IntroductionInternational students make significant contributions to their host institutions and countries. Yet research shows that not all international students have the financial means to fend for themselves and meet their financial obligations for the entire study programme. Such students are at significant risk of food insecurity. The objective of this scoping review is to synthesise available information on the factors related to food insecurity among international students studying at postsecondary educational institutions and identify the types of food insecurity interventions that have been implemented to address this issue.Methods and analysisThe Joanna Briggs Institute scoping review methodology will be used to guide this scoping review, and we will search the following databases: MEDLINE (through Ovid), CINAHL (EBSCO), PubMed, ERIC (via Ovid), PROSPERO and ProQuest. The titles, abstracts, and subsequently full texts of the selected papers will then be screened against the inclusion criteria. Data from articles included in the review will be extracted using a data charting form and will be summarised in a tabular form. Thematic analysis will be used to identify common themes that thread through the selected studies and will be guided by the steps developed by Terryet al.Ethics and disseminationSince this project entails a review of available literature, ethical approval is not required. The findings will be presented at academic conferences and published in a peer-reviewed journal. To make the findings more accessible, they will also be distributed via digital communication platforms.

BackgroundHIV-related stigma negatively impacts HIV prevention, care, and treatment particularly children and adolescents in sub-Saharan Africa. Interventions that are culturally-grounded and relevant to address the root causes of stigma may reduce the stigma experienced by HIV positive and affected young people. This study in a post-conflict, rural setting in Omoro District, Uganda, will create and evaluate a transformative education and arts-based HIV-related stigma intervention rooted in local cultural knowledge to reduce stigma and improve HIV prevention and care for those living with HIV. The study will employ a mixed-methods, stepped-wedge cluster-randomized controlled trial, and community-based participatory research approach. The intervention will be delivered to children and youth in schools by Elders, with the support of teachers, through the transfer of local cultural knowledge and practices. The arts-based intervention aims to re-establish the important cultural and social role of Elders within a community that has suffered the loss of inter-generational transfer of cultural knowledge due to a 25-year civil war.MethodsA formative research phase consisting of interviews with students, teachers and Elders will inform the intervention and provide data for study objectives. Workshops will then be delivered to Elders and teachers in participating schools to build capacity for transformative, arts-based, educational workshops with students in the classroom. Government-funded schools located in Omoro District will be randomized into 3 blocks, each comprised of two primary and two secondary schools (n=1800 students). Schools will be randomly allocated to sequences of 8-week time periods in which they will crossover from control to the intervention condition. A process evaluation will be implemented throughout the study to evaluate intervention development, implementation, and effects.DiscussionThis study will generate comprehensive, in-depth research and evaluation data that will be integrated to inform an effective and sustainable protocol for implementing arts-based HIV-stigma interventions for young people in school settings. Background Early-onset substance use is a risk factor for continued use, dependency, and poor long-term health outcomes. Indigenous youth are more likely to engage in early-onset substance use than their non-Indigenous counterparts. In Canada, culturally appropriate prevention programs are needed for Indigenous youth in elementary schools. Therefore, this scoping review aims to explore the published, international literature examining school-based substance use prevention programs for Indigenous children aged 7–13. Main text Methods: This scoping review followed a six-step approach: 1) identifying the research questions, 2) identifying relevant studies, 3) selecting the studies, 4) charting the data, 5) collating, summarizing, and reporting the results, and 6) consulting with experts. The review was reported using guidelines from Preferred Reporting Items for Systematic reviews and Meta-Analyses extensions for Scoping Reviews (PRISMA-ScR). Results: Eleven articles (3 Canadian; 7 American and; 1 Australian) were included in the review. The prevention programs they studied were based on existing research or were adapted from existing interventions. The programs were tailored to each communities’ culture by including Indigenous stakeholders in developing or adapting prevention programs to be culturally safe and responsive. The articles evaluated the programs’ Effectiveness in changing student knowledge, attitudes, and behaviors using pre- and post-intervention surveys, randomized control trials, longitudinally designed analysis, and mixed methods. Mixed quantitative findings and qualitative findings highlighted the programs’ value in building community capacity and fostering cultural revitalization. Conclusion This review highlights best practices for developing school-based substance use prevention programs for Indigenous youth. Findings suggest that prevention programs should be culturally responsive and provide students with the knowledge and skills to prevent and manage substance use in real-life situations. Making Indigenous beliefs, values, languages, images, and worldviews central to the prevention curriculum enhanced the Effectiveness, appropriateness, and sustainability of prevention programs. Indigenous communities are best positioned to facilitate cultural tailoring without compromising the fidelity of evidence-based prevention programs.

