Carbon dragged at sub-arc depths and sequestered in the asthenospheric upper mantle during cold subduction is potentially released after millions of years during the breakup of continental plates. However, it is unclear whether these deep-carbon reservoirs can be locally remobilized on shorter-term timescales. Here we reveal the fate of carbon released during cold subduction by analyzing an anomalously deep earthquake in December 2020 in the lithospheric mantle beneath Milan (Italy), above a deep-carbon reservoir previously imaged in the mantle wedge by geophysical methods. We show that the earthquake source moment tensor includes a major explosive component that we ascribe to carbon-rich melt/fluid migration along upper-mantle shear zones and rapid release of about 17,000 tons of carbon dioxide when ascending melts exit the carbonate stability field. Our results underline the importance of carbon-rich melts at active continental margins for emission budgets and suggest their potential episodic contributions to atmospheric carbon dioxide.

Very low-volume bowel preparation (BP) for colonoscopy with 1-liter polyethylene glycol plus ascorbate (1L-PEG-Asc) has displayed high tolerability and quality of bowel cleansing. Concerns have been raised regarding its safety. We aimed to evaluate the incidence of adverse events (AEs) following BP with 1L-PEG-Asc or 2L-PEG-Asc.From January 2019 to September 2020, data from all consecutive adult outpatients who underwent colonoscopy in Our Unit were collected. AEs were assessed by reviewing the clinical and laboratory data of patients who attended the Emergency Department (ED) of Modena District Hospitals in the 7 days following the colonoscopy, and were classified as "BP-related" or "BP-unrelated".During the study, 4069 (68.03%) and 1912 (31.97%) patients underwent colonoscopy after taking 2L-PEG-Asc or 1L-PEG-Asc, respectively. Regarding AEs, 77 (1.29%) patients attended ED, 53 (53/4069, 1.30%) and 24 (24/1912, 1.25%) after taking 2L-PEG-Asc and 1L-PEG-Asc. BP-related AEs were observed in 5 (5/4069, 0.12%) and 4 (4/1912, 0.21%) patients, respectively. The most frequent BP-related AEs were tachyarrhythmias (6/5981, 0.10%).The incidence rate of clinically relevant BP-related AEs is extremely low. This strongly suggests that 1L-PEG-Asc colonoscopy BP is as safe as 2L-PEG-Asc BP in a real-life clinical setting of unselected patients.

The treatment of anastomotic post-esophagectomy leaks and fistula is challenging. Endoluminal vacuum-assisted closure (EVAC) is an emerging technique that employs negative pressure wound therapy to treat anastomotic leaks endoscopically. Esosponge is specifically designed for esophageal EVAC therapy. We report on a 49-year-old woman who underwent a totally mini-invasive Ivor–Lewis esophagectomy and developed a giant postoperative leak with a complex pleural collection, but she was not fit for surgical re-intervention. The patient healed almost completely after 14 exchange sessions of Esosponge over 35 days.

Kosovo is one of the most seismically active regions in Europe, lying within the Alpine-Mediterranean tectonic belt. Historical records for the region show several catastrophic earthquakes with epicentral intensity IX (MCS). However, due to Kosovo’s high population density, high prevalence of traditional construction, and insufficient enforcement of building codes, Kosovo is vulnerable to earthquake damage. In this study, we present earthquake hazard maps for bedrock conditions in Kosovo based on the well-known Neo-deterministic Seismic Hazard Assessment (NDSHA) method. NDSHA relies upon the fundamental physics of wave generation and propagation in complex geologic structures to generate realistic time series, used as input for the computation of several ground motion parameters, integrating the available knowledge of seismic history, seismogenic zones and morphostructural nodes. In accordance with continuum mechanics, the tensor nature of earthquake ground motion is preserved, producing realistic signals using structural models obtained by tomographic inversion and earthquake source information readily available in literature. Our maps are generally consistent with the observed intensity IX (MCS) and suggest that, in some instances, intensity X could be reached.

BACKGROUND AND OBJECTIVES Early-onset colorectal cancer (eoCRC), defined as colorectal cancer (CRC) before the age of 50 is increasing in incidence. We evaluated exogenous and endogenous risk factors, and clinical features of eoCRC, compared to late-onset CRC (loCRC). METHODS In this retrospective case-case study, patients were prospectively enrolled from 2015 to 2018. We collected clinical features (age, sex, time from symptom onset to diagnosis, symptoms, family history, smoking and alcohol habits, diabetes, BMI, and genetic analysis) and tumor characteristics. Independent risk factors for eoCRC and odds ratios (ORs) were identified. RESULTS Fifty-four eoCRCs and 494 loCRCs were enrolled. Patients with eoCRC experienced longer delay time from symptom onset to diagnosis: 40.7% were diagnosed within 6 months from symptoms onset, compared to 85.6% of patients with loCRC (P < 0.0001). They differed for sex, presence of symptoms, family history, smoking habit, alcohol intake, and BMI. Rectal localization was more closely associated with eoCRC (64.8%) than loCRC (34.5%, P < 0.0001). Family history of CRC was associated with eoCRC (OR = 8.8). When family history occurred with hereditary cancer syndromes, the OR for eoCRC increased to 21. CONCLUSION In young adults with alarming symptoms, CRC must be suspected to avoid delay time from symptom onset to diagnosis and genetic risk assessment has to be evaluated. Smoking habits, alcohol intake, and BMI are not associated with eoCRC.
