Aim: To investigate potential gender-related differences in the utilization of scope of practice (range of duties and responsibilities) across acute, community and long-term geriatric-care settings, by considering activities authorized for all registered nurses and those restricted to advanced post-basic certification graduates. Design: A cross-sectional approach, using a convenience sample. Methods: A total of 286 registered nurses [207 females (72.4%) and 79 males (27.6%)] with advanced geriatric training, completed a self-administered questionnaire addressing socio-demographic and occupational characteristics, utilization of the scope of practice, and professional satisfaction. Chi square, two-sample t-test, and a multiple regression model were used to analyze the data. Results: Male nurses reported implementation of a statistically significantly broader scope of practice than female nurses, outperforming them in 8 out of 16 activities for all registered nurses and in 5 out of 9 activities for advanced nurses with post-basic certification. Conclusion: Our findings suggest that gender-related differences in the utilization of nursing scope of practice exist, with implications for professional satisfaction and potentially for patient care outcomes. Our results highlight the need to address these differences at multiple levels, ranging from healthcare organizations, and educational institutions, to policy-making bodies. Future research should use longitudinal studies with diverse samples from various settings and include factors like organizational culture, time since certification, and patient demographics in order to better understand any influences on nursing practice. Older and younger adults are offered similar analgesic options after hemorrhoid surgery (HS), but the differences in pain between the two populations are unknown. This study aims to compare postoperative pain outcomes after HS in older and younger individuals. This is a retrospective analysis of electronic medical records of patients who underwent HS between 2018 and 2023. Patients were excluded if additional anorectal procedures were performed at the time of HS. Data related to pain-related outcomes were compiled: (1) need for narcotic prescription refills; (2) documentation of a pain-related phone call within 30 d; (3) urgent postoperative office visit before regular scheduled follow-up; and (4) pain-related postoperative emergency department visits. Associations between age and pain-related outcomes were tested using Fisher's exact test, chi-square test, and covariate adjusted logistic regression modeling. There were a total of 249 patients, 60 older adults, and 189 younger adults. Compared to younger patients, older adults demonstrated a reduced frequency of pain-related phone calls (10.3 versus 32.1%, P < 0.01) and opioid refills (0 versus 14.4%, P < 0.01). After adjusting for confounders, older age remained inversely associated with pain-related postoperative phone calls (odds ratio = 0.25, 95% confidence interval = [0.1-0.6], P = 0.003). Older adults had better pain outcomes after HS in comparison to younger patients. These findings suggest that the postoperative analgesic needs of older patients after HS are lower than those of younger patients. Objectives: Current policies for older patients do not adequately address the barriers to effective implementation of optimal care models in New Zealand, partly due to differences in patient definitions and the in-patient pathway they should follow through hospital. This research aims to: (a) synthesise a definition of a complex older patient; (b) identify and explore primary and secondary health measures; and (c) identify the primary components of a care model suitable for a tertiary hospital in the midland region of the North Island of New Zealand. Method: This mixed-methods study utilised a convergence model, in which qualitative and quantitative data were investigated separately and then combined for interpretation. Semi-structured interviews (n=11) were analysed using a general inductive method of enquiry to develop key codes, categories and themes. Univariate data analysis was employed using six years of routinely collected data of patients admitted to the emergency department and inpatient units (n=261,773) of the tertiary hospital. Results: A definition of a complex older patient was determined that incorporates chronic conditions, comorbidities and iatrogenic complications, functional decline, activities of daily living, case fatality, mortality, hospital length of stay, hospital costs, discharge destination, hospital readmission and emergency department revisit and age – not necessarily over 65 years old. Well-performing geriatric care models were found to include patient-centred care, frequent medical review, early rehabilitation, early discharge planning, a prepared environment and multidisciplinary teams. Conclusions: The findings of this New Zealand study increase understanding of acute geriatric care for complex older patients by filling a gap in policies and strategies, identifying potential components of an optimal care model and defining a complex geriatric patient. Implications for Public Health: The findings of this study present actionable opportunities for clinicians, managers, academics and policymakers to better understand a complex older patient in New Zealand, with significant relevance also for international geriatric care and to establish an effective acute geriatric care model that leads to beneficial health outcomes and provides safeguard mechanisms. The care of the aged becomes a critical priority in the ever-changing field of healthcare, requiring a specialized, and sensitive approach. This population is at the intersection of medical innovation and ethical care, as they are susceptible to the compounding effects of various drugs and complex health issues. In this regard, the Clinical Pharmacological Reconciliation, Review, and Feedback methodology is a trailblazing approach that seeks to improve and optimize care for the aged by applying the concepts of clinical pharmacology. This review explores few facets of this transformative approach.