Background:In Canada, the rate of opioid use, opioid use disorder (OUD), and associated mortality and morbidity are higher among Indigenous Peoples than the general population. Indigenous Peoples on medications for opioid use disorders (MOUD) often face distinct barriers that hinder their clinical progress, leading to treatment attrition.Methods:We used a social-ecological model to inquire into clients’ experiences with a history of treatment failure for OUD. We used exploratory qualitative research to engage 22 clients with a history of OUD treatment dropouts and who are currently on MOUD. In-depth, semi-structured interviews lasting an average of 30 minutes were conducted on-site.Results:We identified 4 themes from the study: (a) risk for substance use; (b) factors sustaining substance use; (c) factors leading to treatment, and (d) treatment failure and re-enrollment.Conclusion:Using a socio-ecological model helps to understand factors that influence an individual’s risk for OUD, decision to pursue treatment, and treatment outcomes. Furthermore, social ecological model also creates possibilities to develop supportive, multilevel interventions to prevent OUD risks and support for clients on MOUD. Such interventions include mitigating adverse childhood experiences, supporting families, and creating safe community environments.

Background Immigrants from culturally, ethnically, and linguistically diverse countries face many challenges during the resettlement phase, which influence their access to healthcare services and health outcomes. The “Healthy Immigrant Effect” or the health advantage that immigrants arrive with is observed to deteriorate with increased length of stay in the host country. Methods An exploratory qualitative design, following a community-based research approach, was employed. The research team consisted of health researchers, clinicians, and community members. The objective was to explore the barriers to healthcare access among immigrants with limited English language proficiency. Three focus groups were carried out with 29 women and nine men attending English language classes at a settlement agency in a mid-sized city. Additionally, 17 individual interviews were carried out with healthcare providers and administrative staff caring for immigrants and refugees. Results A thematic analysis was carried out with transcribed focus groups and healthcare provider interview data. Both the healthcare providers and immigrants indicated that limited language proficiency often delayed access to available healthcare services and interfered with the development of a therapeutic relationship between the client and the healthcare provider. Language barriers also impeded effective communication between healthcare providers and clients, leading to suboptimal care and dissatisfaction with the care received. Language barriers interfered with treatment adherence and the use of preventative and screening services, further delaying access to timely care, causing poor chronic disease management, and ultimately resulting in poor health outcomes. Involving untrained interpreters, family members, or others from the ethnic community was problematic due to misinterpretation and confidentiality issues. Conclusions The study emphasises the need to provide language assistance during medical consultations to address language barriers among immigrants. The development of guidelines for recruitment, training, and effective engagement of language interpreters during medical consultation is recommended to ensure high quality, equitable and client-centered care.

Background Patients with opioid use disorder (OUD) often have complex health care needs. Methadone is one of the medications for opioid use disorder (MOUD) used in the management of OUDs. Highly restrictive methadone treatment—which requires patient compliance with many rules of care—often results in low retention, especially if there is inadequate support from healthcare providers (HCPs). Nevertheless, HCPs should strive to offer patient-centred care (PCC) as it is deemed the gold standard to care. Such an approach can encourage patients to be actively involved in their care, ultimately increasing retention and yielding positive treatment outcomes. Methods In this secondary analysis, we aimed to explore how HCPs were applying the principles of PCC when caring for patients with OUD in a highly restrictive, biomedical and paternalistic setting. We applied Mead and Bower’s PCC framework in the secondary analysis of 40 in-depth, semi-structured interviews with both HCPs and patients. Results We present how PCC's concepts of; (a) biopsychosocial perspective; (b) patient as a person; (c) sharing power and responsibility; (d) therapeutic alliance and (e) doctor as a person—are applied in a methadone treatment program. We identified both opportunities and barriers to providing PCC in these settings. Conclusion In a highly restrictive methadone treatment program, full implementation of PCC is not possible. However, implementation of some aspects of PCC are possible to improve patient empowerment and engagement with care, possibly leading to increase in retention and better treatment outcomes.

