In survivors, different degrees of functional complaints need to be restored or prevented (eg, post-thrombotic syndrome, pulmonary hypertension). Therefore, rehabilitation after venous thromboembolism is recommended in Germany. However, a structured rehabilitation program has not been defined for this indication. Here, we present the experience of a single rehabilitation center. Methods: Data from consecutive pulmonary embolism (PE) patients who were referred for a 3-week inpatient rehabilitation program from 2006 to 2014 were retrospectively evaluated. Results: In all, 422 patients were identified. The mean age was 63.9±13.5 years, the mean body mass index (BMI) was 30.6±6.2 kg/m2, and 51.9% were female. Deep vein thrombosis according to PE was known for 55.5% of all patients. We applied a wide range of therapeutic interventions such as bicycle training with monitored heart rate in 86.7%, respiratory training in 82.5%, aquatic therapy/swimming in 40.1%, and medical training therapy in 14.9% of all patients. Adverse events (AEs) occurred in 57 patients during the 3-week rehabilitation period. The most common AEs were cold (n=6), diarrhea (n=5), and infection of the upper or lower respiratory tract that was treated with antibiotics (n=5). However, three patients under anticoagulation therapy suffered from bleeding, which was clinically relevant in one. Four patients (0.9%) had to be transferred to a primary care hospital for non-PE-associated reasons (acute coronary syndrome, pharyngeal abscess, and acute abdominal problems). No influence of any of the physical activity interventions on the incidence of any AE was found. Conclusion: Since PE is a life-threatening disease, it seems reasonable to recommend rehabilitation at least in PE patients with an intermediate or high risk. It is shown for the first time in this study that a standard rehabilitation program after PE is safe. However, efficacy and safety in the long term need to be studied prospectively. Abstract

The increasing number of elderly individuals in the population and the simultaneous increase of the intensive care demand emphasizes the relevance of an efficient bed capacity analysis. Particularly, cardiovascular diseases represent a frequently occurring disease in the population group over 65 years of age. The objective of the following paper is the analysis of the retrospective and prospective intensive care demand by patients over 65 years with 6 selected (cardiovascular) codes of the International Statistical Classification of Diseases and Related Health Problems (ICD-10).For the retrospective analysis, data from 2015-2017 were analyzed applying descriptive and bivariate methods. The analysis of the intensive care bed demand was based on the queuing theory.The monthly capacity utilization rates were constantly higher than the target capacity utilization rate of a maximum of 80% and in some cases even higher than 100%. In particular, the demand of patients with I50.14 was very high throughout the entire hospital. The bed demand analysis shows an increase from 9 needed beds in 2017 to 11 beds by 2030 for the 6 diagnosis groups. Regarding the 5 diagnosis groups without I50.14, only approximately half of the required beds were needed, retrospectively and in future.The effect of demographic change on the intensive care demand already exists, and a continuing, prospective increase of the demand is expected. The results underline the need of effective and demand-oriented intensive care capacity planning. However, prior to expanding bed capacities, the analysis of admission criteria of intensive care unit patients is necessary to reserve capacities primarily for patients with real intensive care needs.HINTERGRUND: Der Anstieg der älteren Bevölkerungsgruppe und der damit einhergehende Zuwachs des intensivmedizinischen Bedarfs betont die Notwendigkeit einer effizienten Bettenkapazitätsanalyse. Insbesondere Herz-Kreislauf-Erkrankungen stellen ein häufig auftretendes Erkrankungsbild bei über 65-Jährigen dar. Ziel dieser Arbeit war somit die Analyse des retrospektiven und zukünftigen Intensivbedarfs von älteren Patienten über 65 Jahre mit 6 ausgewählten (kardiovaskulären) Codes der Internationalen statistischen Klassifikation der Krankheiten und verwandter Gesundheitsprobleme (ICD-10) am Beispiel einer Einrichtung der Maximalversorgung in einer ländlichen Region.Für die retrospektive Analyse wurden Daten für den Zeitraum 2015–2017 deskriptiv und bivariat ausgewertet. Die Analyse des Intensivbettenbedarfs erfolgte anhand der Warteschlangentheorie.In dem betrachteten Zeitraum lagen die monatlichen Auslastungsraten kontinuierlich über der idealen Auslastungsrate von 80 % und zum Teil sogar über 100 %. Insbesondere die Nachfrage von Patienten mit I50.14 war im gesamten Krankenhaus sehr hoch. Die Bettenbedarfsanalyse zeigt einen Anstieg von 9 benötigten Betten im Jahr 2017 auf 11 Betten bis zum Jahr 2030 für die 6 Diagnosegruppen. Ohne Einschluss der Diagnosegruppe I50.14 wird sowohl retrospektiv als auch zukünftig etwa die Hälfte der Betten, die bei Einschluss aller 6 Diagnosen benötigt wurden, nachgefragt.Der Effekt des demographischen Wandels auf den Intensivbettenbedarf ist bereits heute sichtbar. Zudem zeigen die Analysen, dass ein weiterer Anstieg des Bedarfs in Zukunft zu erwarten ist. Die Ergebnisse bestätigen die Notwendigkeit einer an den Bedarf angepassten Intensivkapazitätsplanung. Vor Erweiterung der Bettenkapazitäten wäre jedoch die Analyse von Kriterien, die eine intensivmedizinische Behandlung bedingen, notwendig, um primär Kapazitäten für Patienten mit einem realen Intensivbedarf vorzuhalten.

