Abstract

Over the past decade, remote monitoring (RM) has become an increasingly popular way to improve healthcare and health outcomes. Modern cardiac implantable electronic devices (CIEDs) are capable of recording an increasing amount of data related to CIED function, arrhythmias, physiological status and hemodynamic parameters, providing in-depth and updated information on patient cardiovascular function. The extensive use of RM for patients with CIED allows for early diagnosis and rapid assessment of relevant issues, both clinical and technical, as well as replacing outpatient follow-up improving overall management without compromise safety. This approach is recommended by current guidelines for all eligible patients affected by different chronic cardiac conditions including either brady- and tachy-arrhythmias and heart failure. Beyond to clinical advantages, RM has demonstrated cost-effectiveness and is associated with elevated levels of patient satisfaction. Future perspectives include improving security, interoperability and diagnostic power as well as to engage patients with digital health technology. This review aims to update existing data concerning clinical outcomes in patients managed with RM in the wide spectrum of cardiac arrhythmias and Hear Failure (HF), disclosing also about safety, effectiveness, patient satisfaction and cost-saving.

Abstract

Background and aims: Interest in the role of atrial substrate in maintaining Atrial Fibrillation (AF) is growing. Fibrosis is the culprit in the electrical derangement of the myocytes. Many cardiovascular risk factors are known to be linked to atrial scarring; among them Uric Acid (UA) is emerging. The purpose of our study is to find whether UA is associated with Left Atrium (LA) with pathological substrate. Methods and results: 81 patients who underwent radiofrequency transcatheter ablation for nonvalvular AF at the cardiological department of the Niguarda Hospital were enrolled in an observational, cross-sectional, single-center study. UA levels were analysed before the procedure. High density electroanatomic mapping of the LA was performed and patients were divided according to the presence or not of areas of pathological substrate (bipolar voltage <0.5 mV in sinus rhythm). 19 patients showed a LA with pathological substrate. These subjects showed a significant higher prevalence of persistent phenotype of AF (84.2 vs. 25.8%, p < 0.001). UA levels were significantly higher in the group of patients with LA with pathological substrate (6.8 ± 1.9 vs 5.3 ± 1.4 mg/dL, p < 0.001) as well as the prevalence of hyperuricemia (26.5 vs. 6.5%, p = 0.021). The association between uric acid LA with pathological substrate remains significant even after correction for confounding factors (age, left ventricular dysfunction, valvular disease, arrythmia phenotype and furosemide use) and also when the ratio UA/creatinine was evaluated. Conclusions: In a population of patients who underwent AF ablation, higher UA levels were significantly associated with pathological LA substrate at electro-anatomical mapping.

14. Transformation of the crystal structure of Ni0.5TiSe2 under in situ heating.

Abstract

The crystal structure of the Ni0.5TiSe2 compound has been studied in situ in the temperature range of 25–1000 °C using synchrotron radiation X-ray diffraction. The previously known order–disorder transition in the Ni sublattice at ∼100 °C was found to be a second-order phase transition and belongs to the 3D Ising universality class. Reversible extraction of nickel selenides was observed in the temperature range of 275–975 °C. It was explained in terms of Ni extraction from Ni0.5TiSe2 due to the thermal widening of the Ni 3d/Ti 3d/Se 4p impurity band. The Ni–TiSe2 phase diagram follows the typical pattern of MxTiSe2 (M – transition metal) compounds. [ABSTRACT FROM AUTHOR]

Abstract

LiDAR (Light Detection And Ranging) laser scanners acquire 3D point clouds of real environments. The process consists in sampling the scene with laser beams rotating around an axis. By construction, the point density decreases with the distance to the scanner. This density heterogeneity is a major issue, in particular for mobile systems in the context of autonomous driving, as usually a single scan is processed simultaneously (instead of mapping applications that can integrate several scans, reducing the density heterogeneity). We propose a dartboard grid with cell size increasing radially in order to adapt the grid size to the point density. The effectiveness of this strategy is demonstrated by means of a ground detection task, a fundamental step in many workflows of analysis of 3D point clouds.

