Aim: All cases of urethelial carcinoma of the bladder undergoing radical cystectomy between 1997 January and 2005 December were retriewed from the files of İzmir Ataturk Training Hospital, Pathology department and reviewed. The aim of this study was to evaluate the histopathological features (such as tumor size, presence of carcinoma in-situ, infiltration of vesicula seminalis and prostatic gland, vascular invasion, perineural invasion, and lymph node status) of these 191 radical cystectomy specimens and to detect if there was a relation between these parameters. Material and Method: The tumours were staged according the TNM staging system and graded according to grading schema proposed by World Heath Organization. The relationship between pathologic stage and grade was analysed by Spearman correlation test. Mann Whitney U tests were used to determine the relation between stage and other parameters. Grade 1 and grade 2 tumors were gathered as one group and chi-square tests were performed to determine the relation between grade and other parameters. The relationship of the other parameters with each other were analysed by chi-square tests. Results: The male/ female ratio was 13/178, and the mean age was 61,3. The pathologic stages of the cases was as follows: 15% (n=29) pTa; 12% (n=23) pT1; 26,7% (n=51) pT2; 26,2% (n= 50) pT3; 19,9% (n=38) pT4. The grades of the tumors were as follows: 3% (n=6) grade I; 12% (n=23) grade II; 84,8 % (n=162) grade III. Lymphovascular invasion was seen in 33% (n=63) of patients and perineural invasion in 8% (n=15). Lymph node metastases was observed in 29,4% (n=35) of the 119 lymph nodes extracted cases. Cases with lymphovascular invasion, perineural invasion, vesicula seminalis infiltration or lymph node metastases were composed of muscle invasive and grade III tumors. Among 178 male patients, prostatic infiltration and vesicula seminalis infiltration was present in 13% (n=23) and %9,6 (n=17) of the cases respectively. Incidental adenocarcinoma of prostat was detected in 9% (n=16) of the specimens. Conclusions: There was a statistically significant relationship between tumoral pathologic stage and grade. High pathologic stage or grade III tumors had statistically significant correlation with the presence of lymphovascular invasion and lymph node metastases. Presence of lymphovascular invasion was found to be a risk factor for the lymph node metastases. All the tumors which showed vesicula seminalis infiltration were grade III and these tumors were statistically associated with the presence of lymphovascular invasion. Key words: Bladder carcinomas, Cystectomy, Urothelial carcinoma, Amaç: Bu çalışmada 1997 Ocak -2005 Aralık yılları arasında İzmir Atatürk Eğitim ve Araştırma Hastanesi, Patoloji bölümünde ürotelyal karsinom tanısı alan 191 adet radikal sistektomi materyalinin histopatolojik özelliklerini (tümör boyutu, karsinoma insitu varlığı, vesiküla seminalis tutulumu, prostat tutulumu, lenfovasküler ve sinir invazyonu ile lenf nodu metastaz varlığı) değerlendirilerek, bu parametrelerin birbirleri ile ilişkisini araştırmak amaçlandı. Gereç ve Yöntem: Tümör evresi ve derece arasındaki ilişki spearman korelasyon analizi ile, evre ve diğer değişkenler arasındaki ilişki ise Mann-Whitney U testi ile değerlendirildi. Derece ile diğer parametreler arasındaki ilişki incelenirken derece 1 ve derece 2 tümörler bir grup altında birleştirilerek ki kare testi uygulandı. Diğer kategorik veri setlerinin karşılaştırılmasında ise ki-kare testi kullanıldı. Evrelemede TNM sınıflaması, Ürotelyal karsinomun derecelendirilmesinde ise Dünya Sağlık Örgütünün (WHO) sınıflaması kullanıldı. Bulgular: Yüzdoksanbir olgunun ( 13 kadın; 178 erkek) ortalama yaşı 61,3 idi. Olguların evrelere göre dağılımı: %15 (n=29) pT1; %12 (n=23) pT1; %26,7 (n=51) pT2; %26,2 (n= 50) pT3; %19,9 (n=38) pT4 şeklinde, derecelere göre dağılımı ise: %3 (n=6) derece I; % 12 (n=23) derece II; % 84,8 (n=162) derece III şeklinde idi. Olguların %33'ünde (n=63) lenfovasküler invazyon, %8'inde (n=15) ise sinir invazyonu vardı. Lenf nodu disseksiyonu uygulanmış 119 olgunun % 29,4'ünde (n=35) metastaz mevcuttu. Lenfovasküler invazyon, sinir invazyonu, vesikulo seminalis infiltrasyonu veya lenf nodu metastazı bulunan olguların tamamını kas invaziv ve derece III tümörler oluşturmaktaydı. Yüzyetmişsekis erkek olgunun % 13'ünde (n=23) prostat tutulumu, %9,6'sında (n=17) vesikula seminalis tutulumu gözlendi. Prostat insidental karsinomu ise 16 (%9) olguda