Clinically relevant parameters, such as stress intensity factor of bilayered resin composite structure with short fiber base and its stability over time, has yet to be investigated. This study investigated the stress intensity factor of pre-cracked bilayered specimens composed of short fiber resin composite base (SFC) and particulate filler resin composite (PFC) as veneering layer, with a crack located in the PFC layer, 0.5 mm away from the PFC-SFC interface. Monolayered specimens served as controls. All specimens were stored in water at 37°C either for 1 week, 1 month or 6 months before testing. Two-way ANOVA (p=0.05) was used to determine the differences among the groups. Results indicated that SFC base improve the brittleness of the PFC. The type of short fibers affected the crack propagation; fiber bridging in millimeter-scale SFC was the main crack arresting mechanism, whereas fiber pulling observed in micrometer-scale SFC mainly deviated the crack path.

Objectives The survival rate of indirect partial adhesive restorations on vital versus endodontically treated teeth is still controversial. The hypothesis is that there may be a difference in the survival rate of partial adhesive restorations performed on non-vital teeth compared to vital teeth. Materials and methods This systematic review was conducted following the PRISMA guidelines. The considered clinical studies investigated the outcomes of adhesive inlays, onlays, and overlays conducted over the past 40 years, focusing on Kaplan–Meier survival curves to calculate the hazard ratio (primary objective) and the survival rate (secondary objective) between vital and non-vital teeth. The risk of bias was assessed using the Newcastle–Ottawa Scale. Studies included in the review were identified through bibliographic research on electronic databases (“PubMed,” “Scopus,” “Cochrane Central Register of Controlled Trial,” and “Embase”). The K agreement between the two screening reviewers was evaluated. Results A total of 55,793 records were identified on PubMed, Scopus, and other bibliographic sources, and after the application of the eligibility and inclusion criteria, eight articles were included for qualitative analysis and six for quantitative analysis. The meta-analysis of the primary and secondary outcomes demonstrated that hazard ratios (HR = 8.41, 95% CI: [4.50, 15.72]) and survival rates (OR = 3.24, 95% CI: [1.76, 5.82]) seemed more favorable for indirect partial adhesive restorations on vital teeth than for those on endodontically treated teeth. Conclusions Within the limits of this study, these findings suggest that the risk of failure of indirect partial adhesive restorations on endodontically treated teeth is higher than on vital teeth. Clinical relevance The use of partial adhesive restorations on vital and endodontically treated teeth showed different long-term clinical outcomes.

Objectives Compare failure modes and fracture origins using fractography on recovered clinically fractured parts of indirect resin composite endocrowns and overlay restorations on endodontically treated teeth (ETT). Methods Four endocrowns (3 molars, 1 premolar) and one overlay (molar) adhesively luted on ETT were recovered after fracturing during function. The time in service ranged between 4 and 48 months. The composite materials were (i) CAD/CAM LAVA Ultimate (N = 1), (ii) Premise Indirect (N = 2), and (iii) Colombus (N = 2). Fractography was performed by means of digital microscopy and SEM. Occlusal surfaces were checked for signs of fatigue degradation and contact wear. Cuspal plane angles were measured from profiles obtained from 3D digital microscope images with respect to the horizontal plane of the occlusal central crown groove. Results All five cases showed a wedge-opening mode I fracture, splitting the crown and tooth in two parts through the crown’s central groove. Classic brittle fracture features (arrest lines, twist and wake hackle) were easily identified on the fracture surfaces. Multiple origins were located along the central groove in conjunction with the presence of fatigue cracks. Contact wear surfaces showed pitting and cracking. Cuspal plane angles were around 30–35°, except a 50° palatal cusp slope for the Lava Ultimate overlay. Significance Fractography on clinical fractures of resin composites was enlightening. Occlusal surface fatigue degradation from cyclic loading, mode I fracture from applied mastication forces on cuspal planes, and stress concentration within the crown’s central groove, indicate limitations of use of these materials for endocrowns in posterior teeth.