Abstract

1144 is a family of Iron Based Superconducting Compounds that attracts interest due to its regular structure, composed by an alternance of alkaline and alkaline-earth planes that intercalate Fe-As layers. This rigidity grants to 1144 compounds a stoichiometric nature, and thus a robust critical temperature not affected by chemical inhomogeneities, and a peculiar pinning landscape enhancing critical currents at high fields. Critical currents can be however further enhanced by introducing further defects by means of irradiation and chemical doping on the Fe site. In our works, as a different approach, we evaluate the effect of the alkaline and alkaline-earth aliovalent substitutions. For this scope, CaKFe4As4 polycrystalline powders have been successfully doped by partially replacing Ca with Na and K with Ba by means of a mechanochemically assisted thermal synthesis. Such substituted compounds crystallize in the P4/mmm structure typical of 1144 compounds. X-ray diffraction analysis revealed that depending on the level of substitution the reflections characteristic of the P4/mmm tend to vanish. In particular, peaks analysis has shown that doping introduces increasing disorder in selected crystalline planes. Barium and sodium, as dopant, do not act the same way, the latter being less invasive, so that 1144 structure is preserved up to 40% substitution. The contemporary replacement of potassium and calcium with barium and sodium exhibits an intermediate behavior. Analysis of X-ray diffraction profiles collected as a function of temperature down to 100 K allowed to calculate the thermal expansion coefficients for the synthesized compounds, with CTE values similar for pristine and substituted compounds close to 1 × 10−6K−1 and 3 × 10−5K−1 respectively along the a-axis and c-axis.

Abstract

The asymmetric n-type Ph-BTBT-C10 derivative 2-decyl-7-phenyl-[1]benzothieno[3,2-b][1]benzothiophene S,S,S′,S′-tetraoxide is structurally investigated in the thin film regime. After film preparation by spin coating and physical vapor deposition, a rather disordered structure is observed, with a strong change of its internal degree of order upon heating. At 95 °C, a transition into a layered structure of upright standing molecules without any in-plane order appears, and at 135 °C, crystallization takes place. This phase information is combined with surface morphological studies and charge carrier mobility measurements to describe the structure and thin film transistor applicability of this molecule.

Abstract

Atrial flutter (AFL) is a common atrial arrhythmia typically characterized by electrical activity propagating around specific anatomical regions. It is usually treated with catheter ablation. However, the identification of rotational activities is not straightforward, and requires an intense effort during the first phase of the electrophysiological (EP) study, i.e., the mapping phase, in which an anatomical 3D model is built and electrograms (EGMs) are recorded. In this study, we modeled the electrical propagation pattern of AFL (measured during mapping) using network theory (NT), a well-known field of research from the computer science domain. The main advantage of NT is the large number of available algorithms that can efficiently analyze the network. Using directed network mapping, we employed a cycle-finding algorithm to detect all cycles in the network, resembling the main propagation pattern of AFL. The method was tested on two subjects in sinus rhythm, six in an experimental model of in-silico simulations, and 10 subjects diagnosed with AFL who underwent a catheter ablation. The algorithm correctly detected the electrical propagation of both sinus rhythm cases and in-silico simulations. Regarding the AFL cases, arrhythmia mechanisms were either totally or partially identified in most of the cases (8 out of 10), i.e., cycles around the mitral valve, tricuspid valve and figure-of-eight reentries. The other two cases presented a poor mapping quality or a major complexity related to previous ablations, large areas of fibrotic tissue, etc. Directed network mapping represents an innovative tool that showed promising results in identifying AFL mechanisms in an automatic fashion. Further investigations are needed to assess the reliability of the method in different clinical scenarios.

