Objective To evaluate the impact of coadministering statins with direct oral anticoagulants (DOACs) on the risk of major bleeding events in patients with venous thromboembolism (VTE).Design Observational cohort analysis based on a multicentre international registry.Setting Data were extracted from the Registro Informatizado de Enfermedad TromboEmbolica Registry, which involves 205 centres across 27 countries.Participants A total of 73 659 patients diagnosed with VTE were classified based on their anticoagulant therapy (DOACs) versus low-molecular-weight heparin (LMWH) or vitamin K antagonists (VKAs) and concurrent use of statins.Methods Multivariable Cox proportional hazards models adjusted for confounding variables to assess the risk of major bleeding events stratified by the type of anticoagulant use and statin use.Results From October 2013 to February 2023, 73 659 patients were recruited: 2573 were statin users on DOACs, 14 090 were statin users on LMWH or VKA therapy, 10 088 were non-statin users on DOACs and 46 908 were non-statin users on LMWH or VKA therapy. Statin users were 10 years older and more likely to have hypertension, diabetes, renal failure or prior artery disease. During anticoagulation (median, 187 days), 1917 patients (2.6%) suffered major bleeding. Rates of major bleeding per 100 patient-years were 2.33 (95% CI 1.72 to 3.09), 3.75 (95% CI 3.43 to 4.10), 1.39 (95% CI 1.13 to 1.69) and 3.10 (95% CI 2.93 to 3.27), respectively. On multivariable analysis, patients treated with DOACs had a significantly lower risk of major bleeding compared with those on LMWH or VKA therapy (adjusted HR 0.59; 95% CI 0.48 to 0.74). The adjusted HR in statin users versus non-users was 1.03 (95% CI 0.92 to 1.14), while in statin users on DOACs versus the rest of patients, it was 1.18 (95% CI 0.79 to 1.76).Conclusions In patients with VTE receiving statins, long-term anticoagulation with DOACs was associated with a reduced risk of major bleeding, regardless of the statin use. These findings support the safety profile of DOACs over VKAs or LMWH in the management of VTE in patients requiring statins.