Background With the aging population, research on elderly care has increasingly become a focal point. Previous surveys on the demand rate for elderly care have shown significant variation (8.54% to 53.15%) and lack large-scale study evidence. Objective To understand the prevalence and influencing factors among community-dwelling older people in three provinces of China. Methods The study subjects were from the baseline survey of Prevention and Intervention on Neurodegenerative Disease for Elderly in China conducted in 2019. This survey employed a multi-stage stratified cluster random sampling method, selecting a total of 16 199 elderly individuals aged 60 and above from 16 districts and counties across Liaoning, Henan, and Guangdong provinces. The questionnaire covered basic demographic characteristics (gender, age, urban/rural residence, marital status, education level, occupation, weight, living situation), current care needs, chronic disease status, the Patient Health Questionnaire (PHQ-9), the Mini-Mental State Examination (MMSE), the Activities of Daily Living (ADL) scale, self-rated health status, daily exercise habits, and the number of falls. The PHQ-9 was used to assess the psychological health of the elderly, the MMSE was used to evaluate cognitive function, and the ADL scale or self-reported need for care was used to assess elderly care needs, defined as the presence of BADL or IADL disabilities, or a self-reported need for care. Logistic regression analysis was used to explore the influencing factors of elderly care needs. Results The prevalence of care needs among community-dwelling older people in three provinces was 14.57% (95%CI=14.02%-15.11%), and the prevalence of care needs increased gradually with age, consistently higher among females than males in all age groups. The results of multifactorial analysis showed that the risk of care need increased 7% (OR=1.07, 95%CI=1.06-1.08) for each additional year of age. Compared with illiteracy, the ORs of care need for those with primary school, secondary school, high school and above were 0.33 (95%CI=0.29-0.37), 0.24 (95%CI=0.20-0.28), 0.17 (95%CI=0.12-0.22), respectively. Compared with daily exercise, the OR of care needs for those who did not exercise daily was 1.17 (95%CI=1.05-1.30) .Compared with those who did not suffer from chronic diseases, the ORs of care needs for those who suffered from 1-2 kinds and those 3 or more kinds of chronic diseases were 1.29 (95%CI=1.10-1.51) and 1.57 (95%CI=1.35-1.82). Compared with normal cognitive function, the OR of care need for those with abnormal cognitive function was 2.02 (95%CI=1.79-2.27). Compared with good self-assessed health status, the OR of care need for those with fair health status and those with poor health status were 1.29 (95%CI=1.14-1.46) and 2.68 (95%CI=2.27-3.16). Compared with those did not having fallen, ORs of care needs for those with 1-2 and 3 or more falls were 1.23 (95%CI=1.06-1.43) and 2.00 (95%CI=1.59-2.52). ORs of care needs for those with mild and moderately severe depression compared to those with a good mental status were 1.14 (95%CI=1.21-1.65) and 2.05 (95%CI=1.69-2.48) . Conclusion The demand for elderly care among community-dwelling older adults of China is notably high. This demand is particularly elevated among individuals who are older, have poorer physical and psychological health, and have experienced a higher frequency of falls.

Abstract Currently, exchange of information between the geriatric clinic and the attending general practitioner (GP) occurs primarily through the doctor's letter after discharging from the clinic. The aim of our study was to reduce readmissions of multimorbid, geriatric patients to the clinic by establishing a new form of care via an electronic case file (ECF) and a consultation service (CS). The discharging geriatric clinic filled out an online ECF. The patient's GP should document quarterly follow-ups in the ECF. The case file was monitored by the discharging clinic due to a consultation service. The primary efficacy endpoint was the rehospitalization rate within one year. The hospitalization rate for patients managed in the project was 83.1/100 person years (PY), while the control group from insurance data had a rate of 69.0/100 PY. The primary endpoint did not show a statistically significant difference (p = 0.15). A total of 195 contacts were documented via CS for 171 participants, mostly initiated by the clinics. The clinical queries primarily concerned drug therapy. The Covid pandemic had an overall impact on hospitalizations. There are many approaches to reducing hospital readmissions after discharge of older patients. Supporting the transition from inpatient to outpatient care by different professional groups or care systems has been shown to have a positive effect. Furthermore, the utilisation of an ECF can also be beneficial in this regard.

Objectives Aim: The purpose of the research is the diagnosis of competencies of people holding managerial positions in the geriatric care sector in Germany. Material and methods Scheitler's concept of competency was employed in this study. The method of in-depth unstructured interviews was also used. The method of qualitative content analysis according to Mayring was applied to evaluate the obtained data. Results As a result of the conducted analyses, professional tasks and competencies of people holding managerial positions in the geriatric care sector in Germany were presented. Conclusions The research results showed that the tasks of people performing managerial functions in geriatric care in Germany are complex and exceed medical or caregiving tasks. They are related to personnel management, responding to their needs, as well as the needs and expectations of people managing medical facilities. An extremely important task is cooperation with the relatives of the elderly. The research results also revealed the importance of having a wide variety of competencies. The respondents were of the opinion that professional skills are less important and social and personal skills are gaining in importance.

Abstract Background Population aging is forcing the transformation of health care. Long-term care in the home is complex and involves complex communication with primary care services. In this scenario, the expansion of digital health has the potential to improve access to home-based primary care; however, the use of technologies can increase inequalities in access to health for an important part of the population. The aim of this study was to identify and map the uses and types of digital health interventions and their impacts on the quality of home-based primary care for older adults. Methods This is a broad and systematized scoping review with rigorous synthesis of knowledge directed by the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR). The quantitative data were analyzed through descriptive statistics, and the qualitative data were analyzed through basic qualitative content analysis, considering the organizational, relational, interpersonal and technical dimensions of care. The preliminary results were subjected to consultation with stakeholders to identify strengths and limitations, as well as potential forms of socialization. Results The mapping showed the distribution of publications in 18 countries and in the Sub-Saharan Africa region. Older adults have benefited from the use of different digital health strategies; however, this review also addresses limitations and challenges, such as the need for digital literacy and technological infrastructure. In addition to the impacts of technologies on the quality of health care. Conclusions The review gathered priority themes for the equitable implementation of digital health, such as access to home caregivers and digital tools, importance of digital literacy and involvement of patients and their caregivers in health decisions and design of technologies, which must be prioritized to overcome limitations and challenges, focusing on improving quality of life, shorter hospitalization time and autonomy of older adults.

