- Abstract
Impacted teeth fail to fully erupt in the oral cavity mainly as a result of reduced space in the dental arch. Several studies have shown the mandibular third molar to be the most commonly impacted tooth, with a prevalence reaching 77% (1), followed by the maxillary third molar. An impacted mandibular third molar can cause the development of an infection, commonly called pericoronitis, in the follicular sac of the impacted third molar; this infection can be seen in a percentage as high as 64% (2) of patients with an impacted third molar, and can be quite painful for the patient. The usual therapy for treating an impacted tooth is its extraction, which can be called for not only for treating a tooth that has been the cause of an infection and can negatively affect nutrients intake (3), but also for maintaining the periodontal health of the second molar (4) or for orthodontic and prosthetic needs. Their extraction is particularly important in patients undergoing hematopoietic stem cell transplantation (HSCT) (5). Their extraction is usually a uneventful surgical procedure, but in rare cases it may cause several complications, such as paresthesia of the lower lip or the development of a dry socket (6). These complications have a widely different reported rate. The aim of this study is to retrospectively highlight the prevalence of these events in our clinic.

- Abstract
Background: The extraction of the mandibular third molar is one of the most frequent intervention in oral surgery. A common indication for wisdom tooth extraction is represented by pericoronitis, which can determine discomfort and pain in patients. The present study aimed to evaluate the impact of patients’ quality of life by comparing a surgical approach with a periodontal approach. Methods: We evaluated 82 patients diagnosed with pericoronitis that occurred at the third molar site. In total, 41 of them received a periodontal treatment and 41 were treated by extraction. The quality of life (QoL) of the patients was assessed by using the Oral Health Impact Profile-14 (OHIP-14) index. Results: A total of 82 patients were included in the study and were followed up for 6 months. Of the patients, 41 received a periodontal treatment and 41 underwent surgical extraction. At the baseline, the OHIP-14 scores of the surgical group were higher (19.71, SD 9.90) than the periodontal group (14.41, SD 8.71). At 1 week, there was a reduction in terms of OHIP- 14 in both groups, but the periodontal group showed lower values (12.3, SD 8.11). Long-term follow- up showed a reduction of the OHIP-14 values, with a difference in favor of the surgical group (0.10, SD 0.45). However, there was a reduction in OHIP-14 scores in both groups. Conclusion: Although the periodontal treatment offered a rapid improvement in terms of quality of life during the first week after the treatment, after 1 month and 6 months, the extraction of the mandibular third molar extraction remained the best treatment, removing the occurrence of re-inflammation of the site.

