Abstract Background In this study, we investigate the utilization of Instagram by public health ministries across the Gulf Cooperation Council (GCC) nations to disseminate health-related information during the COVID-19 pandemic. With Instagram’s visual-centric approach and high user engagement, the research aims to investigate its critical yet complex role in information dissemination amid a health crisis. Methods To examine how Instagram communication strategies align with the CDC’s Crisis and Emergency Risk Communication (CERC) framework, we employ the content analysis method. This approach helps to evaluate the effectiveness and challenges of employing Instagram for health communication within a region known for its significant social media usage. Results Findings indicate that Instagram serves as a vital platform for the rapid dissemination of health information in the GCC, leveraging its visual capabilities and wide reach. The GCC ministries of health utilized Instagram to demonstrate a consistent and strategic approach to communicate essential COVID-19 related information. Kuwait and Bahrain were the most active of all the assessed ministries with respect to the number of engagement metrics (likes and comments). Most of the posts, as per the CERC framework, were informational and related to vaccine infection and death cases. The second most salient theme in line with the CERC framework was about promoting actions, followed by Instagram posts about activities, events, and campaigns. Conclusions The research underscores Instagram’s potential as a powerful tool in enhancing public health resilience and responsiveness during health emergencies in the GCC. It suggests that leveraging social media, with careful consideration of its affordances, can contribute significantly to effective health communication strategies in times of crisis.

This study aimed to explore ACS treatment strategies (ischemia-guided vs early invasive) and risk factors among patients diagnosed with ACS in a tertiary care hospital in Palestine and to evaluate related outcomes regarding future events and standard clinical guidelines.Methods: This retrospective cohort study reviewed patient data from a Palestinian medical hospital. The study included 255 patients ≥ 18 years who were hospitalized between January 2021 and December 2021 and diagnosed with ACS. The data were analyzed using the Statistical Package for Social Science (SPSS).Results: 71% of the participants were males. The mean age was 59.59± 11.56 years. Smoking, diabetes, and hypertension were the most common risk factors. Unstable angina (UA) was the most prevalent ACS type, accounting for 43.1% (110) of cases, whereas NSTEMI accounted for 39.2% (100) and STEMI accounted for 17.6% (45) of cases. An ischemic-guided strategy approach was used in 71% (181) of the patients. Upon discharge, the most prescribed medication classes were antiplatelets (97.6%), statins (87.1%), PPIs (72.5%), and antihypertensives (71.8%). Treatment strategies were selected according to the clinical guidelines for most ACS types.Conclusion: ACS management in Palestine continues to evolve to overcome barriers, decrease patient mortality, and decrease hospital stay. UA and NSTEMI were the most common ACS diagnoses at admission, and the ischemic strategy was the most common modality. The findings of this study call for an increased awareness of CVD risk factors, resource availability, and adherence to clinical guidelines to improve patient outcomes and community health.Keywords: acute coronary syndrome, ST-elevation myocardial infarction, non-ST-elevation myocardial infarction, unstable angina, ischemia-guided strategy, early invasive strategy

Abstract Strongly-interacting nanomagnetic arrays are ideal systems for exploring reconfigurable magnonics. They provide huge microstate spaces and integrated solutions for storage and neuromorphic computing alongside GHz functionality. These systems may be broadly assessed by their range of reliably accessible states and the strength of magnon coupling phenomena and nonlinearities. Increasingly, nanomagnetic systems are expanding into three-dimensional architectures. This has enhanced the range of available magnetic microstates and functional behaviours, but engineering control over 3D states and dynamics remains challenging. Here, we introduce a 3D magnonic metamaterial composed from multilayered artificial spin ice nanoarrays. Comprising two magnetic layers separated by a non-magnetic spacer, each nanoisland may assume four macrospin or vortex states per magnetic layer. This creates a system with a rich 16 N microstate space and intense static and dynamic dipolar magnetic coupling. The system exhibits a broad range of emergent phenomena driven by the strong inter-layer dipolar interaction, including ultrastrong magnon-magnon coupling with normalised coupling rates of $$\frac{\Delta f}{\nu }=0.57$$ Δ f ν = 0.57 , GHz mode shifts in zero applied field and chirality-control of magnetic vortex microstates with corresponding magnonic spectra.

AbstractThis study investigates the insights of ten Arab animators who are actively involved in the production of animated content in seven Arab countries. The research endeavors to shed light on two significant domains: (i) the perspectives of animators regarding the Arabic animation sector and (ii) the numerous determinants that impact the industry’s growth. By utilizing thematic analysis to interpret the data obtained from in-depth interviews, this study reveals that Arab animators hold a pervasive sense of dissatisfaction with the current state of the industry. Notwithstanding these obstacles, the animators strive to elevate their work to an international level and offer insightful critiques of the challenges faced by the regional film and animation industries. The study delineates critical determinants influencing the Arabic animation sector, encompassing technological assistance, financial limitations, access to training and educational resources, and skilled labor. The results of this study have important ramifications for the progress of the film and media industries, especially in developing nations where these sectors encounter substantial obstacles.