Summary: Background: The clinical relevance of recurrent venous thromboembolism (VTE) after discontinuing anticoagulation in patients with COVID-19-associated VTE remains uncertain. We estimated the incidence rates and mortality of VTE recurrences developing after discontinuing anticoagulation in patients with COVID-19-associated VTE. Methods: A prospective, multicenter, non-interventional study was conducted between March 25, 2020, and July 26, 2023, including patients who had discontinued anticoagulation after at least 3 months of therapy. All patients from the registry were analyzed during the study period to verify inclusion criteria. Patients with superficial vein thrombosis, those who did not receive at least 3 months of anticoagulant therapy, and those who were followed for less than 15 days after discontinuing anticoagulation were excluded. Outcomes were: 1) Incidence rates of symptomatic VTE recurrences, and 2) fatal PE. The rate of VTE recurrences was defined as the number of patients with recurrent VTE divided by the patient-years at risk of recurrent VTE during the period when anticoagulation was discontinued. Findings: Among 1106 patients with COVID-19-associated VTE (age 62.3 ± 14.4 years; 62.9% male) followed-up for 12.5 months (p25-75, 6.3–20.1) after discontinuing anticoagulation, there were 38 VTE recurrences (3.5%, 95% confidence interval [CI]: 2.5–4.7%), with a rate of 3.1 per 100 patient-years (95% CI: 2.2–4.2). No patient died of recurrent PE (0%, 95% CI: 0–7.6%). Subgroup analyses showed that patients with diagnosis in 2021–2022 (vs. 2020) (Hazard ratio [HR] 2.86; 95% CI 1.45–5.68) or those with isolated deep vein thrombosis (vs. pulmonary embolism) (HR 2.31; 95% CI 1.19–4.49) had significantly higher rates of VTE recurrences. Interpretation: In patients with COVID-19-associated VTE who discontinued anticoagulation after at least 3 months of treatment, the incidence rate of recurrent VTE and the case-fatality rate was low. Therefore, it conceivable that long-term anticoagulation may not be required for many patients with COVID-19-associated VTE, although further research is needed to confirm these findings. In this paper a statistical study of cirrus geometrical and optical properties based on 4 years of continuous ground-based lidar measurements with the Barcelona (Spain) Micro Pulse Lidar (MPL) is analysed. First, a review of the literature on the two-way transmittance method is presented. This method is a well-known lidar inversion method used to retrieve the optical properties of an aerosol–cloud layer between two molecular (i.e. aerosol and cloud-free) regions below and above, without the need to make any a priori assumptions about their optical and/or microphysical properties. Second, a simple mathematical expression of the two-way transmittance method is proposed for both ground-based and spaceborne lidar systems. This approach of the method allows the retrieval of the cloud optical depth, the cloud column lidar ratio and the vertical profile of the cloud backscatter coefficient. The method is illustrated for a cirrus cloud using measurements from the ground-based MPL and from the spaceborne Cloud-Aerosol Lidar with Orthogonal Polarization (CALIOP). Third, the database is then filtered with a cirrus identification criterion based on (and compared to) the literature using only lidar and radiosonde data. During the period from November 2018 to September 2022, 367 high-altitude cirrus clouds were identified at 00:00 and 12:00 UTC, of which 203 were successfully inverted with the two-way transmittance method. The statistical results of these 203 high-altitude cirrus clouds show that the cloud thickness is 1.8 ± 1.1 km, the mid-cloud temperature is −51 ± 8 ∘C and the linear cloud depolarization ratio is 0.32 ± 0.13. The application of the transmittance method yields an average cloud optical depth (COD) of 0.36 ± 0.45 and a mean effective column lidar ratio of 30 ± 19 sr. Statistical results of the errors associated with the two-way transmittance method retrievals are also provided. The highest occurrence of cirrus is observed in spring and the majority of cirrus clouds (48 %) are visible (0.03 < COD < 0.3), followed by opaque (COD > 0.3) with a percentage of 38 %. Together with results from other sites, possible latitudinal dependencies have been analysed together with correlations between cirrus cloud properties. For example, we noted that in Barcelona the COD correlates positively with the cloud base temperature, effective column lidar ratio and linear cloud depolarization ratio and negatively with the cloud base height.

We derive an explicit (i.e., non-iterative) formula for the retrieval of the overlap function in an aerosol lidar with both elastic and Raman N2 and/or O2 channels used for independent measurements of aerosol backscatter and extinction coefficients. The formula requires only the measured, range-corrected elastic and the corresponding Raman signals, plus an assumed lidar ratio. We assess the influence of the lidar ratio error in the overlap function retrieval and present retrieval examples.

Introduction: Little is known about the clinical characteristics of women at increased risk for abnormal uterine bleeding (UB) during anticoagulation for venous thromboembolism (VTE). Methods: We used the RIETE registry to identify the baseline characteristics of women developing abnormal UB during anticoagulation. We used logistic regression analysis to identify independent predictors for abnormal UB. Then, we built a prognostic score to identify at-risk women. Results: From March 2001 through October 2022, there were 54,372 women with VTE. During anticoagulation (median, 181 days), 318 (0.6%) developed abnormal UB (major bleeding = 88, clinically relevant non-major (CRNM) = 230). On multivariable analysis, women aged 70 kg, with uterine cancer, recent UB, anemia, estrogen-related VTE, or receiving rivaroxaban or apixaban were at increased risk for abnormal UB. Using the prognostic score, 42,273 women (78%) were at low-risk, 8,828 (16%) intermediate-, and 3,271 (6.1%) at high-risk for abnormal UB. Their rates of abnormal UB were: 0.28 (95%CI: 0.23–0.35), 1.32 (95%CI: 1.07–1.61) and 7.12 (95%CI: 5.98–8.41) bleeds per 100 patient-years, respectively. The c-statistic was 0.80 (95%CI: 0.77–0.83). The rates of major UB were: 0.06 (95%CI: 0.04–0.09), 0.43 (95%CI: 0.30–0.60) and 1.85 (95%CI: 1.31–2.53) per 100 patient-years, respectively (c-statistic: 0.84; 95%CI: 0.80–0.89). The rates of CRNM uterine bleeding were: 0.21 (95%CI: 0.17–0.26), 0.85 (95%CI: 0.65–1.08), and 5.02 (95%CI: 4.09–6.10) bleeds per 100 patient-years, respectively (c-statistic: 0.78; 95%CI: 0.75–0.82). Conclusions: Using 7 variables easily available at admission, we built a prognostic score that reliably identified women with VTE at increased risk for abnormal UB during anticoagulation.