Heat therapy is commonly used to treat injured muscles, and recently, hyperthermia which has been used in oncology was introduced as a modality for use in sports medicine. The important physiological response which produces most of the beneficial effects of hyperthermia is increased blood flow (Sekins et al., 1984). Effective clinical response occurs when the temperature reaches 41 to 45 °C (Lehmann and de Lateur, 1982), increasing blood flow up to 15 times (Song, 1984). Sekins et al., 1984 reported that to produce observable variations in blood perfusion, temperature must rise above 41.5 °C as fast as possible. While there are several heating modalities, studies have shown that electromagnetic waves are more effective than other thermal modalities for treating injured muscles at depth of 1-4cm (Giombini et al., 2007). However, because of lack of research-based evidence of the microwave hyperthermia treatment, clinical and research studies need to be completed to confirm the therapeutic effectiveness of hyperthermia. We recently reported that hyperthermia treatment with a 434-MHz microwave and direct- contact applicator increased and maintained the muscle temperature locally by 6.3-11.4°C without causing muscle damage (Ichinoseki-Sekine et al., 2007). This system has also been found to be a highly innovative and reliable modality for treating acute muscle injuries (Giombini et al., 2001). However, most of the hyperthermia systems commonly used in clinical situations is equipped with a 2450-MHz microwave generator and a non-contact applicator. The possibility exists that the muscle temperature is influenced by the frequency and applicator style. Thus, the aim of this study was to investigate the changes in human muscle temperature induced by two different types of microwave hyperthermia systems. Our results could assist to solve the lack of research-based evidence for the clinical effectiveness of hyperthermia treatment. In this study two different microwave hyperthermia systems were used. One was a direct- contact microwave hyperthermia device (ALBA Hyperthermia System, Restek SRL, Rome, Italy) equipped with a 434-MHz microwave generator having a curve-shaped microstrip antenna applicator, and a silicon bolus filled with thermostatic water. The skin temperature was automatically controlled by a decrease/increase in the power output to maintain the skin pilot temperature. The microwave power source was set to turn on/off periodically as the default setting, and the temperature data were measured during the power-off phase. The other device was a non-contact microwave device (Microtizer, MT-SDi, Minato Medical Co. Ltd., Osaka, Japan) equipped with a 2450-MHz microwave applicator including a helical antenna. This system does not contain any temperature measurement system, and the skin temperature was maintained manually by reducing the power output or varying the distance between the applicator and skin surface. The settings of both hyperthermia systems were established in accordance with the manufacturers' instructions. The 434-MHz system was set with a power of 60 W, a skin baseline temperature of 40°C, and a bolus water temperature of 38°C. The applicator was placed on the lateral side of one thigh, and the center of the applicator position was adjusted to the position of the thermocouple. The 2450-MHz system was set with a power of 150 W, and the distance between the skin surface and applicator was approximately 15 cm. The skin and muscle temperatures were measured using a digital thermometer (PTW-301, Unique Medical, Tokyo, Japan) every 3 min for 10 s during the power-off phase, and the center of the applicator position was adjusted to the position of the thermocouple.Eleven healthy adult males (24.3 ± 2.2 years, 1.74 ± 0.06 m, 70.0 ± 5.3 kg; mean ± SD) participated in this study. The subjects were placed in the supine position and underwent 30 min of hyperthermia treatment with either the 434 or 2450-MHz system on different days. At least 1 week elapsed between the two measurements. All procedures described in this study were performed with the approval of the Juntendo University Human Ethics Committee and complied with the Declaration of Helsinki. All subjects gave written informed consent. The thermocouple for determining the skin temperature was placed on the belly of the vastus lateralis muscle. After anesthesia with a 60% lidocaine tape (Penles, Wyeth K.K., Tokyo, Japan), a 23-G thermocouple (IT-23, Physitemp Instruments, Clifton, NJ) was inserted into the muscle, and its temperature at a depth of 2.0 ± 0.2 cm was measured. The room temperature and humidity were controlled at 24.5 ± 0.3°C and 51.6 ± 8.9%, respectively. In addition, to determine the depth of the maximum heating point, we evaluated the vertical heating pattern using a muscle equivalent phantom (Okano, et al., 2000). After microwaves were applied, the temperature distribution on the vertical cutting surface of the phantom was recorded immediately using a thermal camera (Thermo Tracer TH71000, NEC San-ei Instruments, Tokyo, Japan).As results, the muscle temperature with the 434-MHz system showed a single peak at approximately 10 min. Significant differences were detected between the systems in peak muscle temperature, temperature rise, and time to peak temperature (p < 0.001; Table 1). The maximum heating point using the 434-MHz system (approximately 2 cm) was deeper than that of the 2450-MHz system (approximately 1 cm). The peak skin temperatures were not significantly different between the two systems.However, the changes in muscle temperature did show different patterns; a single peak at 10 min was seen with the 434-MHz system, whereas a slope was observed with the 2450-MHz system. This behaviour with the 2450-MHz system caused the substantial variation in the time to peak temperature.In general, the therapeutic range for heat treatment in sports medicine is assumed to be from 41 to 45ºC (Lehmann and de Lateur, 1982). When the local muscle temperature first exceeds a threshold of 42 to 45ºC, a rapid perfusion of cooling blood flow is induced in the high-temperature region (Sekins et al., 1982). This thermal washout reduces the temperature to prevent the muscle from overheating. Our results suggest that the 434-MHz system increased the muscle temperature to this therapeutic range and caused thermal washout. However, some subjects could not reach the therapeutic range with the 2450-MHz system, and thermal washout might not have occurred because a large temperature reduction was not observed. We showed that the radiation frequency and applicator type of a microwave hyperthermia system influence the change in human muscle temperature, but not skin temperature. According to the phantom experiment results, the maximum heating point was shallow in the 2450-MHz system; thus, the actual maximum temperature induced by the 2450-MHz system might have been higher by 1ºC than our results. Even so, the muscle temperature in some subjects may not have reached the temperature necessary to cause thermal washout. There are some studies that showed the benefits of hyperthermia at 434-MHz system. Hyperthermia has benefits in acute muscle injuries, chronic overuse tendinopathies and pain reduction (Giombini et al., 2002), with short-term clinical improvement, good safety and no side effects. The important physiological response which produces most of the beneficial effects of hyperthermia is increased blood flow, and the effective clinical response occurs when the temperature reaches 41 to 45ºC. Hyperthermia produces an increase in nutrients and oxygen in the heated region, and both two events are necessary to affect tissue repair. Our result showed that 434-MHz hyperthermia system rapidly increases muscle temperature to above 41ºC, and it support the previous reports. To obtain the effect of hyperthermia treatment efficiently, both time and temperature of application must be controlled. However, to our knowledge, no study measured the changes in human muscle temperature induced by different types of microwave hyperthermia systems. We believe that our results provided research-based evidence for the clinical effectiveness of hyperthermia treatment.

