BACKGROUND: “Spaceflight associated neuro-ocular syndrome” (SANS) represents a challenging health condition in modern space medicine. Forty-eight percent of astronauts are diagnosed with SANS after long-term space missions. The pathophysiological mechanism seems to be multifactorial, and yet remains unknown. In this proof-of-concept study we plan to investigate retinal microcirculatory changes in weightlessness and aim to identify their role in the development of SANS. METHODS AND DESIGN: Healthy individuals will take part in a parabolic flight campaign, which recreates fractioned total weightlessness periods. The airplane is specifically equipped, and designed for the execution of parabolic flight maneuvers and scientific research in microgravity. Retinal microcirculation will be assessed with a modified fundus camera, which allows dynamic vessel analysis. We will additionally measure intra-ocular pressure and hemodynamic changes during each phase of the flight. Blood samples will be analyzed at baseline, one hour and 24 hours after exposure to weightlessness. CONCLUSIONS: This pilot study aims to investigate the feasibility of retinal microcirculation assessment during varying gravity. Results of this study may generate insights whether venous stasis in the eye, surrogated by the dilatation of retinal vessels and increase in intraocular pressure as signs of venous insufficiency, may potentially contribute to the development of SANS.

Zusammenfassung Hintergrund Im Verlauf der letzten Jahre werden zunehmend mehr OP-Mikroskope mit OCT-Modul (intraoperatives OCT, iOCT) verfügbar, die in Echtzeit hochauflösende Aufnahmen des OP-Situs liefern. Nachdem eine Erhebung 2018 an unserer Klinik nur in 2,4% aller Operationen einen intraoperativen Zusatznutzen der iOCT ergab, der Hersteller aber die Hardware und Software überarbeitet hat, führten wir erneut eine Nutzenanalyse der Technologie durch. Material und Methoden Prospektive monozentrische Analyse zu Einsatz und Anwenderfreundlichkeit eines EnFocus Ultra-Deep OCT (Leica Microsystems) über einen Zeitraum von 25 (2018) bzw. 20 Werktagen (2021). Anhand eines standardisierten Fragebogens wurde die Verwendung der iOCT auf den OP-Verlauf durch die Operateure bewertet. Ergebnisse 2018 wurden über einen Zeitraum von 25 Tagen 118 Operationen und 2021 über 20 Tage 92 Operationen durchgeführt. 2018 wurde die iOCT-Funktion in 24,6% und 2021 in 48,9% aller Eingriffe eingesetzt, wobei sich in beiden Jahren die iOCT nach Einschätzung durch die Operateure in 2,4% bzw. 3,3% als „entscheidend“ für den OP-Verlauf erwies. Hierbei handelte es sich um Operationen, bei denen der intraokulare Einblick limitiert war, wie z. B. bei dekompensierter Hornhaut, Glaskörperblutung oder nach Voroperationen wie z. B. nach perforierender Keratoplastik. Schlussfolgerung Durch die Weiterentwicklung der Nutzeroberfläche wurde eine Verbesserung der Bedienbarkeit erzielt und das iOCT deutlich häufiger verwendet. Die iOCT erwies sich in vergleichbar wenigen Operationen, insbesondere bei komplexen Ausgangslagen, als verlaufsentscheidend.

ZusammenfassungDer intraokulare Druck (IOD) ist ein wichtiger physikalischer Parameter der Physiologie des Auges. Ein erhöhter IOD wurde früh als Ätiologie für Augenerkrankungen vermutet. In diesem 1. Teil des Beitrags wird neben den physiologischen Grundlagen die Kontakttonometrie vorgestellt, die sowohl die erste instrumentelle Methode mit den Indentationstonometern als auch den Goldstandard mit dem Goldmann-Applanationstonometer darstellt.

DEUTSCH Hintergrund: Die korneale Biomechanik hat einen erheblichen Einfluss auf das Ergebnis von hornhautchirurgischen Eingriffen, wie astigmatische Keratotomien und kann Ursache für Komplikationen, wie z. B. einer iatrogenen Keratektasie sein. Bisherige Ansätze die korneale Biomechanik und Spannungsverteilung in vivo zu bestimmen, um Komplikationen zu vermeiden und Operationsergebnisse besser vorherzusagen waren nicht erfolgreich. Ziel der Arbeit: In dieser Übersichtsarbeit soll die Funktionsweise der Brillouin-Spektroskopie erläutert und der gegenwärtige wissenschaftliche Stand der Technik für den Einsatz in der Ophthalmologie dargelegt werden. Methoden: Pubmed-Recherche relevanter experimenteller und klinischer Arbeiten, sowie eigene Erfahrungen mit der Brillouin-Spektroskopie. Ergebnisse: Die Brillouin-Spektroskopie kann verschiedene biomechanische Moduli von okulären Geweben mit einer hohen räumlichen Auflösung bestimmen. Mit den derzeit verfügbaren Geräten können an der Hornhaut fokale Schwächungen (Keratokonus) ebenso wie Versteifungen (nach Crosslinking) identifiziert werden. Auch die biomechanischen Eigenschaften der Linse können gemessen werden. Die Anisotropie und Hydratation der Kornea, ebenso wie die Winkel-Abhängigkeit des Laserstrahls während der Brillouin-Messung erschweren jedoch die Interpretation der gemessenen Daten erheblich. Eine klare Überlegenheit in der Früherkennung von Keratektasien gegenüber der kornealen Tomographie konnte bisher noch nicht gezeigt werden. Schlussfolgerung: Die Brillouin-Spektroskopie ist eine neue Technik zur biomechanischen in vivo-Charakterisierung von okulären Geweben, welche die bisherige diagnostische Lücke schliessen kann. Publizierte Ergebnisse sind vielsprechend, bis zum Einsatz im klinischen Alltag sind jedoch Verbesserungen der Messtechnik, insbesondere hinsichtlich der kornealen Anisotropie erforderlich. ENGLISH Background: Information about corneal biomechanics is crucial for achieving satisfactory outcomes after surgical corneal interventions, e. g. for astigmatic keratotomies, but also to identify corneas that are at risk for postoperative complications such as corneal ectasia. Hitherto, approaches to characterize corneal biomechanics in an in vivo setting have yielded only minor success, demonstrating the unmet medical need for a diagnostic technique to measure ocular biomechanics. Objective: This review shall explain the mechanism of Brillouin spectroscopy and summarize the current scientific knowledge for ocular tissue. Methods: Pubmed research of relevant experimental and clinical publications, as well as reporting of own experience using Brillouin spectroscopy. Results: Brillouin spectroscopy can measure different biomechanical moduli with a high spatial resolution. Currently available devices are able to detect focal corneal weakening, e.g. in keratoconus as well as stiffening after corneal crosslinking. Also, the mechanical properties of the crystalline can be measured. Corneal anisotropy and hydration, together with the dependence on the angle of the incident laser-beam in Brillouin spectroscopy are challenges in the precise interpretation of measured data. A clear advantage in the detection of subclinical keratoconus compared to corneal tomography, however, has not been shown yet. Conclusion: Brillouin spectroscopy is a technique to characterize biomechanical properties of ocular tissue in vivo. Published results confirm ex vivo data of ocular biomechanics, however, further improvements in the acquisition and interpretation of measured data are required until this technique can be used in clinically viable setting.