Abstract Mobility and bioavailability of radionuclides in the environment strongly depend on their aqueous speciation, adsorption behavior and the solubility of relevant solid phases. In the present context, we focus on naturally occurring Th-232 at a location in central Sri Lanka presenting high background radiation levels. Four different soil samples were characterized using X-ray Absorption Spectroscopy (XAS) at the Th L3-edge (16.3 keV), Scanning Electron Microscopy (SEM) and Energy Dispersive X-ray (EDX) spectroscopy. X-ray Absorption Near Edge Structure (XANES) spectra are applied as a fingerprint indication for Th existing in different chemical environments. Linear combination fitting (LCF) of the Extended X-ray Absorption Fine Structure (EXAFS) data involving reference Th-monazite (phosphate) and thorianite (oxide) compounds suggested that Th is mostly present as Th-phosphate (76 ± 2%) and Th-oxide (24 ± 2%), even though minor amounts of thorite (silicate) were also detected by SEM–EDX. Further studies on selected individual particles using micro-focus X-ray Fluorescence (μ-XRF) and micro-X-ray Absorption Spectroscopy (μ-XAS) along with SEM–EDX elemental mapping provided information about the nature of Th-bearing mineral particles regarding mixed phases. This is the first study providing quantitative and XAS based speciation information on Th-mineral phases in soil samples from Sri Lanka.

IntroductionParkinson's disease (PD) research is hampered by slow, inefficient recruitment and burdensome in-person assessments that may be challenging to conduct in a world affected by COVID-19. Fox Insight is an ongoing prospective clinical research study that enables individuals to participate in clinical research from their own homes by completing online questionnaires. To date, over 45,000 participants with and without PD have enrolled. We sought to validate self-reported PD diagnosis in the Fox Insight cohort, assess the validity of other self-reported health information, and evaluate the willingness of participants to participate in video-based research studies.MethodsIndividuals with and without self-reported PD enrolled in Fox Insight were invited to participate in this virtual research study. Participants completed online questionnaires and two virtual visits, during which we conducted standard cognitive and motor assessments. A movement disorder expert determined the most likely diagnosis, which was compared to self-reported diagnosis.ResultsA total of 203 participants from 40 U.S. states, 159 with remote clinician-determined PD and 44 without, completed the study (59% male, mean (SD) age 65.7 (9.8)). Level of agreement between self-reported PD diagnosis in Fox Insight and clinician-determined diagnosis was very good ((kappa = 0.85, 95% CI 0.76-0.94). Overall, 97.9% of participants were satisfied with the study, 98.5% were willing to participate in a future observational study with virtual visits, and 76.1% were willing to participate in an interventional trial with virtual visits.ConclusionAmong the Fox Insight cohort, self-reported diagnosis is accurate and interest in virtual research studies is high.

