Abstract

Abbiamo valutato il ruolo della colangio-RM nella diagnosi delle patologie delle vie biIiari e dei dotti pancreatici in relazione anche ai dati emersi dalla letteratura. Da febbraio2OO2 a luglio 2004 sono stati sottoposti a risonanza magnetica (RM) per lo studio delle vie biliari 252 pazienti consecutivi, con sospetto clinico o strumentale di patologia biliare. La diagnosi deflnitiva di patologia è stata ottenuta mediante intervento chirurgico e/o colangiopancreatografia retrograda perendoscopica (ERCp) nei pazienti con clinica o evidenza diagnostica di malattia, mediante follow-up nei restanti casi. La colangiopancreato-RM (CPRM), associata alla sequenza FSE T2 pesata sul piano assiale, è risultata positiva in 191/252 (75.79yo) pazienti, in 75/191 dei quali (39.26%) si è reso indispensabile il ricorso a ERCP (n = SS) o è stato necessario l,intervento chirurgico b = qZ). CPRM e reperto chirurgico non concordavano tn 3/42 (7.14yo) casi operati; CPRM e ERCP sono risultate dlscordanti in 11/33 (33.38%) casi sottoposti a ERCP Fatta eccezione per i limiti associati alla difficoltà dl acquisire informazioni dinamiche relative a patologie funzionali dello sfintere di Oddi, per le quali peraltro si sta registrando un progressivo aggiornamento tecnologico, la CpRM rappresenta la metodica diagnostica di riferimento nella patologia biliopancreatica, riservando all'esame endoscopico valenza terapeutica.

19. Qualitative research on dementia in ethnically diverse communities: Fieldwork challenges and opportunities

Abstract

Australia, like other ethnically diverse societies, needs to provide culturally appropriate health care to all its citizens. One way of facilitating this is to ensure that health services research adequately reflects the circumstances and needs of culturally and linguistically diverse (CALD) communities within the population. This article discusses the fieldwork phase of a qualitative research project on dementia caregiving in 4 CALD communities in south west Sydney, Australia. Rather than focusing on the study results - which have been published elsewhere - this article presents and discusses crucial fieldwork issues that arose in the conduct of the project, particularly regarding participant recruitment and facilitation of focus groups. In being transparent about some of the difficulties encountered and how these were managed, we offer suggestions for other researchers wanting to include CALD communities in a meaningful way in their research projects. © The Author(s) 2013.

Abstract

Air quality sampling campaigns in three European subway systems (Barcelona, Athens and Oporto) were conducted in order to characterise particulate matter (PM) to better understand the main factors controlling it. PM mass concentrations varied among the European subway platforms, and also within the same underground system, this being mainly associated to differences in the design of the stations and tunnels, system age, train frequency, ventilation and air‐conditioning systems, commuter's density, rails geometry and outdoor air quality. PM concentrations displayed clear diurnal patterns, depending largely on the operation and frequency of the trains and the ventilation system. Chemically, subway PM2.5 on the platforms consisted of iron, carbonaceous material, crustal matter, secondary inorganic compounds, insoluble sulphate, halite and trace elements. Fe was the most abundant element, accounting for 19–46% of the bulk PM2.5, which is generated mainly from mechanical wear at rail‐wheel‐brake interfaces. A source apportionment analysis allowed the identification of outdoor (sea salt, fuel‐oil combustion and secondary aerosol) and subway sources on platforms. The use of air‐conditioning inside the trains was an effective approach to reduce exposure concentrations, being more efficient removing coarser particles. PM concentrations inside the trains were greatly affected by the surrounding (i.e. platforms and tunnels) air quality conditions.