Purpose Aim of this study was to evaluate the frequencies, trends, and antibiotic resistance of bacteria collected from ocular surface or contact lens material in a German tertiary referral center from 2009 to 2019. Methods Microbiological data from 2009 to 2019 was analyzed. Culture-dependent microbial identification and analysis of antibiotic sensitivity was completed by the Institute of Microbiology. Statistical analysis of age- and sex-specific differences as well as changes in the microbial spectrum and resistance over the study period was performed with GraphPad Prism 9.0 applying nonparametric tests (level of significance: p ≦ 0.05). Results A total of 6361 specimens were analyzed. Positivity rate was 18.6%. Sixty-three percent (n = 680) of the bacterial isolates were derived from ocular surface and 37% (n = 399) from contact lens material. The ratio of gram-negative bacteria was significantly higher in contact lens material. Multiresistant bacteria showed a significant increase with patient age (p p = 0.0239) was detected. Only 2.4% and 3.1% isolates were resistant to a combination of moxifloxacin and gentamicin, respectively, levofloxacin and gentamicin. Conclusions The reported bacterial spectrum is similar to comparable centers. Our data show that it should not be assumed that the newest classes of antibiotics have the best efficacy or lowest resistance levels. In suspected bacterial conjunctivitis, we propose using gentamicin as first-line therapy. In therapy refractive cases and in involvement of the cornea, we recommend a combination of gentamicin and ofloxacin or moxifloxacin. Overall, the evaluated organisms showed good sensitivity to the regularly used antibiotics.

ZusammenfassungTraditionell wird das Hornhautendothel als postmitotisch angesehen, sodass von einer spontanen Regeneration nach Verlust von Endothelzellen (z. B. im Rahmen der Fuchsʼschen endothelialen Hornhautdystrophie) nicht ausgegangen wird. Dennoch konnte in mehreren Fällen beobachtet werden, dass eine ödematöse Hornhaut nach Entfernung eines kleineren Areals der Descemet-Membran aufklart. Der posteriore Aspekt dieses Bereichs wurde von Hornhautendothelzellen wieder bedeckt. Hieraus entstand ein operatives Therapiekonzept ohne posteriores lamelläres Transplantat: „Descemet stripping only“. Zugleich legen Daten aus In-vitro-Versuchen nahe, dass Rho-Kinase-Inhibitoren (ROCK-Inhibitoren) pharmakologische Wirkstoffe sind, die eine Proliferation und Migration des Hornhautendothels fördern und somit den Heilungsverlauf nach Descemet-Stripping positiv beeinflussen. Mittlerweile liegen erste Daten aus kleineren prospektiven, kontrollierten klinischen Studien vor, die durchweg gute Ergebnisse nach Descemet-Stripping und ROCK-Inhibitor-Augentropfen berichten. Offene Fragen bestehen jedoch derzeit noch hinsichtlich der Charakteristika der Patientengruppe, die von diesem Therapieansatz besonders profitiert und hinsichtlich der Nachhaltigkeit eines Therapieerfolgs.

ZusammenfassungIn 2. Teil des Beitrags werden die sonstigen Kontakttonometer und die Nonkontakttonometrie präsentiert. Es wird anhand einer Revue der Literatur über den klinischen Wert der verschiedenen Methoden und den Wert der Multimodalität in der Tonometrie diskutiert; ferner werden die letzten Innovationen mit den telemetrischen IOD-Sensoren eingeführt.

Purpose Acanthamoeba keratitis (AK) is a painful and possibly sight-threatening ocular infection. While the correct diagnosis and specific treatment in the early stages significantly improve the prognosis, the disease is often misdiagnosed and in clinical examination confused with other forms of keratitis. Polymerase chain reaction (PCR) for the detection of AK was first introduced in our institution in December 2013 to improve the timely diagnosis of AK. The aim of this study was to assess the impact of implementation of Acanthamoeba PCR on the diagnosis and treatment of the disease in a German tertiary referral center. Patients and methods Patients treated for Acanthamoeba keratitis between 1st of January 1993 and 31st of December 2021 in the Department of Ophthalmology of the University Hospital Duesseldorf were identified retrospectively via in-house registries. Evaluated parameters include age, sex, initial diagnosis, method of correct diagnosis, duration of symptoms until correct diagnosis, contact lens use, visual acuity, and clinical findings as well as medical and surgical therapy by keratoplasty (pKP). In order to assess the impact of implementation of Acanthamoeba PCR, the cases were divided into two groups (before (pre-PCR group) and after PCR implementation (PCR group). Results Seventy-five patients with Acanthamoeba keratitis were included (69.3% female, median age 37 years). Eighty-four percent (63/75) of all patients were contact lens wearers. Until PCR was available, 58 patients with Acanthamoeba keratitis were diagnosed either clinically (n = 28), by histology (n = 21), culture (n = 6), or confocal microscopy (n = 2) with a median duration until diagnosis of 68 (18; 109) days. After PCR implementation, in 17 patients, the diagnosis was established with PCR in 94% (n = 16) and median duration until diagnosis was significantly shorter with 15 (10; 30.5) days. A longer duration until correct diagnosis correlated with a worse initial visual acuity (p = 0.0019, r = 0.363). The number of pKP performed was significantly lower in the PCR group (5/17; 29.4%) than in the pre-PCR group (35/58; 60.3%) (p = 0.025). Conclusions The choice of diagnostic method and especially the application of PCR have a significant impact on the time to diagnosis and on the clinical findings at the time of confirmation of diagnosis and the need for penetrating keratoplasty. In contact lens–associated keratitis, the first crucial step is to take AK into consideration and perform a PCR test as timely confirmation of diagnosis of AK is imperative to prevent long-term ocular morbidity.