- Abstract
“Il ruolo dell’insegnante tra metodo e qualità dell’esperienza scolastica” intitola un percorso di ricerca-formazione realizzato tra aprile e ottobre 2011 con gli insegnanti delle scuole dell’infanzia, primarie e secondarie di primo grado del Comune di Cinisello Balsamo (MI). L’obiettivo della ricerca ha riguardato la possibilità di ricostruire, a partire da situazioni scolastiche concrete, in cosa consistesse il “modo di fare scuola” agito quotidianamente da ogni insegnante; come esso incidesse sulla qualità dell’esperienza che allievi e insegnanti vissuta a scuola e quindi come, a partire dalla pratica, si potesse ridefinire ciò che qualifica la professionalità dell’insegnante oggi, rispetto a situazioni cosiddette “di disagio”. Dal punto di vista pedagogico, la ricerca presuppone la specificità dell’esperienza educativa, sottolineandone da un lato la complessità intrinseca, dall’altro la problematicità delle connessioni con l’educazione informale. Inserita nell’ambito delle ricerche pedagogiche empiriche di tipo qualitativo, idiografico e intensivo, la ricerca ha sviluppato un percorso strutturato in gruppo. Riferendosi all’approccio di Clinica della Formazione, è stata animata dalla preoccupazione di consentire ai partecipanti di pervenire a una propria consapevolezza didattico-metodologica e di individuare i propri riferimenti deontologici a partire da una riflessione sull’esperienza agita. Tra i risultati, è stata evidenziata la presenza di una pratica didattico-educativa esperta e in divenire, che, se riesce a fronteggiare situazioni difficili, tuttavia non viene legittimata come “modalità di fare scuola” nel confronto con modelli di insegnamento tradizionali, a cui, pur riconoscendo l’anacronismo rispetto alla situazione attuale, insegnanti e dispositivi scolastici sembrano comunque riferirsi. A novel series of 1-acetyl-3-(4-hydroxy- and 2,4-dihydroxyphenyl)-5-phenyl-4,5-dihydro-(1H)-pyrazole derivatives
1 −12 have been synthesized and investigated for the ability to selectively inhibit the activity of the A and B isoforms of monoamine oxidase (MAO). The new synthesized compounds1 −12 proved to be more reversible, potent, and selective inhibitors of MAO-A than of MAO-B. Knowing that stereochemistry may be an important modulator of biological activity, we performed the semipreparative chromatographic enantioseparation of the most potent, selective, and chiral compounds,6 and11 . The separated enantiomers were then submitted to in vitro biological evaluation while increasing their inhibitory activity and A selectivity. The (−)-6 enantiomer shows Ki(MAO - A) = 2 nM and SI = 165 000, (+)-6 shows Ki(MAO - A) = 6 nM and SI = 166 666, (−)-11 shows Ki(MAO - A) = 4 nM and SI = 80 000, and (+)-11 shows Ki(MAO - A) = 7 nM and SI = 38 571.- Published