Objectives To evaluate different direct restoration techniques on various cavity designs in anterior endodontically treated teeth (ETT). Materials and methods Ninety upper central incisors (n = 90) were selected, endodontically treated, and divided into three groups (n = 30) accordingly to the cavity design: minimal endodontic cavity access (group A), endodontic access + mesial class III cavity (group B), and endodontic access + two class III cavities (group C). Three subgroups (n = 10) were then created accordingly to the restoration technique: nano hybrid composite restoration (subgroup a), glass fiber post + dual-cure luting cement (subgroup b), and bundled glass fiber + dual-cure luting cement (subgroup c). Samples underwent micro-CT scan, chewing simulation, and a second micro-CT scan. 3D quantification (mm3) of interfacial gap progression was performed; then, samples underwent fracture resistance test. Data were statistically analyzed setting significance at p < 0.05. Results Groups A and B showed significantly lower interfacial gap progression compared with group C. Subgroup b performed significantly better compared with subgroups a and c. Improved fracture strength was reported for group C compared with group A, while both subgroups b and c performed better than subgroup a. Conclusions Cavity design significantly influenced interfacial gap progression and fracture resistance. Fiber posts significantly lowered gap progression and improved fracture resistance while bundled fibers only increased fracture resistance. A significant reduction of non-repairable fractures was recorded when fibers were applied. Clinical relevance A minimally invasive approach, conserving marginal crests, should be applied whenever possible. Inserting a fiber post is indicated when restoring anterior ETT, in order to reduce gap progression, improve fracture resistance, and avoid catastrophic failures.

This study evaluated the effectiveness of chemical-based adhesive techniques on promoting immediate and aged bond strength between zirconia and luting cement. A total of 128 discs of zirconia were divided into 4 groups (n = 32) according to the adhesive treatment: tribochemical silica-coating followed by silane (Silane Primer, Kerr) and bonding (Optibond FL, Kerr), Signum Zirconia Bond (Hereaus), Z-Prime Plus (Bisco), and All-Bond Universal (Bisco). Composite cylinders were cemented on the zirconia sample with Duo-Link Universal (Bisco). Eight specimens per group were subjected to 10,000 thermocycles and subsequently bond strength was tested with shear-bond strength test. ANOVA test showed that artificial aging significantly affected the bond strength to zirconia. Bonferroni test highlighted a significant influence of adhesive treatment (Signum) on bond strength after thermocycling. It was concluded that 10-MDP-based bonding systems showed no improvement in initial bond strength compared with tribochemical treatment. All chemical bonding techniques tested in this study were influenced by thermocycling.

The purpose of this in-vitro study was to evaluate the fracture resistance and failure mode of non-invasively reinforced endodontically treated mandibular molars. Sixty freshly extracted defect-free mandibular molars were divided into four experimental groups with extensive MOD cavities on endodontically treated teeth with different restoration types and one control group with intact teeth (n = 12). The groups were as follows: “Normal”: direct resin composite; “Ring”: glass fiber-reinforced strip (Dentapreg) wrapped around buccal and lingual walls followed by direct resin composite; “Inlay”: indirect CAD/CAM resin composite inlay; “Onlay”: indirect CAD/CAM resin composite onlay; “Intact”: Intact teeth (Control). Tetric EvoCeram and Adhese Universal (Ivoclar Vivadent) were used for direct restorations and Tetric CAD (Ivoclar Vivadent) adhesively luted with Adhese Universal and Variolink Esthetic LC (Ivoclar Vivadent) were used for indirect restorations. All teeth were submitted to thermo-mechanical cyclic loading. All samples were then submitted to a compressive load until fracture. Fracture load was noted and teeth were analyzed to classify the failure mode as either catastrophic (C) or non-catastrophic (NC). No statistically significant difference was found between fracture strength of the five groups when all specimens were considered (p = 0.1461). Intact group showed the lowest percentage of catastrophic failures (41.67%). Ring group presents less catastrophic failures (75%) than Normal group (83.34%), and failures of indirect restorations—Inlay and Onlay—were almost all catastrophic (91.67% and 100%, respectively).