Abstract

Canonical quantization of gravitational systems is obstructed by the problem of time. Due to diffeomorphism symmetry the Hamiltonian vanishes: dynamics with respect to a background time parameter appears "frozen." Two strategies towards the quantization of such systems are the identification of a clock degree of freedom before quantization (deparametrization), and quantization on a kinematical Hilbert space which is subject to constraints (Dirac quantization). The usual canonical quantization in quantum field theory is analogous to deparametrization. Here we introduce a frozen formalism and Dirac quantization for a complex Klein-Gordon scalar field, and show that the resulting theory is equivalent to usual canonical quantization. We then apply the formalism to the group field theory formalism for quantum gravity, for which both deparametrization and a "timeless" quantization have been proposed in past work. We show how a frozen formalism for group field theory links between these two existing approaches, and illustrate in particular the construction of physical observables. We derive effective cosmological dynamics for group field theory in the new formalism and compare these to previous work. OBJECTIVES This study sought to determine the clinical benefit of adding alirocumab to statins in ACS patients with prior CABG in a pre-specified analysis of ODYSSEY OUTCOMES (Evaluation of Cardiovascular Outcomes After an Acute Coronary Syndrome During Treatment With Alirocumab). METHODS Patients (n = 18,924) 1 to 12 months post-ACS with elevated atherogenic lipoprotein levels despite high-intensity statin therapy were randomized to alirocumab or placebo subcutaneously every 2 weeks. Median follow-up was 2.8 years. the primary composite endpoint of major adverse cardiovascular events (MACE) comprised coronary heart disease death, nonfatal myocardial infarction, ischemic stroke, or unstable angina requiring hospitalization. All-cause death was a secondary endpoint. Patients were categorized by CABG status: no CABG (n = 16,896); index CABG after qualifying ACS, but before randomization (n = 1,025); or CABG before the qualifying ACS (n = 1,003). RESULTS in each CABG category, hazard ratios (95% confidence intervals) for MACE (no CABG 0.86 [0.78 to 0.95], index CABG 0.85 [0.54 to 1.35], prior CABG 0.77 [0.61 to 0.98]) and death (0.88 [ 0.75 to 1.03], 0.85 [0.46 to 1.59], 0.67 [0.44 to 1.01], respectively) were consistent with the overall trial results (0.85 [ 0.78 to 0.93] and 0.85 [0.73 to 0.98], respectively). Absolute risk reductions (95% confidence intervals) differed across CABG categories for MACE (no CABG 1.3% [0.5% to 2.2%], index CABG 0.9% [-2.3% to 4.0%], prior CABG 6.4% [0.9% to 12.0%]) and for death (0.4% [-0.1% to 1.0%], 0.5% [-1.9% to 2.9%], and 3.6% [0.0% to 7.2%]). CONCLUSIONS Among patients with recent ACS and elevated atherogenic lipoproteins despite intensive statin therapy, alirocumab was associated with large absolute reductions in MACE and death in those with CABG preceding the ACS event. We assessed the effects of alirocumab on death after index acute coronary syndrome. Methods: ODYSSEY OUTCOMES (Evaluation of Cardiovascular Outcomes After an Acute Coronary Syndrome During Treatment With Alirocumab) was a double-blind, randomized comparison of alirocumab or placebo in 18 924 