Purpose To evaluate the standard of care, in particular the use of topical or subconjunctival interferon-α2b, in treating ocular surface squamous neoplasia or melanocytic tumours in tertiary eye centres in Germany. Methods A survey containing 14 questions was sent to 43 tertiary eye centres in Germany. The questions addressed the surgical and medical management of ocular surface squamous neoplasia and melanocytic tumours (primary acquired melanosis and malignant melanoma), as well as the clinical experiences and difficulties in prescribing off-label interferon-α2b eye drops and subconjunctival injections. Results Twenty-four tertiary eye centres responded to the survey. Eighty-three percent of centres had used interferon-α2b in their clinical practice and 25% prescribed it as the first-line cytostatic agent following surgical excision of ocular surface squamous neoplasia, while 10% would do so for melanocytic tumours. Correspondingly, the majority of respondents selected mitomycin C as their first-line agent. Side effects were uncommon with topical interferon-α2b eye drops but were more frequently reported after subconjunctival interferon-α2b injections. In total, eight centres had experience with interferon-α2b injections. The most significant obstacles perceived by ophthalmologists when prescribing interferon-α2b were its high cost and the reimbursement thereof. Conclusion Off-label mitomycin C was the preferred adjuvant therapy for epithelial and melanocytic tumours, with interferon-α2b being the standard second-line option. Interferon-α2b has predominantly been used to treat ocular surface squamous neoplasia and, to a lesser extent, melanocytic tumours at German tertiary eye centres. Following its market withdrawal, supply shortages of interferon-α2b are likely to have a profound impact on patient care and their quality of life.

BACKGROUND In accordance with worldwide data, the Robert Koch Institute (RKI) has reported a constant increase of syphilis cases in Germany over the past decade. METHODS We analysed the data of all patients, referred to a Department of Ophthalmology in a tertiary referral centre in Dusseldorf, Germany between 2008 and 2019, who were tested for syphilis. The epidemiologic, demographic, clinical, diagnostic and therapeutic data were retrieved from the records and evaluated in a retrospective, descriptive, non-comparative study. RESULTS Syphilis serology was positive in 32/1840 (1.7%) patients, and was evenly distributed over this period. 26 (81.3%) were male, 19 (59.4%) belonged to a risk group. Ocular syphilis was the primary diagnosis for 29 patients (90.6%). The most frequent manifestation was uveitis (n = 20, 62.5%). By the end of therapy, 19 patients (59.4%) had an improved visual acuity. CONCLUSION The incidence of ocular syphilis cases has remained stable over the last decade.

Zusammenfassung Hintergrund Orbitaverletzungen mit organischen Fremdkörpern sind eine klinische Herausforderung, da sie eine Vielzahl von Komplikationen verursachen können und manchmal zu irreversiblen Visuseinschränkungen führen. Material und Methoden Wir berichten über 5 konsekutive Fälle von Patienten mit organischen orbitalen Fremdkörpern, die zwischen 2012 und 2022 vorstellig wurden. Alter, Geschlecht, Zeitpunkt der Verletzung, Verletzungsmechanismus, bestkorrigierter Visus prä- und postoperativ, Vorhandensein von Schmerzen, Motilitätseinschränkungen, durchgeführte Bildgebung, Therapie sowie Komplikationen wurden ausgewertet. Ergebnisse Das mittlere Patientenalter betrug 62 ± 33 Jahre und das Geschlechterverhältnis war 4/1 (weiblich/männlich). In 4 Fällen war von einem Verletzungsgeschehen berichtet worden, welches im Mittel 4,6 (0/14 min/max) Tage zurücklag. Der bestkorrigierte Visus bei Erstvorstellung betrug 0,5 ± 0,2 dezimal und nach einem mittleren Follow-up von 12,7 (0,5/38 min/max) Monaten 0,67 ± 0,3 dezimal (p = 0,2). Der Grund der Erstvorstellung waren Schmerzen (n = 3) und/oder Doppelbilder (n = 2). Bei allen Patienten wurde eine Computertomographie (CT) durchgeführt. In allen 5 Fällen konnte das Fremdkörpermaterial chirurgisch vollständig entfernt werden, wobei eine Patientin wegen verbliebender Fremdkörper mehrmals operiert werden musste. Beim Fremdkörpermaterial handelte sich in 4 Fällen um Holz und einmal um Wachs. Schlussfolgerung Bei Orbitatraumata muss eine sorgfältige Wundexploration durchgeführt werden, insbesondere wenn der Unfallhergang anamnestisch nicht eindeutig zur klären und verbleibende Fremdkörper oder eine Perforationsverletzung nicht ausgeschlossen werden können. Die Bildgebung der Wahl ist die CT, die ggf. im postoperativen Verlauf erneut durchgeführt werden muss.

Corneal Crosslinking (CXL) strengthens the keratoconus cornea and prevents further disease progression. Modified crosslinking protocols and different riboflavin solutions have been proposed to optimize the procedure and improve treatment success.PubMed research of relevant publications and report of own experiences with different CXL protocols.Accelerated CXL shows comparable efficiency with shorter surgery time and similar complication rates. Customized CXL provides improved results with faster epithelial healing. CXL in a hyperoxic environment seems to be a safe and effective transepithelial alternative with presumably less complications and fewer side effects. Thin corneas (400 µm) can be treated safely by corneal swelling using hypoosmolar riboflavin solutions and reducing the applied UV-energy. The combination of CXL with photorefractive keratectomy (PRK) can be considered in patients with contact lens intolerance improving visual acuity, however, with increased risk of visual loss compared to CXL alone. Two-Photon (2Ph) CXL is a promising new technology enabling three-dimensional CXL.Recently developed CXL protocols offer advantages over the standard "Dresden-protocol" and should be considered in patients with progressive keratectasia.

Reliable and repeated IOP measurement are essential in the diagnosis and treatment of glaucoma. In this first part of this review, besides the physiological basics, contact tonometry will be presented, which is both the first instrumental method with the indentation tonometers and the gold standard with the Goldmann applanation tonometer. Both methods and subsequently developments will be discussed.

