Surface hardening procedures such as mechanical shot peening and deep cold rolling introduce extensive surface cold work and compressive residual stress. While both methods are designed to increase material fatigue strength, single crystal nickel-based superalloys used for turbine blade applications are routinely exposed to temperatures above 900 °C. The thermal stability of the cold work and the residual stress induced in the sub-surface of the material is therefore a key design requirement. In this study, mechanically shot peened and deep cold rolled samples of the superalloy CMSX-4 were annealed in vacuum at either 900 °C or 1100 °C for between 100 and 1000 h. Using scanning electron microscopy, electron backscattered diffraction and centre hole drilling, the cold work, the residual stress and the material response to annealing heat treatments were all studied. Significant recrystallisation of the cold worked material was observed, typically after 500 h at 900 °C and after 5 h at 1100 °C. Undesirable topologically close-packed precipitates began forming on slip bands generated by both surface hardening techniques after only 10 h at 900 °C, whereas at 1100 °C their formation was hindered by the recovery of slip bands. Rafting of the γ/γ' microstructure took place at 1100 °C, with the extent of the rafting found to be limited by the depth of compressive residual stress. Samples treated by deep cold rolling generally displayed smaller recrystallisation depths, lower rates of topologically close-packed precipitate formation and deeper rafting than those treated by mechanical shot peening.

BACKGROUND: In a randomized, controlled trial that compared liraglutide, a glucagon-like peptide 1 analogue, with placebo in patients with type 2 diabetes and high cardiovascular risk who were receiving usual care, we found that liraglutide resulted in lower risks of the primary end point (nonfatal myocardial infarction, nonfatal stroke, or death from cardiovascular causes) and death. However, the long-term effects of liraglutide on renal outcomes in patients with type 2 diabetes are unknown. METHODS: We report the prespecified secondary renal outcomes of that randomized, controlled trial in which patients were assigned to receive liraglutide or placebo. The secondary renal outcome was a composite of new-onset persistent macroalbuminuria, persistent doubling of the serum creatinine level, end-stage renal disease, or death due to renal disease. The risk of renal outcomes was determined with the use of time-to-event analyses with an intention-to-treat approach. Changes in the estimated glomerular filtration rate and albuminuria were also analyzed. RESULTS: A total of 9340 patients underwent randomization, and the median follow-up of the patients was 3.84 years. The renal outcome occurred in fewer participants in the liraglutide group than in the placebo group (268 of 4668 patients vs. 337 of 4672; hazard ratio, 0.78; 95% confidence interval [CI], 0.67 to 0.92; P=0.003). This result was driven primarily by the new onset of persistent macroalbuminuria, which occurred in fewer participants in the liraglutide group than in the placebo group (161 vs. 215 patients; hazard ratio, 0.74; 95% CI, 0.60 to 0.91; P=0.004). The rates of renal adverse events were similar in the liraglutide group and the placebo group (15.1 events and 16.5 events per 1000 patient-years), including the rate of acute kidney injury (7.1 and 6.2 events per 1000 patient-years, respectively). CONCLUSIONS: This prespecified secondary analysis shows that, when added to usual care, liraglutide resulted in lower rates of the development and progression of diabetic kidney disease than placebo. The potential importance of the obstructive sleep apnoea syndrome (OSA) has been recognized only in the last few years. Epidemiological studies suggest that symptomatic OSA occurs in 1-2% of middle-aged men and in approximately half that number of women. The relation of OSA to vascular disease is uncertain and the main indication for treatment is the relief of disabling sleepiness. Two recent evidence based analyses have produced diametrically opposed conclusions on the efficacy of treatment with nasal continuous positive airway pressure (CPAP). However, recent controlled studies confirm the overwhelming clinical experience of benefit. Facilities for the investigation and treatment of patients with OSA in the UK are subject to severe financial constraints and the availability of CPAP treatment lags markedly behind that in other countries for which data are available.

