Introduction: In 2009, the World Health Organization identified vehicle crashes, both injury-related and fatal, as a public health hazard. Roadway lighting has long been used to reduce crashes and improve the safety of all road users. Ocular light exposure at night can suppress melatonin levels in humans. At sufficient light levels, all visible light wavelengths can elicit this response, but melatonin suppression is maximally sensitive to visible short wavelength light. With the conversion of roadway lighting to solid state sources that have a greater short wavelength spectrum than traditional sources, there is a potential negative health impact through suppressed melatonin levels to roadway users and those living close to the roadway. This paper presents data on the impact of outdoor roadway lighting on salivary melatonin in three cohorts of participants: drivers, pedestrians, and those experiencing light trespass in their homes., Methods: In an outdoor naturalistic roadway environment, healthy participants ( N = 29) each being assigned to a cohort of either pedestrian, driver, or light trespass experiment, were exposed to five different solid state light sources with differing spectral emissions and one no lighting condition. Salivary melatonin measurements were made under an average roadway luminance of 1.0 cd/m 2 (IES RP-18 Roadway Lighting Requirements for expressway roads) with a corneal melanopic Equivalent Daylight Illuminances (EDI) ranging from 0.22 to 0.86 lux., Results: The results indicate that compared to the no roadway lighting condition, the roadway light source spectral content did not significantly impact salivary melatonin levels in the participants in any of the cohorts., Conclusions: These data show that recommended levels of street lighting for expressway roads do not elicit an acute suppression of salivary melatonin and suggest that the health benefit of roadway lighting for traffic safety is not compromised by an acute effect on salivary melatonin., Competing Interests: The authors declare no conflict of interest

Introduction: Crashes involving roadway objects and animals can cause severe injuries and property damages and are a major concern for the traveling public, state transportation agencies, and the automotive industry. This project involved an in-depth investigation of such crashes based on the second Strategic Highway Research Program (SHRP 2) Naturalistic Driving Study (NDS) data including detailed information and videos about 2,689 events., Methods: The research team conducted a variety of logistic regression analyses, complemented by Support Vector Machine (SVM) analyses and detailed case studies., Results: The logistic regression results indicated that driver behavior/errors, involvement of secondary tasks, roadway characteristics, lighting condition, and pavement surface condition are among the factors that contributed significantly to the occurrence and/or increased severity outcomes of crashes involving roadway objects and animals. Among these factors, improper turning movements (odds ratio = 88), avoiding animal or other vehicle (odds ratio = 38), and reaching/moving object in vehicle (odds ratio = 29) particularly increased the odds of crash occurrence. Factors such as open country roadways, sign/signal violation, unfamiliar with roadway, fatigue/drowsiness, and speeding significantly increased the severity outcomes when such crashes occurred. The sensitivity analysis of the three SVM classifiers confirmed that driver behavior/errors, critical speed, struck object type, and reaction time were major factors affecting the occurrence and severity outcomes of events involving roadway objects and animals. Practical Applications: The study provides insights on risk factors influencing safety events involving roadway objects, including their occurrence and the severity outcomes. The findings allow researchers and traffic engineers to better understand the causes of such crashes and therefore develop more effective roadway- and vehicle- based countermeasures., (Published by Elsevier Ltd.)

Diffusion theory posits that information is disseminated throughout a social network by the persuasion of key opinion leaders (KOLs). This study examined the relative and combined influence of peer-identified KOL teachers (n = 12) and mental health providers (n = 21) on classroom teachers' (n = 61) self-reported use of commonly recommended classroom practices for children with attention-deficit/hyperactivity disorder in 6 low-income urban African American communities, relative to teachers (n = 54) at 4 matched schools who received mental health provider consultation only. Mixed-effects regression models showed that KOLs in collaboration with mental health providers promoted higher rates of teachers' self-reported use of recommended strategies than mental health providers alone, and that these effects were mediated by KOL support but not by mental health provider support. The results suggest an expanded role for KOL teachers as indigenous and natural supports for the dissemination and implementation of school-based mental health programs., ((c) 2008 APA, all rights reserved.)

