1. New Strong Constraints on the Central Behaviour of Spherical Galactic Models

Abstract

First of all, we show that any spherically symmetric galactic model with integrated mass profile M(r)→0 as r→0 is physically correct close to the centre only provided that the circular velocity vc(r)→0 and the gravitational field g(r)→0 as r→0. Next, we apply this statement to a broad class of five-parameter spherical galactic models, including most of those used in astrophysics and cosmology. Specifically, we show that the Jaffe and Hernquist models can be trusted only for r≳0.2Re (Re being the effective radius), while the Navarro–Frank–White (NFW) model cannot describe galaxies in the central region of regular clusters. We also briefly discuss the relevance of our result for the NFW profile of pure dark matter halos. However, we are unable to tell at which central distance the NFW model breaks down in either case, and this is a challenge for future investigations.

Abstract

In this review, we present a self-contained introduction to axion-like particles (ALPs) with a particular focus on their effects on photon polarization: both theoretical and phenomenological aspects are discussed. We derive the photon survival probability in the presence of photon–ALP interaction, the corresponding final photon degree of linear polarization, and the polarization angle in a wide energy interval. The presented results can be tested by current and planned missions such as IXPE (already operative), eXTP, XL-Calibur, NGXP, XPP in the X-ray band and like COSI (approved to launch), e-ASTROGAM, and AMEGO in the high-energy range. Specifically, we describe ALP-induced polarization effects on several astrophysical sources, such as galaxy clusters, blazars, and gamma-ray bursts, and we discuss their real detectability. In particular, galaxy clusters appear as very good observational targets in this respect. Moreover, in the very-high-energy (VHE) band, we discuss a peculiar ALP signature in photon polarization, in principle capable of proving the ALP existence. Unfortunately, present technologies cannot detect photon polarization up to such high energies, but the observational capability of the latter ALP signature in the VHE band could represent an interesting challenge for the future. As a matter of fact, the aim of this review is to show new ways to make progress in the physics of ALPs, thanks to their effects on photon polarization, a topic that has aroused less interest in the past, but which is now timely with the advent of many new polarimetric missions.

Abstract Objectives The aim of this study was to detect possible differences in reversible cardiac remodeling occurring in sport training and twin pregnancy. Background: cardiac remodeling occurs in athletes and pregnant women due to training and fetal requirements, respectively. These changes could be apparently similar. Methods 21 female elite athletes (23.2 ± 5.3 years), 25 women with twin pregnancies (35.4 ± 5.7 years) and 25 healthy competitive female athletes (controls), age-matched with pregnant women (34.9 ± 7.9 years), were enrolled. This latter group was included to minimize the effect of age on cardiac remodeling. Abstract

This study aimed to assess the physical characteristics of elite international soccer referees, compare them with other referee populations in the literature, and establish reference tolerance ellipses for the bioelectrical impedance vector analysis (BIVA) point graph. Forty-one elite international soccer referees (age 38.8 ± 3.6 years) participated in the study. The participants underwent body composition assessments, including dual-energy X-ray absorptiometry, BIVA, and somatotype. The Somatotype Attitudinal Distance (SAD), the two-sample Hotelling’s T2 test and the Mahalanobis test were used to determine somatotype and bioelectrical vector differences with the literature. The average somatotype of the referees was a balanced mesomorph (2.8, 6.5, 2.8). Elite international referees significantly differed from other samples in the literature (SAD = 2.1, 2.6, 2.9 with respect to Zimbabwean, Brazilian, and South African referees, respectively). The bioelectrical vector was significantly different from the general population (T2 ≤ 76.6; F = 38.8; D = 1.44; p < 0.001) and athletes (T2 ≤ 25.3; F = 12.6; D = 0.8; p < 0.001). Somatotype values and tolerance ellipses from this study may be useful as a reference for developing training programs and improving the selection process of referees in soccer.

Background: In recent decades, the Chinese presence in Southern Europe has grown rapidly but no data is available on the influence that residing in Mediterranean countries has on Chinese immigrants. In this study, we aim to examine the association between acculturation and cardiovascular risk factors among first-generation Chinese immigrants in Italy. Design: Population-based, cross-sectional study. Methods: A sample of 2589 Chinese first-generation immigrants (1599 women and 990 men) living in Prato, Italy, underwent blood pressure measurement, blood tests (with measurement of glucose, cholesterol, and triglycerides), and anthropometric measurements. The influence of length of residence (dependent variable) on hypertension, type 2 diabetes, overweight/obesity, and hyperlipidemia (high cholesterol) (independent variables) was investigated with multivariable logistic regression adjusted for age, sex, education and urban/rural home area in China before migration. Results: Mean age of Chinese participants was 47.2 ± 10.7 years and 61.7% were women. Abstract Background Cardiac adaptation to intense physical training is determined by many factors including age, gender, body size, load training and ethnicity. Despite the wide availability of ECG analysis, with a higher presence of abnormalities in different races, echocardiographic studies on young Afro-Caribean (AA) and Caucasian athletes (CA) are lacking in literature. We aimed to assess the effect in the secondary LV remodelling of load training in young AA players compared to matched CA players. Method Seventy-seven AA and 53 CA matched soccer players (mean age 17.35 ± 0.50 and 18.25 ± 0.77 y) were enrolled. They were evaluated with echocardiography. A subgroup of 30 AA and 27 CA were followed up for a period of 4 years. The myocardial contractile function was evaluated by speckle-tracking echocardiographic global longitudinal strain (GLS). Results No significant differences were found in weight and height and in blood pressure response to maximal ergometer test in either group. We offer a pedagogical introduction to axion-like particles (ALPs) as far as their relevance for high-energy astrophysics is concerned, from a few MeV to 1000 TeV. This review is self-contained, in such a way to be understandable even to non-specialists. Among other things, we discuss two strong hints at a specific ALP that emerge from two very different astrophysical situations. More technical matters are contained in three Appendices.

Background: In cancer survivors, quality of life, weight management and physical activity levels are growing areas of interest, also considering such parameters may offer protection to cancer recurrence. Research question: In this study we aimed to objectively verify whether an home-based exercise program is effective in weight management of a cohort of breast-cancer survivors. Type of study: Experimental cohort observational study. Methods: We enrolled 13 women (age 49.1±5.5, height 163±7.3 cm) and evaluated habitual physical activity levels. Afterwards we compared the baseline aerobic capacity by 6 Minutes Walking Test (6MWT), flexibility, grip and lower limbs strength and body composition, to the same parameters measured after 40 days of unsupervised exercise. Results: At baseline (T0), patients displayed a moderate level of physical activity and were overweight. Abstract

