13. Acute liver failure secondary to acute antibody mediated rejection after compatible liver transplant: A case report

Abstract

BACKGROUND The liver has traditionally been regarded as resistant to antibody-mediated rejection (AMR). AMR in liver transplants is a field in its infancy compared to kidney and lung transplants. In our case we present a patient with alpha-1-antitrypsin disease who underwent ABO compatible liver transplant complicated by acute liver failure (ALF) with evidence of antibody mediated rejection on allograft biopsy and elevated serum donor-specific antibodies (DSA). This case highlights the need for further investigations and heightened awareness for timely diagnosis. CASE SUMMARY A 56 year-old woman with alpha-1-antitrypsin disease underwent ABO compatible liver transplant from a deceased donor. The recipient MELD at the time of transplant was 28. The flow cytometric crossmatches were noted to be positive for T and B lymphocytes. The patient had an uneventful recovery postoperatively. Starting on postoperative day 5 the patient developed fevers, elevated liver function tests, distributive shock, renal failure, and hepatic encephalopathy. She went into ALF with evidence of antibody mediated rejection with portal inflammation, bile duct injury, endothelitis, and extensive centrizonal necrosis, and C4d staining on allograft biopsy and elevated DSA. Despite various interventions including plasmapheresis and immunomodulating therapy, she continued to deteriorate. She was relisted and successfully underwent liver retransplantation. CONCLUSION This very rare case highlights AMR as the cause of ALF following liver transplant requiring retransplantation.

Abstract

Introduction:Patients with platelet transfusion refractoriness (PTR) due to HLA-alloimmunization respond to HLA-matched platelets. However, 4/4 HLA-matched platelets or platelets without donor-recipient antigenic mismatches are rarely available. Further, in severe HLA-alloimmunization, it is difficult to find donor products which do not contain antigens to which the recipient has developed antibodies ( i.e.antibody specificity prediction, ASP method). Hence, studies have evaluated platelet transfusion responsiveness in the presence of ≥1 donor specific antibodies (DSAs) to widen donor pools, a strategy termed permissive mismatching (PERMM). Using established semi-quantitative techniques for HLA-antibody detection, we sought to identify a cumulative DSA threshold which predicted platelet corrected count increment (CCI) failure despite PERMM.

Abstract

GATA-3 is necessary for the development of MHC class II-restricted CD4 T cells, and its expression is increased during positive selection of these cells. TCR signals drive this upregulation, but the signaling pathways that control this process are not well understood. Using genetic and pharmacological approaches, we show that GATA-3 upregulation during thymocyte-positive selection is the result of additive inputs from the Ras/MAPK and calcineurin pathways. This upregulation requires the presence of the transcription factor c-Myb. Furthermore, we show that TH-POK can also upregulate GATA-3 in double-positive thymocytes, suggesting the existence of a positive feedback loop that contributes to lock in the initial commitment to the CD4 lineage during differentiation.

Abstract

iNKT cells derive from CD4 +CD8 + DP thymocytes, and are selected by thymocyte-thymocyte interactions through signals from their invariant Vα14-Jα18 TCR and from the costimulatory molecules SLAMF1 and SLAMF6. Genetic studies have demonstrated the contribution of different signaling pathways to this process. Surprisingly, current models imply that the Ras/MAPK pathway, one of the critical mediators of conventional α β T cell positive selection, is not necessary for iNKT cell development. Using mice defective at different levels of this pathway our results refute this paradigm, and demonstrate that Ras, and its downstream effectors Egr-1 and Egr-2 are required for positive selection of iNKT cells. Interestingly our results also show that there are differences in the contributions of several of these molecules to the development of iNKT and conventional αβ T cells. [ABSTRACT FROM AUTHOR]

Abstract

Aim: SP is a 26 year old female who lost her first liver transplant due to recurrence of autoimmune hepatitis disease. Patient received a second ortho-liver on 11/14/12 and experienced acute humoral liver rejection two months after transplant. Methods: T- and B-cell and T-AHG CDC crossmatch (XM) and class I PRA performed at transplant. SAB for DSA studies. Biopsy of secondary liver graft due to rejection. HLA phenotypes: Patient: A3,-;B7,62;Cw7,9;Bw6;DR13,17;DQ6,-;DR51,52. 1st liver: A3,-;B65,55;Cw1,8;Bw6;DR4,13;DQ6,8(3);DR52,53. 2nd liver: A31,33;B44,53;Cw4,5;Bw4;DR4,7;DQ2,7(3);DR53. Results: On 11/14/12 CDC-T-cell PRA was 2%, CDC T-cell and T-AHG XMs were negative, B-cell CDC XM was positive at 1:4 dilution. SP was transplanted and put on high dose immunosupression. On 01/04/13 patient was jaundiced and liver enzymes were elevated. Liver biopsy showed cholangitis and lymphoid infiltrates. C4d stain was negative. DSA study was performed by SAB. DSA was detected against HLA antigens of 1st and 2nd liver grafts. Rejection was treated with plasmapheresis + IvIg + Rituximab + Bortezomib. Sera from 12/16/07 and 11/14/12 were restrospectively analyzed by SAB. 12/16/07 serum was negative. 11/14/12 serum detected the same anti-HLA DSA than 01/04/13. The data strongly suggests that the primary transplant was the sensitizing event. [Table 1] Conclusions: Patient SP experienced humoral rejection, a rare complication in liver transplantation. Liver allocation is based on MELD score and not on HLA compatibility. The SP case was the basis for a modification in the standing lab protocol to perform Luminex Ab screens on all liver patients pre-transplant and reflect to SAB if positive. Abstract

Aim: HLA-Cw antigens are expressed at lower levels on the cell surface when compared to other HLA antigens (McCutcheon 1995). Therefore, using the same MFI cut-off for HLA-Cw antigens as for the other HLA antigens to determine unacceptables may be negatively impacting some patients’ opportunity for transplantation. Methods: Patient’s sera with reported anti-HLA-Cw- antibodies by Luminex Single Antigen Beads (One Lambda) were gathered and surrogate donor cells expressing known Cw-locus antigens were selected. The patients’ sera had ONLY donor specific antibodies to anti-HLA-Cw (DSA-Cw) and the MFI ranged from 1,180 to 21,008 MFI values. 43 patient-donor pairs have been tested and a total of 61 Flow T-Cell XMs (FT-XM) performed. Positive FT-XMs were run by serology AHG T-Cell XM (AHGT-XM). Results: Our results showed that only 15% of the sera with DSA-Cw <7,500 MFI were FT-XM POSITIVE, but 90% of the sera with DSA-Cw >7,500 MFI were POSITIVE. In total we had 25 FT positive XMs that were further analyzed by AHGT-XM. By serology, 100% of the sera with DSA-Cw <7,500 MFI were NEGATIVE and 89% of the sera with DSA-Cw >7,500 MFI were POSITIVE. Recently, it has being described that HLA-Cw alleles have different levels of expression and that this difference plays a role in controlling HIV progression (Thomas 2009; Kulkarni 2011). Therefore, we studied if this variation in the level of HLA-Cw expression also had a predictive value for FT-XM outcomes. We organized our FT-XM results depending on the DSA-Cw being a “high” or “low” expressed allele (Kulkarni 2011). Our results did not detect any significant difference, although more data is needed due to the small sample size. Conclusions: This study suggests that raising the cut-off for HLA-Cw locus antibodies could give patients more organ offers than they are currently receiving. Based on our findings, our lab has raised the HLA-Cw cut-off from 1,500 to 7,500 MFI for one of the kidney transplant programs for which we provide service. [Copyright &y& Elsevier]
