1. Adequacy to immunosuppression management guidelines in kidney transplant recipients with severe COVID-19 pneumonia: a practice survey

Abstract

IntroductionCoronavirus disease 2019 (COVID-19) poses an important risk of morbidity and of mortality, in patients after solid organ transplantation. Recommendations have been issued by various transplantation societies at the national and European level to manage the immunosuppressive (IS) regimen upon admission to intensive care unit (ICU).MethodThe aim of this study was to evaluate the adequacy of IS regimen minimization strategy in kidney transplant recipients hospitalized in an ICU for severe COVID-19, in relation to the issued recommendations.ResultsThe immunosuppressive therapy was minimized in all patients, with respectively 63% and 59% of the patients meeting the local and european recommendations upon admission. During ICU stay, IS was further tapered leading to 85% (local) and 78% (european) adequacy, relative to the guidelines. The most frequent deviation was the lack of complete withdrawal of mycophenolic acid (22%). Nevertheless, the adequacy/inadequacy status was not associated to the ICU- or one-year-mortality.DiscussionIn this single-center cohort, the only variable associated with a reduction in mortality was vaccination, emphasizing that the key issue is immunization prior to infection, not restoration of immunity during ICU stay.

Abstract

BACKGROUND AND OBJECTIVES: The prognosis of patients undergoing kidney tumor resection or kidney donation is linked to many histologic criteria. These criteria notably include glomerular density, glomerular volume, vascular luminal stenosis, and severity of interstitial fibrosis/tubular atrophy. Automated measurements through a deep-learning approach could save time and provide more precise data. This work aimed to develop a free tool to automatically obtain kidney histologic prognostic features. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: In total, 241 samples of healthy kidney tissue were split into three independent cohorts. The “Training” cohort (n=65) was used to train two convolutional neural networks: one to detect the cortex and a second to segment the kidney structures. The “Test” cohort (n=50) assessed their performance by comparing manually outlined regions of interest to predicted ones. The “Application” cohort (n=126) compared prognostic histologic data obtained manually or through the algorithm on the basis of the combination of the two convolutional neural networks. RESULTS: In the Test cohort, the networks isolated the cortex and segmented the elements of interest with good performances (>90% of the cortex, healthy tubules, glomeruli, and even globally sclerotic glomeruli were detected). In the Application cohort, the expected and predicted prognostic data were significantly correlated. The correlation coefficients r were 0.85 for glomerular volume, 0.51 for glomerular density, 0.75 for interstitial fibrosis, 0.71 for tubular atrophy, and 0.73 for vascular intimal thickness, respectively. The algorithm had a good ability to predict significant (>25%) tubular atrophy and interstitial fibrosis level (receiver operator characteristic curve with an area under the curve, 0.92 and 0.91, respectively) or a significant vascular luminal stenosis (>50%) (area under the curve, 0.85). CONCLUSION: This freely available tool enables the automated segmentation of kidney tissue to obtain prognostic histologic data in a fast, objective, reliable, and reproducible way.

4. Deep learning automation of MEST-C classification in IgA nephropathy

Abstract

Background Although the MEST-C classification is among the best prognostic tools in immunoglobulin A nephropathy (IgAN), it has a wide interobserver variability between specialized pathologists and others. Therefore we trained and evaluated a tool using a neural network to automate the MEST-C grading. Methods Biopsies of patients with IgAN were divided into three independent groups: the Training cohort (n = 42) to train the network, the Test cohort (n = 66) to compare its pixel segmentation to that made by pathologists and the Application cohort (n = 88) to compare the MEST-C scores computed by the network or by pathologists. Results In the Test cohort, >73% of pixels were correctly identified by the network as M, E, S or C. In the Application cohort, the neural network area under the receiver operating characteristics curves were 0.88, 0.91, 0.88, 0.94, 0.96, 0.96 and 0.92 to predict M1, E1, S1, T1, T2, C1 and C2, respectively. The kappa coefficients between pathologists and the network assessments were substantial for E, S, T and C scores (kappa scores of 0.68, 0.79, 0.73 and 0.70, respectively) and moderate for M score (kappa score of 0.52). Abstract

International audience; The current frontiers in Biology thus in Medicine and Pharmacy are at the nanoscale. Indeed, this is the relevant scale for extracting or synthetizing, visualizing, counting, characterizing and/or modifying nanoparticles. Nanoparticles are highly diverse including: extracellular vesicles (e.g.: exosomes), proteins, viruses and nanovectors or drug delivery systems for instance. To quantify the concentration of nano-sized objects, a growing number of size-tracking instruments is being developed. However, to date, the generated data is only used to provide a concentration measurement. The objective of this study was to determine which sizes of nanoparticles are statistically significant between 2 groups of samples. Using different samples (in silico data; calibrated beads; various biological samples), an approach that statistically compares 2 groups of samples was developed and validated. The proof of concept of the proposed approach was illustrated with applications in the field of Biology, Medicine and Pharmacy using liposomes and extracellular vesicles. For the first time to our knowledge, our results suggest that the presented approach enables comparing different groups of biological samples. It may be extended to situations such as batch 1 versus batch 2; healthy versus disease or non-treated versus treated.

