4. Impact of low skeletal muscle mass in oropharyngeal cancer patients treated with radical chemo-radiotherapy: A mono-institutional experience

Abstract

Aims: Low skeletal muscle mass index (SMI) has recently emerged as an independent prognostic factor in oncological patients and it is linked with poor survival and higher treatment toxicity. The present study aims to determine the possible impact of low SMI on survival and acute toxicity in oropharyngeal patients.Methods: Seventy-six patients with locally advanced oropharyngeal squamous cell carcinoma (stage III-IVC) were treated in our institution with Helical TomoTherapy® (HT - Accuray, Maddison, WI, USA) between 2005 and 2021. All patients received concomitant platinum-based chemotherapy (CT) (at least 200 mg/m2). The SMI was determined using the calculation of cross-sectional area at C3. Twenty patients (26%) presented pre-treatment low SMI, according to Chargi definitions.Results: All patients concluded the treatment. Thirteen patients with low SMI (65%) and 22 patients with normal SMI (39%) presented acute toxicity greater than or equal to grade 3, but this difference was not statistically significant (p-value = 0.25). Overall survival was analyzed in 65 patients, excluding those who finished CT-RT less than six months before the analysis. Overall survival was significantly lower in low SMI versus normal SMI patients (p-value = 0.035). Same difference was observed in N0-N2a patients, suggesting an important role of SMI also in lower nodal burden and putatively better prognosis.Conclusions: Although the results are limited to a small population, our case series has the advantage to be very homogeneous in patients and treatment characteristics. In our setting, SMI demonstrated a crucial impact on overall survival. Further investigation with larger samples is necessary to confirm our results to improve patient outcomes.

13. Evaluación de la producción de etanol con dos cepas comerciales y dos tipos de mieles de caña provenientes de la región de Gualivá, departamento de Cundinamarca

Abstract

El etanol es una fuente de energía potencial y su producción a partir de biomasa renovable ha adquirido gran popularidad, alrededor del mundo se encuentran investigaciones para su producción mediante la utilización de sustratos regionales de bajo valor comercial, desechos orgánicos y residuos industriales. Con esta iniciativa se realizó un proceso de fermentación, utilizando como sustratos dos tipos de mieles de caña, una miel de caña estándar y otra invertida, las cuales se caracterizaron de acuerdo a pH,°Brix, densidad, viscosidad y pureza relativa, fueron sometidas a un proceso de pasteurización previamente a la inoculación y fermentación. Se utilizaron dos cepas comerciales de Saccharomyces cerevisiae, ATCC® 4098 TM* y ATCC® 9763 TM* , aisladas del vino blanco y de una destilería, se realizó un inóculo de cada una durante cinco días en una habitación con control de temperatura, durante este tiempo se realizaron liberaciones de presión manuales cinco veces al día para evitar la inhibición de las cepas; la fermentación se realizó a una concentración de 15 °Brix y un pH de 4,5 y un 10% v/v del Inóculo durante tres días sin aireación; por último se determinó la producción de etanol usando la técnica de cromatografía de gases acoplada a masas.

17. Early and late CT findings following SBRT for lung tumors: Analysis of a monoinstitutional series.

Abstract

Aims: Radiological lung density changes are commonly observed in patients undergoing SBRT. Early changes include consolidation and ground glass opacities, while late changes include fibrosis, bronchiectasis, loss of lung volume and further consolidation. This study retrospectively analyzed a monoinstitutional series of patients treated with lung SBRT and correlated the CT findings with a number of clinical and dosimetric parameters. Methods: Between December 2016 and March 2021 sixty patients with seventy primary or secondary lung lesions, were treated on a Linac platform with a VMAT technique to a BED10 ≥100 Gy. Patients, disease and treatment characteristics are summarized in Table 1. In order to describe the radiological findings over time, the follow-up was divided into two periods: early (within 6 months) (n=70) and late (> 6 months) (n=43) time after treatment. All the CTs were double-checked by 2 experienced radiation oncologists. Radiation-induced lung injuries were evaluated according to Ikezoe and Koening classification, respectively. The correlation between the CT pattern and clinical and dosimetric parameters was evaluated. Results: Median follow up was 9.6 months (1.5-26.8). Among the 70 CT examined at early time after SBRT, 31 had no evidence of increased density, 13 had diffuse consolidation, 11 had patchy consolidation, 8 had diffuse ground glass opacities, 6 had patchy ground glass and 1 had signs of progression. Late lung injuries were mass like pattern (20), modified conventional pattern (5), scar-like pattern (10). Eight lesions showed signs of progression. Neither clinical nor dosimetric characteristics were found to be significantly associated with a specific CT pattern, either in the early or late timeframe. Likewise, no significant correlation was found between early and late changes. Conclusion: Lung injuries represented common radiological findings after SBRT, irrespective of clinical symptoms.
