Abstract

Summary: Background: Current literature highlights a gap in precise stroke cost data for Latin America. This study measures the real costs associated with acute ischemic stroke care in Latin America using Time–Driven Activity-Based Costing (TDABC). The findings aim to lay a solid foundation for adopting value-based healthcare (VBHC) strategies in the region. Methods: The study is an observational, multicenter, international analysis of direct costs and outcomes for patients hospitalised with acute ischemic stroke from December 2021 to December 2022. Data from stroke centres in Argentina, Brazil, Chile, Colombia, Costa Rica, Mexico, Peru, and Uruguay were analysed. Costs were stratified by country. Factors such as favourable outcomes based on the modified Rankin Scale (mRS 0–2), clinical risk levels, and treatment interventions were considered for the analysis. Generalized Estimating Equation (GEE) models were utilised to assess the relationship of clinical variables with the total cost per patient. Findings: A total of 1106 patients were included in the study. Among these patients, 74% received medical treatment alone, 18% received intravenous thrombolysis (IVT), 4% underwent mechanical thrombectomy (MT), and 3% received combined IVT plus MT. The mean cost per patient was I$ 12,203 (SD I$ 15,055), with 49% achieving a favourable functional outcome. 5. Complicaciones Neurológicas y Extraneurológicas en pacientes con ACV internados en el Hospital de Clínicas de Montevideo durante un período de 2 años

Abstract

Introducción: Los pacientes con Ataque Cerebrovascular (ACV) presentan un elevado riesgo de complicaciones durante la internación, que pueden condicionar su pronóstico. Objetivos: Describir las complicaciones neurológicas y extra neurológicas durante la internación. Evaluar sus características y las diferencias de acuerdo al tipo de ACV. Identificar factores predictores de complicaciones y de mortalidad intrahospitalaria. Pacientes y métodos. Fueron incluidos en el estudio los pacientes atendidos en el Hospital de Clínicas (HC) de Montevideo con diagnóstico de ACV entre 1/1/14 y 31/12/15. Se analizaron las principales características clínico-epidemiológicas de los pacientes y se pesquizó la presencia o no de complicaciones según el subtipo y se clasificaron en Neurológicas y Extra Neurológicas (Sistémicas y Cardiovasculares). Resultados: Se analizaron 206 pacientes, 57% de los cuales presentaron complicaciones, indistintamente del subtipo de ACV. Las más habituales fueron las sistémicas, en 49% de los casos. La edad (OR= 1.073), diabetes (OR= 3.328), NIHSS al ingreso (OR= 1.090), Trombólisis (OR= 3.511) y la HTA (OR= 0.116) se asociaron en forma significativa a complicaciones neurológicas. La mortalidad intrahospitalaria fue 11%, siendo significativamente menor en los pacientes que ingresaron a la Unidad de ACV del centro. Conclusiones: Las complicaciones fueron muy frecuentes y la principal causa de muerte, siendo las sistémicas las más prevalentes. Ciertas variables se asociaron a un mayor riesgo de complicaciones: edad, diabetes, HTA, cardiopatía previa, NIHSS al ingreso, y la trombolisis. La Unidad de ACV descendió la mortalidad en forma significativa.

Abstract

Stroke is the second leading cause of death and the third leading cause of disability worldwide and its burden is increasing rapidly in low-income and middle-income countries, many of which are unable to face the challenges it imposes. In this Health Policy paper on primary stroke prevention, we provide an overview of the current situation regarding primary prevention services, estimate the cost of stroke and stroke prevention, and identify deficiencies in existing guidelines and gaps in primary prevention. We also offer a set of pragmatic solutions for implementation of primary stroke prevention, with an emphasis on the role of governments and population-wide strategies, including task-shifting and sharing and health system re-engineering. Abstract

El ataque cerebrovascular (ACV) en jóvenes es menos frecuente que en los sujetos añosos, pero su incidencia se encuentra en aumento. La etiología puede ser diferente en ambas subpoblaciones, sin embargo los factores de riesgo cardiovascular clásicos cada vez juegan un rol más preponderante en los jóvenes. Se realizó un estudio descriptivo retrospectivo analizando las características de los ACV en dicho grupo etario durante los años 2009 y 2010 en la Unidad de Ataque Cerebrovascular del Hospital de Clínicas de Montevideo, Uruguay. Se identificaron 10 pacientes (8% del total de ACV). Encontramos una supremacía de la naturaleza hemorrágica por sobre la isquémica y una muy buena evolución funcional alejada. Los factores de riesgo cardiovasculares clásicos, como la hipertensión arterial jugaron un rol fundamental en las hemorragias cerebrales, no así en los infartos. En estos últimos, se constató una mayor frecuencia de patología protrombótica.