IntroductionCanada has one of the highest rates of problematic opiate and alcohol use in the world. Globally, Canada was the second country that legalized marijuana for non-medical use. As Canada is an immigrant-receiving country, newcomers and immigrants contend with a substance use landscape that was likely absent in their countries of origin. Although immigrants have lower rates of substance use than the host population, the risk of substance use, especially among youth, increases with acculturation and peer pressure. While parents are best placed to mitigate the risks for substance use among their youth, immigrant parents often do not have the knowledge and skills to do so. Therefore, culturally adaptable family based interventions need be explored to build immigrant parents’ capacities to mitigate substance use risks.Aim and purposeThe aim of this scoping review is to explore family based substance use prevention interventions for immigrant youth, which will be guided by two questions:What is known about family based interventions for preventing immigrant adolescents’ substance use?What are the features and study results of these intervention protocols?Methods and analysisWe will apply Arksey and O’Malley’s procedure for reporting scoping review and report study findings based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines for scoping reviews.DiscussionWe hope that the knowledge translation emanating from this review will increase immigrant parents’ knowledge of substance use and enable them to effectively intervene to prevent substance use among their youth. We also hope that this work can inform policy development on best practices for substance use prevention and for the creation of culturally sensitive programmes and services for immigrant youth.

Clients on methadone maintenance treatment (MMT) have high attrition rates that are attributed to personal and system-related factors. To develop supportive interventions for these clients, it is imperative to understand social demographic characteristics and challenges that clients in the MMT program face.This article aims to describe (a) the sociodemographic characteristics and clinical profiles of clients in a MMT program, (b) factors that impact their positive clinical outcomes, and (c) the study's implications for practice.A retrospective review of 101 randomly selected electronic medical records representing one third of all the records were examined for sociodemographic characteristics, clinical profiles, and outcomes. Descriptive statistics were used to analyze these variables. Interviews with 18 healthcare providers focusing on their experiences of caring for clients in the MMT program were analyzed thematically.The average age of clients on MMT is 35.5 years. Clients had early exposure to alcohol and drugs, and at the time of enrollment to the program, they presented with complex healthcare needs, borne from chronic use, and exposure to adverse traumatic events. Personal and systemic factors impact clients' recovery. These include poverty, homelessness, and inadequate healthcare services. Understanding sociodemographic characteristics, clinical profiles, and clients' challenges is central to the development of supportive interventions that enhance retention to care and recovery.

IntroductionPrescribing methadone as part of opioid agonist therapy is a common treatment approach to manage opioid use disorder. Unfortunately, many clients prematurely discontinue opioid agonist therapy because of restrictions attached to the therapy.PurposeThe purpose of this study was to explore health-care provider experiences as they worked with clients on opioid agonist therapy in a western Canadian city.MethodsIn this descriptive, qualitative study, we interviewed 18 health-care providers working in an opioid agonist therapy setting. The focus of the interviews was on the organization of opioid agonist therapy care at their clinic, their personal experiences and challenges faced when providing care to their clients. Interviews were recorded electronically and transcribed verbatim and thematic analysis was completed using NVIVO software.ResultsThe following three themes emerged from the data relate to the care organization and health care provider (HCP)-clients dynamics. These are: (1) fragmentation of care for a complex problem, (2) enforcing compliance to treatment, and (3) the importance of a therapeutic alliance to improve overall care.ConclusionThe opioid agonist therapy model is biocentric and emphasizes abstinence which can create tension between providers and clients. Making the model of care more patient centred might help to improve client retention rates and successful treatment outcomes.

Background Growing up in an environment where substance use is prevalent creates a climate for adverse childhood experiences. These experiences can contribute to mental and psychological problems later in life. Methods This study used an exploratory study design to explore the experiences of individuals whose upbringing was influenced by substance use at home or who had parents with addiction problems. Results Four themes shaped the participants' lives: (1) impact of substance use at home on children; (2) school influences on substance use; (3) aggravating traumatic life experiences, and d) rebuilding a battered life. Conclusion Growing up in homes with addictions created an environment for traumatic events and encounters. Participants adopted diverse strategies to cope with these experiences, such as leaving home, using substances early in life, and dropping out of school. Screening and intervention for childhood trauma should be encouraged both in schools and community health settings. Such interventions can reduce substance use among school children to deal with ensuing trauma.

