OBJECTIVE: There is limited evidence on the impact of protocolized ventilator weaning in pediatric acute respiratory distress syndrome (ARDS), despite utilization in clinical trials and clinical care. We aimed to determine whether protocolized ventilator weaning shortens mechanical ventilation duration and pediatric intensive care unit (PICU) length of stay in pediatric ARDS survivors. DESIGN: Secondary analysis of a prospective pediatric ARDS (Berlin definition) cohort from July 2011 to June 2019, analyzed using interrupted time series analysis pre- and post-implementation of a ventilator weaning pathway. We compared duration of invasive ventilation and PICU length of stay in survivors before and after implementation of a ventilator weaning pathway. We excluded PICU non-survivors and subjects with > 100 ventilator days. SETTING: Large academic tertiary care PICU. PATIENTS: Children with ARDS who survived to PICU discharge with ≤ 100 days of invasive mechanical ventilation. INTERVENTIONS: Implementation of a ventilator weaning pathway on May 2016. MEASUREMENTS AND MAIN RESULTS: Of 723 children with ARDS, 132 subjects died and 6 subjects with ventilation > 100 days were excluded. Of the remaining 585 subjects, 375 subjects had ARDS prior to pathway intervention and 210 after. Patients in the pre-intervention epoch were younger, more likely to have infectious ARDS, and had increased use of alternative ventilator modes. Pathway adoption was rapid and sustained. Controlling for temporality, pathway implementation was associated with a decrease of a median 3.6 ventilator days (95% CI −5.4 to-1.7; p < 0.001). There was no change reintubation rates. Results were robust to multiple sensitivity analyses adjusting for confounders. CONCLUSIONS: Ventilator weaning pathway implementation shortened invasive ventilation duration in pediatric ARDS survivors with no change in reintubation. The effect size of this intervention was comparable to those targeted in ARDS trials.

OBJECTIVE: Timely empiric antimicrobial therapy is associated with improved outcomes in pediatric sepsis, but minimal data exist to guide empiric therapy. We sought to describe the prevalence of four pathogens that are not part of routine empiric coverage (eg Staphylococcus aureus, Pseudomonas aeruginosa, Clostridium difficile, and fungal infections) in pediatric sepsis patients in a contemporary nationally representative sample. DESIGN: This was a retrospective cohort study using administrative data. SETTING: We used the Nationwide Readmissions Database from 2014, which is a nationally representative dataset that contains data from nearly half of all discharges from nonfederal hospitals in the United States. PATIENTS: Discharges of patients who were less than 19 years old at discharge and were not neonatal with a discharge diagnosis of sepsis. INTERVENTIONS: None MEASUREMENTS AND MAIN RESULTS: Of the 19,113 pediatric admissions with sepsis (6,300 [33%] previously healthy and 12,813 [67%] with a chronic disease), 31% received mechanical ventilation, 19% had shock, and 588 (3.1%) died during their hospitalization. Among all admissions, 8,204 (42.9%) had a bacterial or fungal pathogen identified. Staphylococcus aureus was the most common pathogen identified in previously healthy patients (n=593, 9.4%) and those with any chronic disease (n=1,430, 11.1%). Methicillin-resistant Staphylococcus aureus, Pseudomonas aeruginosa, Clostridium difficile and fungal infections all had high prevalence in specific chronic diseases associated with frequent contact with the healthcare system, early surgery, indwelling devices, or immunosuppression. CONCLUSIONS: In this nationally representative administrative database, the most common identified pathogen was Staphylococcus aureus in previously healthy and chronically ill children. In addition, a high proportion of children with sepsis and select chronic diseases had infections with methicillin-resistant Staphylococcus aureus, fungal infections, Pseudomonas infections, and Clostridium difficile. Clinicians caring for pediatric patients should consider coverage of these organisms when administering empiric antimicrobials for sepsis.