The assessment of the chronotropic response during acute physical exertion can allow the discovery of many cardiovascular diseases even at a young age. However, the increase in heart rate (HR) depends on the age and sex of the subject and the modality of the graded exercise test. This study aims to provide sex- and age-related normative values for heart rate performance in young athletes aged 10-18. A retrospective study was carried out on 7896 young athletes (5356 males and 2540 females) aged between 10 and 18 who underwent pre-participation screening to obtain eligibility for competitive sport. First, anthropometric parameters, performance data, and HR are reported. Thus, each age calculated third, tenth, twenty-fifth, fiftieth, seventy-fifth, ninetieth, and ninety-seventh percentiles for the stage-by-stage HR response, according to sex and graded exercise test modality category. Young female athletes of all ages showed lower performance with fewer stages performed on the cycle ergometer and the treadmill. Young male athletes on treadmill and cycle ergometers show lower HR values at submaximal intensities. The treadmill allows a longer duration than the cycle ergometer for males and females. Sex, age, and the specificity of the movement performed must be considered in assessing the chronotropic response in the young population, particularly for those who carry out a training program. In addition, providing reference values of HR response to acute physical exertion may allow for a better functional assessment of the young athletes. HighlightsGrowth and physical training induce continuous changes in the cardiovascular system. However, each young athlete shows individual features. The chronotropic response to incremental load is a common method for assessing health and fitness.Comparing the heart rate data obtained from the incremental effort with the reference percentiles can provide information in a short time in the evaluation of young athletes and the general youth population. Therefore, this methodology is regularly performed in the evaluation of anthropometric growth.In evaluating the chronotropic response to physical exertion, in addition to age and sex, should take the specificity of the movement performed during the test into account.

12. Left ventricular remodeling and the athlete’s heart, irrespective of quality load training

Abstract

Abstract Background Regular physical exercise determines a progressive increase of the cardiac mass known as adaptive hypertrophy. Up to now, two morphological echocardiographic heart patterns of athletes have been described by Morganroth in 1975: predominant augmentation of wall thickness, and major cavity size in chamber dimensions in the case of prevalent static or dynamic components. The aim of the study was to follow up the impact of physical training on heart morphology and function in a group of elite soccer and rugby players for at least five years. Method From January 1993 to December 2015 a group of 250 elite soccer players and 114 rugby players were examined: 78 soccer players and 60 rugby players were followed up for 5 years. They were matched with a control group. Results LV dimensions and LVMi were significantly higher in the athletes than in the inactive subjects (LVMi : 123.45; LVMi: 81.5 vs 94.36 g/m2 respectively). After the five-year follow up the athletes showed no significant modifications in cardiac dimensions: (LVDd from 52.00 ± mm to 52.90 ± mm; LVSd increased from 31.58 ± mm to 32.33 ± mm; Left Ventricular CMI from 120.77 to 121.45 g/m2;p = NS in soccer; from 50.43 ± mm to 52.22 ± mm; Left Ventricular Systolic diameter increased from 32.51 ± mm to 32.8 ± mm; Left Ventricular Mass index from 81,5 to 87,4 g/m2;p = NS and no significant enhancement of the aortic root diameter was observed (Aortic root: from 27.39 mm to 31.64 mm in soccer players; from 30,68 mm to 30.95 mm). Conclusions No significant differences were found among the athletes practicing sports with different workload components, and resistance training. In trained athletes the dimensions of the LV chamber and LVMi are generally within the upper limits of the normal range. After a five-year follow-up, the dimensions of the chambers of the heart remain within the normal range, despite being within the the upper limits. Regular physical exercise induces mild LV hypertrophy which therefore can be considered an adaptive consequence to stress-exercise.

Abstract

Axion-like particles (ALPs) are very light, neutral, spin zero bosons predicted by many theories which try to complete the standard model of elementary particles. ALPs interact primarily with two photons and can generate photon-ALP oscillations in the presence of an external magnetic field. They are attracting increasing interest since photon-ALP oscillations produce deep consequences in very-high-energy (VHE) astrophysics. Two hints for the existence of an ALP have recently been proposed. In this paper, we study another effect of the photon-ALP interaction: the change of the polarization state of photons. In particular, we study the propagation of the photon-ALP beam starting where photons are produced - we consider photons generated in a galaxy cluster or in the jet of a blazar - crossing several magnetized media (blazar jet, host galaxy, galaxy cluster, extragalactic space, Milky Way) up to their arrival at the Earth. In the presence of photon-ALP interaction, we analyze the final photon survival probability $P_{\gamma \to \gamma}$ and the corresponding photon degree of linear polarization $\Pi_L$ for energies in the range $(1-10^{15}) \, \rm eV$. We observe that photons, which are expected as unpolarized in the absence of ALPs, are made partially polarized by photon-ALP interaction. Our findings can be tested by observatories like IXPE (already operative), and by the proposed missions eXTP, XL-Calibur, NGXP and XPP in the X-ray band and by COSI (approved to launch), e-ASTROGAM and AMEGO in the high-energy range. We also discover a peculiar feature in the VHE band, where photons at energies above $ \sim (1-10) \, \rm TeV$ are fully polarized because of photon-ALP interaction. 14. Adjuvant Therapy Reduces Fat Mass Loss during Exercise Prescription in Breast Cancer Survivors

Improvements in cancer care over the years have increased the numbers of cancer survivors. Therefore, quality of life, fat mass management and physical activity are growing areas of interest in these people. After the surgical removal of a breast cancer, adjuvant therapy remains anyway a common strategy. The aim of this study was to assess how adjuvant therapy can affect the effectiveness of an unsupervised exercise program. Forty-two women were enrolled (52.0 ± 10.1 years). Assessments performed at baseline and after six months of exercise prescription were body composition, health-related quality of life, aerobic capacity by Six-Minute Walk Test, limbs strength by hand grip and chair test and flexibility by sit and reach. Statistical analyses were conducted by ANOVA tests and multiple regression. Improvements in body composition, physical fitness and quality of life (physical functioning, general health, social functioning and mental health items) were found. The percentage change in fat mass has been associated with adjuvant cancer therapy (intercept = −0.016; b = 8.629; p < 0.05). An unsupervised exercise prescription program improves body composition, physical fitness and health-related quality of life in breast cancer survivors. Adjuvant therapy in cancer slows down the effectiveness of an exercise program in the loss of fat mass.

Abstract

Soccer referees are a specific group in the sports population that are receiving increasing attention from sports scientists. A lower fat mass percentage (FM%) is a useful parameter to monitor fitness status and aerobic performance, while being able to evaluate it with a simple and quick field-based method can allow a regular assessment. The aim of this study was to provide a specific profile for referees based on morphological and body composition features while comparing the accuracy of different skinfold-based equations in estimating FM% in a cohort of soccer referees. Forty-three elite international soccer referees (age 38.8 ± 3.6 years), who participated in the 2018 Russian World Cup, underwent body composition assessments with skinfold thickness and dual-energy X-ray absorptiometry (DXA). Six equations used to derive FM% from skinfold thickness were compared with DXA measurements. The percentage of body fat estimated using DXA was 18.2 ± 4.1%, whereas skinfold-based FM% assessed from the six formulas ranged between 11.0% ± 1.7% to 15.6% ± 2.4%. Among the six equations considered, the Faulkner’s formula showed the highest correlation with FM% estimated by DXA (r = 0.77; R2 = 0.59 p < 0.001). Additionally, a new skinfold-based equation was developed: FM% = 8.386 + (0.478 × iliac crest skinfold) + (0.395 × abdominal skinfold, r = 0.78; R2 = 0.61; standard error of the estimate (SEE) = 2.62 %; p < 0.001). Due to these findings, national and international federations will now be able to perform regular body composition assessments using skinfold measurements.