3-(1H-Pyrrol-1-yl)-2-oxazolidinones
1a −i have been synthesized as pyrrole analogues of toloxatone (Humoryl), an antidepressant agent belonging to the 3-phenyl-2-oxazolidinone class, and their monoamine oxidase (MAO) type A and B inhibitory activities have been evaluated. The majority of1a −i showed inhibitory activity against the A isoform of the enzyme higher than that exerted against the MAO-B, the sole exception being the (S)-5-aminomethylderivative1d . (R)-5-Methoxymethyl-3-(1H-pyrrol-1-yl)-2-oxazolidinone1b , the most potent among test derivatives, was 78-fold more potent than toloxatone.- Published

The official inquiry on osteoporosis in Italy, promoted by the Italian Senate in 2002 concluded that proper preventive strategies should be adopted at regional level in order to prevent osteoporotic fractures. Tuscany is the first Italian region who has promoted an official program (the TARGET project) aimed to reduce osteoporotic fractures by ensuring adequate treatment to all people aged ≥65 years old who experience a hip fragility fracture.this paper provides information concerning the implementation of TARGET project in Tuscany, assuming that it may represent an useful model for similar experiences to be promoted in other Italian Regions and across Europe.we have examined the model proposed for the regional program, and we have particularly analyzed the in-hospital and post-hospitalization path of hip fractured patients aged65 years old in Tuscany after the adoption of TARGET project by Tuscany healthcare system and during its ongoing start-up phase.orthopaedic surgeons have been gradually involved in the project and are increasingly fulfilling all the clinical prescriptions and recommendations provided in the project protocol. Different forms of cooperation between orthopaedic surgeons and other clinical specialists have been adopted at each hospital for the treatment of hip fractured elderly patients. GPs involvement needs to be fostered both at regional and local level.The effort of Tuscany region to cope with hip fractures suffered from elderly people must be acknowledged as an interesting way of addressing this critical health problem. Specific preventive strategies modelled on the Tuscany TARGET project should be implemented in other Italian regions.

“Il ruolo dell’insegnante tra metodo e qualità dell’esperienza scolastica” intitola un percorso di ricerca-formazione realizzato tra aprile e ottobre 2011 con gli insegnanti delle scuole dell’infanzia, primarie e secondarie di primo grado del Comune di Cinisello Balsamo (MI). L’obiettivo della ricerca ha riguardato la possibilità di ricostruire, a partire da situazioni scolastiche concrete, in cosa consistesse il “modo di fare scuola” agito quotidianamente da ogni insegnante; come esso incidesse sulla qualità dell’esperienza che allievi e insegnanti vissuta a scuola e quindi come, a partire dalla pratica, si potesse ridefinire ciò che qualifica la professionalità dell’insegnante oggi, rispetto a situazioni cosiddette “di disagio”. Dal punto di vista pedagogico, la ricerca presuppone la specificità dell’esperienza educativa, sottolineandone da un lato la complessità intrinseca, dall’altro la problematicità delle connessioni con l’educazione informale. Inserita nell’ambito delle ricerche pedagogiche empiriche di tipo qualitativo, idiografico e intensivo, la ricerca ha sviluppato un percorso strutturato in gruppo. Riferendosi all’approccio di Clinica della Formazione, è stata animata dalla preoccupazione di consentire ai partecipanti di pervenire a una propria consapevolezza didattico-metodologica e di individuare i propri riferimenti deontologici a partire da una riflessione sull’esperienza agita. Tra i risultati, è stata evidenziata la presenza di una pratica didattico-educativa esperta e in divenire, che, se riesce a fronteggiare situazioni difficili, tuttavia non viene legittimata come “modalità di fare scuola” nel confronto con modelli di insegnamento tradizionali, a cui, pur riconoscendo l’anacronismo rispetto alla situazione attuale, insegnanti e dispositivi scolastici sembrano comunque riferirsi. Lavorare su tale legittimazione potrebbe consentire a insegnanti e scuola di costruire esperienze significative certo dal punto di vista pedagogico e didattico, ma anche dal punto di vista esistenziale, ovvero dell’impatto dell’esperienza scolastica sul “modo di essere-nel-mondo” degli allievi (ma anche degli insegnanti).