This study examined the feasibility and acceptability of an evidence-based HIV prevention program for men who have sex with men (MSM) in Ghana through a participatory approach. The program involved 57 self-identified adult cisgender MSM and led by a community-based organization in collaboration with local nurses. We used an explanatory mixed-method design to evaluate the program. We computed descriptive statistics, relative frequency and paired t-test for the survey data and subjected the focus groups data to summative content analysis. Five key themes from the qualitative data indicated strong evidence of acceptability and efficacy of the program among MSM. The program contributed to building social support networks, sense of social justice among MSM, and facilitated the development of personalized HIV prevention menus by the participants. There were no statistically significant differences in scores between the baseline and 1-week post-program assessments. Nonetheless, we observed increases in HIV testing (from 4% to 17%) and increases in relative frequency of condom use for anal, oral, and vaginal sex. The program served as an example of a successfully implemented culturally grounded intervention that has a potential to increasing HIV and STI awareness and prevention among MSM in Ghana and other highly stigmatized environments.

Background: The impact of addiction extends beyond the individual using a substance. Caring for an individual with addiction creates persistent stressful circumstances that cause worry, anger, depression, shame, guilt, anxiety, and behavioral problems within the family unit.The aim of the study: The paper aims to explore the psychological impact when caring for individuals with active addictions and self-care strategies caregivers employ.Methods: The study adopted an exploratory qualitative design and used purposeful sampling to recruit study participants through posters, word-of-mouth, and e-mail. Study participants were required to have a family member living with addiction and not be active substance users themselves. In-depth individual interviews were conducted among 21 participants to gather their experiences, meanings, and interpretation of how it feels to provide care for a family member suffering from addiction and substance use.Results: Three themes, whose overarching focus is the pains that living and caring for a family with addiction caused the participants, are discussed. These included 1) caring experiences, 2) factors shaping caring experiences, and 3) self-care strategies.Conclusion: The stress associated with caring for individuals with addictions impacts the caregiver’s physical and mental health. Specific care modalities targeting caregivers need to be developed to address the health impact and to support self-care.

Men who have sex with men (MSM) in Ghana are at an increased risk of contracting HIV. Understanding the social networks of MSM may support the development of HIV prevention strategies for this unique population. This article explores the structure and function of the social networks of MSM from 22 focus groups drawn from two urban and one rural setting in Ghana. Gaining insights into the characteristics of these networks will allow health care providers to design HIV prevention efforts and increase access to these programs.

Prince Albert, Saskatchewan, is experiencing a substance use and addiction crisis with devastating consequences. To engage local stakeholders on substance use and addiction issues, nurse researchers at the University of Saskatchewan, Prince Albert Campus, planned and organized a one-day community engagement and knowledge exchange forum. The forum provided the opportunity for interested community groups, members and individuals to share their experiences and to explore novel ways to prevent and respond to the substance abuse and addiction challenges in the region. Participants included community leaders, people and families living with addiction, service providers, local stakeholders, health professionals, researchers and Indigenous Elders. This paper describes the process and outcomes of this event and describes the role of nurse scholars in leading these efforts.

IntroductionPrevious research demonstrated that substance use continues to be one of the most complex and prevalent problems among African, Caribbean and Black (ACB) people. A number of studies were conducted to characterise substance use patterns in this population. To our knowledge, this is the first known review in Canada characterising substance use disorders on ACB people.This scoping review seeks to answer the following research questions: What characterises substance use disorders among ACB people in Canada? What are the different types and prevalence of substance use among ACB people in Canada? Do ACB people in Canada use more than one substance? What factors are associated with substance use among ACB people in Canada? What are the health and social impacts of substance use in ACB people in Canada?Methods and analysesThis study will use the methodological framework for scoping reviews developed by Arksey and O’Malley. We will search electronic bibliographic databases including Ovid MEDLINE, PsycINFO and CINAHL. We will limit our search to English articles published between 2000and2019. In addition, we will conduct a grey literature search. Two investigators will independently screen citations and full-text articles. Our findings will be reported according to the Preferred Reporting Items for Systematic Reviews and Meta-Analysis for scoping reviews guidelines. We will provide a descriptive summary of the studies and summarise the findings with respect to the outcomes and report any gaps that might require further investigation.Ethics and disseminationOur proposed study does not involve human participants; therefore, research ethics approval is not required. This study will provide evidence that will inform the development of strategies for appropriate interventions, as well as policy and further research. The results will be disseminated through publications in open access peer-reviewed journals, presentations at scientific meetings and to the lay public.