Background and objectives: Performing physical exercise after a colorectal cancer diagnosis is associated with lower mortality related to the tumor itself. In order to improve physical recovery after elective surgery, there are no specific exercise protocols after discharge from the hospital. The purpose of this study is to show the preliminary results of an exercise program after colorectal cancer surgery. Materials and Methods: Six patients with non-metastatic colorectal adenocarcinoma addressed to respective laparoscopic were randomly assigned to a mixed supervised/home-based exercise program for six months and compared to a control group without exercise. To assess the effectiveness of the program, functional and body composition parameters were evaluated. Results: Three months after surgery, the exercise group increased flexibility (p p p p Conclusions: Colorectal cancer treatment induces a reduction in physical function, particularly during the first six months after treatment. A mixed exercise approach appears promising in countering this process after colorectal cancer surgery.

17. Photons to axion-like particles conversion in Active Galactic Nuclei

Abstract

The idea that photons can convert to axion-like particles (ALPs) γ→a in or around an AGN and reconvert back to photons a→γ in the Milky Way magnetic field has been put forward in 2008 and has recently attracted growing interest. Yet, so far nobody has estimated the conversion probability γ→a as carefully as allowed by present-day knowledge. Our aim is to fill this gap. We first remark that AGN which can be detected above 100 GeV are blazars, namely AGN with jets, with one of them pointing towards us. Moreover, blazars fall into two well defined classes: BL Lac objects (BL Lacs) and Flat Spectrum Radio Quasars (FSRQs), with drastically different properties. In this Letter we report a preliminary evaluation of the γ→a conversion probability inside these two classes of blazars. Our findings are surprising. Indeed, while in the case of BL Lacs the conversion probability turns out to be totally unpredictable due to the strong dependence on the values of the somewhat uncertain position of the emission region along the jet and strength of the magnetic field therein, for FSRQs we are able to make a clear-cut prediction. Our results are of paramount importance in view of the planned very-high-energy photon detectors like the CTA, HAWK, GAMMA-400 and HISCORE. Keywords: Axion, Photon propagation

Assessing the systolic and diastolic blood pressure during acute physical exertion can allow the discovery of many cardiovascular diseases even at a young age. However, this response depends on the age, sex of the subject, and the modality of the graded exercise test. This study aims to provide sex-and age-related normative values of peak and recovery blood pressure performance and to develop a predicted model of systolic and diastolic blood pressure peak in young athletes.Retrospective-study.We analyzed 8224 young athletes (5516 males and 2708 females) aged between 8 and 18. Anthropometric and blood pressure parameters related to the effort are reported. Then, according to sex, graded exercise test modality, and age were calculated 1) the fifth, tenth, fiftieth, ninetieth, and ninety-fifth percentiles for the systolic and diastolic blood pressure at peak and after 1 min of recovery and 2) predictive equations of systolic and diastolic blood pressure at the peak.Younger athletes show lower peak blood pressure values, gradually increasing as they age. Males showed higher peak systolic blood pressure values starting at 12-13 years on the cycle ergometer and 10-11 years on the treadmill, while there was no difference in peak diastolic blood pressure values.Sex, age, and the specificity of the movement performed must be considered in assessing the blood pressure response in the young population. In addition, providing reference values and predictive equations of blood pressure response to acute physical exertion may allow for a better functional assessment of young athletes.

Abstract

Aims The association between hyperglycemia at hospital admission and relevant short- and long-term outcomes in elderly population is known. We assessed the effects on mortality of hyperglycemia, disability, and multimorbidity at admission in internal medicine ward in patients aged ≥ 65 years. Methods Data were collected from an active register of 102 internal medicine and geriatric wards in Italy (RePoSi project). Patients were recruited during four index weeks of a year. Socio-demographic data, reason for hospitalization, diagnoses, treatment, severity and comorbidity indexes (Cumulative Illness rating Scale CIRS-SI and CIRS-CI), renal function, functional (Barthel Index), and cognitive status (Short Blessed Test) and mood disorders (Geriatric Depression Scale) were recorded. Mortality rates were assessed in hospital 3 and 12 months after discharge. Results Of the 4714 elderly patients hospitalized, 361 had a glycemia level ≥ 250 mg/dL at admission. Compared to subjects with lower glycemia level, patients with glycemia ≥ 250 mg/dL showed higher rates of male sex, smoke and class III obesity. These patients had a significantly lower Barthel Index (p = 0.0249), higher CIRS-SI and CIRS-CI scores (p = 0.0025 and p = 0.0013, respectively), and took more drugs. In-hospital mortality rate was 9.2% and 5.1% in subjects with glycemia ≥ 250 and p = 0.0010). Regression analysis showed a strong association between in-hospital death and glycemia ≥ 250 mg/dL (OR 2.07; [95% CI 1.34–3.19]), Barthel Index ≤ 40 (3.28[2.44–4.42]), CIRS-SI (1.87[1.27–2.77]), and male sex (1.54[1.16–2.03]). Conclusions The stronger predictors of in-hospital mortality for older patients admitted in general wards were glycemia level ≥ 250 mg/dL, Barthel Index ≤ 40, CIRS-SI, and male sex.

20. Diagnostic Pathway and Clinical Significance of Premature Ventricular Beats (PVBs) in Trained Bicuspid Aortic Valve (BAV) Athletes

Abstract

Purpose: Bicuspid aortic valve (BAV) represents a common congenital cardiac disease (1−2%) normally compatible with sports activity. In the case of competitive sports, eligibility can be barred by the presence of symptoms, aortic valve dysfunction, or arrhythmias. This investigation of a large cohort of BAV athletes aims to verify the prevalence of premature ventricular beats (PVBs) found in the exercise test (ET) at the first sports medicine clinical evaluation. Methods: A sample of 356 BAV athletes, regularly examined over a period of 10 years at the Sports Medicine Center of the University of Florence, was retrospectively evaluated for arrhythmic events found in the first sports medicine check-up carried out. The athletes (321 M and 79 F), aged between 8−50 years (mean age 21.8 ± 11.6), practised sports at high dynamic cardiovascular intensity (mainly soccer, basketball, and athletics). Criteria for participation included a 2D echocardiography and ET conducted at 85% of maximal effort. Ventricular arrhythmic events were reported if found to be ≥3 at rest and/or during the exercise test and for subjects with any other cardiac or systemic structural diseases. Individuals aged >50 were excluded from the study. The selected participants were matched with a control group of 400 athletes with similar levels of training (age 20.0 ± 9.9) without BAV. Results: Only 25 (7.02%) of BAV athletes showed PVBs at the ET. A total of 403 single PVBs and four monomorphic couples were observed; a polymorphic pattern was present in only three athletes, and only five had exercise-induced PVBs at peak. None had acute events or major arrhythmias. The difference in PVBs prevalence in BAV athletes vs. controls (PVBs 6.25%) was not significant (p > 0.05). Conclusions: The prevalence of PVBs is low in BAV athletes and appears not to differ from athletes without BAV. Despite this, the behaviour of PVBs at the ET should be considered for the major suspicion for arrhythmic events. More data in this field could optimize the cost/effectiveness ratio for eventual ECG Holter indications.

