Dirofilaria immitis infection is routinely detected in dogs during veterinary care in tropical and subtropical regions worldwide. Parasitological tests for the detection of this infection are routinely performed only in areas with a high prevalence. Baixada Fluminense, a region in Rio de Janeiro, was considered heartworm-free until local veterinarians began to receive blood exams results indicating the presence of microfilariae (MF). A laboratory database was hence used to collect data from 2017 to 2020 to understand the extent of spread of the parasite in this area. The results of complete blood count analysis and MF or heartworm antigen detection tests conducted on canine samples sent from veterinary clinics in Baixada Fluminense (Magé, Duque de Caxias, Guapimirim, Nova Iguaçu, and São João de Meriti municipalities) were included. In total, the results of 16,314 hematological tests were considered. The overall prevalence of D. immitis was 3.4% (554/16,314), considering that only one test result was obtained per animal on the same day. This study is highly relevant because it indicates the spreading geographic distribution of the worms, heightens awareness among local health professionals and the general population, and encourages compliance with prophylactic measures to prevent further spread of parasite.

ABSTRACT Spotted fever (SF) is a tick-borne zoonosis caused by bacteria of the genus Rickettsia . The disease varies in severity from mild clinical forms to severe cases. In Brazil, Rickettsia rickettsii SF is the most serious rickettsiosis and can result in death if not diagnosed and treated at the onset of symptoms. The SF mild form is caused by Rickettsia parkeri strain Atlantic Rainforest, and this etiological agent has been reported in the South, Southeast and Northeast regions of the country, in areas of preserved or little antropized Atlantic Rainforest. Amblyomma ovale is the proven vector and dogs are the hosts associated with the bioagent cycle. During a SF case investigation in Paraty municipality, Rio de Janeiro State, an Atlantic Rainforest biome area in Southeastern Brazil, the human pathogen R. parkeri strain Atlantic Rainforest was detected by PCR in a sample of human skin inoculation eschar and in a female A. ovale tick collected from a dog. These results expand the known area of occurrence of this mild form rickettsiosis in Brazil. In addition, the results of the present study indicate the importance of implementing programs to control canine ectoparasites and to raise awareness of the risks of infection, signs and symptoms of SF caused by R. parkeri strain Atlantic Rainforest.

Spotted fever rickettsiosis is a zoonosis transmitted by ticks, having a varied clinical course that can lead to death if not managed properly. In Brazil it is more commonly observed in the Southeast, being an emerging public health problem. Hazard mapping models are common in different areas of knowledge, including public health, as a way of inferring reality and seeking to reduce or prevent damage. The aim of this study is to offer a spatial heuristic methodology for assessing the potential risk of transmission of spotted fever in the Rio de Janeiro state, located in the southeastern region of Brazil. For this, we used geospatial tools associated with eco-epidemiological data related to the clinical profile of the disease. The results achieved were substantially encouraging, considering that there are territories with greater or lesser expectation of risk for spotted fever in the study area. We observed that there are important distinctions between the two rickettsiosis scenarios in the same geographic space and that the areas where there is a greater potential risk of contracting rickettsiosis coincide with the administrative regions that concentrated the cases of hospitalization and deaths from the disease, concluding that the scenery found are relevant to the case series for the disease and that the planning of surveillance actions can gain in quality if the use of this spatial analysis tool is incorporated into the routine of local health management.