patients who had an ACS 1 to 12 months previously and elevated atherogenic lipoproteins despite intensive statin therapy. Alirocumab dose was blindly titrated to target achieved low-density lipoprotein cholesterol (LDL-C) between 25 and 50 mg/dL. We examined the effects of treatment on all-cause death and its components, cardiovascular and noncardiovascular death, with log-rank testing. Joint semiparametric models tested associations between nonfatal cardiovascular events and cardiovascular or noncardiovascular death. Results: Median follow-up was 2.8 years. Death occurred in 334 (3.5%) and 392 (4.1%) patients, respectively, in the alirocumab and placebo groups (hazard ratio [HR], 0.85; 95% CI, 0.73 to 0.98; P=0.03, nominal P value). This resulted from nonsignificantly fewer cardiovascular (240 [2.5%] vs 271 [2.9%]; HR, 0.88; 95% CI, 0.74 to 1.05; P=0.15) and noncardiovascular (94 [1.0%] vs 121 [1.3%]; HR, 0.77; 95% CI, 0.59 to 1.01; P=0.06) deaths with alirocumab. in a prespecified analysis of 8242 patients eligible for >= 3 years follow-up, alirocumab reduced death (HR, 0.78; 95% CI, 0.65 to 0.94; P=0.01). Patients with nonfatal cardiovascular events were at increased risk for cardiovascular and noncardiovascular deaths (P= 100 mg/dL (2.59 mmol/L) had a greater absolute risk of death and a larger mortality benefit from alirocumab (HR, 0.71; 95% CI, 0.56 to 0.90; P-interaction=0.007). in the alirocumab group, all-cause death declined with achieved LDL-C at 4 months of treatment, to a level of approximately 30 mg/dL (adjusted P=0.017 for linear trend). Alirocumab reduced first occurrence of the primary composite end point-coronary heart disease death, nonfatal myocardial infarction, fatal or nonfatal ischemic stroke, or hospitalization for unstable angina-as well as total nonfatal cardiovascular events and all-cause deaths. the present analysis determined whether alirocumab reduced total (first and subsequent) hospitalizations and death and increased days alive and out of hospital (DAOH) and percent DAOH in ODYSSEY OUTCOMES. Methods and Results: in prespecified analyses, hazard functions for total hospitalizations and death were jointly estimated by a semiparametric model, while in post hoc analyses, DAOH and percent DAOH were compared between treatment groups with Poisson regression and one-inflated beta regression, respectively. With 16629 total hospitalizations and 726 deaths, 331 fewer hospitalizations, and 58 fewer deaths were observed with alirocumab compared with placebo, translating to 15.6 total hospitalizations or deaths avoided with alirocumab per 1000 patient-years of assigned treatment. Alirocumab reduced total hospitalizations (hazard ratio, 0.96 [95% CI, 0.92-1.00]; P=0.04) and increased DAOH relative to placebo (rate ratio, 1.003 [95% CI, 1.000-1.007]; P=0.05), primarily through a reduction in days dead (rate ratio, 0.847 [95% CI, 0.728-0.986]; P=0.03). Patients randomized to alirocumab were also more likely to survive to the end of the study without hospitalization (odds ratio, 1.06 [95% CI, 1.00-1.13]; P=0.03). Conclusions: Alirocumab reduced total hospitalizations with corresponding small increases in DAOH and percent DAOH. These outcomes provide alternative patient-centered metrics to capture the totality of alirocumab clinical efficacy after acute coronary syndrome. Abstract