21. Revisiting the evidences for spectral anomalies in distant blazars: new data on the photon-ALP mixing

Abstract

We re-examine possible dependencies on redshift of the spectral parameters of blazars observed at very-high energies (VHEs) with Imaging Atmospheric Cherenkov telescopes (IACTs). This is relevant to assess potential effects with the source distance of the photon to axion-like particle (ALP) mixing, that would deeply affect the propagation of VHE photons across the Universe. We focus our spectral analysis on 38 BL Lac objects (32 high-peaked and 6 intermediate-peaked) up to redshift $z\simeq 0.5$, and a small sample of 5 Flat Spectrum Radio Quasars up to $z=1$ treated independently to increase the redshift baseline. The 78 independent spectra of these sources are first of all carefully corrected for the gamma-gamma interaction with photons of the Extragalactic Background Light, that are responsible for the major redshift-dependent opacity effect. Then, the corrected spectra are fitted with simple power-laws to infer the intrinsic spectral indices $\Gamma_{\rm em}$ at VHE, to test the assumption that such spectral properties are set by the local rather than the global cosmological environment. We find some systematic anti-correlations with redshift of $\Gamma_{\rm em}$ that might indicate, although with low-significance, a spectral anomaly potentially requiring a revision of the photon propagation process. More conclusive tests with higher statistical significance will require the observational improvements offered by the forthcoming new generation of Cherenkov arrays (CTA, ASTRI, LHAASO)., Comment: 17 pages, 12 figures

Abstract

Background The Central Mediterranean route from Libya to Italy has been for years the major port of entry from Africa to Europe. However, information on the early effects of permanence in Italy on cardiovascular risk factors is limited. Therefore, the association between length of stay in Italy and blood pressure or blood glucose was explored in young asylum seekers. Methods Cross-sectional study performed on male asylum seekers (aged 18–40 years), hosted for 0–30 months in Prato, Italy. Blood pressure and blood glucose, measured with validated instruments, were classified according to European Society of Hypertension and American Diabetes Association guidelines respectively. The relationship of quartiles of months of stay in Italy (independent variable) with blood pressure and fasting glucose was investigated with multivariate linear regression adjusted for years of age, world area of origin, education level, travel duration in months, smoking habit, alcohol use, body mass index, triglycerides. Results On average, the 217 asylum seekers lived in Italy for 8.4 months (95% CI 7.5–9.3; range 0–30 months). At multivariate adjusted linear regression analysis, quartiles of months in Italy were associated with a forward shift in the blood pressure categories of the European Society of Hypertension (B = 0.396; 95% CI 0.190 to 0.602) and in the categories fasting glucose levels of the American Diabetes Association (B = 0.450; 95% CI 0.023 to 0.876). Conclusions When considering the importance of high blood pressure and type 2 diabetes for ethnic minorities living in Europe, changes of cardiovascular risk factors in the new environment probably need more attention.

Abstract

Soccer referees are a specific group of the athletes’ population whose careers peak from 30 to 45 years old. An athlete's performance is not only determined by physical training but also by a lifestyle, e.g. eating habits. The purpose of this study was to verify current eating habits and resulting body composition of a group of elite international soccer referees. At an international FIFA seminar 60 elite international soccer referees (aged 39.2 ± 4.2 years) were enrolled. A body composition assessment was performed with skinfold thickness and bio impedance analysis, while eating habits were evaluated with a multi-pass 24-hour dietary recall. The body composition showed a normal weight condition with a fat content of 11.4 ± 2.5%. Macronutrients showed a low level of carbohydrates (43.6 ± 5.4%) and a high level of fat (40.0 ± 4.5%). Micronutrients showed a low level of calcium, potassium, zinc, magnesium, iodine, vitamin B12 and vitamin B9. Even though their body composition was within the normal range, the current eating habits of elite international soccer referees did not appear to follow the nutrition guidelines. Therefore, it would be advisable to provide knowledge on nutrition for this particular category of sports subjects, an individualized nutritional plan would be advisable, in order to achieve and maintain better performance and appropriate body composition for their role.

Abstract

Axion-like particles (ALPs) are very light, neutral, spin zero bosons predicted by superstring theory. ALPs interact primarily with two photons and in the presence of an external magnetic field they generate photon-ALP oscillations and the change of the polarization state of photons. While well motivated from a theoretical point of view, hints on ALP existence come from astrophysics. In this paper, we state and demonstrate some theorems about a strict relationship between initial photon polarization and photon-ALP conversion probability - which can be extrapolated by observed astrophysical spectra - so that, in the presence of ALPs, flux-measuring observatories become also porarimeters., 9 pages, 3 figures. Abstract

Background: Currently, diabetes represents the seventh leading cause of death worldwide, with a significant economic burden. The number and severity of comorbidities increase with age, and are identified as important determinants that influence the prognosis. We aimed to investigate comorbidities and outcomes in a cohort of hospitalized elderly patients affected by diabetes. Methods: In this observational study, we retrospectively analyzed data collected from the REgistro dei pazienti per lo studio delle POlipatologie e politerapie in reparti della rete Simi (RePoSi) registry. Socio-demographic, clinical characteristics, and laboratory findings were considered. The association between variables and in-hospital and 1-year follow-up were analyzed. Results: Among 4708 in-patients, 1378 (29.3%) had a diagnosis of diabetes. Patients with diabetes had more previous hospitalization, a clinically significant disability, and more need for a urinary catheter in comparison with subjects without diabetes. Patients affected by diabetes took more drugs, both at admission, at in-hospital stay, at discharge, and at 1-year follow-up. Thirty-five comorbidities were more frequent in patients with diabetes, and the first five were hypertension (57.1%), ischemic heart disease (31.4%), chronic renal failure (28.8%), atrial fibrillation (25.6%), and chronic obstructive pulmonary disease (22.7%). Heart rate was an independent predictor of in-hospital mortality. At 1-year follow-up, cancer and male sex were strongly independently associated with mortality. Conclusions: Our findings showed the severity of the impact of diabetes and its comorbidities in the real life of internal medicine and geriatric wards, and provide data to be used for a better tailored management of elderly in-patients with diabetes.