Within two decades, there have emerged three highly pathogenic and deadly human coronaviruses, namely SARS-CoV, MERS-CoV and SARS-CoV-2. The economic burden and health threats caused by these coronaviruses are extremely dreadful and getting more serious as the increasing number of global infections and attributed deaths of SARS-CoV-2 and MERS-CoV. Unfortunately, specific medical countermeasures for these hCoVs remain absent. Moreover, the fast spread of misinformation about the ongoing SARS-CoV-2 pandemic uniquely places the virus alongside an annoying infodemic and causes unnecessary worldwide panic. SARS-CoV-2 shares many similarities with SARS-CoV and MERS-CoV, certainly, obvious differences exist as well. Lessons learnt from SARS-CoV and MERS-CoV, timely updated information of SARS-CoV-2 and MERS-CoV, and summarized specific knowledge of these hCoVs are extremely invaluable for effectively and efficiently contain the outbreak of SARS-CoV-2 and MERS-CoV. By gaining a deeper understanding of hCoVs and the illnesses caused by them, we can bridge knowledge gaps, provide cultural weapons for fighting and controling the spread of MERS-CoV and SARS-CoV-2, and prepare effective and robust defense lines against hCoVs that may emerge or reemerge in the future. To this end, the state-of-the-art knowledge and comparing the biological features of these lethal hCoVs and the clinical characteristics of illnesses caused by them are systematically summarized in the review.

The recent ongoing outbreak of severe pneumonia associated with a novel coronavirus (SARS-CoV-2), currently of unknown origin, creates a world emergency that has put global public health institutions on high alert. At present there is limited clinical information of the SARS-CoV-2 and there is no specific treatment recommended, although technical guidances and suggestions have been developed and will continue to be updated as additional information becomes available. Preventive treatment has an important role to control and avoid the spread of severe respiratory disease, but often is difficult to obtain and sometimes cannot be effective to reduce the risk of deterioration of the underlining lung pathology. In order to define an effective and safe treatment for SARS-CoV-2-associated disease, we provide considerations on the actual treatments, on how to avoid complications and the undesirable side effects related to them and to select and apply earlier the most appropriate treatment. Approaching to treat severe respiratory disease in infants and children, the risks related to the development of atelectasis starting invasive or non-invasive ventilation support and the risk of oxygen toxicity must be taken into serious consideration. For an appropriate and effective approach to treat severe pediatric respiratory diseases, two main different strategies can be proposed according to the stage and severity of the patient conditions: patient in the initial phase and with non-severe lung pathology and patient with severe initial respiratory impairment and/or with delay in arrival to observation. The final outcome is strictly connected with the ability to apply an appropriate treatment early and to reduce all the complications that can arise during the intensive care admission.

Summary Background : Our objective was to evaluate the efficacy of selective bronchial intubation and independent lung ventilation during thoracic surgery in children up to 3 years, using a double lumen tube. Methods : We studied retrospective (cases 1–6) and prospective cases (7–17) between January 1996 and December 2000 at the All India Institute of Medical Sciences, New Delhi, India and at Fatebenefratelli and Ophthalmiatric Hospital, Milan, Italy. Seventeen children, 1 day to 3 years of age and weighing 2.7–12 kg, were submitted to thoracic surgery for a variety of surgical conditions. Anesthesia was conducted as usual in this type of patient and selective intubation was performed using a double lumen tube (Marraro Pediatric double lumen tube). During the operation one lung ventilation was applied and at the end of surgery the collapsed lung was reexpanded independently from the contralateral lung. Results : Six children remained intubated with a double lumen tube for between 8 and 48 h and one (case no. 11) with a single lumen tube for 24 h, while 10 of the older children were extubated on the table. No serious complications during or after surgery were noted and after extubation all the children recovered completely without sequelae. Conclusions : The double lumen tube appears to be very effective in allowing one lung ventilation in this age group during thoracic surgery.