Purpose: To explore fracture strength and failure behaviour of minimally invasive CAD-CAM composite resin overlay restorations. Methods: Eighty bi- and tri-layer cylindrical overlay model including the restoration bonded over bovine tooth dentin (Groups D) and enamel-dentin (Groups E) were assembled (diameter 9 mm). Restorations were milled from CAD-CAM composite resin blocks (Brilliant Crios, Coltène/Whaledent AG) in different thicknesses (0.5mm, 1mm, 1.5mm, 2mm) and equally distributed in four Groups D and four Groups E (n=10). All specimens were submitted to an Hertzian load-to-failure contact test with spherical indenter. Critical loads were recorded in Newton and data were analysed using Kruskal–Wallis test for multiple and Mann-Whitney test for 2-samples comparisons (p < 0.05). Fragments were examined using SEM. The stress distribution for specimens with restorations of 0.5 mm and 2 mm was also investigated with FEA. Results: For all specimens, the mean static loads in Newton increased with an increase in restoration thickness. On contrary, restorations with the same thickness displayed higher resistance values when bonded over enamel than dentin, except for the 2-mm thick restorations. A damage competition was detected between cone/median cracks originating at the loading contact area of the restorations and radial cracks beginning at their inner surface, with the former prevailing in restorations bonded on enamel and the latter being dominant for restorations bonded on dentin. Conclusions: For bonded ultra-thin resin composite restorations (0.5 mm to 1.5 mm) enamel as bonding substrate assures higher critical loads to fracture than dentin. Objectives: To explore fatigue limits of ceramic endocrowns for premolars. Methods: Forty-eight devitalized premolars were cut at the CEJ. They were restored with standardized CAD–CAM lithium disilicate reinforced ceramic restorations (IPS e.max CAD, Ivoclar-Vivadent) and divided into four Groups (n = 12): overlays (Group A, no endo-core, negative control), endocrowns with an endo-core of 2 mm (Group B), 4 mm (Group C) and crowns with post and core (Group D, positive control). All specimens were first submitted to thermo-mechanical cyclic loading (TCML)(1.7 Hz, 49 N, 600000 cycles, 1500 thermo-cycles). Margins were analysed before and after the loading. Survived specimens were then submitted to cyclic isometric stepwise loading (5 Hz, 200N to 1200N) until completion of 105000 cycles or failure. In case of fracture, fragments were analysed using SEM and failure mode was determined. Results of stepwise loading were statistically analysed by Kaplan–Meier life survival analysis and log rank test (p = 0.05). Results: All the specimens survived the TCML test except four specimens of Group A (early restorations’ debonding). No difference in percentages of closed margins was found between endocrowns (Groups B, C) and crowns (Group D). After the stepwise test, differences in survival within the groups were not statistically significant. Most of restorations experienced non-reparable fracture. Conclusions: Endocrowns with both 2-mm and 4-mm long endo-cores displayed outcomes after fatigue equivalent to classical crowns. Results of this study discourage the use of flat overlays with only adhesive retention. Clinical significance: When restoring extremely destroyed devitalized premolars, adhesive strategies should be coupled to a macro-mechanical retention in the root.

We subjected a commercial dental composite formed by interconnecting polymer-ceramic scaffolds to extensive quasi-static, static and cyclic experiments under biaxial flexure. By this we meant to obtain static and cyclic subcritical crack growth exponents that, based on established relationships describing the degradation of frictional bridging mechanisms, challenging the notion of a suggested R-curve behavior. By exploring the fracture statistics of specimens with increasing effective volumes and effective areas, we demonstrated the presence of a bi-modal defect size distribution in disaccord with the Weibull behavior across length scales. Lifetime distributions seemed to follow the strength distributions, and were used to derive crack growth velocity diagrams for combined levels of applied stress. Ultimately, the claim of an R-curve behavior could not be supported based on the absence of any significant cyclic fatigue effect, i.e. bridging degradation.

Cracked endodontically treated molars (ETMs) are commonly treated with full crowns. Less invasive techniques could include fiber-reinforced composite (FRC) base and adhesively bonded endocrowns. The purpose of this study was to assess the fatigue resistance of ETMs restored with CAD/CAM resin composite endocrowns and reinforced with different FRC bases. Cracks were simulated in fifty mandibular molars by cutting the teeth longitudinally and reassembling the two parts. After performing endodontic treatments and preparing cavities, teeth were restored in one of the following methods (n = 10). In Group 1, cavity floors were lined with 0.5 mm of flowable composite (Universal Injectable; GUI, shade A2). In Groups 2 and 3, the cavity floors were covered with one and three layers of FRC-net (ES Net) respectively, whereas in Groups 4 and 5 with 1 mm and 2 mm of flowable FRC-resin (EverX Flow Dentin; EXFD) respectively. Endocrowns (Cerasmart 270; CE, A2 HT) with 5 mm thickness were adhesively luted with preheated composite and all specimens were subjected to cyclic loading in water at 5 Hz, starting with a load of 600 N (5000 cycles), followed by stages of 1000, 1300, 1600, 1900, 2200 and 2500 at a maximum of 20 000 cycles each. Results were analyzed using Kaplan-Meier survival analysis and the Log-Rank test (p = 0.05). The fatigue failure load of ES Net applied in three layers (1617 N ± 371) or in one layer (1499 N ± 306), as well as EXFD applied in 2 mm (1496 N ± 264) or in 1 mm (1434 N ± 372) did not differ significantly (p > 0.05). Control teeth fractured at 1255 N ± 350 (p > 0.05). In the fiber-reinforced groups, restorable fractures were observed in 50%-80%, with only 30% deemed restorable in the control group. The fractures originated mainly at the occlusal contact points and the main crack propagated in the corono-apical direction. No crack origin (primary or secondary) starting from the artificial pre-existing crack was observed in fractured specimens. FRC bases did not improve the fatigue resistance of cracked ETMs, but favorably influenced the failure mode.