Abstract

This study was undertaken to evaluate the accuracy of contrast-enhanced magnetic resonance angiography (CE-MRA) in detecting renal artery stenosis using intra-arterial digital subtraction angiography (DSA) as the gold standard.Thirty-five consecutive patients with possible renovascular hypertension were prospectively studied; 26 of them underwent both MRA and DSA. In these 26 cases, two readers assessed the number of renal arteries, the presence of stenoses and their degree. Results were compared with DSA, and sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and diagnostic accuracy of MRA were determined. Interobserver variability was also calculated.DSA showed 51 main renal arteries (one patient had a single kidney) and six accessory arteries (total number of arteries 57) in the 26 patients considered. Both MRA readers detected all of the 51 main renal arteries and only one accessory vessel. When the presence of stenosis was considered, the readers' results, respectively, were as follows: sensitivity 77% and 72%, specificity 69% and 69%, PPV 86% and 85%, NPV 55% and 50% and diagnostic accuracy 75% and 71%. When the detection of significant stenosis was considered, the results, respectively, were: sensitivity 83% and 83%, specificity 73% and 78%, PPV 60% and 65%, NPV 90% and 91%, and diagnostic accuracy 76% and 80%. Interobserver variation was good when considering stenosis detection (kappa=0.69) and excellent when considering detection of significant stenosis (kappa=0.85).MRA results do not appear as positive as in the majority of papers in the literature. Multiple reasons can probably be invoked to explain this difference. The mean age of our patients, higher than in many other studies, should be noted and may have accounted for their possible poor cooperation. Moreover, all of the missed significant stenoses were distally located, and therefore, the failure to detect them might be related to the suboptimal spatial resolution of MRA. Nevertheless, MRA showed a high NPV for detecting significant stenoses, a finding of considerable clinical relevance in that it allows patients with normal MRA findings to be spared additional more invasive procedures.

Abstract

Il paziente anziano e spesso coinvolto in un complesso di patologie multiorgano che possono manifestarsi con il rilievo clinico di una tumefazione scrotale o con la sintomatologia dello scroto acuto. Le tumefazioni scrotali possono essere dovute a un aumento di spessore della parete dello scroto, o ad aumento di volume del contenuto scrotale per la presenza di idrocele, di grosse cisti dell’epididimo, o di lesioni infiammatorie granulomatose o croniche. Nell’anziano le neoplasie del testicolo sono piu rare che nel giovane e i tumori non germinali prevalgono su quelli della serie germinale. Il varicocele, frequente nel giovane, e piuttosto raro nell’anziano ed e spesso secondario ad altre patologie. Mentre nel giovane con scroto acuto la diagnosi differenziale riguarda soprattutto la torsione testicolare e la patologia flogistica, nell’anziano la torsione testicolare e piu rara dell’ernia strozzata e degli infarti lobulari. L’ecografia rappresenta l’indagine di scelta nella valutazione della patologia dello scroto; la combinazione dei reperti ecografici e clinici consente di raggiungere la diagnosi corretta nella maggior parte dei casi.

Abstract

The purpose of this study was to evaluate the capability and the reliability of diffusion-weighted MRI in the evaluation of normal kidney and different renal lesions. 39 patients (10 normal volunteers and 29 patients with known renal lesions) underwent MRI of the kidneys by using a 1.5 T superconducting magnet. Axial fat suppressed turbo spin echo (TSE) T(2) and coronal fast field echo (FFE) T(1) or TSE T(1) weighted images were acquired for each patient. Diffusion-weighted (DW) images were obtained in the axial plane during breath-hold (17 s) with a spin-echo echo planar imaging (SE EPI) single shot sequence (repetition time (TR)=2883 ms, echo time (TE)=61 ms, flip angle=90 degrees ), with b value of 500 s mm(-2). 16 slices were produced with slice thickness of 7 mm and interslice gap of 1 mm. An apparent diffusion coefficient (ADC) map was obtained at each slice position. The ADC was measured in an approximately 1 cm region of interest (ROI) within the normal renal parenchyma, the detected renal lesions and the collecting system if dilated. ADC values in normal renal parenchyma ranged from 1.72 x 10(-3) mm(2) s(-1) to 2.65 x 10(-3) mm(2) s(-1), while ADC values in simple cysts (n=13) were higher (2.87 x 10(-3) mm(2) s(-1) to 4.00 x 10(-3) mm(2) s(-1)). In hydronephrotic kidneys (n=6) the ADC values of renal pelvis ranged from 3.39 x 10(-3) mm(2) s(-1) to 4.00 x 10(-3) mm(2) s(-1). In cases of pyonephrosis (n=3) ADC values of the renal pelvis were found to be lower than those of renal pelvis of hydronephrotic kidneys (0.77 x 10(-3) mm(2) s(-1) to 1.07 x 10(-3) mm(2) s(-1)). Solid benign and malignant renal tumours (n=7) showed ADC values ranging between 1.28 x 10(-3) mm(2) s(-1) and 1.83 x 10(-3) mm(2) s(-1). In conclusion diffusion-weighted MR imaging of the kidney seems to be a reliable way to differentiate normal renal parenchyma and different renal diseases. Clinical experience with this method is still preliminary and further studies are required.