We describe salient individual and organizational factors that influenced engagement of registered nurses in a 12-month clinical mentorship intervention on HIV care in Canada. The intervention included 48 nurses and 8 people living with HIV (PLWH) who were involved in group-based and one-on-one informal mentorship informed by transformative learning theory. We evaluated the process of implementing the mentorship intervention using qualitative content analysis. The inclusion of PLWH as mentors, the opportunities for reciprocal learning, and the long-term commitment of individual nurses and partner organizations in HIV care were major strengths. Challenges included the need for multiple ethical approvals, the lack of organizational support at some clinical sites, and the time commitment required by participants. We recommend that clinical mentorship interventions in HIV care consider organizational support, adhere to the Greater Involvement of People Living with HIV/AIDS principles, and explore questions of professional obligations.

Best practices in HIV care have the potential to improve patient outcomes and inform practice. We conducted a systematic review of best practices in HIV care that were published from 2003 to 2013. Practices that demonstrated success in achieving desired results based on their objectives were included in the review. Two themes emerged from the eight articles reviewed: (a) the importance of linking newly diagnosed people living with HIV to care and (b) the role of integrated and comprehensive service provision in improving patient outcomes. Inconsistencies in reporting and arbitrary use of the term "best practices" were hurdles in this review.

Since the introduction of antiretroviral medications, HIV has been regarded as a chronic illness. However, people living with HIV continue to experience social consequences of HIV infection such as stigma, discrimination, violence, and other human rights violations. In this paper, we focus on the experiences of Atoti, a person living with HIV in Kenya. We argue that HIV remains a biographically disruptive and exceptional illness that is complicated by its invisibility and unpredictable trajectory. Based on Atoti’s experiences, we argue that Bury’s (1982) concept of biographical disruption, used to explain the social processes of a person suffering a chronic illness, does not fully capture the complexity of experiencing living with HIV. Focusing on life as a whole, rather than on the disease response and process as a biographical disruption, allows for a deeper appreciation of HIV’s complexity as a medical illness with major social ramifications.

Background The prevalence of HIV in Ghana is 1.3%, compared to 17% among men who have sex with men (MSM). There is limited empirical data on the current health care climate and its impact on HIV prevention services for Ghanaian MSM. The purposes of this study were to investigate (1) MSM’s experiences using HIV prevention resources, (2) what factors, including health care climate factors, influenced MSM’s use of prevention resources and (3) MSM self-identified strategies for improving HIV/sexually transmitted infection (STI) prevention among MSM in Ghanaian communities. Methods We conducted 22 focus groups (n = 137) with peer social networks of MSM drawn from three geographic communities in Ghana (Accra, Kumasi, Manya Krobo). The data were examined using qualitative content analysis. Interviews with individual health care providers were also conducted to supplement the analysis of focus group findings to provide more nuanced illuminations of the experiences reported by MSM. Results There were four major findings related to MSM experiences using HIV prevention resources: (1) condom quality is low, condom access is poor, and condom use is disruptive, (2) inaccurate information undermines HIV testing (3), stigma undermines HIV testing, and (4) positive attitudes towards HIV prevention exist among MSM. The main healthcare climate factors that affected prevention were that MSM were not free to be themselves, MSM were not understood by healthcare providers, and that MSM did not feel that healthcare providers cared about them. To improve HIV prevention MSM suggested increased education tailored to MSM should be provided to enable self-advocacy and that education and awareness are needed to protect human rights of MSM in Ghana. Conclusion MSM in Ghana are exposed to negative health care climates. Health care spaces that are unsupportive of MSM’s autonomy undermine the uptake of prevention measures such as condoms, HIV testing, and accurate sexual health education. These findings contribute to knowledge to inform development of HIV prevention interventions for MSM in Ghana, such as culturally appropriate sexual health education, and digital technology to connect individuals with resources supportive of MSM.