OBJECTIVE: Recently, natural exogenous surfactant replacement has been used in experimental models and clinical trials for the treatment of severe respiratory syncytial virus (RSV) disease. The present study was aimed at verifying this hypothesis and confirming the results of our previous pilot study by assessing the effect of surfactant treatment in mechanically ventilated infants with severe RSV-induced respiratory failure. DESIGN: Multicenter, randomized, controlled study. SETTING: Six pediatric intensive care units staffed by full-time intensive care physicians. PATIENTS: A total of 40 infants (20 treated and 20 controls) with RSV-induced respiratory failure requiring conventional mechanical ventilation (CMV) were randomly assigned to either exogenous surfactant (treated group) or conventional treatment (control group) over a 1-yr period. INTERVENTIONS: Fifty milligrams per kilogram of body weight of porcine-derived natural surfactant (Curosurf) was administered. The drug was instilled by means of a syringe attached to a small suction catheter inserted into the endotracheal tube down to its tip, momentarily disconnecting the patient from CMV. Main Outcome Measures: The assessment consisted of the following outcome variables: duration of CMV, length of intensive care unit stay, gas exchange, respiratory mechanics, re-treatment need, complications, and mortality. RESULTS: The two groups were similar with regard to demographics, Pediatric Risk of Mortality scores, and baseline Pao(2)/Fio(2), Paco(2), and ventilator settings. A marked increase in Pao(2)/Fio(2) and decrease in Paco(2) were observed in the treated group after surfactant administration. Hemodynamic parameters remained unchanged throughout the study period. Peak inspiratory pressure and static compliance were similar at baseline in the two groups. A decrease in peak inspiratory pressure and increase in static compliance were observed in the treated group after surfactant administration. Among surfactant-treated patients, 15 received the treatment within 24 hrs of admission, whereas the remainder (five patients) were treated later. Among children who were treated later, three needed an additional dose of surfactant. None of the children treated within 24 hrs needed an additional dose. Duration of CMV and length of stay in the intensive care unit were significantly shorter in the treated group (4.6 +/- 0.8 and 6.4 +/- 0.9 days, respectively) compared with the control group (5.8 +/- 0.7 and 8.2 +/- 1.1 days, respectively) (p

Acute respiratory distress syndrome (ARDS) is a heterogeneous syndrome that lacks definitive treatment. The cornerstone of management is sound intensive care treatment and early anticipatory ventilation support. A mechanical ventilation strategy aiming at optimal alveolar recruitment, judicious use of positive end-respiratory pressure (PEEP) and low tidal volumes (VT) remains the mainstay for managing this lung disease. Several treatments have been proposed in rescue settings, but confirmation is needed from large controlled clinical trials before they be recommended for routine care. Non-invasive ventilation (NIV) is suggested with a cautious approach and a strict selection of candidates for treatment. Mild and moderate cases can be efficiently treated by NIV, but this is contra-indicated with severe ARDS. The extra-corporeal carbon dioxide removal (ECCO2 R), used as an integrated tool with conventional ventilation, is playing a new role in adjusting respiratory acidosis and CO2. The proposed benefits of ECCO2 R over extra-corporeal membrane oxygenation (ECMO) consist in a reduction of artificial surface contact, avoidance of pump-related side effects and technical complications, as well as lower costs. The advantages and disadvantages of inhaled nitric oxide (iNO) are better recognized today and iNO is not recommended for ARDS and acute lung injury (ALI) in children and adults because iNO results in a transient improvement in oxygenation but does not reduce mortality, and may be harmful. Several trials have found no clinical benefit from various surfactant supplementation methods in adult patients with ARDS. However, studies which are still controversial have shown that surfactant supplementation can improve oxygenation and decrease mortality in pediatric and adolescent patients in specific conditions and, when applied in different modes and doses, also in neonatal respiratory distress syndrome (RDS) of preemies. Management of ARDS remains supportive, aimed at improving gas exchange and preventing complications. Progress in the treatment of ARDS must be addressed toward the new paradigm of the disease pathobiology to be applied to the disease definition and to predict the treatment outcome, also with the perspective to develop predictive and personalized medicine that highlights new and challenging opportunities in terms of benefit for patient's safety and doctor's responsibility, with further medico-legal implication.