Spotted Fever Rickettsioses (SFR) are diseases caused by bacteria of the genus Rickettsia, and are transmitted mainly by ticks. Its eco-epidemiological scenarios vary spatially, and may also vary over time due to environmental changes. It is the main disease transmitted by ticks to humans in Brazil, with the state of Paraná (PR) having the sixth highest number of notified incidences in the country. However, information is lacking regarding the SFR disease cycles at likely infection sites within PR. During case investigations or environmental surveillance in PR for SFR, 28,517 arthropods were collected, including species known or potentially involved in the SFR cycles, such as Amblyomma sculptum, Amblyomma aureolatum, Amblyomma ovale, Amblyomma dubitatum, Amblyomma parkeri, Ctenocephalides felis felis, and Rhipicephalus sanguineus sensu lato. From these Rickettsia asembonensis, Rickettsia bellii, Rickettsia felis, Rickettsia parkeri strain Atlantic Rainforest and Candidatus Rickettsia paranaensis were detected. Ectoparasite abundance was found to be related with specific hosts and collection environments. Rickettsiae circulation was observed for 48 municipalities, encompassing 16 Health Regions (HR). As for socio-demographic and assistance indicators, circulation occurred largely in the most urbanized HR, with a higher per capita Gross Domestic Product, lower Family Health Strategy coverage, and with a higher ratio of beds in the Unified Health System per thousand inhabitants. For environmental variables, circulation occurred predominantly in HR with a climatic classified as “subtropical with hot summers” (Cfa), and with forest type phytogeographic formations. In terms of land use, circulation was commonest in areas with agriculture, pasture and fields and forest cover. Rickettsiae were circulating in almost all hydrographic basins of PR state. The results of this study provide the first descriptive recognition of SFR in PR, as well as outlining its eco-epidemiological dynamics. These proved to be quite heterogeneous, and analyzed scenarios showed characteristics strongly-associated with the outbreaks, with cases presenting clinical variation in space, so illustrating the complexity of scenarios in PR state. Due to the diversity of the circumstances surrounding SFR infections in PR, public health initiatives are necessary to foster a better understanding of the dynamics and factors effecting vulnerability to SFR in this Brazilian state.

Regional differences in tick-borne disease epidemiology may be related to biological variations between vector populations. Amblyomma aureolatum (Ixodida: Ixodidae), a neotropical tick, is known from several regions in Brazil. However, only in the metropolitan area of São Paulo (SP) state are there studies that establish its role as a vector of a pathogenic rickettsia (Rickettsia rickettsii). The aim of the study was to analyze the genetic diversity, population dynamics, and rickettsia infection in A. aureolatum populations from different spotted fever scenarios in Brazil. Samples were subjected to DNA extraction, amplification and sequencing of 12S rDNA, cytochrome oxidase subunit II and D-loop mitochondrial markers for tick population analyses, and gltA, htrA, ompA, and ompB genes for rickettsia researches. Of the 7–17 tick haplotypes identified, 5–13 were exclusive to each population and 2–12 for each epidemiological scenario, as well as three haplotypes shared by all populations. Amblyomma aureolatum populations are expanding, and do not appear to be genetically structured vis-a-vis the different epidemiological scenarios studied. Rickettsia bellii (in SP) and Rickettsia felis (in Santa Catarina) were identified as infecting A. aureolatum. No relationship between tick haplotypes and rickettsia types were observed.

In Brazil, the spotted fever group (SFG) Rickettsia rickettsii and Rickettsia parkeri related species are the etiological agents of spotted fever rickettsiosis. However, the SFG, Rickettsia rhipicephali, that infects humans, has never been reported. The study of growth dynamics can be useful for understanding the infective and invasive capacity of these pathogens. Here, the growth rates of the Brazilian isolates R. rickettsii str. Taiaçu, R. parkeri str. At#24, and R. rhipicephali HJ#5, were evaluated in Vero cells by quantitative polymerase chain reaction. R. rhipicephali showed different kinetic growth compared to R. rickettsii and R. parkeri.

Them, manually 105 ticks were collected from 10 species in two families. The dominant tick species among rodents was Amblyomma longirostre and among the marsupials was Ixodes loricatus. There was a significant correlation between temperature and tick infestations by demonstrating that higher temperature, greater is the number of ticks.

E-mail: nmsf@ioc.fiocruz.br One nymph of Ixodes wood (Ixodidae: Ixodinae) was self-collected from a woman that had just arrived in Rio de Janeiro after a holiday period in Norway, Europe. She traveled by plane presenting a skin irritation on right leg and preserved the parasite that was in her skin. This is the first brazilian record of I. woodi. Also, in South America Ixodes wood hadn’t been previously reported. In United State of America, rodents appear to be the main hosts for immature stages, although there are other records from members of Mammalia class. I. woodi doesn’t seem to be a threat to men and domestic mammals, from which has seldom been reported, but there is quoting of his relationship between trophic and rickettsiae bacteria, but no transmission.

