Medial vascular calcification (mVC) is closely related to cardiovascular disease, especially in patients suffering from chronic kidney disease (CKD). Even after successful kidney transplantation, cardiovascular mortality remains increased. There is evidence that immunosuppressive drugs might influence pathophysiological mechanisms in the vessel wall. Previously, we have shown in vitro that mVC is induced in vascular smooth muscle cells (VSMCs) upon treatment with azathioprine (AZA). This effect was confirmed in the current study in an in vivo rat model treated with AZA for 24 weeks. The calcium content increased in the aortic tissue upon AZA treatment. The pathophysiologic mechanisms involve AZA catabolism to 6-thiouracil via xanthine oxidase (XO) with subsequent induction of oxidative stress. Proinflammatory cytokines, such as interleukin (IL)-1ß and IL-6, increase upon AZA treatment, both systemically and in the aortic tissue. Further, VSMCs show an increased expression of core-binding factor α-1, alkaline phosphatase and osteopontin. As the AZA effect could be decreased in NLRP3−/− aortic rings in an ex vivo experiment, the signaling pathway might be, at least in part, dependent on the NLRP3 inflammasome. Although human studies are necessary to confirm the harmful effects of AZA on vascular stiffening, these results provide further evidence of induction of VSMC calcification under AZA treatment and its effects on vessel structure.

Calcification of the vessel wall as one structural pathology of aged vessels is associated with high cardiovascular mortality of elderly patients. Aging is linked to chronic sterile inflammation and high burden of reactive oxygen species (ROS), leading to activation of pattern recognition receptors (PRRs) such as Nlrp3 in vascular cells. The current study investigates the role of PRR activation in the calcification of vascular smooth muscle cells (VSMCs). Therefore, in vitro cell culture of primary rat VSMCs and ex vivo aortic stimulations were used to analyze osteogenic, senescence and inflammatory markers via real-time PCR, in situ RNA hybridization, Western Blot, photometric assays and histological staining. Induction of ROS and DNA-damage by doxorubicin induces a shift of VSMC phenotype toward the expression of osteogenic, senescence and inflammatory proteins. Induction of calcification is dependent on Nlrp3 activity. Il-1 beta as a downstream target of Nlrp3 induces the synthetic, pro-calcifying VSMC phenotype. Inhibition of PRR with subsequent reduction of chronic inflammation might be an interesting target for reduction of calcification of VSMCs, with subsequent reduction of cardiovascular mortality of patients suffering from vessel stiffness.

The acute phase protein group IIA secretory phospholipase A2 (sPLA2-IIA) has intrinsic proatherosclerotic properties. The present prospective cohort study investigated whether plasma sPLA2-IIA associates with graft failure, cardiovascular, and all-cause mortality in renal transplant recipients (RTRs), patients with accelerated atherosclerosis formation both systemically and within the graft. In 511 RTRs from a single academic center with stable graft function &gt, 1 year, baseline plasma sPLA2-IIA was determined by ELISA. Primary end points were death-censored graft failure and mortality (median follow-up, 7.0 years). Baseline sPLA2-IIA was higher in RTRs than in healthy controls (median 384 ng/dL (range 86&ndash, 6951) vs. 185 ng/dL (range 104&ndash, 271), p &lt, 0.001). Kaplan&ndash, Meier analysis demonstrated increased risk for graft failure (p = 0.002), as well as cardiovascular (p &lt, 0.001) and all-cause mortality (p &lt, 0.001), with increasing sPLA2-IIA quartiles. Cox regression showed strong associations of sPLA2-IIA with increased risks of graft failure (hazard ratio (HR) = 1.42 (1.11&ndash, 1.83), p = 0.006), as well as cardiovascular (HR = 1.48 (1.18&minus, 1.85), p = 0.001) and all-cause mortality (HR = 1.39 (1.17&minus, 1.64), p &lt, 0.001), dependent on parameters of kidney function. Renal function during follow-up declined faster in RTRs with higher baseline sPLA2-IIA levels. In RTRs, sPLA2-IIA is a significant predictive biomarker for chronic graft failure, as well as overall and cardiovascular disease mortality dependent on kidney function. This dependency is conceivably explained by sPLA2-IIA impacting negatively on kidney function.