Background:. Body contouring surgery removes excess skin and fat, often after massive weight loss. Some reports suggest that patients who have previously undergone obesity (bariatric) surgery are at excess risk of subsequent bleeding, possibly due to complex nutritional and metabolic sequelae of massive weight loss. Methods:. A retrospective cohort study of intraoperative blood loss and postoperative bleeding indicators were examined for patients who had undergone abdominoplasty. Participants were categorized based on their history of previous obesity surgery, and outcome variables were compared using odds ratio, followed by subgroup comparison between a history of restrictive versus malabsorptive obesity surgery, Results:. The study included 472 patients, of which 171 (36.2 %) had a history of obesity surgery. Mean age was 40.4 years, and 402 (85.1%) participant were women. Fifty-five (11.6%) patients were smokers whereas 65 (13.7%) were hypertensive. Mean body mass index before surgery was 30.2 kg per m2, and average time between obesity and body contouring surgery (if applicable) was 35.8 months. Patients with a history of obesity surgery exhibited greater intraoperative blood loss (162.2 mL versus 132.1 mL; P = 0.001), drainage volume at 24 h (155 mL versus 135 mL; P = 0.001), and total drainage volume (300ml versus 220 mL; P = 0.001). Postoperative hematoma requiring surgical re-exploration was almost three times higher following a history of obesity surgery (4.7% versus 1.7 %; P = 0.05). Conclusions:. History of obesity surgery increases intraoperative blood loss, postoperative serosanguinous drainage volumes, and the risk of postoperative hematoma requiring surgical evacuation after abdominal body contouring procedures.

We describe RA patients with high and normal UA and study its association with RA activity.Methods: Adult RA patients from the Kuwait Registry for Rheumatic Diseases (KRRD) were studied from February 2012 through March 2022. Patients with documented UA levels were included. UA of > 357 μmol/L (6mg/dL) was considered high. Statistical comparison and correlation were made using multivariate logistic regression.Results: Overall, 1054 patients with documented UA. A total of 158 patients (15%) had high UA level with a mean of 409± 44.4μmol/L. The mean age for the high UA group and low UA group were 59.3 ± 10.7 years and 54.5 ± 12.4 years, respectively (p< 0.001). 49.4% were female in high UA group, and 62.2% were female in low UA group, respectively (p< 0.05). Logistic analysis showed an inverse relation between DAS28 and UA, as lower DAS28 score was associated with higher UA level (p=0.032) OR 1.39. There was a direct relation with HAQ, creatinine and UA. A higher HAQ is associated with a higher UA level (p=0.019) OR 0.78. High creatinine level is also associated with high UA level (p< 0.001) OR 0.24. The use of antirheumatic drugs was similar among patients with high and normal UA.Conclusion: RA patients with a higher UA had a lower disease activity despite using similar antirheumatic drugs. مقدمة : يعد القصور الكلوي المزمن مشكلة صحية عامة في جميع أنحاء العالم مع معدل إمراضية ووفيات هامة وهو آخذ في الازدياد . ومن بين الخيارات العلاجية المتاحة للقصور الكلوي المزمن هو التحال الدموي الأكثر استخدماً . لإجراء التحال يجب إنشاء مدخل وعائي مؤقت أو دائم، ويعتبر الناسور الشرياني الوريدي الذاتي الوسيلة الأكثر ملاءمةً فهو منفذ طويل الأمد يتيح تحال فعال وآمن مع نسبة إنتان وتكاليف رعاية طبية منخفضة . الخثار الباكر يعرف بأنه الخثار الذي يحدث خلال شهر من الإجراء ويشخص سريرياً (غياب الهرير أو النبض بالجس أو النفخة بالإصغاء) وبالأمواج فوق الصوتية. هدف البحث : يهدف البحث إلى دراسة عوامل الخطورة التي تتوقع حدوث الخثار الباكر للناسور الشرياني الوريدي الذاتي المعد للتحال الدموي المجرى حديثاً وبالتالي محاولة التقليل من حدوث الفشل والخثار. طرائق البحث: تم إجراء دراسة رصدية تقدمية ، شارك فيها 80 مريض قصور كلوي إنتهائي أُجري لهم نواسير شريانية وريدية حديثة بقصد إجراء التحال الدموي ، و ذلك خلال الفترة الممتدة بين عامي 2022 _2023 ، حيث تم دراسة المتغيرات التالية : العمر،الجنس ،مكان الناسور ، قطر الشريان المغذي ، قطر الوريد ، إعطاء المميعات ،رأي الجراح ، تقنية المفاغرة. تم متابعة المرضى لمدة شهر بعد الجراحة و تقييم نتيجة الإجراء من حيث نضج أو خثار الناسور . النتائج: حدث الخثار الباكر لدى 17 مريض (21.2 %) . كان معدل الخثار أعلى في الأعمار المتقدمة ( متوسط عمر 61.23±9.3 )،و كان أعلى عند الإناث من الذكور بنسبة 8% . لم يلاحظ وجود فروقات ذات دلالة إحصائية فيما يتعلق بمكان الناسور الشرياني الوريدي ، إلا أن 7% من حالات الخثار الباكر كان في المواقع القاصية . بلغت نسبة حالات الخثار الباكر 76.5 % مع قطر وريدي أقل من 2ملم ، و 52.9 % مع قطر شريان مغذ أقل من 2 ملم . لم نجد فروق ذات أهمية احصائية ما بين تناول مضادات التخثر و حدوث الخثار الباكر للناسور الشرياني الوريدي . لوحظ أن 7 % من حالات الخثار الباكر كانت مع بضع الشريان الأقل من 6 ملم ، و عدم وجود أهمية احصائية فيما يتعلق بتقنية المفاغرة باستخدام خيطين (P value= 0.5)