Meningococcal arthritis is a recognized manifestation of Neisseria meningitidis infection, the presentation of which may be confusing. Although arthritis occurs in the setting of meningococcal meningitis, it may also be seen as a primary event without neurological involvement and with or without cutaneous manifestations. We describe a patient with primary meningococcal arthritis and review the literature relating to the clinical types and pathogenic mechanisms. Comparisons of patient series from 1980 to the present with those reported before 1980 are described.

Low-dose methotrexate is a commonly prescribed medication for the treatment of rheumatoid arthritis and other rheumatic diseases. Its effectiveness in ameliorating the clinical symptoms and findings has been well established, and adverse effects have been relatively infrequent, particularly compared to other immunosuppressive agents. Pneumocystis carinii pneumonia is most often seen in severely immunosuppressed patients related to the acquired immunodeficiency syndrome or treatment of malignancies with potent cytotoxic agents. However, P. carinii pneumonia can occur in patients receiving low-dose methotrexate concurrent with corticosteroids, nonsteroidal anti-inflammatory drugs, renal impairment, or a combination of factors. We report 1 patient and review 10 additional cases that support this relationship.

The association of left-sided pleural effusion and subcapsular splenic hematoma was reported in 1980. We describe a patient in whom a left-sided pleural effusion resulted from splenic vein thrombosis, a relationship not previously reported. Splenectomy was performed, demonstrating the spleen to be enlarged and hemorrhagic. The pleural effusion did not recur after splenectomy. The three effusions due to splenic hematomas reported previously cleared only after splenectomy, suggesting splenectomy may be necessary to prevent reaccumulation. We propose possible pathophysiologic mechanisms and recommend that splenic vein thrombosis be considered in the differential diagnosis of unexplained left-sided pleural effusions.

The eosinophilia-myalgia syndrome is a newly described disorder related to the ingestion of L-tryptophan-containing products. Its presentation may mimic other disorders characterized by eosinophilia and muscle pain and/or weakness, but can be differentiated by certain characteristic laboratory and pathologic findings. We report two such cases, describe their features, and review similar syndromes.

Chrysotherapy has become a cornerstone in the treatment program of patients with rheumatoid arthritis. Unfortunately, approximately one third of these individuals will experience an adverse drug reaction at some time. Cutaneous manifestations are most commonly recognized and do not necessarily preclude reinstitution of gold after their resolution. Other complications involving the hematopoietic and renal systems are less frequent but may be severe. Recently, investigators have described serious toxic reactions involving the liver and lungs. The pathophysiologic mechanism of these untoward effects appears to be immunologic, although there are conflicting data. Treatment is primarily supportive, and the role of corticosteroids and chelating agents remains controversial. Emphasis should be placed primarily on early detection of adverse reactions through patient education and careful monitoring of blood and urine values.

Latrodectism-envenomation by the black widow spider, Latrodectus mactans-has been reported in virtually every state. The spider is particularly prevalent in the Southwest, Southeast and California, and symptoms from its bite are painful and may be confusing to clinicians. Prompt improvement can be expected in most cases with administration of calcium and muscle relaxant, but some patients may require treatment with antivenin. No deaths occurred in our series of 11 patients, although one case was complicated by serum sickness.

The harvesting and consumption of wild mushrooms has become a popular pastime in many areas of the United States. Reports of mushroom poisonings and fatalities have been increasing since 1964. Between May 1973 and April 1978, 16 cases of mushroom poisoning were treated at our institution, including 13 within a 12-month period ending April 1978. All patients exhibited either gastrointestinal or neurologic symptoms, which responded to supportive therapy. There were no fatalities. In most cases, the clinical course can be predicted and appropriate therapy instituted if the mushroom is identified or if the interval between the ingestion and the onset of symptoms can be ascertained and certain characteristic symptoms are observed.

Intermittent intravenous cyclophosphamide therapy has been shown to be effective in the management of lupus nephritis. There have been no reports of malignancies associated with such treatment regimens. We describe a patient who developed acute nonlymphocytic leukemia after receiving a total dose of 4 gm of cyclophosphamide over a 1-year period.

An acute arthritis may accompany the onset of cancer. A case of metastatic adenocarcinoma of the synovium presenting as an acute arthritis is described. The diagnostic importance of a careful synovial fluid analysis and the use of closed synovial biopsy are emphasized.