27. The Effect of Hyperlipidemia on Peri-implant Health: A Clinical and Radiographical Prospective Study.

High levels of cholesterol and triglycerides may have a negative effect on the immune system and bone health, leading to lower bone mineral density, an increased risk of osteoporosis, and bone fractures, and could therefore also be related to a significant worsening of peri-implant health. The purpose of the following study was to evaluate whether the altered lipid profile in patients who undergo implant insertion surgery represents a prognostic factor capable of influencing clinical outcomes. This prospective observational study was conducted on 93 subjects; patients were required to have taken blood tests to obtain triglycerides (TG), total cholesterol, low-density lipoprotein (LDL), and high-density lipoprotein (HDL) levels prior to the surgical procedure to classify them according to current American Heart Association guidelines. The outcomes considered were marginal bone loss (MBL) 3 years after implant placement, full-mouth plaque score (FMPS), and full-mouth bleeding score (FMBS) 3 years after surgery. A statistically significant correlation was found between hypertriglyceridemia and MBL as well as between total cholesterol and MBL. There is no statistically significant correlation between the variables analyzed and the secondary outcomes 3 years after implant placement. Peri-implant marginal bone loss may be influenced by hyperlipidemia. 28. Polyextremophilic Chitinolytic Activity by a Marine Strain (IG119) of Clonostachys rosea .

The investigation for novel unique extremozymes is a valuable business for which the marine environment has been overlooked. The marine fungus Clonostachys rosea IG119 was tested for growth and chitinolytic enzyme production at different combinations of salinity and pH using response surface methodology. RSM modelling predicted best growth in-between pH 3.0 and 9.0 and at salinity of 0-40‱, and maximum enzyme activity (411.137 IU/L) at pH 6.4 and salinity 0‱; however, quite high production (>390 IU/L) was still predicted at pH 4.5-8.5. The highest growth and activity were obtained, respectively, at pH 4.0 and 8.0, in absence of salt. The crude enzyme was tested at different salinities (0-120‱) and pHs (2.0-13.0). The best activity was achieved at pH 4.0, but it was still high (in-between 3.0 and 12.0) at pH 2.0 and 13.0. Salinity did not affect the activity in all tested conditions. Overall, C. rosea IG119 was able to grow and produce chitinolytic enzymes under polyextremophilic conditions, and its crude enzyme solution showed more evident polyextremophilic features. The promising chitinolytic activity of IG119 and the peculiar characteristics of its chitinolytic enzymes could be suitable for several biotechnological applications (i.e., degradation of salty chitin-rich materials and biocontrol of spoiling organisms, possibly solving some relevant environmental issues).

31. Piercing and Oral Health: A Study on the Knowledge of Risks and Complications.

The aim of the present study is to verify the knowledge of risks and complications of oral piercings, and to observe the main complications associated with piercings, using a sample from central Italy of patients wearing intraoral piercings. Through piercing and tattoo studios selected randomly in Rome, Latina and Campobasso, and a tattoo and piercing convention in Latina, a group of 387 individuals with oral piercings were selected and asked to complete an anonymous questionnaire. After filling in questionnaires, 70 individuals of the 387 selected agreed to be visited to allow the observation of the integrity of their teeth and gums (especially close to the oral piercing), oral hygiene conditions, piercing cleaning, bad habits and gingival recession. Among the respondents, 46.8% said they had not been informed about these risks, 48.5% claimed not to clean the piercing, 70.6% stated that they had not been made aware of gingival problems that can arise, 60.4% subjects stated that they were not informed about the complications of piercings concerning teeth, 52.8% had insufficient oral hygiene conditions, 42% showed signs of generalised gingivitis, 20% had 3-4 mm recessions and 22% had tooth fracture(s) due to piercing. From this study, it emerged that oral piercings can represent a risk to oral health and that there is a widespread lack of awareness of the complications and correct methods of maintaining oral piercings. Periodic checks by both dentists and dental hygienists, for patients with oral piercings, could play a decisive role in preventing, intercepting and treating the complications that they can cause.