Background and Objectives: Ropivacaine 1% has recently been used in clinical trials for peribulbar anesthesia. This study aims to compare the safety and the efficacy of ropivacaine 0.75% with that of a 1:1 mixture of bupivacaine 0.5% and mepivacaine 2% for peribulbar anesthesia. Methods: Two thousand patients undergoing peribulbar anesthesia for elective cataract phacoemulsification were prospectively studied over a 1-year period and randomly assigned to 1 of 2 groups according to the local anesthetic used. One thousand patients were administered peribulbar anesthesia with 9 mL of ropivacaine 0.75% plus 1 mL of hyaluronidase (group R), and 1,000 patients received peribulbar anesthesia with 4 mL of bupivacaine 0.5% plus 4 mL of mepivacaine 2% plus 1 mL of hyaluronidase plus 1 mL of sodium bicarbonate (group BM). Peribulbar anesthesia was always accomplished by the same physician by 2 injections of 5 mL each, with a 25-gauge 25-mm needle. Evaluation was performed by another physician blinded to the technique used and included assessment of pain on local anesthetic injection, ocular and eyelid akinesia, need for top-up injections, onset time and duration of anesthesia, intraoperative analgesia, duration of surgery, hemodynamic parameters, and incidence of perioperative complications. This study evaluates the efficiency of heat and moisture exchangers (HMEs) in allowing adequate humidification and warming during anaesthesia in children. Eighteen paediatric patients undergoing anaesthesia were divided into two groups: group A ten patients: infants up to 10 kg-->Hygrobaby HME; group B 8 patients: children above 10 kg-->Hygroboy HME. The following parameters were evaluated: body temperature (bT), room temperature (rT), fresh gas temperature, HME warm-up time, inspired and expired gases temperature and humidity, conserving efficiency, and duration of anaesthesia. Gas temperatures were recorded by means of a recorder fitted with four thermal probes. Humidity values were mathematically derived. The correlation between efficiency and rT, bT, and fresh gas temperature was computed. In both groups the inspired gases temperatures were below 30 degrees C. Inspired absolute humidity was never more than 28 mgH2O.l(-1). The conserving efficiency was good (0.93 in both groups). A positive correlation was found between efficiency and fresh gas temperature. HMEs did not meet the minimum standards for humidity and heating during anaesthesia in children, although their conserving efficiency was found to be satisfactory.

Objective: This study compares perfluorocarbon broncho-alveloar lavage (PFC-BAL) with isotonic saline broncho-alveolar lavage (saline-BAL) in an experimental model of meconium aspiration in adult guinea pigs. Design: Prospective controlled experimental study. Patients and methods: Ten male guinea pigs were given 1 ml of human meconium, diluted to 10 % in isotonic saline, via tracheostomy and then artificially ventilated. After stabilization, five animals (control group) underwent BAL with 10 ml/kg isotonic saline solution. After bronchial suctioning, a further BAL with 2 ml/kg saline was performed. The other five animals (study group) underwent BAL with 10 ml/kg of PFC (RIMAR 101). Bronchosuction was effected at 5 min and then a BAL with PFC 2 ml/kg was performed. Both groups received conventional mechanical ventilation during the lavage procedures. Within 20 min all the saline-BAL treated animals died, whereas the PFC-BAL treated animals survived and were then treated with total liquid ventilation (TLV) by gravity. Measurements and results: After meconium inhalation in both groups, a large alveolar-arterial oxygen difference, hypercarbia, severe acidosis and tachycardia were noticed. In PFC treated animals, an improvement in blood gases was noted and acid-base balance remained stable compared to saline-BAL treated animals. No haemodynamic change was observed during or after PFC-BAL, while during saline-BAL there was evidence of bradycardia, hypotension and respiratory failure, which led to the death of the animals. The histological lung sections in the PFC-BAL group showed evidence of normal alveolar expansion with a minimal presence of meconium debris in the small bronchioles. In saline-BAL treated animals, the lung structure appeared severely compromised with the presence of meconium in bronchioles and alveoli, intra-alveolar oedema and haemorrhagic areas. Conclusions: This study confirms the detrimental effects of meconium aspiration and the impairment of lung function following saline-BAL. By contrast, PFC-BAL and TLV by gravity seemed to prevent absorption of meconium, facilitate its removal, improve gas exchange and reduce lung barotrauma.

