Objective: To describe the outcome after acute biliary pancreatitis in patients treated during the acute attack by endoscopic sphincterotomy without cholecystectomy. Design: Prospective observational study. Setting: University hospital, Norway. Subjects: 130 patients with gallstones and acute pancreatitis. In 62 patients with common bile duct stones the bile duct was cleared by sphincterotomy and stone extraction. The remaining 68 patients had prophylactic sphincterotomy. Cholecystectomy was not planned later. Main outcome measures: Incidence of recurrent acute pancreatitis and need for cholecystectomy. Results: 21 patients were dead or not available for the follow-up. Consequently 109 patients were followed-up for a median of 39 months (range 23-62). One patient had recurrent pancreatitis. 20 patients had a cholecystectomy later for symptoms related to gallstone disease. Of the rest, 25 patients had moderate or mild gallstone-related symptoms. There was no difference in gallstone-related symptoms between those who had had stones in the bile duct and those who had not. 63 patients had no symptoms related to gallstones. Conclusion: Endoscopic sphincterotomy during or immediately after acute gallstone pancreatitis resulted in half the patients being free of symptoms during the next three years. [ABSTRACT FROM AUTHOR]

23. Closure of Burst Abdomen After Major Gastrointestinal Operations - Comparison of Different Surgical Techniques and Later Development of Incisional Hernia.

Objective: To find out the incidence of incisional hernia in patients who had resuture of a burst abdomen and to compare different methods of wound closure and the development of incisional hernia. Design: Retrospective study. Setting: University hospital, Norway. Subjects: 78 adults patients who had their burst abdomens resutured between January 1986 and December 1995. Interventions: Five different methods were used to close the burst abdomen: interrupted or continuous sutures with or without retention sutures, or retention sutures alone. Main outcome measure: Incisional hernia after at least one year follow-up. Results: Postoperative mortality was 14% (11/78), and 53 patients were followed up for at least a year. Incisional hernias developed in 43% (23/53) of the patients. When interrupted sutures were used (with or without retention sutures) 34% (13/38) of patients developed incisional hernias compared with 6/10 when the wound was closed with a continuous suture. Retention sutures did not reduce the incidence of incisional hernia. Conclusions: Incisional hernia is a common complication after resuture of a burst abdomen. We found no significant differences in the incidence of incisional hernias when continuous and interrupted techniques were compared. Retention sutures do not reduce the incidence of incisional hernias. There is still a need for refinements of the technique of closure of a burst abdomen. [ABSTRACT FROM AUTHOR]

This study was undertaken to examine the role of prostaglandins and histamine in the hyperemic response to gastric mucosal damage followed by H
+ back-diffusion. Cat stomachs were exposed to 2 mol/liter NaCl for 10 min followed by luminal perfusion at pH 1. Hypertonic saline caused extensive (microscopic) damage to the surface epithelium, increased gastric mucosal blood flow, and increased release of histamine, PGE2 , and 6-keto PGF1α (prostacyclin) into portal venous blood. The effect of indomethacin and histamine blockers (H1 +H2 ) on the hyperemic response to acid back-diffusion was related to the depth of the mucosal injury and the region of the stomach. In the corpus, indomethacin enhanced mucosal injury. In areas with superficial damage, the hyperemia was inhibited by indomethacin and antihistamines and eliminated by the combination of both. In corpus areas with indomethacin-induced deep lesions, the blood flow was very high, and this hyperemia was partly inhibited by antihistamines. In the antrum the hyperemic response was reduced by antihistamines. Indomethacin increased the release of histamine into the portal venous blood (baseline recordings) and reduced basal gastric mucosal blood flow.- Published

- Abstract
Background: There are data on the safety of cancer surgery and the efficacy of preventive strategies on the prevention of postoperative symptomatic COVID-19 in these patients. But there is little such data for any elective surgery. The main objectives of this study were to examine the safety of bariatric surgery (BS) during the coronavirus disease 2019 (COVID-19) pandemic and to determine the efficacy of perioperative COVID-19 protective strategies on postoperative symptomatic COVID-19 rates., Methods: We conducted an international cohort study to determine all-cause and COVID-19-specific 30-day morbidity and mortality of BS performed between 01/05/2020 and 31/10/2020., Results: Four hundred ninety-nine surgeons from 185 centres in 42 countries provided data on 7704 patients. Elective primary BS (n = 7084) was associated with a 30-day morbidity of 6.76% (n = 479) and a 30-day mortality of 0.14% (n = 10). Emergency BS, revisional BS, insulin-treated type 2 diabetes, and untreated obstructive sleep apnoea were associated with increased complications on multivariable analysis. Forty-three patients developed symptomatic COVID-19 postoperatively, with a higher risk in non-whites. Preoperative self-isolation, preoperative testing for SARS-CoV-2, and surgery in institutions not concurrently treating COVID-19 patients did not reduce the incidence of postoperative COVID-19. Postoperative symptomatic COVID-19 was more likely if the surgery was performed during a COVID-19 peak in that country., Conclusions: BS can be performed safely during the COVID-19 pandemic with appropriate perioperative protocols. There was no relationship between preoperative testing for COVID-19 and self-isolation with symptomatic postoperative COVID-19. - Abstract
Background: Laparoscopic gastric bypass surgery leads to significant weight loss and reduced morbidity in patients with severe obesity. The technique was introduced at the private Aleris hospital the autumn 2005. We here present the method and results from our initial experience., Material and Methods: Inclusion criteria were BMI > 40 kg/m2 or BMI 35-39,9 kg/m2 , with obesity-related co-morbidity for more than 5 years and insufficient effect of conservative treatment. The first 121 (103 women) consecutive patients who underwent gastric bypass surgery at Aleris Hospital were included. Characteristics were; mean age 40 years (18-68), mean preoperative body weight 134 kg (91-211) and BMI 44 m/kg 2 (35-64). Serious complications or major morbidity were defined as anastomosis- or intestinal leakage and bleeding requiring surgical treatment., Results: No deaths or major morbidity was observed the first 30 days after surgery. One patient underwent successful surgery of a perforated ulcer localized distally to the gastro-jejunal anastomoses 3 months p.o. Mean operating time was 62 min (40-124) for patients who underwent isolated laparoscopic bypass. The mean hospitalisation time was 2.9 days (2-6)., Conclusion: Our results demonstrate that bariatric surgery can be established successfully with a low rate of perioperative complications. We believe that thorough planning and experienced bariatric surgeons contribute to our favourable results. .

- Abstract
Background/aims: Superficial mucosal damage is associated with mucosal hyperemia and release of histamine into venous blood and gastric lumen. This study was designed to examine if histamine administrated directly to gastric mucosa becomes absorbed and transported away with blood, and if so whether it is capable of increasing gastric mucosal blood flow., Methodology: Superficial mucosal injury was induced by exposing chambered rat stomachs to 2 M NaCl for 10 min, and 45 min later 4.5 x 10(-2) or 4.5 x 10(-3) M histamine was instilled into the chamber. Mucosal blood flow was determined by radioactive microspheres and continuously by Laser Doppler flowmetry., Results: Histamine concentration in arterial blood was markedly increased 15 min after administration of histamine. The mucosal blood flow did not increase after application of histamine. Significant increase in heart rate and fall in arterial blood pressure were observed. Similar results were obtained after application of histamine to normal mucosa., Conclusions: Histamine applied to injured or normal gastric mucosa of rats becomes transported via capillaries and venules to the systemic circulation, but does not reach terminal arterioles in quantities large enough to cause vasodilatation.