Das Schleimhautpemphigoid (SHP) ist eine Pemphigoid-Erkrankung, die sich überwie-gend an der Schleimhaut manifestiert. Betroffen sind vor allem die Schleimhäute von Mund, Augen, Nase und Pharynx, aber auch von Larynx, Trachea, Ösophagus sowie die Genital- und Perianal-Region. Die Ausprägung der Erkrankung ist variabel und reicht von gingivalem Erythem und einzelnen oralen Läsionen bis zu schwerwiegen-den trachealen Strikturen, die die Atmung behindern sowie konjunktivaler Vernarbung mit deutlichen Visuseinschränkungen und, nicht selten, Erblindung. Die Diagnose beruht neben dem klinisch vorherrschenden Schleimhautbefall auf der direkten Im-munfluoreszenz eines nichtläsionalen Biopsats und der Serologie. Das hauptsächliche Zielantigen ist BP180 (Kollagen XVII) und Reaktivität mit Laminin 332 ist in circa 25 % der SHP-Patienten mit einem Malignom assoziiert. Die Behandlung des SHP ist eine Herausforderung. Einerseits ist durch den Befall verschiedener Schleimhäute eine gute interdisziplinäre Zusammenarbeit der verschiedenen Fachdisziplinen gefordert, ande-rerseits stehen auf Grund der Seltenheit der Erkrankung keine randomisierten kont-rollierten klinischen Studien zur Verfügung. Ziel der Leitlinie ist es, das klinische Bild einschließlich Schweregrade und Scoring-Systeme darzustellen sowie Anleitung zu geben für die Diagnosestellung und Therapie dieser komplexen Erkrankung. Beim SHP spielt die interdisziplinäre Zusammenarbeit eine wesentliche Rolle sowie die möglichst zeitnahe Diagnosestellung und Einleitung einer adäquaten Therapie, um irreversible Schäden der Schleimhäute mit schwerwiegenden Komplikationen zu vermeiden.

Summary Mucous membrane pemphigoid (MMP) is a pemphigoid disease with predominant mucous membrane involvement. It mainly affects the mucous membranes of the mouth, eyes, nose and pharynx, but also the larynx, trachea, esophagus, genital and perianal regions. The manifestation of the disease covers a wide spectrum from gingival erythema and single oral lesions to severe tracheal strictures that obstruct breathing and conjunctival scarring with marked visual impairment and, not infrequently, blindness. In addition to a clinical picture of predominant mucosal involvement, diagnosis is based on direct immunofluorescence of a peri‐lesional biopsy and serology. The main target antigen is BP180 (collagen XVII), and reactivity with laminin 332 is associated with malignancy in approximately 25 % of MMP patients. The treatment of MMP is challenging. On the one hand, due to the involvement of different mucous membranes, good interdisciplinary cooperation is required; on the other hand, due to the rarity of the disease, no randomized controlled clinical trials are available. The aim of this guideline is to present the clinical picture, including severity and scoring systems, and to give guidance for diagnosing and treating this complex disease. In MMP, interdisciplinary cooperation plays an essential role as well as the prompt diagnosis and initiation of adequate therapy in order to avoid irreversible damage to the mucous membranes with serious complications.

ZusammenfassungDie Therapie des Trockenen Auges stellt eine schwierige Aufgabe für den behandelnden Augenarzt dar. Fortschritte in unserem Verständnis der Risikofaktoren, der Ätiologie und Pathophysiologie des Trockenen Auges haben zu einer Weiterentwicklung der Behandlungsstrategien geführt. Vor Beginn der Therapie ist die korrekte Zuordnung in eine wässrig-muzinöse Tränenfilmdysfunktion (Mindersekretion), in eine lipidbedingte Tränenfilmdysfunktion (hyperevaporative Form durch eine abnormen Meibom-Drüsen-Physiologie) oder in eine variable Kombination wichtig. Unter Berücksichtigung aktueller Empfehlungen der DOG, des BVA und des TFOS DEWS II Unterausschusses wurde ein Behandlungsalgorithmus abgeleitet, der ein schrittweises Vorgehen je nach Schweregrad der Erkrankung darstellt.

Descemet membrane endothelial keratoplasty (DMEK) accounts for50% of all corneal transplants in Germany. So far, no data from such a large multicenter study have been published.This retrospective study included 3200 DMEKs at seven departments performed for Fuchs endothelial corneal dystrophy (FECD) or bullous keratopathy (BK). We evaluated best corrected visual acuity (BCVA, logMAR), endothelial cell density (ECD, cells/mmFor patients without vision-limiting comorbidities (74% of all analysed eyes, n = 2270), mean BCVA improved from 0.6 ± 0.4 logMAR to 0.2 ± 0.2 logMAR 6 months (p 0.001, n = 1441) and 0.1 ± 0.2 logMAR 12 months (p = 0.001, n = 1402) postoperatively. BK- had a worse BCVA compared to FECD-patients (0.3 ± 0.5 vs. 0.1 ± 0.2 logMAR [p 0.001] at 1 year). ECD declined from 2465 ± 259 cells/mmDescemet membrane endothelial keratoplasty increases visual acuity with low transplant failure- and rejection-rates. FECD has a better outcome than BK. Since a quarter of all patients need a rebubbling, this should be included in the informed consent. Remarkably, one rebubbling has no influence on the outcome.

Zusammenfassung Hintergrund Die DMEK ist ein etabliertes Verfahren, um endotheliale Hornhauterkrankungen zu behandeln. Dabei wird eine Endotamponade der Vorderkammer genutzt, um das Transplantat am Empfängerbett zu fixieren. Bei Z. n. nach Aphakisierung, großen Irisdefekten oder fistulierender Glaukomoperation kommt es oft früh postoperativ zum Verlust der Gastamponade in den Glaskörperraum oder nach subkonjunktival. Wir berichten über 3 Fälle, bei denen eine simultane Gastamponade der Vorder- und Hinterkammer durch primäre Kombination von DMEK und 23-G-Pars-plana-Vitrektomie (ppV) durchgeführt wurde. Fallbeschreibungen Im 1. Fall zeigte sich bei einem 70-jährigen Mann früh postoperativ nach DMEK eine vollständige Gasdislokation in den Glaskörperraum, sodass ein Re-Bubbling in Kombination mit einer 23-G-ppV durchgeführt wurde. In Fall 2 wurde bei einem 80-jährigen Mann aufgrund eines vorhandenen Ahmed-Glaukom-Implantates und in Fall 3 bei einer 61-jährigen Frau mit traumatischer Aphakie die DMEK unmittelbar mit einer ppV kombiniert. In allen Fällen erfolgte eine bestmögliche Gasfüllung von Glaskörperraum und Vorderkammer mittels 20% SF6. In Fall 2 wurde ein Re-Bubbling aufgrund einer partiellen Transplantatdehiszenz am 5. Tag nach DMEK erforderlich. In allen Fällen blieb der intraokulare Druck postoperativ kompensiert und es kam trotz der visuslimitierenden komplexen Vorerkrankungen zu einem Visusanstieg. Diskussion Die kombinierte Endotamponade von Vorder- und Hinterkammer im Rahmen einer DMEK mit 23-G-ppV kann bei Augen mit komplexer Vorgeschichte die Gasendotamponade verlängern und so eine erfolgreiche Transplantatadhärenz sichern.