Amblyomma rotundatum Koch is a parthenogenetic tick usually associated with reptiles and amphibians. However, relatively few studies on occurrences of ticks in wild reptile populations in Brazil have been produced. The aim of this study was to analyze the presence of ticks associated with reptile species in the Grussaí restinga, in the municipality of São João da Barra, state of Rio de Janeiro, Brazil. Between December 2010 and January 2011, 131 individuals belonging to nine species of reptiles of the order Squamata were sampled: the lizards Tropidurus torquatus (n = 51), Hemidactylus mabouia (n = 25), Mabuya agilis (n = 30), Mabuya macrorhyncha (n = 6), Cnemidophorus littoralis (n = 5) and Ameiva ameiva (n = 10); and the snakes Philodryas olfersii (n = 2), Oxyrhopus rhombifer (n = 1) and Micrurus corallinus (n = 1). The only tick species found to be associated with any of the reptiles sampled was A. rotundatum. One adult female was detected on one individual of the lizard A. ameiva, one nymph on one individual of the lizard T. torquatus and four nymphs on one individual of the snake P. olfersii. This study is the first record of parasitism of A. rotundatum involving the reptiles T. torquatus and P. olfersii as hosts. Our results suggest that in the Grussaí restinga habitat, A. rotundatum may use different species of reptiles to complete its life cycle.Amblyomma rotundatum Koch é um carrapato partenogenético geralmente associado a répteis e anfíbios. O objetivo deste estudo foi analisar a presença de carrapatos associados às espécies de répteis em uma comunidade na restinga de Grussaí, município de São João da Barra, Estado do Rio de Janeiro, Brasil. O presente estudo constitui o primeiro registro do parasitismo de A. rotundatum nos répteis T. torquatus e P. olfersii como hospedeiros. Nossos resultados sugerem que no habitat da restinga de Grussaí, A. rotundatum pode utilizar diferentes espécies de répteis para a realização do seu ciclo de vida.

A partir de teleógina de Amblyomma nodosum Neumann, 1899, removida de Tamandua tetradactyla (Linnaeus, 1758) foram obtidas posturas que originaram larvas, em condições de laboratório. Um total de 219 larvas foi examinado em microscopia de luz, com objetiva de imersão e microscopia de confocal com varredura a laser. Buscou-se analisar a variação do número das cerdas internas dos lados direito e esquerdo, no fundo da cápsula do Órgão de Haller, com intuito de utilizá-las como um parâmetro quetotáxico na diagnose específica. A média do número de cerdas foi 5,06±0, 71, com valores limites da amplitude de 4-11.Larvae from Amblyomma nodosum Neumann, 1899 from engorged female ticks removed from Tamandua tetradactyla (Linnaeus, 1758) were obtained under laboratory conditions. Two hundred and nineteen larvae were examined under brightfiel microscopy with and imersion objective and confocal laser scanning microscopy in order to analyze the variation of the number of internal setae of the right and left side of the Haller's organ chamber. The mean number of internal setae was 5,06±0,71, with parametric valves of 4-11 setae.

Ácaros da família Cheyletidae são de grande importância na veterinária, por estarem associados com parasitismo em aves e mamíferos. Permanecem, contudo, pouco estudados como fatores etiológicos em dermatites humanas, apesar de serem encontradas referências a esse respeito desde o início do século 20. O objetivo dos autores foi o de analisar, em paciente de seis meses de idade, um caso clínico de dermatite por ácaros da família Cheyletidae. O achado de três espécies dessa família no ecossistema domiciliar desse paciente, associado às manifestações cutâneas e cura definitiva após higienização do ambiente, faz acreditar na necessidade de se considerar essa hipótese diagnóstica nas dermatites humanas.Cheyletidae mites are of great importance in the veterinary sciences, as they are associated with parasitism in birds and mammals. Yet they are barely studied as etiological factors to human dermatitis in spite of being referred to since the beginning of the 20th century. The authors propose to analyze a clinical case of dermatitis caused by mites of the Cheyletidae family in a six-month old patient. The discovery of three species of that family in the patient's domiciliary ecosystem, associated to the cutaneous manifestations correlated with its definitive cure after this environment was sanitized, leads the authors to believe in the need to consider this diagnostic hypothesis for human dermatitis