Abstract

Background: Improving stroke services is critical for reducing the global stroke burden. The World Stroke Organization–World Health Organization–Lancet Neurology Commission on Stroke conducted a survey of the status of stroke services in low and middle-income countries (LMICs) compared to high-income countries. Methods: Using a validated World Stroke Organization comprehensive questionnaire, we collected and compared data on stroke services along four pillars of the stroke quadrangle (surveillance, prevention, acute stroke, and rehabilitation) in 84 countries across World Health Organization regions and economic strata. The World Health Organization also conducted a survey of non-communicable diseases in 194 countries in 2019. Results: Fewer surveillance activities (including presence of registries, presence of recent risk factors surveys, and participation in research) were reported in low-income countries than high-income countries. The overall global score for prevention was 40.2%. Stroke units were present in 91% of high-income countries in contrast to 18% of low-income countries (p < 0.001). Acute stroke treatments were offered in ∼ 60% of high-income countries compared to 26% of low-income countries (p = 0.009). Compared to high-income countries, LMICs provided less rehabilitation services including in-patient rehabilitation, home assessment, community rehabilitation, education, early hospital discharge program, and presence of rehabilitation protocol. Conclusions: There is an urgent need to improve access to stroke units and services globally especially in LMICs. Countries with less stroke services can adapt strategies from those with better services. This could include establishment of a framework for regular monitoring of stroke burden and services, implementation of integrated prevention activities and essential acute stroke care services, and provision of interdisciplinary care for stroke rehabilitation.

Abstract

Stroke is the second leading cause of death and the third leading cause of disability worldwide and its burden is increasing rapidly in low-income and middle-income countries, many of which are unable to face the challenges it imposes. In this Health Policy paper on primary stroke prevention, we provide an overview of the current situation regarding primary prevention services, estimate the cost of stroke and stroke prevention, and identify deficiencies in existing guidelines and gaps in primary prevention. We also offer a set of pragmatic solutions for implementation of primary stroke prevention, with an emphasis on the role of governments and population-wide strategies, including task-shifting and sharing and health system re-engineering. Resumen: Introducción: Los pacientes con Ataque Cerebrovascular (ACV) presentan un elevado riesgo de complicaciones durante la internación, que pueden condicionar su pronóstico. Objetivos: Describir las complicaciones neurológicas y extra neurológicas durante la internación. Evaluar sus características y diferencias de acuerdo al tipo de ACV. Identificar factores predictores de complicaciones y de mortalidad intrahospitalaria. Pacientes y métodos: Fueron incluidos en el estudio los pacientes atendidos en el Hospital de Clínicas (HC) de Montevideo con diagnóstico de ACV entre 1/1/14 y 31/12/15. Se analizaron las principales características clínico-epidemiológicas de los pacientes, se pesquisó la presencia o no de complicaciones según el subtipo y se clasificaron en Neurológicas y Extra Neurológicas (Sistémicas y Cardiovasculares). Resultados: Se analizaron 206 pacientes, 57 % de los cuales presentaron complicaciones, indistintamente del subtipo de ACV. Las más habituales fueron las sistémicas, en 49 % de los casos. La edad, diabetes, NIHSS al ingreso, cardiopatía previa y trombólisis se asociaron en forma significativa e independiente a complicaciones. La mortalidad intrahospitalaria fue 11 %, siendo significativamente menor en los pacientes que ingresaron a la Unidad de ACV del centro. Conclusiones: Las complicaciones fueron muy frecuentes y la principal causa de muerte, siendo las sistémicas las más prevalentes. Ciertas variables se asociaron a un mayor riesgo de complicaciones: edad, diabetes, cardiopatía previa, NIHSS al ingreso, y la trombolisis. La Unidad de ACV descendió la mortalidad en forma significativa. Abstract: Introduction: Stroke patients present high risk of complications conditioning its prognosis. Objectives: To describe neurological and extra-neurological complications during admission. To evaluate complications according with stroke subtype, identifying morbidity and mortality predictive factors. Patients and methods : All patients treated at the Hospital de Clínicas (HC) with a diagnosis of stroke between 01/01/14 and 12/31/15 were analyzed. The presence or absence of complications was also analyzed according to stroke subtype during the hospital stay and were classified into three groups: neurological and extra-neurological, the last ones subdivided into systemic and cardiovascular diseases. Results: Sample of 206 patients. 57 % of them presented complications, regardless to the stroke subtype. The most common complications were the systemic ones (49 %). Age, diabetes, NIHSS at admission, previous cardiac disease and thrombolysis were significantly associated with complications. Overall mortality was 11 %. Mortality was significantly lower for patients admitted to the institutional Stroke Unit. Conclusions : Stroke complications were very frequent and are the main cause of mortality, with systemic complications being the most common. Furthermore, there are other variables which are associated with a greater risk of complications such as older age, diabetes, previous heart disease, NIHSS at admission, and thrombolysis. Admission to stroke unit resulted in a significant decrease in the mortality rate. Objetivos: Descrever as complicações neurológicas e extra neurológicas durante a hospitalização. Avalie suas características e diferenças de acordo com o tipo de ACV. Identifique fatores que predizem complicações e mortalidade hospitalar. Pacientes e métodos: Os pacientes atendidos no Hospital de Clínicas (HC) de Montevidéu com diagnóstico de acidente vascular cerebral entre 1/1/14 e 31/12/15 foram incluídos no estudo. As principais características clínico-epidemiológicas dos pacientes foram analisadas e a presença ou ausência de complicações de acordo com o subtipo foi investigada e classificada em Neurológica e Extra Neurológica (Sistêmica e Cardiovascular). Resultados: foram analisados 206 pacientes, 57 % dos quais apresentaram complicações, independentemente do subtipo ACV. Os mais comuns foram sistêmicos, em 49 % dos casos. Idade , diabetes, NIHSS na admissão, Trombólise foram significativamente associados a complicações. A mortalidade intra-hospitalar foi de 11 %, sendo significativamente menor nos pacientes internados na Unidade de ACV do centro. Conclusões: As complicações foram muito freqüentes e a principal causa de morte, sendo as sistêmicas as mais prevalentes. Certas variáveis foram associadas a um risco aumentado de complicações: idade, diabetes, doença cardíaca prévia, NIHSS na admissão e trombólise. A Unidade de ACV diminuiu significativamente a mortalidade.