مقدمة: يعتبر الداء السكري أكثر الإضطرابات الاستقلابية شيوعاً حول العالم، وقرح القدم السكرية أحد أهم المضاعفات الصحية المرتبطة بالداء السكري المزمن وهي السبب الرئيسي لبتر الساق غير الرضي. هدفت الدراسة الحالية إلى تقييم تأثير تطبيق تقنية الضغط السلبي على درجة التئام قرح القدم السكرية لدى مرضى السكري من النمط الثاني في مشفى تشرين الجامعي في اللاذقية. المناهج: وفق المنهج التجريبي طبقت الدراسة على عينة عشوائية شملت 30 مريضاً من كلا الجنسين في قسم جراحة الأوعية، تم تقسيمهم إلى مجموعتين تجريبية (15 مريضاً) خضعت لتغيير ضماد قرح القدم باستخدام تقنية الضغط السلبي، وضابطة (15مريضاً) خضعت لتغيير الضماد بالطريقة الروتينية المعتمدة في المشفى. تم تقييم درجة التئام القرح ودرجة القرح باستخدام مقياس ريسفتش ومقياس درجة القرح خلال الأيام (1،5 ,10،15، 20) بعد بدء تطبيق الضماد. النتائج: أظهرت النتائج وجود فروق ذات دلالة إحصائية مهمة جداً في متوسطات رتب درجة التئام قرح القدم السكرية (U=44,P=0.004)، ووجود فروق ذات دلالة إحصائية مهمة في اجمالي درجة القرح (X2=10.09، p=0.018) مقارنة بمرضى المجموعة الضابطة. الاستنتاجات: تقنية الضغط السلبي لها دور في تحسين شفاء قرح القدم السكرية، لذلك توصي الدراسة بضرورة اعتمادها كطريقة ضماد روتينية لمرضى قرح القدم السكرية واجراء المزيد من الأبحاث حولها وذلك لتحسين شفاء المرضى والتقليل من الحاجة لإجراء بتر الأطراف السفلية.

Abstract Background Low back pain (LBP) is a highly prevalent musculoskeletal condition and causes activity limitations resulting in reduced productivity and high medical expenditure. Muscle energy technique (MET) is a therapeutic technique that has the potential to be successful in LBP, although the evidence for this notion is still inconclusive. The effectiveness of the muscular energy technique on pain intensity and disability for individuals with chronic low back pain was evaluated in published studies through this systematic review of the literature. Methods Studying the English language and humans, as well as scanning article reference lists from PubMed, Scopus, ScienceDirect, the Cochrane Library, Ovid, ClinicalTrials.gov, and Embase, was searched until October 30, 2022. Randomised controlled studies reporting on the effectiveness of muscle energy technique on pain intensity and disability for chronic low back patients were included. Information related to demographics, number and duration of treatment, MET protocol, assessment tools used for pain and disability, and key findings was extracted. The Physiotherapy Evidence Database (PEDro) classification scale was used to assess the methodological quality of studies, and two authors assessed the risk of bias and extracted the data independently. Results Seventeen research studies (including 817 participants) were retrieved and included for qualitative analysis. The studies published between 2011 and 2022 were retrieved, and the sample size ranged from 10 to one hundred twenty-five participants. The age of the subjects ranged between 18 and 60 years, and interventions were done between 2 days and 12 weeks. Of the included 17 studies, five were from Egypt, four were from India, two each from Iran and Nigeria, and one each from Brazil, Poland, Thailand, and Pakistan. Compared to other interventions or the control groups, MET was found to significantly, although modestly, decrease the severity of pain and reduce functional disability in patients with chronic LBP. Most of the included studies had moderate to high study quality. Conclusion In patients with chronic low back pain (CLBP), it was observed that MET alone as well as in conjunction with other interventions was found to be beneficial in reducing pain intensity, improving lumbar spine range of motion, and decreasing the degree of functional disability.