izlendi. Sonuç : Tümör patolojik evresi ve derecesi arasında istatistiksel olarak anlamlı ilişki saptandı. İleri patolojik evreye sahip tümörler yanısıra derece III tümörlerde lenfovasküler invazyon ve lenf nodu metastazı bulunma olasılığının istatistiksel olarak arttığı izlendi. Lenfovasküler invazyonun lenf nodu metastazı için risk oluşturduğu gözlemlendi. Vesikulo seminalis infiltrasyonu olan tümörlerin hepsi derece III olup, bu olgularda lenfovasküler invazyon bulunma olasılığı istatistiksel olarak anlamlı derecede yüksek bulundu. Primary adenocarcinoma of the urinary bladder is a rare neoplasm sometimes causing diagnostic challenge because it can be indistinguishable morphologically from adenocarcinoma secondarily involving the bladder by metastasis or direct extension. Identifying the primary origin of the tumor is important in staging, determining appropriate therapy and assessing prognosis. We report herein a case of primary adenocarcinoma of the bladder misdiagnosed as metastatic adenocarcinoma and reviewed the histopathologic features and diagnostic characteristics of primary adenocarcinoma of bladder in view of literature. Amaç: Monosemptomatik enürezisli çocuklarda ve annelerinde tedavi öncesi dönemde hayat ve uyku kalitesini değerlendirmek.Gereç ve Yöntemler: İki yüz kırk olgu çalışmaya dahil edildi. Olguların 124’ü monosemptomatik enürezisli hasta ve 116’sı sağlık problemi olmayan çocuklardı. Çocuklarda yaşam kalitesi Pediatric Quality of Life Inventory PEDSQL ile değerlendirildi. Annelerde hayat kalitesi Short-Form Health Survey SF-36 ile değerlendirildi. Çocuklarda ve annelerinde hayat kalitesi Pittsburgh Sleep Quality Index PSQI ile değerlendirildi.Bulgular: Emosyonel ve okul fonksiyon skalası monosemptomatik enüzesli çocuklarda ‘child self-report and parent proxy-report’ kontrol grubuna göre anlamlı olarak yüksekti p=0.02, p=0.03 ve p=0.03, p=0.04 . Annelerin yaşam kalitesinin değerlendirildiği SF-36 ya göre emosyonel ve vücut ağrı skoru enürezisli çocukların annelerinde anlamlı olarak daha düşüktü p=0.02 ve 0.04 . Çocuklar arasında toplam PSQI skoru ve alt gruplarında anlamlı fark saptanmazken subjektif uyku kalitesi, uyku latensi ve PSQI toplam skoru enürezisli çocukların annelerinde anlamlı olarak daha yüksekti p=0.04, p=0.03 ve p=0,02 .Sonuç: Monosemptomatik enürezis tedavi öncesi dönemde çocukların ve annelerin hayat kalitesini olumsuz olarak etkileyen bir hastalıktır. Bu nedenlerden dolayı çocuklarda monosemptomatik enürezis tedavi edilmelidir, Objective: To evaluate quality of life and sleep in monosymtomatic enuretic children and their mothers before treatment. Materials and Methods: Two hundrend fourty cases were included to this study. Of the cases 124 had monosymptomatic enuresis and 116 voluntary children had no health problems. The quality of life was evaluated according to Pediatric Quality of Life Inventory PEDSQL in children. Quality of life for their mothers were evaluated using the Short-Form Health Survey SF-36 . Sleep quality of children and their mothers were evaluated according to Pittsburgh Sleep Quality Index PSQI . Results: Emotional and school functioning were significantly higher in children with monosymptomatic enuresis than that of controls for child self-report and parent proxy-report p=0.02, p=0.03 and p=0.03, p=0.04 respectively . The quality of life of the mothers as shown by SF-36, the emotional domain score and bodily pain score were significantly lower compared to controls p=0.02 and 0.04 . However PSQI total and subgroup scores were not stasitically significant in between children groups, subjective sleep quality, sleep latency domain scores and total scores in enuretic children’s mothers were significantly higher than healty group p=0.04 ,p=0.03 and p=0,02 respectively . Conclusions: Monosymptomatic enure sis is a disease that negatively affects the quality of life of children and their mothers before treatment. Urethral calculi represent less than 2% of all urinary stone disease. Primary urethral stone is a rare condition. It usually develop secondary to congenital urethral diverticulum. The majority of urethral calculi are migrant form the upper urinary system or bladder. Treatment is contingent on the size and location of the stones and condition of the urethra. We present penile urethra stone in this report, Üretra taşı üriner sistem taşlarının %2’sinden azını oluşturmaktadır. Primer üretra taşları genellikle konjenital üretral divertiküle sekonder olarak