Abstract

The aim of this study was to estimate the incidence of COVID-19 disease in the French national population of dialysis patients, their course of illness and to identify the risk factors associated with mortality. Our study included all patients on dialysis recorded in the French REIN Registry in April 2020. Clinical characteristics at last follow-up and the evolution of COVID-19 illness severity over time were recorded for diagnosed cases (either suspicious clinical symptoms, characteristic signs on the chest scan or a positive reverse transcription polymerase chain reaction) for SARS-CoV-2. A total of 1,621 infected patients were reported on the REIN registry from March 16th, 2020 to May 4th, 2020. Of these, 344 died. The prevalence of COVID-19 patients varied from less than 1% to 10% between regions. The probability of being a case was higher in males, patients with diabetes, those in need of assistance for transfer or treated at a self-care unit. Dialysis at home was associated with a lower probability of being infected as was being a smoker, a former smoker, having an active malignancy, or peripheral vascular disease. Mortality in diagnosed cases (21%) was associated with the same causes as in the general population. Higher age, hypoalbuminemia and the presence of an ischemic heart disease were statistically independently associated with a higher risk of death. Being treated at a selfcare unit was associated with a lower risk. Thus, our study showed a relatively low frequency of COVID-19 among dialysis patients contrary to what might have been assumed., Graphical abstract

Abstract

Fusobacterium nucleatum is a common oral commensal bacterium capable of severe invasive infections. We report a case of a diffuse bilateral pneumopathy with F. nucleatum-positive blood culture successfully treated by common antibiotics in a patient receiving eculizumab for a drug-induced thrombotic microangiopathy (TMA). It is the first described case of a severe F. nucleatum-associated infection in a patient undergoing terminal complement inhibitor therapy. We suggest providing preventive dental care before eculizumab initiation.

Abstract

Hepatitis B virus (HVB) infection is a worldwide epidemic that, in addition to the liver, causes damage in other organs. HBV-membranous nephropathy (HBV-MN) is the most common renal manifestation in patients with HBV infection,1, 2, 3 possibly linked to subepithelial deposits of viral immune complexes.4, 5 HBV RNA and DNA were detected in glomerular and tubular cells, although a pathogenetic role for these viral nucleic acids remains to be confirmed.6 The effects of vaccination programs on the decline of HBV-associated MN, particularly in Asian and African children,7, 8 further support a possible relationship between HBV infection and MN.9 Identification in 2009 of PLA2R as the major antigen associated with idiopathic membranous nephropathy provided the first diagnostic biomarker.10 The 2 available assays (immunofluorescence test assay [IFTA] and enzyme-linked immunosorbent assay) for the detection of circulating anti-PLA2R antibodies (PLA2R-Abs) have shown that PLA2R-Abs are a specific and sensitive biomarker of MN, being detected in 70% to 80% of patients with primary MN worldwide in early serum samples, and consistently absent in healthy individuals and in patients with other nephropathies or autoimmune diseases.11 Because anti-PLAR antibody levels fluctuate and can rapidly decrease under immunosuppressive treatment, detection of PLA2R antigen in subepithelial immune deposits of patients with MN is even more sensitive than serology.12, 13 Although Hoxha et al. suggested that the diagnosis between primary MN and secondary MN could rely on the presence or absence of PLA2R antigen in the immune deposits,14 other investigators showed that the antigen could be detected in cases of active sarcoidosis,15, 16 hepatitis C virus,17 and HBV infection–associated MN.13, 18 In the Chinese population, 25 of 39 patients (64%) with HBV-MN had deposited subepithelial PLA2R antigen, and all tested sera contained anti-PLA2R antibody.18 These findings suggest that HBV infection could be the trigger of autoimmunity anti-PLA2R. Whatever the mechanism of autoimmunity, we are facing the conundrum of antiviral versus immunosuppressive therapy with the risk of aggravating the viral infection. Most data have been obtained in small-scale case series published before the availability of anti-PLA2R antibody assays, and a standard of care has not yet been established.19, 20 Here, we report the first 2 cases in which rituximab was used after controlling viral replication.