Orbital injuries with organic foreign bodies are a clinical challenge as they can cause a variety of complications and sometimes lead to irreversible visual impairment.We report five consecutive cases of patients with organic orbital foreign bodies who presented between 2012 and 2022. Documented parameters include age, gender, time of injury, mechanism of injury, best-corrected visual acuity (BCVA), presence of pain, restriction of motility, performed imaging, location and type of foreign body, treatment and complications.The mean patient age was 61.8 ± 32.3 years and the gender ratio was 4:1 (female:male). In 4 cases the injury had been reported on average 4.6 days (0/14 days min/max) previously. Best corrected visual acuity at first presentation was 0.5 ± 0.2 decimal and after a mean follow-up of 12.7 months (0.5/38 months min/max) at last presentation 0.67 ± 0.3 decimal (p = 0.2). The reason for initial presentation was pain (n = 3) and/or double vision (n = 2). All patients underwent computer tomography (CT). The diagnosis was confirmed in two cases by means of CT. In all five cases the foreign body material could be completely surgically removed, while one patient needed repeated surgery due to retained foreign body material. The organic foreign body material included wood (4) and wax (1). In two cases, a foreign body-induced infection was successfully treated with systemic antibiotics. There were no other complications.In orbital trauma a thorough wound exploration must be performed, especially if the mechanism of injury is uncertain and residual foreign bodies or a perforation injury cannot be excluded. The imaging of choice is CT, which may have to be performed again in the postoperative course.HINTERGRUND: Orbitaverletzungen mit organischen Fremdkörpern sind eine klinische Herausforderung, da sie eine Vielzahl von Komplikationen verursachen können und manchmal zu irreversiblen Visuseinschränkungen führen.Wir berichten über 5 konsekutive Fälle von Patienten mit organischen orbitalen Fremdkörpern, die zwischen 2012 und 2022 vorstellig wurden. Alter, Geschlecht, Zeitpunkt der Verletzung, Verletzungsmechanismus, bestkorrigierter Visus prä- und postoperativ, Vorhandensein von Schmerzen, Motilitätseinschränkungen, durchgeführte Bildgebung, Therapie sowie Komplikationen wurden ausgewertet.Das mittlere Patientenalter betrug 62 ± 33 Jahre und das Geschlechterverhältnis war 4/1 (weiblich/männlich). In 4 Fällen war von einem Verletzungsgeschehen berichtet worden, welches im Mittel 4,6 (0/14 min/max) Tage zurücklag. Der bestkorrigierte Visus bei Erstvorstellung betrug 0,5 ± 0,2 dezimal und nach einem mittleren Follow-up von 12,7 (0,5/38 min/max) Monaten 0,67 ± 0,3 dezimal (p = 0,2). Der Grund der Erstvorstellung waren Schmerzen (n = 3) und/oder Doppelbilder (n = 2). Bei allen Patienten wurde eine Computertomographie (CT) durchgeführt. In allen 5 Fällen konnte das Fremdkörpermaterial chirurgisch vollständig entfernt werden, wobei eine Patientin wegen verbliebender Fremdkörper mehrmals operiert werden musste. Beim Fremdkörpermaterial handelte sich in 4 Fällen um Holz und einmal um Wachs.Bei Orbitatraumata muss eine sorgfältige Wundexploration durchgeführt werden, insbesondere wenn der Unfallhergang anamnestisch nicht eindeutig zur klären und verbleibende Fremdkörper oder eine Perforationsverletzung nicht ausgeschlossen werden können. Die Bildgebung der Wahl ist die CT, die ggf. im postoperativen Verlauf erneut durchgeführt werden muss.

Introduction Severe aqueous tear deficiency is caused by primary or secondary main lacrimal gland insufficiency. The transplantation of a human lacrimal gland could become a potential treatment option to provide physiological tears with optimal properties. To this end, we performed an ex vivo study to develop a surgical strategy that would ensure a vascular supply for a lacrimal gland transplant using microvascular techniques. Material and methods Five cadaver heads were used to perform a lateral orbitotomy in order to identify the vascular pedicle and the lacrimal gland itself. The principal feasibility and the time of the required surgical steps for an intraorbital microvascular re-anastomosis of the human lacrimal gland were documented. Patency and potential leakage of the anastomosis were tested with hematoxylin intraoperatively. Postoperatively, routine histological, as well as scanning electron microscopy (SEM) of the gland and vascular anastomosis, were performed. Results The vascular pedicle of all five glands could be isolated over a minimum stretch of at least 1 cm, severed, and successfully reanastmosed microsurgically. Time for arterial anatomization (n = 4) was 23 ± 7 min and 22 ± 3 min for the vein (p = 0.62). The total time for the entire microvascular anastomosis was 46 ± 9 min. All anastomosis were patent upon testing. SEM revealed well-aligned edges of the anastomosis with tight sutures in place. Conclusion Our study demonstrates as proof of principle the feasibility of intraorbital microvascular re-anastomosis of a human lacrimal gland within the presumed window of ischemia of this tissue. This should encourage orbital surgeons to attempt lacrimal gland transplantation in humans in vivo.