gelişen nadir bir durumdur. Üretra taşlarının çoğu üst üriner sistemden veya mesaneden üretraya migre olan taşlardır. Amaç: Bu çalışmada 7 cm’den küçük renal kitlelerde uygulanan radikal nefrektomi RN ve nefron koruyucu cerrahi NKC sonuçlarını değerlendirmeyi amaçladık.Gereç ve Yöntem: Yedi cm’den küçük renal kitle ön tanısı ile opere edilen toplam 86 hasta çalışmaya dahil edildi ve retrospektif değerlendirmeye alındı. NKC uygulanan 30 hasta 17 erkek, 13 kadın I.grup, RN uygulanan 56 hasta 31 erkek, 25 kadın II.grup olarak belirlendi. I.grupta patolojisi renal hücreli karsinom RHK saptanan 22 hastanın, II.grupta ise RHK saptanan 54 hastanın sağkalım verilerine ulaşıldı ve istatistiksel analizler bu hastalar üzerinden yapıldı. Gruplar; yaş, izlem süresi, ortalama tümör çapı, tümör evresi, histopatoloji, preoperatif ve postoperatif serum kreatinin değerleri ve sağkalım yönünden karşılaştırıldı.Bulgular: NKC uygulan I grupta histopatoloji 22 hastada RHK %73.3 , 7 hastada benign histoloji %23.3 , 1 hastada metastatik tümör %3.3 olarak rapor edilirken, RN uygulanan II.grupta 54 hastada RHK %96.4 , 2 hastada benign histoloji %3.6 , tespit edildi. Hastaların yaş ortalaması I grupta 56.33±13.18, II. grupta 66.63± 11.65 idi P, Objectives: In this study we aimed to compare the outcomes of nephrone sparing surgery NSS and radical nephrectomy RN on renal masses smaller than 7 cm.Materials and Meth ods: Eighty-six patients treated by surgery, with renal masses smaller than 7 cm were evaluated retrospectively. There were thirty patients 17 male, 13 female in the first group who underwent NSS, and fifty-six patients 31 male, 25 female in the second group who underwent RN. After histopathological evaluation, renal cell carcinoma RCC was detected in 22 patients in first group while RCC was detected 54 patients in second group and these patients were analyzed. Age, follow-up, mean tumour dimater, stage, histopathology, preoperative and postoperative serum creatinine levels and survival were compared between the groups.Results: While final pathological examination demonstrated 22 73.3% RCC, 7 23.3% benign pathology and 1 3.3% metastatic cancer in first group, 54 RCC 96.4% and 2 3.6% benign pathology was demonstrated in second group. Mezotelyal orjinli retroperitoneal kistler çok nadir rastlanılan kistlerdir. Retroperitoneal kistlerin kesin tanı ve tedavisinde standart yöntem cerrahidir. İlk tanı anında diğer retroperitoneal kitlelerden ayırımı oldukça zordur. On yaşında kız çocuğu sol hidronefrotik nonfonksiyone böbrek öntanısıyla opere edildi. Makroskopik incelemede 25 cm lik kistin kenarında yaklaşık 2 cm lik hipoplazik böbrek dokusu olduğu görüldü .Histopatolojik incelemede kistin duvarının tek sıra kübik epitelle döşeli olduğu ve immünohistokimyasal olarak sitokeratin 7 + , WT-1 + , kalretinin + saptandı ve mezotelyal kist tanısı kondu. İki cm lik böbrek dokusu ise hipoplazik böbrek ile uyumlu olarak değerlendirildi. Bu yazımızda literatürde çok az sayıda bulunan primer retroperitoneal mezotelyal kist olgusunu sunduk, The retroperitoneal cysts which have mesothelial origin are very rare. Ten-year-old girl was operated with the diagnosis of left nonfunctional hydronephrotic kidney. In macroscopic examination at the edge of the cyst about 2 cm of tissue was hypoplastic kidney. In histopathological examination, cyst wall was paved with a single row of cubical epithelium and immunohistochemically cytoceratin 7 + , WT-1 + , calretinin + was detected and the cyst was diagnosed as mesothelial cyst. Retroperitoneal bölgedeki tümörler oldukça nadir görülmekle birlikte, bu kitlelerin yaklaşık %70-80’inin malign olması ve ürolojik organlarla olan yakın ilişkisi nedeniyle önem arz etmektedirler. Ürinom; perirenal, paraüreteral veya retroperitotenal alanda, ekstravaze idrarın birikimi olarak tanımlanmaktadır. Ürinomlar genellikle obstrüksiyona sekonder veya üriner sistemde yaralanmaya neden olan künt veya delici yaralanmalar sonucu meydana gelir. Retroperitoneal kitleyi taklit eden ürinom olgusunu sunduk, Although retroperitoneal tumors are relatively uncommon, about 70 to 80% of this mass is malignant and association with urological organs. Urinoma is defined as a collection of extravasated urine in the perirenal, paraureteral or retroreritoneal space. Urinomas usually ocur secondary to obstruction or due to penetrating or blunt trauma leading to injuries of the urinary system. We present a case of an unironoma mimicking retroperitoneal mass