Home parenteral nutrition (HPN) is increasingly prescribed for patients with advanced cancer. This therapy improves free-fat mass, quality of life and survival, but it is not free from complications, especially catheter-related bloodstream infections (CRBSIs). The use of commercial multichamber bags in HPN has not been extensively explored in oncologic patients and their association with complications is not well known. In this prospective cohort study, we included 130 patients with advanced cancer and HPN. We compared the effects of individual compounded bags (n = 87) vs. commercial multichamber bags (n = 43) on complications. There were no differences in any complication, including thrombosis (p > 0.05). There were 0.28 episodes of CRBSI per 1000 catheter days in the individual compounded bag group and 0.21 in the multichamber bag group (p > 0.05). A total of 34 patients were weaned off HPN, 22 with individual bags and 12 with multichamber bags (p = 0.749). Regarding survival when on HPN, the group with individual bags showed a median of 98 days (95% CI of 49–147), whereas those with multichamber bags showed a median of 88 days (95% CI of 43–133 (p = 0.913)). In conclusion, commercial multichamber bags for HPN in patients with advanced cancer are non-inferior when compared to individual compounded bags in terms of complications.

This paper presents the estimation of the hygroscopic growth parameter of atmospheric aerosols retrieved with a multi-wavelength lidar, a micro-pulse lidar (MPL) and daily radiosoundings in the coastal region of Barcelona, Spain. The hygroscopic growth parameter, γ, parameterizes the magnitude of the scattering enhancement in terms of the backscatter coefficient following Hänel parameterization. After searching for time-colocated lidar and radiosounding measurements (performed twice a day, all year round at 00:00 and 12:00 UTC), a strict criterion-based procedure (limiting the variations of magnitudes such as water vapor mixing ratio (WMVR), potential temperature, wind speed and direction) is applied to select only cases of aerosol hygroscopic growth. A spectral analysis (at the wavelengths of 355, 532 and 1064 nm) is performed with the multi-wavelength lidar, and a climatological one, at the wavelength of 532 nm, with the database of both lidars. The spectral analysis shows that below 2 km the regime of local pollution and sea salt γ decreases with increasing wavelengths. Since the 355 nm wavelength is sensitive to smaller aerosols, this behavior could indicate slightly more hygroscopic aerosols present at smaller size ranges. Above 2 km (the regime of regional pollution and residual sea salt) the values of γ at 532 nm are nearly the same as those below 2 km, and its spectral behavior is flat. This analysis and others from the literature are put together in a table presenting, for the first time, a spectral analysis of the hygroscopic growth parameter of a large variety of atmospheric aerosol hygroscopicities ranging from low (pure mineral dust, γ γ > 1.0) hygroscopicity. The climatological analysis shows that, at 532 nm, γ is rather constant all year round and has a large monthly standard deviation, suggesting the presence of aerosols with different hygroscopic properties all year round. The annual γ is 0.55 ± 0.23. The height of the layer where hygroscopic growth was calculated shows an annual cycle with a maximum in summer and a minimum in winter. Former works describing the presence of recirculation layers of pollutants injected at various heights above the planetary boundary layer (PBL) may explain why γ, unlike the height of the layer where hygroscopic growth was calculated, is not season-dependent. The subcategorization of the whole database into No cloud and Below-cloud cases reveals a large difference of γ in autumn between both categories (0.71 and 0.33, respectively), possibly attributed to a depletion of inorganics at the point of activation into cloud condensation nuclei (CCN) in the Below-cloud cases. Our work calls for more in situ measurements to synergetically complete such studies based on remote sensing.