In recent decades, two techniques for large full-thickness upper eyelid reconstruction have been described, the Cutler-Beard lid sharing flap and the Mustardé eyelid switch flap. In the literature, modifications are being introduced to improve those techniques. Here, we present our approach for the reconstruction of total full-thickness upper eyelid defects and compare it with these established upper lid reconstruction techniques.Three patients presented with upper eyelid tumors and required extensive excision resulting in total full-thickness upper eyelid defects. Reconstruction consisted of a two-stage procedure: a Mustardé eyelid switch flap was performed followed by division of the rotation flap and lateral canthoplasty using a periosteal bipedicled flap and Tenzel flap. Patients were followed-up every 3 months for at least 1 year. During every preoperative and postoperative check-up, palpebral fissure height, levator function, margin reflex distance, and presence of lagophthalmos were measured.Histopathological examination revealed a Merkel cell carcinoma in two cases and a Basal cell carcinoma in one case. Postoperatively, all patients showed a stable reconstructed upper eyelid with preserved motility and satisfying aesthetic results when compared to the fellow eye. In one case, a lagophthalmos of 1.5 mm was observed, which was treated conservatively to prevent exposure keratopathy.The eyelid switch flap combined with a bipedicled periosteal and a Tenzel flap is a good alternative for the reconstruction of total upper eyelid defects with the advantage of leaving the contralateral eye untouched. It achieves satisfying anatomical results, including an upper eyelid margin with eyelashes and well-matched skin color.In den letzten Jahren wurden 2 Techniken für die Rekonstruktion großflächigen durchgreifenden Oberliddefekten beschrieben, der Cutler-Beard-Lid-Sharing-Lappen und der Mustardé-Switch-Lappen. In der Literatur werden Modifikationen vorgestellt, um diese Techniken zu verbessern. Hier stellen wir unseren Ansatz für die Rekonstruktion von totalen, durchgreifenden Oberliddefekten vor und vergleichen ihn mit diesen etablierten Oberlidrekonstruktionstechniken.Drei Patienten stellten sich mit Oberlidtumoren vor, die eine ausgedehnte Exzision erforderten, die zu totalen, durchgreifenden Oberliddefekten führte. Die Rekonstruktion bestand aus einem zweizeitigen Verfahren: Es wurde ein Mustardé-Switch-Lappen durchgeführt, gefolgt von einer Teilung des Rotationslappens und einer lateralen Winkelplastik unter Verwendung eines bipedikulären Periostlappens und eines Tenzel-Lappens. Die Patienten wurden über einen Zeitraum von mindestens 1 Jahr alle 3 Monate nachuntersucht. Bei jeder prä- und postoperativen Kontrolle wurde die Lidspaltenhöhe, die Levatorfunktion, der Randreflexabstand und das Vorhandensein eines Lagophthalmus gemessen.Die histopathologische Untersuchung ergab in 2 Fällen ein Merkel-Zell-Karzinom und in 1 Fall ein Basalzellkarzinom. Postoperativ zeigten alle Patienten ein stabiles rekonstruiertes Oberlid mit erhaltener Beweglichkeit und zufriedenstellenden ästhetischen Ergebnissen im Vergleich zum anderen Auge. In einem Fall wurde ein Lagophthalmus von 1,5 mm beobachtet, der konservativ behandelt wurde, um eine Expositionskeratopathie zu verhindern.Der Eyelid-Switch-Lappen in Kombination mit einem bipedikulären Periost- und einem Tenzel-Lappen ist eine gute Alternative für die Rekonstruktion von durchgreifenden Defekten des gesamten Oberlids mit dem Vorteil, dass das kontralaterale Auge unangetastet bleibt. Es werden zufriedenstellende anatomische Ergebnisse erzielt, einschließlich eines Oberlidrandes mit Wimpern und einer guten Übereinstimmung der Hautfarbe.

Cornea transplants are tissue transplants and, as such, must be distinguished from organ transplants (e.g. heart or kidney transplants). However, tissue transplants can only be performed if there are enough donors available to attend to patients in need. Unfortunately, there are too few organ and tissue donors in Germany. All steps involved in processing donor tissues must be performed in accordance with the highest quality standards. All tasks and measures are aimed at improving patient care in the surgical units that are to be supplied. Cornea banks are subject to complex requirements, whose implementation is essential in terms of both infrastructure and personnel. The analysis and identification of essential topics reveal central fields of action that are decisive for implementing the challenges facing cornea banks. Questions of employee qualification, strategic questions due to new transplantation techniques, and changes in the societal perception of organ and tissue donation require the development of strategies that should have a holistic and sustainable effect.Keratoplastiken sind Gewebetransplantationen und als solche von Organtransplantationen (z. B. Herz- oder Nierentransplantationen) zu unterscheiden. Doch auch Gewebetransplantationen können nur durchgeführt werden, wenn Spender in ausreichender Zahl zur Verfügung stehen, um den Bedarf von erkrankten Patienten zu decken. Bedauerlicherweise gibt es in Deutschland zu wenig Organ- und Gewebespender. Für die Bearbeitung der Spendergewebe gelten grundsätzlich hohe Ansprüche an die Qualität und die sachgerechte Durchführung aller Arbeitsschritte. Alle Aktivitäten und Maßnahmen zielen darauf ab, die Patientenversorgung in den zu versorgenden operativen Zentren zu verbessern. Eine Hornhautbank unterliegt dabei gesetzlichen Anforderungen, deren Umsetzung sowohl infrastrukturell als auch personell bedeutsam sind. Fragen der Mitarbeiterqualifikation, strategische Fragen aufgrund von veränderten Transplantationstechniken, Veränderungen in der gesellschaftlichen Wahrnehmung von Organspenden und Gewebespenden fordern die Entwicklung von Planungen, die ganzheitlich und nachhaltig wirken sollten.

Technological progress and societal change are transforming medicine, and cornea banks are no exception. New infectiological factors, statutory requirements, management concepts, globalisation and digitalisation are also influencing how such facilities will operate in the future. The goal of providing high quality material to patients with corneal disease remains unaltered. The present article seeks to shed light on the type of material this will involve and under what circumstances it is to be obtained.Technologischer Fortschritt und gesellschaftliche Entwicklungen verändern die Medizin und machen auch vor Hornhautbanken nicht Halt. Neue infektiologische Aspekte, gesetzliche Vorgaben, Managementkonzepte, Globalisierung und Digitalisierung beeinflussen zusätzlich die zukünftigen Arbeitsinhalte dieser Einrichtungen. Das Ziel einer Bereitstellung von hochwertigem Material zur Versorgung von Patienten mit Hornhauterkrankungen bleibt bestehen. Welche Art von Material dies sein kann und unter welchen Rahmenbedingungen es entsteht soll der vorliegende Beitrag beleuchten.

Stummer et al. established fluorescence-guided surgery (FGS) for glioblastoma (GBM) using 5-aminolevulinic acid (5-ALA). Its metabolite, protoporphyrin IX (PPIX), is also a photosensitizer and can be used for photodynamic therapy (PDT) using a laser beam of 635 nm. The porphyrin derivate verteporfin (VP) was discovered to have properties to penetrate the brain, pharmacologically target glioma cells, and is approved for PDT of choroidal neovascularization in wet age-related macular degeneration at 689 nm.To elucidate whether GBM cell lines are susceptible to PDT with second-generation photosensitizer VP.Human glioma cell lines LN229, HSR-GBM1, and a low-passage patient-derived GBM cell line P1 were treated with variable concentrations of VP for 24 h, followed by PDT at 689 nm using a diode laser light. Cell viability was measured using the MTT assay and VP uptake was measured using a desktop cytometer.Significantly higher cell death following PDT with VP compared to VP treatment alone or no treatment was detected in all cell models (LN229, HSR-GBM1, P1). Flowcytometric measurements revealed a concentration-dependent cellular uptake of VP after 24 h incubation up to 99% at 10 µM (HSR-GBM1).This study demonstrates that PDT with VP causes cell death in GBM cells at marginal concentrations. Additionally, red spectrum fluorescence was detected at therapeutic concentrations in all cell lines, validating the cellular uptake of VP in GBM cells. VP, therefore, is not only a potential drug for targeting GBM pharmacologically but can be used as an optical imaging dye in surgery and photosensitizer to make GBM susceptible to PDT.