Coprolite samples of human and animal origin from the excavations performed at the archaeological site of Furna do Estrago, at Brejo da Madre de Deus in the state of Pernambuco, Brazil and sent to the Paleoparasitology Laboratory at Escola Nacional de Saúde Pública-Fiocruz, Rio de Janeiro, were analyzed for mites. After rehydratation and sedimentation of the coprolites, the alimentary contents and the sediments were examined and the mites collected and prepared in definitive whole mounts, using Hoyer's medium. Mites of the following suborders and orders were recovered: suborder Acaridia; order Gamasida; order Ixodida with the familiy Ixodidae (Ixodes sp. and Amblyomma sp. larvae, scutum, idiosoma, gnathosoma); order Oribatida (Aphelacarus sp., Apolohmannia sp., Eophypochthonius sp., Cosmochthonius sp., Pterobates sp., Poronoticae with pteromorphae not auriculate); order Astigmata with the families Atopomelidae (Chirodiscoides caviae), Anoetidae hypopus, Acaridae (Suidasia pontifica), Glycyphagidae (Blomia tropicalis), Pyroglyphidae (Hirstia passericola); order Actinedida with the family Tarsonemidae (Iponemus radiatae). The present work discusses the possibility of the preservation of the mite groups found up to the present day. We also discuss their relationship with the environment and their importance to present populations.

Variation between aeropile numbers of the right and left peritrematic plate in male and female Anocentor nitens (Neumann, 1897) is reported from a site in Brazil. From January to December 1998, 146 males and 247 females of A. nitens were recovered from Equus caballus L. in Silva Jardim District, State of Rio de Janeiro. Asymmetry of numbers of aeropiles between right and left plates occurred in 83.6% of the males and 82.2% of the females. Differences in the number of aeropiles between the sexes were not significant. Quantitative variation of aeropiles was correlated to the period of recovery, with significant asymmetry detected in August-September and November-December, mainly in males. Results suggest an adaptation, especially in the male ticks, that expresses itself as greater variation in the number of aeropiles in some periods of the year.

Mites collected from the auditory canal of Cebus apella (capuchin monkey), family Cebidae, were identified as Fonsecalges johnjadini (Psoroptidae, Cebalginae). It is the first record of this parasite from this monkey. This paper emphasizes the importance of clinical and anatomopathological examinations for parasitic diagnosis in wild animals.

A respiração bucal (RB) pode ter diversas causas, tais como: rinite alérgica, desvios de septo nasal, postura incorreta, pólipos nasais, alterações anatômicas congênitas, abscessos, tumores, constituindo a rinite alérgica uma das mais comumente encontradas. O objetivo deste trabalho foi avaliar a importância da rinite alérgica como causa de respiração bucal. Foram analisados 31 pacientes, seguindo-se um protocolo preestabelecido: anamnese, aplicação de questionário aos pacientes, exame clínico, exames radiográficos, análise de saliva e testes cutâneos de leitura imediata. Reação positiva para ácaros esteve presente, sobretudo, para Dermatophagoides pteronyssinus e Blomia tropicalis. A análise da razão de chance (OR) demonstrou que são muito acentuadas as chances de um paciente com rinite alérgica desenvolver RB em relação aos pacientes normais. Este resultado foi corroborado pelo Teste de Correlação de Spearman.