In 2003 the ESH/ESC recommended the arterial stiffness measurement for arterial hypertension management in their guidelines for the first time. In addition to the conventional blood pressure measurement, the clinicians are advised to measure following parameters: pulse wave velocity (PWV), augmentation pressure (AugP) and central blood pressure (zBD). A change in shape of the blood pressure wave can indicate early atherosclerotic changes of the vascular walls of the elastic arteries. The evaluation of noninvasively recorded pulse wave shapes in real-time with a usable, valid, automated and independent of age operating system is at the moment an unsolved problem. In this scientific work, a new process for arterial stiffness measurement was methodically developed in three main parts: In the first part, the pulse wave recording was realised with a blood pressure cuff at the upper arm. A data capturing system was developed and used for data collection in a heart catheter laboratory. Simultaneous pulse wave recordings noninvasively from the upper arm and invasively from the aorta ascendens were taken from 92 persons undergoing cardiac catheterisation. This data collection of syncing pulse waves established the basis for the second part of the process development. With a systematic selection of 29 datasets a transfer function (ÜF) was determined. With this ÜF, new noninvasively recorded pulse waves from the upper arm can be converted into the individual pulse wave shapes of the respective central aortic wave (calculation of zBD). Subsequently, the process was advanced by an implementation of a Windkessel model (WKM) for pulse wave separation (WSA). The model is dimensioned for the individual examined patient with every measurement. The WKM evaluates the characteristic shape of the central pulse wave for every new patient und separates it into a forward and a reflected backward travelling wave from the vascular bed. By means of those pulse wave analysis (PWA) algorithms, the other parameters of arterial stiffness are determined. In the last part, the procedure was examined for reproducibility and repeatability with 20 different test datasets. Besides, the results of the developed algorithms were retrospectively compared with the original results from the heart catheter laboratory. Furthermore, the new measurement technique was compared with gold-standard measurements with SphygmoCor (Co. Atcor Medical). The retrospective Bland-Altman-analysis showed a good agreement of the new arterial stiffness determining algorithms with the standard procedures. The correlation with the invasively measured blood pressure was r = 0.89 (P = 99 %). The mean difference for PWV-determination in comparison with SphygmoCor was 0.09 m/s (SD 1.6 m/s). This work presents the results of the development of a new blood pressure device advanced with a function for arterial stiffness measurement. The developed algorithms proceed in real-time and were implemented in a compact measurement system. After successful market approval, the system will be usable for arterial stiffness screening in clinical practice. Im Jahre 2003 nahm die ESH/ESC in ihre Leitlinien erstmals Empfehlungen zur Messung der Parameter der arteriellen Gefäßsteifigkeit auf. Zusätzlich zum konventionellen Blutdruck erfasst werden sollen: Pulswellengeschwindigkeit (PWV), Augmentationsdruck (AugP) und zentraler Blutdruck (zBD). Eine frühzeitige Veränderung der Pulswellenform lässt Rückschlüsse auf beginnende atherosklerotische Prozesse in den Gefäßen zu. Die benutzerfreundliche, valide, automatisierte und altersunabhängige Bewertung nichtinvasiv aufgezeichneter Pulswellen in Echtzeit stellt ein bisher noch weitgehend ungelöstes technisches Problem dar. In dieser Arbeit wurde in drei Schritten ein neues Verfahren zur Bestimmung der Parameter der arteriellen Gefäßsteifigkeit entwickelt: Im ersten Teil wurde die Pulswellenaufzeichnung mit einer Druckmanschette am Oberarm realisiert. Mit dem aufgebauten Datenerfassungsgerät fand eine Erhebung im Herz-Katheter-Labor (HKL) an 92 Personen statt, bei der zeitgleich nichtinvasiv Pulswellen am Oberarm und invasiv in der Aorta ascendens aufgezeichnet wurden. Diese einander zugeordneten Pulswellen waren die Grundlage für den zweiten Teil der Verfahrensentwicklung. Es erfolgte die Berechnung einer Übertragungsfunktion (ÜF) anhand von 29 systematisch ausgewählten Datensätzen. Mithilfe der entwickelten ÜF kann die individuelle zentrale Pulswelle auf Basis neu, mit Blutdruckmanschette aufgezeichneter Oberarmpulswellen berechnet und damit der zBD bestimmt werden. Anschließend wurde das Verfahren um die Anwendung eines Windkesselmodells (WKM) zur Pulswellenseparation (WSA) erweitert, welches patientenspezifisch für jede Messung dimensioniert wird. Das WKM dient zur Bewertung der charakteristischen zentralen Pulswellenform des Patienten, welche in eine hinlaufende Welle und eine im Gefäßbett reflektierte Welle zerlegt werden kann. Aus der entwickelten Pulswellenanalyse (PWA) lassen sich die o. g. Parameter der Gefäßsteifigkeit ableiten. Im letzten Teil der Arbeit wurde das Verfahren auf Reproduzierbarkeit und Wiederholbarkeit an 20 verschiedenen Probandendatensätzen getestet. Zudem erfolgten ein retrospektiver Vergleich der Ergebnisse des Messverfahrens mit den HKL-Testdaten, eine Verifizierung an 30 Probanden mit dem bisherigen Standardverfahren SphygmoCor (Fa. Atcor Medical) sowie eine praktische Überprüfung der gestellten Nutzungsanforderungen. Die retrospektive Bland-Altman-Analyse zeigte eine gute Übereinstimmung der durch das neue Verfahren bestimmten Parameterberechnungen mit den Standardverfahren. Die Korrelation zum invasiv gemessenen zentralen Blutdruck betrug r = 0,89 (P = 99 %). Die mittlere Abweichung für die Bestimmung der PWV im Vergleich zu SphygmoCor betrug 0,09 m/s (SD 1,6 m/s). Im Ergebnis der vorliegenden Arbeit entstand ein Messverfahren, welches die Parameter der arteriellen Gefäßsteifigkeit in Echtzeit unmittelbar im Anschluss an die Blutdruckmessung anzeigt. Das Verfahren ist in einem kompakten Messsystem implementiert und kann zukünftig nach erfolgreicher Zulassung als Screening-Gerät in der klinischen Anwendung genutzt werden.