In the case of thermal or caustic burns of the ocular surface, loss of limbal epithelial stem cells leads to compromised self-renewal of the corneal epithelium. This results in permanent loss of vision. In these situations, transplantation of cultured limbal epithelial cells on an amniotic membrane or fibrin gel as substrate (Holoclar®) can help to regenerate the corneal surface. The required cells are obtained from the healthy partner eye, if available. Adult stem cells from other parts of the body potentially serve as alternative cell sources: hair follicles, oral mucosa, mesenchymal stromal cells, or induced pluripotent stem cells (originally, e.g., skin fibroblasts). The reprogramming of such cells can be achieved with the help of transcription factors. In addition, work is being done on biosynthetic or synthetic matrices, which not only serve as substrate material for the transplantation but also support the functional properties of these cells (self-renewal, corneal epithelial-typical phenotype).Bei einer Verbrennung oder Verätzung der Augenoberfläche kann der Verlust von Limbusepithelstammzellen zu einer kompromittierten Selbsterneuerung des Hornhautepithels führen. Hierdurch kommt es zu einem dauerhaften Sehverlust. In diesen Situationen kann die Transplantation von kultivierten Limbusepithelzellen auf einer Amnionmembran oder einem Fibringel als Träger (Holoclar®, Holostem Terapie Avanzate s.r.l., Modena, Italien) zu einer Regeneration der Hornhautoberfläche beitragen. Die erforderlichen Zellen werden dem gesunden Partnerauge entnommen – sofern vorhanden. Alternative Zellquellen sind adulte Stammzellen aus anderen Körperregionen (Haarfollikel, Mundschleimhaut, mesenchymale Stromazellen) oder induzierte pluripotente Stammzellen (ursprünglich z. B. Hautfibroblasten). Das Umprogrammieren solcher Zellen kann unter Zuhilfenahme von Transkriptionsfaktoren erfolgen. Zusätzlich wird an biosynthetischen oder synthetischen Matrices gearbeitet, die nicht nur als Trägermaterial für die Transplantation dienen, sondern auch die Funktionen der Zellen (Selbsterneuerung, hornhautepitheltypischer Phänotyp) unterstützen sollen.

The demands on conjunctival replacement tissues are high: they need to be elastic, clinically compatible, surgically feasible and support goblet cell growth.This article provides an overview of currently applied conjunctival replacement tissues and those under investigation.Current publications on clinically applied conjunctival replacement tissues and substrates which are the subject of scientific research and those already tested in animal models are presented and discussed.Replacement tissues in clinical use are autologous and allogenic conjunctiva, nasal and oral mucous membranes, amniotic membrane and decellularized tissues. Autologous conjunctiva shows good results but is not suitable for large defects due to limited availability. In these cases autologous nasal and oral mucous membranes can be used; however, success is limited in cases of autoimmune diseases. Amniotic membranes are frequently applied clinically but goblet cell growth is limited. Different decellularized tissues are used clinically and goblet cell growth was found in vivo. Robust comparative studies are not yet available. Biological matrices such as fibrin, collagen, elastin, gelatin or hyaluronate and synthetic tissues from the group of polyesters are being investigated in the laboratory and in animal models. These studies show good epithelialization and goblet cell growth in vivo.Transplantation of conjunctiva, nasal and oral mucous membranes and amniotic membranes show satisfactory clinical results but exhibit individual weaknesses. Further studies in animal models and clinical settings are required to further evaluate the benefits of other matrices, such as cell-free tissues or other biological and synthetic matrices.HINTERGRUND: Die Ansprüche an ein Bindehautersatzgewebe sind hoch: Es muss elastisch, klinisch verträglich sowie chirurgisch handhabbar sein und das Becherzellwachstum unterstützen.Diese Arbeit gibt einen Überblick über aktuell verwendete und in der Forschung untersuchte Bindehautersatzgewebe.Es erfolgen eine Darstellung und Diskussion aktueller Publikationen zum klinischen Bindehautersatz und zu Geweben, die sich in der wissenschaftlichen Erforschung befinden und bereits im Tiermodell getestet wurden.Ersatzgewebe in der klinischen Anwendung sind autologe oder allogene Bindehaut, Nasen- oder Mundschleimhaut, Amnionmembran und dezellularisierte Gewebe. Hierbei zeigt autologe Bindehaut sehr gute Ergebnisse, ist aber aufgrund der limitierten Verfügbarkeit nicht für große Defekte geeignet. Hier kann autologe Nasen- und Mundschleimhaut angewendet werden. Allerdings sind die Erfolgsraten bei Autoimmunerkrankungen reduziert. Amnionmembran wird klinisch vielfach eingesetzt, hat aber den Nachteil, dass keine vitalen Zellen transplantiert werden und dass das Becherzellwachstum eingeschränkt ist. Des Weiteren werden dezellularisierte Gewebe klinisch angewendet, und in vivo war Becherzellwachstum nachweisbar. Aussagekräftige vergleichende Studien für den Bindehautersatz fehlen allerdings. Biologische Matrices wie Fibrin, Kollagen, Elastin, Gelatine oder Hyaluronat und synthetische Gewebe aus der Gruppe der Polyester werden im Labor und im Tiermodell untersucht. Studien zeigen eine gute Epithelialisierung und Becherzellbesiedlung in vivo.Die Bindehaut‑, Nasen‑/Mundschleimhaut- und Amnionmembrantransplantation zeigen klinisch befriedigende Ergebnisse, haben aber individuelle Nachteile. Es bedarf weiterer Untersuchungen im Tiermodell und in der Klinik, um andere Matrices wie zellfreie Gewebe oder weitere biologische und synthetische Matrices genauer zu evaluieren.