Abstract

Many extensions of the Standard Model of particle physics and in particular superstring and superbrane theories predict the existence of axion-like particles (ALPs). ALPs are very elusive, extremely light and interact primarily with photons. In the presence of an external magnetic field two effects show up: (i) photon-ALP oscillations and (ii) a change of the photon polarization state. The astrophysical context represents the best opportunity to get indirect evidence for the ALP existence thanks to various effects that the photon-ALP interaction produces in the sky. Great attention has been paid so far to photon-ALP oscillations, since they modify the transparency of the crossed media at very high energies and so the final spectra of faraway sources exhibit a flux excess and a characteristic oscillatory behavior. Two hints at the ALP existence have hitherto been discovered. But less interest has been attracted by the modification of the photon polarization. In this paper we address it in the X-ray and in the high energy (HE) bands. Specifically, we analyze the photon degree of linear polarization and the polarization angle induced by the photon-ALP interaction for photons generated in the central region of two galaxy clusters: Perseus and Coma. We find a substantial departure from conventional physics in both considered bands. We conclude that the ALP-induced polarization effects are more likely detectable with the proposed missions like COSI (approved to launch), e-ASTROGAM and AMEGO in the HE range. Abstract

Comorbidity indexes were designed in order to measure how the disease burden of a patient is related to different clinical outcomes such as mortality, especially in older and intensively treated people. Charlson's Comorbidity Index (CCI) is the most widely used rating system, based on diagnoses, but when this information is not available therapy-based comorbidity indices (TBCI) are an alternative: among them, Drug Derived Complexity Index (DDCI), Medicines Comorbidity Index (MCI), and Chronic Disease Score (CDS) are available.This study assessed the predictive power for 1-year mortality of these comorbidity indices and polypharmacy.Survival analysis and Receiver Operating Characteristic (ROC) analysis were conducted on three Italian cohorts: 2,389 nursing home residents (Korian), 4,765 and 633 older adults admitted acutely to geriatric or internal medicine wards (REPOSI and ELICADHE).Cox's regression indicated that the highest levels of the CCI are associated with an increment of 1-year mortality risk as compared to null score for all the three samples. DDCI and excessive polypharmacy gave similar results but MCI and CDS were not always statistically significant. The predictive power with the ROC curve of each comorbidity index was poor and similar in all settings.On the whole, comorbidity indices did not perform well in our three settings, although the highest level of each index was associated with higher mortality.

Abstract

Very-high-energy (VHE) BL Lac spectra extending above $10 \, \rm TeV$ provide a unique opportunity for testing physics beyond the standard model of elementary particle and alternative blazar emission models. We consider the hadron beam, the photon to axion-like particle (ALP) conversion, and the Lorentz invariance violation (LIV) scenarios by analyzing their consequences and induced modifications to BL Lac spectra. In particular, we consider how different processes can provide similar spectral features (e.g. hard tails) and we discuss the ways they can be disentangled. We use HEGRA data of a high state of Markarian 501 and the HESS spectrum of the extreme BL Lac (EHBL) 1ES 0229+200. In addition, we consider two hypothetical EHBLs similar to 1ES 0229+200 located at redshifts $z=0.3$ and $z=0.5$. We observe that both the hadron beam and the photon-ALP oscillations predict a hard tail extending to energies larger than those possible in the standard scenario. Photon-ALP interaction predicts a peak in the spectra of distant BL Lacs at about $20-30 \, \rm TeV$, while LIV produces a strong peak in all BL Lac spectra around $\sim 100 \, \rm TeV$. The peculiar feature of the photon-ALP conversion model is the production of oscillations in the spectral energy distribution, so that its detection/absence can be exploited to distinguish among the considered models. The above mentioned features coming from the three models may be detected by the upcoming Cherenkov Telescope Array (CTA). Abstract

Introduction: Renal transplant recipients (RTRs) are at high cardiovascular risk (CV) compared to the general population, especially after surgical treatment. The literature supports the role of supervised exercise intervention; however no data are available regarding the effects of unsupervised exercise programs. We investigated whether a home exercise program could reduce CV risk in RTR based on possible changes in renal and cardiometabolic parameters and myocardial performance measured by echocardiography. Methods: From a large cohort of 60 RTRs, 30 RTRs (12 females and 18 males 48.3 ± 12.3 years) participated in individualized and unsupervised training programs for 6 months, at moderate intensity. Cardiometabolic risk factors, anthropometric parameters, lipid and glycemic blood sample profiles were studied as was myocardial performance from the 2D echo examination at T0, and T6 months. Results: The lipid profile remained in the range of a low level of risk, although there was no significant improvement, whereas myocardial performance, in particular the EF, was significantly improved. Conclusions: A home exercise program for at least 6 months produces positive effects on myocardial function and helps maintain a low cardiovascular risk profile. The trend supports the importance of highlighting the role of a correct reconditioning of lifestyle in RTR, from the exercise program without supervision to moderate intensity, where well tolerated.

Abstract

Cancer is the second leading cause of death, and the most common diagnosis among the general population is breast and colon cancer. Recently, an increasing number of new cases of invasive breast and colon cancer have been estimated, and more people die from these diseases. In addition to the genetic pattern, diet and lifestyle including smoking, alcohol consumption, and sedentary behaviour have also been identified as potential risks factors. Recent studies of cancer survivors have shown the beneficial effects of regular physical activity to reduce the prevalence of comorbidity, muscle atrophy, weight changes, reduced aerobic capacity, fatigue, depression, and reduced quality of life. Dedicated and individual programs are crucial for achieving the goals of improving quality of life and reducing comorbidities. A multidisciplinary approach is fundamental: lifestyle assessment, including estimating the level of physical activity, as well as nutritional habits, may be the first step. A periodic cardiovascular examination is crucial for detecting asymptomatic early myocardial failure. According to current ACSM guidelines, different levels of exercise (low-moderate 40% and moderate up to 60% of the maximal HR) may be prescribed, and patients enrolled may follow the exercise program if in the absence of contraindications. The current paper reports observations from our clinical practice and provides practical strategies that bridge contemporary, published guidelines into practice within a multi-disciplinary team working with cancer survivors in Italy.

Diabetes mellitus Type 2 (DMT2) carries a high risk of cardiovascular (CV) morbidity and mortality. Physical activity (PA) is widely prescribed for this population. However, detection of the eventual risk level and the potential additional increase through physical exercise are of paramount importance in these patients. A model for predicting absolute risk for coronary artery disease in diabetes has been provided in the investigation by the UKPDS (United Kingdom Prospective Diabetes Study). The present study aims to verify the potential feasibility of the UKPDS calculation in an out-patient context, where patients with DMT2 can be selected for an exercise prescription program. All patients investigated were consecutively enrolled in the study at the time of the first clinical check-up in the outpatient center of the University of Perugia-Internal Medicine Department. A group of 101 Caucasian patients (62 males; 39 females; aged 67 ± 2 years) were studied. In agreement with the UKPDS score, gender, age, ethnicity, Body Mass Index, systolic blood pressure and smoking habits were considered. The global mean cardiovascular risk calculated by the UKPDS risk-engine was 25.8% of a first cardiovascular event within 10 years after the initial observation. Despite an absence of symptoms, the risk was globally high and in agreement with current literature. Conclusion: The data are suggestive of a potential large clinical application of UKPDS risk-engine score in an outpatient context, especially in those asymptomatic DMT2 patients potentially enrolled in a physical activity program. This pilot investigation supports the importance for immediate estimation of cardiovascular risk in DMT2 subjects to implement basic therapeutic strategies such as physical activity.