المقدمة: الكيسات الجلدانية هي آفات حميدة نسيجياً قد تسبب أعراضاً موضعية نتيجة لحجمها وموقعها، وقد يحدث التحول الخبيث كاختلاط متأخر لهذه الكيسات. الحالة السريرية: راجعت مريضة بعمر 57 عام مستشفى تشرين الجامعي بكتلة عرطلة في منطقة الظهر. ذكرت المريضة أن هذه الكتلة كانت كيساً صغيراً لمدة طويلة تصل إلى عشر سنوات، لكن وفي الفترة الأخيرة حدث نمو هام في الكيس خلال الشهرين الأخيرين. بالفحص السريري كانت المريضة في حالة عامة جيدة ولديها جنف خفيف. أجري لها تصوير طبقي محوري أظهر كتلة كبيرة تحت الجلد بقياس 25×20×9 سم تمتد من مستوى الفقرات الرقبية حتى الفقرات القطنية العلوية مع ارتشاح في العضلات الظهرية العريضة، والعضلات الوربية وتصل للعضلات الناصبة للعمود الفقري. تم إجراء الاستئصال الجراحي غير الكامل بسبب الارتشاح العميق في العضلات. ثم وضع التشخيص بعد الفحص النسيجي الباثولوجي الذي بيَن وجود تحول خبيث إلى كارسينوما الخلايا الحرشفية SCC. وقد تلقت المريضة العلاج الشعاعي بعد الاستئصال الجراحي. الخلاصة: التحول الخبيث في الكيسات الجلدانية ممكن الحدوث رغم قلَة شيوعه. هذه الكيسات قد تسبب خللاً وظيفياً أو جمالياً، لذلك من الأفضل أن يتم علاجها أبكر ما يمكن.

الهدف: دراسة عوامل الخطورة لدى مرضى الخثار الوريدي العميق الحاد في الطرفين السفليين، وتحديد نسبة كل منها، إضافة لتحديد حالات الخثار البدئي غير المحرض والتي لم يتبين وجود أي عامل مؤهب فيها. طرائق الدراسة والمرضى: تم إجراء دراسة رصدية مقطعية تضمنت مرضى الخثار الوريدي العميق الحاد في الطرفين السفليين، والذين تم قبولهم في شعبة جراحة الأوعية الدموية- مستشفى تشرين الجامعي خلال الفترة بين 1-1-2022 و 31-12-2022.تم أخذ قصة مرضية مفصلة لجميع المرضى مع فحص سريري كامل، وإجراء إيكو دوبلر للطرفين السفليين، إضافة لتقييم مخبري وشعاعي يتضمن إيكو بطن و/أو صورة صدر عند الضرورة. تم اعتماد تشخيص الخثار الوريدي العميق الحاد بناء على الفحص وموجودات الإيكو دوبلر، حيث تضمنت المعايير التشخيصية:تغير في صدوية اللمعة، عدم قابلية الوريد للانخماص بالضغط وغياب الجريان العفوي أو المحرض. لم يتم الاعتماد على التصوير الوريدي الظليل أو الطبقي المحوري الوريدي في أي من الحالات المشخصة، وعلى ذلك تمت دراسة البيانات. الـنـتـائـج: شملت عينة البحث 46 مريضاً وبلغت نسبة الذكور 56.5% والإناث 43.5% مع متوسط عمر بلغ56.7 ±14.9 سنة. توزعت عينة الدراسة على جميع الفئات العمرية وبلغت أعلى نسبة 54.3% ضمن الفئة 51-70 سنة. وفقاً للموقع التشريحي: توضع الخثار لدى 52.2% في الطرف السفلي الأيسر، 39.1% في الأيمن و8.7% في كلا الطرفين. لوحظ وجود عامل مؤهب في دراستنا لدى 25 مريضاً وبنسبة بلغت 54.34%، في حين لم يتبين وجود أي عامل خطورة محدد لدى 45.6% من المرضى. أشيع عوامل الخطورة في دراستنا تمثلت بما يلي: التدخين 37% ،وجود خباثة 15.2% (أشيعها ورم المثانة 6.5% يليه ورم الكولون والمبيض)، قصة عمل جراحي حديث 15% (أشيعها التداخلات على البطن 6.5% يليها الجراحات النسائية والبولية) ، البدانة و سوابق خثار وريدي 6.5%، دوالي الطرفين السفليين وقصة استلقاء مديد 4.3%. لم يتبين في دراستنا وجود قصة عائلية صريحة لدى أي من مرضى الدراسة. الخلاصة:بلغت نسبة المرضى الذين لديهم عوامل خطورة لتطور خثار وريدي 54.3%، وأشيع العوامل المؤهبة لديهم تمثلت بـ : الجراحة الحديثة، الخباثة، التدخين وزيادة الوزن

AbstractThe study aims to investigate the attitude of students with postgraduate diplomas in teaching towards the teaching profession and its relationship with specialization, cumulative average, school quality, and school grade variables. The study sample consisted of 300 second-semester students enrolled in the educational training program for the academic year 2021–2022. Students Attitudes Scale towards the teaching profession was applied to identify the attitudes of the students. The results showed statistically significant differences in the attitudes of the students of the educational training program towards the teaching profession on account of the specialization and cumulative average variables. There were no statistically significant differences in the student’s attitudes towards the teaching profession on account of the school grade variable in which the student was trained. The study concluded that various programs, training and activities are required to develop positive attitudes in students towards the teaching profession and to encourage them to adopt the teaching profession.