Abstract Little attention has been given to the development of remediation strategies for soils polluted with mixture of pollution (metal(loid)s and organic compounds). The present study evaluates the effectiveness of different types of commercial iron nanoparticles (nanoscale zero valent iron (nZVI), bimetallic nZVI-Pd, and nano-magnetite (nFe3O4)), for the remediation of an industrial soil co-contaminated with Cr and PCBs. Soil samples were mixed with nZVI, nZVI-Pd, or nFe3O4 at doses selected according to their reactivity with PCBs, homogenized, saturated with water and incubated at controlled conditions for 15, 45 and 70 days. For each sampling time, PCBs and chromium were analyzed in aqueous and soil fractions. Cr(VI) and Cr leachability (TCLP test) were determined in the soil samples. The treatment with the three types of iron nanoparticles showed significant reduction in Cr concentration in aqueous extracts at the three sampling times (> 98%), compared to the control samples. The leachability of Cr in treated soil samples also decreased and was stable throughout the experiment. Results suggested that nZVI and nZVI-Pd immobilized Cr through adsorption of Cr(VI) on the shell and reduction to Cr(III). The mechanism of interaction of nFe3O4 and Cr(VI) included adsorption and reduction although its reducing character was lower than those of ZVI nanoparticles. PCBs significantly decreased in soil samples (up to 68%), after 15 days of treatment with the three types of nanoparticles. However, nFe3O4 evidenced reversible adsorption of PCBs after 45 days. In general, nZVI-Pd reduced PCB concentration in soil faster than nZVI. Control soils showed a similar reduction in PCBs concentration as those obtained with nZVI and nZVI-Pd after a longer time (45 days). This is likely due to natural bioremediation, although it was not effective for Cr remediation. Results suggest that the addition of nZVI or nZVI-Pd and pseudo-anaerobic conditions could be used for the recovery of soil co-contaminated with Cr and PCBs.

The versatile Generalized Retrieval of Aerosol and Surface Properties (GRASP) algorithm exploits the advantages of synergic ground-based aerosol observations such as radiometric (sensitive to columnar aerosol optical and microphysical properties) and lidar (sensitive to vertical distribution of the optical properties) observations. The synergy is possible when the complementary data is mutually constrained by GRASP parametrization that includes, for the first time ever, the degree of linear polarization (DoLP) parameter measured by a polarized sun-sky-lunar AERONET photometer (380, 440, 500, 675, 870, 1020, and 1640 nm) in synergy with the vertical profiles from an elastic lidar (355, 532, and 1064 nm). First, a series of numerical tests is performed using simulated data generated using a climatology of data and ground-based measurements. The inversions are performed with and without random noise for five different combinations of input data, starting from the AERONET-like dataset and increasing to the complex one by adding more information for three aerosol scenarios: I—high aerosol optical depth (AOD) with dominant coarse mode; II—low AOD with dominant coarse mode; III—high AOD with dominant fine mode. The inclusion of DoLP improves (i) the retrieval accuracy of the fine-mode properties when it is not dominant; (ii) the retrieval accuracy of the coarse-mode properties at longer wavelengths and that of the fine-mode properties at shorter wavelengths; (iii) the retrieval accuracy of the coarse-mode real part of the refractive index (up to 36% reduction), but has no effect on the retrieval of the imaginary part; (iv) reduces up to 83% the bias of the sphere fraction (SF) retrieval in coarse-mode dominated regimes; and (v) the root mean square error (RMSE) of the retrieval for most of the parameters in all scenarios. In addition, the addition of more photometer channels in synergy with a three-wavelength elastic lidar reduces the RMSE for the real part (67% in the coarse mode) and the imaginary part (35% in the fine mode) of the refractive index, the single scattering albedo (38% in the fine mode), the lidar ratio (20% in the coarse mode), and the SF (43%).

The productivity of common bean cultivars is highly dependent on seed quality. In the present study, the agronomic response and seed quality with basic category of 13 commercial cultivars of common bean was determined in a carbonated fluffy brown soil. The work was carried out in Placetas municipality, Villa Clara province, between November and March of the 2020-2021 cycle. Agricultural yield and its components were determined, as well as seed quality parameters. The cultivars ‘CUFIG 48’ and ‘BAT-93’ presented significantly higher values than the rest of the cultivars in agricultural yield, number of legumes and seeds per plant. Analyses carried out at the Seed Testing and Plant Health Laboratories of Villa Clara province indicated that the seeds of all cultivars meet the quality specifications for the basic category.

•In a RIETE registry analysis of 103 818 patients with VTE, 20.3% were tested for IT, showing a substantial variance in outcomes.•A thoughtful IT testing approach should consider patients’ VTE risk factors and comorbidities.