Background Supported by the International Society for Translational Medicine (ISTM), Wenzhou Medical College and the First Affiliated Hospital of Wenzhou Medical College, the International Conference on Translational Medicine (ICTM) was held on October 22–23, 2011 in Wenzhou, China. Nearly 800 registrants attended the meeting, primarily representing institutes and hospitals in Europe, The United States of America, And Asia, and China. The meeting was chaired and organized by Dr. Xiangdong Wang, Xiaoming Chen, Richard Coico, Jeffrey M. Drazen, Richard Horton, Francesco M. Marincola, Laurentiu M. Popescu, Jia Qu and Aamir Shahzad. Findings The meeting focused on the communication of the need to foster translational medicine (TM) by building and broadening bridges between basic research and clinical studies at the international level. The meeting included distinguished TM experts from academia, the pharmaceutical and diagnostics industries, government agencies, regulators, and clinicians and provided the opportunity to identify shared interests and efforts for collaborative approaches utilizing cutting edge technologies, innovative approaches and novel therapeutic interventions. The meeting defined the concept of TM in its two‐way operational scheme and emphasized the need for bed to bench efforts based directly on clinical observation. Conclusions It was the meeting participants’ realization that the shared main goals of TM include breaking the separation between clinic practice and basic research, establishing positive feedback by understanding the basis of expected and unexpected clinical outcomes and accelerating basic research relevant to human suffering. The primary objectives of the meeting were two‐fold: to accelerate the two‐way translation by informing the participants representing the different disciplines about the state of art activities around TM approaches; and to identify areas that need to be supported by redirecting limited resources as well as identifying new sources of funding. This report summarizes key concepts presented during the meeting representing the state‐of‐art translational research and salient aspects of the ensuing discussions.

The Italian Hospital, as the institution at the source of health care delivery, has been operating under crisis conditions for a long time because of an increasing demand for health care from a rapidly and constantly changing population. In Italy, child birth survival has increased threefold [1], infantile mortality has fallen to 4.3 per 1,000 live births while the general population has basically aged [2]. It is estimated that the ageing index will double before 2050 and thus future public health expenditure will be more and more subordinate to long-time care [3]. In today’s Italy, health care is universally provided under the law to all Italian citizens and the state, in various guises, sustains the bulk of the expenditure [4]. The last decades have witnessed the development of a private health care sector, although state as well as insurance contributions still account for most of the expense. The Italian health system is characterised by marked regional differences. Italian economic growth has never been uniform and the gap between the wealthiest northern and central regions and the less affluent southern ones has never been bridged. Regional socio-economic disparities have caused wide differences in the quality and efficiency of health services in this country, especially in the south where a high percentage of low-income families mostly live [5]. In the course of time, the evolution of the health care system has been marked by various attempts at reform [6] in the face of ever more stringent budgetary limits on the one hand, and an ever increasing process of decentralisation on the other. This devolution is ongoing and could lead to the creation of as many diverse local health systems as there are regions in the country [7].

Allocation is a frequently recurring concept from the economic literature which has sneaked into the field of medicine and calls on physicians, patients, their families and policy-makers to face up to complex challenges.