Objective To evaluate the influence of different types of modifications with resin on fatigue resistance and failure behavior of CAD–CAM resin nano ceramic (RNC) restorations for maxillary first premolars. Methods Sixty standardized resin composite root dies received CAD–CAM RNC endocrowns (n = 30) and crowns (n = 30) (Lava Ultimate, 3M Espe). Restorations were divided into six groups: full anatomic endocrowns (group A) and crowns (group D), buccal resin veneered endocrowns (group B) and crowns (group E) and buccal resin veneered endocrowns (group C) and crowns (group F) with a central groove resin filling. A nano-hybrid resin composite was used to veneer the restorations (Filtek Supreme, 3M Espe). All specimens were first submitted to thermo-mechanical cyclic loading (1.7 Hz, 49 N, 600 000 cycles, 1500 thermo-cycles) and then submitted to cyclic isometric stepwise loading (5 Hz) until completion of 105 000 cycles or failure after 5000 cycles at 200 N, followed by 20 000 cycles at 400 N, 600 N, 800 N, 1000 N and 1200 N. In case of fracture, fragments were analyzed using SEM and modes of failure were determined. Results were statistically analyzed by Kaplan–Meier life survival analysis and log rank test (p = 0.05). Results The differences in survival between groups were not statistically significant, except between groups D and F (p = 0.039). Endocrowns fractured predominantly with a mesio-distal wedge-opening fracture (82%). Partial cusp fractures were observed above all in crowns (70%). Analysis of the fractured specimens revealed that the origin of the fracture was mainly at the occlusal contact points of the stepwise loading. Significance Veneering of CAD–CAM RNC restorations has no influence on their fatigue resistance except when monolithic crowns are modified on their occlusal central groove.

To evaluate the marginal adaptation of endodontically treated molars restored with CAD/CAM composite resin endocrowns either with or without reinforcement by fibre reinforced composites (FRCs), used in different configurations. 32 human endodontically treated molars were cut 2 mm over the CEJ. Two interproximal boxes were created with the margins located 1 mm below the CEJ (distal box) and 1 mm over the CEJ (mesial box). All specimens were divided in four groups (n = 8). The pulp chamber was filled with: group 1 (control), hybrid resin composite (G-aenial Posterior, GC); group 2, as group 1 but covered by 3 meshes of E-glass fibres (EverStick NET, Stick Tech); group 3, FRC resin (EverX Posterior, GC); group 4, as group 3 but covered by 3 meshes of E-glass fibres. The crowns of all teeth were restored with CAD/CAM composite resin endocrowns (LAVA Ultimate, 3M). All specimens were thermo-mechanically loaded in a computer-controlled chewing machine (600,000 cycles, 1.6 Hz, 49 N and simultaneously 1500 thermo-cycles, 60 s, 5–55 °C). Marginal analysis before and after the loading was carried out on epoxy replicas by SEM at 200× magnification. For all the groups, the percentage values of perfect marginal adaptation after loading were always significantly lower than before loading (p < 0.05). The marginal adaptation before and after loading was not significantly different between the experimental groups (p > 0.05). Within the limitations of this in vitro study, the use of FRCs to reinforce the pulp chamber of devitalized molars restored with CAD/CAM composite resin restorations did not significantly influenced their marginal quality.