ABSTRACT: Equine piroplasmosis is a tick-borne disease caused by the intraeytrhocytic protozoans Babesia caballi and Theileria equi. It has been reported as a main equine parasitic disease. In addition, Anaplasma phagocytophilum, the causative agent of granulocytic ehrlichiosis, causes a seasonal disease in horses. Both diseases, can be detrimental to animal health. In this sense, blood samples and ticks were collected from 97 horses raised in the microregion of Baixada Maranhense, Maranhão State, Brazil. Serum samples were subjected to Indirect Fluorescence Antibody Test (IFAT) and blood samples and ticks to Polymerase Chain Reaction (PCR) to evaluate the infection by Theileria equi, Babesia caballi and Anaplasma phagocytophilum. The overall seroprevalence was 38.14%, 18.55% and 11.34% for T. equi, B. caballi and A. phagocytophilum, respectively. The results of PCR from blood samples showed 13.40% and 3.09% positive samples to T. equi and B. caballi, respectively. A total of 170 tick specimens were collected and identified as Dermacentor nitens, Amblyomma cajennense sensu lato and Rhipicephalus (Boophilus) microplus. It was detected 2.35% (4/170) and 0.59% (1/170) positive tick samples by PCR for T. equi and B. caballi, respectively. All samples were negative to A. phagocytophilum. No statically difference (p>0.05) was observed when gender, age, use of ectoparasiticide and tick presence were analyzed. A BLASTn analysis of the sequenced samples indicated 97 to 100% similarity with T. equi 18S rRNA gene sequences in GenBank and 98 to 100% with B. caballi. Genetic analysis classified the obtained sequences as T. equi and B. caballi cluster, respectively. It can be concluded that these pathogens occur and are circulating in the studied area.

Resumo Introdução A febre maculosa é uma doença infecciosa aguda causada por bactérias do gênero Rickettsia e transmitida por carrapatos que pode levar à hospitalização e, eventualmente, a óbito por causa de quadros mais graves. Objetivo O objetivo deste artigo foi descrever o cenário epidemiológico da doença nas três unidades federativas de maior número de notificações e óbitos pela doença no Brasil entre 2007 e 2016. Método Estudo epidemiológico descritivo com base nos dados das fichas de investigação para a doença, sendo analisados dados demográficos, geográficos, ambientais e de hospitalização, diagnóstico e desfecho dos casos na área de estudo. Resultados Foram confirmados 953 casos da doença, em sua maioria por critérios laboratoriais, sendo observada maior prevalência entre homens com idade média de 35 anos, pouco relacionados ao ambiente de trabalho, mas amplamente relacionados ao meio urbano. Do total de casos confirmados, 64% pacientes afirmaram ter tido algum contato com carrapato, assim como com cães (36%), capivaras (20%) e equinos (19%). Conclusão Existem questionamentos a respeito da doença que não foram elucidados neste artigo, entretanto abordagens criativas, validadas e ancoradas na associação de áreas de conhecimento distintas poderão dar bons resultados se objetivamente aplicadas em ações estratégicas da gestão da saúde.

One Health é uma estratégia global de expansão das colaborações interdisciplinares e de comunicação entre grupos de várias nações beneficiando os cuidados de saúde, em seus aspectos mais amplos, para seres humanos, animais e o meio ambiente. Ecohealth é um movimento que busca a saúde sustentável para seres vivos e ecossistemas, promovendo a compreensão dessa complexidade através de metodologias que encorajem a resolução de problemas pela reunião de múltiplos conhecimentos oriundos das ciências sociais, naturais e da saúde. O objetivo desse artigo é descrever possíveis desafios para os que militem na Saúde Ambiental analisando similaridades e discrepâncias dos diferentes discursos quanto à emergência e reemergência de antropozoonoses por meio de uma revisão narrativa da literatura.