Abstract

The phase diagram of the quasi-binary Cu-TiS2 system has been studied in situ in the temperature-concentration plane for the first time. Phase transitions between 2D (layered) and 3D (cubic) phases in CuxTiS2 (x = 0-0.5) intercalation compounds have been studied by the X-ray diffraction technique in the temperature range 20-1000 °C. It has been found that the stability of the layered phase is determined by the distribution of copper atoms between the octahedral and tetrahedral crystallographic sites. The occupation of octahedral sites dominates at low temperatures. Upon heating, tetrahedral site occupation is limited due to elastic lattice distortion and the layered phase becomes unstable and transforms to the spinel. Further heating allows the distribution of copper between octahedral and tetrahedral sites; the layered phase becomes stable again. [ABSTRACT FROM AUTHOR]

In this work, the step by step thermal dehydration process of four ZSM-5 zeolites with different Si/Al ratio (SAR), has been studied by in situ synchrotron radiation powder diffraction. ZSM-5 samples were prepared starting from the following synthesis gel: TPABr (98%, Fluka), NaOH (97%, Carlo Erba Reagenti), Al(OH)3 (98%, Fluka), precipitated silica gel (100%, Merck) and distilled water [4]. In order to achieve different Si/Al ratio in the sample, the molar composition of the gel was changed by varying the amount of Al2O3 in the starting mixture: 0.1Na2O−0.08TPABr−xAl2O3−1SiO2–20H2O, with the alumina amount (0.005 < x < 0.033) varied according to the expected Si/Al molar ratio: 15, 25, 50 and 100 [4]. The time-resolved experiment was performed at the MCX beamline at Elettra. ZSM-5 powder samples were placed in a quartz-glass capillary using a gas blower to control the sample temperature. The evolution of the structural features was monitored through 36 structure refinements in the temperature range from 25 to 800 °C by full profile Rietveld analysis. A picture of both kinetics and dynamic behavior of these materials including Temperature-dependent variation of cell parameters and crystal-structure modifications upon heating is presented. High temperature experiments confirm that all ZSM-5 maintain their crystallinity within the investigated T-range. No significant differences in the thermal stability are highlighted. On the contrary, the beta angle variation was found to be affected by the Si/Al ratio. Specifically, the higher the Silicium content the greater the beta angle. Lattice parameters vary continuously on increasing temperature, with a strong change in their evolution close to the temperature at which the monoclinic to orthorhombic phase transition takes place (T100°C). Abstract

This study reports for the first time crystal-structure data for amosite and fibrous anthophyllite. The chemical composition of the two fibre species was determined from EMPA. Crystal structures were refined using powder-diffraction data, using both laboratory sources and synchrotron radiation. Results were compared with the available literature data for the non-fibrous varieties grunerite and anthophyllite, respectively. The calculated site-occupancies for all samples are in agreement with the chemical compositions calculated from EMPA. The existing structure models of grunerite and orthorhombic anthophyllite also applies to the corresponding fibrous varieties amosite and fibrous anthophyllite, respectively. In amosite, both Fe2+ and Fe3+ atoms are found at the sites M(1), M(2) and M(3) and Fe2+ ions is the only atomic species found at site M(4). Mg is disordered over the C sites with a preference for site M(2). Minor Ca and Na have been assigned to the A site. In fibrous anthophyllite, Mg is the only atomic species found at the M1, M2 and M3 sites. Fe2+, Mg (and minor Mn) have been assigned to the M4 site, whereas minor Ca has been assigned to the A site. In both structures, the environment at the M(4) site in amosite and M4 site in fibrous anthophyllite is highly distorted. This work can be considered a basis for studies aimed at understanding the potential toxicity/pathogenicity of these mineral fibres

Abstract

Cardiac laminopathies, associated with mutations in the LMNA gene, encompass a wide spectrum of clinical manifestations, involving electrical and mechanical alterations of cardiomyocytes. Thus, dilated cardiomyopathy, bradyarrhythmias and atrial or ventricular tachyarrhythmias may occur in a number of combined phenotypes. Nowadays, some attempt has been made to identify clinical predictors for the most life-threatening complications of LMNA-associated heart disease, i.e. sudden cardiac death and end-stage heart failure. The goal of this manuscript is to combine the most recent evidences in an updated review to show the state-of-the-art of such a complex disease group. This is supposed to be the starting point to collect more data and design new ad hoc studies to identify clinically useful predictors to stratify risk in mutation carriers, including probands and their asymptomatic relatives.