One reason is the lack of availability of a suitable assessment tool that is not strongly influenced by social desirability. Here, we present and evaluate a new item for the assessment of relationship quality (RQ) in the context of informal caregiving of older people. Methods N = 962 informal caregivers of older people participated. Our item assessed RQ by providing three answer categories (positive, neutral, and negative) that were presented through the use of smiley faces. Ikigai, a Japanese term often referred to as “purpose in life” or “the feeling that life is worthwhile”, is a commonly used indicator of well-being in studies focused on the elderly population in Japan. It can be identified within the different realms like family, work, and friendship. However, as individuals age, their sense of ikigai is prone to diminish. Teachable agents (TAs) have been employed in educational settings for decades to facilitate learning by teaching students. Previous research has indicated that TAs can have a positive impact on self-esteem which is an important aspect of ikigai. TAs may hold the potential to address issues related to declining ikigai that are often associated with aging. By engaging in the act of teaching the agent, elderly individuals may experience a sense of being needed, consequently enhancing their self-esteem and potentially leading to an improvement in their ikigai levels. In this paper, we introduce a TA designed around the concept of ikigai, aiming at assisting the elderly in maintaining a high level of ikigai to support healthy aging. Based on a user study conducted using the phenomenographical approach, we demonstrate the effectiveness of our proposed TA design.

Aims Methods and results Conclusion Although the prevalence of heart failure (HF) increases markedly with advancing age, surprisingly little is known about HF in the very elderly. The aim of this study was to describe the clinical characteristics and outcomes of octogenarians with HF.Individual participant meta‐analysis of patients with HF and reduced, mildly reduced, and preserved ejection fraction (HFrEF, HFmrEF, and HFpEF, respectively) enrolled in eight large randomized trials. Overall, the proportion of octogenarians was 1518 of 20 168 patients (7.5%) with HFrEF, 610 of 4609 (13.2%) with HFmrEF, and 3130 of 15 354 (20.4%) with HFpEF. Regardless of HF phenotype, octogenarian patients were more often female and had more comorbidities, more symptoms and signs of congestion, and worse health status (but not quality of life), in comparison to patients aged <80 years. The incidence (per 100 person‐years) of the composite of cardiovascular death or HF hospitalization was 13.3 (95% confidence interval [CI] 12.7–14.0) in octogenarians versus 9.5 (95% CI 9.3–9.7) in non‐octogenarians (adjusted hazard ratio [aHR] 1.40, 95% CI 1.32–1.48). Each component of the composite was more frequent in octogenarians with rates of cardiovascular mortality of 7.0 (95% CI 6.5–7.4) per 100 person‐years versus 4.9 (95% CI 4.8–5.1) in non‐octogenarians (aHR 1.60, 95% CI 1.48–1.72, p < 0.001). Octogenarians received less evidence‐based therapy, especially mineralocorticoid receptor antagonists, than younger patients.Despite worse health status and higher hospitalization and mortality rates, octogenarians were undertreated compared to younger patients. AbstractThis qualitative study explores how geriatric inpatients (n = 103) experience a one-on-one chaplaincy visit. Although some patients expressed neutral or negative feelings about the chaplaincy visit, the large majority spoke in a very positive way about the chaplain’s care. Using thematic analysis, we identified four main themes characterizing patients’ experiences with chaplaincy visits. (1) Patients experienced the chaplaincy visit as a place to tell personal stories and express their religious beliefs. (2) They had a safe and trusting relationship with the chaplain. (3) The interaction with the chaplain brought new perspectives. (4) After the visit, they felt satisfied, relieved, peaceful, and strong. This study demonstrates that the experiences of geriatric patients contribute to a better understanding of the value of chaplaincy care. Integrating patients’ perspectives on chaplaincy care is recommended, both in future research and healthcare chaplaincy. [ABSTRACT FROM AUTHOR]

Vasculitides are a heterogeneous group of disorders producing inflammation of blood vessels (e.g. arteries or veins). All major vasculitides potentially have ophthalmological symptoms and signs including visual loss. Co-morbidity, multimorbidity, polypharmacy, and geriatric syndromes all play important roles in patient outcomes for these rheumatic conditions in the elderly. This monograph reviews the NCBI PubMed database (Feb 2023) literature on the neuro-ophthalmic and geriatric considerations in vasculitis. Cogan Syndrome, Granulomatosis with Polyangiitis, Giant Cell Arteritis, Polyarteritis Nodosa, Takayasu Arteritis, Vasculitis epidemiology, and neuro-ophthalmological symptoms. Geriatric patient care for vasculitis with neuro-ophthalmological manifestations can be complicated by the interplay of multiple co-morbidities, polypharmacy, and specific geriatric syndromes. The valuation and treatment of vasculitis and the complications associated with the disease can negatively impact patient care. Advances in noninvasive imaging and updates in diagnostic criteria have enabled increased identification of patients at earlier stages with less severe disease burden. Novel therapeutic agents can be glucocorticoid sparing and might reduce the adverse effects of chronic steroid use. Holistic care models like the 5 M geriatric care model (mind, mobility, medications, multicomplexity, and matters most) allow patients' needs to be in the forefront with biopsychosocial aspects of a patient being addressed. Conclusions: We conclude that our item is well-suited for the assessment of RQ in the context of informal caregiving of older people. Because it uses language-free answer categories by means of smiley faces, our item can be applied easily. Bias due to social desirability can be minimized by dichotomization (i.e., combining the negative and neural answer categories). In future research, our tool should be evaluated in other contexts. The present non-randomized clinical trial examined the short-term outcomes of one-on-one chaplaincy interventions with 416 geriatric patients in Belgium. Participants were interviewed one or two days before a potential chaplaincy intervention (baseline measurement), and one or two days after a potential intervention (post-measurement). Patients in the non-randomized intervention group received an intervention by the chaplain, while the non-randomized comparison group did not. Patients in the intervention group showed a significant decrease in state anxiety and negative affect, and a significant improvement in levels of hope, positive affect, peace, and Scottish PROM-scores, compared to the comparison group. Levels of meaning in life and faith did not significantly change after the chaplaincy intervention. This study suggests that geriatric patients may benefit from chaplaincy care and recommends the integration of chaplaincy care into the care for older adults. [ABSTRACT FROM AUTHOR]