32. The Role of Biosimilars in Uveitis: Long-Term Real-World Outcomes of the Switch From Original to Biosimilar TNF-Alpha Inhibitors.

Background: Recent expiry of patents for tumor necrosis factor (TNF)-α inhibitors has led to the employment of biosimilars in clinical practice. The aim of the study was to identify any change in the control of ocular inflammatory manifestations among patients with non-infectious uveitis switching from an originator to a corresponding anti-TNF-α biosimilar. Methods: Thirty-seven consecutive patients (62 eyes involved) with non-infectious uveitis undergoing the switch from anti-TNF-α originators to biosimilars were retrospectively enrolled; the frequency of ocular flares before and after the switch as well as best corrected visual acuity (BCVA), central macular thickness (CMT), daily systemic corticosteroid intake, and frequency of uveitic macular edema (UME) at the switch and at the following assessments were statistically analysed. Results: The number of ocular flares during the 12 months preceding the switch was 16, corresponding to 3.6 flares/100 patients/12 months; the number of flares after the switch was 14, corresponding to 2.0 flares/100 patients/12 months. No statistically significant differences were identified in the frequency of flares ( p = 0.84) and in the number of patients experiencing ocular flares ( p = 0.39) between the twelve months preceding the switch and the period thereafter. No statistically significant changes were observed in the BCVA ( p = 0.27), CMT ( p = 0.50), frequency of UME ( p = 0.57) and daily corticosteroid intake ( p = 0.42) between the time of the switch and the last follow-up visit. Introduction: Description of the lifestyles of employees of the Siena University Hospital (Azienda Ospedaliera Universitaria Senese: AOUS), as assessed through a Workplace Health Promotion (WHP) project in the two-year period 2017-18; assessment of possible short-term effects of integrated health promotion interventions in the workplace, within the framework of the Tuscany WHP network, as applied in the AOUS and involving about 1,000 workers., Methods: A cross-sectional study and a pre-post evaluation of data collected by means of anonymous questionnaires in two phases: before the beginning of the programme and after 12 months., Results: Twelve months after the start of the programme regarding diet (consumption of fruit and vegetables) and physical activity, the positive effects that emerged were not statistically significant. No differences were observed between gender or professional categories. The employees' perception of the programme was satisfactory., Conclusions: Albeit within the methodological limits of the assessment, the results showed that the diffusion of some major risk factors for chronic diseases had not decreased after 12 months' exposure to the programme. However, monitoring of these risk factors needs to be continued over a longer period, in order to detect the appearance of the expected changes in the long term. Moreover, it is essential to continue monitoring by sex and professional category, in order to pick out any differences and, if possible, take remedial actions. Further studies, in collaboration with the pertinent physicians, are desirable, since integrating data collected during health surveillance with a limited set of indicators of general risk factors may help to promptly identify possible health needs among employees., (©2019 Pacini Editore SRL, Pisa, Italy.)

34. High Production of Chitinolytic Activity in Halophilic Conditions by a New Marine Strain of Clonostachys rosea .

Twenty-eight fungal strains have been isolated from different natural marine substrates and plate screened for their production of chitinolytic activity. The two apparent best producers, Trichoderma lixii IG127 and Clonostachys rosea IG119, were screened in shaken cultures in media containing 1% colloidal chitin, 1% yeast nitrogen base and 38‰ NaCl, for their ability to produce chitinolytic enzymes under halophilic conditions. In addition, they were tested for optimal growth conditions with respect to pH, salinity and temperature. The Trichoderma strain appeared to be a slight halotolerant fungus, while C. rosea IG119 clearly showed to be a halophilic marine fungus, its optimal growth conditions being very coherent for life in the marine environment (i.e., pH 8.0, salinity 38‰). Due to its high and relatively fast activity (258 U/L after 192 h of growth) accompanied by its halophilic behaviour (growth from 0 to 160‰ of salinity), C. rosea was selected for further studies. In view of possible industrial applications, its medium for chitinolytic enzyme production was optimized by Response Surface Methodology using 1% colloidal chitin and different concentrations of corn step liquor and yeast nitrogen base (0-0.5%). Time course of growth under optimized condition showed that maximum activity (394 U/L) was recorded after 120 h on medium containing Corn Steep Liquor 0.47% and Yeast Nitrogen Base 0.37%. Maximum of productivity (3.3 U/Lh) was recorded at the same incubation time. This was the first study that demonstrated high chitinolytic activity in a marine strain of C. rosea.

