time (p

Abstract

BACKGROUND: Preclinical data show that belinostat (Bel) is synergistic with carboplatin and paclitaxel in ovarian cancer. To further evaluate the clinical activity of belinostat, carboplatin, and paclitaxel (BelCaP), a phase 1b/2 study was performed, with an exploratory phase 2 expansion planned specifically for women with recurrent epithelial ovarian cancer (EOC).METHODS: Thirty-five women were treated on the phase 2 expansion cohort. BelCap was given as follows: belinostat, 1000 mg/m² daily for 5 days with carboplatin, AUC 5; and paclitaxel, 175 mg/m² given on day 3 of a 21-day cycle. The primary end point was overall response rate (ORR), using a Simon 2 stage design.RESULTS: The median age was 60 years (range, 39-80 years), and patients had received a median of 3 prior regimens (range, 1-4). Fifty-four percent had received more than two prior platinum-based combinations, sixteen patients (46%) had primary platinum-resistant disease, whereas 19 patients (54%) recurred within 6 months of their most recent platinum treatment. The median number of cycles of BelCaP administered was 6 (range, 1-23). Three patients had a complete response, and 12 had a partial response, for an ORR of 43% (95% confidence interval, 26%-61%). When stratified by primary platinum status, the ORR was 44% among resistant patients and 63% among sensitive patients. The most common drug-related adverse events related to BelCaP were nausea (83%), fatigue (74%), vomiting (63%), alopecia (57%), and diarrhea (37%). With a median follow-up of 4 months (range, 0-23.3 months), 6-month progression-free survival is 48% (95% confidence interval, 31%-66%). Median overall survival was not reached during study follow-up.CONCLUSIONS: Belinostat, carboplatin, and paclitaxel combined was reasonably well tolerated and demonstrated clinical benefit in heavily-pretreated patients with EOC. Abstract

The 4th Ovarian Cancer Consensus Conference of the Gynecologic Cancer InterGroup was held in Vancouver, Canada, in June 2010. Representatives of 23 cooperative research groups studying gynecologic cancers gathered to establish international consensus on issues critical to the conduct of large randomized trials. Group C, 1 of the 3 discussion groups, examined recurrent ovarian cancer, and we report the consensus reached regarding 4 questions. These included the following: (1) What is the role of cytoreductive surgery for recurrent ovarian cancer? (2) How do we define distinct patient populations in need of specific therapeutic approaches? (3) Should end points for trials with recurrent disease vary from those of first-line trials? (4) Is CA-125 progression alone sufficient for entry/eligibility into clinical trials?

27. The mutational and copy number landscape of ovarian carcinosarcoma.

Abstract

Introduction Ovarian carcinosarcomas (OCS) account for approximately 5% ovarian cancer cases, but have been specifically excluded from large treatment-defining clinical trials. They present at late stage and have a poor prognosis. Originally considered to be a type of sarcoma, these biphasic tumours are now considered to be epithelial, with biphasic differentiation occurring during tumour development. Due to their exclusion from clinical trials, little is known of the biology of this disease. We sought to identify the genomic abnormalities in OCS. Materials /Patients and methods Archival FFPE samples from 18 OCS cases were identified. Next-generation sequencing (custom 377-gene panel; Illumina NextSeq500; 2 x 75bp PE) was performed on microdissected carcinomatous and sarcomatous components as well as metastatic deposits. Copy number (CN), indel and SNV analysis was performed on each component. Immunohistochemistry for immune cell populations and RNAseq analysis on matched carcinoma/sarcoma samples are on-going. Results Median age at diagnosis was 70.9 years (range 49.4-74.6). 11 cases were stage 3 or 4 at diagnosis. Median overall survival was 32.2 months. 17/18 cases had high-grade serous carcinoma (HGSC) morphology in the carcinoma component; the remaining case arose within a G2 endometrioid tumour. Tumours had genomic profiles similar to HGSC, including near-ubiquitous TP53 mutation (17/18 cases, including 17/17 HGSC-derived cases), CCNE1 amplification (4/18), BRCA2 loss or mutation (4/18), inactivation (by SNV) or disruption (by rearrangement) of NF1 and CDKN2A (2/18 each), deletion of RB1 and PTEN (2/18 each) and MYC amplification (1/18 cases). The endometrioid-derived case lacked TP53 mutations but contained mutations in KRAS and ERBB2 in both carcinoma and sarcoma components. As with HGSC, genomes were structurally and numerically chaotic with an average of 3.3 high-level gains and 1.4 likely homozygous losses per sample, as well as 3.9 sequence-level mutations. There were no consistent SNV/indel differences between carcinoma and sarcoma components, with at least one point mutation shared in all sample pairs. On average, carcinoma-sarcoma pairs differed by a single mutation, with 100% SNV concordance in 50% of cases. By contrast, cases showed much more diversity in CN profiles between carcinoma and sarcoma, with a mean of 10.6 genes (range 0-36) having different CN states between the two. The most commonly different genes were FGF3, which showed relative gain in the carcinoma in 4/16 cases, and MDM2, which showed relative gain in the sarcoma in 3/16 cases. Metastases showed substantial genomic divergence from their corresponding primary, indicating early seeding of metastatic sites. Conclusions There are few mutational differences between carcinoma and sarcoma components in OCS, with shared point mutations indicating a clonal origin. By contrast, there is diversity in CN profiles. Metastases show substantial genomic divergence from their corresponding primary, indicating early metastatic seeding. Overall, OCS cases with a high grade serous carcinoma component have genomic profiles that are very similar to classical HGSC, including ubiquitous TP53 mutation. Abstract