Abstract

Aging with multimorbidity and polytherapy are the most significant factors that could led to inappropriate prescribing of contraindicated medications in patients with chronic kidney disease (CKD). The aim of this study was to evaluate the prescriptions of contraindicated drugs in older adults in CKD and to identify their associated factors in a hospital context. An observational retrospective study was carried out considering all patients ≥65 years with at least one serum creatinine value recorded into the REPOSI register into 2010–2016 period. The estimated glomerular filtration rate (eGFR) was applied to identify CKD. A descriptive analysis was performed to compare demographic and clinical characteristics; logistic regression models were used to estimate factors of inappropriate and percentage changes of drug use during hospitalization. A total of 4,713 hospitalized patients were recorded, of which 49.8% had an eGFR 2; the 21.9% were in treatment with at least one inappropriate drug at the time of hospital admission with a decrease of 3.0% at discharge (p = 0.010). The probability of using at least one contraindicated drug was significantly higher in patients treated with more several drugs (OR 1.21, 95% CI 1.16–1.25, p p < 0.001; G5: 19.38, 11.51–32.64, p < 0.001). Low-dose acetylsalicylic acid was the contraindicated drug mainly used at the time of admission, reducing 1.2% at discharge. An overall increase in therapeutic appropriateness in hospitalized older patients with CKD was observed, despite a small percentage of therapeutic inappropriateness at discharge that underlines the need for a closer collaboration with the pharmacologist to improve the drug management.

Abstract

Background: In recent decades, the Chinese presence in Southern Europe has grown rapidly but no data is available on the influence that residing in Mediterranean countries has on Chinese immigrants. In this study, we aim to examine the association between acculturation and cardiovascular risk factors among first-generation Chinese immigrants in Italy. Design: Population-based, cross-sectional study. Methods: A sample of 2589 Chinese first-generation immigrants (1599 women and 990 men) living in Prato, Italy, underwent blood pressure measurement, blood tests (with measurement of glucose, cholesterol, and triglycerides), and anthropometric measurements. The influence of length of residence (dependent variable) on hypertension, type 2 diabetes, overweight/obesity, and hyperlipidemia (high cholesterol) (independent variables) was investigated with multivariable logistic regression adjusted for age, sex, education and urban/rural home area in China before migration. Abstract

Background Cardiac adaptation to intense physical training is determined by many factors including age, gender, body size, load training and ethnicity. Despite the wide availability of ECG analysis, with a higher presence of abnormalities in different races, echocardiographic studies on young Afro-Caribean (AA) and Caucasian athletes (CA) are lacking in literature. We aimed to assess the effect in the secondary LV remodelling of load training in young AA players compared to matched CA players. Method Seventy-seven AA and 53 CA matched soccer players (mean age 17.35 ± 0.50 and 18.25 ± 0.77 y) were enrolled. They were evaluated with echocardiography. A subgroup of 30 AA and 27 CA were followed up for a period of 4 years. The myocardial contractile function was evaluated by speckle-tracking echocardiographic global longitudinal strain (GLS). Results No significant differences were found in weight and height and in blood pressure response to maximal ergometer test in either group. In AA a higher level of LV remodelling, consisting in higher LV wall thickness, higher interventricular septum (IVS) and posterior wall (PW) thickness were found (IVS: 10.04 ± 0.14 and 9.35 ± 0.10 in AA and CA respectively, p p n = 27) and CA (n = 25), respectively). At the beginning of the follow-up study AA showed a significantly higher left ventricular remodelling (IVS = 9.29 ± 0.3 and 8.53 ± 0.12 mm in AA and CA respectively, p p = 0.1). During the next four years of follow-up we observed a regular parallel increase in LV wall thickness and chamber diameters in both groups, proportionally to the increase in body size and LV mass. (IVS = 10.52 ± 0.17 and 9.03 ± 0.22 mm in AA and CA respectively, p p Conclusion The study shows that the ventricular remodelling observed in AA appears to be a specific phenotype already present in pre-adolescence. These data also suggest that genetic/ethnic factors play a central role in left ventricular remodelling during the first years of life in elite athletes.

Abstract

Prompted by the increasing interest of axion-like particles (ALPs) for very-high-energy (VHE) astrophysics, we have considered a full scenario for the propagation of a VHE photon/ALP beam emitted by a BL Lac and reaching us in the light of the most up-to-date astrophysical information and for energies up to above $100 \, \rm TeV$. During its trip, the beam -- generated in a small region of a BL Lac jet -- crosses a variety of magnetic structures in very different astronomical environments: the BL Lac jet, the host elliptical galaxy, the extragalactic space and the Milky Way. We have taken an effort to model all these magnetic fields in the most realistic fashion and using a new model developed by us concerning the extragalactic magnetic field. Assuming an intrinsic spectrum with a power law exponentially truncated at a fixed cut-off energy, we have evaluated the resulting observed spectra of Markarian 501, the extreme BL Lac 1ES 0229+200 and a similar source located at $z = 0.6$ up to above $100 \, \rm TeV$. We obtain interesting results: the model with photon-ALP oscillations possesses features (spectral energy oscillatory behaviour and photon excess above $20 \, \rm TeV$) which can be tested by $\gamma$-ray observatories like CTA, HAWC, GAMMA 400, LHAASO, TAIGA-HiSCORE and HERD. In addition, our ALP can be detected in dedicated laboratory experiments like the upgrade of ALPS II at DESY, the planned IAXO and STAX experiments, as well as with other techniques developed by Avignone and collaborators., Comment: 11 pages, 5 figures, accepted for publication by MNRAS

Reduced physical activity in pregnancy is often associated with a progressive increase of chronic metabolic disease and to an enhanced risk for the child. The majority of women are less physically active during pregnancy, despite the motivation to improve their health, quality of life, and fitness. Education on the benefits of regular physical activity in pregnancy is determinant as suggested by obstetricians and gynecologists. The specific programs need to be supported by specialists in Sports Medicine. Counseling and support by nurses can also be helpful in encouraging women to maintain a sufficient level of physical activity during pregnancy or to modify their lifestyle, adapting the single models and specific physical activity programs to their needs.

Background. Bicuspid aortic valve (BAV) is one of the most common congenital heart disease (0.9%–2%) and is frequently found in the athletes and in the general population. BAV can lead to aortic valve dysfunction and to a progressive aortic dilatation. Trained BAV athletes exhibit a progressive enlargement of the left ventricle (LV) compared to athletes with normal aortic valve morphology. The present study investigates the possible relationship between different aortic valve morphology and LV dimensions. Methods. In the period from 2000 to 2011, we investigated a total of 292 BAV subjects, divided into three different groups (210 athletes, 59 sedentaries, and 23 ex-athletes). A 2D echocardiogram exam to classify BAV morphology and measure the standard LV systo-diastolic parameters was performed. The study was conducted as a 5-year follow-up echocardiographic longitudinal and as cross-sectional study. Results. Typical BAV was more frequent in all three groups (68% athletes, 67% sedentaries, and 63% ex-athletes) than atypical. In BAV athletes, the typical form was found in 51% (107/210) of soccer players, 10% (21/210) of basketball players, 10% track and field athletics (20/210), 8% (17/210) of cyclists, 6% (13/210) swimmers, and 15% (32/210) of rugby players and others sport. Abstract