Abstract Piroplasm species were analyzed by molecular tools in total 31 blood samples from positive dogs, previously checked by stained slides, stored until DNA extraction between 2016 to 2018 in the laboratory Clinical Analyzes in Niterói, Rio de Janeiro. The piroplasms were identified by PCR, targeting the 18S rRNA gene and sequencing. From the total number of samples only 24 (77.4%) were positive and show adequate nucleotide sequences for interpretation with identity between 93%-100% with Babesia vogeli in compared to the sequences isolated of infected dogs from other states in Brazil deposited on GenBank. Most of dogs infected with B. vogeli had anemia (62.5%) and thrombocytopenia (95.8%). The findings of this study are compatible with previous reports in the literature and highlight B. vogeli as the most incriminated species in canine piroplasmosis in Brazil, and thrombocytopenia the hematological alteration most frequently identified in this infection. It is important to note that this is the first study involving the molecular characterization of piroplasms in the metropolitan region of Rio de Janeiro, based on PCR followed by sequencing.

Abstract. Four bulb mites deutonymphs (hypopus) from Rhizoglyphus echinopus (Fumouze & Robin) (Acari: Acaridae) were found attached to the head of a subterranean termite belonging to Coptotermes genus in Rio de Janeiro, Brazil. These mite species are commonly associated with ornamental plants and trees with bulbs, corms and tubers. All four hypopus were found attached to the surface of termite’s head. Results from this study provided an insight on the phoretic relationship of mites and termites, indicating the role of the latter in the dispersion of the first. Quatro deutoninfas de ácaros do bulbo (hipopus) da espécie Rhizoglyphus echinopus (Fumouze & Robin) (Acari: Acaridae) foram encontradas fixadas na cabeça do cupim subterraneo pertencente ao gênero Coptotermes no Rio de Janeiro, Brasil. Estas espécies de ácaros estão comumente associadas a plantas ornamentais e plantas com bulbos e turbéculos. As quatros hipopus foram encontradas fixadas na superfície da cabeça do cupim. Os resultados desse estudo fornecem uma visão sobre a relação forética de ácaros e cupins, indicando o papel deste último na dispersão do primeiro.

The Rickettsia bacteria include the aetiological agents for the human spotted fever (SF) disease. In the present study, a SF groupRickettsia amblyommii related bacterium was detected in a field collected Amblyomma sculptum (Amblyomma cajennense species complex) tick from a Brazilian SF endemic site in southeastern Brazil, in the municipality of Juiz de Fora, state of Minas Gerais. Genetic analysis based on genes ompA,ompB and htrA showed that the detected strain, named R. amblyommii str. JF, is related to the speciesR. amblyommii.

ABSTRACT Spotted fevers are diseases caused by bacterial agents belonging to the spotted-fever (SF) group of the genus Rickettsia. The first documented case of SF in Pernambuco State, Northeast Brazil, was reported here. Also, it is the first case described of fatal SF in Northeast region of Brazil. The patient was a resident of Arcoverde municipality and the probable site of infection lies in Sertania municipality, both in Pernambuco State, a semi-arid region of Brazil. The patient had not visited other areas where SF is endemic. The patient showed clinical manifestations and epidemiological exposure compatible with SF, and the infection was confirmed by molecular biology techniques.

This work analyses the performance of the Brazilian spotted fever (SF) surveillance system in diagnosing and confirming suspected cases in the state of Rio de Janeiro (RJ), from 2007 to 2016 (July) using machine-learning techniques. Of the 890 cases reported to the Disease Notification Information System (SINAN), 11.7% were confirmed as SF, 2.9% as dengue, 1.6% as leptospirosis, and 0.7% as tick bite allergy, with the remainder being diagnosed as other categories (10.5%) or unspecified (72.7%). This study confirms the existence of obstacles in the diagnostic classification of suspected cases of SF by clinical signs and symptoms. Unlike man–capybara contact (1.7% of cases), man–tick contact (71.2%) represents an important risk indicator for SF. The analysis of decision trees highlights some clinical symptoms related to SF patient death or cure, such as: respiratory distress, convulsion, shock, petechiae, coma, icterus, and diarrhea. Moreover, cartographic techniques document patient transit between RJ and bordering states and within RJ itself. This work recommends some changes to SINAN that would provide a greater understanding of the dynamics of SF and serve as a model for other endemic areas in Brazil.