Open lung and low tidal volume ventilation appear to be a promising ventilation for chest trauma as it can reduce ARDS and improve outcome. Local therapy (e.g. BAL) can be synergic to remove from the lung the debris, mitigate inflammatory cascade and avoid damage spreading to not compromised lung areas.44 pulmonary contused patients were randomized to receive broncho-suction and volume controlled low tidal volume ventilation-VCLTVV (Control Group) or the same ventilation plus medicated (saline + surfactant) BAL (Treatment Group). Tidal volume10 ml/kg, PEEP of 10-12 cm H(2)O and PaO(2) 60-100 mm Hg and PaCO(2) 35-45 mm Hg were used in both groups. BAL was performed using a fiberscope. 4 boluses of 25 ml saline with 2.4 mg/ml of surfactant were introduced into each contused lobe in which, subsequently, 240 mg of surfactant was instilled.All patients survived. In the Control Group 18 patients developed pneumonia, 5 ARDS and days of intubation were 11.50 (3.83) compared to 5.05 (1.21) of Treatment Group in which OI and PaO(2)/FiO(2) significantly improved from 36 h.VCLTVV alone was not able to prevent ARDS and infection in the Control Group as the reduction of intubation. In the Treatment Group, VCLTVV and medicated BAL facilitated the removal of degradated lung material and recruited the contused lung regions, enabling the healing of the lung pathology.

To study the ability of volume-controlled ventilation and medicated (normal saline plus surfactant) bronchoalveolar lavage in aspiration to reduce the duration of intubation and improve gas exchange.: Randomized controlled clinical trial.Pediatric intensive care unit.Twenty children, 1 month to 16 yrs old, who were intubated and mechanically ventilated, were randomized within 6 hrs of aspiration to receive volume-controlled ventilation plus medicated bronchoalveolar lavage (treatment group) or the same ventilation and bronchosuction (control group).Volume-controlled ventilation and positive end-expiratory pressure (10-12 cm H2O) were applied. Medicated bronchoalveolar lavage was performed using five aliquots of 5 mL of saline plus 10 mg/mL Curosurf (porcine surfactant, Chiesi Pharmaceutical SpA, Parma, Italy) in infants, five boluses of 10 mL of saline plus 5 mg/mL Curosurf in children, and four boluses of 25 mL of saline with 2.4 mg/mL Curosurf in adolescents for each affected lobe. One hour after bronchoalveolar lavage, 240 mg of Curosurf was administered locally.All patients survived. In the treatment group, days of intubation were 4.6 (+/-1.07), oxygenation index and Pao2/Fio2 improved significantly at 24 hrs, and statistical reduction in tidal volume mL/kg was observed from 36 hrs. In the control group, days of intubation were 11.8 (+/-3.22) (p.0001), no improvement in oxygenation was noted, and pneumonia was observed in seven children (70%).Even though this was an unblinded small clinical trial and low tidal volume strategy was not employed at an early stage after lung injury, there is some evidence that bronchoalveolar lavage with normal saline and surfactant may have clinical value in treating severe aspiration syndrome in children. More clinical studies are warranted to overcome study limitations and potential bias.

During inhalation anesthesia, lung alveolar and Clara cells are exposed directly to volatile anesthetic. Porcine-derived natural lung surfactant (Curosurf) was used for in vitro measurements of surface activity under exposure to N2O mixed with air at concentration used in prolonged anesthesia. The study suggests that prolonged use of N2O may be associated with increased value of the minimum surface tension and reduction of surface tension-bubble radius hysteresis obtained in the pulsating bubble surfactometer used in the experiments. These phenomena indicate the physicochemical inactivation of surfactant.