Für die Ätiopathogenese der Atherosklerose spielen eine Vielzahl von Mediatoren eine Rolle. Dabei werden durch das Endothel sowohl protektive als auch schädliche Mediatoren sezerniert. High Density Lipoproteine (HDL) stellen einen bedeutenden protektiven Marker für das kardiovaskuläre Risiko dar, u.a. durch die Aktivierung der endothelialen NO-Synthase (eNOS). HDL besteht zu 50 % aus Proteinen und zu 50 % aus Lipiden. Welche Komponenten des HDL für die eNOS Aktivierung verantwortlich sind, ist nicht bekannt gewesen. Im ersten Abschnitt dieser Promotionsarbeit konnte erfolgreich gezeigt werden, dass die Lysophospholipide, Sphingosin-1-Phosphat (S1P) und Sphinsosylphosphorylcholin (SPC), die strukturelle Bestandteile der Lipidfraktion von HDL darstellen, für einen Teil der HDL induzierten eNOS Aktivierung durch Stimulation des S1P3-Rezeptors verantwortlich sind. Diese eNOS Aktivierung wird durch den intrazellulären Einstrom von Calcium und durch die Aktivierung der Akt-Kinase induziert. Im zweiten Abschnitt dieser Promotionsarbeit konnte nachgewiesen werden, dass das oral verfügbare Lysophospholipid-basierte Medikament, FTY720, das ein strukturelles Analogon des S1P ist, den HDL induzierten Signaltransduktionsweg der eNOS Aktivierung in gleicher Weise induziert. Im dritten Abschnitt dieser Promotionsarbeit konnte ein neuartiges endothelabhängig sezerniertes gemischtes Dinukleosidpolyphosphat, Uridin-Adenosin-Tetraphosphat (Up4A), identifiziert werden. Up4A ist ein Agonist an den P2X- und P2Y-Purinrezeptoren. Up4A induziert bei Applikation in eine isoliert perfundierte Rattenniere hauptsächlich über die Aktivierung des P2X1-Rezeptors und des P2Y2/P2Y4-Rezeptors eine starke Vasokonstriktion im renalen Perfusionsgebiet mit einhergehender Erhöhung des mittleren renalen Perfusionsdrucks. Die direkte Infusion von Up4A in vivo in eine WKY-Ratte führt zu einer signifikanten Erhöhung des mittleren arteriellen Blutdrucks. In the pathogenesis of atherosclerosis many mediators are included. Therefore the endothelium plays a crucial part by secreting protective but also deleterious factors. High density lipoproteins are an established protective factor in the risk profile of cardiovascular events especially by activating the endothelial NO synthase (eNOS). HDL is composed of 50 % proteins and 50 % lipids. Which component of HDL is responsible for the eNOS activation was not known. In the first part of this dissertation it could be shown, that the lysophospholipids, sphingosine-1-phosphate (S1P) and sphingosylphosphorylcholine (SPC), which are structural compounds of the lipid fraction of HDL, are responsible for a significant part of the HDL induced eNOS activation by stimulating the specific S1P3 receptor. In the signal transduction mechanism the activation of Akt kinase and an influx of calcium is involved. In the second part of this dissertation it could be shown, that the orally active lysophospholipide based drug FTY720, which is a structural analogue of S1P, is able to induce the same signal transduction mechanism activated by HDL including the stimulation of the S1P3 receptor. In the last part of this dissertation a new endothelium dependent vasoconstrictor, the dinucleoside polyphosphate uridine-adenosine-tetraphosphate (Up4A), could be for the first time identified. Up4A is a potent agonist of the P2X- and P2Y-purinoceptors. Via activating the P2X1 receptor and the P2Y2/P2Y4 receptor Up4A induce a strong vasoconstriction in the renal perfusion system in the model of the isolated perfused rat kidney with an adjacent increase of the mean perfusion pressure. By injection of Up4A in vivo in a Wistar Kyoto rat the mean arterial pressure also increase significantly.