Reduced estimated creatinine clearance (eCrCl) is prevalent in older patients and impacts on drug prescription. In this study, the burden of eCrCl reduction and its associated factors and impact on outcomes were analyzed. Moreover, the rate of inappropriate drug prescription according to eCrCl and its impact on outcomes were described.Data were obtained from "REgistro POliterapie SIMI" (REPOSI), a prospective observational register enrolling hospitalized patients aged ≥ 65 years. Patients enrolled from 2010-2016 with available data to calculate eCrCl according to the Cockcroft-Gault formula were included in this analysis.A total of 5046 patients were available for analysis. Among these, we found an eCrCl of 45-59 mL/min in 1163 patients (23.0%), an eCrCl of 30-44 mL/min in 1128 (22.4%), an eCrCl of 15-29 mL/min in 702 (13.9%), and an eCrCl 15 mL/min in 152 (3.0%), with several clinical factors associated with decreasing eCrCl. During follow-up, a progressively higher risk for all-cause death, cardiovascular (CV) death, any death/re-hospitalization, and CV death/re-hospitalization was found across the renal function classes. Among patients with hypertension, diabetes mellitus, atrial fibrillation, coronary artery disease, and heart failure, 476 (10.9%) were inappropriately prescribed medications according to eCrCl. During follow-up, inappropriate prescription was associated with increased risk of all-cause death (odds ratio [OR] 1.49, 95% confidence interval [CI] 1.13-1.97) and any death/re-hospitalization (OR 1.30, 95% CI 1.03-1.63).In older hospitalized patients, impaired eCrCl is prevalent and associated with several factors, polypharmacy in particular. Patients with reduced eCrCl have a higher risk of major clinical outcomes, and 10% of them are prescribed an inappropriate drug, with a higher risk for major clinical outcomes.

Abstract

Objectives Psycholeptic drugs have been used in the older population for years, especially to control delirium and neuropsychiatric symptoms (NPS) of dementia. However, data from the literature confirm that the prolonged use of psycholeptics may be responsible for adverse reactions in older patients. The aim of this study was (1) to identify how many patients receive the first prescription of a psycholeptic drug during the hospital stay; (2) to evaluate the main sociodemographic and clinical characteristics of these patients; and (3) to verify if the prescribed psycholeptic drugs are continued after 3 months from the hospital discharge. Design Our retrospective study was based on data from the REPOSI (REgistro POliterapie SIMI) registry, a cohort of older patients hospitalized in internal medicine and geriatric wards throughout Italy from 2010 to 2018. Setting and Participants Patients aged 65 years or older who were not on home therapy with psycholeptic drugs were considered in the analyses. Methods We did both univariate and multivariate analyses in order to find the variables associated independently to an increased risk for first psycholeptic prescription at hospital discharge. Results At hospital discharge, 193 patients (5.8%) out of a total sample of 3322 patients were prescribed at least 1 psycholeptic drug. Cognitive impairment was the main risk factor for the introduction of psycholeptic drugs at discharge. Among them, 89.1% were still on therapy with a psycholeptic drug after 3 months from the hospital discharge. Conclusions and Implications Cognitive impairment represents the main risk factor for psycholeptic initiation in hospitalized older patients. The vast majority of these treatments are chronically continued after the discharge. Therefore, special attention is needed in prescribing psycholeptics at discharge, because their prolonged use may lead to cognitive decline. Moreover, their continued use should be questioned by physicians providing post-acute care, and deprescribing should be considered.

Logistic regression was used to assess whether LLD prescription was associated with patients’ health state profiles and with short-term mortality.Results: Four clusters of patients were identified according to their health state: two of them (Cluster III and IV) were the epitome of frailty conditions with poor short-term outcomes, whereas the others included healthier patients. The average prevalence of LLD use was 27.6%. The lowest prevalence was found among the healthier patients in Cluster I and among the oldest frail patients with severe functional and cognitive impairment in Cluster IV. The highest prevalence was among multimorbid patients in Cluster III (OR=4.50, 95% CI=3.76– 5.38) characterized by a high cardiovascular risk. Being prescribed with LLDs was associated with a lower 3-month mortality, even after adjusting for cluster assignment (OR=0.59; 95% CI = 0.44– 0.80).Conclusion: The prevalence of LLD prescription was low and in overall agreement with guideline recommendations and with respect to patients’ health state profiles.Keywords: statins, health state profile, multimorbidity, polypharmacy

Abstract

Background: To know burden disease of a patient is a key point for clinical practice and research, especially in the elderly. Charlson’s Comorbidity Index (CCI) is the most widely used rating system, but when diagnoses are not available therapy-based comorbidity indices (TBCI) are an alternative. However, their performance is debated. This study compares the relations between Drug Derived Complexity Index (DDCI), Medicines Comorbidity Index (MCI), Chronic Disease Score (CDS), and severe multimorbidity, according to the CCI classification, in the elderly. Methods: Logistic regression and Receiver Operating Characteristic (ROC) analysis were conducted on two samples from Italy: 2579 nursing home residents (Korian sample) and 7505 older adults admitted acutely to geriatric or internal medicine wards (REPOSI sample). Results: The proportion of subjects with severe comorbidity rose with TBCI score increment, but the Area Under the Curve (AUC) for the CDS (Korian: 0.70, REPOSI: 0.79) and MCI (Korian: 0.69, REPOSI: 0.81) were definitely better than the DDCI (Korian: 0.66, REPOSI: 0.74). All TBCIs showed low Positive Predictive Values (maximum: 0.066 in REPOSI and 0.317 in Korian) for the detection of severe multimorbidity. Conclusion: CDS and MCI were better predictors of severe multimorbidity in older adults than DDCI, according to the CCI classification. A high CCI score was related to a high TBCI. However, the opposite is not necessarily true probably because of non-evidence-based prescriptions or physicians’ prescribing attitudes. TBCIs did not appear selective for detecting of severe multimorbidity, though they could be used as a measure of disease burden, in the absence of other solutions.

Abstract

The contribution to the cosmic opacity of ultra-high-energy (UHE) ($Eg{10}^{18}\text{ }\text{ }\mathrm{eV}$) $\ensuremath{\gamma}$ rays has been computed so far for the $\ensuremath{\gamma}\ensuremath{\gamma}\ensuremath{\rightarrow}{e}^{+}{e}^{\ensuremath{-}}$ and $\ensuremath{\gamma}\ensuremath{\gamma}\ensuremath{\rightarrow}{e}^{+}{e}^{\ensuremath{-}}{e}^{+}{e}^{\ensuremath{-}}$ processes alone. We go a step further by systematically evaluating the additional opacity brought about by other leptons and hadrons. We find that the new dominant channels are those leading to the production of ${\ensuremath{\mu}}^{\ifmmode\pm\else\textpm\fi{}}$ and hadrons (mainly ${\ensuremath{\pi}}^{\ifmmode\pm\else\textpm\fi{}}$, ${\ensuremath{\pi}}^{0}$, ${K}^{\ifmmode\pm\else\textpm\fi{}}$, ${K}^{0}$, $\ensuremath{\eta}$). Our result can be expressed in terms of the probability distribution for the interaction lengths. For a few interaction lengths such a probability gets reduced by up to 20%--30% with respect to current estimates.