Body mapping, an artistic method of narrating experiences alongside people living with HIV/AIDS (PHAs), was used to educate first year nursing students about social and personal aspects of the illness, including stigma. Body mapping creates a safe space where individuals can discuss personal, emotional, cultural, political and socio-economic dimensions of their lives in relation to HIV/AIDS. The aim of this study is to understand how individuals from multiple perspectives experience body mapping as an educational tool, including a nursing student, an expert facilitator, a PHA and a course professor. The stories they live and tell motivate and inform the content of this study. We conducted three individual in-depth interviews with a student, a PHA and an expert facilitator on their experiences of participating in body mapping. The interviews were transcribed and analyzed thematically. Field and observational notes were also collected and reflections have been incorporated in this paper. Three themes were derived from our analyses: 1) stock taking narratives, where body mapping serves as a mediator for reflecting, accessing and organizing past experiences; 2) transactional narratives where body mapping creates safe spaces within which relationships are created, and experiences are shared, and 3) give and take narratives that connote transformation of self as a result of participating in body mapping. In conclusion, the body mapping exercise as an educational tool is an opportunity to share experiences, and to learn about and to shift attitudes surrounding HIV/AIDS. Thus, body mapping can be a valuable tool for HIV education for first year nursing students.

Nurses may have inadequate basic education and opportunities for continuing education in relation to HIV care. As well nurses may perpetuate and impose stigma. We developed, implemented and evaluated an educational intervention to reduce stigma and discrimination among nurses providing HIV care. The intervention used a mentorship model that brought experienced nurses in HIV care and people living with HIV together with nurses who wanted to learn more about HIV nursing care. We examined our findings in relation to past experiences, current realities and future possibilities for HIV nursing education and care in Canada. Our findings demonstrated that many nurses were interested in improving their HIV care, yet few opportunities existed for them to do so. We found that HIV nursing education and expertise were significantly different among participants and across clinical sites. This difference was visible in basic education, services offered for HIV and AIDS care, the collaborative and inter-professional nature of care, and opportunities for continuing education. Mentorship education is an effective strategy to not only address a critical void in knowledge, but also to promote a fundamental shift in attitudes. With the recent call by the World Health Organization to place nurses in key positions to provide HIV care, treatment and prevention, it is imperative to prepare nurses at both the undergraduate and graduate level, as well as those in practice, to fulfill this call.

Background: Nurses must understand the needs and barriers of people who are homeless when they are in a health care environment in order to render effective care. Objective: The objective of this review is to synthesize research findings on studies that examine the needs and barriers of people who are homeless in Canada when they intersect with health care providers. Methods: A scope of manuscripts, published in English from 1980 to 2011 that assess the needs and barriers of people experiencing homelessness in Canada when in the health sector without limitation on study design from different electronic databases and manual searches, was conducted. Results: Six articles (N=4 qualitative, N=2 quantitative) met the inclusion and exclusion criteria. Eight themes emerged from a narrative synthesis of the findings. Conclusion: Individuals who are homeless have multiple needs when in the health care sector. Thus, collaboration among different disciplines is essential in order to provide them with holistic care.

AIM: To assess the response of health professionals working in a Kenyan hospital emergency department to patients who had experienced intimate partner violence and the obstacles they encountered. METHODS: In-depth interviews were conducted with 11 health professionals. Transcribed interviews were analysed using QSR NVivo 7 software to reveal themes and sub-themes. FINDINGS: Health professionals managed victims of intimate partner violence symptomatically without following any predetermined guidelines and protocols. Participants were hindered in their attempts to screen and offer subsequent help to victims by other health professionals, victims of partner violence and the police. To identify health professionals' perceptions of their role working in the emergency department (ED) in managing and preventing intimate partner violence in Kenya.A qualitative research study was conducted involving in-depth interviews with one doctor, six nurses and four clinical officers who had worked in an ED for at least one year. Interviews were recorded on a digital voice recorder. Transcription and subsequent analysis of the interviews were done using NVivo 7 software for qualitative data. Emerging ideas were collated into themes and sub-themes.Participants found themselves assuming diverse roles within and without the health setting, for example that of clinician, liaison officer, counsellor and health and community educators. These roles represented the wide scope of involvement of healthcare professionals in relation to intimate partner violence and were perceived as enhancing effectiveness in addressing domestic violence towards women.Healthcare providers identified several roles that they assume to assist abused women. These roles were considered over and above the usual clinical roles that they are expected to perform.