Background: Improving the work ability of nurses is highly important for ensuring a functioning healthcare system, which can be achieved via exercising and enhancing ergonomic work behaviors of the nurses. The objective of this systematic review was to synthesize work ability-enhancing exercise and ergonomic workplace health promotion interventions in different nursing settings and their effects on work ability. Methods: Intervention studies incorporating at least one component of physical exercise or ergonomic work behavior for nurses were eligible for inclusion. Three electronic databases (Medline, PsycInfo, CINAHL) were searched for studies published until October 2022. The methodological quality of interventions was assessed. Due to intervention and outcome heterogeneity, narrative synthesis was conducted. Results: The search identified a total of 37 studies. Sample size ranged from 14 to 316 (N = 3487). Overall quality was moderate (M = 0.76). Programs comprised aerobic exercising, "back school," ergonomics, increasing physical activity, multicomponent programs, physiotherapy, and yoga. Positive effects on work ability (e.g., reduced back pain or stress levels) were reported in 34 studies. However, if follow-up analyses were included, the effects typically diminished. Common training principles and training control mechanisms were applied in studies. Discussion: The review indicates the efficacy of exercise-based health promotion for increasing nurses' work ability. However, results are limited by the lack of long-term improvements and scarcity of application of training principles. Abstract Background The progressive aging of the population and the increasing complexity of health issues contribute to a growing number of older individuals seeking emergency care. This study aims to assess the state of the art of care provided to older people in the Emergency Departments of Lombardy, the most populous region in Italy, counting over 2 million people aged 65 years and older. Methods An online cross-sectional survey was developed and disseminated among emergency medicine physicians and physicians affiliated to the Lombardy section of the Italian Society of Geriatrics and Gerontology (SIGG), during June and July 2023. The questionnaire covered hospital profiles, geriatric consultation practices, risk assessment tools, discharge processes and perspectives on geriatric emergency care. Results In this mixed method research, 219 structured interviews were collected. The majority of physicians were employed in hospitals, with 54.7% being geriatricians. Critical gaps in older patient’s care were identified, including the absence of dedicated care pathways, insufficient awareness of screening tools, and a need for enhanced professional training. Conclusions Tailored protocols and geriatric educational programs are crucial for improving the quality of emergency care provided to older individuals. These measures might also help relieve the burden on the Emergency Departments, thereby potentially enhancing overall efficiency and ensuring better outcomes.

Abstract Background Early discharge of frail older adults from post-acute care service may result in individuals’ reduced functional ability to carry out activities of daily living, and social, emotional, and psychological distress. To address these shortcomings, the Montreal West Island Integrated University Health and Social Services Centre in Quebec, Canada piloted a post-acute home physiotherapy program (PAHP) to facilitate the transition of older adults from the hospital to their home. This study aimed to evaluate: (1) the implementation fidelity of the PAHP program; (2) its impact on the functional independence, physical and mental health outcomes and quality of life of older adults who underwent this program (3) its potential adverse events, and (4) to identify the physical, psychological, and mental health care needs of older adults following their discharge at home. Methods A quasi-experimental uncontrolled design with repeated measures was conducted between April 1st, 2021 and December 31st, 2021. Implementation fidelity was assessed using three process indicators: delay between referral to and receipt of the PAHP program, frequency of PAHP interventions per week and program duration in weeks. A battery of functional outcome measures, including the Functional Independence Measure (FIM) and the Patient-Reported Outcomes Measurement Information System (PROMIS) Global-10 scale, as well as fall incidence, emergency visits, and hospitalizations were used to assess program impact and adverse events. The Patient’s Global Impression of Change (PGICS) was used to determine changes in participants’ perceptions of their level of improvement/deterioration. In addition, the Camberwell Assessment of Need for the Elderly (CANE) questionnaire was administered to determine the met and unmet needs of older adults. Results Twenty-four individuals (aged 60.8 to 94 years) participated in the PAHP program. Implementation fidelity was low in regards with delay between referral and receipt of the program, intensity of interventions, and total program duration. Repeated measures ANOVA revealed significant improvement in FIM scores between admission and discharge from the PAHP program and between admission and the 3-month follow-up. Participants also reported meaningful improvements in PGICS scores. However, no significant differences were observed on the physical or mental health T-scores of the PROMIS Global-10 scale, in adverse events related to the PAHP program, or in the overall unmet needs. Conclusion Findings from an initial sample undergoing a PAHP program suggest that despite a low implementation fidelity of the program, functional independence outcomes and patients’ global impression of change have improved. Results will help develop a stakeholder-driven action plan to improve this program. A future study with a larger sample size is currently being planned to evaluate the overall impact of this program. Abstract Background The oral health of older people is closely related to their overall health. Timely and effective intervention in oral issues is necessary to maintain their overall health. This study aimed to evaluate the feasibility and effectiveness of an Oral Health Promotion Program (OHPP) in Geriatric Care Facilities (GCFs). Methods The OHPP was implemented in two GCFs and evaluated using a pre/post-design. Questionnaires on self-efficacy and attitude for providing oral care were sent to 42 nurse participants before and three months after the implementation of the OHPP. Outcomes of 295 patient participants were assessed at four time points (T1-baseline, T2-one month, T3-two months, and T4-three months post-implementation) including Activities of Daily Living (ADL), Mini-Mental State Examination (MMSE), and Oral Health Assessment Tool (OHAT). Results The oral health and daily activity ability of patient participants showed an improving trend at four time points pre/post-implementation of the OHPP. Drawing on data from ethnographic fieldwork and interprofessional focus group discussions, this study enquires into staff’s everyday life on a geriatric ward to explore and understand conditions for engaging in narrative relations in in-patient geriatric care. Avoiding individualistic understandings of narrative practices, we applied a narrative-in-action methodology built on a relational understanding of narrativity, where individual narratives are not separated from social and cultural features. This helped us explore how individual interpretations of the conditions for everyday practices come together with broader social or cultural understandings to gain situated insights about how these are continuously related and reformed by one another in everyday situations of geriatric care. The findings offer insights into the opportunities to engage in narrative relations based on how healthcare staff on a geriatric ward interpret conditions for their practices, and how they act based on such interpretations. While some interpretations were associated with attitudes and activities encouraging narrative relations, others simultaneously thwarted narrative relations by enacting task-orientation, division, or a focus on measurable biomedical or function-related outcomes. Moreover, the findings suggest and discuss consequences of the tensions created as interpretations are enacted in everyday healthcare situations, thus questioning assumptions about conditions as something static and linear.