To enable efficient control of a virus disease in an agricultural crop, it is desirable to understand the mechanisms that underlie its spread. Observations from small-scale field experiments, in which disease incidence is intensively mapped in space and at several time points, may shed some light on these mechanisms, and a range of analytic tools is available for their analysis and interpretation. For example, spatiotemporal distance class analysis (12,22) has been used to study the spatial distribution of newly detected infections in a population in relation to previous infections. Techniques based on spatiotemporal autocorrelation (1) such as STARIMA (spatiotemporal autoregressive integrated moving average) methods (13,23,24) have also been applied to model changes in disease incidence over time. A related approach is the use of ordinal regression (2,19,29), in which the probability of a change in the disease status of an individual at an observation time is modeled as a function of its status and the status of individuals in some neighborhood at a previous time. In common with STARIMA methods, this latter approach involves the fitting of a discretetime Markov model to the evolution of the epidemic. For discrete-time models to be applicable, frequent observation of a process may be required. If, however, the interval between observations is long in comparison with the time scales over which the disease spreads, then a continuous-time stochastic model may be more appropriate. Although, continuous-time stochastic models have been studied in the literature for many years (5,6,20,21), they have seen comparatively little use in the analysis of experimental data, with most emphasis placed on understanding their theoretical behavior and the macroscopic properties of the patterns they produce. However, recent developments in statistical methodology and computing power have led to a greater role for these models as analytic tools. Recently, Gibson and Austin (11) fitted continuous-time, spatiotemporal stochastic models to observations of a virus disease using stochastic integration methods. Their methods have been further refined using Markov chain Monte Carlo (MCMC) (3,9,14,15,28) by Gibson (10). The present paper illustrates and extends the techniques of Gibson (10) by applying them to observations of epidemics of citrus tristeza virus (CTV) in two experimental plots, each consisting of 216 trees of Washington navel orange (Citrus sinensis (L.) Osbeck) on Troyer citrange (Poncirus trifoliata (L.) Raf.), observed in Eastern Spain, where disease is spread by the vector Aphis gossypii. These data, illustrated in Figure 1, describe the distribution of CTV in the plots (which were originally disease free) as observed at three times, each 1 year apart. They were first reported and analyzed (along with several other CTV epidemics) by Gottwald et al. (12) using several methods mentioned above. The main goals of the analysis were to determine the relationship of diseased trees with each other and to detect directionality in the spread. Conclusions of Gottwald et al. (12) were that there was little evidence of anisotropy or aggregation in the patterns and that spread did not appear to occur preferentially to trees adjacent to those already diseased. The simultaneous presence of interand intraplot spread was suggested as the mechanism underlying the observations. The purpose of the new analysis presented here is to shed further light on the mechanisms that might underlie these data, by relating the disease maps directly to spatiotemporal stochastic models of spread. I believe the approach illustrated is a valuable addition to the methodology applicable to spatiotemporal data, which complements existing methods by allowing investigation of alternative transmission mechanisms using data in a direct, efficient manner. Spatiotemporal stochastic models. I consider a simple stochastic model for the spread of a virus disease through a discrete population. The model is more complex than related models considered by Gibson and Gibson and Austin (10,11) in that it represents virus transmission both from individuals in the population and from sources outside the population. The members of the population are represented by the vertices of a rectangular lattice. For ease of notation, I shall use x to denote both the spatial coordinates of a vertex and the individual located there. An individual x may be in one of two states: susceptible or diseased. I denote the set of diseased individuals at time t by D(t). The model considered is stochastic, meaning it represents the element of chance, or probability, in the acquisition of the disease by any susceptible individual. The probabilistic nature of disease transmission is incorporated as follows. If the individual x is susceptible at time t and, if dt is small, then x acquires the disease in the interval (t, t + dt) with some probability that is proportional to dt and is related to the locations of the diseased individuals in the population. Background: Although childhood respiratory tract infections and low birth weight have both been associated with reduced adult lung function, little is known about the timing of these associations during life. We used data from the Newcastle Thousand Families Study to examine how these and other factors influenced FEV(1) at age 14 years and between 14 and 49 to 51 years.Methods: Detailed information was collected prospectively during childhood. At age 14 years, 252 members of the cohort were recruited into a case-control study of respiratory health, which included measurement of FEV(1). One hundred twenty-two of these were measured again at age 49 to 51 years. Linear regression models were used to examine cross-sectional and longitudinal influences on FEV(1).Results: Lower height (P < .001), lower BMI (P < .001), being breast fed for less than 4 weeks (P = .028), childhood history of severe respiratory illness (P = .014), childhood history of asthma (P = .004), childhood history of TB (P = .023), and birth into a lower social class (P = .049) were all significant independent predictors of lower FEV(1) at 14 years of age. Correspondingly, being a women (P < .001), and having a higher FEV(1) at age 14 years (P < .001), a lower standardized birth weight (P = .025), a greater lifetime number of cigarettes smoked (P = .007), and a childhood history of severe respiratory illness (P = .047) were all independently associated with a greater decline (or a smaller increase) in FEV(1) between age 14 and 49 to 51 years.Conclusions: This study suggests that the change in FEV(1) between youth and middle age depends on several factors acting throughout life, including FEV(1) in adolescence, sex, cigarette smoking, birth weight, and childhood respiratory health. 29. Cancellous bone properties and matrix content of TGF-beta2 and IGF-I in human tibia: a pilot study.