Background: Rucaparib is an orally available potent selective small-molecule inhibitor of poly(ADP-ribose) polymerase (PARP) 1 and 2. Rucaparib induces synthetic lethality in cancer cells defective in the homologous recombination repair pathway including BRCA-1/2. We investigated the efficacy and safety of single-agent rucaparib in germline (g) BRCA mutation carriers with advanced breast and ovarian cancers. Methods: Phase II, open-label, multicentre trial of rucaparib in proven BRCA-1/2 mutation carriers with advanced breast and or ovarian cancer, WHO PS 0–1 and normal organ function. Intravenous (i.v.) and subsequently oral rucaparib were assessed, using a range of dosing schedules, to determine the safety, tolerability, dose-limiting toxic effects and pharmacodynamic (PD) and pharmacokinetic (PK) profiles. Results: Rucaparib was well tolerated in patients up to doses of 480 mg per day and is a potent inhibitor of PARP, with sustained inhibition ⩾24 h after single doses. The i.v. rucaparib (intermittent dosing schedule) resulted in an objective response rate (ORR) of only 2% but with 41% (18 out of 44) patients achieved stable disease for ⩾12 weeks and 3 patients maintaining disease stabilisation for >52 weeks. The ORR for oral rucaparib (across all six dose levels) was 15%. In the oral cohorts, 81% (22 out of 27) of the patients had ovarian cancer and 12 out of 13, who were dosed continuously, achieved RECIST complete response/partial response (CR/PR) or stable disease (SD) ⩾12 weeks, with a median duration of response of 179 days (range 84–567 days). Conclusions: Rucaparib is well tolerated and results in high levels of PARP inhibition in surrogate tissues even at the lowest dose levels. Rucaparib is active in gBRCA-mutant ovarian cancer and this activity correlates with platinum-free interval. The key lessons learned from this study is that continuous rucaparib dosing is required for optimal response, the recommended phase 2 dose (RP2D) for continuous oral scheduling has not been established and requires further exploration and, thirdly, the use of a PD biomarker to evaluate dose–response has its limitations.

Abstract

Objective: To investigate quality of life (QoL) and association with surgical complexity and disease burden after surgical resection for advanced ovarian cancer in centres with variation in surgical approach., Design: Prospective multicentre observational study., Setting: Gynaecological cancer surgery centres in the UK, Kolkata, India, and Melbourne, Australia., Sample: Patients undergoing surgical resection (with low, intermediate or high surgical complexity score, SCS) for late-stage ovarian cancer., Main Outcome Measures: Primary: change in global score on the European Organisation for Research and Treatment of Cancer (EORTC) core quality-of-life questionnaire (QLQ-C30). Secondary: EORTC ovarian cancer module (OV28), progression-free survival., Results: Patients' preoperative disease burden and SCS varied between centres, confirming differences in surgical ethos. QoL response rates were 90% up to 18 months. Mean change from the pre-surgical baseline in the EORTC QLQ-C30 was 3.4 (SD 1.8, n = 88) in the low, 4.0 (SD 2.1, n = 55) in the intermediate and 4.3 (SD 2.1, n = 52) in the high-SCS group after 6 weeks (p = 0.048), and 4.3 (SD 2.1, n = 51), 5.1 (SD 2.2, n = 41) and 5.1 (SD 2.2, n = 35), respectively, after 12 months (p = 0.133). In a repeated-measures model, there were no clinically or statistically meaningful differences in EORTC QLQ-C30 global scores between the three SCS groups (p = 0.840), but there was a small statistically significant improvement in all groups over time (p < 0.001). The high-SCS group experienced small to moderate decreases in physical (p = 0.004), role (p = 0.016) and emotional (p = 0.001) function at 6 weeks post-surgery, which resolved by 6-12 months., Conclusions: The global QoL of patients undergoing low-, intermediate- and high-SCS surgery improved at 12 months after surgery and was no worse in patients undergoing extensive surgery., Tweetable Abstract: Compared with surgery of lower complexity, extensive surgery does not result in poorer quality of life in patients with advanced ovarian cancer., (© 2021 The Authors.
