Abstract In regions characterized by a high concentration of small reservoirs, there is often public debate about the effectiveness of these structures in locally adapting to and mitigating drought impacts, bearing in mind their potential to modify or induce drought events in downstream areas. In this study, we investigated the influence of a Dense Network of Small Reservoirs (DNR) on the emergence and intensification of drought impacts at catchment scale, as well as their local social benefits. This analysis was based on the Socio‐Hydrological‐Agricultural‐Reservoir (SHARE) model, specially developed for this purpose, with a medium‐sized catchment in the semi‐arid region of Brazil as a case study. We identified that, while a DNR can prolong the effects of a hydrological drought on storage in a large strategic reservoir at the catchment outlet by obstructing surface‐runoff connectivity, it plays a crucial role in mitigating drought impacts at a local level. Specifically, the presence of small reservoirs has the potential to boost local agricultural production by up to 5 times compared to scenarios without these structures. In addition, our simulation results suggest there is a notable reduction in the need for emergency water distribution by water trucks in the presence of a DNR. This study highlights the need for a balanced approach to implementing public policies, weighing the local benefits of small reservoirs against the possible downstream impacts on large reservoirs.

Abstract Drought management is currently informed by a variety of approaches, mostly responding to drought crisis when it happens. Toward more effective and integrated drought management, we introduce a conceptual drought diagnosis framework inspired by diagnostic concepts from the field of medicine. This framework comprises five steps: 1. Initial diagnostic assessment; 2. Diagnostic testing; 3. Consultation; 4. Communication of the diagnosis; and 5. Treatment and prognosis. To illustrate the need for the proposed approach, four case studies of recently drought‐affected regions were selected: the city of Cape Town, the state of California, the Northeast region of Brazil, and the Horn of Africa. Contrasting elements for these cases include the geographic extent and political boundaries, climate, socio‐economics, and the relevance of different water resources (e.g., rainfall, reservoirs, and aquifers). For each case, we identified documented practices and policies and reflected on them in terms of drought misdiagnosis or incomplete diagnosis that have aggravated socio‐economic and environmental drought impacts. A common example is the preference for technical solutions (e.g., installing infrastructure to augment water supply), rather than measures that reduce vulnerability. Analysis of these four drought case studies confirmed the anticipated need for a comprehensive approach to drought diagnosis for more successful treatment and prevention of drought. Using an analogy with medical science can be helpful toward comprehensively diagnosing droughts for a variety of contexts and assessing the effectiveness of proposed interventions. This framework can help drought managers to be more proactive in enabling drought‐affected regions to become more drought resilient in the future.

Recently, many studies regarding consumer perception of cell-based meat have been published. However, the opinion of the professionals involved in animal production also seems relevant. In particular, veterinarians and animal scientists may be important players in the new cell-based meat production, acting as proponents or barriers to this major improvement for farm animal welfare. Therefore, our aim is to analyse the knowledge and perspective of Brazilian veterinarians and animal scientists regarding cell-based meat. Veterinarians (76.8%; 209/272) and animal scientists (23.2%; 63/272) responded to an online survey. Logistic regression, latent class and logit models were used to evaluate objective answers, and the Discourse of the Collective Subject method was used to interpret open-ended answers. Specialists who were women (62.5%; 170/272), veterinarians (76.8%; 209/272), vegetarians (7.0%; 19/272) and vegans (1.1%; 3/272) were more supportive of cell-based meat. Lack of knowledge and the connection with artificiality, the most frequent spontaneous word associated with cell-based meat by all respondents, were the main negative points highlighted. Thus, it seems fundamental to offer higher education to veterinarians and animal scientists regarding cell-based meat, since engaging them with this novel technology may mitigate both the resistance and its negative consequences for the professionals, society, the animals involved and the environment.

Human actions induce and modify droughts. Yet, there remain scientific gaps regarding how anthropogenic dynamics and hydrological processes are intrinsically entangled in drought evolution. This poses the challenge of developing ways to evaluate human behavior and its pattern of co-evolution with the hydrological cycle, mainly related to water use and landscape modifications. We propose that prospect theory explains the emergence of drought impacts, such as crop losses and water shortage, if they are considered as failed welfare expectations ("prospects") due to water shortage. This behavioral economic theory is dominantly applied to explain decision-making processes under uncertainty. We argue that it can also contribute to explaining socio20 hydrological phenomena such as reservoir effects. Abstract Backgrounds Laparoscopic surgery is widely used in abdominal emergency surgery (AES), and the possibility of extending this approach to the more recent robotic surgery (RS) arouses great interest. The slow diffusion of robotic technology mainly due to high costs and the longer RS operative time when compared to laparoscopy may represent disincentives, especially in AES. This study aims to report our experience in the use of RS in AES assessing its safety and feasibility, with particular focus on intra- and post-operative complications, conversion rate, and surgical learning curve. Our data were also compared to other experiences though an extensive literature review. Methods We retrospectively analysed a single surgeon series of the last 10 years. From January 2014 to December 2023, 36 patients underwent urgent or emergency RS. The robotic devices used were Da Vinci Si (15 cases) and Xi (21 cases). Results 36 (4.3%) out of 834 robotic procedures were included in our analysis: 20 (56.56%) females. The mean age was 63 years and 30% of patients were ≥ 70 years. 2 (5.55%) procedures were performed at night. No conversions to open were reported in this series. According to the Clavien-Dindo classification, 2 (5.5%) major complications were collected. Intraoperative and 30-day mortality were 0%. Conclusions Our study demonstrates that RS may be a useful and reliable approach also to AES and intraoperative laparoscopic complications when performed in selected hemodynamically stable patients in very well-trained robotic centers. The technology may increase the minimally invasive use and conversion rate in emergent settings in a completely robotic or hybrid approach.