Ocular involvement in mucous membrane pemphigoid (MMP) is relatively rare, with a prevalence of 25 cases per million population, equating to approx. 2,100 patients throughout Germany. Diagnosis can be difficult - especially in cases of isolated ocular involvement - and treatment can be complex and lengthy. Immunosuppressants or immunomodulatory drugs are often used. Due to the complexity of diagnosis and treatment, MMP patients are usually referred to specialized centers. The aim of this project was to evaluate the current care situation of patients with ocular MMP in Germany.A paper-based survey was designed and sent to all university eye clinics and other specialized centers in Germany in April 2020. The survey asked about the existence of a specialized outpatient service, the total annual number of patients with MMP, the annual number of newly diagnosed patients, any interdisciplinary collaboration for diagnostic or therapeutic purposes, as well as the local and systemic therapy used.Of a total of 44 clinics, 28 (64%) responded, reporting a total average of 27 ± 42 (0 - 200) patients and 3.6 ± 2.2 (0 - 10) new cases per year. This corresponds to a total of 741 patients. Only nine (32%) of the responding clinics offer specialized MMP clinics. 93% of the centers collaborate with the local dermatology department. 79% perform serological and histological diagnostics in-house. About half of the centers (n = 16) apply a standardized treatment regime. Systemic glucocorticoids (66.7%) are most commonly used, followed by mycophenolate mofetil and dapsone (57.1%), rituximab (33.3%), azathioprine and cyclophosphamide (28.6%), as well as methotrexate (19.0%). The least frequently used treatment is intravenous immunoglobulin (14.3%).This survey of German ophthalmology departments obtained data from about one third of the estimated total cohort of all patients with MMP in Germany. These are presumed to be exclusively patients with at least one ocular involvement. The complex care of these patients is usually provided in collaboration with a dermatologist and with the use of systemic anti-inflammatory medication. Currently, an ophthalmological MMP register is being established to better record the epidemiology and care situation of this rare disease in Germany and to improve it in the long term.Die okuläre Beteiligung bei vernarbendem Schleimhautpemphigoid (SHP) ist mit einer Prävalenz von 25 Fällen je 1 Mio. Einwohner und damit ca. 2100 Patienten in ganz Deutschland selten. Die Diagnosestellung kann – besonders in Abwesenheit anderer Beteiligungen – schwierig und die Therapie komplex und langwierig sein. Nicht selten kommen Immunsuppressiva zum Einsatz. Aufgrund der Komplexität von Diagnose und Therapie sind SHP-Patienten meist an entsprechend spezialisierte Zentren angebunden. Ziel dieses Projektes war die Erfassung der aktuellen augenärztlichen Versorgungssituation von Patienten mit SHP in Deutschland.Eine papierbasierte Umfrage wurde konzipiert und im April 2020 an alle Universitätsaugenkliniken und weitere potenzielle Zentren versandt. Gefragt wurde nach dem Bestehen einer spezialisierten Sprechstunde, der jährlichen Gesamtzahl der betreuten Patienten, der jährlichen Anzahl von neu diagnostizierten Patienten, den klinischen Kooperationspartnern in Diagnostik und Therapie sowie nach der angewendeten lokalen und systemischen Therapie.Von insgesamt 44 angeschrieben Kliniken erfolgten 28 (64%) vollständige Rückmeldungen. Im Mittel werden in den Kliniken 27 ± 42 (0 – 200) Patienten betreut und jährlich pro Zentrum 3,6 ± 2,2 (0 – 10) neue Fälle diagnostiziert. Dies entspricht einer Gesamtpatientenzahl von 741 Patienten. Lediglich 9 (32%) der antwortenden Kliniken bieten eine spezialisierte SHP-Sprechstunde an. 93% der Zentren kooperieren mit der lokalen Klinik für Dermatologie. 79% führen die serologische und histologische Diagnostik intern durch. Etwa die Hälfte der Zentren (n = 16) wendet ein standardisiertes Therapieschema an. Systemisch werden Glukokortikoide (66,7%) am häufigsten verwendet, gefolgt von Mycophenolatmofetil und Dapson (57,1%), Rituximab (33,3%), Azathioprin und Cyclophosphamid (28,6%) sowie Methotrexat (19,0%). Am seltensten werden i. v. Immunoglobuline eingesetzt (14,3%).Mit dieser Umfrage unter deutschen augenärztlichen Zentren wurden Daten von etwa einem Drittel der geschätzten Gesamtzahl aller in Deutschland an einem SHP erkrankten Menschen erhoben. Dabei handelt es sich vermutlich ausschließlich um Patienten mit mindestens einer okulären Beteiligung. Aktuell wird eine augenärztliche SHP-Registerstudie etabliert, um die Epidemiologie und Versorgungssituation besser zu erfassen und langfristig zu verbessern.

Traditionally, corneal endothelium is considered post-mitotic, which means that spontaneous regeneration after loss of endothelial cells (e.g., in the context of Fuchs endothelial corneal dystrophy) is not expected. Nevertheless, it has been observed in several cases that an edematous cornea can clear after removal of a smaller area of Descemet's membrane. The posterior aspect of this area becomes covered again by corneal endothelial cells. This has resulted in a new surgical approach that does not involve a posterior lamellar graft: "Descemet stripping only". At the same time, data from in vitro experiments suggest that Rho kinase inhibitors (ROCK inhibitors) are pharmacological agents that promote proliferation and migration of corneal endothelium, and thus have a positive influence on healing.Traditionell wird das Hornhautendothel als postmitotisch angesehen, sodass von einer spontanen Regeneration nach Verlust von Endothelzellen (z. B. im Rahmen der Fuchsʼschen endothelialen Hornhautdystrophie) nicht ausgegangen wird. Dennoch konnte in mehreren Fällen beobachtet werden, dass eine ödematöse Hornhaut nach Entfernung eines kleineren Areals der Descemet-Membran aufklart. Der posteriore Aspekt dieses Bereichs wurde von Hornhautendothelzellen wieder bedeckt. Hieraus entstand ein operatives Therapiekonzept ohne posteriores lamelläres Transplantat: „Descemet stripping only“. Zugleich legen Daten aus In-vitro-Versuchen nahe, dass Rho-Kinase-Inhibitoren (ROCK-Inhibitoren) pharmakologische Wirkstoffe sind, die eine Proliferation und Migration des Hornhautendothels fördern und somit den Heilungsverlauf nach Descemet-Stripping positiv beeinflussen. Mittlerweile liegen erste Daten aus kleineren prospektiven, kontrollierten klinischen Studien vor, die durchweg gute Ergebnisse nach Descemet-Stripping und ROCK-Inhibitor-Augentropfen berichten. Offene Fragen bestehen jedoch derzeit noch hinsichtlich der Charakteristika der Patientengruppe, die von diesem Therapieansatz besonders profitiert und hinsichtlich der Nachhaltigkeit eines Therapieerfolgs.