ObjectivesTo evaluate the fracture strength and the mode of failure of endodontically treated molars restored with CAD/CAM overlays with fiber reinforced composite build-up of the pulp chamber.Methods40 Devitalized molars were cut over the CEJ and divided into five groups (n = 8). The pulp chamber area was filled with: group 0 (control), no resin build-up; group 1, hybrid composite build-up (G-aenial posterior, GC); group 2, as in group 1 but covered with 3 nets of bi-directional E-glass fibers (EverStickNET, Stick Tech Ltd.); group 3, a FRC resin (EverX posterior, GC); group 4, as in group 3 but covered by the bi-directional fibers. The crowns were restored with CAD-CAM composite restorations (Lava Ultimate, 3M ESPE). Maximum fracture loads were recorded in Newton and data were analyzed using Kruskal-Wallis test (p < 0.05). Fragments were analyzed using SEM.ResultsThe mean static loads in Newton were: group 0: 2448 (546); group 1: 2817 (873); group 2: 2128 (952); group 3: 2429 (1091); group 4: 2577 (833). No significant differences were found between the groups (p > 0.05). All specimens fractured in a catastrophic way, under the CEJ. The main crack evolved in the corono-apical direction. In groups 2 and 4 secondary fracture paths with apico-coronal direction were detected close to the bi-directional fibers’ layer.ConclusionsFor the restoration of endodontically treated molars, the incorporation of FRCs did not influence the load-bearing capacity of the tooth-restoration complex. The SEM analysis showed a low ability of the bi-directional fibers net in deviating the fracture but this effect was not sufficient to lead more favorable fracture patterns, over the CEJ.Clinical significanceThe use of FRCs to reinforce the "core" of devitalized molars against vertical fractures under static loads seems useless when the thickness of the CAD/CAM composite overlay restoration is high.

The complete polymerization of luting resins through thick indirect restorations is still questioned.The purpose of this study was to evaluate the degree of polymerization of light- and dual-polymerizable luting resins under thick indirect composite resin and ceramic endocrowns by means of Vickers microhardness measurements.The Vickers microhardness measurements of a light-polymerizable microhybrid composite resin and a dual-polymerizable luting cement directly polymerized in a natural tooth mold for 40 seconds with a high-power light-emitting diode lamp (control) were compared with measurements after indirect irradiation through 7.5-mm-thick composite resin and ceramic endocrowns for 3 × 90 seconds. A test-to-control microhardness values ratio of 0.80 at a depth of 0.5 mm below the surface was assumed as the criterion for adequate conversion.For the Vickers microhardness measurements of a dual-polymerizable luting cement, no differences (P.05) were found between Vickers microhardness control values and values reported after polymerization through composite resin and ceramic endocrowns. For The Vickers microhardness measurements (±SD) of a light-polymerizable microhybrid composite resin, control values were significantly (P.05) higher (111 ±3.3) than those reported after polymerization through composite resin (100.5 ±3.8) and ceramic (99.7 ±2.3) endocrowns. However, the hardness values of The Vickers microhardness measurements of a light-polymerizable microhybrid composite resin polymerized through the endocrowns were approximately 10% to 12% lower than those of the control values. Two-way ANOVA showed the influence of the luting material on the Vickers microhardness values (P.05). The effect of endocrown material was not significant (P.05).Under the conditions of this in vitro study, Vickers microhardness values of the dual-polymerizable resin cement and the light-polymerizable restorative composite resin irradiated for 3 × 90 seconds with a high irradiance light-emitting diode lamp through 7.5-mm-thick endocrowns reached at least 80% of the control Vickers microhardness values, which means that both materials can be adequately polymerized when they are used for luting thick indirect restorations.

The evolution in adhesive dentistry has broadened the indication of esthetic restorative procedures especially with the use of resin composite material. Depending on the clinical situation, some restorative techniques are best indicated. As an example, indirect adhesive restorations offer many advantages over direct techniques in extended cavities. In general, the indirect technique requires two appointments and a laboratory involvement, or it can be prepared chairside in a single visit either conventionally or by the use of computer-aided design/computer-aided manufacturing systems. In both cases, there will be an extra cost as well as the need of specific materials. This paper describes the clinical procedures for the chairside semidirect technique for composite onlay fabrication without the use of special equipments. The use of this technique combines the advantages of the direct and the indirect restoration.

This article describes a direct composite resin technique for the closure of multiple diastemas. The described procedure allows an anatomic and esthetic emergence profile of the apical third of the tooth to be obtained in periodontally compromised teeth without the use of interproximal wedges.

Bonded indirect computer-aided design/computer-aided manufacturing (CAD/CAM) restorations are increasingly gaining popularity for the restoration of large defects in posterior teeth. In addition to ceramic blocks, composite resin blocks have been developed. Composite resins blocks may have improved mechanical properties, but have poor esthetics. Thus, an esthetic modification of the restoration after machine milling may be necessary. A step-by-step procedure for the external esthetic layering of a composite CAD/CAM restoration is described. This technique can be used to repair or modify any composite resin restoration.