Summary:. Gunshot injuries to the hand and upper extremity can be divided into low and high-energy injuries. Nonballistic firearms such as pellet guns are generally considered low-energy guns but can be associated with serious morbidity and even mortality. Management is tailored according to the severity and type of injured structures. Here, we present the case of a 21-year-old soldier who accidently shot his thumb with an air gun while on a bird hunting trip, whose case was managed with extraction placement of bone graft and collagen matrix to prevent nail deformity. This case was written to increase the awareness about the need for safe use of such guns and to display the authors preferred way of management.

Background:. Fingertip injuries are common; when the bone is exposed, it requires more than skin to cover the defect. After the introduction of the concept of perforator flaps, trials have been made to reconstruct finger defects with perforator flaps. Here, we describe a series of fingertip injuries that were treated with reverse flow island finger perforator flap. Methods:. We had 36 patients from January 2019 to June 2021, most of them male workers. We reported on the sizes of the defects and donor sites, the need to cover the pedicle with a skin graft, and complications. Patients were followed up for a period of 6 months, and the detailed elevation technique was mentioned. Results:. Thirty-four flaps survived completely. In 50% of the cases, patients had congestion and epidermolysis with preserved underlying flap, which had normal healing course. Two patients with diabetes had infections; one of them had 40% necrosis of the flap, and one additional case had superficial eschar with preserved underlying flap tissues. Conclusions:. Fingertip reconstruction rather than bone shortening is preferred to maintain the finger length. Multiple choices were suggested, but this flap had the disadvantage of limited coverage. After introducing the concept of perforator flaps into finger reconstruction, still more choices were added. The retrograde island digital artery perforator flap is a reliable choice, which is done in one stage and does not require loss of the digital artery.

Nanostructures of metal oxides of CuO, and TiO2 were prepared by the hydrothermal method. Also, CuO/TiO2 nanocomposites were prepared by the hydrothermal technique using an equal ratio of 1:1 The properties of the prepared compounds were investigated through Field Emission Scanning Electron Microscopy FE-SEM, Transmission Electron Microscopy TEM, X-Ray Diffraction. The images of FE-SEM have confirmed that the prepared TiO2 , CuO, and nanoparticles were structured as Nanoparticles, Spherical, and CuO/ TiO2 nanocomposites which were mixed in an equal ratio were structured as rocks-like structures. The results of the X-ray diffraction indicated the presence of CuO, and TiO2 as (monoclinic structures, Tetragonal) respectively. with the average crystallite sizes,( 13.10,8.65 and 10.12)nm, for CuO , TiO2 and CuO/TiO2 respectively. The optical properties of the prepared nanoparticles CuO , TiO2 , and CuO/TiO2 were studied through UV-Vis Spectroscopy. The optical band gap of the prepared products was determined by UV-Visible spectrophotometry and was found to be 2.1, 3.0, and 2.8 eV for TiO2, CuO, NPs, and CuO /TiO2 nanocomposites respectively. Dye-sensitized solar cells (DSSCs) fabricated based on TiO2, CuO, NPs, and CuO /TiO2 nanocomposites are casted onto (FTO) substrates (front electrode), while the back electrode is carbon/FTO-glass substrate. Two types of natural dyes were used in this study. Chlorophyll pigment in green leek and anthocyanin red pigment in cochineal pomegranate whereas iodine/ iodide KI/I2 is used as the electrolyte solution. The solar cells efficiency rates (1.51,0.74 and 0.94) for CuO, TiO2 NPs, and CuO /TiO2 nanocomposites/ are chlorophyll pigments present in green leek respectively. and The solar cells efficiency rates (1.20,0.558 and 0.88) for CuO, TiO2 Nps, and CuO/TiO2 nanocomposites/ anthocyanin pigment present in red pomegranate respectively

Objective: The objective of this study was to evaluate the resin-ceramic adhesion of a long-carbon-chain silane (LCSI)-containing resin cement. Methods: Polished lithium disilicate ceramic discs were etched with hydrofluoric acid and randomly assigned into four groups; (PSAP), cemented using a silane-free resin cement with no prior priming; (PSAP-S), primed using a silane-containing primer before cementation using a silane-free resin cement; (PSAU), cemented using a LCSI-containing resin cement with no prior priming; (PSAU-S), primed as for the group (PSAP-S) and cemented using a LCSI-containing resin cement. The cemented blocks were sectioned into microbeams. The resin-ceramic microtensile bond strength (μTBS) was measured at 1 week and after thermocycling. The failure modes of the tested microbeams were evaluated. Results: The μTBS of the LCSI-containing and silane-free resin cements, either with or without a prior priming step, did not significantly differ. The adhesion of the LCSI-containing resin cement to lithium disilicate ceramic, either with or without a prior priming step, did not significantly deteriorate after artificial aging. Conclusions: The long-carbon-chain silane (LCSI) monomer incorporated in the resin cement eliminated the need for a silane priming step of a hydrofluoric acid-etched lithium disilicate ceramic.