Abstract

Background: Improving stroke services is critical for reducing the global stroke burden. The World Stroke Organization-World Health Organization-Lancet Neurology Commission on Stroke conducted a survey of the status of stroke services in low and middle-income countries (LMICs) compared to high-income countries. Method(s): Using a validated World Stroke Organization comprehensive questionnaire, we collected and compared data on stroke services along four pillars of the stroke quadrangle (surveillance, prevention, acute stroke, and rehabilitation) in 84 countries across World Health Organization regions and economic strata. The World Health Organization also conducted a survey of non-communicable diseases in 194 countries in 2019. Result(s): Fewer surveillance activities (including presence of registries, presence of recent risk factors surveys, and participation in research) were reported in low-income countries than high-income countries. The overall global score for prevention was 40.2%. Stroke units were present in 91% of high-income countries in contrast to 18% of low-income countries (p < 0.001). Acute stroke treatments were offered in ~ 60% of high-income countries compared to 26% of low-income countries (p = 0.009). Compared to high-income countries, LMICs provided less rehabilitation services including in-patient rehabilitation, home assessment, community rehabilitation, education, early hospital discharge program, and presence of rehabilitation protocol. Conclusion(s): There is an urgent need to improve access to stroke units and services globally especially in LMICs. Countries with less stroke services can adapt strategies from those with better services. This could include establishment of a framework for regular monitoring of stroke burden and services, implementation of integrated prevention activities and essential acute stroke care services, and provision of interdisciplinary care for stroke rehabilitation.Copyright © 2021 World Stroke Organization.

Abstract

Resumen Introduccion Existe controversia en la forma de presentacion clinica y etiologia de los infartos de la arteria coroidea anterior. Objetivos Describir las caracteristicas clinicas y etiopatogenicas de una serie de pacientes con infarto de dicho territorio vascular. Materiales y metodos Estudio retrospectivo y descriptivo. Criterio de inclusion: tomografico o por resonancia magnetica nuclear. Clasificacion clinica: Oxfordshire Community Stroke Project. Clasificacion etiologica: Laussane Stroke Registry. Resultados Poblacion: 8 pacientes. Sintomas mas habituales: hemiparesia y hemihipoestesia. Etiologia mas prevalente: ateromatosis de grandes arterias. Conclusiones Hubo una gran heterogeneidad en la presentacion clinica. La ateromatosis de grandes arterias fue la etiologia mas frecuente.

Abstract

We describe the case of an HIV positive 49 yo man under protease inhibitors (ritonavir) pre-senting with total reversible left hemiparesis after the intake of 3 g of ergotamine. After 20 minutes he was spontaneous-ly asymptomatic. TIA was diagnosed. Parenchymal MRI was normal, cervical doppler ultrasound showed symmetric nar-rowing in both internal carotid arteries, causes of cardiac em -bolism were properly excluded. Angio Magnetic Resonance Imaging (Figures 1 and 2) was performed in acute stage and evolution, as well as cerebral angiography, leading to the di-agnosis of cerebral ergotism. Between both MRI showed, only aspirin 325 mg and bed rest was indicated.

Resumen La neuralgia occipital es una enfermedad frecuente que se debe casi siempre a afectacion del nervio occipital mayor o nervio de Arnold. Puede manejarse con farmacos por via oral, o con infiltracion anestesica del nervio, la cual esta infrautilizada. Se presentan 2 casos de pacientes que no respondieron al primer tratamiento y lo hicieron de forma satisfactoria a la infiltracion. Se analizan los principales conceptos sobre anatomia, fisiopatologia, etiologia y clinica de la enfermedad, asi como de la tecnica de la infiltracion anestesica.