Purpose The use of narration in healthcare has been accentuated as a response to the requested shift towards person-centred care. The notion of narrative relations refers to a process of involving several people in mutual and ongoing narrative exchange. This study aimed to explore how and where narrative relations may be adopted and enacted in everyday healthcare practice. Methods The study has a qualitative, explorative design. Seven interprofessional focus group discussions with healthcare staff were prompted by vignettes. A multidisciplinary team of healthcare staff (n = 31) were recruited on a geriatric ward. Data were analysed using a constant comparative method. Results A core theme shows how narrative relations are adopted and enacted both as part of an approved practice—the work procedures commonly approved as part of healthcare, and as a disregarded practice where covert but important narrative relations take place to support fundamental qualities of healthcare. Moreover, the findings consider arenas of healthcare practice where approved or disregarded practices are enacted in the clinic frontstage and the clinic backstage. Conclusions Narrative relations may take place in different arenas of healthcare practice yet simultaneously become a cohesive force interconnecting those arenas and uphold continuity. Impeded narrative relations in one arena may have unintended consequences in another.

Background: The Geriatric Interprofessional Team Transformation for Primary Care (GITT-PC) is a model developed to deliver optimal care to older adults in primary care. GITT-PC is an expansion of the John A. Hartford Foundation Geriatric Interdisciplinary Team Training (GITT) program developed at New York University and funded from 1995 to 2002 (Fulmer et al., 2004). GITT was designed to create training models that reflect the needs of the changing health care system and the challenge of caring for older adults with complex conditions (Fulmer et al., 2005). The GITT-PC model builds on the lessons learned from GITT and the development of curricula and training materials based on best practices. Methods: Implementation of GITT-PC is accomplished through systems and practices that meet the needs and preferences of patients and their families and that are implemented by teams of health professionals and community service providers. GITT-PC is focused on four core components of high-quality geriatric care: (1) health promotion and prevention, (2) chronic disease management, (3) advanced care planning, and (4) transitional care management, each component corresponding to a Medicare-reimbursable visit. Results: Implementation of these reimbursable services enables practices to provide evidence-based geriatric care while realizing a potential significant return on investment. Conclusions: The GITT-PC model has evolved from an academic training program to a financially sustainable model that serves to improve the care of older adults through a systematic team transformation process that makes a clear business case for primary care (Tabbush et al., 2021). Background: Opioids are recommended for pain management in patients being cared for and transported by emergency medical services, but no specific guidelines exist for older adults with fall‐related injury. Prior research suggests prehospital opioid administration can effectively manage pain in older adults, but less is known about safety in this population. We compared short‐term safety outcomes, including delirium, disposition, and length of stay, among older adults with fall‐related injury according to whether they received prehospital opioid analgesia. Methods: We linked Medicare claims data with prehospital patient care reports for older adults (≥65) with fall‐related injury in Illinois between January 1, 2014 and December 31, 2015. We used weighted regression models (logistic, multinomial logistic, and Poisson) to assess the association between prehospital opioid analgesia and incidence of inpatient delirium, hospital disposition, and length of stay. Results: Of 28,150 included older adults, 3% received prehospital opioids. Patients receiving prehospital opioids (vs. no prehospital opioids) were less likely to be discharged home from the emergency department (adjusted probability = 0.30 [95% CI: 0.25, 0.34] vs. 0.47 [95% CI: 0.46, 0.48]), more likely to be discharged to a non‐home setting after an inpatient admission (adjusted probability = 0.43 [95% CI: 0.39, 0.48] vs. 0.30 [95% CI: 0.30, 0.31]), had inpatient length of stay 0.4 days shorter (p < 0.001) and ICU length of stay 0.7 days shorter (p = 0.045). Incidence of delirium did not vary between treatment and control groups. Conclusions: Few older adults receive opioid analgesia in the prehospital setting. Prehospital opioid analgesia may be associated with hospital disposition and length of stay for older adults with fall‐related injury. However, our findings do not provide evidence of an association with inpatient delirium. Background: We sought to define surgical outcomes among elderly patients with Alzheimer's disease and related dementias (ADRD) following major thoracic and gastrointestinal surgery. Methods: A retrospective cohort study was used to identify patients who underwent coronary artery bypass grafting (CABG), abdominal aortic aneurysm (AAA) repair, pneumonectomy, pancreatectomy, and colectomy. Individuals were identified from the Medicare Standard Analytic Files and multivariable regression was utilized to assess the association of ADRD with textbook outcome (TO), expenditures, and discharge disposition. Results: Among 1,175,010 Medicare beneficiaries, 19,406 (1.7%) patients had a preoperative diagnosis of ADRD (CABG: n = 1,643, 8.5%; AAA repair: n = 5,926, 30.5%; pneumonectomy: n = 590, 3.0%; pancreatectomy: n = 181, 0.9%; and colectomy: n = 11,066, 57.0%). After propensity score matching, patients with ADRD were less likely to achieve a TO (ADRD: 31.2% vs. no ADRD: 40.1%) or be discharged to home (ADRD: 26.7% vs. no ADRD: 46.2%) versus patients who did not have ADRD (both p < 0.001). Median index surgery expenditures were higher among patients with ADRD (ADRD: $28,815 [IQR $14,333–$39,273] vs. no ADRD: $27,101 [IQR $13,433–$38,578]; p < 0.001) (p < 0.001). On multivariable analysis, patients with ADRD had higher odds of postoperative complications (OR 1.32, 95% CI 1.25–1.40), extended length‐of‐stay (OR 1.26, 95% CI 1.21–1.32), 90‐day readmission (OR 1.37, 95% CI 1.31–1.43), and 90‐day mortality (OR 1.76, 95% CI 1.66–1.86) (all p < 0.001). Conclusion: Preoperative diagnosis of ADRD was an independent risk factor for poor postoperative outcomes, discharge to non‐home settings, as well as higher healthcare expenditures. To assess