مقدمة : تعد جراحة استئصال بطانة الشريان السباتي CEA هي العلاج الأولي الفعال في الوقاية من السكتة الإقفارية عند مرضى تضيق الشريان السباتي العصيدي العرضي ,بالرغم من ذلك , دراسات عديدة وثقت وأكدت حدوث اختلاطات هامة لا يمكن تجاهلها حول الجراحة . . الهدف: الهدف الأولي هو معرفة معدل الوقوع الحقيقي للاختلاطات حول العمل الجراحي المرافقة لاستئصال بطانة الشريان السباتي في شعبة جراحة الاوعية في مستشفى تشرين الجامعي في اللاذقية. أما الهدف الثانوي فهو البحث عن عوامل خطورة موجودة عند المريض وأخطاء تقنية ضمن العمل الجراحي تؤهب لحدوث مثل هذه الاختلاطات. الطرائق: دراسة استقبالية _ استرجاعية من نمط حالة شاهد, شملت 56 مريضاً أجري لهم استئصال بطانة شريان سباتي في شعبة جراحة الأوعية في مستشفى تشرين الجامعي في اللاذقية خلال الفترة الممتدة بين 1/1/2017 و 1/1/2022 م حيث تم جمع البيانات فيما يتعلق بالجنس والعمر والتشخيص ونوع الجراحة والمرافقات من سكري وتدخين وسوابق سكتة دماغية وضغط ونشبة دماغية عابرة وداء عصيدي إكليلي, وجود أو غياب داء سباتي مقابل (تضيق أو انسداد), بالإضافة إلى تقييم حالة الشرايين الفقارية , والاختلاطات الحاصلة حول العمل الجراحي وخلال فترة المتابعة أثناء الإقامة في المستشفى وحتى 30 يوماً من الجراحة. النتائج : نسبة الذكور إلى الإناث 3.3:1 sex ratio(M:F) وكان 53.5% من المرضى من الفئة العمرية (61-70) سنة .كان تكنيك استئصال البطانة التقليدي هو التكنيك الأكثر إجراءً بنسبة 80.3% من الحالات. نسبة المدخنين 71.4% من عينة البحث, ونسبة المرضى السكريين 39.2% فقط مقابل 60.8% من غير السكريين. معدل الاختلاطات حول الجراحة 33.9% توزعت على الشكل التالي: سكتة بعد الجراحة بنسبة 17.8%, عدم استقرار هيموديناميكي (هبوط الضغط – بطء قلب) بنسبة 8.9%, احتشاء عضلة قلبية بعد الجراحة بنسبة 5.3%, بينما هيماتوم الجرح كان بنسبة 1.7% فقط, وكانت نسبة حدوث أذية أحد الأعصاب القحفية وإنتان جرح ما بعد الجراحة ومتلازمة فرط التروية الدماغية 0% لكل منها. إن وجود داء عصيدي سباتي مقابل (تضيق أو انسداد) – هبوط ضغط أثناء إجراء الكلامب خلال العمل الجراحي – طول مدة العمل الجراحي أكثر من ساعتين بالإضافة لوجود ارتفاع توتر شرياني مرافق لدى المريض كلها عوامل خطورة لتطوير سكتة دماغية حول الجراحة. الاستنتاج: الغالبية العظمى من الاختلاطات حول الجراحة للتداخل على الشريان السباتي الباطن هي اختلاطات ثانوية ويمكن تدبيرها طبياً بشكل محافظ, لكن الحذر والتمييز الباكر لهذه الاختلاطات والتدبير المناسب لها على قدر كبير من الأهمية في تحسين النتائج الوظيفية لدى المريض.

One of the most important ways to combat the previously mentioned disaster is the use of natural alternatives like medicinal plants. Our study aimed to estimate the anti-inflammatory property, and antibacterial effects of probiotics Lactiplantibacillus plantarum and ethanol extracts of Lawsonia inermis leaves against Staphylococcus aureus when they were used separately or collectively as synergism.Material and Methods: Experimentally induced infected wound model in mice was created and divided into 10 groups then treated for two days by L. plantarum and L. inermis individually or in combination, followed by biochemical assays. The antibacterial, anti-inflammatory, and wound healing activity were evaluated through histopathological sections taken before and after treatment.Results: Our results revealed that L. plantarum and L. inermis mixture could inhibit growth of S. aureus and decrease the minimal inhibitory concentration (MIC) of L. plantarum to 2 mg/mL. The mixture decreased level of both interleukin 6 (IL-6) and interferon-alpha (TNF-α) to a level near the normal uninfected group. Histopathological study showed that animals treated with both L. plantarum and L. inermis had achieved almost 90% healing.Conclusion: These results suggest that L. plantarum and L. inermis mixture has synergistic effect on healing of infected wound.Keywords: Lactiplantibacillus plantarum, Lawsonia inermis, Staphylococcus aureus, wound healing

In recent decades, conflicts between hamadryas baboons (Papio hamadryas) and the rapidly growing human population in the mountainous areas of Western, Southwestern, and Southern Saudi Arabia have accelerated. This conflict, historically occurring mainly between farmers and baboons, has now moved to the urbanized areas in the baboon range and is mainly caused by the common use of spatial and other resources by baboons and humans. The goal of this study was to describe the spatial distribution of baboon groups and to estimate the population size of baboons in the Al-Baha region. The results indicate that baboons are present in all administrative areas of the Al-Baha region with a concentration along the mountain chain running from northwest to southeast and the western part of the region. As expected, rubbish dumps constitute baboon hotspots due to the large amounts of human-derived food. However, the baboons also travel into towns for foraging. The prevention of baboon accessibility to human-derived food would be an important step to reduce causes of conflicts between humans and baboons.