33. A Multidisciplinary Quality of Care Path For Couple Infertility: Development of An Integrated Approach Founded on A Multidisciplinary, Personalistic, and Ecological Model For Couple Fertility.

Abstract

The Path of Fertile Family (FFP) is a service for couples experiencing infertility, aimed to obtain natural conception, as an alternative to assisted reproductive techniques, developed in 2018 and provided by the Counseling Centre of Centro della Famiglia Foundation, an institute of culture and pastoral care of the Diocese of Treviso, in cooperation with the local socio-health unit. FFP offers to infertile couples a specific multidisciplinary path of care, including the involvement of specific gynecological, andrological, endocrinological, and psycho-relational competences, together with fertility awareness. It represents an "experiential laboratory" for the multidisciplinary diagnosis and treatment of infertility, for understanding relational processes, for the development of operational strategies and above all for the verification of the goodness of a lifestyle that is able to harmonize the couple's project and the family's project, couple satisfaction and parental openness. The setting of the service draws inspiration and reference from three paradigms from which significant ethical choices derive: the ecological perspective of fertility, the relational intimacy of the couple and a multidisciplinary approach. From the three founding paradigms, FFP model has been developed and progressively implemented in order to define a specific FFP Quality of Care Path (QCP) for couple infertility. Although further studies are needed to better clarify the role of each studied variable in a complex monitoring system, we identified for the first time a QCP Path based on 7 variables we present here two illustrative cases: the first one in which a natural conception has been obtained and the second one, in which the focus has been moved from natural conception to the reconstruction of relational competence. Further studies will demonstrate, on large scale, the efficacy of our protocol and of the analyzed variables. Summary: This paper provides an overview of the conceptual and methodological process of implementing the monitoring of the path of infertile couples towards natural conception in harmony with their relational life project. A multi-professional team accompanies couples in overcoming the obstacles to fertility and in the progressive awareness that parenting is not only biological. The result is a range of goals to strive for. The reference paradigms of the service justify the ethical choices of the couple and the team. The operating protocol supports the couple commitment and guides professionals in their effective action if competent and concerted. Abstract

The case of a young female patient suffering from gastrointestinal stromal tumour (GIST) with significant gynaecological implications is reported. The first signs and symptoms of the tumour were seen at pelvic level. The roles of laparoscopy-laparotomy, immunohistochemical study (c-kit positivity), abdominal CT scan, PET and gastroscopy proved important and are discussed. Problems relating to anaemia, metrorrhagia and the presence of ovarian cysts associated with pelvic abdominal colic are examined from the clinical and gynaecological viewpoint. Medical oncological therapy was able to make use of a new approach: administration of anti-tyrosine kinase drugs which directly contrasted the development of this malignant tumour.

Abstract

The aim of the study was to assess the role of magnetic resonance cholangiopan-creatography (MRCP) in the diagnosis of biliary tract and pancreatic duct diseases, also in relation to the data reported in the literature. Over the period from Februrary 2002 to July 2004, 252 consecutive patients, with a clinical or instrumental suspicion of biliary disease were submitted to magnetic resonance to investigate the biliary tract. The definitive diagnosis of disease was obtained by surgery and/or endoscopic retrtograde cholangiopancreatography (ERCP) in patients with clinical or diagnostic evidence of disease and by means of follow-up in the remaining cases. MRCP, combined with T2-weighted FSE sequence on the axial plane yielded positive findings in 191/252 patients (75.79%); in 75/191 (39.26%) either ERCP was indispensable (n = 33) or surgery proved necessary (n = 42). MRCP and surgical sample findings proved discordant in 3/42 cases operated on (7.14%); MRCP and ERCP yielded discordant findings in 11/33 cases (33.33%) submitted to ERCP. With the exception of the limitations associated with the difficulty in obtaining dynamic information relating to functional diseases of Oddi's sphincter, for which, moreover, a progressive tecnological updating is underway, MRCP is the diagnostic reference procedure in biliopancreatic disease, reserving a therapeutic role for ERCP.

Abstract

Dynamic cardiomyoplasty was performed in a patient using a new cardio-myostimulator (LD-PACE II) designed to enable a novel stimulation regimen that utilizes a new range of stimulation options, including cessation during sleep. After treatment, left ventricular ejection fraction improved in 24 months from 15% to 25% and New York Heart Association classification improved from class IV to II.