the prevalence of hyponatremia in critically ill elderly patients on admission to acute care facility; its clinic-demographic correlates, impact on clinical course and outcome. Method: The study was done at a centre for geriatric medicine palliative care hospital & hospice at its critical care unit at Lucknow between 2021–2022 for one year. A total of 267 subjects from the hospital records of non-COVID ICU admissions were analysed. Serum sodium levels < 135 mEq/L at admission was considered as hyponatremia. Demographic profile and comorbidity profile of the patients were also noted as per profroma. Severity of hyponatremia was assessed as mild (130–134.9 mEq/L), moderate (125–129.9 mEq/L) and severe (< 125 mEq/L) respectively. Total duration of ICU stay was noted as clinical course of illness and in-ICU mortality was noted as the final outcome. Data was analysed using SPSS 21.0 software. Chi-square, independent samples t-test and ANOVA were calculated. Results: Mean age of the patients was 72.65 ± 7.82 years (range 60–93 years), majority of them were men (56.2%). At admission, hyponatremia was seen in 36.3% (n = 97). Of this majority (n = 50; 51.5%) had mild, followed by moderate (n = 31; 32.0%) and severe (n = 16; 16.5%) hyponatremia. Mean duration of ICU stay was 8.65 ± 5.51 days (range 1–45 days). Mortality was seen in only 10 (3.7%) subjects. History of cardiovascular disease and higher mean number of comorbidities showed a significant association with at-admission hyponatremia. Mortality rate was significantly higher in hyponatremic (8.2%) as compared to non-hyponatremic patients (1.2%). Severity of hyponatremia showed a significant association with hypertension, cardiovascular disease, number of comorbidities, type of hyponatremia, but did not show a significant association with duration of ICU stay and mortality. Conclusion: Hospitalised critically ill elderly had a high prevalence of hyponatremia in just over 1/3rd subjects (36.3%) at the time of admission. Aim: The purpose of the research is the diagnosis of competencies of people holding managerial positions in the geriatric care sector in Germany. Methods: Scheitler’s concept of competency was employed in this study. The method of in-depth unstructured interviews was also used. The method of qualitative content analysis according to Mayring was applied to evaluate the obtained data. Results: As a result of the conducted analyses, professional tasks and competencies of people holding managerial positions in the geriatric care sector in Germany were presented. Discussion: The research results showed that the tasks of people performing managerial functions in geriatric care in Germany are complex and exceed medical or caregiving tasks. They are related to personnel management, responding to their needs, as well as the needs and expectations of people managing medical facilities. An extremely important task is cooperation with the relatives of the elderly. The research results also revealed the importance of having a wide variety of competencies. Conclusions: This study highlights the feasibility of implementing OHPP within GCFs, potentially enhancing the oral health and daily living activities of older individuals. Integrating the OHPP into routine care in geriatric settings is not only practical but also widely acceptable, offering a proactive approach to address oral health disparities among older residents. Stakeholders can maximize the impact of the OHPP by fostering collaboration among healthcare professionals, administrators, and residents, ultimately improving oral health outcomes and overall quality of life of older residents. The relevance of the stated topic of this research is conditioned upon the numerous problems of providing medical care to elderly people who suffer from chronic diseases characteristic of various disability groups, accompanied by a complete or partial loss of the ability to self-care, as a result of which there is a steady trend towards an increase in mortality among people of these age groups. The purpose of this research is to study the features of constructing an algorithm for providing geriatric and gerontological care in the Republic of Kazakhstan, considering the international experience accumulated in this field. The basis of the methodological approach in this study is a combination of a systematic analysis of the accumulated international experience in the field of geriatric and gerontological care, with an analytical study of the actual possibilities of constructing an algorithm for providing assistance based on this international experience. The main results obtained in the course of this study are the definition of the main stages of the algorithm for providing geriatric and gerontological care, and the definition of the main age groups of patients, more than other categories in need of this kind of assistance, formulated on the basis of accumulated international experience in this area. The results of this research work, including the conclusions formulated on their basis, have significant practical significance for disabled elderly people in need of medical care, and for social and medical service workers whose direct professional responsibilities include providing medical care to elderly people. The rapidly expanding older population across Low- and Middle-Income Countries (LMICs) underscores the urgent need to strengthen the health systems. However, the extent of service coverage and healthcare workforce allocation strategies for geriatric and palliative care in LMICs remains inadequately explored. To address this, we conducted this systematic review, following the standard guidelines, to synthesize available data from four databases spanning 2011 to 2022. Utilizing the Risk of Bias Assessment tool for non-randomized studies (ROBANS) for quantitative studies and the JBI (formerly known as Joanna Briggs Institute) checklist for qualitative research, we assessed the risk of bias and the quality in the included studies. Despite a comprehensive search, only three eligible studies were found, revealing a scarcity of research. The identified challenges include deficiencies in leadership, governance, financing, and essential resources, leading to compromised service coverage. The health workforce, often limited and lacking inadequate training, exacerbates the situation. Our evidence synthesis proposes some potential solutions such as telemedicine and cross-country collaboration through distance learning mechanisms to address these issues. The findings and recommendations from this review offer valuable insights applicable across various healthcare service contexts within LMICs and advocate for more health systems research in this topic area.