Background: Posterior neck defects are uncommon and are mainly caused by infections or tumors. Consequently, the reconstruction options are limited in the literature. They vary according to the size and type of the defect, and options range from grafts to free flaps. In this article, we present a series of cases where we used a transpositional locoregional flap as a simple and effective way for the coverage of posterior neck defects. Materials and Methods: In a series of 11 patients, we designed locoregional transpositional flaps unilaterally or bilaterally, according to the defect size. Dissection was carried on a subfascial plane. Results: All flaps survived without necrosis. We had two incidents of minimal wound gaping that healed without any intervention. Conclusion: In this series, we introduce a new option and its algorithm to reconstruct moderate-sized posterior neck defects using locoregional transpositional flaps, either unilaterally or bilaterally. It is simple, easy to conduct, and has a better color match and less complication rate than other options mentioned in the literature.

Background:. Hidradenitis suppurativa is a chronic inflammatory condition that affects apocrine gland-bearing areas‚ causing abscesses and sinuses. Multimodality treatment is suggested for management. The surgical option is becoming more widely used, especially in drug-resistant cases. In this article, we describe a series of bilateral axillary hidradenitis cases which we treated with wide local excision and immediate reconstruction with lateral chest flap. Methods:. Fourteen patients presented to our clinic with bilateral hidradenitis suppurativa of the axilla. The cases were all resistant to medical treatment. They were managed by excision and simultaneous reconstruction with lateral chest flaps. Results:. At 3 months postoperatively, all patients had full shoulder range of motion and were completely satisfied with the aesthetic outcome, except for one patient who complained of the bulky look of his axilla. Liposuction was performed for him‚ with a pleasant resultant outcome. Conclusions:. Our patients underwent wide local excision of bilateral disease plus reconstruction with lateral chest flaps in the same session. Our aim was to introduce a treatment option for moderate to severe axillary hidradenitis suppurativa that offers good aesthetic and functional outcomes.

Traumatic arteriovenous fistulas (AVFs) are pathologic conditions caused by abnormal connection between an artery and a nearby vein following trauma to the region. In general AVF is rare in the superficial temporal artery. In this article, we report a case of arteriovenous fistula of Superficial temporal artery which was confirmed with 3D CTA and was treated surgically after six years of beginning.Arteriovenous fistula of Superficial temporal artery (STA) is not common, and the most important cause is trauma which causes a pulsatile mass in the temporal region after undeterminable time.We report here a case of a 12 year old child who came to the hospital seeking medical advice for a mass in his right temporal region that was growing progressively.The patient told us that this mass has appeared after a blunt trauma to his head six years ago.The patient underwent radiological examinations, and arteriovenous fistula of Superficial temporal artery (STA) was diagnosed.He underwent a complete surgical resection of the mass after ligation of the feeding artery and the draining vein.

The present study attempted to examine the effect of using the connectivist approach on developing secondary-stage students' cross-cultural awareness and translation performance. The study comprised thirty-two first-year secondary stage students enrolled in El-Jalawea Institute, Sohag Governorate. The study adopted the quasi-experimental design. Thirty-two participants were randomly assigned to one group, and they were taught through a suggested web-based program. Data were gathered and analysed using both quantitative and qualitative instruments. A pre-post online translation and cross-cultural awareness test, an online questionnaire, and a web-based training program/blog were used in the study. Findings indicated that participants showed significantly higher levels of translation performance samples in the areas of reading comprehension, writing fluency, Computer-assisted Translation (CAT) tool usage, text analysis, and terminology search. Findings also showed significantly higher levels of cross-cultural awareness in both cognitive and affective aspects compared to students' achievement before the treatment. Participants also showed evidence of transition from word-for-word to meaning-for-meaning translation. The development of the students' performance was demonstrated by multiple measures that imply that the suggested web-based program is effective for developing secondary-stage students' translation performance and cross-cultural awareness skills.

