Abstract

In this paper we extend the work in the conference paper 'On the Controllability and Observability of Heterogeneous Networked Systems with distinct node dimensions and inner-coupling matrices' wherein the controllability and observability of a heterogeneous networked system with distinct node dimensions were studied. This paper adds to the conference paper a necessary and sufficient condition for controllability of the networked system. The result demonstrates the dependence of controllability of the network on factors like network topology, inner interactions among nodes and nodal dynamics. The result is formulated by characterizing the left eigenvectors of the network state matrix. Another necessary and sufficient condition for controllability, which is a reformulation of the \textit{Popov-Belevitch-Hautus} controllability condition, a necessary and sufficient condition for observability of the networked system and certain necessary conditions for controllability of the networked system are the other results established in this paper. 2. Exact Null Controllability of Non-Autonomous Conformable Fractional Semi-Linear Systems with Nonlocal Conditions

Abstract

This paper investigates the existence and uniqueness of the mild solutions and the exact null controllability for a class of non-autonomous parabolic evolution systems with nonlocal conditions in Hilbert spaces. We present sufficient conditions for achieving exact null controllability in these systems using the theory of linear evolution systems and the Schauder fixed point theorem. Importantly, our results do not require the compactness or Lipschitz conditions for the function \( g \) in the nonlocal conditions, which are often needed in other studies. Abstract

BackgroundThe impact of the 2017 American College of Cardiology (ACC)/American Heart Association (AHA) guidelines for diagnosis and management of hypertension on the prevalence of hypertension in India is unknown.MethodsWe analyzed data from the Cardiac Prevent 2015 survey to estimate the change in the prevalence of hypertension. The JNC8 guidelines defined hypertension as a systolic blood pressure of ≥140 ​mmHg or diastolic blood pressure of ≥90 ​mmHg. The 2017 ACC/AHA guidelines define hypertension as a systolic blood pressure of ≥130 ​mmHg or diastolic blood pressure of ≥80 ​mmHg. We standardized the prevalence as per the 2011 census population of India. We also calculated the prevalence as per the World Health Organization (WHO) World Standard Population (2000-2025).ResultsAmong 180,335 participants (33.2% women), the mean age was 40.6 ​± ​14.9 years (41.1 ​± ​15.0 and 39.7 ​± ​14.7 years in men and women, respectively). Among them, 8,898 (4.9%), 99,791 (55.3%), 35,694 (11.9%), 23,084 (12.8%), 9,989 (5.5%) and 2,878 (1.6%) participants belonged to age group 18-19, 20-44, 45-54, 55-64, 65-74 and ​≥ ​75 years respectively. The prevalence of hypertension according to the JNC8 and 2017 ACC/AHA guidelines was 29.7% and 63.8%, respectively- an increase of 115%. With the 2011 census population of India, this suggests that currently, 486 million Indian adults have hypertension according to the 2017 ACC/AHA guidelines, an addition of 260 million as compared to the JNC8 guidelines.ConclusionAccording to the 2017 ACC/AHA guidelines, 3 in every 5 Indian adults have hypertension.

Abstract

There is ongoing debate around rate versus rhythm control strategies for managing atrial fibrillation (AF), however, much of the data comes from Western cohorts. Kerala‐AF represents the largest prospective AF cohort study from the Indian subcontinent. To compare 12‐month outcomes between rate and rhythm control strategies. Patients aged ≥18 years with non‐transient AF were recruited from 53 hospitals across Kerala. Patients were stratified by rate or rhythm control. The primary outcome was a composite of all‐cause mortality, arterial thromboembolism, acute coronary syndrome or hospitalization due to heart failure or arrhythmia at 12 months. Secondary outcomes included bleeding events and individual components of the primary. Predictors of the composite outcome were analysed by logistic regression. A total of 2901 patients (mean age 64.6 years, 51% female) were included (2464 rate control, 437 rhythm control). Rates of the primary composite outcome did not differ between groups (29.7% vs 30.0%; p= .955), nor did any component of the primary. Bleeding outcomes were also similar (1.6% vs 1.9%; p= .848). Independent predictors of the primary composite outcome were older age (aOR 1.01; p= .013), BMI <18 (aOR 1.51; p= .025), permanent AF (aOR 0.78; p= .010), HFpEF (aOR 1.40; p= .023), HFrEF (aOR 1.39; p= .004), chronic kidney disease (aOR 1.36; p< .001), and prior thromboembolism (aOR 1.31; p= .014). In the Kerala‐AF registry, 12‐month outcomes did not differ between rate and rhythm control cohorts. No significant difference was found between rate and rhythm control approaches to atrial fibrillation management in the 1‐year follow‐up of the Kerala‐AF cohort. Independent predictors of adverse outcomes included age, underweight BMI, heart failure, and chronic renal failure.

36. Hematological and Biochemical Profile in Acute ST Elevation Myocardial Infarction and Correlation with Early Complications.

Abstract

Background. The world’s biggest killer is ischemic heart disease, responsible for 16% of the world’s total deaths. Transient alterations in the hematological and biochemical profile are known to occur of patients admitted with acute ST elevation myocardial infarction. Some of these parameters may help to identify high risk patients with increased likelihood of developing adverse cardiac events. Identifying such high-risk patients help the treating physician to plan more aggressive treatment strategies for these patients resulting in improved clinical outcome. Objective. To study the baseline hematological and biochemical laboratory parameters in patients admitted with acute ST elevation myocardial infarction and correlate with their in-hospital outcome. Methodology. 554 consecutive patients admitted to the Intensive Coronary Care Unit, Government Medical College Hospital, Kottayam with first Acute ST Elevation Myocardial Infarction during a period of one year were included in the study. Blood samples were drawn at the time of admission for estimation of hemoglobin, total and differential WBC counts, blood sugar, blood urea, serum creatinine, serum electrolytes and lipid profile. Patients were treated according to the standard protocol and any adverse cardiac events during the hospitalization period was documented and correlated with the laboratory parameters. Results. There was statistically significant association between the following laboratory parameters and development of in-hospital complications: lower hemoglobin values (p =0.003), higher total white blood corpuscle count (p =0.05), higher Erythrocyte Sedimentation Rate (p =0.02) and impaired renal function (p =0.0). Conclusion. In the present study, hematological and biochemical profile in 554 consecutive cases of first Acute ST Elevation Myocardial Infarction were analyzed and found to have significant correlation with major adverse cardiac events during the hospitalization period. Abstract

In this manuscript, we have established conditions for the existence and uniqueness of mild and classical solutions to the fractional order Cauchy problem by including and without including impulses over the completed norm linear space (Banach space). Conditions are established using the concept of generators and the generalised Banach fixed point theorem, which are weaker conditions than the previously derived conditions. We have also established the conditions under which a mild solution to the problem gives rise to a classical solution to the given problem. Finally, illustrations of the existence and uniqueness of the solution are provided to validate our derived results.