11. Unusual Case of HIT With Cardiac Arrest During Hemodialysis

Abstract

Objective: To report an unusual case of heparin-induced thrombocytopenia (HIT) with cardiac arrest during hemodialysis (HD). Case Summary: An 88-year-old man previously treated with HD under enoxaparin for 3 years presented with dizziness and cyanosis at the beginning of HD on 3 consecutive sessions. Even though the dialyzer membrane was changed, he presented with cardiac arrest, from which he recovered quickly. At the same time, the platelet count fell, and HIT was suspected. No thrombosis was found. Anti-PF4/H, IL8, and NAP2 antibodies were negative, but platelet aggregation tests and serotonin-release assay were positive. After implementing HD with danaparoid, the platelet count returned to normal, and the patient remained asymptomatic. Discussion: Given the clinical context (low-molecular-weight heparin), complications (cardiac arrest and no thrombosis), and timing (3 years), this was an unusual case of HIT. According to the Naranjo probability scale, the causality of enoxaparin was evaluated as probable. In most reported cases, time to onset was short, clotting occurred in the extracorporeal system, and biological tests, including ELISA (enzyme-linked immunosorbent assay) anti-PF4/heparin, were positive. We found no triggering factor in this case, and given the biological results, a new antigenic target may be involved. Conclusions: HIT must be considered when acute systemic reactions occur at the beginning of HD sessions, even after several years of HD and with no change of anticoagulant, including low-molecular-weight heparin. The platelet count should be measured immediately after the reaction. The diagnosis is important because of possible cardiac arrest in this context.

Abstract

Mycophenolate mofetil (MMF) is a drug which decreases the frequency of renal transplantation rejection. However, cytomegalovirus infections are a common feature of this treatment leading the physicians to prescribe antiviral prophylactic drugs like valacyclovir. During this association, neutropenia occur and the cause of this adverse effect is difficult to define. This report presents a case of neutropenia in a woman treated with MMF and valacyclovir. As the duration of the valacyclovir treatment exactly corresponds to the neutropenia duration, and the mycophenolate trough levels increased with the neutrophil count, the responsibility of this neutropenia was ascribed to valacyclovir. However, an examination of the literature for cases of neutropenia led to the suspicion of an interaction between MMF and valacyclovir. Mycophenolate may increase intracellular concentrations of valacyclovir up to haematotoxic levels. This mechanism may explain the interaction and further research is needed to confirm this interaction.

Abstract

A 72-year-old patient treated with BCG immunotherapy for relapse of urothelial cancer was admitted for diffuse oedema. His past history was characterized by recurrent pleuropericarditis treated by pericardotomy 16 years earlier and by retroperitoneal fibrosis (RPF) treated by steroids 15 years earlier. Serum creatinine rose progressively from 190 to 618 μmol/L and he was admitted in the nephrology department. Clinical examination showed diffuse oedema, dyspnoea and mild tibial tenderness. Haemoglobin was 99 g/L with normal platelet and leukocyte counts. Calcaemia, phosphoraemia, magnesaemia, haptoglobin and lactate deshydrogenase levels were within the normal range. Serum was negative for antinuclear and antineutrophil cytoplasm antibodies. Bilateral pyelic dilatation was diagnosed and treated by bilateral ureteral stenting. Echocardiography, cardiac magnetic resonance imaging (MRI) and right cardiac catheterization confirmed restrictive cardiopathy related to constrictive pericarditis with severe thickening of the lateral wall of the pericardium (not shown). An abdominal CT scan was performed and is shown in Figure 1.

Abstract

Foscarnet nephrotoxicity has been reported to be associated with acute tubulointerstitial nephritis. Crystals in glomerular capillary lumens have also been observed in patients with acquired immunodeficiency syndrome who were treated with foscarnet for cytomegalovirus disease. We describe a kidney transplant recipient who developed a nephrotic syndrome with microscopic hematuria and nonoliguric acute renal failure within 15 days after starting foscarnet therapy for cytomegalovirus infection. A kidney biopsy specimen showed the presence of crystals in all glomeruli and in proximal tubules. Fourier transform infrared microscopy analysis demonstrated that crystals were made from several forms of foscarnet salts: mixed calcium and sodium salts, and unchanged trisodium foscarnet salts. Renal function and proteinuria spontaneously improved, and a second transplant biopsy performed 8 months after the first one revealed fibrotic organization of half of the glomeruli and of interstitial tissue, and crystal vanishing. We were thus able to provide proof of the possible precipitation of foscarnet in a transplanted kidney.

Abstract

Although described in a number of necropsy studies, endocarditis on mitral annular calcification (MAC) has rarely been reported during life. The study aim was to assess the frequency and specific features of bacterial endocarditis complicating MAC.Data relating to 62 cases of infective endocarditis of the native mitral valve diagnosed with multiplane transesophageal echocardiography (TEE) over a five-year period were collected prospectively.Among 62 patients, 15 (24%) had vegetations originating from a calcified mitral annulus (group 1), while 47 had classic leaflet endocarditis (group 2). Group 1 patients differed significantly from group 2 patients with regard to: (i) higher incidence of diabetes mellitus and cancers; (ii) initial clinical presentation, with febrile coma or meningoencephalitis in 53% of cases; (iii) echocardiographic features, with significantly greater vegetations, presence of calcium-dense echoes within the vegetation, high frequency of ring abscess, and high frequency of para-annular ventriculoatrial leakage; and (iv) poorer clinical outcome, with 53% in-hospital mortality.MAC appears to be an underestimated predisposing factor for a particularly severe type of bacterial endocarditis. The use of multiplane TEE should improve current knowledge of this disease.