الملخــــص مقدمة : تعتبر أم دم الأبهر البطني تهديداً حقيقياً للحياة الأمر الذي يتطلب المراقبة أو العلاج اعتماداً على الحجم أو الأعراض المرافقة، فمن الممكن أن تصل نسبة الوفيات إلى 80% في حال تمزقها، الهدف الأساسي من الدراسة هو تحديد معدل الاختلاطات المباشرة والقريبة للعلاج الجراحي لأم دم الأبهر البطني تحت الشرايين الكلوية والبحث عن عوامل الخطورة عند المرضى التي تؤهب لحدوث مثل هذه الاختلاطات. طرائق البحث : شملت الدراسة 72 مريض (66 ذكور، 6 إناث) أعمارهم تتراوح بين (45-90) سنة } المتوسط = 64.79 { ممن لديهم أم دم أبهر بطني تحت الشرايين الكلوية وأجري لهم عمل جراحي انتخابي لإصلاحها في مشفى تشرين الجامعي في اللاذقية خلال فترة 5 سنوات بين عامي 2017-2021 حيث أجريت دراسة استرجاعية استقبالية على العينة المذكورة. النتائج : تمت متابعة المرضى خلال 30 يوم من الجراحة، وكان عدد الاختلاطات الإجمالي 19 اختلاط توزعت على 12 مريض، وكانت الاختلاطات الرئوية هي الأكثر شيوعاً وحدثت الوفاة عند 6 مرضى. الخلاصة : تشير هذه الدراسة إلى وجود علاقة هامة بين الجراحة الانتخابية وانخفاض معدل المضاعفات والوفيات في أم دم الأبهر البطني تحت الشرايين الكلوية.

إن هدف البحث هو دراسة المضاعفات الموضعية بعد البزل ونسبة حدوثها و عوامل خطورتها و طرق علاجها في مستشفى تشرين الجامعي .

Summary:. Nerve sheath tumors comprise 5% of soft tissue masses of the upper limb in adults. Neurofibromas are divided into three types: localized, diffuse, and plexi- form. The diffuse type is rare and is typically found in the head and neck region. We present a rare case of diffuse type neurofibroma found in the forearm, presented to our clinic as a slowly enlarging mass of the left forearm of 3 years duration. The lesion was suspicious in the magnetic resonance imaging, and biopsy revealed diffuse type neurofibroma. We opted for total excision of the lesion that was found to be not possible due to involvement of the major nerves. The final pathology report showed no malignancy. Nerve tumors of the upper limb can be either benign or malignant. Neurofibroma associated with neurofibromatosis has malignant potential. The diffuse type is rare, and it most commonly occurs in the head and neck region. It has a low malignant transformation rate. Magnetic resonance imaging is the diagnostic modality of choice; however, it can be inconclusive. Biopsy should be taken to confirm the diagnosis and plan for management. Our case was managed by near total excision in order to preserve the major forearm nerves because of high clinical suspicion.

Organizing pneumonia (OP) is a rare inflammatory lung condition affecting bronchioles and alveoli. Symptoms may include cough, fever, malaise, fatigue, and weight loss with a wide variety of radiological manifestations. Both the unspecific symptoms and the radiological manifestations wide spectrum can make the diagnosis of OP challenging in daily practice. A pathological biopsy is a gold standard for diagnosis. However, there is usually a delay in making the diagnosis as a result of the time that is taken to decide to perform the biopsy. We present a case of OP in which its radiological manifestation first was mistaken for a cavity instead of the reversed halo sign (atoll sign) due to the similarity in some radiological features.

Summary:. Upper limb muscle anomalies and their clinical implications have been described frequently in the literature reviews. In this article, we are presenting a case of aberrant forearm muscle that had not been described before, and could be considered as a palmaris longus muscle variation. A 24-year-old man presented to the emergency department, Hamad General Hospital, Doha, Qatar, with right forearm laceration with multiple cut structures for which he was admitted for exploration and repair. Intraoperatively, flexor digitorum superficialis of the third, fourth, and fifth digits, flexor carpi radialis, and palmaris longus were injured‚ and all of them were repaired. We noticed an aberrant muscle—which was also injured—that originated from the distal third of the radius on its medial aspect to insert into the palmar fascia; pulling this muscle’s tendon resulted in tightening of palmar fascia‚ same as the palmaris longus. Along with the importance of deep knowledge of typical human anatomy, hand surgeons must be aware that an aberration from normal anatomy might be anticipated, to provide the best care to our patients.

Summary:. Most cases of hand infections are caused by gram-positive cocci and gram-negative bacilli. Atypical hand infections are rare and are caused by uncommon pathogens like Mycobacterium tuberculosis (TB). Musculoskeletal tuberculosis accounts for 20% of TB cases, and only 2% of these cases involve the hand and foot. In this article, we describe a case of hand TB that had spread from the thenar space to the space of Parona. The patient was treated with a combined surgical and medical approach. A 29-year-old male patient presented to our clinic with the complaint of pain and swelling of the right hand that had been worsening for 4 months before presentation. It was associated with mild weakness and night sweats. On physical examination, he had two separate swellings: one at the thenar eminence and one at the volar side of the wrist. He was taken for incision, which showed caseous material on both sites. It was sent for culture that revealed mycobacterium tuberculosis. The patient was placed on anti-TB medications for a total of 6 months. Tuberculous infection of the hand is a rare condition; the most common musculoskeletal sites are the spine, hip, and knee. Early diagnosis and treatment of TB infection in hand are essential for retaining optimal function. The diagnosis usually depends on the clinical presentation supported by laboratory tests and imaging studies. Treatment consists of medical, surgical, or combined treatment. Surgical intervention should aim to remove all the infected material in addition to debridement of the involved tissues.