Amblyomma oblongoguttatum is a tick that is a parasite of mammals and its preferred hosts are the wild pigs, tapirs and small rodents, being collected also parasitizing domestic dogs and eventually humans. An Amblyomma oblongoguttatum larva with morphological abnormality was collected along with other specimens (larvae and females) of this species and with morphologically normal Amblyomma naponense nymphs in a wild pig - Pecari tacaju - in a rural area of the state of Rondônia / Brazil, during research of tick-borne disease environments. The morphologically abnormal specimen showed a bifurcation of the posterior region of the opisthosoma, without other altered morphological characters. This is the first record of morphological abnormality at the larval stage of Amblyomma oblongoguttatum.

Abstract Background Spotted fever is a tick-borne rickettsial disease. In Brazil, its notification to the Ministry of Health is compulsory. Since 2007, cases of spotted fever have been integrated to the Notifiable Diseases Information System, and epidemiological analyzes are part of the routines on surveillance programs. Methods This descriptive study updates epidemiological information on cases of spotted fever registered in Brazil between 2007 and 2015. Results In Brazil, 17,117 suspected cases of the disease were reported and 1,245 were confirmed in 12 states, mainly in São Paulo (550, 44.2 %) and Santa Catarina (276, 22.2 %). No geographic information was registered for 132 cases (10.6 %). Most of the infected people were men (70.9 %), mainly in rural areas (539, 43.3 %), who had contact with ticks (72.7 %). A higher number of suspected cases were registered between 2011 and 2015, but the number of confirmed cases and the incidence were relatively low. Moreover, 411 deaths were registered between 2007 and 2015, mainly in the southeastern region of the country, where the case-fatality rate was 55 %. Lack of proper filling of important fields of notification forms was also observed. Conclusions The results showed expansion of suspected cases of spotted fever and high case-fatality rates, which could be related to diagnostic difficulties and lack of prompt treatment. These factors may comprise limitations to the epidemiological surveillance system in Brazil, hence improvement of notification and investigation are crucial to reduce morbidity and mortality due to spotted fever in Brazil.

OBJETIVO: Avaliar a prevalência dos atendimentos por crises de asma nos serviços públicos de Juiz de Fora (MG), no período de 01/02/2002 a 31/01/2003. MÉTODOS: Realizou-se estudo seccional, mensalmente, avaliando-se os atendimentos por crises de asma. As variáveis analisadas foram sexo, idade, local de residência dos pacientes, diferenças no perfil dos atendimentos segundo a faixa etária e o local de atendimento, além da correlação das crises asmáticas com temperatura e umidade relativa do ar. RESULTADOS:Foram atendidos 25.243 pacientes com crises asmáticas, representando 11,1% do total de consultas da clínica pediátrica e 2,7% do total de consultas da clínica médica. A média de idade dos pacientes foi de 11,6 anos e a mediana de quatro anos. Eram do sexo masculino 52,9% dos pacientes. A análise da distribuição anual evidenciou um aumento do número de atendimentos por crises asmáticas nos meses de maio (11,5%), junho (10,8%) e julho (10,9%), com correlação inversa e forte para temperatura (-0,86) e inversa e fraca para umidade relativa (-0,27). Houve diferença estatisticamente significativa entre as médias de idade e sexo, quando se compararam os serviços de urgência e as unidades básicas de saúde. A avaliação segundo a faixa etária demonstrou predomínio do sexo masculino nos pacientes menores de doze anos e do feminino nos maiores ou com doze anos. CONCLUSÃO: Observou-se a importância da asma como problema de saúde pública no município, o qual necessita de estratégias locais urgentes que permitam aumentar os investimentos em tratamentos profiláticos.OBJECTIVE: To evaluate the prevalence of asthma attacks treated in public health facilities in the city of Juiz de Fora, Brazil from February 1, 2002 to January 31, 2003. METHODS:This was a cross-sectional study involving monthly analyses performed in order to evaluate the asthma attacks treated. The variables analyzed were patient gender, age, and place of residence, as well as the profile of the visits by age bracket and by the site of the visit. In addition, we attempted to determine whether asthma attacks correlated with temperature or relative humidity. RESULTS: A total of 25,243 patients were treated for asthma attacks, accounting for 11.1% of the total number of visits to pediatric clinics and 2.7% of the total number visits to medical clinics. The mean age of the patients was 11.6 years (median, 4 years), and 52.9% of the patients were male. The analysis of the annual distribution revealed an increase in the number of asthma attacks treated in the months of May (11.5%), June (10.8%), and July (10.9%). Asthma attack presented a strong inverse correlation with temperature (-0.86) and a weak inverse correlation with relative humidity (-0.27). Statistically significant differences, in terms of mean age and gender, were found between patients with asthma attacks treated in emergency rooms and those treated in basic health clinics. The evaluation by age bracket revealed a predominance of males among patients younger than 12 years of age and a predominance of females among those aged 12 or older. CONCLUSION: We found that asthma represents a significant public health problem in this city. Local strategies are urgently needed in order to increase in the availability of prophylactic treatment.