- Abstract
Early hospital readmission after an episode of Acute Decompensated Heart Failure (ADHF) is an emerging issue that is causing a relevant clinical and economic burden. Although there might be several reasons for early readmissions, in many cases these might be effectively prevented by a more adequate post-discharge management, including recommendations on lifestyle and rehabilitation programs. However, almost half of hospitalizations are unrelated to specific cardiac causes and thus increases the difficulty in analyzing risks prediction. Many episodes are related to social environment, poor familiar assistance and inadequate followup program. In addition, the national and insurance companies constantly quest for a reduction of costs that could lead to inappropriately shortened hospital stays. Therefore, the suitability of early re-hospitalization as a correct target for good medical practice is highly debated. Nevertheless, the post-discharge phase after episodes of ADHF remains poorly analyzed in clinical trials and specific investigations should be considered during the transition period from acute to chronic status. A validated program, which focuses on an appropriate risk algorithm including cardiac and extracardiac precipitating factors is lacking. - Abstract
Headache occipital neuralgia is an example of pain-disease for which treatment both pharmacological protocols and invasive methods are used. Among the latter, the RF (Radiofrequency) pulse-dose has been of interest for the prospects of analgesic efficacy, safety and patient compliance, although at the moment only data concerning the pulsed RF and not the RF pulse-dose, that represents its evolution, are discussed in scientific literature. The purpose of this study is a "simple" economic evaluation of this method in headache occipital neuralgia.

37. Compliance to antifracture treatments in Tuscany: a regional survey based on institutional pharmaceutical dataset.

- Abstract
Objective: compliance to any antifracture therapy is the most important parameter affecting the effectiveness of the treatment. The aim of this study was to evaluate patients compliance to antifracture therapies in the whole Tuscany population who benefit from treatments reimbursed by the Regional Healthcare System., Methods: we have analyzed all antifracture drug prescriptions recorded in Tuscany regional pharmaceutical database concerning year 2009, from both in-hospital distribution database (direct drug delivery, FED), and private pharmacies across the whole region (SPF). Patients who started the treatment in 2008 and those continuing it in 2010 were also considered in the analysis. The sample size consisted in 92,250 people (1:9 male to female ratio). Compliance to antifracture treatments at 3, 6, 9, and 12 months was analyzed by single drug (using the defined daily dose, DDD), and by age group (40-50, 50-60, 60-70, 70-80, 90-100 years)., Results: patients compliance decreased under 80% after the first 3 months of treatment, reaching no more than 50% at 1 year. Our results show that compliance to antifracture treatments reflects the age of the patients. People aged 70-80 years old represent the age group most frequently treated with antifracture therapies (36.57% of total prescriptions), with alendronic acid being the most prescribed drug (29.73% of total drug prescribed). Monthly dosing did not increase compliance if compared to oral weekly regimens, while daily oral or s.c. dosing were associated to lower compliance rates., Conclusion: serious efforts need to be implemented to foster patients motivation in assuming their antifracture treatments for at least one year.