- Abstract
Transforming and insulin-like growth factors are important in regulating bone mass. Thus, one would anticipate correlations between matrix concentrations of growth factors and functional properties of bone. We therefore investigated the relationships of (1) TGF-beta2 and (2) IGF-I matrix concentrations with the trabecular microstructure, stress distribution, and mechanical properties of tibial cancellous bone from six male human cadavers. Trabecular stress amplification (VMExp/sigma(app)) and variability (VMCOV) were calculated using microcomputed tomography (muCT)-based finite element simulations. Bone volume fraction (BV/TV), surface/volume ratio (BS/BV), trabecular thickness (Tb.Th), number (Tb.N) and separation (Tb.Sp), connectivity (Eu.N), and anisotropy (DA) were measured using 3-D morphometry. Bone stiffness and strength were measured by mechanical testing. Matrix concentrations of TGF-beta2 and IGF-I were measured by ELISA. We found higher matrix concentrations of TGF-beta2 were associated with higher Tb.Sp and VMExp/sigma(app) for pooled data and within subjects. Similarly, a higher matrix concentration of IGF-I was associated with lower stiffness, strength, BV/TV and Tb.Th and with higher BS/BV, Tb.Sp, VMExp/sigma(app) and VMCOV for pooled data and within subjects. IGF-I and Tb.N were negatively associated within subjects. It appears variations of the stress distribution in cancellous bone correlate with the variation of the concentrations of TGF-beta2 and IGF-I in bone matrix: increased local matrix concentrations of growth factors are associated with poor biomechanical and architectural properties of tibial cancellous bone. 31. Silencing the snorers: no gain without pain?

- Abstract
OBJECTIVE: To compare the estimated effects of conservative surgery to those of a mandibular advancement splint (MAS) in the treatment of habitual snoring. METHOD: Prospective, observational, non-randomized cohort study. RESULTS: Adequate follow-up data were available for 88 participants (23 following coblation, 65 after provision of an MAS). The mean reduction in snoring symptoms inventory (SSI) score for the 23 subjects undergoing coblation was 9.83 (+/-standard deviation 11.43). Reported pain duration was greatest with uvula amputation, but uvula coblation did not always adequately reduce its bulk. Of the 65 MAS patients, 39 (60 per cent) used the device regularly, with a mean fall in SSI of 12 (+/-16.4, p=0.001). Approximately one in four patients in both groups achieved a significant fall in SSI (>15 points), and the measured effect sizes were close to 0.75 for both treatments. CONCLUSIONS: Mandibular advancement splints and coblation have similar efficacies. However, their efficacy does not match that of radical surgery. [ABSTRACT FROM AUTHOR]