Abstract Ischemia with non-obstructive coronary artery (INOCA) is a common cause of hospital admissions, leading to negative outcomes and reduced quality of life. Central to its pathophysiology is endothelial dysfunction, which contributes to myocardial ischemia despite the absence of significant coronary artery blockage. Addressing endothelial dysfunction is essential in managing INOCA to alleviate symptoms and prevent cardiovascular events. Recent studies have identified diabetes mellitus (DM) as a significant factor exacerbating INOCA complications by promoting endothelial impairment and coronary microvascular dysfunction. MicroRNAs (miRNAs) have emerged as potential biomarkers and therapeutic targets in various biological processes, including endothelial dysfunction and cardiovascular diseases. However, research on miRNA biomarkers in INOCA patients is sparse. In this study, we examined a panel of circulating miRNAs involved in the regulation of endothelial function in INOCA patients with and without DM. We analyzed miRNA expression using RT-qPCR in a cohort of consecutive INOCA patients undergoing percutaneous coronary intervention. We detected a significant dysregulation of miR-363-5p and miR-92a-3p in INOCA patients with DM compared to those without DM, indicating their role as biomarkers for predicting and monitoring endothelial dysfunction in INOCA patients with DM.

Abstract Background Hypertension and chronic kidney disease (CKD) pose significant public health challenges, sharing intertwined pathophysiological mechanisms. Prediabetes is recognized as a precursor to diabetes and is often accompanied by cardiovascular comorbidities such as hypertension, elevating the risk of pre-frailty and frailty. Albuminuria is a hallmark of organ damage in hypertension amplifying the risk of pre-frailty, frailty, and cognitive decline in older adults. We explored the association between albuminuria and cognitive impairment in frail older adults with prediabetes and CKD, assessing cognitive levels based on estimated glomerular filtration rate (eGFR). Methods We conducted a study involving consecutive frail older patients with hypertension recruited from March 2021 to March 2023 at the ASL (local health unit of the Italian Ministry of Health) of Avellino, Italy, followed up after three months. Inclusion criteria comprised age over 65 years, prior diagnosis of hypertension without secondary causes, prediabetes, frailty status, Montreal Cognitive Assessment (MoCA) score 15 ml/min. Abstract Objective This study aims to present an adaptation and evidence of construct validity of the Varieties of Sadistic Tendencies for a sample in the Brazilian context. Method To this end, 260 participants from different states, with ages ranging from 18 to 64 years (M = 27.91; SD = 8.96), answered three measurement instruments: (a) a version of the Varieties of Sadistic Tendencies, adapted to the Brazilian context through back translation; (b) the Dark Triad Dirty Dozen and; (c) a Demographic Questionnaire. Results The answers were submitted to Exploratory Factor Analysis using Factor v10.10.03, which indicated a bifactor structure with 16 items. In addition, Spearman's rho correlations were performed among the factors to identify evidence of validity with other variables, which indicated weak and moderate relationships. Conclusion As an initial study, the evidence of validity reported here points to psychometric adequacy.

IntroductionLung carcinoids (LCs) are a type of neuroendocrine tumor (NET) that originate in the bronchopulmonary tract. LCs account for 20–25% of all NETs and approximately 1–2% of lung cancers. Given the highly vascularized nature of NETs and their tendency to overexpress vascular growth factor receptors (VEGFR), inhibiting angiogenesis appears as a potential therapeutic target in slowing down tumor growth and spread. This study evaluated the long-term antitumor activity and related mechanisms of axitinib (AXI), a VEGFR-targeting drug, in LC cell lines.MethodsThree LC cell lines (NCI-H727, UMC-11 and NCI-H835) were incubated with their respective EC50 AXI concentrations for 6 days. At the end of the incubation, FACS experiments and Western blot analyses were performed to examine changes in the cell cycle and the activation of apoptosis. Microscopy analyses were added to describe the mechanisms of senescence and mitotic catastrophe when present.ResultsThe primary effect of AXI on LC cell lines is to arrest tumor growth through an indirect DNA damage. Notably, AXI triggers this response in diverse manners among the cell lines, such as inducing senescence or mitotic catastrophe. The drug seems to lose its efficacy when the DNA damage is mitigated, as observed in NCI-H835 cells.ConclusionThe ability of AXI to affect cell viability and proliferation in LC tumor cells highlights its potential as a therapeutic agent. The role of DNA damage and the consequent activation of senescence seem to be a prerequisite for AXI to exert its function.

Background: Muscular strength and muscle mass are considered key factors for healthy ageing. Modification of body composition and redistribution of adipose tissue has been described in advanced age. Muscle strength has an important predictive role for health outcomes. However, little is known regarding the relationship between muscle strength and epicardial fat. Methods and materials: In a cohort of healthy adults following physical capacity evaluations, anthropometric measurements, handgrip strength (HGS), echocardiography and bioimpedance analysis (BIA) were performed.