- Abstract
Background: Since the first outbreak of a respiratory illness caused by H1N1 virus in Mexico, several reports have described the need of intensive care or extracorporeal membrane oxygenation (ECMO) assistance in young and often healthy patients. Here we describe our experience in H1N1-induced ARDS using both ventilation strategy and ECMO assistance., Methods: Following Italian Ministry of Health instructions, an Emergency Service was established at the Careggi Teaching Hospital (Florence, Italy) for the novel pandemic influenza. From Sept 09 to Jan 10, all patients admitted to our Intensive Care Unit (ICU) of the Emergency Department with ARDS due to H1N1 infection were studied. All ECMO treatments were veno-venous. H1N1 infection was confirmed by PCR assayed on pharyngeal swab, subglottic aspiration and bronchoalveolar lavage. Lung pathology was evaluated daily by lung ultrasound (LUS) examination., Results: A total of 12 patients were studied: 7 underwent ECMO treatment, and 5 responded to protective mechanical ventilation. Two patients had co-infection by Legionella Pneumophila. One woman was pregnant. In our series, PCR from bronchoalveolar lavage had a 100% sensitivity compared to 75% from pharyngeal swab samples. The routine use of LUS limited the number of chest X-ray examinations and decreased transportation to radiology for CT-scan, increasing patient safety and avoiding the transitory disconnection from ventilator. No major complications occurred during ECMO treatments. In three cases, bleeding from vascular access sites due to heparin infusion required blood transfusions. Overall mortality rate was 8.3%., Conclusions: In our experience, early ECMO assistance resulted safe and feasible, considering the life threatening condition, in H1N1-induced ARDS. Lung ultrasound is an effective mean for daily assessment of ARDS patients.

Introduction: Critically ill patients who require intensive care unit (ICU) treatment may experience psychological distress with increasing development of psychological disorders and related morbidity. Our aim was to determine whether intra-ICU clinical psychologist interventions decrease the prevalence of anxiety, depression and posttraumatic stress disorder (PTSD) after 12 months from ICU discharge., Methods: Our observational study included critical patients admitted before clinical psychologist intervention (control group) and patients who were involved in a clinical psychologist program (intervention group). The Hospital Anxiety and Depression Scale (HADS) and Impact of Event Scale-Revised questionnaires were used to assess the level of posttraumatic stress, anxiety and depression symptoms., Results: The control and intervention groups showed similar demographic and clinical characteristics. Patients in the intervention group showed lower rates of anxiety (8.9% vs. 17.4%) and depression (6.5% vs. 12.8%) than the control group on the basis of HADS scores, even if the differences were not statistically significant. High risk for PTSD was significantly lower in patients receiving early clinical psychologist support than in the control group (21.1% vs. 57%; P < 0.0001). The percentage of patients who needed psychiatric medications at 12 months was significantly higher in the control group than in the patient group (41.7% vs. 8.1%; P < 0.0001)., Conclusions: Our results suggest that that early intra-ICU clinical psychologist intervention may help critically ill trauma patients recover from this stressful experience.

40. The TARGET project in Tuscany: the first disease management model of a regional project for the prevention of hip re-fractures in the elderly.

- Abstract
Background: The official inquiry on osteoporosis in Italy, promoted by the Italian Senate in 2002 concluded that proper preventive strategies should be adopted at regional level in order to prevent osteoporotic fractures. Tuscany is the first Italian region who has promoted an official program (the TARGET project) aimed to reduce osteoporotic fractures by ensuring adequate treatment to all people aged ≥65 years old who experience a hip fragility fracture., Objective: this paper provides information concerning the implementation of TARGET project in Tuscany, assuming that it may represent an useful model for similar experiences to be promoted in other Italian Regions and across Europe., Methods: we have examined the model proposed for the regional program, and we have particularly analyzed the in-hospital and post-hospitalization path of hip fractured patients aged >65 years old in Tuscany after the adoption of TARGET project by Tuscany healthcare system and during its ongoing start-up phase., Results: orthopaedic surgeons have been gradually involved in the project and are increasingly fulfilling all the clinical prescriptions and recommendations provided in the project protocol. Different forms of cooperation between orthopaedic surgeons and other clinical specialists have been adopted at each hospital for the treatment of hip fractured elderly patients. GPs involvement needs to be fostered both at regional and local level., Conclusions: The effort of Tuscany region to cope with hip fractures suffered from elderly people must be acknowledged as an interesting way of addressing this critical health problem. Specific preventive strategies modelled on the Tuscany TARGET project should be implemented in other Italian regions.