The recognition that alveolar overdistension rather than peak inspiratory airway pressure is the primary determinant of lung injury has shifted our understanding of the pathogenesis of ventilator-induced side effects. In this review, contemporary ventilatory methods, supportive treatments, and future developments relevant to pediatric critical care are reviewed.A strategy combining recruitment maneuvers, low-tidal volume, and higher positive end-expiratory pressure (PEEP) decreases barotrauma and volutrauma. Given that appropriate tidal volumes are critical in determining adequate alveolar ventilation and avoiding lung injury, volume-control ventilation with high PEEP levels has been proposed as the preferable protective ventilatory mode. Pressure-related volume control ventilation and high-frequency oscillatory ventilation (HFOV) have taken on an important role as protective lung strategies. Further data are required in the treatment of children, confirming the preliminary results in specific lung pathologies. Spontaneous breathing supported artificially during inspiration (pressure support ventilation) is widely used to maintain or reactivate spontaneous breathing and to avoid hemodynamic variation. Volume support ventilation reduces the need for manual adaptation to maintain stable tidal and minute volume and can be useful in weaning. Prone positioning and permissive hypercapnia have taken on an important role in the treatment of patients undergoing artificial ventilation. Surfactant and nitric oxide have been proposed in specific lung pathologies to facilitate ventilation and gas exchange and to reduce inspired oxygen concentration. Investigation of lung ventilation using a liquid instead of gas has opened new vistas on several lung pathologies with high mortality rates.The conviction emerges that the best ventilatory treatment may be obtained by applying a combination of types of ventilation and supportive treatments as outlined above. Early treatment is important for the overall positive final result. Lung recruitment maneuvers followed by maintaining an open lung favor rapid resolution of pathology and reduce side effects.The methods proposed require confirmation through large controlled clinical trials that can assess the efficacy reported in pilot studies and case reports and define the optimal method(s) to treat individual pathologies in the various pediatric age groups.

Asthma is a chronic inflammatory disorder of the airways characterized by an obstruction of airflow. It affects people of all ages and is characterized by repeated attacks of difficulty in breathing. Airway inflammation may cause recurrent or persistent bronchospasm, wheezing, breathlessness, tightness in the chest and coughing, particularly at night, in the early morning, or after exercise.

The ethical problem of whether or not to treat a given patient arises before the technical problem. Making a choice, it is necessary to know what benefits will derive from a given treatment in a certain ward and how successful a given operation will be. The doctor must evaluate the management costs but must also be the custodian of community funds.

Of 67 leukaemic children transplanted in our BMT unit 3 presented with severe acute respiratory syndrome associated with pulmonary thromboembolism (PTE) as diagnosed by scintiscan and/or angiography in the first month after BMT. Intervention with continuous positive pressure ventilation, urokinase (loading dose, then continuous infusion for 12-18 h) and heparin (continuous infusion for an average of 10 days) has been carried out successfully in two cases. In conclusion, when evaluating patients undergoing BMT and developing early pulmonary complications, PTE must be considered. The pathogenesis of PTE is still difficult to ascertain but urokinase therapy may reduce early morbidity.

Intensive care of a patient requires heavy monitoring and versatile therapeutic actions. These produce a huge amount of patient information. A problem exists in managing this data and other information from all supporting activities creating a need for an automated information management system. To have a sound basis for future automated information systems in intensive care unit (ICU), a conceptual model is created to cover both the clinical and other activities of the ICU. The conceptual model consists of data flow diagrams and entity-relationship diagrams with underlying common data dictionary. A modern CASE tool is utilized to build the model. The work forms a part of AIM-INFORM project, which has a purpose to develop information management and decision support systems for high dependency environment.

Two adolescents (12 years old, suffering from acute lymphoid leukaemia (LLA) and 16 years old with acute myeloid leukaemia (LMA)) were treated with porcine surfactant (Curosurf). The indication for this treatment was a severe Respiratory Distress Syndrome (RDS) which appeared during leukaemia treatment. Pulmonary thrombo-embolism was suspected. The surfactant administered improved ventilation and oxygenation, enabling confirmation of the diagnosis which would otherwise have been impossible due to the seriousness of their condition.

A follow-up was carried out on twenty-two children (thirteen of whom were newborn) who had been treated with porcine surfactant (Curosurf) for respiratory distress syndrome. All the infants were enrolled in the follow-up at different times after surfactant replacement to evaluate the clinical status and the incidence (if any) of respiratory disease, adverse drug effects and allergic conditions. Twelve of the twenty-two children treated completed the follow-up. Seven prematurely-born children died of endocranial haemorrhage; two children died of malignant development of the basal pathology (leukaemia). Treatment with pig surfactant appears to cause no allergic side effects. Skin and laboratory tests performed were negative.