Ticks were collected from vegetation and animals at monthly intervals during one year (1993-1994) in an endemic area of Brazilian spotted fever in the County of Pedreira, State of São Paulo. Six species of ticks were identified Amblyomma cajennense, Amblyomma cooperi, Amblyomma triste, Anocentor nitens, Rhipicephalus sanguineus and Boophilus microplus. Only the first species was sufficiently numerous to permit a quantitative study with seasonal activity, although the distribution and source of capture of other species were observed and are reported. This information is correlated with the epidemiology of tick-borne rickettsiosis.Carrapatos de vegetação e de animais foram coletados mensalmente durante o período de um ano (1993-1994) em uma área endêmica de febre maculosa brasileira no município de Pedreira, São Paulo. Seis espécies de carrapatos foram identificadas Amblyomma cajennense, Amblyomma cooperi, Amblyomma triste, Anocentor nitens, Rhipicephalus sanguineus e Boophilus microplus. Somente a primeira espécie foi suficientemente abundante para permitir um estudo quantitativo com atividade sazonal, embora a distribuição e fonte de captura de outras espécies fossem observadas e aqui relatadas. Estas informações são correlacionadas com a epidemiologia da rickettsiose transmitida por carrapato.

AbstractTicks represent a large group of pathogen vectors that blood feed on a diversity of hosts. In the Americas, the Ixodidae ticks Amblyomma cajennense are responsible for severe impact on livestock and public health. In the present work, we present the isolation and molecular identification of a group of culturable bacteria associated with A. cajennense eggs from females sampled in distinct geographical sites in southeastern Brazil. Additional comparative analysis of the culturable bacteria from Anocentor nitens, Rhipicephalus sanguineus and Ixodes scapularis tick eggs were also performed. 16S rRNA gene sequence analyses identified 17 different bacterial types identified as Serratia marcescens, Stenotrophomonas maltophilia, Pseudomonas fluorescens, Enterobacter spp., Micrococcus luteus, Ochrobactrum anthropi, Bacillus cereus and Staphylococcus spp., distributed in 12 phylogroups. Staphylococcus spp., especially S. sciuri,was the most prevalent bacteria associated with A. cajennenseeggs, occurring in 65% of the samples and also frequently observed infecting A. nitens eggs. S. maltophilia, S. marcescens and B. cereus occurred infecting eggs derived from specific sampling sites, but in all cases rising almost as pure cultures from infected A. cajennense eggs. The potential role of these bacterial associations is discussed and they possibly represent new targets for biological control strategies of ticks and tick borne diseases.

Abstract Brazilian spotted fever (BSF) is caused by the bacterium Rickettsia rickettsii. Because of its high case-fatality rate and apparent increase in areas of transmission, it is considered to be the rickettsial illness of primary public health interest. Cases of this disease have historically occurred in Southeastern Brazil. This article reports the first fatal case of BSF in Southern Brazil. This case high lights the importance of BSF to be considered as a differential diagnosis for acute hemorrhagic fever in areas where cases of BSF may not be expected.