The aim of this in vitro study was to observe the effect of hydrofluoric acid (HF) on the surface of two glass ceramics for Cerec and to compare it with the effect on a conventional glass ceramic. Discs were cut from a feldspathic ceramic block (VitaMKII) and from a leucite reinforced glass ceramic (IPS EMPRESS CAD) for Cerec. 5% and 9% HF concentrations were used during 1 min and 2 min each. Afterwards samples were thoroughly water rinsed for 30 s. Half of the 9% HF 1 min samples were subsequently submitted to a complex post-etching cleaning. All samples were observed under a scanning electron microscope (SEM). The conventional feldspathic ceramic samples were built up on a refractory die and a platinum foil. They were treated with 9% HF for 2 min and water rinsed for 30 s. Half of the samples were submitted to the same post-etching cleaning protocol. All samples were examined under SEM and EDX. The Cerec ceramic samples and the platinum foil ones were clean and free of any precipitate after 30 s of water rinsing. Acid concentration, times of application and the postetching cleaning treatment did not influence the cleanliness of the samples. A thick layer of deposit was observed only on the refractory die samples. This was only diminished after the post-etching treatment. The EDX analysis detected the presence of fluoride (F) only on the refractory die samples.

Coronal rehabilitation of endodontically treated posterior teeth is still a controversial issue. New materials and therapeutic options based entirely on adhesion are nowadays available. They allow performing a more conservative, faster and less expensive dental treatment. All clinical cases presented in this paper are solved by using these modern techniques, from direct composite restorations to indirect endocrowns.

SUMMARY Coronal rehabilitation of endodontically treated posterior teeth is still a controversial issue. Although the use of classical crowns supported by radicular metal posts remains widespread in dentistry, their invasiveness has been largely criticized. New materials and therapeutic options based entirely on adhesion are available nowadays, from direct composite resins to indirect endocrowns. They allow for a more conservative, faster, and less expensive dental treatment. However, the absence of a metal or high-strength ceramic substructure as in full-crown restorations can expose this kind of restoration to a higher risk of irreversible fracture in case of crack propagation. The aim of this case report is to present a technique to reinforce the cavity of an endodontically treated tooth by incorporating a fiber-reinforced composite (FRC) layer into the resin coating of the tooth preparation, before the final impressions of the cavity. This technique allows the use of FRCs in combination with any kind of restorative material for an adhesive overlay/endocrown.

The present study evaluated the influence of different composite bases and surface treatments on marginal and internal adaptation of class II indirect composite restorations, after simulated occlusal loading. Thirty-two class II inlay cavities were prepared on human third molars, with margins located in cementum. A 1-mm composite base extending up to the cervical margins was applied on all dentin surfaces in the experimental groups; impressions were made and composite inlays fabricated. The following experimental conditions were tested: no liner (control group), flowable composite treated with soft air abrasion (experiment 1), flowable composite sandblasted (experiment 2) and restorative composite sandblasted (experiment 3). All specimens were submitted to 1,000,000 cycles with a 100-N eccentric load. Tooth-restoration margins were analysed semi-quantitatively by scanning electron microscopy before and after loading; internal adaptation was also evaluated after test completion. The percentage of perfect adaptation in enamel was 79.5% to 92.7% before loading and 73.3% to 81.9% after loading. Perfect adaptation to dentin was reduced before loading (54.8% to 77.6%) and after loading (41.9% to 63%), but no difference was found among groups for pre- and post-loading conditions. No debonding occurred between the base and composite luting. A significant, negative influence of cyclic loading was observed. The results of the present study support the use of flowable or restorative composites as base/liner underneath large class II restorations. Soft air abrasion represents a potential alternative to airborne particle abrasion for treating cavities before cementation. The application of a composite base underneath indirect composite restorations represents a feasible non-invasive alternative to surgical crown lengthening to relocate cavity margins from an intra-crevicular to supra-gingival position.

Classic bonded indirect restorations are laboratory fabricated and require 2 appointments: one for provisionalization and one for luting. This article describes the adhesive luting procedure, from try-in of the workpiece to finishing and polishing after the adhesive cementation, and it represents the second part of an updated technique for bonded inlays and onlays. The clinical sequence is described step by step with 2 cases. Particular attention is given to the adhesive treatment of the restoration and the cavity as well as the choice of the resin cement. Following the adhesive philosophy and due to improvements in materials and techniques, the simplified approach described may provide predictable luting restoration of the posterior dentition.