Recent results have shown how quantum cosmology models can be derived from the effective dynamics of condensate states in group field theory (GFT), where 'cosmology is the hydrodynamics of quantum gravity': the classical Friedmann dynamics for homogeneous, isotropic universes, as well as loop quantum cosmology (LQC) corrections to general relativity have been shown to emerge from fundamental quantum gravity. We take one further step towards strengthening the link with LQC and show, in a class of GFT models for gravity coupled to a free massless scalar field and for generic initial conditions, that GFT condensates dynamically reach a low spin phase of many quanta of geometry, in which all but an exponentially small number of quanta are characterised by a single spin $j_0$ (i.e. by a constant volume per quantum). As the low spin regime is reached, GFT condensates expand to exponentially large volumes, and the dynamics of the total volume follows precisely the classical Friedmann equations. This behaviour follows from a single requirement on the couplings in the GFT model under study. We present one particular simple case in which the dominant spin is the lowest one: $j_0=0$ or, if this is excluded, $j_0=1/2$. The type of quantum state usually assumed in the derivation of LQC is hence derived from the quantum dynamics of GFT. Abstract

The beneficial effect of exercise training and exercise-based cardiac rehabilitation on symptom-free exercise capacity,cardiovascular and skeletal muscle function, quality of life, general healthy lifestyle, and reduction of depressive symptoms and psychosocial stress is nowadays well recognized. However, it remains largely obscure, which characteristics of physical activity (PA) and exercise training--frequency, intensity, time (duration), type (mode), and volume (dose: intensity x duration) of exercise--are the most effective. The present paper, therefore, will deal with these exercise characteristics in the management of individuals with cardiovascular disease, i.e. coronary artery disease and chronic heart failure patients, but also in patients with congenital or valvular heart disease. Based on the current literature, and if sufficient evidence is available, recommendations from the European Association on Cardiovascular Prevention and Rehabilitation are formulated regarding frequency, intensity, time and type of PA, and safety aspects during exercise inpatients with cardiovascular disease. This paper is the third in a series of three papers, all devoted to the same theme: the importance of the exercise characteristics in the management of cardiovascular health. Part I is directed to the general population and Part II to individuals with cardiovascular risk factors. In general, PA recommendations and exercise training programmes for patients with coronary artery disease or chronic heart failure need to be tailored to the individual's exercise capacity and risk profile, with the aim to reach and maintain the individually highest fitness level possible and to perform endurance exercise training 30–60 min daily (3–5 days per week) in combination with resistance training 2–3 times a week. Because of the frequently reported dose–response relationship between training effect and exercise intensity, one should seek sufficiently high training intensities, although more scientific evidence on effect sizes and safety is warranted. Abstract

One proposal for deriving effective cosmological models from theories of quantum gravity is to view the former as a mean-field (hydrodynamic) description of the latter, which describes a universe formed by a 'condensate' of quanta of geometry. This idea has been successfully applied within the setting of group field theory (GFT), a quantum field theory of 'atoms of space' which can form such a condensate. We further clarify the interpretation of this mean-field approximation, and show how it can be used to obtain a semiclassical description of the GFT, in which the mean field encodes a classical statistical distribution of geometric data. In this sense, GFT condensates are quantum homogeneous geometries that also contain statistical information about cosmological inhomogeneities. We show in the isotropic case how this information can be extracted from geometric GFT observables and mapped to quantities of observational interest. Basic uncertainty relations of (non-commutative) Fourier transforms imply that this statistical description can only be compatible with the observed near-homogeneity of the Universe if the typical length scale associated to the distribution is much larger than the fundamental 'Planck' scale. Abstract

In the context of group field theory condensate cosmology, we clarify the extraction of cosmological variables from the microscopic quantum gravity degrees of freedom. We show that an important implication of the second quantized formalism is the dependence of cosmological variables and equations on the quantum gravitational atomic number N (number of spin network vertices/elementary simplices). We clarify the relation of the effective cosmological equations with loop quantum cosmology, understood as an effective (hydrodynamic-like) approximation of a more fundamental quantum gravity theory. By doing so, we provide a fundamental basis to the idea of lattice refinement, showing the dependence of the effective cosmological connection on N, and hence indirectly on the scale factor. Abstract

Importance Secretory phospholipase A 2 (sPLA 2 ) generates bioactive phospholipid products implicated in atherosclerosis. The sPLA 2 inhibitor varespladib has favorable effects on lipid and inflammatory markers; however, its effect on cardiovascular outcomes is unknown. Objective To determine the effects of sPLA 2 inhibition with varespladib on cardiovascular outcomes. Design, Setting, and Participants A double-blind, randomized, multicenter trial at 362 academic and community hospitals in Europe, Australia, New Zealand, India, and North America of 5145 patients randomized within 96 hours of presentation of an acute coronary syndrome (ACS) to either varespladib (n = 2572) or placebo (n = 2573) with enrollment between June 1, 2010, and March 7, 2012 (study termination on March 9, 2012). Interventions Participants were randomized to receive varespladib (500 mg) or placebo daily for 16 weeks, in addition to atorvastatin and other established therapies. Main Outcomes and Measures The primary efficacy measure was a composite of cardiovascular mortality, nonfatal myocardial infarction (MI), nonfatal stroke, or unstable angina with evidence of ischemia requiring hospitalization at 16 weeks. Six-month survival status was also evaluated. Results At a prespecified interim analysis, including 212 primary end point events, the independent data and safety monitoring board recommended termination of the trial for futility and possible harm. The primary end point occurred in 136 patients (6.1%) treated with varespladib compared with 109 patients (5.1%) treated with placebo (hazard ratio [HR], 1.25; 95% CI, 0.97-1.61; log-rank P = .08). Varespladib was associated with a greater risk of MI (78 [3.4%] vs 47 [2.2%]; HR, 1.66; 95% CI, 1.16-2.39; log-rank P = .005). The composite secondary end point of cardiovascular mortality, MI, and stroke was observed in 107 patients (4.6%) in the varespladib group and 79 patients (3.8%) in the placebo group (HR, 1.36; 95% CI, 1.02-1.82; P = .04). Conclusions and Relevance In patients with recent ACS, varespladib did not reduce the risk of recurrent cardiovascular events and significantly increased the risk of MI. The sPLA 2 inhibition with varespladib may be harmful and is not a useful strategy to reduce adverse cardiovascular outcomes after ACS. Trial Registration clinicaltrials.gov Identifier:NCT01130246