34. Swallow-induced alterations in breathing in normal older people.

- Abstract
Respiratory parameters in 29 normal older subjects (mean age-73 years, SD = 5.7) were studied at rest, during single water swallows, and in continuous drinking. Respiration was recorded by intranasal air pressure changes and the moment of swallowing by pharyngoesophageal manometry. Compared with respiration at rest, respiratory rate increased immediately after 5 mL swallows as duration of breath cycles decreased from a mean of 3.8 s at rest to 3.5 s after swallowing (p = 0.003), but regularity of respiration was maintained. Deglutition apnea showed a trend toward increasing duration from a median of 1.06 to 1.24 s (p = 0.096) as bolus volume increased from 5 to 20 mL. Oxygen saturation levels were also compared, with a median fall of 2% during swallowing, but with larger and unexplained falls in several subjects. Direction of airflow monitoring showed 91% of single 5 mL swallows were followed by expiration, with postswallow inspiratory breaths occurring in 41% of subjects. Continuous swallowing of 100 mL of water from a cup and straw was typically performed with a pattern of multiple swallows per breath, with expiration occurring after 78.5% (cup) and 63.5% (straw) of swallows. This definition of swallowing-induced respiratory changes in normal older people forms a valuable basis for future comparison with elderly dysphagic patients. [ABSTRACT FROM AUTHOR]

- Abstract
We have studied the effects of chlormethiazole and diazepam given orally on the ventilatory and mouth occlusion pressure (P0.1) responses to CO2 in a placebo controlled study in 10 healthy volunteers. Diazepam 10 mg produced a significant reduction in both the ventilatory and P0.1 responses to CO2, and this was not associated with any effect on respiratory muscle power. Chlormethiazole 250 mg produced less drowsiness than diazepam 10 mg. Therefore in a subsequent study chlormethiazole 500 mg was compared with placebo. Chlormethiazole in either dose had no effect on CO2 responses or on maximum static respiratory pressures. We conclude that diazepam has a direct depressant effect on chemoreceptors and its effects on indices of ventilatory control are not due to impaired muscle function; chlormethiazole in the doses used has no such effects despite producing drowsiness. Sleep apnoea and hypopnoea have been reported in myotonic dystrophy, but it is unclear whether this is simply attributable to the respiratory muscle weakness which is common in this condition. We therefore investigated whether breathing and oxygenation during sleep were more abnormal in patients with myotonic dystrophy than in patients with non-myotonic muscle weakness. Seven subjects were studied in each of three groups: normal controls, myotonic dystrophy and non-myotonic weakness. Patients in the latter group were chosen to represent a similar range of severity of respiratory muscle weakness to those with myotonic dystrophy. Detailed polysoranography was performed; the severity of breathing disorders during sleep was quantified in terms of the frequencies of apnoea and hypopnoea and the degree of arterial desaturation. The myotonic patients showed more frequent apnoea and hypopnoea and more severe desaturation than the other two groups; the results in the non-myotonic patients were generally intermediate. The results suggest that abnormal breathing during sleep is common in myotonic dystrophy and is not due solely to the direct effects of respiratory muscle weakness. Somnolence, which is a well recognized symptom of myotonic dystrophy, was not clearly attributable to the sleep apnoea/hypopnoea syndrome nor to abnormal sleep architecture in the myotonic patients.

50. Robust methodology for the EBSD local misorientation analysis of surface cold work.

- Abstract
The amount of cold work induced by a surface hardening technique and the depth to which it is produced within a metallic material are both important parameters within the field of surface engineering. In this paper a methodology of establishing reliable estimates of the depth and magnitude of cold work in surface hardened nickel-based superalloy single crystals from a dataset (map) of electron backscattered diffraction images through the analysis of local misorientations is described in detail. The impact of varying a number of acquisition parameters within the scanning electron microscope and the impact of the various post-acquisition analysis parameters on the outcome of the analysis are both described and discussed in detail. The Python script used to perform this analysis is published in full.