Abstract Bakground Worldwide, older people were more severely affected during the COVID-19 pandemic than others. In Sweden, those living in residential care facilities had the highest mortality rate, followed by those receiving home care services. The Swedish and international literature on the working environment for assistant nurses and care aides during the pandemic shows an increase in stress, anxiety, depression and post-traumatic stress syndromes. Care organisations were badly prepared to prevent the virus from spreading and to protect the staff from stress. In order to be better prepared for possible future pandemics, the health and well-being of the staff, the care of older people and the experiences of the staff both during and after a pandemic are important aspects to take into account. Therefore, this study aims to describe the experiences of assistant nurses and care aides working in the care of older people during the COVID-19 pandemic in Sweden, their working conditions and the impact all this had on their lives. Methodology The study has a qualitative, descriptive design. The data was collected in four focus group interviews with 21 participants and analysed using qualitative content analysis. Results The results revealed the theme, Being used for the greater good while fighting on the frontline, which was then divided into three categories: portrayed as a risk for older people, not being valued and being burnt out. The worsening working conditions that the pandemic contributed to resulted in a high degree of stress and risk of burnout, with staff members both wanting to and actually leaving their employment. After the pandemic they felt forgotten again and left to cope in an even worse situation than before. Conclusions The pandemic had a major effect on assistant nurses and care aides in terms of their working environment and their private lives. To be better prepared for future pandemics or disasters, organisations with responsibility for the care of older people will need to ensure that their staff have the necessary competencies and that there is adequate staffing in place. This also means that adequate government funding and multiple interventions will be needed.

A statistical significance was declared at a p-value < 0.05.Results: The majority of graduating MPN students had either neutral (70.76%) or positive (16.28%) attitudes towards older people; however, a greater amount (88%) of them had poor knowledge of geriatric care. The knowledge of female students was significantly less favorable than the knowledge of male students (p-value < 0.001). The knowledge and attitudes of pharmacy and nursing students were significantly lower than medical students (p-value < 0.001). The attitude mean rank score of students aged ≤ 25 years was significantly lower than students aged > 25 years (p-value < 0.001).Conclusion: The majority of graduating MPN students had poor knowledge despite having either a neutral or positive attitude toward geriatric care. Female students had lower knowledge scores than male students. Similarly, pharmacy and nursing students had lower knowledge and attitude scores than medical students, and the attitude score of students aged ≤ 25 years was lower than students aged > 25 years. Bakground: Worldwide, older people were more severely affected during the COVID-19 pandemic than others. In Sweden, those living in residential care facilities had the highest mortality rate, followed by those receiving home care services. The Swedish and international literature on the working environment for assistant nurses and care aides during the pandemic shows an increase in stress, anxiety, depression and post-traumatic stress syndromes. Care organisations were badly prepared to prevent the virus from spreading and to protect the staff from stress. In order to be better prepared for possible future pandemics, the health and well-being of the staff, the care of older people and the experiences of the staff both during and after a pandemic are important aspects to take into account. Therefore, this study aims to describe the experiences of assistant nurses and care aides working in the care of older people during the COVID-19 pandemic in Sweden, their working conditions and the impact all this had on their lives. Methodology: The study has a qualitative, descriptive design. The data was collected in four focus group interviews with 21 participants and analysed using qualitative content analysis. Results: The results revealed the theme, Being used for the greater good while fighting on the frontline, which was then divided into three categories: portrayed as a risk for older people, not being valued and being burnt out. The worsening working conditions that the pandemic contributed to resulted in a high degree of stress and risk of burnout, with staff members both wanting to and actually leaving their employment. After the pandemic they felt forgotten again and left to cope in an even worse situation than before. Conclusions: The pandemic had a major effect on assistant nurses and care aides in terms of their working environment and their private lives. To be better prepared for future pandemics or disasters, organisations with responsibility for the care of older people will need to ensure that their staff have the necessary competencies and that there is adequate staffing in place. Purpose: A study analyzing the application of a protocol of comprehensive geriatric assessment (CGA) in older patients with lymphoma was carried out to allow frailty-based patient classification and individualized treatment. Methods: Lymphoma patients older than 70 years referred to the Geriatric Clinic at a tertiary hospital between May 2016 and March 2021 were included. The assessment protocol included comorbidity, polypharmacy, nutritional, functional, and mental status, geriatric syndromes, and life expectancy. CGA enabled patient classification into four groups (Type I to Type IV) based on frailty assessment instrument scoring and clinical, functional, and mental status. Variables were compared using parametric and non-parametric statistical tests and Kaplan–Meier survival curves. Results: Ninety-three patients (55.9% women) were included. Median age was 81.1 years (± 5.7). 23 patients (24.7%) were classified as robust (type I), 30 (32.3%) as pre-frail (type II) with potentially reversable deficits, 38 (40.9%) as frail (type III), and 2 (2.2%) as requiring palliative care (type IV). Patients received oncospecific treatment with modifications carried out in 64.5% of cases based on CGA results. Differences in overall survival (p = 0.002), response to treatment (p < 0.001) and likelihood of increased frailty (p = 0.024) were observed, with type III–IV patients showing significantly worse outcomes. Conclusion: Performance of standardized, systematic CGA by geriatricians permits older lymphoma patients to be classified according to frailty, with significant differences in terms of clinical outcomes across groups. Older adults are a group vulnerable to health problems, and cases continue to increase. One of the reasons is living in a nursing home. So far, efforts have been made to improve older adults’ care services for their Quality of Life (QoL). However, one thing that is forgotten is spiritual services for older adults. This research aims to find relevant aspects between care and spiritual services to improve the QoL for older adult people living in nursing homes. This was a cross-sectional study. The sample size was 131 older adult people living in nursing homes. Samples were taken through simple random sampling with the criteria of being able to communicate and having a good cognitive function. Care services were measured using the Services Assessment Scale (SVAS) questionnaire, spiritual services were measured using the Spiritual Assessment Scale (SAS) questionnaire, and QoL was measured using the World Health Organization Quality of Life Older (WHOQOL-OLD) questionnaire. Data analysis used the gamma test. Respondents in this study were mostly women (55.5%), aged 60-74 years (62.6%), who had an elementary school graduation education (56.5%). Most respondents received low care services (74.0%), low spiritual services (84.0%), and most of them had low QoL (82.4%). The results showed a significant correlation between care services and QoL (p=0.000 and r=0.623), and between spiritual services and QoL (p=0.000 and r=0.755). Older adults in nursing homes need spiritual services and care services to improve their QoL.