Abstract

BACKGROUND The aim of this study was to assess if the evaluation of Body Mass Index is sufficient to define an overweight index in young athletes, or if a more effective evaluation is preferable in order to examine body fat mass, free-fat mass and hydration status in young athletes. METHODS Two hundred ninety-nine young athletes between the ages of 8 to 18 have been analyzed in this study. Data from evaluation in body composition of young athletes were studied and subdivided by age, sex and method used. In order to measure body composition in young people, the participants who attend our Department for sport eligibility examination, were evaluated through anthropometric measurements as far as, fat mass, fat-free mass and hydration status are concerned. Abstract

Excess weight leads to an impaired cardiovascular response to physical exertion even at a young age. Sports training during youth promotes cardiovascular adaptations. The aim of the study is to verify the impact of weight status on cardiovascular parameters related to physical effort in young people who engage in competitive sports. A retrospective study was conducted on 8307 young athletes (5578 males and 2729 females) aged 6–18 years (mean age 13.9 ± 2.2 years). The data concerning graded exercise tests of young athletes in normal weight and overweight were compared. Approximately, 13.4% of the sample had excess weight. Young overweight athletes show a higher resting heart rate as well as systolic and diastolic pressure than young normal weight athletes. Excess weight condition leads to a reduction in the duration of the graded exercise test, reaching higher blood pressure values at the end of the test compared to those with normal weight. After four min from the end of the test, heart rate and systolic/diastolic blood pressure remained higher in the young overweight athletes. Excess weight affects cardiovascular parameters both at rest and in response to physical exertion during youth; however, competitive sport seems to be able to keep these parameters within the normal range even in young overweight athletes.

Abstract

It is well recognized that regular physical activity may improve cardiac autonomic regulation preventing chronic non-communicable diseases. Accordingly, the assessment of cardiac autonomic regulation (CAR) with non-invasive techniques, such as RR interval Variability (V) might be of practical interest. We studied 56 soccer players (21.2 &plusmn, 4.2 years.) and 56 controls (22.2 &plusmn, 1.5 years.) and used a ranked Autonomic Nervous System Index (ANSI), resulting from the combination of multivariate statistical methodologies applied to spectral analysis derived indices from RRV. We hypothesized that ANSI would be higher in soccer players as compared to controls (p &lt, 0.001) and that values would be greatest in defenders and midfielders, who are known to run longer distances during competitions. Conversely in the intrinsically stationary goalkeepers ANSI would be similar to controls. Our data show that it is possible to assess the overall level of autonomic performance in soccer players as compared to the general population, using a ranked composite autonomic proxy (ANSI). This approach suggests as well that CAR is better in those players who during competitions run for a greater distance. We conclude that it is possible to highlight the differences in autonomic profile due to distinct exercise routines, using ANSI, a simple ranked, composite autonomic proxy.

Abstract

Performing physical exercise after a colorectal cancer diagnosis is associated with lower mortality related to the tumor itself. In order to improve physical recovery after elective surgery, there are no specific exercise protocols after discharge from the hospital. The purpose of this study is to show the preliminary results of an exercise program after colorectal cancer surgery. Six patients with non-metastatic colorectal adenocarcinoma addressed to respective laparoscopic were randomly assigned to a mixed supervised/home-based exercise program for six months and compared to a control group without exercise. To assess the effectiveness of the program, functional and body composition parameters were evaluated. Three months after surgery, the exercise group increased flexibility (p &lt, 0.01, ES = 0.33), strength of lower limbs (p &lt, 0.01, ES = 0.42) and aerobic capacity (p &lt, 0.01, ES = 0.28). After surgery, the six patients experienced a significant reduction in body mass index (BMI) and free fat mass. More specifically, fat mass reached the lowest values, with a concomitant increase in cell mass after six months (p &lt, 0.01, ES = 0.33). This did not occur in the control group. Colorectal cancer treatment induces a reduction in physical function, particularly during the first six months after treatment. A mixed exercise approach appears promising in countering this process after colorectal cancer surgery.

Abstract

Soccer referees are a specific group in the sports population that are receiving increasing attention from sports scientists. A lower fat mass percentage (FM%) is a useful parameter to monitor fitness status and aerobic performance, while being able to evaluate it with a simple and quick field-based method can allow a regular assessment. The aim of this study was to provide a specific profile for referees based on morphological and body composition features while comparing the accuracy of different skinfold-based equations in estimating FM% in a cohort of soccer referees. Forty-three elite international soccer referees (age 38.8 &plusmn, 3.6 years), who participated in the 2018 Russian World Cup, underwent body composition assessments with skinfold thickness and dual-energy X-ray absorptiometry (DXA). Six equations used to derive FM% from skinfold thickness were compared with DXA measurements. The percentage of body fat estimated using DXA was 18.2 &plusmn, 4.1%, whereas skinfold-based FM% assessed from the six formulas ranged between 11.0% &plusmn, 1.7% to 15.6% &plusmn, 2.4%. Among the six equations considered, the Faulkner&rsquo, s formula showed the highest correlation with FM% estimated by DXA (r = 0.77, R2 = 0.59 p &lt, 0.001). Additionally, a new skinfold-based equation was developed: FM% = 8.386 + (0.478 &times, iliac crest skinfold) + (0.395 &times, abdominal skinfold, r = 0.78, R2 = 0.61, standard error of the estimate (SEE) = 2.62 %, p &lt, 0.001). Due to these findings, national and international federations will now be able to perform regular body composition assessments using skinfold measurements.

Abstract

Introduction Pain and frailty are prevalent conditions in the older population. Many chronic diseases are likely involved in their origin, and both have a negative impact on quality of life. However, few studies have analysed their association. Methods In light of this knowledge gap, 3577 acutely hospitalized patients 65 years or older enrolled in the REPOSI register, an Italian network of internal medicine and geriatric hospital wards, were assessed to calculate the frailty index (FI). The impact of pain and some of its characteristics on the degree of frailty was evaluated using an ordinal logistic regression model after adjusting for age and gender. Results The prevalence of pain was 24.7%, and among patients with pain, 42.9% was regarded as chronic pain. Chronic pain was associated with severe frailty (OR = 1.69, 95% CI 1.38–2.07). Somatic pain (OR = 1.59, 95% CI 1.23–2.07) and widespread pain (OR = 1.60, 95% CI 0.93–2.78) were associated with frailty. Osteoarthritis was the most common cause of chronic pain, diagnosed in 157 patients (33.5%). Polymyalgia, rheumatoid arthritis and other musculoskeletal diseases causing chronic pain were associated with a lower degree of frailty than osteoarthritis (OR = 0.49, 95%CI 0.28–0.85). Conclusions Chronic and somatic pain negatively affect the degree of frailty. The duration and type of pain, as well as the underlying diseases associated with chronic pain, should be evaluated to improve the hospital management of frail older people.