BackgroundDespite numerous government initiatives, concerns and disparities among older adults have continually been growing. Empirical studies focused on older adults in the Philippines and Vietnam appear minimal and mostly regarding perceptions of aging. An effective geriatric care strongly relies on functional service providers requiring their perspectives to be explored toward inclusive service delivery.ObjectiveTo investigate the perceived gaps and opportunities in geriatric care service delivery among health and social care workers in selected urban areas in the Philippines and Vietnam.MethodsA qualitative case study approach drawn on social constructivism theory, examined working experiences, observed characteristics of older adults, geriatric services and needs, difficulties on service delivery, and recommended solutions. A total of 12 semi-structured interviews and 29 focus group discussions were conducted in the Philippines, with 174 health and social care workers, while in Vietnam, there were 23 semi-structured interviews and 29 focus group discussions with 124 participants. An inductive thematic analysis was employed.ResultsInterview participants highlighted the increasing unmet needs such as accessibility, availability, and acceptability of geriatric care services. The implementation of interventions on the older population faced multiple challenges, including issues related to older adult conundrums and dilemmas in geriatric care providers and facilities. The participants from the two countries felt that strengthening implementation of collaboration toward an integrated geriatric care structure and expansion of training and capability in handling older adults can be potential in addressing the gaps at both individual and institutional levels. Additionally, a committed leadership was viewed to be the important step to effectively operationalize the strategy.ConclusionHealth and social workers emphasized that the needs of older adults are exacerbated by various challenges within a fragmented geriatric care system. To address this issue, an establishment of an integrated service delivery mechanism with dedicated leadership is needed. The findings from this study may help develop appropriate solutions for addressing the health and social care needs of older adults in similar settings across Southeast Asia. Further examination of the impact of these challenges and solutions on service delivery and the wellbeing of older adults is essential.

Nurses Improving Care for Healthsystem Elders (NICHE), a program of NYU Rory Meyers College of Nursing, is a national quality and safety program that enhances clinical nursing in the care of older adults. Hospitals, long-term care, and hospice organizations adopt the NICHE practice model to advance professional practice, improve quality of care, and expand capacity to meet the growing demand for age-friendly care. In this month's column, I highlight three clinical improvement projects that were developed during the NICHE Leadership Training Program. In Turkey, physical frailty instruments have not been studied in the nursing home setting. We determined the reliability and validity of a Turkish version of the SHARE-Frailty Instrument for primary care (SHARE-FI) in Turkish nursing home residents. Cronbach's alpha reliability analysis was performed to determine internal consistency. Factor analysis was conducted to explore construct validity. Concurrent validity was assessed by correlation with the Care Dependency Scale (CDS). One hundred and fifty-one residents were included (mean age 73 years, 41% women). Fifty (33.1%) were identified as non-frail, 49 (32.5%) as pre-frail, and 52 (34.4%) as frail by SHAREFI. The overall Cronbach's alpha coefficient was 0.81. Factor analysis identified two components accounting for 69% of the variance, with the first and most important component being handgrip strength. SHARE-FI groups were significantly correlated with CDS scores (p<0.05). The Turkish version of SHARE-FI had good reliability and validity in a nursing home setting. [ABSTRACT FROM AUTHOR]

Abstract Background Vietnam’s aging population is growing rapidly, but its health workforce’s capacity to provide quality geriatric care is not clearly understood. We aimed to provide a cross-culturally relevant and validated instrument to assess evidence-based geriatric knowledge among healthcare providers in Vietnam. Methods We translated the Knowledge about Older Patients Quiz from English to Vietnamese using cross-cultural adaptation methods. We validated the translated version by evaluating its relevance to the Vietnamese context, as well as its semantic and technical equivalence. We fielded the translated instrument on a pilot sample of healthcare providers in Hanoi, Vietnam. Results The Vietnamese Knowledge about Older Patients Quiz (VKOP-Q) had excellent content validity (S-CVI/Ave) and translation equivalence (TS-CVI/Ave) of 0.94 and 0.92, respectively. The average VKOP-Q score was 54.2% (95% CI: 52.5—55.8) and ranged from 33.3 to 73.3% among 110 healthcare providers in the pilot study. Healthcare providers in the pilot study had low scores on questions related to the physiopathology of geriatric conditions, communication techniques with sensory impaired older adults, and differentiating age related changes from abnormal changes or symptoms. Conclusions The VKOP-Q is a validated instrument to assess geriatric knowledge among healthcare providers in Vietnam. The level of geriatric knowledge among healthcare providers in the pilot study was unsatisfactory, which supports the need for further assessment of geriatric knowledge among a nationally representative sample of healthcare providers.
